目的:观察高压氧(HBO)综合治疗对脑外伤受损脑组织的影响。方法:选用Wistar雄性同月龄(4个月)大白鼠132只,体重350~380g,在无菌条件下制成脑外伤模型。于伤后第8、9、10天各处死2只,经病理检查确定已进入脑外伤后软化灶形成期,然后随机分为3个组:HBO组、HBO综合组和药物组,并分别经3~9个疗程治疗后进行各组相互及治疗前后比较,观察各组动物脑外伤软化灶及其附近组织的病理改变差异情况。结果:药物组脑外伤软化灶的体积明显大于HBO组和HBO综合组(P<0.001),而HBO综合组的软化灶体积又较HBO组为小(P<0.001)。HBO组及HBO综合组在分别治疗3个疗程与治疗9个疗程后,脑外伤软化灶的体积变化亦有显著性差异(P<0.001)。病理检查可见在有效治疗后,脑外伤软化灶有神经细胞新生、再生和组织修复现象。在治疗6、9个疗程后,HBO组及HBO综合组总的基本恢复率均明显优于药物组(P<0.001)。结论:HBO治疗对脑外伤脑受损组织具有促进细胞新生和脑组织修复的良好作用。  相似文献

脂质体是近年来发展起来的一种新型制剂,具有靶向给药的特性,可作为载体将药物选择性地导向靶器官、靶组织或靶细胞,从而获得最大的治疗效果并减少毒性。Colley等将脂质体包裹的甲氨喋呤(MTX)进行动物试验,证明其在肝,脾内的浓度较高。我们将MTX脂质体经导管直接在肝动脉内灌注治疗肝癌,使药物更县靶向性,疗效也更理想。材料与方法本文所用的MTX脂质体是由上海医药工业研究院研制并提供。我们未加选择地对3例男性肝癌患者进行治疗。治疗方法是按照肝动脉插管灌注化疗并栓  相似文献

目的:观察高压氧(HBO)对肝硬变患者肝储备功能的影响。方法:将38例肝硬变患者按肝功能Pugh等分级方法[1]分为肝功能良好组(n=18)和损害组(n=20)。于HBO治疗前及治疗后6、12天进行口服葡萄糖耐量试验(计算服糖后120、60分钟血糖浓度比值即OGTTR120/60),检测血清前白蛋白(PA)、白/球蛋白比值(A/G)、总胆红素(TBi)、凝血酶原时间(PT)及靛青绿15分钟潴留率(ICGR15),根据以上检查结果判断HBO治疗对肝储备功能的影响。结果:HBO可明显提高A/G值及PA水平,降低OGTTR120/60、TBi及ICGR15,对PT亦有明显改善及控制作用。结论:HBO可提高肝硬变患者肝储备功能,是肝硬变患者术前准备的有助方法  相似文献

目的:观察联合应用甲氨蝶吟、米非司酮治疗异位妊娠患者的临床效果。方法选取接受治疗的异位妊娠患者100例,按信封法将其分成实验组50例,对照组50例,对照组单独应用甲氨蝶吟治疗,实验组联合应用甲氨蝶吟、米非司酮治疗,用药5天后评价治疗效果。结果实验组患者异位妊娠的治疗效果明显优于对照组,治疗后实验组β-HCG、盆腔包块直径、药物有效率分别为(695.1±521.2)IU/L、(1.0±0.3)cm、82.0%,均优于对照组的(1023.2±412.0)IU/L、(2.5±0.6)cm、62.0%,(P<0.05)。结论与单纯应用甲氨蝶吟相比,联合应用甲氨蝶吟、米非司酮治疗异位妊娠患者的效果更佳,联合用药适合有生育要求的妇女。  相似文献

目的:是通过单次肌肉注射甲氨喋呤治疗异位妊娠,避免手术造成的痛苦及瘢痕等。方法:按甲氨喋呤50 mg/m^2深部肌肉注射,若效果不满意1周后重复注射1次。结果:治愈77例,其余14例手术治疗,1例自动出院,成功率83.7%。结论:甲氨喋呤肌肉注射治疗异位妊娠效果好值得推广。  相似文献

目的:探讨米非司酮联合甲氨喋呤治疗异位妊娠的疗效及不良反应。方法:60例异位妊娠患者随机分为米非司酮联合甲氨喋呤组(观察组)和单用甲氨喋呤组(对照组),对两组疗效及不良反应进行比较。结果:观察组治愈率93.33%,对照组治愈率86.67%,两组比较无显著性差异(P〉0.05)。观察组血β-HCG值在用药后5 d即开始下降,对照组于10 d开始下降,两组比较有显著性差异(P〈0.05)。观察组(13.33%)不良反应率明显低于对照组(33.33%),P〈0.05。结论:米非司酮联合甲氨喋呤治疗异位妊娠安全有效。  相似文献

目的:探讨高压氧(HBO)对荷S-180肉瘤小鼠在化学治疗中的增效作用及其机理。方法:240只荷S-180肉瘤的ICR小鼠被随机等分为不治疗对照组、高压氮氧组、HBO组、SOD-HBO组、HBO-平阳霉素组、HBO-环磷酰胺组、平阳霉素组和环磷酰胺组共8个组。分别使用化学治疗结合HBO疗法,采用电子自旋共振(ESR)技术直接检测S-180肉瘤组织中的氧自由基(OFR)含量,并应用分光光度计法测定其脂质过氧化物丙二醛(MDA)的含量,同时对各组动物肉瘤的体积、重量、坏死率及小鼠致死率进行了观察。结果:HBO组、HBO-平阳霉素组及HBO一环磷酰胺组的动物与相应的对照组比,肉瘤组织中的OFR和MDA含量均显著增高(P<005~0.001);同时瘤组织坏死率较高和坏死所需时间较短,且动物存活率较高,肉瘤体积和重量也显著小于对照组(P<0.05~0.001)。结论:荷S-180肉瘤的小鼠经HBO暴露可减缓荷瘤的生长速度并加速肉瘤细胞的坏死,平阳霉素或环磷酰胶结合HBO治疗荷S-180肉瘤的小鼠,能显著增强药物的抗肿瘤作用,同时减轻药物的副作用和延长其生命。  相似文献

目的:探讨高压氧(HBO)对实验性腹膜粘连的防治作用。方法:36只Wistar大鼠采用创伤法复制实验性腹膜粘连动物模型,随机分为HBO-A组、HBO-B组、HBO-C组和对照组。HBO-A组和HBO-B组于创伤后立即行HBO治疗,分别治疗10天和5天。HBO-C组推迟5天才治疗,只治疗5天。创伤后10天解剖所有大鼠,观察局部肠管色泽,并按腹膜粘连程度按级评分。结果:HBO-A组和HBO-B组的腹膜粘连积分分别为1.25±0.89和1.63±0.92,显著低于对照组3.08±0.79(P<0.01),而HBO-C组的腹膜粘连积分为2.88±0.83,与对照组相比无统计学差异(P>0.05)。结论:术后及时行HBO治疗能防治实验性腹膜粘连。  相似文献

目的观察甲氨喋呤联合米非司酮治疗异位妊娠的有效性和安全性。方法将70例异位妊娠患者随机分为两组:治疗组35例,采用甲氨喋呤联合米非司酮治疗;对照组35例,单用甲氨喋呤治疗。观察两组治愈率、血β-HCG下降情况、转阴时间及药物副反应。结果①治愈率:治疗组治愈率88.6%、对照组治愈率62.9%,组间差异有统计学意义(P〈0.05);②血β-HCG下降情况:治疗组、对照组组间差异有统计学意义(P〈0.05);③转阴时间:治疗组、对照组组间差异有统计学意义(P〈0.05);④治疗组包块消失时间:治疗组、对照组组间差异无统计学意义(P〉0.05);⑤治疗后输卵管通畅率:治疗组、对照组组间差异无统计学意义(P〉0.05);⑥药物副反应:治疗组、对照组组间差异无统计学意义(P〉0.05)。结论甲氨喋呤联合米非司酮治疗异位妊娠在治愈率方面明显优于单用甲氨喋呤。  相似文献

目的探讨常规治疗压力(0.25MPa)的高压氧增强血脑屏障开放、促进神经保护剂(神经节苷脂,GM-1)进入脑组织的作用。方法30只Wistar雄性大鼠,随机分为对照组、实验-1组、实验-2组、实验-3组、实验-4组;利用HPD 1700流体撞击仪,建立颅脑损伤动物模型,经毛细管电泳仪测定各组脑组织匀浆中GM-1相对浓度。结果各HBO治疗实验组GM-1浓度较无HBO治疗组和对照组有一定程度的提高,但对照组与各实验组、各实验组之间q检验均没有显著差异(P〉0.05)。结论常规治疗压力的高压氧无明显促进BBB通透性作用,但有增加BBB开放的趋势,促进神经保护剂进入脑组织。  相似文献

目的 观察高压氧(hyperbaric oxygen,HBO)治疗对急性脑创伤后血脑屏障损伤大鼠MMP-9 mRNA表达的影响,探讨高压氧治疗急性脑损伤的机制.方法 选取40只SD大鼠,采用自由落体打击法制备急性脑创伤模型.造模成功后采用数字表法随机分为4组,每组10只.分别为正常对照组:动物置于加压舱内,模拟除压力和氧浓度以外的其他实验条件.急性颅脑损伤24 h组:动物颅脑打击后1h,置于加压舱内,模拟除压力和氧浓度以外的其他实验条件,于伤后24 h断头取材.急性颅脑损伤高压氧治疗24h组:动物颅脑打击后1h和12 h,置于高压氧舱内,在0.25 MPa HBO下各停留40 min,于伤后24 h断头取材.急性颅脑损伤常氧高氮治疗24 h组:动物颅脑打击后1h和12 h,置于高压氧舱内,在0.25 MPa常氧高氮环境下各停留40 min,于伤后24 h 6只断头取材行含水量及RT-PCR测定,4只行脑组织伊文蓝(Evans blue,EB)测定.结果 急性颅脑损伤伤侧和非伤侧脑组织含水量为77.39 mg和72.25 mg,与急性颅脑损伤HBO治疗24 h组(70.83 mg、70.27 mg)比较差异有统计学意义(P<0.05).急性颅脑损伤经0.25 MPa HBO治疗后,脑组织含水量较未治疗组下降(P<0.01),高压氧治疗组半球及海马EB均显著增加(P<0.05,P<0.01).急性颅脑损伤经0.25 MPa HBO治疗后,损伤侧和非损伤侧半球及海马EB较未治疗组下降(P<0.0,1),急性颅脑损伤组及0.25 MPa常氧高氮组损伤侧半球及海马EB多于非损伤侧(P<0.05),0.25 MPa HBO治疗组损伤侧半球及海马EB高于非损伤侧(P<0.05).急性颅脑损伤经0.25 MPa HBO治疗后,损伤侧和非损伤侧半球及海马MMP-9 mRNA较未治疗组下降(P<0.0,1).急性颅脑损伤组、0.25 MPa常氧高氮组及0.25 MPa HBO治疗组损伤侧半球及海马MMP-9mRNA高于非损伤侧(均P<0.05).结论 HBO治疗急性脑创伤可以保护血脑屏障,从而减轻脑水肿,机制之一是HBO治疗减少了MMP-9 mRNA表达.  相似文献

Objective To investigate effects of hyperbaric oxygen (HBO) on the occludin and ZO-1 in the brain tissues and permeability of blood brain barrier (BBB) after cerebral ischemia-reperfusion in mice. Methods Male Wistar mice were randomly divided into 4 groups; the sham group, the ischemia-reperfusion group (IR) , the HBO group and the IR + HBO group. Following development of the cerebral ischemia reperfusion model, 5 sessions of HBO at 0.25 Mpa were applied during the reperfusion period, and 2% Evens blue (EB) was injected into the tail veins an hour before the animals were executed. Following ischemia reperfusion, changes in the permeability of BBB were monitored with the EB method. Following ischemia reperfusion, the distributions of occludin and ZO-1 in the brain tissue were observed with immunohistochemical method, and the expressions of occludin, ZO-1, and the contents of EB were determined by Western Blot and spectrophotometer. Results When compared with that of the sham group, the levels of EB in the brain tissue for the IR group increased significantly, reaching peak at 4 h following IR injury (P<0. 01). In the IR + HBO group, EB contents in the brain tissue decreased significantly, when compared with those of the IR group ( P< 0.01). However, no significant changes in the expression of occludin, ZO-1 and the content of EB in the brain tissue could be noted in the HBO, when compared with those of the sham group (P>0.05). In the IR group, the expressions of occludin and ZO-1 at 4, 12, 24, 48, 72 h after reperfusion decreased significantly, when compared with those at 0 h( P<0. 01). However, for the IR + HBO group, the expressions of occludin and ZO-1 at 72 h after reperfusion increased markedly (P<0. 01). No obvious changes in the expression of occludin and ZO-1 could be noted in the brain tissue in the animals in the HBO group, when compared with those in the sham group (P>0. 05). Conclusions HBO intervention could increase markedly the expressions of occludin and ZO-1 in the ischemia-reperfusion brain tissue, thus decreasing permeability of BBB.  相似文献

目的 探讨高压氧(hyperbaric oxygen,HBO)通过p38丝裂原激活蛋白激酶(p38 MAPK)信号通路调节大鼠脑缺血再灌注紧密连接蛋白claudin-3的表达及血脑屏障的通透性.方法 240只雄性Wistar大鼠,按数字表法随机分成5组:假手术(sham)组、脑缺血再灌注(ischemia reperfusion,IR)组、脑缺血再灌注+ HBO( IR+ HBO)组、脑缺血再灌注+SB203580 (IR+ SB203580)组、HBO+脑缺血再灌注+ SB203580( IR+ HBO+ SB203580)组,每组48只.复制局灶性脑缺血再灌注模型,IR+ SB203580组和IR+ HBO+ SB203580组于脑缺血再灌注前30 min经侧脑室注射100μl p38 MAPK信号传导通路抑制剂SB203580,IR+ HBO组与IR+ HBO+ SB203580组于再灌注期间行0.25 MPa HBO治疗5次,在处死动物前1h经尾静脉注射2%伊文思蓝(Evans blue,EB),采用EB法检测缺血再灌注后血脑屏障通透性的变化.分别应用免疫组织化学的方法和Western blot法观察缺血再灌注72 h后脑组织中claudin-3蛋白的分布及表达水平的变化.结果 与sham组相比,其他4组再灌注后72 h claudin-3蛋白表达显著降低,差异有统计学意义(P<0.01),同时伴有脑组织EB的含量显著增高,差异有统计学意义(P<0.01).IR+ HBO组(平均面密度值:0.330±0.12)和IR+SB203580组(平均面密度值0.190±0.011)与IR组(平均面密度值:0.120±0.08)比较脑组织claudin-3蛋白表达水平明显增加,脑组织EB的含量显著降低;与IR+ HBO组相比IR+ HBO+ SB203580组(平均面密度值:0.240±0.011)脑组织中claudin-3蛋白表达明显减少;脑组织EB的含量明显增加,差异均有统计学意义(P<0.01,P<0.05).IR+ HBO+ SB203580组与IR+ SB203580组比较claudin-3含量明显增加,EB的含量明显减少,差异有统计学意义(P<0.01).结论 HBO从蛋白水平可明显增加脑缺血再灌注脑组织中紧密连接相关蛋白claudin-3的表达,从而降低血脑屏障通透性.p38 MAPK信号传导通路可能是其发挥作用的机制之一.  相似文献

目的 应用磁共振(MRI)动态观察高压氧(HBO)治疗对创伤性脑损伤导致的脑水肿、血脑屏障和神经行为学评分的影响.方法 将24只兔按数字随机表法分为3组:假手术组5只,仅开直径8 mm颅骨窗;对照组10只,颅骨开窗后给予0.2 MPa液压冲击;HBO组9只,在对照组的基础上给予HBO治疗.造模成功后分别于6、24、72、120 h进行MRI扫描,计算脑水肿的体积和表观弥散系数(apparent diffusion coefficient,ADC)值,并行神经行为学评分.结果 HBO组经过治疗后,神经行为学评分较对照组显著改善(P<0.05),如伤后第5天时,HBO组(1.375±0.518)分,对照组(2.250±0.463)分.MRI观察假手术组未见异常,对照组和HBO组治疗前均可见明显脑水肿及血脑屏障破坏,HBO组经HBO治疗后水肿吸收及血脑屏障修复均较对照组快(P<0.01),如伤后第5天时,HBO组脑水肿体积为(183.41±17.56)mm3,对照组为(246.63±24.27)mm3;ADC值小于对照组(P<0.05).结论 HBO治疗可以加速液压冲击脑损伤后血脑屏障的恢复,减轻脑水肿,显著减轻其症状,降低致残率.
Abstract:
Objective To observe the effects of HBO therapy on cerebral edema, blood-brain barrier and neural behavioral scores, induced by brain trauma injury in rabbits with magnetic resonance imaging ( MRI). Methods Twenty-four rabbits were randomly divided into three groups: the sham operation group (n =5), with a skull window of only 8 mm in diameter; the control group (n = 10) , sustained with shock injury (shock intensity of 0.2 Mpa) following skull fenestration; the HBO group(n=90), also sustained with shock injury following skull fenestration, but with HBO treatment. Following the development of the model, MRI scans were made at 6th, 24th, 72nd, and 120th hours to calculate the volume of cerebral edema, apparent diffusion coefficient ( ADC), and neurological behavioral scores. Results After HBO treatment, neurological behavioral scores of the HBO group improved significantly, when compared with that of the control group (P< 0. 05). MRI showed that abnormality was not noted in the sham operation group. Before treatment, significant cerebral edema and damage of blood-brain barrier could be seen in both the HBO group and the control group. However, following HBO treatment, cerebral edema and blood-brain barrier damage of the HBO group recovered more rapidly, when a comparison was made between the groups (P<0. 01) and ADC value of the HBO group was lower than that of the control group (P<0. 05). Conclusions HBO could hasten recovery of the blood-brain barrier damage induced by cerebral damage with hydraulic impetus, reduce cerebral edema, significantly decrease symptoms of brain injury, and reduce disability rate.  相似文献

目的:探讨高压氧(HBO)暴露对荷瘤小鼠血液指标的影响。方法:用雄性Balb/C/小鼠20只,随机分成4组,每组5只。A组为正常对照;B组只行HBO暴露;C组只做肿瘤接种;D组行HBO暴露并接种肿瘤。B、D组经压力0.2MPa、氧浓度87%、氧分压0.174MPa的HBO暴露20次后,所有鼠摘眼球取血对其血红蛋白、白细胞总数、中性白细胞数、淋巴细胞数、中性白细胞百分比、淋巴细胞百分比作了观察。结果:(1)血红蛋白在HBO肿瘤组比单纯HBO组高(P<0.01);(2)白细胞总数在HBO肿瘤组分别高于对照组(P<0.05)、单纯HBO组(P<0.01)和单纯肿瘤组(P<0.05);(3)中性白细胞数在HBO肿瘤组和单纯肿瘤组,均比对照组高(P<0.01,P<0.05),而HBO肿瘤组分别比单纯HBO组和单纯肿瘤组高(P<0.01,P<0.05);(4)淋巴细胞数各组之间均无明显差异;(5)中性白细胞百分比在HBO肿瘤组和单纯肿瘤组,均比对照组高(P<0.01,P<0.05),而HBO肿瘤组比单纯HBO组高(P<0.01);(6)淋巴细胞百分比在对照组分别比HBO肿瘤组和单纯肿瘤组高(P<0.01,P<0.05),而单纯HBO组比HBO肿瘤组高(P<0.01)。结论:(1)HBO(氧分压=0.174MPa)可使荷瘤小鼠的白细胞总数和中性白细胞数量提高,但未能改变正常  相似文献

目的:对大鼠进行急性冷暴露,探讨冷暴露对大鼠血脑屏障(BBB)通透性的影响及木樨草素对大脑保护的可能性。方法:建立大鼠冷暴露模型,用比色法测定脑组织中伊文思兰(EB)含量关系及干-湿重法检测脑组织含水量变化,反映大鼠BBB通透性的影响。结果:冷暴露后大鼠脑组织水分含量和伊文思兰明显增高,BBB通透性明显增加,添加木樨草素的防护组则通透性受到不同程度的改变。结论:脑组织在冷暴露下BBB通透性增高,木樨草素有一定的保护作用。  相似文献

目的观察在离心机高G值反复作用下,血脑屏障通透性的变化特点,为高G值作用下脑功能改变的解释提供生理学依据。方法选雄性大鼠20只,分为对照组及+GZ暴露后不同时间组,实验组经增长率为1G/s,持续3min的+10GZ暴露3次,立即经大鼠心脏灌注镧固定液,开颅取脑,制成切片,置透射电镜下观察。结果+GZ重复暴露3次后即刻,即见镧颗粒通过少数开放的毛细血管内皮细胞间的紧密连接,沉积于血管外围;暴露后1h及6h,可见大部分毛细血管内皮细胞间的紧密连接开放,镧颗粒弥漫性地分布于神经细胞间隙,部分镧颗粒已进入神经细胞内部,分布于线粒体、内质网等细胞器的表面;暴露后24h,则见镧颗粒仅局限分布在血管腔内,毛细血管外的间隙则未见镧颗粒沉积,与对照组大鼠相似。结论+10GZ/3min重复作用3次可引起大鼠血脑屏障通透性增加,这种改变是可逆的。其可能在+GZ致脑水肿的发生、发展过程中起重要作用。  相似文献

目的 观察急性中、重度低压缺氧条件下,大鼠血脑屏障通透性的变化特点,为阐明低压缺氧作用下脑功能障碍的可能机理提供生理学依据。方法 选择雄性SD大鼠18只,随机分为对照组、5000m及8000m急性低压缺氧暴露组。实验组动物于低压舱内,以20m/s的速率上升,至5000m(或8000m),停留5h,而后以20m/s的速率下降至地面,出舱后立即经心脏灌注硝酸镧固定液,开颅取脑,制成切片,置透射电镜下观  相似文献
