<正> 髋臼骨折多系高能量损伤,常伴髋关节脱位或/和股骨头颈部的损伤。由于髋关节骨折的类型繁多,局部解剖关系比较复杂,仅依靠X线片和CT二维扫描难以全面、直观、清晰地反映骨折的情况,从而影响了治疗方法的选择和手术方案的制订。我们2001年1月~2003年6月对39例髋臼骨折常规行X线片、螺旋CT加三维重建检查,效果良好,报告如下。 1 临床资料 1.1 一般资料:本组39例,男25例,女14例,年龄18~61岁,平均41.5岁。致伤因素:交通事故伤31例,挤压伤3例,高空坠落伤2例,砸伤2例,合并骨盆骨折6例,内脏损伤2例,脊柱损伤2例,坐骨神经损伤1例,后尿道损伤1例。 相似文献

泌尿系结石是急腹症中最常见的疾病之一,临床主要表现为疼痛与血尿,疼痛常急性发作。传统的X线检查方法是采用泌尿系统平片(kidney—ureter bladder,KUB),由于肠道内容物干扰或与骨骼重叠等原因.传统X线检查不易发现和确诊。随着多排螺旋CT问世以来。CT尿路造影(computer tomography urography,CTU)及三维重建技术已广泛应用于泌尿系检查及临床诊断。本文就CTU在尿路结石诊断中的临床应用价值进行探讨。 相似文献

目的 探讨螺旋CT重建在胸腰椎骨折患者诊断和治疗中的应用价值。方法 前瞻性分析了2001年6月至2002年3月对25例胸腰椎骨折患者进行三维CT扫描重建情况。其中男16例,女9例。年龄19—86岁,平均35.6岁。车祸伤10例,高处坠落15例。25例均行X线平片、普通CT、二维及三维CT重建检查。结果 单纯性压缩骨折12例,爆裂性骨折13例,其中合并椎管轻度狭窄3例,中度狭窄6例,重度狭窄4例,脊椎滑脱3例。结论 螺旋CT重建有利于胸腰椎骨折分型、早期诊断、手术治疗和治疗效果评价。 相似文献

目的 探讨CT检查在髋臼骨折的诊断、治疗上的临床意义。方法对我院2001年1月到2002年6月间因髋臼骨折而收住院的66例有完整平片和CT的病例进行回顾性分析,逐个对两者在放射学上的影像结果进行仔细分析并作横向对照。结果66例中,有6例经CT检查而更改诊断。对于边缘压缩骨折、关节内游离体、股骨头压缩、骶髂关节损伤和坐骨结节骨折,平片和CT的阳性诊断例数之比分别为3:17、7:29、6:15、0:5和1:5,并且CT能多层次清晰显示髋臼顶部的骨折形态。结论CT对于平片是不可缺少的有效补充检查,对于髋臼负重区的损伤判断,关节面的边缘压缩骨折,关节内游离体,股骨头骨折,骨盆后方骨折有决定性作用,能进一步明确并完善诊断,有效指导治疗。故CT检查应与正位、髂骨斜位和闭孔斜位X线片一并作为髋臼骨折的常规放射学检查,并仔细阅读分析。 相似文献

目的探讨齿状突骨折的CT三维重建分型及临床意义。方法2000年9月~2004年6月,对32例寰枢椎损伤患者均行X线检查、螺旋CT薄层扫描及三维重建检查,评价CT三维重建分型及临床意义。结果X线片检查对齿状突骨折的漏诊率为28.13%(9/32),CT平扫漏诊率为6.25%(2/32),CT三维重建无1例漏诊。结论X线片对齿状突骨折易造成漏诊或诊断不确切。CT三维重建能直观、精确地显示病变的立体形态及各解剖结构的空间关系,提供分型依据,对治疗具有指导作用。 相似文献

目的 探讨螺旋CT三维图像重建显示关节内骨折的价值。方法 在20例关节内骨折患者中应用Hispeed螺旋CT机进行扫描,再将资料通过Windows工作站应用成像软件进行表面遮盖法三维成像,或再以关节解体技术充分显示损伤的关节面,细致观察关节损伤情况。结果 CT三维图像重建能清晰显示所有病例的关节内骨折及损伤细节情况,结合关节解体技术,能逼真地显示关节损伤的立体形态。所示结构清晰,与临床实际相符,无失真现象。结论 螺旋CT三维重建技术,结合关节解体技术和图像旋转技术,能显示关节内骨折的细节,术前应用有利于选择手术入路及内固定方法,减少手术创伤和失误。 相似文献

目的探讨多排螺旋CT(MSCT)检查在急腹症中的应用价值。方法收集我院收治的80例急腹症患者作为研究对象,利用飞利浦16排螺旋CT进行扫描,然后在工作站再进行处理,结合手术和病理结果进行回顾性分析。结果80例患者通过多排螺旋CT检查可确诊76例,确诊率达95%。4例患者因为无法避免的伪影干扰影响而漏诊。结论MSCT在诊断急腹症方面有着明显的效果,具有准确性高、无痛苦、快速的优点,可为临床提供更多诊断帮助。 相似文献

目的:探讨三维CT重建在评估胸腰椎骨折中的临床应用价值。方法:对14例胸腰椎骨折患者行X线片、CT扫描和三维CT重建检查,评估胸腰椎骨折脱位的程度,拟定治疗方案。结果:14例胸腰椎骨折X线片、CT扫描和三维CT重建诊断的创伤量分别为29、49和50。其中13例采取手术治疗。结论:三维CT重建能多角度立体显示椎体及附件的骨折,显示小关节水平方向的移位;能立体定位椎管内的碎骨块,显示继发性椎管狭窄的程度,指导术者对胸腰椎骨折准确分型,术中良好复位,解除骨块对脊髓的压迫,取得良好疗效 相似文献

Anterior skull base trauma evaluation and management has historically been difficult to systematically study secondary to the relative rarity of its occurrence, associated major morbidity and high mortality, and poor radiographic techniques. It has only been recently that improved care has allowed for decreased morbidity and mortality. The advent of computed tomography imaging techniques has led to anatomic characterization and detailed study of injury patterns. This article reviews current techniques for evaluation and management of the associated sinonasal, orbital, and neurologic sequelae of anterior skull base injuries. 相似文献

Postoperative evaluation by three-dimensional (3D) computed tomography (CT) was performed in 15 patients who underwent anterior skull base reconstruction using two musculopericranial flaps and a grafted bone sandwiched between the two flaps. Especially in the case of large bone grafts (4 x 4 cm or more), the graft was positioned with the convex side facing upward into the cranial cavity to avoid creating an intracranial dead space. The extent of the absorption and changes in the contour of the grafted bone were evaluated. Imaging was done at a CT level of 150 using a Proceed 3-D CT scanner (Yokogawa Medical Co., Tokyo, Japan). Donor bone for grafting to the skull base was harvested the cranium in 10 patients and the ilium in 5 others. Patients ranged in age from 7 to 76 years (mean, 47); there were 10 men and 5 women. The grafted bone ranged in size from 1 x 2 cm to 5 x 8 cm (1 x 2 cm to about 4 x 3 cm: 10 patients; 4 x 4 to about 5 x 8 cm: 5 patients).No marked absorption of grafted bone was seen in these 15 patients, and no brain hernia occurred. In patients with large bone grafts (4 x 4 cm to about 5 x 8 cm), a graft that had been positioned with the convex side facing upward into cranial cavity at the time of transplant was subsequently observed to be transformed into a downward convex contour of normal skull base. Fostoperative infection did not occur because of the dead space nor were there any symptom of pressure on the brain because of the upward convexity of the grafted bone.The present reconstructive method is not only a minimum invasive but also a safe and reliable method for anterior skull base surgery. 相似文献

To increase the safety of laparoscopic cholecystectomy, we have analyzed the biliary tract in a three-dimensional fashion. The union of the biliary ducts was studied in 50 patients using spiral computed tomography (CT) after intravenous infusion cholangiography. Depending on the union and course of the cystic duct to the common bile duct, cystic duct anatomy was classified into six categories: ipsilateral gallbladder side (type I) and contralateral side with anterior course (type II); contralateral side with posterior course (type III); intrahepatic side (type IV); intrapancreatic side (type V); and unclassified (type VI). The length of the cystic duct was also determined. The cystic duct was identified in 42 cases (84%); 30 cases (60%) were type I, 9 cases (18%) type III, and a single case (2%) of types II, IV and V, respectively. The length of the cystic duct was < or =2 cm in 30 cases that had a shorter operating time compared to 12 cases with a cystic duct <2 cm (p <0.01). In conclusion, three-dimensional reconstruction of the cystic duct anatomy using spiral CT provides simple classification of bile duct anatomy, and this preoperative information may increase the safety of laparoscopic cholecystectomy. 相似文献

A very rare case of mesenteric teratoma is reported. Complete tumor resection is the main prognostic factor for survival in such cases. Therefore, an important factor for successful surgery is the exact preoperative visualization of the extent of the tumor, including visualization of the abdominal arteries. This report describes the use of spiral computed tomography with 3-dimensional reconstruction for visualization of the extent of multiple mesenteric teratomas. 相似文献
