[Purpose] This study evaluated the effective selective activation method of the vastus
medialis oblique for knee joint stabilization in patients with patellofemoral pain
syndrome. [Subjects and Methods] Fifteen healthy college students (9 males, 6 females);
mean age, height, and weight: 22.2 years, 167.8 cm, and 61.4 kg, respectively)
participated. The knee angle was held at 60°. Muscle activities were measured once each
during an ordinary squat and a squat accompanied by hip joint adduction. The muscle
activities of the vastus medialis oblique and vastus lateralis were measured by
electromyography for five seconds while maintaining 60° knee flexion. Electromyography
signals were obtained at a sampling rate of 1,000 Hz and band pass filtering at 20–50 Hz.
The obtained raw root mean square was divided by the maximal voluntary isometric
contraction and expressed as a percentage. The selective activity of the vastus medialis
oblique was assessed according to the muscle activity ratio of the vastus medialis oblique
to the vastus lateralis. [Results] The activity ratio of the vastus medialis oblique was
higher during a squat with hip joint adduction than without. [Conclusion] A squat
accompanied by hip joint adduction is effective for the selective activation of the vastus

[Purpose] To investigate the effects of a seven-week quadriceps stretching program on the muscle fibre orientation of the vastus medialis oblique and vastus lateralis in the lower limbs by ultrasound imaging. [Participants and Methods] Twenty-seven healthy, physically fit, asymptomatic females and males (age 21.5 ± 1.3, Tegner activity level score ≥4) were recruited. Their initial vastus medialis oblique and vastus lateralis fibre angles were determined using ultrasound. They then undertook a seven-week quadriceps stretching program, 3 sets of stretches to be performed on both lower limbs, 3 times a week on 3 separate days. One volunteer was assigned as an intra-rater control and did not take part in the stretching program. The vastus lateralis and vastus medialis oblique fibre angles were measured again on completion of the exercise regime. [Results] A statistically significant decrease in muscle fibre angle was observed in both the right and left vastus medialis oblique, and the right and left vastus lateralis. [Conclusion] A 7-week stretching program can result in a significant decrease in muscle fibre angle in both the vastus medialis oblique and the vastus lateralis. This can help in understanding the effects of prescribed stretching exercises on athletic patients with PFP. 相似文献

[Purpose] The purpose of this study was to identify the effects of performing squat
exercises with visual feedback on the activation of the vastus medialis oblique (VMO) and
vastus lateralis (VL) muscles in young adults with an increased quadriceps angle
(Q-angle). [Subjects] This study used a motion analysis program (Dartfish, Switzerland) to
select 20 young adults with an increased Q-angle, who were then divided into a squat group
that received visual feedback (VSG, n=10) and a squat group that received no visual
feedback (SG, n=10). [Methods] The intensity of exercises was increased every two weeks
over a six-week exercise period in both groups. A visual marker was attached to the
patella of the subjects in the VSG, and they then performed squat exercises with a maximum
of 90° of knee flexion within a route marked on a mirror. The SG performed squat exercises
with a maximum 90° of knee flexion without attaching a visual feedback device. [Results]
Analysis of the muscle activation due to 90° squat exercises indicated that both groups
had statistically significant increases in activation of the VL. The VSG exhibited
statistically significant increases in activation of the VMO. [Conclusion] This study
confirmed that squat exercises with visual feedback are effective in activation of the VMO
and VL muscles. The findings are meaningful in terms of preventing the occurrence of

[Purpose] The purpose of this study was to examine the vastus medialis oblique to vastus
lateralis ratio in two pelvic tilt positions while performing the sit-to-stand task.
[Subjects and Methods] Activation of the vastus medialis oblique and the vastus lateralis
muscles of 46 healthy subjects (25 males, 21 females) were recorded by surface
electromyography during the STS task with anterior pelvic tilt (sit with thoracolumbar
spine extended and pelvis in an anterior tilt) and neutral pelvic tilt (sit with
thoracolumbar spine relaxed and pelvis in the neutral tilt position) positions. Changes in
vastus medialis oblique, vastus lateralis activation and the vastus medialis
oblique/vastus lateralis ratio were analyzed. [Results] Vastus medialis oblique and vastus
lateralis muscle activation significantly increased in neutral pelvic tilt position, but
the vastus medialis oblique/vastus lateralis ratio was not statistically different.
[Conclusion] The sit-to-stand procedure with neutral pelvic tilt position increased
activation of the vastus medialis oblique and vastus lateralis, usefully strengthening the

[Purpose] The purpose of the present study was to examine the effects of squat exercises
performed on different surfaces on the activity of the quadriceps femoris muscle in order
to provide information on support surfaces for effective squat exercises. [Subjects and
Method] Fourteen healthy subjects performed squat exercises for five seconds each on three
different support surfaces: hard plates, foam, and rubber air discs. Their performance was
measured using electromyography. As the subjects performed the squat exercises on each
surface, data on the activity of the vastus medialis oblique and the vastus lateralis, and
the vastus medials oblique/vastus lateralis ratio, were collected. [Results] The activity
of the vastus medialis oblique and the vastus medialis oblique/vastus lateralis ratio were
found to be statistically significantly higher on rubber air discs than when the squats
were performed on hard plates or foam. [Conclusion] To activate the vastus medialis
obilique, and to enhance the vastus medialis oblique/vastus lateralis ratio, unstable

[Purpose] This study attempted to identify how genu varum or valgum affects the
electromyographic activities of the vastus medialis, vastus lateralis, and rectus femoris
during knee isometric contraction. [Subjects] Fifty-two healthy young adults were enrolled
in this study. They were enrolled and classified into three groups by knee alignment
conditions: the genu varum, genu valgum, and control groups. [Methods] The
electromyographic activity ratio of the vastus medialis to the vastus lateralis and rectus
femoris were calculated using the percentage of maximum voluntary contraction. The
participants contracted their quadriceps during isometric contraction at 30 and 60° of
knee flexion. [Results] The genu varum group had more activity in the vastus medialis than
in the vastus lateralis and rectus femoris, whereas the genu valgum group had more
activity in the vastus lateralis and rectus femoris than in the vastus medialis. There was
a significant difference in the muscle activity ratio between the vastus medialis and
vastus lateralis by angle of knee flexion degree only in the genu valgum. There were no
significant differences in any of the three groups in terms of the muscle activity ratio
of the vastus medialis to the rectus femoris by angle of knee flexion. [Conclusion] The
quadriceps femoris was used for different strategies according to knee alignment during
isometric contraction at 30 and 60°. This study suggests that rehabilitation training
programs used to strengthen the quadriceps should consider the knee alignment conditions

[Purpose] The purpose of this study was to determine the effect of ground
tilt on the lower extremity muscle activity of stroke patients performing squat exercises.
[Subjects] Fifteen hemiparetic patients volunteered to participate in this study.
[Methods] The subjects performed squat exercises at three different ground tilt angles:
15° plantar flexion, a neutral position, and 15° dorsiflexion. A surface electromyogram
(sEMG) was used to record the electromyographic activities of the leg extensor muscle in
the vastus lateralis (VL), vastus medialis (VM), gastrocnemius lateralis (GL), and
gastrocnemius medialis (GM). The sEMG activity was analyzed using a one-way repeated
measures ANOVA and a post hoc Bonferroni correction. [Results] The results of this study
are summarized as follows. Significant differences were noted for the VL and the GL when
the angle of the ankle joint was between the 15° plantar flexion and neutral positions
during squat exercises involving the VL and when the angle of the ankle joint was between
the neutral position and 15° dorsiflexion during squat exercises involving the VM.
[Conclusion] In this study, sEMG showed that the VL and GL changed significantly during
squat exercises according to the ground tilt angle of hemiparetic patients. Therefore,
squat exercises with different ground tilt angles can be used to improve VL and GL

[Purpose] This study compared the electromyographic activity of the quadriceps in
hemiplegic patients during the downward, maintenance, and upward phases of squat exercises
performed with the feet parallel and with the non-paretic foot lifted. [Subjects] A total
of 17 hemiplegic patients (9 males and 8 females) volunteered for this study. [Methods]
All subjects performed squat exercises with the knees flexed to 30° and with the feet
parallel (shoulder-width apart) or with lifting of the non-paretic foot (normalized to 25%
of the knee height). [Results] The activity of the rectus femoris, vastus medialis
oblique, and vastus lateralis muscles was significantly higher during squat exercises
performed with the non-paretic foot lifted than with the feet parallel to each other. The
activity of all muscles during the maintenance phase of the exercises was greater than
that during the downward and upward phases. [Conclusion] Lifting the non-paretic foot
during squats may represent an effective exercise for motor function rehabilitation in

[Purpose] The muscle strength of the quadriceps muscle is critical in patellofemoral pain
syndrome. The quadriceps muscle supplies the power for dynamic patellar movement, and the
vastus medialis oblique (VMO) and vastus lateralis (VL) enable the patella to stabilize
during tracking. We followed the theories about open and closed kinetic chain exercises to
design two exercises, sling open chain knee extension (SOCKE) exercise and sling closed
chain knee extension (SCCKE) exercise. The purpose of our study was to research the
changes in quadriceps muscle activity during both exercises. [Methods] Electromyographic
analysis was used to explore the different effects of the two exercises. The MVC% was
calculated for the VMO and VL during exercise for analysis. [Results] We found that the
mean MVC% values of the VMO and VL during the SOCKE exercise were higher than those during
the SCCKE exercise. The ratio of the VMO to VL was 1.0 ± 0.19 during the SOCKE exercise
and 1.11 ± 0.15 during the SCCKE exercise. [Conclusions] The SOCKE exercise is targeted at
quadriceps muscle training and has a recruitment effect on the VMO. The beneficial effect

[Purpose] The purpose of this study was to investigate the difference in onset timing
between the vastus medialis and lateralis among the different knee alignments, as well as
the best isokinetic angular velocity for an isokinetic concentric contraction. [Subjects]
Fifty-two adults (20 with genu varum, 12 genu valgum, and 20 controls) were enrolled in
this study. Subjects with > 4 cm between the medial epicondyles of the knee were placed
in the genu varum group, whereas subjects with > 4 cm between the medial malleolus of
the ankle were placed in the genu valgum group. [Methods] Surface electromyography was
used to measure the onset times of the vastus medialis and vastus lateralis during
concentric contractions at 30, 60, and 90°/sec. [Results] The vastus lateralis showed more
delayed firing than the vastus medialis in the genu varum group, whereas vastus medialis
firing was delayed more than that of the vastus lateralis in the genu valgum group. No
differences in onset timing were observed between the vastus medialis and lateralis
according the different angular velocities during concentric contractions in all three
groups. [Conclusion] Genu varum and valgum affect quadriceps firing. Therefore, selective
rehabilitation training of the quadriceps femoris should be considered to prevent pain or

[Purpose] We aimed to evaluate knee joint movement and muscle activity ratio changes in stroke hemiplegic patients in recovery phase after using a knee-ankle-foot orthosis with an adjustable knee joint for 1 month; we also aimed to discuss the practical implications of our findings. [Participants and Methods] The participants were 8 hemiplegic patients in the recovery phase of stroke who were prescribed knee-ankle-foot orthosis with adjustable knee joint. We measured knee joint angles and electromyographic activity of the vastus medialis and biceps femoris during walking in two conditions: the knee-ankle-foot orthosis knee joint fixed in the extended position and the knee joint moved from 0° to 30° in the flexion direction. Measurements were taken 2 weeks after completion to account for habituation of the orthosis and repeated 1 month later. [Results] When the knee joint was moving from 0° to 30° in the flexion direction, the knee joint angle at initial contact and the minimum flexion angle of the gait cycle decreased significantly between the first and second measurements. When knee joint flexion was 30°, the muscle activity ratio of the vastus medialis increased significantly in the loading response and mid-stance compared to when it was fixed. [Conclusion] Setting the knee joint of a knee-ankle-foot orthosis in accordance with the knee joint movement may increase the muscle activity ratio of the vastus medialis from loading response to mid-stance.Key words: Stroke, Electromyography, Knee-ankle-foot orthosis 相似文献

group, whereas subjects with > 4 cm between the medial malleolus of the ankle were
placed in the genu valgum group. [Methods] Surface electromyography was used to measure

[Purpose] This investigation evaluated the effect of real-time feedback on electrical
activation of the quadriceps during 3 weight-bearing tasks of the Wii Fit
Plus®. [Subjects] Thirty male healthy volunteers were recruited. [Methods]
Activation of the vastus medialis, vastus lateralis, and rectus femoris muscles was
recorded during virtual lunge, single leg extension, and single leg reach exercises. Each
exercise was performed twice in 3 randomized experimental conditions (with visual
feedback, with auditory feedback, and with no feedback). The normalized electromyographic
data (using maximum voluntary isometric contraction) were analyzed using repeated measures
analysis of variance and Tukey’s test. [Results] No significant difference was found in
the muscles among the feedback conditions during the 3 exercises. However, the variation
in the muscle activity of the vastus medialis and vastus lateralis (18.23–29.20% of
maximum voluntary isometric contraction) was higher (47–62%) than that in the rectus
femoris (7.35–12.98% of maximum voluntary isometric contraction). [Conclusion] Real-time
feedback did not alter quadriceps activation during the Wii tasks. Additionally, these
games showed electromyographic activation levels similar to those for the same tasks
outside the virtual environment. The Wii weight-bearing tasks could therefore constitute a

The influence of weakness in the vastus medialis oblique muscle on the patellofemoral joint: an in vitro biomechanical study 总被引:7,自引:0,他引:7
OBJECTIVE: Investigation of the influence of weakness in the vastus medialis oblique muscle on patellar tracking. DESIGN: In vitro biomechanical study. BACKGROUND: Currently, the influence of weakness in the vastus medialis oblique muscle on patellar tracking has not been well understood. METHODS: Seven human cadaveric knees were used. The direction of loading forces in the rectus femoris/vastus intermedius, vastus medialis oblique, and the vastus lateralis muscles was decided by the muscle alignment of each cadaver knee measured at the time of dissection. The loads used were 60 N in the rectus femoris, 50 N in the vastus lateralis, and 40 N in the vastus medialis oblique, according to the ratio calculated from the cross-sectional study. The weakness of vastus medialis oblique was simulated at 30 N (75%), 20 N (50%), 10 N (25%), and 0 N (0%), and the patellar position was measured for each condition using a magnetic 3 Space Tracker System. The influence of weakness in the vastus medialis oblique muscle on patellar position was investigated with seven cadaver knees using a magnetic 3 Space Tracker System. RESULTS: At 0 degrees and 15 degrees of knee flexion, 75%, 50%, 25% and 0% of the normal vastus medialis oblique muscle led to a significant difference in lateral patellar shift compared to the normal (P<0.05). CONCLUSIONS: Weakness of the vastus medialis caused the patellar lateral shift at 0 degrees and 15 degrees of knee flexion. RELEVANCE: Weakness of the vastus medialis is thought to be an important factor causing patellar subluxation and dislocation. Understanding the relationship between the vastus medialis weakness and patellar tracking will be useful in diagnosis, treatment and prevention of patellar subluxation and dislocation. 相似文献

[Purpose] This study assessed the exercise capacity of healthy adults while performing the inline lunge exercise by using Functional Movement Screen (FMS). Compared the difference in muscle activity of the quadriceps according to the exercise capacity. [Participants and Methods] Thirty two healthy participants (12 males, 20 females) participated in this study. The surface electromyography (sEMG) was used to measure the electrical activities for the vastus medialis (VM), rectus femoris (RF), vastus lateralis (VL) of quadriceps. [Results] Both groups had significant difference when sitting up and getting up during the inline lunge. In scores 3 group, vastus medialis showed higher muscle activity than vastus lateralis. On contrary, in scores 2 group, vastus lateralis had higher muscle activity than vastus medialis. [Conclusion] Therefore, this study suggests that inline lunge can help to strengthen the quadriceps effectively by showing the difference of quadriceps activity according to exercise capacity.Key words: Inline lunge, Muscle activity, Functional movement screen 相似文献

[Purpose] The present study aimed to determine changes in muscle activity while moving on
a treadmill at various speeds. [Subjects] The activities of the left vastus lateralis,
vastus medialis, hip adductors, lateral head of gastrocnemius, medial head gastrocnemius,
soleus, and tibialis anterior of 10 healthy male university students were analyzed.
[Methods] University students walked, jogged, and ran for 10 minutes each in random order,
and then myogenic potentials were measured 10 minutes later for 30 seconds. The flexion
angle of the lower limb upon initial contact, mid stance, and toe off were measured.
[Results] The average walking, jogging, and running speeds were 3.6 ± 0.4, 6.7 ± 0.6, and
10.4 ± 1.3 km/h, respectively. The average electromyographic activities of the vastus
medial, tibialis anterior, medial head of gastrocnemius, and lateral head of gastrocnemius
significantly differed. All muscles were more active during jogging and running than
walking. Only the soleus was more active during running than walking, and the activities
of the hip adductors and vastus lateralis did not significantly differ. [Conclusion]
Velocity is faster and the angles of the lower limbs and ground reaction force (GRF) are
larger during running than walking. The vastus medialis and soleus worked more easily
according to the angle of the knee joint, whereas the tibialis anterior worked more easily
at faster velocities and the medial and lateral heads of the gastrocnemius worked more

[Purpose] We evaluated the relationship between knee alignment and the electromyographic
(EMG) activity of the vastus medialis (VM) to the vastus lateralis (VL) muscles in
patients with knee osteoarthritis (OA) in a cross-sectional study. [Subjects and Methods]
Forty subjects with knee OA were assessed by anatomic radiographic knee alignment and the
VM/VL ratio was calculated. Surface EMG from both the VM and VL muscles were evaluated
during maximal isometric contraction at 60° knee flexion. Simultaneously, peak quadriceps
torque was assessed using an isokinetic dynamometer. Subjects were categorized into low,
moderate, and high varus groups according to knee malalignment. The peak quadriceps torque
and VM/VL ratio across groups, and their relationships with varus malalignment were
analyzed. [Results] All subjects had medial compartment OA and the VM/VL ratio of all
subjects was 1.31 ± 0.28 (mean ± SD). There were no significant differences in the peak
quadriceps torque or VM/VL ratios across the groups nor were there any significant
relationships with varus malalignment. [Conclusion] The VM/VL ratio and peak quadriceps

[Purpose] The purpose of this study was to compare the effects of visibility and types of
ground surface (stable and unstable) during the performance of squats on the muscle
activities of the vastus medialis oblique (VMO) and vastus lateralis (VL). [Subjects and
Methods] The subjects were 25 healthy adults in their 20s. They performed squats under
four conditions: stable ground surface (SGS) with vision-allowed; unstable ground surface
(UGS) with vision-allowed; SGS with vision-blocked; and UGS with vision-blocked. The
different conditions were performed on different days. Surface electromyogram (EMG) values
were recorded. [Results] The most significant difference in the activity of the VMO and VL
was observed when the subjects performed squats on the UGS, with their vision blocked.
[Conclusion] For the selective activation of the VMO, performing squats on an UGS was

OBJECTIVE: This study examined the immediate effect of patellar taping with a standard force on the onset of vastus medialis obliquus and vastus lateralis activities before and after muscle fatigue in able-bodied subjects. DESIGN: This study tested 29 mature able-bodied subjects. The surface electromyographic onset time of their vastus medialis obliquus and vastus lateralis was measured after a posteroanterior perturbation at the knee in a single-legged standing position. The measurements were taken under three conditions in random order of true patellar taping, sham patellar taping, and no patellar taping. Afterward, subjects performed a knee-extension exercise until their quadriceps fatigued, and the above tests were repeated to test the effect of fatigue on the outcome. RESULTS: There was no statistically significant difference with patellar taping on the electromyographic onset time of the vastus medialis obliquus and vastus lateralis compared with the placebo-taping and no-taping conditions (P = 0.455). There was no statistically significant difference in onset of vastus medialis obliquus and vastus lateralis muscles before and after muscle fatigue (P = 0.304). CONCLUSIONS: The present study suggests that patellar taping does not enhance the temporal activation of vastus medialis obliquus in both fatigue and nonfatigue conditions on able-bodied subjects. 相似文献

BackgroundTo investigate the extent to which quadriceps muscle activation and strength are responsible for patellofemoral pain.MethodsA pain on–off switch system synchronized with a force transducer and surface electromyography was utilized on 32 volunteer patellofemoral pain patients during maximal isometric and squat exercises.FindingsThere were 26 patients out of the 32 tested who complained of pain during the squat or isometric test, of these 20 subjects presented a significant advantage for the vastus lateralis compared to the vastus medialis obliquis activation and 12 patients had decreased quadriceps strength of the symptomatic compared to the non symptomatic leg. All patients who demonstrated weak vastus medialis obliquis activation during the isometric exercise possessed the same symptoms during the squat. On the other hand, 9 patients who showed diminished vastus medialis obliquis activation during the squat displayed equal activation between the vastus medialis obliquis and the vastus lateralis during the isometric task. With regard to the timing for the onset of muscle activation, there were only 4 patients who had a difference (P = 0.03) between the symptomatic (0.042 s) and non-symptomatic legs (0.011 s).InterpretationCauses for patellofemoral pain vary and are not necessarily a result of quadriceps strength deficit or vastus medialis obliquis activation weakness. Patellofemoral pain patients who possess lower vastus medialis obliquis activation compared to the vastus lateralis do not necessarily have quadriceps weakness while patients presenting with quadriceps strength deficits do not always have an imbalance between vastus medialis obliquis and vastus lateralis activation. 相似文献
