Adenosine-Sensitive VT. Idiopathic ventricular tachycardia (VT) is a term that refers to tachycardia that arises from ventricles devoid of apparent structural abnormalities. This form of VT is now recognized to be related to several distinct entities and includes a reentrant form typically located in the region of the left posterior fascicle, an automatic form that may originate from either ventricle, and a form that originates from the right ventricular outflow tract. This last type can account for up to 80% of cases of idiopathic VT and with few exceptions can be further subdivided into repetitive monomorphic VT and paroxysmal stress-induced VT, Evidence has accumulated suggesting that both forms of VT are related to cAMP-mediated triggered activity. The experimental underpinnings of this conclusion as well as the clinical characteristics of this form of idiopathic VT are elucidated in this review.  相似文献

Left Ventricular Outflow Tract Tachycardia. Idiopathic ventricular tachycardia (VT) originating from the left ventricular outflow tract (LVOT) is rare. We report two patients whose QRS configuration during VT commonly showed an inferior axis and monophasic R waves in all the precordial leads. The mechanism of these VTs appeared to be triggered activity. From mapping and ablation, the origin of these VTs was determined to be in the most posterior LVOT, corresponding to the aortomitral continuity (left fibrous trigone).  相似文献

A 36‐year‐old male presented with verapamil‐sensitive narrow QRS tachycardia. The patient underwent the catheter ablation of common idiopathic left fascicular ventricular tachycardia (ILVT) 2 years ago. During narrow QRS tachycardia, the diastolic and presystolic potentials (P1 and P2) were recorded at the left septum. Activation sequences of P1 and P2 were opposite from those in common ILVT. Entrainment of P1 at the upper septum exhibited concealed fusion and S‐QRS equal to P1‐QRS. Radiofrequency current to P1 suppressed VT. Idiopathic left upper septal VT might be the antidromic macroreentry of the common form of ILVT.  相似文献

Idiopathic Left Ventricular Tachycardia. Introduction: Idiopathic left ventricular tachycardia with a QRS pattern of right bundle branch block and left-axis deviation constitutes a rare but electrophysiologically distinct arrhythmia entity. The underlying mechanism of this tachycardia, however, is still a matter of controversy. This report describes findings in a 42-year-old man who underwent successful radiofrequency catheter ablation of idiopathic left ventricular tachycardia.
Methods and Results: On electrophysiologic study, the tachycardia was reproducibly induced and terminated with double ventricular extrastimuli. Intravenous verapamil terminated the tachycardia whereas adenosine did not. Detailed left ventricular catheter mapping during sinus rhythm revealed a fragmented delayed potential at the mid-apical region of the inferior site near the posterior fascicle of the left bundle branch. At the same site, continuous electrical activity throughout the entire cardiac cycle was recorded during ventricular tachycardia. Repeated spontaneous termination of this continuous electrical activity in late diastole was followed immediately by termination of the tachycardia. Single application of radiofrequency current for 20 seconds at this site completely abolished inducibility of the tachycardia. After catheter ablation, at the identical site of preablation recording of the fractionated potential during sinus rhythm, no fragmented delayed activity could be recorded. There was no complication from the ablation procedure.
Conclusion: The preablation recordings of fragmented delayed potentials during sinus rhythm and continuous diastolic electrical activity during tachycardia, together with ablation characteristics and previously reported electrophysiologic properties of this arrhythmia, may further support microreentry as the underlying mechanism in idiopathic left ventricular tachycardia.  相似文献

目的:对经射频消融术证实的特发性室性心动过速的病例进行总结分析,探讨室性心动过速的发病状况、心电图特点、消融靶点的确定及消融结果。方法:对68 例特发性室性心动过速的起源部位和体表心电图进行分析,所有患者在诱发出室性心动过速后进行射频消融治疗,观察特发性室性心动过速的射频消融成功率和复发率以及它们和消融靶点的关系。结果:本组特发性室性心动过速患者中右室室性心动过速较左室室性心动过速多见。右室特发性室性心动过速心电图表现为左束支传导阻滞,左室特发性室性心动过速心电图则多表现为右束支传导阻滞。消融靶点的确定右室特发性室性心动过速主要采用起搏标测法,左室特发性室性心动过速主要采用激动顺序标测法。右室流出道室速组在起搏标测时起搏ECG和VT时ECG的12导联QRS波完全相同处消融成功率较高。结论:室性心动过速发作时的体表心电图可初步估计特发性室性心动过速的起源部位,射频消融术治疗特发性室性心动过速成功率高,并发症少。  相似文献

A 45-year-old woman underwent radiofrequency ablation (RFA) for symptomatic idiopathic left ventricular tachycardia (ILVT). The clinical arrhythmias had two different patterns, a wide QRS tachycardia with right bundle branch block (RBBB) and left axis deviation (LAD) and another with RBBB and right axis deviation (RAD). The electrophysiology study localized the origin of tachycardias to the midinferior and superior ventricular septum, respectively. RFA terminated successfully ILVT with RBBB and LAD morphology, but another pattern could not be ablated. Noncontact mapping revealed the earliest site of activation at the superior septum. RFA at this site terminated successfully ILVT with RBBB and RAD.  相似文献

Idiopathic left ventricular aneurysm (LVA) is a very rare clinical condition. This article describes a patient with idiopathic LVA associated with episodes of ventricular tachycardia and ventricular fibrillation. Clinical and instrumental examinations did not reveal the pathogenesis of the aneurysm. The malignant clinical course suggests that an aggressive antiarrhythmic treatment, including ICD implantation, may be warranted.  相似文献

Digitalis and Ventricular Tachycardia. Digoxin was used to treat a patient with an adenosine-sensitive ventricular arrhythmia. The patient had an exercise-induced ventricular tachycardia that was evaluated electrophysiologically and displayed characteristics of a triggered arrhythmia. The tachycardia was terminated reproducibly with 12 mg of intravenous adenosine. After treatment with digoxin (serum level = 1.7 ng/mL), the arrhythmia could no longer be initiated with programmed electrical stimulation or exercise treadmill testing. The patient has since remained symptom free for 10 months. The autonomic effects of digitalis are proposed to mediate drug efficacy in this form of ventricular tachycardia.  相似文献

"Idiopathic" Ventricular Tachycardia. Idiopathic ventricular tachycardias occur in "normal" hearts and are generally benign arrhythmias. They can arise from either the left or right ventricle, and the origin is usually predictable from the surface ECG. These arrhythmias are produced by diverse mechanisms. When treatment is indicated, empiric pharmacotherapy can he successful. However, if drugs are not tolerated or tail, radiofrequency (RF) ablation may he indicated. During electrophysiologic study, arrhythmia mechanism tan he determined, and pace and activation mapping can he used to localize the site of ventricular tachycardia origin to direct application of RF lesions. Entrance Site in ILVT. The entrance site of the slow conduction zone was identified by entrainment study in an 18 year-old woman with verapamil-sensitive idiopathic left ventricular tachycardia. Radiofrequency catheter ablation at this site eliminated the tachycardia. The entrance site was at a mid-septal location and was more than 2 cm away from the exit site. Electrophysiologic findings suggested macroreentry in the Purkinje system as the mechanism of idiopathic left ventricular tachycardia.  相似文献

报道 1 0例 (男 8、女 2 )左室特发性室性心动过速 (简称室速 )折返路径标测结果和选择折返路径的不同部位为消融点的消融效果。电生理检查常规插入右室心尖与冠状静脉窦电极 ,并经左、右股动脉分别插入大头电极和2 8 2mm间距冠状静脉窦 1 0极标测电极至左室 ,后者贴靠在室间隔表面。窦性心律时各电极对可依次记录到His束电位 (HP)、左束支电位 (LBP)和左后分支的蒲氏纤维电位 (PP) ,室速时仍可同时记录到上述各电位 ,但顺序相反 ,PP领先 ,HP最后 ;而各部位的V波激动顺序在窦性心律和室速时是相同的 ,都是远端电极 (PP以远 )的V波最早 ,近端电极 (HP)的V波最晚。大头电极置于PP电极对附近。结果 :1 0例中 9例能记录到折返路径各电位心内电图 ,折返路径记录成功率为 90 % ( 9/1 0 )。第 1例大头电极位于PP电极对略上方处放电 ,消融成功 ,但导致完全性左束支阻滞。第 2 ,3例开始在PP电极对略下方处放电 ,但凡未记录到PP的点 ,虽然V波最早 ,都是放电无效点。最后消融成功的点 ,都记录到最领先的PP。第 4例以后 ,都必须记录到最领先的PP后才放电 ,除 1例 2次放电成功外 ,都是 1次放电成功。 1 0例随访至今 3~ 1 8个月 ,未服任何抗心律失常药均无室速发作。结论 :左室标测法不仅对研究左室特发性室速的折返  相似文献

Idiopathic Left VT and Purkinje Potentials . We describe two patients with idiopathic left ventricular tachycardia that were cured by radiofrequency catheter ablation. Tachycardia was inducible by ventricular stimulation and was verapamil sensitive. Two distinct presystolic potentials (PI and P2) were recorded during tachycardia in the mid-septal or inferoapical area, but only one potential (P2) was recorded during sinus rhythm. After catheter ablation at this site, the PI potential was noted after the QRS complex during sinus rhythm, while the P2 was still observed before the QRS complex. The P1 potential showed a decremental property during atrial or ventricular pacing. These data suggest that Purkinje tissue with decremental properties was responsible for the tachycardia mechanism, and that the reentry circuit involving this tissue is likely to be of considerable size.  相似文献

总结不同起源部位特发性室性心动过速(IVT)经导管射频消融(RFCA)治疗的成功经验。103例IVT行RFCA治疗,左室特发性VT(ILVT)起自间隔部者以最早的P电位处为靶点,右室特发性VT(IRVT)和其他部位的IVT均以起搏与VT发作时12导联心电图QRS波形态完全相同处或最早心室激动处为靶点。结果:RFCA治疗IVT的成功率为96.12%,ILVT为92.9%,IRVT为98.4%,复发率为2.9%。IVT起源部位分别位于左室后间隔部32例,左室游离壁1例,左室流出道9例,右室流出道60例、流入道1例。结论:IVTRFCA的关键是消融靶点的标测和确定,可根据VT发作时的心电图表现估计其起源位置。IVT的RFCA成功率高。  相似文献

Most idiopathic ventricular tachycardia (VT) arises from the area of the outflow tracts, and the most common left ventricular site is the aortic root, usually from the right and left sinuses of Valsalva. This site of origin is suggested by specific patterns on the electrocardiogram. Activation mapping and pace mapping are both useful strategies, and their relative benefits and limitations need to be appreciated. The mapping strategy for a VT of suspected aortic root origin is based on the consideration that multiple chambers may need to be mapped, and the temptation to ablate at suboptimal sites based on the justification that it was easy to get to should be resisted. The entire surface of each cusp needs to be sampled, as distinct activation times and pacemaps are obtained at each site. Standard radiofrequency energy is typically adequate and the precision of mapping rather than the amount of tissue ablated is tantamount to success. In my opinion, the indications for ablation of aortic root VT are similar to those for other idiopathic VT. Although I offer patients both pharmacologic and nonpharmacologic options, I feel that ablation is first line therapy for patients with sufficient symptoms to warrant therapy.  相似文献

BACKGROUND: Idiopathic verapamil-sensitive left ventricular tachycardia (ILVT) is the most common form of idiopathic left ventricular tachycardia (VT). Different methods have been proposed for ablation of ILVT. METHODS: Between June 2002 and February 2004, 15 patients (12 men; age 28 +/- 11 years, range 12 to 51) with ILVT underwent radiofrequency (RF) ablation at our center. We retrospectively assessed the significance of recording purkinje potential (PP) and late diastolic potential (DP) and its effect on selection of ablation target and number of RF application. RESULTS: Sixteen VTs were observed. The clinical VT had either RBBB and left axis morphology (14 cases) or RBBB and right axis morphology (2 cases). The QRS duration during tachycardia was 124 +/- 12 ms and the tachycardia cycle length was 356 +/- 53 ms. DP and PP were recorded at the targeted area for RF ablation in 11 and 9 patients respectively. The PP-Q interval, DP-Q interval and DP width were 18 +/- 4, 53 +/- 18 and 14 +/- 8 ms, respectively. The number of RF application was 7.2 +/- 4.3. Fewer applications were needed in whom RF ablation was initially targeted to PP (with or without DP) recording site (10 patients, 4.7 +/- 1.8) compared to those targeted to DP recording site (5 patients, 12.2 +/- 3.3) ( P < 0.05). CONCLUSION: Compared to DP alone, earliest PP (with or without concomitant DP) might be superior for selection of target site of RF ablation in patients with ILVT.  相似文献

Ventricular tachycardia (VT) arising from the right ventricular outflow tract (RVOT) in the absence of overt structural heart disease is a common entity. Exclusion of occult structural disease such as arrhythmogenic right ventricular cardiomyopathy is critical as this diagnosis impacts both ablation outcomes and long-term prognosis. VT is most commonly due to triggered activity. Induction of the target arrhythmia in the laboratory is often problematic, and is frequently facilitated by catecholamine infusion. Recent data indicate that high-density three-dimensional activation mapping facilitates identification of target sites for ablation, and that the spatial resolution of pacemapping may be more limited than previously recognized. A standard 12-lead electrocardiogram is useful in providing an initial approximation of the site of origin within the outflow tract, and may contain subtle clues to potentially confounding foci on the left ventricular endocardial or epicardial surface. When sufficient arrhythmia is present to permit mapping, successful ablation can be expected in 90–95% of patients, with a recurrence risk of approximately 5%. In experienced centers, major complications are ≤1% and outcomes should approach those obtained for the common forms of supraventricular tachycardia.  相似文献

Idiopathic Reentrant RVOT VT With Presystolic Potential . A 12‐year‐old girl with recurrent palpitation due to idiopathic ventricular tachycardia (VT) with a left bundle branch block configuration and inferior axis was referred to our hospital. During the VT, a spiky presystolic potential (SP) was recorded at the septum of right ventricular outflow tract (RVOT) just below pulmonary valve. The SP was entrained with a decremental property by pacing from right ventricular apex. Concealed entrainment was observed by pacing where the SP was recorded. Delivery of radiofrequency current targeting the SP abolished the VT. The SP with the decremental property could represent the central pathway of this idiopathic RVOT reentrant VT. Idiopathic Ventricular Tachycardia and Fibrillation. Important data have recently been added to our understanding of sustained ventricular tachyarrhythmias occurring in the absence of demonstrable heart disease. Idiopathic ventricular tachycardia (VT) is usually of monomorphic configuration and can be classified according to its site of origin as either right monomorphic (70% of all idiopathic VTs) or left monomorphic VT. Several physiopathological types of monomorphic VT can be presently individualized, according to their mode of presentation, their relationship to adrenergic stress, or their response to various drugs. The long-term prognosis is usually good. Idiopathic polymorphic VT is a much rarer type of arrhythmia with a less favorable prognosis. Idiopathic ventricular fibrillation may represent an underestimated cause of sudden cardiac death in ostensibly healthy patients. A high incidence of inducibility of sustained polymorphic VT with programmed ventricular stimulation has been found by our group, but not by others.
