Purpose

To evaluate the feasibility and performance of SPECT/CT arthrography of the wrist in comparison with MR arthrography in patients with suspected ulnocarpal impaction.

Methods

This prospective study included 28 wrists of 27 patients evaluated with SPECT/CT arthrography and MR arthrography. Iodine contrast medium and gadolinium were injected into the distal radioulnar and midcarpal joints. Late-phase SPECT/CT was performed 3.5 h after intravenous injection of approximately 650 MBq 99mTc-DPD. MR and SPECT/CT images were separately reviewed in relation to bone marrow oedema, radionuclide uptake, and tears in the scapholunate (SL) and lunotriquetral (LT) ligaments and triangular fibrocartilage complex (TFCC), and an overall diagnosis of ulnar impaction. MR, CT and SPECT/CT imaging findings were compared with each other, with the surgical findings in 12 patients and with clinical follow-up.

Results

The quality of MR arthrography and SPECT/CT arthrography images was fully diagnostic in 23 of 28 wrists (82 %) and 25 of 28 wrists (89 %), respectively. SPECT/CT arthrography was not diagnostic for ligament lesions due to insufficient intraarticular contrast in one wrist. MR and SPECT/CT images showed concordant findings regarding TFCC lesions in 22 of 27 wrists (81 %), SL ligament in 22 of 27 wrists (81 %) and LT ligament in 23 of 27 wrists (85 %). Bone marrow oedema on MR images and scintigraphic uptake were concordant in 21 of 28 wrists (75 %). MR images showed partial TFCC defects in four patients with normal SPECT/CT images. MR images showed bone marrow oedema in 4 of 28 wrists (14 %) without scintigraphic uptake, and scintigraphic uptake was present without MR bone marrow oedema in three wrists (11 %). Regarding diagnosis of ulnar impaction the concordance rate between CT and SPECT/CT was 100 % and reached 96 % (27 of 28) between MR and SPECT/CT arthrography. The sensitivity and specificity of MR, CT and SPECT/CT arthrography were 93 %, 100 % and 100 %, and 93 %, 93 % and 93 %, respectively.

Conclusion

SPECT/CT arthrography of the wrist is feasible. Regarding diagnosis of ulnar impaction we found a high concordance with MR arthrography. SPECT/CT arthrography of the wrist is an alternative to MR arthrography in patients with contraindications to MR imaging.  相似文献

PURPOSE: To evaluate prospectively the diagnostic accuracy of magnetic resonance (MR) imaging in the identification of cartilage abnormalities at 3.0 and 1.5 T in cadaveric wrists, with gross pathologic findings as the standard of reference. MATERIALS AND METHODS: The study was approved by the hospital review board, and informed consent for scientific use of body parts had been provided by the subjects. Ten cadaveric wrists from nine subjects were evaluated (seven left wrists, three right; five women, four men; age range, 46-99 years; mean age, 80 years). All wrists were examined with MR imaging in a 1.5-T unit and a 3.0-T unit, with the same imaging protocol used with both systems. Imaging protocol included intermediate-weighted fast spin-echo sequences and three-dimensional gradient-recalled-echo sequences. Cartilage surfaces of the proximal and distal carpal row, including the scaphotrapeziotrapezoidal joint, were analyzed in blinded fashion by two musculoskeletal radiologists working independently and then in consensus. Open inspection of the wrists was used as the standard of reference. Sensitivity, specificity, accuracy, and positive and negative predictive values were calculated. The McNemar test was used to assess differences in diagnostic assessment. Weighted kappa values were calculated for interobserver agreement. RESULTS: One hundred seventy cartilage surfaces were graded. The sensitivity and specificity for cartilage lesions were 43%-52% and 82%-89%, respectively, at 1.5 T and 48%-52% and 82% at 3.0 T. Differences in assessment did not reach statistical significance (P > .99). Highest sensitivities were found in the proximal carpal row (67%-71%); lowest sensitivities were found in the distal carpal row (14%-24%). Interobserver agreement was higher for imaging at 3.0 T (kappa = 0.634) than at 1.5 T (kappa = 0.267). CONCLUSION: The performance of MR imaging for the detection of articular cartilage abnormalities in the wrist depends on anatomic location. Interobserver agreement is higher for imaging at 3.0 than at 1.5 T, but diagnostic performances were not significantly different (P > .99) at either field strength.  相似文献

ObjectivesUlnar-sided injuries of the non-dominant wrist are common in elite tennis players using a double-handed backhand technique. This study investigated the radiological changes of the non-dominant wrist in elite symptomatic and asymptomatic players using this technique as well as healthy controls. We compared clinical findings to radiological abnormalities.DesignCross-sectional design with blinded radiological assessment, and contemporaneous clinical assessment of symptomatic players.MethodsMagnetic resonance images (MRI) of wrists related to non-dominant ulnar-sided pain, were taken in 14 symptomatic tennis players, 14 asymptomatic tennis players, and 12 healthy controls which were then independently reviewed for abnormalities by blinded radiologists. Total abnormalities and global between-group differences in the triangular fibrocartilage complex (TFC), ulnar collateral ligament (UCL), extensor carpi ulnaris (ECU) and supporting structures, osseous-articular lesions and ganglia were assessed. These were then compared to clinical examinations of the symptomatic players to assess agreement.ResultsSymptomatic players reported a mean 3.64 abnormalities, being exactly 1 abnormality greater than asymptomatic players (2.64) and controls (2.50), suggesting similar asymptomatic lesions in all three groups. Players with pain reported significantly more osseous-articular lesions, ECU tendon and dorsal radio-ulnar ligament abnormalities, while changes to the UCL may reflect an isolated problem in specific wrists. There were no between-group differences in the presence of ganglia, most TFC structures nor ECU subsheath tear and subluxation.ConclusionsClinicians should carefully consider radiological changes alongside their clinical diagnosis of non-dominant wrist pain in tennis players due to possible tennis-related changes and/or asymptomatic findings.  相似文献

PURPOSE: To characterize triangular fibrocartilage (TFC) defects in symptomatic and contralateral asymptomatic wrists. MATERIALS AND METHODS: Communicating and noncommunicating defects of the TFC were depicted on bilateral wrist arthrograms in 56 patients with unilateral wrist pain and without associated lesions of the scapholunate or lunotriquetral ligaments. The exact location of each TFC lesion was noted. RESULTS: Communicating defects were noted in 36 (64%) of 56 symptomatic and in 26 (46%) of 56 asymptomatic wrists. Twenty-five (69%) of 36 communicating defects were bilateral. Except for one defect in each group of symptomatic and asymptomatic wrists, all communicating defects were noted radially. Noncommunicating defects were noted in 28 (50%) of 56 symptomatic wrists and in 15 (27%) of 56 asymptomatic wrists. Eleven (39%) of 28 noncommunicating defects were bilateral. On the symptomatic side, 28 of 36 noncommunicating defects (including eight multiple defects) were located proximally at the ulnar side. On the asymptomatic side, 11 of 17 noncommunicating defects (including two multiple defects) were at or near the ulna. CONCLUSION: Noncommunicating TFC defects, which typically are located on the proximal side of the TFC near its ulnar attachment, have a more reliable association with symptomatic wrists than do communicating defects. Radial-sided communicating defects described in the literature (Palmer type 1A and 1D) as posttraumatic commonly are seen bilaterally and in asymptomatic wrists.  相似文献

High-resolution magnetic resonance (MR) images of the hands and wrists of six healthy volunteers were obtained at 1.5 T with a surface coil. Additionally, the hands and wrists of two fresh cadavers were imaged and were subsequently sectioned for anatomic correlation. High contrast when depicting soft-tissue, coupled with impressive spatial resolution and multiplanar capabilities, enabled delineation of fine structures including nerves, tendons, and blood vessels. Osseous and cartilaginous structures were also well depicted. MR imaging gives anatomic definition of the hand and wrist unmatched by other diagnostic imaging methods.  相似文献

Previously developed criteria of normal distal radioulnar joint (DRUJ) axial geometry were applied to routine magnetic resonance (MR) images of 50 wrists. All wrists lacked clinically evident DRUJ instability. An attempt to apply the three geometric criteria to seven of the cases was not possible, since the prescribed landmarks were not visible. The remaining 43 cases were retrospectively divided into a symptomatic group (25 wrists), with clinical abnormalities referable to the ulnar side of the wrist, and an asymptomatic group (18 wrists), with no such abnormalities. Abnormal DRUJ geometry was found in 12 symptomatic and 2 asymptomatic wrists. This difference between the groups is significant (p=0.02), suggesting that many symptomatic wrists exhibit different DRUJ geometry than is found in most asymptomatic wrists.  相似文献

Hemangioma in the cirrhotic liver: diagnosis and natural history   总被引:10,自引:0,他引:10  
PURPOSE: To investigate the natural history and diagnosis of cavernous hemangioma in the cirrhotic liver with computed tomography (CT) and magnetic resonance (MR) imaging. MATERIALS AND METHODS: Imaging and pathologic findings of 21 hemangiomas in 17 patients were retrospectively reviewed. CT of the liver was performed in all patients; MR imaging, in four. Cirrhosis was confirmed histologically in all patients, and the diagnosis of hemangioma was based on histopathologic findings (15 patients, 18 hemangiomas) or strict imaging criteria (two patients, three hemangiomas). Ten patients underwent imaging follow-up. The number, sizes, location, attenuation, pattern of enhancement, exophytic growth, presence of capsular retraction, and size stability were evaluated. RESULTS: Of the 21 hemangiomas, five were not detected at CT or MR imaging. Twelve (75%) of 16 hemangiomas were subcapsular, two (12%) of 16 demonstrated exophytic growth, 14 (87%) of 16 demonstrated nodular peripheral enhancement, and 16 (100%) of 16 were isoattenuating to blood vessels. At MR imaging, all five hemangiomas demonstrated nodular peripheral enhancement and hyperintensity on T2-weighted images. Seven lesions were smaller at follow-up, and five lesions developed retraction of the hepatic capsule. CONCLUSION: Even within the cirrhotic liver, larger hemangiomas can usually be diagnosed confidently with CT or MR imaging. With progressive cirrhosis, however, hemangiomas are likely to decrease in size, become more fibrotic, and are difficult to diagnose radiologically and pathologically.  相似文献

目的:比较分析肝脏局灶性结节增生(FNH)的MRI、MSCT征象,以提高对FNH影像诊断水平。方法:回顾性分析13例经手术病理证实为FNH病例的影像资料,所有病例均行MRI、MSCT检查,且两种检查间隔时间为1个月以内。结果:12例为肝内单发病灶、1例为多发(2个)病灶。病灶中79%(11/14)MRI平扫T2WI为等或稍高信号,T1WI为等或稍低信号,21%(3/14)T2WI为高信号、T1WI为低信号;所有病灶(14/14)MSCT平扫均呈等或稍低密度;同时在显示病灶内部结构时MRI中79%(11/14)、MSCT中57%(8/14)见中央瘢痕或纤维分隔。MRI及MSCT两种检查方法的动态增强中,病灶表现为动脉期实质部分明显强化的比率MRI为100%(14/14)、MSCT为93%(13/14);病灶周边或中央异常增粗、扭曲的血管影MRI为14%(2/14)、MSCT为43%(6/14);病灶周边假包膜MRI为21%(3/14)、MSCT为7%(1/14);所有病灶均表现出"快进慢出"强化方式;中央瘢痕延迟强化MRI为82%(9/11)、MSCT为63%(5/8)。结论:绝大多数FNH在MRI和MSCT上有特征性的征象,MRI对其中央瘢痕病理特征的显示优于MSCT,MSCT则在病灶周边或中央增粗、扭曲血管病理特征的显示上优于MRI。  相似文献

This study aims (1) to assess the prevalence and distribution of multiple occult injuries of the carpal bones and the distal forearm in patients with wrist pain and negative radiographs following trauma and (2) to evaluate the distribution and significance of joint effusions in the wrists with multiple osseous injuries. One hundred and thirty-one subjects, 74 men and 57 women, were consecutively examined in two institutions. All were acute trauma patients with negative X-rays whose clinical examination suggested possible fracture at the wrist or the distal forearm. Magnetic resonance (MR) wrist imaging was performed with and without fat saturation sequences. The MR images were analysed for detection of occult trabecular contusions and cortical discontinuity in the carpus, the distal forearm and the metacarpal bases. The prevalence and distribution of the injuries were assessed along with the distribution of joint effusions. Eight patients were excluded due to inadequate image quality. Two patients had bilateral injury. A total of 125 wrists were analysed. Seventy-eight (62.4%) wrists had occult bone injuries. Among these 78, 53 (68%) wrists had more than one injured bone. Twenty-five wrists (32%) had one injured bone. The highest number of injured bones per wrist was six. Injuries with a visible fracture line were seen in 29 (37.1%) wrists on MRI. The distal radius was the most frequent location for occult fracture line (11 cases). The injuries without a fracture line (contusion) were present in 49 (63%) wrists; they were detected more frequently in the scaphoid (35 cases). The lunate (29 cases) and the triquetrum (26 cases) were almost equally affected. The bone that was less frequently injured was the pisiform (four cases). Joint effusions were present in all 53 wrists with multiple bone lesions but more often situated in the ulnocarpal space [27 (50.3%) wrists]. There was no correlation between effusions in multiple locations (grades III and IV) and multiple bone injuries. This study revealed the presence of multifocal trabecular contusions without correlation with increased joint effusions in patients with negative radiographs and persistent pain. The clinical significance of these findings deserves further investigation.  相似文献

OBJECTIVE: Previous MR imaging studies have produced evidence of changes to structures within the wrist believed to be associated with carpal tunnel syndrome. In an attempt to resolve the conflicting and inconclusive results of these studies, we report here the results of an MR imaging study at a field strength of 3.0 T, which is higher than that previously reported. SUBJECTS AND METHODS: Patients with carpal tunnel syndrome and control groups of asymptomatic subjects were studied using MR imaging. We evaluated electrophysiologically the median nerve function of the affected wrists of all patients. A gradient-recalled echo pulse sequence was used to study 13 3-mm-thick slices within the wrist of each patient or asymptomatic subject. Spatial resolution was approximately 0.3 x 0.3 mm2. The median nerve and other structures associated with the carpal tunnel, which were clearly shown on the MR images, were analyzed to yield structural data. RESULTS: Analysis revealed that the cross-sectional area of the nerve within and proximal to the carpal tunnel was approximately 50% larger in patients with carpal tunnel syndrome than in asymptomatic subjects. We found no significant difference in the area of the nerve within the carpal tunnel compartment compared with the area of the nerve proximal to the carpal tunnel either in patients or in asymptomatic subjects. Also, flattening of the nerve on entering the carpal tunnel was not significantly different in patients than in asymptomatic subjects. In patients an increase in the palmar bowing of the flexor retinaculum was found only at the level of the hamate compared with that found in asymptomatic subjects. The cross-sectional area of the carpal tunnel was of a similar size in patients and in asymptomatic subjects. Comparison of electrodiagnostic results indicated no correlations between the MR parameters and electrophysiologic dysfunction of the median nerve for patients. CONCLUSION: The only statistically significant differences found between patients with carpal tunnel syndrome and asymptomatic subjects were that the median nerve was approximately 50% larger within and proximal to the carpal tunnel in patients with carpal tunnel syndrome and palmar bowing of the flexor retinaculum occurred in patients only at the level of the hamate.  相似文献

Objective The objective was to perform detailed analysis of the involved soft tissues, tendons, joints, and bones in the hands and wrists of patients with psoriatic arthritis (PsA). Materials and methods We reviewed 23 contrast-enhanced MR imaging studies (13 hands and 10 wrists) in 10 patients with the clinical diagnosis of PsA. We obtained clinical information from medical records and evaluated images for the presence of erosions, bone marrow edema, joint synovitis, tenosynovitis, carpal tunnel, and soft tissue involvement. Two board-certified musculoskeletal radiologists reviewed all images independently. Differences were resolved during a subsequent joint session. Results The average duration of disease was 71.3 months, ranging from 1 month to 25 years. Eight of the 10 wrists (80%) and 6 of the 13 hands demonstrated bone erosions. Bone marrow abnormalities were shown in 5 of the 10 wrists (50%) and 4 of the 14 hands (31%). Triangular fibrocartilage tears were seen in 6 of the 10 wrists (60%). Wrist and hand joint synovitis were present in all studies (67 wrist joints and 101 hand joints). Wrist soft tissue involvement was detected in 9 of the 10 wrists (90%) and hand soft tissue involvement was present in 12 of the 13 wrists (92%). Findings adjacent to the region of soft tissue involvement included synovitis (4 wrists) and tenosynovitis (3 wrists). Bone marrow edema adjacent to the region of soft tissue involvement was seen in one wrist. Bulge of the flexor retinaculum was seen in 4 of the 10 wrists (40%) and median nerve enhancement was seen in 8 of the 10 wrists (80%). Tenosynovitis was seen in all studies (all 10 of the hands and all 13 of the wrists). The “rheumatoid” type of distribution of bony lesions was common in our study. Interobserver agreement for various findings ranged from 83% to 100%. Conclusion Contrast-enhanced MRI unequivocally demonstrated bone marrow edema, erosions, tendon and soft-tissue disease, and median nerve involvement, with good interobserver reliability in patients with PsA of the hands and wrists. Disease was more extensive in the wrists than in the hands.  相似文献

RATIONALE AND OBJECTIVES: The authors' purpose was to elucidate the various computed tomographic (CT) and magnetic resonance (MR) imaging findings in intracranial aspergillosis. MATERIALS AND METHODS: Retrospective analysis of cranial imaging findings was performed in eight proved cases of central nervous system aspergillosis. The patients ranged in age from 17 to 75 years. Four patients were immunocompromised, and four were immunocompetent. CT was performed in all eight patients, and MR imaging in five. RESULTS: Six patients (75%) had multiple lesions seen on the imaging studies, with a total of 27 focal brain lesions demonstrated. The lesions were most commonly seen in the cerebral hemispheres (n = 21), with lesser involvement of the basal ganglia (n = 2) and the posterior fossa (n = 4). Seven lesions were hemorrhagic on CT and/or MR images. There was a correlation between lesion size and hemorrhage, with hemorrhage more likely in larger lesions (>15 mm). At pathologic examination, foci of hemorrhage were noted within both infarcts and abscesses. Enhancement was noted in five lesions, four of which were confirmed abscesses. Contrast enhancement of the lesions was vague and week in immunocompromised patients but solid and strong in immunocompetent patients. There were 18 lesions without hemorrhage or enhancement; they were either infarcts or abscesses at pathologic examination. Some of these small nonhemorrhagic nonenhancing brain lesions in the subcortical white matter mimicked lacunar infarcts. CONCLUSION: Typical imaging findings of intracranial aspergillosis include multifocal lesions involving the cerebral hemispheres, with hemorrhage in approximately 25% of lesions. Lesional contrast enhancement tends to be stronger in immunocompetent hosts.  相似文献

The hands and wrists of 20 top-level rock climbers with sports injuries and overstress abnormalities were compared with the hands and wrists of 10 normal volunteers. They were all studied with MR imaging at 1.5 T. The imaging protocol included spin-echo and gradient-echo sequences with 1- to 5-mm-thick contiguous slices in the axial, coronal and/or sagittal planes, depending on the location and nature of the suspected injury. Typical hand and wrist lesions depicted with MRI in climbers consisted of anular ligament tears, lesions of the flexor tendons, tenosynovitis, ganglion cysts, joint effusion and functional carpal tunnel syndrome. The MRI findings on these abnormalities were compared to normal findings and those with ultrasound and plain films. In addition, hypertrophic changes in the muscles, tendons, ligaments, and bones of top-level rock climbers were assessed morphometrically. MRI proved to be the superior imaging modality in the diagnosis of sports injuries and overstress abnormalities of the hand and wrist in rock-climbing athletes.  相似文献

Bilateral three-compartment wrist arthrography was performed in 30 patients with unilateral posttraumatic wrist pain to assess the incidence of bilateral findings. The mean age of patients was 30 (range 18–55) years. Thirty-three percent of patients were normal bilaterally, 30% had unilateral communication in the symptomatic wrist, 30% had communications in both the symptomatic and asymptomatic wrists and 7% had communication in the asymptomatic wrist only. Unilateral three-compartment wrist arthrography is not recommended in the assessment of unilateral post-traumatic wrist pain; no advantage of three-compartment injection over radiocarpal injection alone was shown.  相似文献

Anatomic variants of the bones, ligaments, tendons and muscles are frequent findings in imaging of the wrist and hand. Many findings especially changes in the triangular fibrocartilage (TFC) and the interosseous ligaments are asymptomatic, their incidence is increasing with age, and they are frequently found bilaterally. Abnormalities such as increased signal within tendons are common in asymptomatic subjects. They may be explained by normal physiology, anatomical variability, MR artifacts or true abnormalities without clinical importance. Although it is not always possible to differentiate variants and artifacts from clinically relevant findings it is important to know their potential etiology and clinical importance and not to over report them as abnormality requiring additional imaging or treatment.  相似文献

To assess tissue changes responsible for enhancement of the meninges adjacent to meningiomas on magnetic resonance (MR) images, the authors correlated the MR imaging characteristics of meningeal lesions seen before and after administration of gadolinium diethylenetriamine-pentaacetic acid (DTPA) with the appearance of these lesions on contrast material-enhanced computed tomographic (CT) images and with histopathologic findings in four patients. Histopathologic examination of meninges showed increased loose connective tissue, hypervascularity, and dilated vessels. There was neoplastic infiltration of the dura mater in two patients but it was restricted to the immediate junction of neoplasm and dura mater, with maximum peripheral extension within 1 mm of the tumor margin. In both patients the meninges were enhanced far beyond the neoplastic infiltration. The other two patients showed no infiltration of dura mater. These findings suggest that pathologic enhancement of meninges adjacent to meningioma after administration of Gd-DTPA mainly represents reactive changes to the neoplasm and does not necessarily indicate neoplastic involvement.  相似文献

High-resolution magnetic resonance (MR) images of the hands and wrists of 12 patients with a variety of suspected pathologic conditions were obtained at 1.5 T using a prototype surface coil. Lesions included ganglions, rheumatoid arthritis, carpal fractures, carpal tunnel syndrome, and arteriovenous malformations. In the selected cases studied, MR images provided potentially relevant information. Surgical, pathologic, and radiographic correlations were obtained. MR imaging affords delineation of soft-tissue structure that is unmatched by other imaging methods, including computed tomography. It is anticipated that MR imaging of the hand and wrist will afford sufficient valuable clinical information in certain conditions to justify its expense outside a research setting. Further clinical testing, however, is warranted.  相似文献

Objective. To evaluate the diagnostic value of the MR imaging syndrome before and after performance of provocative exercises in patients with dynamic carpal tunnel syndrome. Design. Fat-suppressed proton-density and T2-weighted spin-echo images of the wrist were obtained prior to and after provocative, standardized exercises. Images were interpreted in masked fashion with regard to six MR criteria of carpal tunnel syndrome: (a) bowing of the transverse ligament, (b) and (c) deformation of the median nerve at the pisiform and hamate levels respectively, (d) signal abnormality of the median nerve, (e) presence of fluid in the wrist joints and/or carpal tunnel, and (f) presence of synovial swelling. Patients. Twenty-one wrists in 20 patients with subjective complaints of carpal tunnel syndrome and equivocal or negative clinical findings and negative electrodiagnostic examinations were included (age range 21–61 years, mean 37 years, 2 men and 18 women). The diagnosis of dynamic carpal tunnel syndrome was made and confirmed by surgery in 18 of the 21 symptomatic wrists. The control group consisted of 15 asymptomatic wrists in volunteers (age range 22–60 years, mean 35 years, 8 men and 7 women). Results and conclusions. Sensitivities and specificities of the six MR criteria were 90.5–100%, and 6.7–86.7%, respectively, both before and after exercise. Likelihood ratios proved statistically significant differences between the symptomatic and asymptomatic wrists (P<0.0001–0.0002) for the prevalence of all MR criteria with the exception of fluid within the carpal joints and/or carpal tunnel. Changes of the MR appearance after exercise had a low sensitivity (4.8–71.4%) but high specificity (86.7–100%) for dynamic carpal tunnel syndrome. In conclusion, MR imaging contributes to the diagnosis of carpal tunnel syndrome when clinical signs are confusing and electrodiagnostic studies are negative. Dynamic examinations improve specificity of MR imaging for such diagnosis.  相似文献

To provide further understanding of the magnetic resonance (MR) signal intensities in the triangular fibrocartilage (TFC) and interosseous ligaments of the wrist, the authors performed MR imaging with gross pathologic and histologic analysis in 10 cadaveric wrists. Spin-echo T1- and T2-weighted coronal images were obtained, and 3-mm coronal sections of the specimens were then made that correlated precisely with the MR images. Normal portions of the TFC showed asymmetrical bow tie-like low signal intensity, except near the radial and ulnar attachments. Degeneration of the TFC, present in all cases, was more severe on the proximal surface and was characterized by high signal intensity on T1-weighted images and less high signal intensity on T2-weighted images. These findings differed from those in TFC perforation, which showed high signal intensity on T2-weighted images. Similar signal intensity characteristics could allow differentiation of degeneration and perforation of the scapholunate and lunotriquetral ligaments. These findings suggest that in vivo MR imaging may accurately delineate degeneration and perforation of the TFC and intercarpal ligaments.  相似文献

A case of osteoid osteoma of the capitate in a 29-year-old male is reported. The patient suffered from unspecific clinical findings and a 3-year history of uncharacteristic wrist pain. Conventional radiographs of the wrist revealed a circumscribed sclerosis in the proximal part of the capitate bone beside a diffuse demineralisation of the carpal bones. Magnetic resonance imaging demonstrated a circumscribed, tumorous lesion with marked enhancement after IV administration of contrast agent and a highly calcified nidus, which was sharply demarcated by a small rim of granulation tissue from the surrounding spongious bone. Based on MRI findings, the diagnosis of an osteoid osteoma was established and confirmed after operation and histologic analysis.
