Purpose

We sought to evaluate the feasibility and efficiency of dual energy (DE) bone and plaque removal in head and neck CT angiography.

Materials and methods

20 patients with suspected carotid stenoses received head and neck DE-CTA as part of their pre-interventional workup. Visual grading using multiplanar reformations (MPR), thick slab maximum intensity projections (MIP) and quantitative vessel analysis (QVA) of stenoses was performed prior and after DE bone removal. Results were evaluated for the detection of relevant stenoses (vessel area reduction >70%). Vessel segmentation errors were analyzed.

Results

Segmentation errors occurred in 19% of all vessel segments. Nevertheless, most post-bone removal artifacts could be recognized using the MPR technique for reading. Compared to MPR reading prior to bone removal, sensitivity, specificity, positive and negative predictive values after bone removal were 100%, 98%, 88% and 100% for MPR reading and 100%, 91%, 63% and 100% for exclusive MIP reading, respectively. There was a good agreement between the QVA results prior and post-DE plaque removal (r2 = 0.8858).

Conclusion

DE bone and plaque removal for head and neck angiography is feasible and offers a rapid and highly sensitive overview over vascular head and neck studies. Due to a slightly limited specificity of the MIP technique due to segmentation errors, possible stenoses should be verified and graded using MPR techniques.  相似文献

Purpose

To evaluate whether dual-energy CT angiography (DE-CTA) could identify links between morphologic and functional abnormalities in chronic pulmonary thromboembolism (CPTE).

Materials and methods

Seventeen consecutive patients with CPTE without underlying cardio-respiratory disease were investigated with DE-CTA. Two series of images were generated: (a) transverse diagnostic scans (i.e., contiguous 1-mm thick averaged images from both tubes), and (b) perfusion scans (i.e., images of the iodine content within the microcirculation; 4-mm thick MIPs). Two radiologists evaluated by consensus the presence of: (a) pulmonary vascular features of CPTE and abnormally dilated systemic arteries on diagnostic CT scans, and (b) perfusion defects of embolic type on perfusion scans.

Results

Diagnostic examinations showed a total of 166 pulmonary arteries (166/833; 19.9%) with features of CPTE, more frequent at the level of peripheral than central arteries (8.94 vs 0.82; p < 0.0001), including severe stenosis with partial (97/166; 58.4%) or complete (20/166; 12.0%) obstruction, webs and bands (37/166; 22.3%), partial filling defects without stenosis (7/166; 4.2%), focal stenosis (4/166; 2.4%) and abrupt vessel narrowing (1/166; 0.6%). Perfusion examinations showed 39 perfusion defects in 8 patients (median number: 4.9; range: 1-11). The most severe pulmonary arterial features of CPTE were seen with a significantly higher frequency in segments with perfusion defects than in segments with normal perfusion (p < 0.0001). Enlarged systemic arteries were observed with a significantly higher frequency ipsilateral to lungs with perfusion defects (9/12; 75%) compared with lungs without perfusion defects (5/22; 22.7%) (p = 0.004).

Conclusion

Dual-energy CTA demonstrates links between the severity of pulmonary arterial obstruction and perfusion impairment, influenced by the degree of development of the systemic collateral supply.  相似文献

Objective

To evaluate the value of dual-source computed tomography (DSCT) in quantitatively measuring pulmonary arteries and major aortopulmonary collateral vessels in comparison with conventional angiographic (CA) on preoperative patients with pulmonary artery atresia and ventricular septal defect (PAA-VSD).

Materials and methods

Twenty PAA-VSD patients who had complete imaging data of DSCT, CA and echocardiography (ECHO) studies were retrospectively analyzed. Using final clinical diagnosis as the standard, results of DSCT, CA and ECHO on the detection of cardiac malformations, measurement of diameters of pulmonary artery and collateral vessel, as well as the values of McGoon ratio, pulmonary arterial index (PAI) and total neopulmonary arterial index (TNPAI) were derived and compared.

Results

In 20 patients, 51 of 54 (94.4%) cardiac malformations were visualized by DSCT, whereas 42 (77.8%) by ECHO (p = 0.027). Fourteen cases with aortopulmonary collateral vessels were all (100%) detected by DSCT, whereas 5 cases (35.7%) by ECHO (p = 0.001), and 13 cases (92.9%) by CA (p = 0.995). Sixteen cases with confluence of native pulmonary arteries were diagnosed by DSCT, whereas 10 cases by CA (p = 0.024). Measurement of the diameters of pulmonary arteries, collateral vessels, and descending aorta at the level of diaphragm were correlated well between DSCT and CA (r = 0.95-0.99). McGoon ratio (DSCT = 1.18 ± 0.60, CA = 1.23 ± 0.64), PAI (DSCT = 130.96 ± 99.38 mm2/m2, CA = 140.91 ± 107.87 mm2/m2) and TNPAI (DSCT = 160.31 ± 125.62 mm2/m2, CA = 169.14 ± 122.81 mm2/m2) were calculated respectively, without significant differences between DSCT and CA by paired t-tests (all p > 0.05).

Conclusion

DSCT was efficient for evaluating and measuring native pulmonary artery and aortopulmonary collateral vessels prior to surgical procedures in PAA-VSD patients. Combined with echocardiography, DSCT showed potential to replace CA for evaluating pulmonary artery atresia noninvasively.  相似文献

Introduction

In preliminary studies DSCT provides robust image quality over a wide range of heart rates and excludes CAD with high accuracy.The aim of the present study was to evaluate the reproducibility of these results in a large, unselected and consecutive group of patients scheduled for invasive coronary angiography (ICA).

Material and methods

170 patients (124 men, 46 women; mean age: 64 ± 9 years) with known CAD (101 patients) or suspected CAD (69 patients) scheduled for ICA were examined by coronary CTA prior to ICA. All coronary segments were assessed for image quality (1: excellent; 5: non-diagnostic). The presence of significant vessel stenosis (>50%) was calculated using ICA as standard of reference.

Results

A total of 680 vessels were analyzed. Despite of 45 arrythmic patients all analyzed coronary segments were diagnostically evaluable. Mean Agatston score equivalent was 686 (range 0-4950). ICA revealed 364 lesions with ≥50% diameter stenosis. DSCT correctly identified 336 of these lesions. 115 lesions with a diameter stenosis ≤50% were overestimated by DSCT and thus considered as false-positive findings. On a per-segment basis, sensitivity was 92%, specificity 93%, positive predictive value (PPV) was 75% and negative predictive value (NPV) 98%. On a per-vessel basis DSCT revealed a sensitivity of 93%, a specificity of 88%, a PPV of 78% and a NPV of 97%. On a per-patient basis sensitivity was 94%, specificity 79%, PPV 88% and NPV 90%.

Conclusions

Initial results of preliminary studies showing robust image quality and high accuracy in DSCT cardiac imaging could be approved with the present study enclosing a large consecutive population. However severe coronary calcifications and irregular heart rate still remain limiting factors for coronary CTA.Despite improved image quality and high accuracy of coronary DSCT angiography, proof of indication is necessary, due to still remaining limiting factors.  相似文献

Purpose

The aim of this study was to evaluate the usefulness of three-dimensional CT angiography (3D CTA) with bone subtraction in a comparison with 3D CTA without bone subtraction for the detection of intracranial aneurysms.

Materials and methods

Among 337 consecutive patients who had intracranial aneurysms detected on 3D CTA, 170 patients who underwent digital subtraction angiography (DSA) were included in the study. CTA was performed with a 16-slice multidetector-row CT (MDCT) scanner. We created the 3D reconstruction images with and without bone subtraction by using the volume rendering technique. Three neuroradiologists in a blinded fashion interpreted both 3D CTA images with and without bone subtraction. The diagnostic accuracy of both techniques was evaluated using the alternative free-response receiver operating characteristic (ROC) analysis. The sensitivity and positive predictive value were also evaluated.

Results

A total of 200 aneurysms (size: 2-23 mm) were detected in 170 patients. The area under the receiver operating characteristic curve (Az) for 3D CTA with bone subtraction (mean, Az = 0.933) was significantly higher than that for 3D CTA without bone subtraction (mean, Az = 0.879) for all observers (P < 0.05). The sensitivity of 3D CTA with bone subtraction for three observers was 90.0, 92.0 and 92.5%, respectively, while the sensitivity of 3D CTA without bone subtraction was 83.5, 83.5 and 87.5%, respectively. No significant difference in positive predictive value was observed between the two modalities.

Conclusions

3D CTA with bone subtraction showed significantly higher diagnostic accuracy for the detection of intracranial aneurysms as compared to 3D CTA without bone subtraction.  相似文献

Background and purpose

Parenchymal lymphomatous brain masses have not been investigated considering if they are primary or as a part of systemic lymphoma (secondary) on imaging studies previously. We aimed to determine characteristics of the secondary parenchymal lymphomatous involvement of the brain and to find if there is any radiologic feature to help discrimination of untreated primary and secondary central nervous system lymphoma on patients’ initial magnetic resonance imaging.

Materials and methods

We evaluated MR images of 18 patients with the diagnosis of primary (n = 12) and secondary central nervous system lymphoma (n = 6). We considered the number, localization, enhancement pattern, signal characteristics, diffusion properties, presence of hemorrhage and presence of butterfly pattern on MR imaging at initial presentation.

Results

Secondary central nervous system lymphomas predominantly presented as multiple (n = 4, 66.7%) lesions. Homogenous nodular enhancement and supratentorial white matter involvement were present in all patients with butterfly pattern and infiltrative/perivenular enhancement in half (n = 3) of the patients. Deep gray matter (n = 1, 16.7%) and infratentorial involvement (n = 1, 16.7%) were scarce and no ring enhancement was observed. There was no statistically significant difference in any of the investigated MR features between the two groups.

Conclusion

Statistical analyses revealed no significant distinctive radiologic characteristics between primary and secondary lymphoma of the brain parenchyma.  相似文献

The purpose of this study is to define the relationship between SPECT and CTA measured parameters of left ventricular (LV) function and volumes obtained in a single session using SPECT/64-slice CT hybrid imaging device, and in addition, to assess the reproducibility of LV parameters measured using 64-slice CTA.

Materials and methods

Seventy-six patients with suspected or known coronary artery disease underwent cardiac CTA and GSPECT in one session using a hybrid SPECT/CT device.LV end-diastolic volume (EDV), end-systolic volume (ESV) and ejection fraction (EF) were measured on each component of the hybrid device. For the CTA component, these parameters were re-measured by the same investigator and by a second investigator with an interval of 3-54 weeks. Corresponding GSPECT and CTA measured parameters were compared. For CTA, intra-observer and inter-observer variability of LV function and volume measurements were calculated.

Results

A very good correlation was found between the GSPECT and CTA measured LVEF (r = 0.81), ESV (r = 0.90) and EDV (r = 0.82). There was a small positive difference by CTA measured LVEF (3.9 ± 14.2%), and more prominent positive differences by CTA measured ESV and EDV (9.8 ± 14.8 and 44.9 ± 23.1 cm3, respectively). There was excellent reproducibility in the measurements of all parameters with very low intra- and inter-observer variability (r = 0.93 for EF and 0.98 for EDV and ESV).

Conclusions

Although a good correlation was found between the EF measurements obtained from CTA and SPECT, interchangeable use of EF measurements between the two modalities should be done cautiously and interchangeable use of LV EDV and ESV should be avoided.  相似文献

Objective

To evaluate dose performance and image quality of 64-slice dual source CT (DSCT) in comparison to 64-slice single source CT (SSCT) in cardiac CT angiography (CTA).

Methods

100 patients examined by DSCT and 60 patients scanned by SSCT were included in this study. Objective indices such as image noise, contrast-to-noise ratio and signal-to-noise ratio were analyzed. Subjective image quality was assessed by two cardiovascular radiologists in consensus using a four-point scale (1 = excellent to 4 = not acceptable). Estimation of effective dose was performed on the basis of dose length product (DLP).

Results

At low heart rates (<70 bpm), image quality of SSCT was equivalent to that of DSCT (P > 0.05), but, at high heart rates (>70 bpm), DSCT provided robust image quality (P < 0.05). The average effective dose of SSCT was 9.3 ± 0.9 mSv at low heart rates (<70 bpm) while, the average estimated effective doses of DSCT were 9.1 ± 1.3 mSv, 8.3 ± 1.1 mSv, 7.9 ± 1.1 mSv, 6.9 ± 0.7 mSv, and 5.9 ± 1.3 mSv, corresponding to heart rates of 50-59 bpm, 60-69 bpm, 70-79 bpm, 80-89 bpm, and 90-100 bpm.

Conclusion

For cardiac CTA, both DSCT and SSCT can get good image quality at low heart rates (<70 bpm) with a similar radiation dose, but, at high heart rates (>70 bpm), DSCT is able to provide robust diagnostic image quality at doses far below that of SSCT.  相似文献

Objective

To describe the main indications and the technical steps to perform ultrasound guided procedures in patients with rheumatic diseases. To access procedures accuracy, safety and effectiveness.

Materials and methods

27 patients with pain related to articular complications of rheumatic diseases and according to previous radiographic or US exam were submitted to several US-guided procedures. 42% of patients (n = 11) had rheumatoid arthritis, 11% (n = 3) spondyloarthropathies, 18% (n = 5) psoriatic arthritis, 15% (n = 4) undifferentiated arthritis, 3% (n = 1) Sjögren syndrome and 11% (n = 3) had gout.Described procedures are synovial biopsies, intra-articular injections of corticosteroids, radiation synovectomy and synovial cysts drainage procedures. When a therapeutical procedure was made, patients were evaluated by 2 rheumatologists.Corticosteroids used were Prednisolone and Triamcinolone. Yttrium-90 was used for synovectomy.

Results

In all cases success was achieved with correct needle placement inside the joint. After injection/aspiration symptoms successfully solved with all patients improving their health status. No complications were recorded during follow-up period.

Conclusions

US-guidance is very reliable to afford a safety procedure always checking the injection, biopsy or aspiration. Guided-biopsy has high success rates obtaining several samples. Thus is also possible to use more powerful/long acting therapeutic drugs aggressive to extra-articular structures avoiding complications.  相似文献

Purpose

To compare 64-multi-slice-CT angiography (64-MSCTA) to color-coded duplex sonography (CCDS) in the follow-up after carotid artery stenting (CAS).

Methods

Thirty patients who had an MSCTA and CCDS examination prior and after CAS were included. Twelve closed-cell and 24 open-cell stents were implanted. Neointimal surface, in-stent-restenosis (ISR), stent expansion, and fracture were evaluated. In addition, the occurrence of atherosclerotic lesions leading to a > 50% stenosis in supraaortic vessels was assessed.

Results

With MSCTA, >50% ISR was found in 5.6% of cases during a mean follow-up of 41.7 months. Comparing MSCTA and CCDS, grading of ISR and absolute diameters of neointimal surface correlated moderately (Spearman = 0.402, p = 0.015; Pearson = 0.404, p = 0.03). Assessment of the neointimal surface was significantly better with MSCTA (100% vs. 80.6%; p = 0.011). Stent expansion was significant, compared to the basic value, with both modalities and stent types (p < 0.001). Of 237 additionally assessed vessel segments, a > 50% stenosis was detected in 38 (16.0%) vessel segments. Findings were stable in 25 (10.5%) and progressed in 11 (4.6%) vessel segments. Five small intracranial aneurysms were detected in four (13.3%) patients. Of 21 incidental findings in 16 (51.6%) patients there was one with malignancy (4.8%).

Conclusion

With regard to ISR and stent expansion, no significant difference was found, when MSCTA and CCDS were compared. CTA is quite applicable as a complementary imaging method for the follow-up of patients with carotid artery stents. Additional advantages are the detection of supraaortic vessel pathologies and incidental findings.  相似文献

Purpose

The aim of this study was (a) to compare arterial enhancement in simultaneously acquired high- and low-kilovoltage images; and (b) to determine whether low tube-voltage imaging would permit PE evaluation on routine chest CT studies or CTPA studies performed with a low volume of contrast media.

Materials and methods

We compared 20 CTPA studies (CTPA group), 20 routine thoracic CT studies (RT group) and 10 CTPA studies performed with reduced volume of contrast media (RC group). HU values were measured in all groups at 80 kVp and 140 kVp images in multiple pulmonary arterial segments bilaterally. The diagnostic quality of the central and peripheral vascular enhancement and the image noise were evaluated at both energies using a five-point scale.

Results

For all patients, the mean CT attenuation values were greater at 80 kVp than 140 kVp images (p < 0.001). At 80 kVp, CTPA group attenuation values were greater than RT group (p = 0.03) with a similar trend at 140 kVp (p = 0.08). At both 140 kVp and 80 kVp, CTPA group attenuation values were greater than RC group (p = 0.02 and p = 0.03, respectively). Qualitative analysis showed that at 140 kVp CTPA studies had better global image quality scores than RT (p = 0.003) and RC (p = 0.001) groups. However, at 80 kVp, there was no significant difference of global image quality between CTPA and the other groups (p = 0.4 and p = 0.5, respectively). Although measurable image noise was greater at 80 kVp than 140 kVp (p < 0.001), qualitative analysis revealed lower image noise at 80 kVp images.

Conclusion

DECT at 80 kVp increases arterial enhancement in both CTPA and routine studies. For routine studies this results in central and peripheral enhancement quality equivalent to that of CTPA studies. Low tube-voltage imaging allows marked contrast volume reduction for CTPA. In selected cases, satisfactory lower radiation dose CT might be achievable using lower kVp imaging alone.  相似文献

Objective(s)

Only few information exist about the diagnostic accuracy of PET/CT for restaging patients with metastatic recurrence of breast carcinoma. Therefore, our study hypothesis was to perform diagnostic contrast enhanced CT (ce-CT) and FDG-PET in a one-step investigation, to prove sensitivity of each modality and to determine whether diagnostic PET/CT adds information over PET or contrast enhanced CT alone for restaging of patients with suspected recurrence of breast cancer.

Methods

Fifty-two patients with suspected recurrence of breast cancer were included in our study. All of them were free of metastasis after the first line therapy. Indications for restaging were: Elevated tumor markers n = 32, clinical deterioration n = 16 and/or suspicious findings on other imaging studies n = 48. Integrated PET/CT was performed using contrast-enhanced diagnostic CT for attenuation correction.

Results

PET was correct in 44/52 patients (85%), ce-CT in 38/52 patients (73%) and PET/CT in 50/52 patients (96%). Sensitivity and specificity of lesion detection of PET, CT and PET/CT were 84%, 66% and 93%, and 100%, 92%, and 100%, respectively.

Discussion

PET/CT can improve staging and alter therapeutic options in patients suspected to have breast cancer recurrence and distant metastatic disease, primarily by demonstrating local or distant nodal involvement occult at other imaging studies. The added value of FDG-PET/CT over other diagnostic modalities is mainly expressed by the fact that a noninvasive whole-body evaluation is possible in a single examination.  相似文献

Purpose

Comparative evaluation of a low-dose scan protocol for a novel bone-subtraction (BS) algorithm, applicable to 64-row cervico-cranial (cc) CT angiography (MSCTA).

Methods and patients

BS algorithm assessment was performed in cadaveric phantom studies by stepwise variation of tube current and head malrotation using a 64-row CT scanner. In order to define minimum dose requirements and the rotation correction capacity, a low dose BS MSCTA protocol was defined and evaluated in 12 patients in comparison to a common manual bone removal algorithm. Standard MIPs of both modalities were evaluated in a blinded manner by two neuroradiologists for image quality composed, of vessel contour sharpness and bony vessel superposition, by using a five-point score each. Effective Dose (E) and data post-processing times were defined.

Results

In experimental studies prescan tube current could be cut down to one-sixth of post-contrast scan doses without compromise of bone-subtraction whereas incomplete subtraction appeared from four degrees head malrotation on. Prescan E amounted to additional 1.1 mSv (+25%) in clinical studies. BS MSCTA performed significantly superior in terms of bony superposition for vascular segments C3-C7 (p < 0.001), V1-V2, V3-V4 (p < 0.05, p < 0.001 respectively) and the ophthalmic artery (p < 0.05), whereas vessel contour sharpness in BS MSCTA only proved superior for arterial segments V3-V4 (p < 0.001) and C3-C7 (p < 0.001). MBR MSCTA received higher ratings in vessel contour sharpness for C1-C2 (p < 0.001), callosomarginal artery (p < 0.001), M1, M2, M3 (p < 0.001 each) and the basilar artery (p < 0.001). Reconstruction times amounted to an average of 1.5 (BS MSCTA) and 3 min (MBR MSCTA) respectively.

Conclusion

The novel BS algorithm provides superior skull base artery visualisation as compared to common manual bone removal algorithms, increasing the Effective Dose by one-fourth. Yet, inferior vessel contour sharpness was noted intracranially, thus limiting the BS algorithm use to patients with suspected vessel pathology at the skull base level.  相似文献

Objective

To assess the relationship between left anterior descending (LAD) coronary artery myocardial bridging detected by 64-slice computed tomography (CT) and clinical findings.

Methods

221 consecutive patients were examined with coronary 64-slice CT angiography. 21 patients with coronary stenosis >50% were excluded. The length, depth, and luminal narrowing of LAD myocardial bridges during systole and diastole were measured. CT findings were compared with the treadmill ECG-stress test, and clinical symptoms.

Results

Myocardial bridges of the LAD were found in 23% of patients (51/221) (length, 14.9 ± 6.5 mm; depth, 2.6 ± 1.6 mm). A significant difference was noted between the LAD luminal diameter before the intramyocardial course and intramyocardially, for both diastole and systole (p < 0.001); with a higher diameter reduction of 27% for end-systole compared to end-diastole with 15% (p = 0.006). Systolic LAD intramyocardial luminal narrowing >50% was found in 3/25 (8%). 30/51 (59%) of bridges were “deep” (>2 mm myocardial depth), 21/51 (41%) were “superficial”. The prevalence of a positive ECG-stress tests for the anterior myocardial region was significantly higher in patients with LAD myocardial bridges (34/50; 68%) compared to those without (28/144; 19.4%) (p < 0.001). There was no difference between “superficial” and “deep” LAD myocardial bridges in regard to a positive treadmill ECG-stress test. Typical angina was rare with 6%.

Conclusion

LAD myocardial bridges are common findings and can possibly explain a positive exercise ECG-stress test for anterior myocardial ischemia. Intramyocardial LAD segments show mild-to-moderate luminal narrowing at rest, which is higher during end-systolic phase.  相似文献

Purpose

To examine the manner in which cardiac function affects the magnitude and timing of aortic contrast enhancement during coronary CT angiography (CTA).

Materials and methods

Twenty-nine patients (21 men, 8 women; mean age, 64.4 ± 13.4 years; mean weight, 59.4 ± 10.3 kg) underwent measurement of cardiac output within 2 weeks of coronary CTA. The cardiac output of each patient was measured by the thermodilution technique and the cardiac index was calculated from the body surface area. During coronary CTA, attenuation of the descending aorta was measured at the workstation every 3 s. The aortic peak time (APT) and aortic peak enhancement (APE) of each patient were calculated. Pearson's correlation coefficient analysis was used to investigate the relationships between the cardiac output or cardiac index and APT or APE. Furthermore, the relationship between patient factors or parameters on test bolus injection and APT or APE was also evaluated.

Results

The range of cardiac output, cardiac index, APT, and APE was 1.55-10.46 L/min (mean: 4.77 ± 2.13), 1.11-5.30 L/(min-m2) (mean: 3.28 ± 1.08), 25-51 s (mean: 38.3 ± 7.5), and 273.1-598.1 HU (mean: 390.4 ± 72.1), respectively. With an increase in the cardiac index, both APT (r = −0.698, p < 0.0001) and APE (r = −0.573, p = 0.0009) decreased. There were significant correlations between the patient body weight and APT and APE with the test bolus injection, and with APT and APE during coronary CTA.

Conclusion

The APT and APE during coronary CTA are closely related to cardiac function.  相似文献

Objective

The purpose of this study was to determine transit times for excretion of Gd-EOB-DTPA into different segments of the hepatobiliary system in patients with normal liver function.

Methods

This retrospective study was IRB approved with a waiver of consent granted. 61 patients (39 female, 22 male, mean age 52.5 years) with normal liver and renal function who underwent contrast enhanced hepatic MRI after injection of 10 mLGd-EOB-DTPA at 1.5 T and 3 T were included. Two readers evaluated all delayed images (3-20 min post contrast) for the presence of contrast agent in the intrahepatic bile ducts (IBD), the common bile duct (CBD), the gallbladder and the duodenum. A two-tailed, unpaired Student's t-test with p < 0.05 deemed significant was used to determine whether transit times were affected by patient gender, age or body mass index.

Results

20 min after contrast initiation, Gd-EOB-DTPA could be detected in the IBD and the CBD in all patients (100%); gallbladder reflux was visible in 53 (86.9%), duodenal excretion in 40 patients (65.5%), respectively. Mean transit times for contrast appearance in the various segments were as follows: IBD 12 min 13 s; CBD 12 min 27 s; gallbladder 13 min 32 s. Transit times were not significantly affected by patient gender, age or BMI.

Conclusion

Within 20 min post contrast initiation, Gd-EOB-DTPA can be expected in the IBD and the CBD in patients with normal liver function. However, functional information about the sphincter Oddi complex can be ascertained only in about two thirds of these patients within this timeframe.  相似文献

Objective

To study the differences in vascular image quality, bone subtraction, and dose of radiation of dual energy CT angiography of the supraaortic trunks using different tube voltages.

Material and methods

We reviewed the CT angiograms of the supraaortic trunks in 46 patients acquired with a 128-slice dual source CT scanner using two voltage protocols (80/140 kV and 100/140 kV). The “head bone removal” tool was used for postprocessing. We divided the arteries into 15 segments. In each segment, we evaluated the image quality of the vessels and the effectiveness of bone removal in multiplanar reconstructions (MPR) and in maximum intensity projections (MIP) with each protocol, analyzing the trabecular and cortical bones separately. We also evaluated the dose of radiation received.

Results

Of the 46 patients, 13 were studied using 80/140 kV and 33 with 100/140 kV. There were no significant differences between the two groups in age or sex. Image quality in four segments was better in the group examined with 100/140 kV. Cortical bone removal in MPR and MIP and trabecular bone removal in MIP were also better in the group examined with 100/140 kV. The dose of radiation received was significantly higher in the group examined with 100/140 kV (1.16 mSv with 80/140 kV vs. 1.59 mSv with 100/140 kV).

Conclusion

Using 100/140 kV increases the dose of radiation but improves the quality of the study of arterial segments and bone subtraction.  相似文献

Objective

We aimed to determine predictors of image quality in consecutive patients who underwent coronary computed tomography (CT) for the evaluation of acute chest pain.

Method and materials

We prospectively enrolled patients who presented with chest pain to the emergency department. All subjects underwent contrast-enhanced 64-slice coronary multi-detector CT. Two experienced readers determined overall image quality on a per-patient basis and the prevalence and characteristics of non-evaluable coronary segments on a per-segment basis.

Results

Among 378 subjects (143 women, age: 52.9 ± 11.8 years), 345 (91%) had acceptable overall image quality, while 33 (9%) had poor image quality or were unreadable. In adjusted analysis, patients with diabetes, hypertension and a higher heart rate during the scan were more likely to have exams graded as poor or unreadable (odds ratio [OR]: 2.94, p = 0.02; OR: 2.62, p = 0.03; OR: 1.43, p = 0.02; respectively). Of 6253 coronary segments, 257 (4%) were non-evaluable, most due to severe calcification in combination with motion (35%). The presence of non-evaluable coronary segments was associated with age (OR: 1.08 annually, 95%-confidence interval [CI]: 1.05-1.12, p < 0.001), baseline heart rate (OR: 1.35 per 10 beats/min, 95%-CI: 1.11-1.67, p = 0.003), diabetes, hypertension, and history of coronary artery disease (OR: 4.43, 95%-CI: 1.93-10.17, p < 0.001; OR: 2.27, 95-CI: 1.01-4.73, p = 0.03; OR: 5.12, 95%-CI: 2.0-13.06, p < 0.001; respectively).

Conclusion

Coronary CT permits acceptable image quality in more than 90% of patients with chest pain. Patients with multiple risk factors are more likely to have impaired image quality or non-evaluable coronary segments. These patients may require careful patient preparation and optimization of CT scanning protocols.  相似文献

Purpose

To explore feasibility of dual-source CT (DS-CT) prospective ECG-gated coronary angiography in patients with heart rate (HR) higher than 70 beat per minute (bpm), and evaluate image quality and radiation dose with comparison to retrospective ECG-gated spiral scan.

Materials and methods

One hundred patients who underwent DS-CT coronary angiography (DS-CTCA) with mean HR higher than 70 bpm but below 110 bpm were enrolled in the study, 50 were scanned by adaptive sequential scan and another 50 were analyzed by retrospectively gated CT scan. The imaging quality of coronary artery segments in the two groups was evaluated using a four-point grading scale by two independent reviewers. Patient radiation dose was calculated by multiplying dose length product by conversion coefficient of 0.017.

Results

There was no significant difference between the two groups for mean HR (p = 0.305), HR variability (p = 0.103), body mass index (p = 0.472), and scan length (p = 0.208). There was good agreement for image quality scoring between the two reviewers (Kappa = 0.72). Coronary evaluability of adaptive sequential scan was 99.7% (608 of 610 segments), while that of retrospective gated scan was 98.7% (614 of 622 segments), showing similar coronary evaluability (p = 0.061). Effective doses of adaptive sequential scan and retrospective gated scan were 5.1 ± 1.6 and 11.8 ± 4.5 mSv, respectively (p < 0.001), showing that adaptive sequential scan reduced radiation dose by 57% compared with that of retrospective gated scan.

Conclusions

In patients with 70-110 bpm HR, DS-CTCA adaptive sequential scan shows similar image quality as retrospective ECG-gated spiral scan with 57% reduction of radiation dose.  相似文献

Purpose

The purpose of this study was to characterize the heartbeat-related displacement of the thoracic aorta in patients with chronic aortic dissection type B (CADB).

Materials and methods

Electrocardiogram-gated computed tomography angiography was performed during inspiratory breath-hold in 11 patients with CADB: Collimation 16 mm × 1 mm, pitch 0.2, slice thickness 1 mm, reconstruction increment 0.8 mm. Multiplanar reformations were taken for 20 equidistant time instances through both ascending (AAo) and descending aorta (true lumen, DAoT; false lumen, DAoF) and the vertex of the aortic arch (VA). In-plane vessel displacement was determined by region of interest analysis.

Results

Mean displacement was 5.2 ± 1.7 mm (AAo), 1.6 ± 1.0 mm (VA), 0.9 ± 0.4 mm (DAoT), and 1.1 ± 0.4 mm (DAoF). This indicated a significant reduction of displacement from AAo to VA and DAoT (p < 0.05). The direction of displacement was anterior for AAo and cranial for VA.

Conclusion

In CADB, the thoracic aorta undergoes a heartbeat-related displacement that exhibits an unbalanced distribution of magnitude and direction along the thoracic vessel course. Since consecutive traction forces on the aortic wall have to be assumed, these observations may have implications on pathogenesis of and treatment strategies for CADB.  相似文献
