The association between recurrent thyroglossal duct cyst (TGDC) and obstructive lingual tonsil hypertrophy has not previously been discussed. We present the case of a 7-year-old child whose medical and surgical course was complicated by these synchronous factors. The aim of this report is to present the risk factors for recurrence of TGDC and the unique challenge that concurrent lingual tonsil hypertrophy and airway obstruction present to management of patients.  相似文献

A patient with a primary oral epidermoid carcinoma with presumed neck metastasis is presented who at operation was found to have a cervical mucoepidermoid carcinoma arising in the wall of a benign cyst. The case for considering this tumor a primary branchioma of mucoepidermoid type is presented, and the criteria for making the diagnosis of branchiogenic carcinoma are discussed.  相似文献

Cervical thymic cysts are rare embryonic remnants that develop along the course of thymic migration in the neck. They usually occur during infancy and childhood, and they are extremely rare in adults. We report a case of cervical thymic cyst in a 53-year-old man. The patient presented with a small mass of the thyroid gland and a cystic mass at the left level II area of the neck. On histopathology, the thyroid mass was identified as a papillary carcinoma and the left-sided neck mass was diagnosed as a cervical thymic cyst lined with nonkeratinizing, flattened squamous epithelium. The cyst wall contained atrophic thymic tissue composed of lymphoid cells, epithelial cords, and Hassall corpuscles. Although it is rare, cervical thymic cyst should be considered in the differential diagnosis of a lateral cystic neck mass in an adult.  相似文献

Squamous carcinomas of the oro-pharynx are associated with a high incidence of distant metastases at post-mortem. A case of a massive distant metastasis from a squamous cell carcinoma of the tonsil is presented. This occurred despite excellent local control. In 1974, we reported on six patients with squamous cell carcinoma arising in the tonsillar area who presented with a characteristic form of cystic nodal metastasis. This study reports on an additional 21 patients with this entity. Treatment of these patients included removal of the cystic metastasis with or without radical neck dissection and irradiation of Waldeyer's ring. Two patients have died of unrelated disease and two were lost to follow-up, but the remainder are alive without disease 2 months to 12 years following initial diagnosis. We believe that cystic metastases from tonsillar squamous cell carcinomas have often been mistaken for either primary squamous cell carcinomas of branchiogenic origin or with branchial cleft cysts. The distinctive histological features of cystic metastases reviewed after its correct identification can lead to the discovery of an unsuspected primary lesion and result in specific treatment options.  相似文献

The occult primary is a problem well known to head and neck surgeons. Less frequently discussed, but well documented, is cystic degeneration of metastatic nodes. When these conditions combine, they can suggest tumor arising in a pre-existing cervical cyst. This is a controversial condition, rare if it occurs at all, and is not to be invoked lightly.  相似文献

We present a case of solitary cystic lymph node metastasis from an occult thyroid papillary carcinoma mimicking a lateral cervical cyst, and a case of thyroid papillary carcinoma with a true lateral cervical cyst. Based on a comparison of the radiographical findings of the two cases, differential diagnosis between cystic lymph node metastasis of papillary carcinoma and branchial cyst is discussed.  相似文献

The report is of a 50-year-old man with renal cell carcinoma (RCC) who had rapidly progressing metastasis to the tongue at 10 months after the left radical nephrectomy. The metastatic lingual tumor was not resectable, therefore treated with radiation (50 Gy). The tumor disappeared macroscopically after the radiation therapy, but enlarged again 4 months later. The patient died of respiratory failure due to multiple lung metastases 12 months after the appearance of the lingual metastasis. Radiation therapy is an acceptable palliative strategy for advanced lingual metastasis of RCC.  相似文献

Pharyngeal tuberculosis is very rare and normally appears as an seemingly malignant process. As a result of which it is essential to perform a differential diagnosis before beginning anticancer treatment. We present a clinical case of lingual tonsil tuberculosis. A review is made of the pathogenic mechanisms, the clinical manifestations and the diagnosis of pharyngeal tuberculosis, whose response to treatment with antituberculous drugs proves to be excellent.  相似文献

Lingual lymph node metastases are rarely seen in carcinoma of the tongue, and these nodes are not removed during neck dissection. Lingual lymph nodes are classified into medial and lateral groups, and metastasis to the former is extremely rare. A 55-year-old male with squamous cell carcinoma of the tongue, (stage T4aN0M0), underwent hemiglossectomy with neck dissection and free flap reconstruction. The lingual septum had a mass, 8 mm in size, which was diagnosed as medial lingual lymph node metastasis on histopathology. The patient developed multiple distant metastases and died of disease 18 months after the initial surgery. The presence of medial lymph node metastasis could result in contralateral neck metastases and worsen prognosis. Such cases may warrant more intensive therapy than recommended by current guidelines.  相似文献

A patient was seen with a thyroid cyst. Fine- and core-needle biopsies revealed no evidence of tumor. A thyroid cyst lined by malignant squamous cells was found at surgery. Invasive squamous carcinoma was found to arise in adjacent trachea. The thyroid gland was normal except for inflammatory changes. Aspiration biopsies of thyroid cysts may be misleading, especially when hemorrhagic; sclerosis should be performed with caution.  相似文献
