PURPOSE: To evaluate the effectiveness of prophylactic topical apraclonidine 1% in preventing an intraocular pressure (IOP) rise in the early period after uneventful phacoemulsification with intraocular lens (IOL) implantation. SETTING: District general hospital, United Kingdom. METHODS: In this prospective masked randomized trial, 61 patients had elective, routine, corneal tunnel, sutureless phacoemulsification with in-the-bag foldable IOL implantation. A single surgeon operated on all the patients. Patients were randomized to receive topical apraclonidine 1% eyedrops (n = 31) or artificial tears (control group, n = 30) 1 hour preoperatively and at the end of the surgery. An observer masked to the perioperative drops used measured the IOP preoperatively and 3 to 6 hours and 16 to 24 hours postoperatively. The primary outcome was the change in IOP between the baseline and the 2 postoperative intervals. The IOP changes within and between the groups were analyzed using the t test and chi-square test. RESULTS: The changes between the postoperative and preoperative IOPs in the study groups were statistically significant (apraclonidine, P = 0.018 and P = 0.007, respectively; artificial tears, P = 0.028 and P = 0.023, respectively; paired t test). There was no significant difference in the postoperative IOP between the apraclonidine and control groups 3 to 6 hours and 16 to 24 hours postoperatively (P = 0.717 and P = 0.497, respectively; independent t test). The mean difference was 0.2 mm Hg (95% confidence interval [CI], -3.4 to 3.1) in the apraclonidine group and 2.2 mm Hg (95% CI, -2.5 to 7.0) in the control group. In each group, a few patients had an IOP greater than 30 mm Hg in the first 24 hours. CONCLUSION: Prophylactic topical perioperative apraclonidine 1% did not cause a significant reduction in the postoperative IOP when compared with a control group.  相似文献

目的:探讨超声乳化(Phacoemulsification,Phaco)联合折叠型人工晶状体植入术对急性闭角型青光眼的临床疗效。方法:选择>50岁患有急性闭角型青光眼伴有晶状体混浊的连续性患者20例23眼,行超声乳化联合折叠型人工晶状体植入术,比较手术前后视力和眼压的变化,超声生物显微镜(ultrasound biomicroscopy,UBM)检测手术前后前房深度(anterior chamber depth,ACD)和房角开放距离(an-gle opening distance,AOD500)的变化。结果:手术前视力和眼压是4.16±0.38和48.65±7.34mmHg,手术后视力和眼压为4.75±0.10和16.65±1.97mmHg;术前术后的前房深度为1.35±0.04mm和3.51±0.07mm,房角开放距离分别为225.09±12.43μm和511.18±12.20μm,手术前后比较差异有统计学意义(P<0.01)。结论:超声乳化联合人工晶状体植入术治疗急性闭角型青光眼安全、有效。  相似文献

PURPOSE: To compare intraocular pressure (IOP) after phacoemulsification and foldable intraocular lens (IOL) implantation using a temporal sclerocorneal or clear corneal incision. SETTING: Department of Ophthalmology, Johannes Gutenberg-University, Mainz, Germany. METHODS: One hundred patients (100 eyes) with cataract having phacoemulsification with posterior chamber IOL implantation were randomly assigned to receive a temporal sclerocorneal or clear corneal tunnel incision. Intraocular pressure was measured preoperatively and 6 hours, 1, 2, and 3 days, and 5 months postoperatively. Statistical significance was determined by nonparametric group comparisons using 2-sample random Wilcoxon tests. RESULTS: Six hours postoperatively, the median IOP increase was significantly higher in the sclerocorneal tunnel group (57%) than in the clear corneal incision group (18%) (P <.001). No significant between-group difference in IOP was found at 1, 2, or 3 days or 5 months. At 5 months, IOP was 0.6 mm Hg lower than preoperatively in the sclerocorneal tunnel group and 1.5 mm Hg lower in the clear corneal group. CONCLUSIONS: After phacoemulsification and foldable IOL implantation, the immediate postoperative IOP increase was higher in eyes having a sclerocorneal incision than in those having a clear corneal tunnel incision. These results could be important in eyes with decreased outflow facility or preexisting optic nerve damage.  相似文献

PURPOSE: To investigate the influence of uneventful phacoemulsification on anterior chamber depth (ACD), iridocorneal angle (ICA) width, and intraocular pressure (IOP) in nonglaucomatous eyes with open ICA preoperatively. SETTING: Beyoglu Eye Education and Research Hospital, Istanbul, Turkey. METHODS: Fifty-three eyes of 49 patients were evaluated for 6 months postoperatively. The nonparametric Wilcoxon signed rank test was used to compare preoperative and postoperative IOP, ACD, and ICA width. Univariate and multivariate regression analyses were used to evaluate the relationships between IOP, ACD, and ICA width and between preoperative patient characteristics. RESULTS: The mean preoperative IOP of 15.1 mm Hg +/- 2.8 (SD) dropped postoperatively to 13.4 +/- 3.4 mm Hg at 1 day, 13.3 +/- 2.6 mm Hg at 1 week, 13.2 +/- 3.1 mm Hg at 1 month, 13.3 +/- 2.7 mm Hg at 3 months, and 14.1 +/- 2.5 mm Hg at 6 months (P<.05). The mean preoperative ICA grade of 2.97 +/- 0.72 increased to 3.55 +/- 0.48 at 1 week and 3.68 +/- 0.45 at 1 month (P<.05). The mean preoperative ACD of 3.06 +/- 0.49 mm increased to 3.57 +/- 0.47 mm at 4 weeks, 3.69 +/- 0.32 mm at 1 month, and 3.70 +/- 0.36 mm at 3 months (P<.05). The IOP decrease was not correlated with the changes in ICA width or ACD. Multiple regression analysis showed preoperative IOP was the single predictor of the postoperative IOP drop (P<.001). CONCLUSIONS: In nonglaucomatous eyes with an open ICA preoperatively, uneventful phacoemulsification reduced IOP, increased ACD, and widened the ICA. 目的:探讨利用Pentacam三维眼前节分析诊断系统观察年龄相关性白内障患者超声乳化手术前后前房容积(anteriorchamber volume,ACV)、前房深度(anterior chamber depth,ACD)、前房角(anterior chamber angle,ACA)的变化。方法:临床选取2007-10/2009-05在本院眼科中心行超声乳化吸除折叠型人工晶状体(intraocular lens,IOL)植入手术的年龄相关性白内障患者200例220眼,闭角型青光眼病史及其他眼疾的患者排除。对全部患者应用Pentacam分别测量术前、术后1,6mo的ACV,ACD和ACA(鼻侧,颞侧,上方,下方),Goldmann压平眼压计测量各期眼内压(intraocular pressure,IOP)。应用t检验和one-wayANOVA数据分析,并应用Pearson相关分析IOP与ACV,ACD,ACA之间关系。结果:术前ACV,ACD,ACA,IOP依次为152.25±39.87mm3,2.86±0.55mm,28.56°±4.87°,15.54±2.23mmHg;术后1mo,ACV,ACD,ACA,IOP依次为189.65±36.73mm3,4.02±0.53mm,39.72°±5.02°,13.02±1.97mmHg;术后6mo,ACV,ACD,ACA,IOP依次为191.47±33.79mm3,4.01±0.38mm,42.02°±5.98°,12.76±1.52mmHg。ACV,ACD及鼻侧,颞侧,上方,下方4个象限的ACA在术后1,6mo均较术前显著增大,眼压显著下降,差异有统计学意义;术后6mo与1mo相比,差异无统计学意义。结论:Pentacam系统检查手术前后眼前节为非接触性,自动快速易操作。Pentacam测量年龄相关性白内障超声乳化吸除联合折叠型IOL植入术后1,6mo ACV,ACD均增大,4个象限ACA均增大,同时伴有眼压显著下降。术后6mo与1mo基本无差异。  相似文献

目的分析白内障超声乳化吸出人工晶状体植入术后24h眼压变化。方法选取125例(140只眼)进行白内障超声乳化吸出植入折叠式人工晶状体术患者,术中无并发症,在术前、术后4~8h、术后24h测眼压进行观察和分析。结果术后4~8h为眼压升高峰值期,眼压升高发生率为32.14%,眼压≥24mmHg为10.71%,最高值可达到40mmHg,术后24h后眼压逐渐下降。结论术后眼压的变化与粘弹剂的应用、核硬度及超声能量等因素有关。要重视白内障超声乳化术后早期眼压的监测,特别是术后4~8h的眼压测量。术中应该注意选择优质的粘弹剂或者合理的结合使用,并早期进行眼压观察。  相似文献

PURPOSE: To investigate the position of 3-piece foldable intraocular lenses (IOLs) after piggyback implantation for high hyperopia. SETTING: University Eye Hospital, Johann Wolfgang Goethe University, Frankfurt am Main, Germany. METHODS: Eight eyes of 5 highly hyperopic patients had phacoemulsification and implantation of 2 foldable IOLs. In 3 eyes, both IOLs were implanted in the capsular bag. In 5 eyes, 1 IOL was placed in the capsular bag and the second IOL in the ciliary sulcus. Intraocular lens optic tilt and decentration, combined thickness of both IOLs, and anterior chamber depth (ACD) were measured postoperatively over a period of 18 months using Scheimpflug photography. RESULTS: All eyes with both IOLs in the capsular bag showed interpseudophakic opacification, with a mean increase in combined IOL thickness of 0.4 mm, a decrease in ACD of 0.3 mm, and a corresponding hyperopic shift of 4.00 diopters. Eyes in which the anterior IOL was placed in the ciliary sulcus showed no changes in refraction or combined IOL thickness. In these eyes, the anterior IOL had a higher mean decentration (0.49 mm +/- 0.20 [SD] after 12 months) than the posterior IOL (0.21 +/- 0.13 mm after 12 months). CONCLUSIONS: Piggyback IOL implantation with placement of 2 foldable IOLs in the capsular bag can be followed by a hyperopic shift that may be caused in part by displacement of the IOLs. Placement of the anterior IOL in the ciliary sulcus can lead to higher decentration of this IOL.  相似文献

目的利用UBM(ultrasound bio microscopy)对高度近视眼及其对照组正视眼白内障患者在超声乳化白内障吸除折叠式人工晶状体植入的术前、术后进行房角形态量化观察并比较分析,旨在探讨高度近视眼合并白内障手术的安全性。方法选取高度近视眼组和正视眼组做为其对照组,每组白内障的患者各30例(30只眼),采用透明角膜切口白内障超声乳化折叠式人工晶状体植入术,于术前、术后1、3个月用UBM测量前房深度(anteri-orchamber distance,ACD)、房角开放度(openangle,AA)、房角开放距离(angle opening distance,AOD)、小梁睫状体距离(trabecular-ciliary processes distance,TCPD)和虹膜睫状体距离(iris-ciliary processes distance,ICPD),同时记录眼压的变化并进行分析。结果对照组术前与术后1、3个月相比,ACD、AA、AOD500、TCPD、ICPD有显著性差异,术后1个月和术后3个月相比无显著性差异。术后眼压与术前相比有显著性差异。高度近视眼组术前与术后1、3个月相比各项测量指标有显著性差异,术后1个月和术后3个月相比无显著性差异。术后眼压与术前相比有显著性差异。高度近视眼与正视眼相比较,术前及术后除外ICPD,其它各项指标均存在显著性差异。结论高度近视眼合并白内障选择透明角膜切口超声乳化吸除折叠式人工晶状体植入术术后与术前相比前房明显加深,房角明显加宽,眼压也有一定程度的下降;与正视眼相比术前和术后的前房深度、房角宽度均大于后者,进一步证实了高度近视眼独特的前节结构,提示透明角膜切口超声乳化白内障吸除折叠式人工晶状体植入术安全有效。  相似文献

AIM: To evaluate the safety and efficacy of scleral-fixated 3-looped haptics intraocular lens (IOL) implantation for surgical management of microspherophakia. METHODS: A retrospective case series include 10 microspherophakic patients (15 eyes) who underwent lens removal plus a modified surgical treatment of scleral-fixated 3-looped haptics IOL implantation. The primary outcomes involved visual acuity, intraocular pressure (IOP). Secondary outcomes were spherical equivalent (SE), anterior chamber depth (ACD), corneal endothelial cell density and postoperative complications. RESULTS: After a postoperative follow-up of 17.60±15.44mo, improved visual outcomes can be observed. The uncorrected distance visual acuity (UCVA) logMAR improved from 1.54±0.59 preoperatively to 0.51±0.35 postoperatively (P=0.001), and best corrected visual acuity (BCVA) logMAR improved from 0.97±0.91 preoperatively to 0.24±0.23 postoperatively (P=0.003). Moreover, the SE decreased from -9.58±7.47D preoperatively to -0.65±2.21 D postoperatively (P<0.001). In terms of safety profile, the average IOP decreased from 21.10±12.94 mm Hg preoperatively to 14.03±3.57 mm Hg postoperatively (P=0.044), and the previously elevated IOP of three eyes decreased to the normal range. The ACD increased from 2.25±1.45 mm preoperatively to 3.35±0.39 mm postoperatively (P=0.017). The density of corneal endothelial cells did not change significantly after surgery (P=0.140). The posterior chamber IOLs were well centered and no severe complications were found. CONCLUSION: Lens removal plus the modified surgical treatment of scleral-fixated 3-looped haptics IOL implantation can help in improvement of visual acuity, which can be regarded as a relative safe method for the surgical management of microspherophakia.  相似文献

BACKGROUND AND OBJECTIVE: To determine whether the diameter of the capsulorhexis has an effect on anterior chamber depth (ACD) following phacoemulsification surgery. PATIENTS AND METHODS: Fifty-one consecutive patients were selected for cataract extraction by phacoemulsification with intraocular lens (IOL) implantation. Twenty-two of 51 patients underwent 4 mm capsulorhexis, while the rest underwent 6 mm. All were implanted with a multi-piece polymethyl methacrylate posterior chamber IOL with 5.0 mm diameter biconvex optic and flexible haptic. They were followed 3 months postoperatively. The width of the capsulorhexis was assured according to the IOL optic implanted intraoperatively, and by the help of slit-lamp measurement after dilatation of the pupil on the first postoperative day. ACD and axial length (AL) of patients was obtained by ultrasonography on both the days before surgery, and the first and seventh postoperative days, and after 30, 60 and 90 days. RESULTS: Early significant increase of ACD and ACD/AL ratios were observed in only the 6 mm capsulorhexis group on the first day postoperatively (P = .012, and P = .018). On the 90th postoperative day, ACD increased significantly both in the 4 mm (P = .002) and the 6 mm capsulorhexis groups (P = .049) when compared to preoperative values. For the same period, meaningful increase in ACD/AL ratio in the eyes with both 4 mm and 6 mm capsulorhexis groups was also noted compared with preoperatively (P = .002 and P = .019). There was a statistical difference between the 90th day ACD values of 4 mm (3.73 +/- 0.32 mm, mean +/- standard deviation) and 6 mm capsulorhexis groups (3.50 +/- 0.33 mm) (P = .028). For the same period, ACD/AL ratio was also significantly different for both groups (0.152 +/- 0.01, and 0.142 +/- 0.01 respectively) (P = .004). The refractive error changes followed the ACD changes and showed meaningful differences between 1st and 90th days postoperative values of each group (P = .029, and P = .014, respectively). CONCLUSION: A 4 mm capsulorhexis results in a longer postoperative ACD than does a 6 mm capsulorhexis for the IOL type used in this study.  相似文献

PURPOSE: To use anterior segment optical coherence tomography (AS-OCT) to evaluate anterior chamber depth (ACD) and angle after phacoemulsification and intraocular lens (IOL) implantation. SETTING: Department of Ophthalmology, Yeditepe University, Istanbul, Turkey. METHODS: Forty-seven eyes (37 patients) had uneventful phacoemulsification and IOL implantation through a clear corneal incision. Anterior segment OCT was performed preoperatively and 1 week and 1 month postoperatively. The angle-referenced (ACD(1)), pupil-referenced (ACD(2)), and lens-referenced (ACD(3)) ACDs, crystalline lens rise (CLR), pseudophakic posterior chamber depth (PPCD), and nasal and temporal iridocorneal angles were measured. Statistical analysis was by a 1-way analysis of variance and Pearson correlation analysis. RESULTS: The mean age of patients was 70 years+/-10.17 (SD). The mean ACD(1) (mean increase) was 3.06+/-0.25 mm preoperatively, 3.16+/-0.22 mm at 1 week (0.1 mm), and 3.16+/-0.19 mm at 1 month (0.1 mm); the mean ACD(2), 2.76+/-0.47 mm preoperatively, 3.62+/-0.24 mm at 1 week (0.86 mm), and 3.63+/-0.20 mm (0.87 mm) at 1 month; and the mean ACD(3), 2.54+/-0.46 mm preoperatively, 3.97+/-0.28 mm at 1 week (1.43 mm), and 3.91+/-0.25 mm at 1 month (1.37 mm). The mean CLR was 0.497+/-0.363 mm and the mean PPCD, 0.322+/-0.150 mm. The increase in nasal and temporal iridocorneal angles was statistically significant at both postoperative examinations (P<.01). CONCLUSION: Deepening of the anterior chamber and widening of the nasal and temporal angles after cataract extraction was shown on AS-OCT.  相似文献

目的:观察超声乳化白内障吸除术联合后房型折叠式人工晶状体植入术治疗急性闭角型青光眼合并白内障患者的临床疗效。方法:42例(42眼)急性闭角型青光眼合并白内障患者入院后均行超声乳化白内障吸除术联合后房型折叠式人工晶状体植入术,术后随访3mo。结果:术后最佳校正视力,较术前显著提高(P<0.05);患者术后3mo眼压平均为(13.5±3.2)mmHg,较术前用药前后眼压相比均明显下降(P<0.05);前房角镜检查发现术后前房角均有增宽,房角关闭及周边虹膜前粘连范围减小;中央前房深度为(3.12±0.46)mm,手术前后相比,差异有统计学意义(P<0.05)。结论:超声乳化白内障吸除术联合后房型折叠式人工晶状体植入术可有效降低眼压、提高视力,为急性闭角型青光眼同时合并白内障患者安全有效的治疗途径之一。  相似文献

PURPOSE: To evaluate intraocular pressure (IOP) after phacoemulsification and intraocular lens (IOL) implantation in nonglaucomatous eyes with and without exfoliation. SETTING: Department of Ophthalmology, Helsinki University Central Hospital, Helsinki, Finland. METHODS: One hundred ninety-six eyes that had phacoemulsification with IOL implantation were examined. Eyes with a history of intraocular disease or surgery that could affect IOP were excluded. The study comprised the remaining 160 eyes: 23 with exfoliation (EXF group) and 137 without exfoliation (non-EXF group). Follow-up data were available for 136 eyes. The same surgeon performed all surgeries. Patients were examined on the first postoperative day and after 1 week, 4 months, and 1.0 to 2.7 years. RESULTS: One day postoperatively, IOP rose in the EXF group from a mean preoperative level of 16.3 mm Hg +/- 2.7 (SD) to 21.0 +/- 8.5 mm Hg, a 28.4% increase (P =.0061). In the non-EXF group, mean IOP rose from 16.2 +/- 3.4 mm Hg to 20.5 +/- 5.7 mm Hg, a 29.9% increase (P =.001). In 4 eyes (17.4%) in the EXF group and 8 eyes (5.8%) in the non-EXF group, IOP increased to 30 mm Hg or higher at 1 day. After this, significant IOP decreases occurred in both the EXF and non-EXF group, respectively, as follows: 14.2 +/- 3.0 mm Hg (12.0% decrease from preoperative value; P =.001) and 15.0 +/- 2.9 mm Hg (5.9%; P =.001) 1 week postoperatively; 12.8 +/- 2.7 mm Hg (20.2%; P =.0002) and 13.8 +/- 2.7 mm Hg (13.2%; P =.001) after 4 months; and 12.3 +/- 2.6 mm Hg (23.2%; P =.0001) and 12.7 +/- 2.7 mm Hg (21.2%; P =.001) after 1.0 to 2.7 years. There was no significant difference between the 2 groups. CONCLUSION: After phacoemulsification with IOL implantation, IOP decreased significantly and remained lower than preoperatively in eyes with and without exfoliation. One day postoperatively, transient pressure peaks were more common in eyes with exfoliation. One eye without exfoliation developed glaucoma.  相似文献

PURPOSE: To compare best corrected visual acuity (BCVA) and intraocular pressure (IOP) in eyes that had a foldable intraocular lens (IOL) implanted with the use of an anterior chamber maintainer (ACM) in 1 eye and hyaluronate 3.0% (Vitrax) viscoelastic material in the other eye. SETTING: Private practice in a large urban center. METHODS: Sixty-six eyes of 33 patients had consecutive bilateral cataract extraction by the same surgeon using an identical temporal approach, clear corneal phacoemulsification, and implantation of an Allergan SI-40NB or SA-40NB IOL through an injection delivery system. The first operative eye was arbitrarily assigned to the ACM or Vitrax group, while the fellow eye received the opposite technique. Patients with coexisting ocular pathology that could influence the measured outcomes (eg, glaucoma, age-related macular degeneration) were excluded. Patients were evaluated preoperatively and 1 day and 3 to 6 weeks postoperatively. RESULTS: No serious intraoperative or postoperative complications were reported. The only statistical difference in BCVA or IOP was on the first postoperative day, when the mean IOP was approximately 2 mm Hg lower in the ACM group than in the Vitrax group. CONCLUSION: The use of an ACM for implantation of an injected 3-piece silicone IOL may lower IOP on the first postoperative day.  相似文献

Purpose: This study set out to evaluate the influences of uneventful phacoemulsification on the anterior segment parameters obtained with the Pentacam rotating Scheimpflug camera and intraocular pressure (IOP). Methods: A total of 42 eyes of 34 patients (26 men, eight women) were evaluated preoperatively, and at 1, 3 and 6 months postoperatively with the Pentacam. Intraocular pressure was measured with the Goldmann applanation tonometer. The non‐parametric paired t‐test was used to compare preoperative and 1‐, 3‐ and 6‐month postoperative measurements of anterior chamber depth (ACD), anterior chamber volume (ACV), anterior chamber angle (ACA) width, central corneal thickness (CCT), the central 3‐, 5‐ and 7‐mm corneal volume (CV), pupil size, and IOP. Pearson’s correlation test was used to evaluate the relationships between IOP and ACD, ACV and ACA width. Results: The differences between ACD, ACV, ACA and IOP values taken preoperatively and those taken postoperatively at 1, 3 and 6 months were statistically significant (p < 0.05). The differences between CCT, central 3‐, 5‐ and 7‐mm CV, and pupil size measurements taken preoperatively and those taken postoperatively at 1, 3 and 6 months were not statistically significant (p > 0.05). The decrease in IOP was not correlated with the changes in ACD, ACV and ACA (p > 0.05). Conclusions: Uneventful phacoemulsification significantly reduced IOP, increased ACD and ACV, and widened the ACA. However, alterations in CV, CCT and pupil size values were not statistically significant. Alterations in ACD, ACV, ACA and IOP remain stable after the first month of surgery.  相似文献

PURPOSE: To examine prospectively the effects of small-incision phacoemulsification and intraocular lens implantation (PEA+IOL) on anterior chamber depth (ACD) and intraocular pressure (IOP) using a newly developed scanning peripheral ACD analyzer (SPAC). MATERIALS AND METHODS: Twenty-eight eyes of 21 patients who underwent PEA+IOL without any complications were examined and divided into 7 eyes each of grades 1 to 4 according to the Van Herick technique. The SPAC measured ACD consecutively from the vicinity of the pupil center to the periphery at 0.4-mm intervals. Changes in ACD and IOP as a result of PEA+IOL were investigated, and the factors contributing to the changes in ACD and IOP were examined. RESULTS: PEA+IOL increased ACD significantly at all groups. The average changing rates were 3.19 +/- 0.67 times (Van Herick grade 1), 2.00 +/- 0.80 times (Van Herick grade 2), 1.92 +/- 0.32 times (Van Herick grade 3), and 1.36 +/- 0.65 times (Van Herick grade 4), respectively. The closer to the pupil center the measurement point was, the larger was the increase in ACD. However, the rates of increase in ACD were similar among the measurement points. The increases in ACD were significantly large in patients having a shallow preoperative ACD and a small optic axis length. The IOP reduction became significantly large in eyes with a shallow preoperative ACD. CONCLUSIONS: The SPAC enabled quantitative measurement of changes in ACD from the vicinity of the pupil center to the periphery as a result of PEA+IOL. PURPOSE: To determine the incidence of hypotony and intraocular pressure (IOP) elevation in the immediate and early postoperative period after temporal posterior limbal phacoemulsification and intraocular lens (IOL) implantation. SETTING: Ambulatory surgical center. METHODS: This prospective analysis comprised 310 eyes that had temporal posterior limbal phacoemulsification with IOL implantation. Surgical parameters included keratome incision of 2.85 mm, incision length of 2.5 mm, peribulbar anesthesia, case-completion IOP of 20 mm Hg, and postoperative lid taping. The IOP measurements were collected preoperatively and 30 minutes and 1 day after surgery. RESULTS: Nineteen eyes (6.1%) had an IOP lower than 5 mm Hg 30 minutes postoperatively in the absence of incision leakage at the paracentesis or keratome sites. Eighteen of the 19 eyes with postoperative hypotony received hydroxypropyl methylcellulose 2% (OcuCoat) and 1 received hypromellose 2% (Cellugel). None of the 23 eyes with an acrylic IOL implanted via a cylindrical lens inserter had an IOP lower than 5 mm Hg postoperatively. Suturing did not significantly affect the incidence of hypotony, and there were no postoperative complications related to hypotony. The mean IOP at 30 minutes was lower than at 1 day in the normal, glaucoma, and glaucoma-suspect groups. Twenty-one normal eyes (8.1%), 5 glaucoma eyes (15.6%), and 1 glaucoma-suspect eye (5%) had an IOP greater than 30 mm Hg 1 day postoperatively. CONCLUSIONS: Postoperative hypotony (IOP <5 mm Hg) occurred in 19 (6.1%) of 310 eyes. At 1 day, IOP higher than 30 mm Hg was more frequent in glaucoma eyes than in normal eyes. Although there were no direct problems related to hypotony at 30 minutes or to elevated IOP (>30 mm Hg) at 1 day, surgeons should be aware of and check for IOP variability (low and high) that can occur in normal, glaucoma, and glaucoma-suspect eyes within the first 24 hours after surgery.  相似文献

PURPOSE: To examine the change in intraocular pressure (IOP) within 1 week of phacoemulsification and foldable posterior chamber intraocular lens (PC IOL) implantation using Adatocel (hydroxypropyl methylcellulose 2% [HPMC]). SETTING: Department of Ophthalmology, University of Sciences, Faculty of Medicine, Pécs, Hungary. METHODS: In this prospective study, the IOP in 118 eyes of 118 patients (57 men, 61 women, mean age 68 years +/- 7.8 [SD]) with no history of glaucoma was assessed by Goldmann applanation tonometry 2 to 3, 6 to 8, and 22 to 24 hours and 1 week after uneventful phacoemulsification and PC IOL implantation. The effect of the removal of Adatocel ("partial removal" from the anterior chamber [AC] only versus "complete removal" from behind of the IOL as well), the lens type (Medicontur 601 HP versus Bausch & Lomb Hydroview), and the type of anesthesia (topical versus parabulbar) were compared. Statistical analysis was performed using the Student t test, and P< or =.05 was considered statistically significant. RESULTS: The mean preoperative IOP was 13.83 +/- 2.5 mmHg. There were no significant differences at any time in postoperative IOP measurements between the 2 IOL types and the 2 modes of anesthesia. At 2 to 3 hours, 6 to 8 hours, and 22 to 24 hours, the IOP was significantly higher in the 30 eyes in which the Adatocel was partially removed (from the AC only) than in the 88 eyes in which it was completely removed (from behind the PC IOL as well) (P< or =.05, P< or =.01, and P< or =.001, respectively). CONCLUSION: Severe postoperative IOP spikes in nonglaucomatous patients after uneventful phacoemulsification cataract surgery are rare. The type of implanted PC IOL and the mode of anesthesia had no significant effect on postoperative IOP. Total removal of the ophthalmic viscosurgical device, even when using HPMCs such as Adatocel, is necessary to prevent postoperative IOP spikes.  相似文献

目的 观察和分析剥脱综合征性青光眼患者行超声乳化白内障摘除人工晶体植入术后的眼压变化。方法 对7例伴有高眼压、明显影响视力的白内障但尚无严重的视神经、视野损害的剥脱综合征患者施行超声乳化白内障摘除人工晶体植入术,比较手术前后的眼压变化。结果 术前平均眼压31.57mmHg,术后1周时平均眼压18.19mmHg,降幅为13.38mmHg,术前术后眼压差异极显著。结论 伴有明显影响视力白内障的剥脱综合征性青光眼病人在行超声乳化白内障摘除人工晶体植入术后眼压会显著下降,对那些尚未发生明显视神经视野损害的剥脱综合征性青光眼患者,行超声乳化白内障摘除人工晶体植入术可有效降低眼压。  相似文献

PURPOSE: To analyze the results of 1-way phacoemulsification and posterior chamber intraocular lens (IOL) implantation combined with trabeculectomy. SETTING: Department of Ophthalmology and Neurosurgery, University of Siena, Siena, Italy. METHODS: This retrospective study comprised 42 eyes of 36 patients with glaucoma and cataract who had phacoemulsification with posterior chamber IOL implantation combined with trabeculectomy. The mean follow-up of 28.24 months +/- 10.99 (SD) (range 11 to 52 months) included measurement of intraocular pressure (IOP), visual acuity, visual field, endothelial cell loss, and notation of complications. RESULTS: There was a statistically significant postoperative improvement in visual acuity (P < .001). Mean preoperative best corrected visual acuity (BCVA) was 20/200 (range 20/30 to hand movements). Mean 1 year postoperative BCVA was 20/30 (range 20/20 to 20/60). The preoperative mean IOP of 24.06 mm Hg decreased to 15.36 mm Hg at 1 year (P < .001). All 42 eyes had a postoperative IOP of less than 21.00 mm Hg. Mean central cornea endothelial cell density preoperatively was 2238 +/- 396 cells/mm2 (range 1697 to 2906 cells/mm2) and postoperatively, 2005 +/- 397 cells/mm2 (range 1302 to 2801 cells/mm2). Early postoperative complications consisted of a choroidal detachment in 2 patients (4.76%). Three and 4 days after surgery, respectively, 2 patients (4.76%) had surgery to remove viscoelastic substance under the IOL. Late complications included posterior synechias in 3 eyes (7.14%). One year after surgery, because of a significant decrease in vision, a neodymium:YAG laser posterior capsulotomy was necessary in 2 eyes, 1 with an acrylic IOL (3.70%) and 1 with a silicone lens (9.09%). CONCLUSION: Combined phacoemulsification, posterior chamber IOL implantation, and trabeculectomy was safe and effective in patients with coexisting glaucoma and cataract.  相似文献
