难治性抑郁症患者海马代谢的磁共振质子波谱研究   总被引:2,自引:0,他引:2  
目的 探讨难治性抑郁症患者双侧海马磁共振质子波谱(1H-MRS)的代谢特点.方法 运用1H-MRS成像系统检测16例难治性抑郁症患者(患者组)和16名健康对照者(对照组)双侧海马的N-乙酰天门冬氨酸(NAA)、胆碱复合物(Cho)、肌醇(mI)及肌酸(Cr)4种代谢产物,分别计算双侧NAA/Cr、Cho/Cr及mI/Cr.采用配对t检验、独立样本t检验及偏相关分析进行统计处理.结果 对照组左侧海马NAA/Cr(1.43±0.19),明显高于右侧(1.21±0.10),P<0.01.患者组双侧海马NAA/Cr的差异无统计学意义(P>0.05),右侧海马NAA/Cr(1.44±0.31),明显高于对照组(1.21±0.10),P<0.01.未发现患者组海马的任何代谢指标与病程及汉密尔顿抑郁量表(17项)评分的相关性(P>0.05).结论 难治性抑郁症患者右侧海马代谢增强,双侧海马NAA/Cr不对称性消失.  相似文献

目的 探讨重性抑郁症首次发病(以下简称首发)患者生化物质改变与认知功能变化之间的关系.方法 利用质子磁共振波谱分析(1H-MRS)测量21例首发重性抑郁症患者(患者组)双侧额叶和海马N-乙酰天门冬氨酸(NAA)、谷氨酸复合物(Glx)、胆碱复合物(Cho)、肌酸(Cr)及肌醇(ml)水平,计算NAA/Cr、Glx/Cr、Cho/Cr、ml/Cr,并与21名正常对照者(对照组)作比较.此外,分别对两组进行神经心理学测试及比较.将1H-MRS所测异常生化物质与异常的认知功能测验项目进行相关性分析.结果 患者组左额叶NAA/Cr(1.8±0.6)低于对照组(2.6±1.0),且与修订韦氏成人智力量表手册中的相似分量表得分及威斯康星卡片分类测验分类数呈正相关(r=0.60,r=0.53);右侧额叶NAA/Cr(2.0±0.6)低于对照组(2.9±1.8),且与连线测验时间呈负相关(r=-0.52).上述差异均有统计学意义(P<0.05).结论 首发重性抑郁症患者抽象思维能力下降、思维灵活度下降及执行功能障碍与左额叶NAA/Cr降低相关,注意力障碍与右额叶NAA/Cr降低相关.  相似文献

目的:观察广泛性焦虑症(GAD)首次发病患者坦度螺酮治疗前后双侧前额叶皮质及海马神经生化物质的变化。方法:对39例首发的GAD患者(GAD组)给予坦度螺酮治疗8周;分别于治疗前后进行汉密尔顿焦虑量表(HAMA)评分及磁共振质子波谱分析(1H-MRS)检测双侧前额叶及海马N-乙酰天门冬氨酸(NAA)、谷氨酸复合物(Glx)、胆碱复合物(Cho)、肌酸(Cr)水平;并与30名正常对照者(正常对照组)比较。结果:治疗前GAD组右侧额叶NAA/Cr、Cho/Cr值显著低于正常对照组(P0.05或P0.01),Glx/Cr值明显高于正常对照组(P0.01);治疗后右侧额叶NAA/Cr、Cho/Cr值显著高于正常对照组(P0.05或P0.01);治疗前后左侧额叶及双侧海马各代谢指标相对浓度与正常对照组差异无统计学意义。相关分析显示,GAD组HAMA总分与右侧额叶NAA/Cr呈负相关(r=-0.732),与Glx/Cr呈正相关(r=0.569)(P均0.01)。结论:GAD患者发病初期即存在右侧前额叶神经元代谢异常;坦度螺酮抗焦虑疗效可能与其调整脑组织代谢有关。  相似文献

抑郁症患者前额叶、海马磁共振质子波谱成像的研究   总被引:9,自引:2,他引:9  
目的探讨抑郁症患者前额叶、海马的磁共振质子波谱(1H-MRS)变化特点。方法应用1H-MRS成像技术检测26例抑郁症患者(抑郁症组)和20名健康对照者(对照组)的前额叶、海马N-乙酰天门冬氨酸(NAA)、胆碱(Cho)、肌酸(Cr)3种代谢物,计算NAA/Cr和Cho/Cr比值。以抑郁症组年龄的中位数(38.5岁)为界将抑郁症组和对照组分为低年龄组[(28±7)岁,14例;(22±6)岁,9名]与高年龄组[(54±9)岁,12例;(53±7)岁,11名]。按病程分为首次发病组(19例,以下简称首发组)和复发组(7例)。采用方差分析进行统计比较。结果高年龄组患者左侧前额叶Cho/Cr(1.60±0.45)×10-6高于对照组(1.28±0.20)×10-6,差异有统计学意义(P<0.05);低年龄组患者右侧前额叶Cho/Cr(1.74±0.51)×10-6高于对照组(1.22±0.40)×10-6,差异有统计学意义(P<0.01)。高年龄组患者左侧海马NAA/Cr(1.16±0.31)×10-6低于对照组(2.21±0.83)×10-6,差异有统计学意义(P<0.01);低年龄组患者左侧海马Cho/Cr(1.54±0.44)×10-6高于对照组(1.08±0.35)×10-6,差异有统计学意义(P<0.05)。首发组患者左侧前额叶Cho/Cr(1.57±0.42)×10-6高于对照组(1.26±0.21)×10-6,复发组患者右侧前额叶Cho/Cr(1.77±0.50)×10-6高于对照组(1.29±0.32)×10-6,差异均有统计学意义(P均<0.01)。首发组患者左侧海马NAA/Cr(1.19±0.26)×10-6及复发组患者左侧海马NAA/Cr(1.10±0.29)×10-6均低于对照组(1.87±0.81)×10-6,复发组患者右侧海马Cho/Cr(2.13±0.51)×10-6高于对照组(1.51±0.52)×10-6,差异均有统计学意义(P均<0.01)。结论抑郁症患者可能存在前额叶、左侧海马细胞膜代谢异常,高年龄抑郁症患者的左侧海马神经元活力下降。  相似文献

目的 探讨抑郁症首次发病患者海马的磁共振质子波谱(1H-MRS)代谢物质的变化.方法 对99例首次发病的抑郁症患者和26例健康对照组行磁共振常规扫描及1H-MRS检查,测量双侧海马N-乙酰天门冬氨酸(NAA)、胆碱复合物(Cho)和肌酸(Cr)三种代谢物质,计算NAA/Cr和Cho/Cr比值.结果 抑郁症患者海马NAA/Cr左右侧比值(1.23±0.16;1.16±0.16)低于对照组NAA/Cr左右侧比值(1.38±0.23;1.31±0.26),差异有显著性(P<0.05);抑郁症患者海马体部Cho/Cr左右侧比值(1.19±0.14;1.18±0.12)高于对照组Cho/Cr左右侧比值(1.14±0.12;1.11±0.14),差异有显著性(P<0.05).对照组左右侧NAA/Cr和Cho/Cr比较差异无显著性(P>0.05).抑郁症组右侧NAA/Cr低于左侧,差异显著(P<0.05);左侧Cho/Cr高于右侧,差异不明显(P>0.05).结论 抑郁症患者可能存在双侧海马神经细胞代谢功能障碍,右侧神经细胞功能障碍较左侧明显.  相似文献

目的 :探讨慢性前列腺炎 (CP)患者伴有焦虑抑郁症的解决方法。方法 :将 98例CP伴有焦虑抑郁症的患者随机分为A、B两组 ,其中A组 5 3例采用可乐必妥 +哈乐 +百优解 ;B组 45例采用可乐必妥 +哈乐 ,连续门诊治疗 6周。结果 :A、B两组CP总有效率分别为 87%和 5 6% ,焦虑抑郁症的缓解率分别为 65 %和 2 1%。结论 :对伴有焦虑抑郁症的CP患者在对前列腺炎本身治疗的同时也应针对焦虑抑郁症予以足够的重视和治疗 ,才能达到理想效果  相似文献

目的 探讨脑小血管病(Cerebral small vessel disease,CSVD)相关认知功能障碍碍(Mild cognitive impairment,MCI)患者的海马代谢水平与临床危险因素。方法 连续入组的CSVD患者按照蒙特利尔认知评估表(Montreal cognitive assessment,MoCA)评分分为CSVD-MCI组和CSVD组,对所有患者海马进行氢质子磁共振波谱成像(Hydrogen proton Magnetic resonance spectroscopy,1H-MRS)扫描,收集相关临床资料和生化指标水平,采用Logistic回归分析CSVD-MCI的危险因素。结果 共入组97例患者,CSVD-MCI组42例,CSVD组55例; 2组患者在命名、语言、抽象思维、延迟记忆等方面有显著性差异(P<0.01),CSVD-MCI组两侧海马的代谢产物[肌醇(Myo-Inositol,MI)/肌酸(Creatine,Cr)、MI/N-乙酰天门冬氨酸(N-acetyl aspartate acid,NAA)]高于CSVD组(P<0.01),右侧海马NAA/Cr较CSVD组偏低(P<0.01)。Logistic回归分析显示年龄≥65岁、合并冠心病史、高血压病史、左侧大脑半球病变和超敏C反应蛋白(Hypersensitive-c-reactive-protein,hs-CRP)水平是脑小血管病相关认知功能障碍的危险因素(P<0.01)。结论 脑小血管相关认知功能障碍在命名、语言、抽象思维和延迟记忆方面均受到影响,可能与海马的代谢产物分泌不均相关; 其高龄、冠心病史、高血压病史、左侧大脑病变是其危险因素,超敏C反应蛋白的水平升高也是导致脑小血管相关认知功能障碍的一项危险因素。  相似文献

目的 探讨双相障碍Ⅱ型患者前额叶白质、豆状核的氢质子磁共振波谱(1H-MRS)特征. 方法 以自2012年9月至2013年4月在中山大学附属第三医院住院的双相障碍Ⅱ型患者30例为患者组,同期20例健康志愿者为对照组,采用多体素磁共振波谱技术检测2组研究对象前额叶白质、豆状核的代谢物质含量,包括N-乙酰天门冬氨酸(NAA)、胆碱化合物(Cho)、肌酸(Cr)、肌醇(mI),并计算NAA/Cr、Cho/Cr、mI/Cr、NAA/Cho、NAA/(Cho+Cr)的比值. 结果 患者组右侧前额叶白质NAA、Cho、mI绝对含量及NAA/Cr比值和对照组相比明显下降,差异均有统计学意义(P<0.05);左侧前额叶白质NAA绝对含量及NAA/Cr、NAA/Cho、NAA/(Cho+Cr)比值与对照组相比明显下降,差异均有统计学意义(P<0.05);右侧豆状核NAA、Cho绝对含量和对照组相比明显下降,差异有统计学意义(P<0.05). 结论 双相障碍Ⅱ型患者存在双侧前额叶白质纤维受损和神经胶质细胞功能异常,以及右侧豆状核神经元缺失或功能异常.  相似文献

目的 探讨重复经颅磁刺激(rTMS)对酒依赖患者戒断期记忆功能的影响及与海马代谢物水平变化的关系.方法 将38例男性戒断期酒依赖患者按随机数字表分为rTMS组和伪rTMS组,rTMS组给予1 Hz刺激4周,刺激部位为右侧背外侧前额叶皮质(DLPFC);伪rTMS组给予无效刺激.治疗前后分别评估言语记忆、视觉记忆,并采用氢质子磁共振波谱(1 H-MRS)检测双侧海马部位代谢物N-乙酰基天门冬氨酸(NAA)、胆碱化合物(Cho)和肌酸复合物(Cr).结果 (1)rTMS组治疗后言语记忆和视觉记忆评分较治疗前提高(P<0.05),而伪rTMS组治疗后记忆评分较治疗前差异无统计学意义(P>0.05).治疗后,rTMS组仅视觉记忆评分优于伪rTMS组,差异有统计学意义(P<0.01),而言语记忆未见明显改善(P>0.05).(2)rTMS组治疗后双侧海马NAA/Cr、Cho/Cr较治疗前升高(P<0.01),而伪rTMS组治疗前后双侧海马NAA/Cr、Cho/Cr的差异无统计学意义(P>0.05).治疗后,rTMS组双侧海马NAA/Cr、Cho/Cr高于伪rTMS组,差异有统计学意义(P<0.01).(3)左侧海马NAA/Cr、右侧海马Cho/Cr治疗后与治疗前的差值分别与言语记忆、视觉记忆评分呈正相关(P<0.01).结论 rTMS可以改善戒断期酒依赖患者的记忆功能,其可能机制与升高海马NAA/Cr和Cho/Cr有关.  相似文献

Electroacupuncture was performed at the Wangu (GB 12) acupoint, whose position is similar to the cerebellar fastigial nucleus in rats with post-stroke depression. Results showed that the expression of nuclear factor-κB and the levels of tumor necrosis factor-α and interleukin-1β decreased. 目的研究帕金森病(PD)患者额叶磁共振波谱(1H-MRS)的改变及其临床意义。方法对36例PD患者(Hoehn-YahrⅠ级14例、Ⅱ级8例、Ⅲ级8例、Ⅳ级6例)和20名年龄匹配的正常对照者的双侧额叶进行1H-MRS检测,对比分析不同Hoehn-Yahr级别PD患者与正常对照组的额叶N-乙酰基天门冬氨酸(NAA)/肌酸复合物(Cr)和含胆碱化合物(Cho)/Cr值的变化。结果Hoehn-YahrⅢ级和Ⅳ级PD患者双侧额叶的NAA/Cr值显著低于正常对照组(P<0.005,P<0.01),Cho/Cr值显著高于正常对照者(均P<0.005);Hoehn-YahrⅠ级和Ⅱ级PD患者双侧额叶的NAA/Cr和Cho/Cr值与正常对照组差异无统计学意义。结论Hoehn-YahrⅢ级和Ⅳ级PD患者额叶1H-MRS有明显异常。提示1H-MRS有助于PD病情的评估。  相似文献

The present study established a rat model of post-stroke depression using incomplete ischemia induced by unilateral carotid artery ligation in combination with solitary raising and subcutaneous injection of a small dose of reserpine. After intragastric perfusion with 45 mg/100 g, 15 mg/100 g, and 7.5 mg/100 g of Xingnao Jieyu for 7, 14 and 21 days, neuronal morphology in the frontal lobe and hippocampus was improved, depression state and voluntary behaviors were also effectively improved in rats with post-stroke depression. Moreover, the effects of Xingnao Jieyu at a dose of 45 and 15 mg/100 g were similar to the traditional antidepressant Prozac.  相似文献

Several studies of temporal lobe epilepsy (TLE) patients have investigated the relationship between the seizure focus and 1H magnetic resonance spectroscopy (1H-MRS). There have also been a few reports in other types of partial epilepsy. We examined the relationship between the seizure focus and the reduction in N-acetylaspartate: creatine (NAA : Cr) ratio using 1H-MRS in both TLE and frontal lobe epilepsy (FLE) patients. We studied 21 patients with unilateral TLE and seven patients with unilateral FLE. We used a 1.5 Tesla magnetic resonance unit (Signa Horizon; General Electric). Approximately 15 x 15 x 20 mm3 voxel of interest (VOI) was placed over the anterior portion of the bilateral hippocampus in the TLE patients, and the anterodorsal position of bilateral frontal lobe in the FLE patients. The seizure focus was identified by interictal scalp electro-encephalogram (EEG). In the TLE patients the NAA : Cr ratios were reduced in the seizure focus, while in the FLE patients they were not always reduced in the seizure focus. In the TLE patients the coincidence rate between the seizure focus and the reduction in the NAA:Cr ratio was 90% (19 of 21 patients), while in the FLE patients the coincidence rate was only 57% (four of seven patients).  相似文献

Summary We studied the usefulness of measuring volumes of the hippocampus, amygdala and frontal lobes with coronal magnetic resonance imaging (MRI) scans in the diagnosis of early Alzheimer's disease (AD). We examined 32 patients diagnosed according to the NINCDS-ADRDA criteria of probable AD and 16 age-matched healthy cognitively normal controls. The AD patients had mild dementia with a mean score of 22.8 in the Mini-Mental Status Examination (MMSE). We used a 1.5T magnetic resonance imager and normalized the volumes for brain area. The AD patients had significantly smaller volumes of the right and the left hippocampus (–38%) (ANOVA, p<0.001) and the left frontal lobe (–16%, p<0.05) compared to controls. The reductions in volumes of the right frontal lobe (–13%), the right amygdala (–14%) or the left amygdala (–18%) were not statistically significant. In the discriminant function analysis which included the volumes of the hippocampus, amygdala, and the frontal lobes and age, the volumes of the left and right hippocampus, the left and right frontal lobe, and the right amygdala entered the model and we could correctly classify 92% of the subjects into AD and control groups (Chi-square 42.6, df 5, p<0.0001). By using the volumes of the hippocampus, the frontal lobes or the amygdala on their alone, the correct classification was achieved in 88%, 65% and 58% of the subjects, respectively. In addition, in AD patients the volumes of the left hippocampus correlated significantly with the MMSE score and with immediate and delayed verbal memory; the smaller the volume the more impaired was their performance. Our data indicate that measurements of volumes of the hippocampus might be useful in diagnosis of early AD.  相似文献

Most research with 31P-magnetic resonance spectroscopy (31P-MRS) in affective disorders has been done in the field of bipolar disturbances. Reduced frontal and temporal lobe phosphomonoester (PME) concentrations were measured in the euthymic state, whereas increased values were found in the depressed state. In bipolar-II patients reduced phosphocreatine (PCr) concentrations were reported in the euthymic, depressed, and manic state. The aim of the present study was to explore whether PME and PCr were also altered in the frontal lobe of major depressed, unipolar patients. Therefore, we used 31P-MRS to investigate the relative phospholipid and high-energy phosphate concentrations in the frontal lobe of 14 unipolar patients, mostly medicated, and 8 age-matched controls. We found increased PME and decreased ATP values. Other 31P-MRS parameters were not different in both groups. Phosphomonoester percentages correlated negatively with the degree of depression. Thus, the main alterations found in bipolar depressed patients could also be demonstrated in unipolar depressed patients. The results are discussed with regard to disturbed phospholipid and intracellular high-energy phosphate metabolism in depressed patients. Received: 2 December 1997 / Accepted: 14 July 1998  相似文献

Myo-inositol (mI) as a precursor in the phosphatidylinositol second messenger system has been reported to be reduced in depression. By means of proton-magnetic resonance spectroscopy (1H-MRS) the mI levels in the frontal brain were investigated in vivo in the present study. Twenty-two patients (mean age: 42.8±10.7 years) with depressive episodes according to ICD 10 (HAMD score> 17) were compared to 22 healthy subjects (28.0±5.3 years). Two voxels (30×20×20 mm3) in the frontal lobes were examined in a Siemens Magnetom SP 4000 at 1.5 T (STEAM sequence: TR = 3500 ms, TE = 55 ms). With the total creatine (Cr) as an internal standard, mI\Cr ratios were calculated to follow the mI levels. In the left frontal lobe, mI\Cr was 0.43±0.06 in depressive patients and 0.46±0.07 in healthy subjects; concerning the right frontal lobe, mI\Cr was 0.46±0.08 and 0.48±0.06, respectively. There were neither significant differences between the two groups nor between the hemispheres. Since there was a significant positive correlation (R = 0.6) between the age and the mI\Cr in the right frontal lobe of depressed patients, age matched pairs analysis was performed (n = 2×10, in each group: nine females, one male, < 40 years). In the right frontal lobe, the patients mI\Cr of 0.40±0.05 was now significantly lower than the controls mI\Cr of 0.45±0.06. However, most of the patients were on antidepressive medication. Interestingly, it was exactly this group of patients which showed significantly lower mI levels.

We regard our investigation as a pilot study which suggests an influence of age and antidepressants on mI levels and should be taken into consideration in further investigations in depressive patients.  相似文献

实验建立正常对照组、假手术组、高低频电针组和中药薏苡仁干预组,横断T10左半侧的脊髓损伤模型大鼠,4h后使用5,100Hz电针刺激环跳(GB30)、足三里(ST 36)、至阳(DU9)及悬枢(DU5)或腹腔注射0.4μL中药薏苡仁粗提液(与生药比例为1:1),连续8周,观察发现低高频电针刺激及薏苡仁治疗能改善脊髓组织损伤区域形态,促进运动诱发电位的恢复,抑制损伤区胶质纤维酸性蛋白表达,改善半横断性脊髓损伤大鼠运动功能,以100Hz电针刺激和薏苡仁干预效果明显。  相似文献

Numerous studies using proton magnetic resonance spectroscopy (1H-MRS) have detected signal changes in schizophrenia. However, most studies investigated relative concentrations such as N-acetylaspartate/creatine plus phosphocreatine (NAA/Cre) and choline-containing compounds/creatine plus phosphocreatine (Cho/Cre), and individual metabolite concentrations have not been clarified. Using absolute quantification measurement of 1H-MRS, the aim of the present paper was to demonstrate the changes in metabolite concentrations in the frontal lobe of patients with chronic schizophrenia. The 1H-MRS was performed in the left frontal lobe in 14 patients with schizophrenia and in 13 healthy comparison subjects. Individual MRS peak concentration was quantified based on a frequency-domain fitting program: LCModel. The scores on the Positive and Negative Symptoms Scale and Wisconsin Card Sorting Test were used for clinical assessment. The NAA concentration was reduced in schizophrenic patients (average, 7.94 mmol/L, t=2.28, P<0.05) compared with healthy subjects (average=8.45 mmol/L) while choline, creatine or NAA/Cre ratio did not show any differences. The reduction in NAA concentration had a significant correlation with the severity of negative symptoms (r=-0.536, P<0.05) and poor performance in Wisconsin Card Sorting Test (r=-0.544, P<0.05). Using quantitative MRS, decreased NAA concentration was confirmed in the left frontal lobe of schizophrenic patients and was demonstrated to be correlated with negative symptoms and cognitive dysfunction in schizophrenia.  相似文献

目的:探讨老年期抑郁症患者海马的磁共振质子波谱(1H-MRS)特点。方法:应用1H-MRS成像技术检测11例晚发性抑郁症患者(患者组)予文拉法辛缓释剂治疗6周前后两侧海马头、体、尾部N-乙酰天门冬氨酸(NAA)、胆碱(Cho)和肌酸(Cr)3种代谢物,计算NAA/Cr和Cho/Cr比值。并且与15名健康对照者(对照组)的检测结果进行比较。结果:治疗前后患者组右侧海马头部NAA和Cho含量及左侧海马头部Cho含量均显著低于对照组(P<0.05或P<0.01)。患者组右侧海马头部Cho/Cr治疗前显著低于对照组(P<0.05),治疗后较治疗前显著升高(P<0.05),并且与对照组的差异无显著统计学意义(P>0.05)。患者组右侧海马体部Cho含量治疗前显著低于对照组(P<0.05),治疗后右侧海马体部NAA/Cr和Cho/Cr显著高于治疗前,与对照组的差异无显著统计学意义(P>0.05)。结论:老年期抑郁症患者可能存在左右两侧海马头部细胞膜代谢异常,右侧海马头部神经元活力下降。  相似文献
