A case of endometrioid carcinoma arising from endometriosis of the rectum]

A very rare case with rectal endometriosis heterotopic transformed into carcinoma is reported. A 44-year-old female patient underwent colostomy due to ileus. Postoperative examinations revealed submucosal tumors all around the rectum. Low anterior resection and the closure of the colostomy were performed. Thickening in the muscular layer and serosa were observed in the resected specimen, but the mucosal surface was maintained. Histopathological observation revealed glands of the endometrium and connective fibers with glandular adenocarcinoma in the periphery. Transformation of the benign endometrial tissues into carcinoma and squamous metaplasia were observed. A diagnosis of endometrial carcinoma in the rectum was made.     

Bilateral endometriosis of the ureter

A case of bilateral endometriosis of the ureter with bilateral ureteral obstruction is reported. Ureteroscopy failed to detect endometriosis, and only bilateral partial ureterecomy with end-to-end anastomosis revealed the diagnosis. Bilateral endometriosis is very rare. Either extrincic periureteral compression or intrinsic involvement of the ureteral wall causes ureteral obstruction, and may destroy the kidneys with few or no gynecological or urological symptoms. To facilitate early diagnosis and the initiation of appropriate therapy, the gynecologist should regularly search for clinical signs and symptoms of upper urinary tract obstruction in all women with genital endometriosis or a history of previous surgery for genital endometriosis. When renal ultrasound shows upper urinary tract dilatation, ureteral endometriosis should be suspected. The urologist should think of ureteral endometriosis in a premenopausal woman with unilateral or bilateral distal ureteral obstruction of unknown cause, especially when she suffers from genital endometriosis or has a history of this disease.     

Intrinsic endometriosis of the ureter

Ureteral involvement by endometriosis especially of the intrinsic type is a rare condition. Most of such cases are accompanied by severe renal impairment requiring radical excision. A similar case is described in conjunction with nephroureterectomy.     

Endometrioid adenosarcoma of the bladder arising from endometriosis

Vesical endometriosis is an uncommon entity characterized by the deposition of benign, hyperplastic endometrial tissue in the bladder. To date, only about 160 cases have been reported in the world literature. More uncommon, however, are case reports of extrauterine malignancies arising from pre-existing endometriosis. We report a case of a large endometrial adenosarcoma in a patient who had undergone total abdominal hysterectomy and bilateral salpingo-oophorectomy 10 years previously for endometriosis. To our knowledge this malignancy has never been reported previously to involve the bladder. We review the pathological findings, possible mode of entry into the bladder and current concepts in its treatment.

We report an unusual case of ureteral obstruction from a toothpick that migrated from the cecum to the right ureter.     

Case report: Sarcomatoid carcinoma arising from the ureter: A rare case and a treatment dilemma

We report an interesting case of a patient with sarcomatoid carcinoma arising from the left ureter who presented with left iliac fossa pain. The diagnosis was based on CT and histological findings. To our knowledge, this is the first case of sarcomatoid carcinoma arising from the ureter reported. The literature review also highlights the rarity and the poor prognosis of the disease despite aggressive interventions.     

Papillary clear cell adenocarcinoma of the groin arising from endometriosis

A 67-year-old woman presented with rapidly enlarging right anterior thigh mass. Clinical impression was of a primary sarcoma, but the histologic analysis of an incisional biopsy specimen showed adenocarcinoma with clear cell differentiation. Initially thought to be a metastasis, the malignant tumor appears to have arisen from extraabdominal endometriosis of the right groin. This case shows clinical, radiographic, and histologic findings that may help the orthopaedic surgeon recognize an unusual complication of extrapelvic endometriosis.     

Small-bowel obstruction caused by intraluminal migration of prosthetic grafts

Two patients who had undergone prosthetic grafting in the venous system presented with small-bowel obstruction. The first had undergone mesocaval shunting with a Dacron graft and the second, replacement of the inferior vena cava with a polytetrafluoroethylene graft after an extensive resection of retroperitoneal tumor. In both instances the grafts were within the lumen of the small bowel, causing a mechanical obstruction. No evidence of retroperitoneal bleeding subsequent to detachment of the grafts was noted.
