Experimentelle Untersuchungen zum Einfluss des Prostazyklin–Analogons Iloprost auf die Kontraktilität humaner Vorhofmuskulatur

ZusammenfassungFragestellung Das stabile Prostazyklin–Analogon Iloprost hat während der Rechtsherzkatheteruntersuchung von Patienten, die für eine Transplantation evaluiert wurden, eine klinisch nachweisbare Zunahme des linksventrikulären Herzindex gezeigt. Ausgehend von diesen Ergebnissen formulierten wir die Arbeitshypothese, dass Iloprost einen direkt positiv inotropen Effekt ausübt.Methodik Stellvertretend für linksventrikuläres Myokard wurde von 22 Patienten rechtsatriales Myokard des Vorhofohres im Rahmen elektiv durchgeführter kardiochirurgischer Operationen (Bypass, Aortenklappenersatz, Kombination) entnommen. Aus den gewonnenen Präparaten wurden vier Trabekel präpariert und je zwei Trabekel der Kontroll– und Verumgruppe zugeordnet. Als Kontrollgruppe wurde diejenige Trabekelpopulation bezeichnet, die nur mit Iloprost–Trägerlösung (ohne Wirkstoff) stimuliert wurde. Die Verumgruppe enthielt alle Trabekel, die mit Iloprost in einer Konzentrationsreihe von 1 pg/ml bis 100 ng/ml stimuliert wurden. Die Versuche wurden im Organbad durchgeführt, so dass alle Präparate mit gepufferter Tyrodelösung gespült und elektrisch mit 1 Hertz (Hz) stimuliert worden sind. Die Ermittlung der Kontraktionskräfte erfolgte mit einem Kraftaufnehmer. Die maximal mögliche Kontraktionskraft wurde durch Stimulation mit 10^–7 M Isoprenalin ermittelt.Ergebnisse Iloprost in den getesteten Konzentrationen übt weder einen positiv noch negativ inotropen Effekt auf rechtsatriales Myokardgewebe aus. Die ermittelten Kontraktionskräfte waren weder abhängig von der Masse der präparierten Trabekel, noch von einer medikamentösen Vorbehandlung mit Betablockern.Schlussfolgerung Die Arbeitshypothese konnte durch den gewählten Versuchsaufbau nicht bestätigt werden. Ob Iloprost direkt positiv inotrop auf linksventrikuläres Myokard wirkt, muss in einem anderen experimentellen Ansatz, z.B. in einem Tiermodell, überprüft werden.

The risk for developing postoperative complications increases with the degree of surgical trauma, an altered wound healing capability of the patient due to comorbidities and environmental conditions and the selection of an inadequate implant material, the latter offering options for improvement at least in some patients. In general a mesh with large pores made of monofilaments provides a reduced surface area and causes less scarring and inflammation than those with small pores and thereby reduces the rate of scar contraction, pain and the challenge to explant a mesh from a scar bed. When placing the mesh in the abdominal cavity an additional surface coating of polypropylene should prevent the formation of a fistula between mesh and bowel. The risk of recurrence mainly depends on the extent of overlap; however, the flexibility of some meshes may increase the technical difficulties of some implants. In cases of bacterial contamination of the wound there is an increased risk for late onset mesh infection and monofilament meshes offer the best option for complete healing by conservative means. An impaired function of the spermatic cord because of the material, apart from the consequences of the surgical trauma, has not been confirmed in experimental and clinical studies. Revision of mesh sites always is a surgical challenge but could be much easier with implants which are visible in computed tomography (CT) or magnetic resonance imaging (MRI) scans.

Kryokonservierung humaner Spermatozoen — Auswertung von 93 Samenlangzeitdepots: Long-term semen storage — Evaluation of 93 semen depots

Cryopreserved semen depots from 93 patients who had terminated contract with the CRYO-BANK-KREFELD were evaluated for prefreeze quality and post-thaw motility after a few days and after long-term storage (mean = 44 month). 2/3 of these patients had onkological disease. Only 20% of the ejaculates were normozoospermic and showed motility after thawing of more than 20%. The average recovery rate after long term storage was only 28% and revealed a significant decrease with time of about 20%. The concentration of motile spermatozoa, especially in patients with testicular cancer (x = 2.1 mill/ml), was so much reduced one must assume that in the most cases of semen banking there was no chance to achieve pregnancy.

Cultivation and transplantation of autologous keratinocytes has been used in the last 15 years to treat complicated wounds of different origin. In spite of excellent technical advancements and clinical experiences cultured keratinocyte grafting still is associated with practical limitations. Application of hyaluronic acid ester membranes as carrier substrate for the transfer of keratinocytes allows improved graft handling, reduces total time required for tissue cultivation and furthermore enhances vitality of the keratinocytes because of possible grafting at semiconfluence.

Der Einfluss des T-Stadiums auf das Metastasierungsrisiko des Peniskarzinoms: T1 vs. T2

Background

Controversies persist over the therapeutic approach to T1 penile carcinoma, particularly in patients with negative inguinal lymph nodes. Available data on lymph nodes metastases (LNM) in T1 carcinoma are contradictory. The aim of this study was to evaluate the metastatic risk of T1 carcinoma and to compare it with that of T2 carcinoma.

Material and methods

A total of 37 patients (pts) with T1 or T2 tumors were reviewed. Assessment of the inguinal lymph node condition was based on node dissection in 29 pts and surveillance in eight pts (mean 62 months, range 22–162).

Results

Grading was classified as good (G1), moderate (G2) and poor (G3) in seven, 26 and four pts, respectively. Tumor stage was T1 in 21 and T2 in 16 pts. LNM were observed in eight of 21 T1 (38%) and six of 16 T2 tumors (38%). No G1 and all G3 tumors developed LNM independently of tumor stage. Ten of the 26 G2 carcinomas (38%) harboured LNM and seven of these pts (70%) had a T1 tumor.

Conclusions

According to our data, the metastatic potential of T1 penile carcinoma has been underestimated in the recent literature. Tumor grading has a substantially stronger impact on the metastatic risk in T1 and T2 penile carcinoma than tumor stage, indicating a surgical lymph node staging starting at the pT1G2 stage.
