Ultrasound-guided percutaneous thrombin injection has recently been described as a treatment for postcatheterization femoral pseudoaneurysms. Although ultrasound guided compression offers another nonoperative treatment option, thrombin injection has shown superior initial success rates. Reports of follow-up for thrombin injection longer than 30 days are currently lacking. The authors reviewed their initial experience with thrombin injection and prospectively evaluated patients for occult late recurrences of pseudoaneurysm and for distal circulatory complications. Records and vascular laboratory data for all patients treated with ultrasound-guided thrombin injection were reviewed for an 18-month period. Tibial vessel Doppler waveforms and ankle/brachial indices were routinely obtained before and after thrombin injection. Follow-up duplex examinations were performed within 24 hours of initial treatment. In the prospective portion of the study, successfully treated patients underwent a repeat femoral duplex scan and lower extremity arterial examination for comparison with the pretreatment studies. Forty-nine of 52 femoral pseudoaneurysms (94%) were successfully treated with ultrasound guided thrombin injection. One immediate failure and 2 early recurrences were treated surgically. There was 1 thrombotic complication of the native circulation identified at the time of injection. Follow-up studies were obtained in 32 of 46 available patients with a mean length of follow-up of 9 months (range 3-17 months). No late recurrences of the pseudoaneurysms or arterial-venous fistulas were observed. No distal circulatory complications were detected by arterial waveform analysis. Three deaths occurred in the interim (cardiac related). Two patients were lost to follow-up. The remaining 12 patients reported no additional limb complications but declined to be restudied. Ultrasound-guided thrombin injection is a safe, effective, and durable treatment for iatrogenic pseudoaneurysms. Thrombin injection should be the therapy of choice for catheter-related femoral false aneurysms.  相似文献

BACKGROUND: Infected femoral artery pseudoaneurysms (IFAPs) secondary to percutaneous arterial access, injection of illegal substances, and from infected synthetic grafts, appear to be increasing in incidence. Ligation of IFAPs without revascularization offers control of infection but may risk limb ischemia. Revascularization with extraanatomic synthetic grafts may risk reinfection and abrupt thrombosis. Excision of IFAPs with revascularization using superficial femoral popliteal vein (SFPV) provides both control of infection and excellent limb perfusion. STUDY DESIGN: A retrospective review was conducted of patients diagnosed with IFAP who underwent resection and revascularization with SFPV at a single medical center. Outcomes measured included reinfection and amputation rate. These were compared with other series using various methods to treat IFAPs. RESULTS: Eleven patients with IFAP were encountered from 1992 to 2004. Mean age was 64 years (+/-10 SD). Five patients developed IFAP secondary to percutaneous arterial access procedures. Four patients developed infected femoral artery pseudoaneurysms secondary to synthetic graft infection. Two patients developed IFAP secondary to injection of illegal substances in the femoral region. All patients had positive wound cultures initially. Staphylococcus was the most common organism found in wound cultures. All patients underwent resection of IFAP with lower extremity revascularization using SFPV. There was no incidence of limb ischemia and no perioperative deaths in this series. CONCLUSIONS: Excision of IFAP with revascularization can be successfully achieved using SFPV. This method may prove to be superior to other methods with apparent higher patency rates and resistance to reinfection.  相似文献

AIM: To report a clinical experience about surgical treatment of iatrogenic peripheral artery pseudoaneurysms (FPA).\METHODS: This is a retrospective review of 90 consecutive patients (46 males, 44 females, mean age 66.2 years, range 33-86) with FPA complicating coronary angiography or angioplasty, observed between October 1990 through June 2006. \RESULTS: A 3 cm pseudoaneurysm or larger was confirmed by duplex ultrasound scanning in 90 out of 21 454 cardiac patients (0.42%), occurring more frequently in interventional (59/3 983) rather than diagnostic (31/17 471) procedures (1.48% vs 0.17%). The surgical treatment consisted in direct closure with polypropilene suture and occasionally, patch angioplasty or bypass. No limb loss occurred. There were 4 wound complications (4.4%), one pulmonary embolism (1.1%), 3 deaths (3.3%).\CONCLUSION: Classical results reported in literature demonstrate that the surgical repair of femoral pseudoaneurysms following cardiac catheterization is safe, effective and durable. In these series, although low major morbidity (1.1%) and no cases of limb loss were reported, the authors observed 3 death (4.4%), resulting from the severity of cardiac disease in 2 cases and from the vascular repair itself in one case (femoral endoarteritis). These results substantiate the common observation that patients who actually require invasive coronary diagnosis and treatment are often affected by advanced cardiovascular disease and suffer the occurrence of complications, having a high risk of death. Therefore, any surgical treatment should be performed with strict adherence to sound vascular surgical principles.  相似文献

OBJECTIVES: to evaluate the feasibility of treatment of post-catheterisation pseudoaneurysms with duplex-guided thrombin injection. TECHNIQUE: the technique of duplex-guided injection of thrombin in post-catheterisation pseudoaneurysms of the femoral artery is described and illustrated. RESULTS: between December 1998 and October 1999, eight post-catheterisation pseudoaneurysms of the femoral artery were successfully thrombosed with thrombin injection. One patient developed a new pseudoaneurysm within 6 hours and this was also successfully treated with thrombin. Follow-up duplex-scanning at 3 months revealed no recurrences. No other adverse events occurred. CONCLUSIONS: this initial experience suggests that duplex-guided thrombin injection in the treatment of post-catheterisation pseudo-aneurysms is feasible and safe.  相似文献

We reviewed 13 cases of ultrasound-guided thrombin injection of femoral pseudoaneurysms. All cases occurred within a 17-month period from January 1998 through May 1999 and were complications of femoral artery puncture. Immediate total thrombosis occurred in nine of 13 patients. Twenty-four-hour follow-up ultrasound in seven patients revealed no recurrence of pseudoaneurysm. Two of 13 patients required operative repair. One pseudoaneurysm thrombosed with 15 minutes of compression after injection and one case required a second injection. No cases of arterial thrombosis were noted. Ultrasound-guided thrombin injection for femoral artery pseudoaneurysm represents a safe and effective alternative to operative repair.  相似文献

目的探讨毒品注射所致的慢性感染性股部动脉假性动脉瘤(CIFA)的合理外科治疗方法。方法对我院血管外科2007年7月至2010年6月收治的44例CIFA患者按住院号单双号分组,单号为第一组(20例)行主干动脉血流重建术,双号为第二组(24例)行主干动脉结扎术。结果第一组14例(70%)术后无任何并发症。6例(30%)术后出现严重感染.取出移植物后行二期血流重建术;第二组20例(83.3%)术后无任何并发症,4例(16.7%)出现间歇性跛行因而行二期血流重建术。所有患者行一期或二期手术后均康复出院,无一例患者行截肢术。两组最终手术成功率差异无统计学意义(P〉0.05),但第二组一期手术成功率显著高于第一组(P〈0.05),第二组出现严重并发症并行二期血流重建的病例显著低于第一组(P〈0.05)。随访5个月至3年,所有患者均未出现间歇性跛行等缺血性症状及患肢缺血性坏死等严重并发症。结论主干动脉结扎术是CIFA的合理手术方式,必要时可行二期血流重建术。  相似文献

Methods 197 patients were diagnosed as iatrogenic femoral artery pseudoaneurysms from Apri 1199 to April 2008.There were 122 male and 75 female, aged 59.7 years (rimed 40- 81 years). One. Hundrel and severty-one stahle cases were managed by ultrasound-guided local compression initially and 26 patients were directly treated with surgical repair because d the rupture of femoral artery pesudoaneurysms or the pseudoaneurysms≥40mm in diameter. Results 171 patients received llocal com- pression therapy, 137 cases were cured directly (the effective rate was 80%), but the last failed 34 cases were required conversion to surgical reparr. The nymber of the surgical repaired patients was 60 (incluing 26 cases with direct operation and 34 cases with required conversion to surgical repair). Forty-seven patients received direct excision of femoral artery pseudoaneurysm, six patients underwent angioplasty with autogenously saphenous vein patch, and seven patients got bypass operation with artificial vascular graft. During the perioperative period, no serious complications including bleeding, neuralgia, and lymphatic fistula even arterio venous fis- tula and so on, no death occurred. All patients were followed up for 1 month to5 years after the procedures, no local FAP recurred, no limb ischemia developed and no deaths occurred. Conclusion Uitrasound-guided compression, surgical repair, and ultrasound- guided percutaneous thrombin injection are the three main modalities of treating iatrogenic FAP, while ultrasound-guided compresson and magical therapy get popularized domestically. Ultrasound-guided comperession seems a safe, inexpensive, and effective method for the managerment of iatrogenic femoral artery pseudoaneurysms. It may be used as a lust-line therapetic modality for mint of the un-complicated patients. 目的 探讨股动脉结扎术治疗吸毒者股动脉假性动脉瘤的疗效.方法 回顾性分析2003年1月至2010年3月采用股动脉结扎术+局部清创术治疗32例吸毒者股动脉假性动脉瘤的临床资料,术后3、6、9、12个月进行随访,采用肢体末梢氧饱和度及其运动功能评价患肢血供.结果 32例患者手术均获得成功.术后患肢末梢氧饱和度较术前无明显改变(95.1%±2.8%比96.9%±1.9%;t =1.26,P=0.25),出院血白细胞计数及中性粒细胞百分比较入院明显下降.随访期为1年,随访率为100%;1例术后6h出现缺血表现而行自体大隐静脉髂股旁路转流术,术后恢复良好;术后3个月内5例轻度间歇性跛行;术后6个月仅2例轻度间歇性跛行;术后9个月所有病例间歇性跛行症状基本消失;所有患者均能维持正常生活. 结论 股动脉结扎术为治疗吸毒者股动脉假性动脉瘤为一种安全、有效和简单的方法.  相似文献

目的总结股动脉结扎治疗注射吸毒所致感染性假性股动脉瘤的体会。方法对45例注射吸毒所致感染性假性股动脉瘤患者资料进行回顾性分析。采用股动脉或髂外动脉结扎+瘤体切除术治疗感染性假性股动脉瘤,术后预防血栓形成及抗感染治疗,并随访患肢功能。结果44例患者保肢成功,近期患肢行走功能良好。1例术后因深静脉血栓形成,出现肢体坏死而截肢。结论34例(75.6%)获3~12个随访,疗效满意。股动脉结扎是治疗感染性假性股动脉瘤的有效方法。  相似文献

目的 评估超声引导下局部压迫和手术治疗医源性股动脉假性动脉瘤的疗效并介绍治疗经验.方法 1995年4月至2008年4月对197例医源性股动脉假性动脉瘤病人进行治疗,其中171例首选超声引导下局部压迫治疗(假性动脉瘤稳定者),26例(假性动脉瘤破裂或瘤腔直径≥40 mm者)直接手术治疗.结果 压迫治疗的171例中137例压迫成功,有效率80%,34例失败者改行手术治疗.直接手术治疗的26例及上述改行手术的34例中,47例行股动脉假性动脉瘤切除、动脉壁破口修补术,6例行自体大隐静脉补片成形术,7例行人工血管转流术.围手术期所有手术病例无出血、神经痛、淋巴瘘、动静脉瘘等严重并发症和死亡.随访1个月至5年,均未见假性动脉瘤复发或肢体缺血症状.随访期间无死亡.结论 局部压迫疗法治疗医源性股动脉假性动脉瘤安全、有效、经济,可作为大部分稳定病人的首选治疗方法.不适合压迫治疗者及压迫治疗失败的病人可手术治疗,疗效确切.  相似文献
