Thirty-two patients with acute, complete tears of the anterior cruciate ligament (ACL) proved at surgery underwent examination with magnetic resonance (MR) imaging. Bone impaction sites were present in the posterolateral tibial plateau in 30 patients (94%) and in the lateral femoral condyle (LFC) in 29 patients (91%). The bone abnormalities had low signal intensity on T1-weighted images and high signal intensity on T2-weighted images when compared with the signal intensity of normal marrow. It is assumed that the bone changes occur during injury when the LFC impacts into the posterior tibia, either during the initial rotary subluxation or as the LFC recoils to return to anatomic alignment. Only one of six partial ACL tears had a bone signal change. In patients with acute knee injury, bone impaction sites in the posterolateral tibia and the LFC suggest that a complete ACL tear is present.  相似文献

MR imaging of the postoperative knee: a pictorial essay.   总被引:5,自引:0,他引:5
Magnetic resonance (MR) imaging of the postoperative knee has become more common because more arthroscopic repair procedures are being performed. The most common procedures include partial meniscectomy and meniscal repair, anterior cruciate ligament (ACL) reconstruction, and cartilage repair procedures. Specific findings of a retorn meniscus following meniscal repair or partial meniscectomy are increased signal intensity extending through the site of repair on T2-weighted images, displaced meniscal fragments, and abnormal signal intensity at a site distant from the repair. Findings of ACL graft disruption on T2-weighted MR images include absence of intact graft fibers and increased signal intensity similar to that of fluid within the expected region of the graft. Partial tears of the graft appear as areas of increased signal intensity affecting a portion of the graft with some intact fibers still present. An impinged ACL graft may appear to be draped over the anterior inferior edge of the intercondylar roof or be posteriorly bowed. Localized anterior arthrofibrosis appears on T1-weighted MR images as a focal nodular lesion of low signal intensity that is anterior to the ACL graft in the intercondylar notch and is indistinguishable from adjacent joint fluid. On T2-weighted images, the nodule is well differentiated from high-signal-intensity joint fluid. Finally, MR imaging has been shown to be accurate in the evaluation of cartilage repair tissue. Knowledge of the normal MR imaging appearance of the knee after the more common repair procedures will allow radiologists to recognize complications associated with such procedures.  相似文献

Transient osteoporosis of the knee   总被引:2,自引:0,他引:2
The MR findings in transient osteoporosis of the knee have been described as showing a diffuse area of decreased signal intensity (relative to normal bone marrow) on T1-weighted images and increased signal intensity on T2-weighted images. We report a case of transient osteoporosis, in which MRI showed a crescentic area of abnormal signal intensity in the posterior portion of the lateral femoral condyle, which was bordered by a rim of low signal intensity, best seen on the T2-weighted images. This abnormality was shown to resolve on follow-up MR scans.  相似文献

The aim of this study was to determine whether solitary pulmonary tuberculoma and malignant tumor can be differentiated on the basis of magnetic resonance (MR) signal intensity. Twenty-eight patients with solitary pulmonary lesions were prospectively studied with MR imaging: T1-weighted, enhanced T1-weighted, proton density-weighted, and T2-weighted spin echo images were obtained. The confirmation methods used were computed tomography (CT)-guided biopsy in seven patients with lung cancer and four patients with tuberculosis; surgery in ten patients with lung cancer and five patients with tuberculosis; and laboratory data in two patients with tuberculosis. Morphologic features and MR signal intensity were examined in detail. As the test for detection of tuberculoma, signal difference on T2-weighted images was carefully analyzed. The signal intensity ratio of the nodule to thoracic muscle signal intensity was measured. The signal intensities obtained from the lung cancers and tuberculomas were variable on pre-and post-enhanced T1-weighted images and proton density-weighted images. Masses were hypointense in 2 of 17 patients with lung cancer and in 9 of 11 patients with tuberculoma on T2-weighted images (sensitivity 82%, specificity 89%, accuracy 87%). The mean signal intensity ratios of the tuberculomas to muscle were significantly lower than those of malignant tumors on T1-weighted, enhanced T1-weighted, proton density-weighted, and T2-weighted images (P < 0.0001). After gadolinium-DTPA enhancement, 2 malignant tumors and 7 tuberculomas showed a marginal rim enhancement pattern, whereas 15 malignant tumors and 2 tuberculomas revealed a diffuse enhancement. The results of MR imaging were consistent with those of CT in 84% of the patients. MR imaging is a helpful adjunctive method in terms of differentiating a tuberculoma from a malignant tumor.  相似文献

Objective. To assess the value of Gd-DTPA magnetic resonance (MR) imaging in the demonstration of marginal destructive discovertebral Romanus lesions in ankylosing spondylitis. Design and patients. A prospective study of Gd-DTPA MR imaging was performed in 39 patients with a clinical diagnosis of ankylosing spondylitis and typical Romanus lesions seen on radiographs of the thoracolumbar spine. MR morphological appearances and signal intensity changes at the discovertebral junctions were analysed and compared with the radiographic findings. Results. Ninety-nine discovertebral junctions with Romanus lesions showed low signal intensity on T1-weighted and high signal on T2-weighted and T1-weighted postcontrast images at the vertebral corners consistent with oedematous hyperaemic inflammatory tissue. There were nine discovertebral junctions with similar MR findings but normal radiographs. Fifty-three discovertebral junctions showed syndesmophyte formation with increased signal intensity on both T1- and T2-weighted images with no contrast enhancement. Sixty-five discovertebral junctions showed a mixture of radiographic features and varied high and low signal changes at the vertebral rim on MR imaging with rims of enhancement in the vertebral body following contrast administration. Conclusion. Gd-DTPA MR imaging demonstrates a variable signal pattern and degree of contrast enhancement which may reflect the evolutionary stages of discovertebral enthesitis in ankylosing spondylitis. MR imaging may identify early erosive changes in radiographically normal vertebra. The role of MR imaging needs further investigation. OBJECTIVE: This study was undertaken to evaluate the diagnostic efficacy of additional oblique coronal magnetic resonance (MR) imaging of the knee for the grading of anterior cruciate ligament (ACL) injury. METHODS: We retrospectively reviewed MR images of the knee in 169 patients. The MR examinations included routine sequences and oblique coronal T2-weighted images, which oriented in parallel to the course of the femoral intercondylar roof. Two independent readers evaluated the status of the ACL by routine knee MR imaging and then by additional oblique coronal imaging. The severity of the ACL injury was graded using a 4-point system from MR images, namely, intact, low-grade partial tear, high-grade partial tear, and complete tear, and results were compared with arthroscopic findings. Weighted kappa statistics were used to analyze the diagnostic accuracy of routine knee MR imaging with and without additional oblique coronal imaging. RESULTS: The weighted kappa scores (kappaws) were 0.752 (reader 1) and 0.784 (reader 2) by routine knee MR imaging only; with additional oblique coronal imaging, the kappaws increased to 0.809 (reader 1) and 0.843 (reader 2). Interobserver agreements for routine knee MR imaging and additional coronal imaging were considered to be "very good" (kappaw = 0.851, 0.868, respectively). CONCLUSION: Additional use of oblique coronal MR imaging of the knee improves diagnostic accuracy in the grading of ACL injury.  相似文献

Conventional MR, diffusion-weighted, and diffusion tensor imaging were performed in an 8-day-old girl with citrullinemia. She had severe hyperammonemia for several days. On conventional T2-weighted MR images, symmetric, confluent high signal intensity was found in the bilateral thalami, basal ganglia, cortex, and subcortical white matter. Diffusion-weighted imaging demonstrated decreased apparent diffusion coefficient in these areas, reflecting cytotoxic edema. Follow-up MR imaging at the age of 4 months revealed subcortical cysts, ulegyric changes, and atrophy, which were most prominent in the occipital lobes. Diffusion tensor imaging revealed decreased anisotropy throughout the brain, consistent with diffuse injury to the oligodendro-axonal unit. Diffusion-weighted and diffusion tensor imaging are valuable techniques for the detection of irreversible brain damage and for the characterization of hyperintense lesions on T2-weighted MR images in patients with the neonatal form of citrullinemia.  相似文献

Twenty patients were enrolled in a prospective study to evaluate MR imaging in the differentiation of epidural scar and herniated disk material. Fourteen patients had surgical verification of imaging findings. In 12 (86%) of these patients, the MR interpretations fully agreed with the observations at surgery. Careful integration of the findings on sagittal and axial T1-weighted images with more T2-weighted axial images was important for analysis. Anterior and lateral recess scars were hypo- or isointense on T1-weighted sequences and hyperintense on more T2-weighted sequences relative to the "parent" anulus intensity. Free fragments demonstrated a slightly hyperintense signal intensity on T1-weighted images relative to epidural fibrosis but had a similar hyperintense signal intensity on T2-weighted sequences. Prolapsed or extruded disk fragments were hypo- or isointense relative to the parent anulus on all sequences. Morphology, epidural location, mass effect, and often signal intensity were the important parameters by which scar and herniated disk could be differentiated with MR.  相似文献

BACKGROUND AND PURPOSE: MR imaging can now be used safely in extremely preterm infants. The aim of this study was to compare the MR imaging appearance of the immature brain with neuropathologic findings at postmortem examination. METHODS: Seven extremely sick preterm infants, born at a median of 24 weeks' gestation, were studied using T1- and T2-weighted MR sequences. Infants died at a median of 3 days after initial MR imaging, and postmortem examinations were carried out. RESULTS: The cortex and germinal matrix were seen as areas of low signal intensity on T2-weighted images, which corresponded to their highly cellular histologic appearance. The periventricular and subcortical layers of white matter had a high signal intensity, corresponding to high fiber and relatively low cellular density; the intermediate layer of low signal intensity corresponded to a dense band of migrating cells. Regions of acute hemorrhage were seen as low signal intensity and regions of infarction as high signal intensity on T2-weighted images. One infant with mild periventricular leukomalacia had some low signal intensity on T1-weighted images, but no focal changes on T2-weighted images. Regions of neuronal mineralization, seen in association with infarction and capillary proliferation, within the basal ganglia and thalami were characterized by very low signal intensity on T2-weighted images and by very high signal intensity on T1-weighted images. There were no imaging abnormalities detected in regions with more subtle histologic abnormalities, such as increased glial or apoptotic cells. CONCLUSION: MR imaging can be used to observe normal developing brain anatomy in extremely premature infants; it can detect areas of hemorrhage and infarction within the developing brain, but conventional MR imaging may not detect more subtle histologic abnormalities.  相似文献

Five patients with a palpable mass at presentation underwent magnetic resonance (MR) imaging. The final diagnosis was myositis ossificans (MO). MR imaging features, particularly after injection of gadopentetate dimeglumine, mimicked those of an inflammatory mass or neoplasm. The lesions were excised in three patients, and the Images were correlated with histologic findings. Three different appearances were noted on MR images, corresponding to the stages of maturation of MO. Two cases Involved early-stage lesions, and Tl-weighted MR images showed a mass with homogeneous intermediate signal intensity. Both lesions showed rim enhancement after contrast agent injection and high signal intensity on T2-weighted images. Pathologic specimens demonstrated stroma with masses of spindle cells in which osteoid production was interspersed. The enhanced rim of the lesion mimicked the expected MR appearance of an abscess or necrotic tumor. Areas of enhancement in adjacent muscle were also seen on postcontrast T1-weighted images. Intermediate-stage MO was present in one case; there was evidence of a thin rim of calcification on plain radiographs and fatty changes in the lesion on T1-weighted Images, corresponding with histologic findings. One case of a mature lesion showed a considerable degree of peripheral calcification both on MR images and at histology. MR imaging is nonspecific in the diagnosis of early-stage MO.  相似文献

BACKGROUND AND PURPOSE: Our purpose was to investigate transient MR signal changes on periictal MR images of patients with generalized tonicoclonic seizure or status epilepticus and to evaluate the clinical significance of these findings for differential diagnosis and understanding of the pathophysiology of seizure-induced brain changes. METHODS: Eight patients with MR images that were obtained within 3 days after the onset of generalized tonicoclonic seizure or status epilepticus and that showed seizure-related MR signal changes had their records retrospectively reviewed. T1- and T2-weighted images were obtained of all eight patients. Additional diffusion-weighted images were obtained of five patients during initial examination. After adequate control of the seizure was achieved, follow-up MR imaging was performed. We evaluated the signal changes, location of the lesions, and degree of contrast enhancement on T1- and T2-weighted images and the signal change and apparent diffusion coefficient (ADC) on diffusion-weighted images. We also compared the signal changes of the initial MR images to those of the follow-up MR images. RESULTS: The initial MR images revealed focally increased T2 signal intensity, swelling, and increased volume of the involved cortical gyrus in all eight patients. The lesions were located in the cortical gray matter or subcortical white matter in seven patients and at the right hippocampus in one. T1-weighted images showed decreased signal intensity at exactly the same location (n = 6) and gyral contrast enhancement (n = 4). Diffusion-weighted images revealed increased signal intensity at the same location and focally reduced ADC. The ADC values were reduced by 6% to 28% compared with either the normal structure opposite the lesion or normal control. Follow-up MR imaging revealed the complete resolution of the abnormal T2 signal change and swelling in five patients, whereas resolution of the swelling with residual increased T2 signal intensity at the ipsilateral hippocampus was observed in the other two patients. For one of the two patients, hippocampal sclerosis was diagnosed. For the remaining one patient, newly developed increased T2 signal intensity was shown. CONCLUSION: The MR signal changes that occur after generalized tonicoclonic seizure or status epilepticus are transient increase of signal intensity and swelling at the cortical gray matter, subcortical white matter, or hippocampus on periictal T2-weighted and diffusion-weighted images. These findings reflect transient cytotoxic and vasogenic edema induced by seizure. The reversibility and typical location of lesions can help exclude the epileptogenic structural lesions.  相似文献

The magnetic resonance (MR) imaging characteristics of normal aortic graft healing were compared with those of perigraft infection in 57 patients after aortic graft implantation. Thirty-three patients without postoperative complications underwent MR imaging in a 0.35-T unit 1 week after graft implantation, and 13 of those patients were reexamined 2-3 months after graft implantation. Twenty-four patients with clinically suspected perigraft infection underwent MR imaging 6 weeks to 18 years after graft implantation. Early normal postoperative changes were characterized by a perigraft collar of low to medium signal intensity on T1-weighted images and of high intensity on T2-weighted images in all 33 cases, consistent with perigraft fluid collection. In 10 of 13 patients reexamined 2-3 months postoperatively, the MR images demonstrated a collar of tissue consistent with perigraft fibrosis. In cases of clinical suspicion of retroperitoneal graft infection, MR imaging showed eccentric fluid collections of low to medium signal intensity on T1-weighted images and high intensity on T2-weighted images at more than 3 months after surgery. The MR findings were diagnostic of retroperitoneal perigraft infection in 17 of 20 patients shown to be infected at surgery. Retroperitoneal infection was correctly excluded on the basis of MR findings in four patients. Thus, MR imaging is an accurate imaging method for the diagnosis of aortic graft infection. In the early postoperative phase, resolving perigraft fluid cannot be differentiated from perigraft infection.  相似文献

PURPOSE: To describe the magnetic resonance (MR) appearance of the posterior cruciate ligament (PCL) graft in the 1st postoperative year and to correlate the MR findings with clinical examination findings. MATERIALS AND METHODS: Nineteen MR examinations were performed in 15 patients (with 15 grafts) 1-33 months after PCL reconstruction. Results of clinical follow-up were available in 14 patients. Graft shape and thickness were recorded, and intrasubstance signal intensity at T2-weighted imaging was graded. The MR findings were correlated with the time between surgery and clinical examination. RESULTS: At MR imaging, 13 grafts appeared intact, one could not be assessed owing to hardware artifact, and one initially appeared disrupted. Graft thickness was 7-19 mm. There was no significant difference between graft thickness versus time since surgery and signal intensity versus time since surgery (P =.14). In two of three patients who underwent sequential MR examinations, graft thickness and intrasubstance signal intensity decreased as the time between reconstruction and MR imaging increased. Two of 14 patients who underwent physical examination had a posterior drawer, and one also had an anterior cruciate ligament graft tear. Both patients with knee instability demonstrated intact PCL graft fibers at MR imaging. There was no correlation between knee stability and graft thickness, signal intensity, or shape. CONCLUSION: After PCL reconstruction, MR imaging in the 1st year depicts a thickened graft with increased signal intensity. There does not appear to be a relationship between clinical stability and findings at MR imaging.  相似文献

We report a 22-year-old man with a solid mass in the right proximal leg, which was furned out to be a gossypiboma. MR imaging revealed a well-defined mass lesion that showed intermediate signal intensity at T1-weighted imaging (T1WI) and slightly high signal intensity at T2-weighted imaging (T2WI). Wavy, low-signal-intensity stripes were visible within the fluid-filled central cavity. At surgical exploration, a sponge, retained after previous knee surgery, was discovered, and it was found that a granuloma had developed. Pathologic examination revealed granulomatous inflammation, with lymphocyte and giant cell infiltration. The presence of wavy, low-signal-intensity gauze fibers at T2WI may be a characteristic MR appearance of gossypiboma.  相似文献

OBJECTIVE: To describe the MR imaging features of tuberculous osteomyelitis. DESIGN AND PATIENTS: MR imaging features of 11 patients (14-65 years) with proven extra-spinal tuberculous osteomyelitis were reviewed. Osseous and adjacent soft-tissue changes were analyzed. RESULTS: On the basis of the signal intensity characteristics compared with the normal marrow fat, two kinds of lesions were observed: (a) predominantly intermediate to low signal intensity lesions on T2-weighted images with low signal intensity on T1-weighted images, and (b) lesions which had a discrete peripheral zone of marginally higher signal intensity than the center on T1-weighted images and surrounding edema and lower signal intensity than the fatty bone marrow with variable signal intensity on T2-weighted images. Soft-tissue abscesses and marrow edema were each noted in eight cases. Soft-tissue edema was noted in most cases. CONCLUSION: An osseous lesion with intermediate to low signal intensity on T2-weighted images and associated soft-tissue abscess may be suggestive of tuberculous osteomyelitis. Lesions with a rim of mildly increased signal intensity on T1-weighted images, a non-specific indicator of an infective process, may also be seen.  相似文献

PURPOSE: To correlate magnetic resonance (MR) signal characteristics of bone bruises with histological findings. MATERIALS AND METHODS: In 14 tibiae of young pigs, bone bruises were created in the proximal tibial metaphysis. The signal intensity seen on the MR images were correlated with histological findings. The following findings were evaluated: (a) changes of signal intensity on the tibiae; (b) changes of histology on the tibiae; and (c) changes of (a) and (b) on follow-up examinations. RESULTS: We observed three types of injuries on T1-weighted images: focal or diffuse low signal, normal signal and linear low signal intensities. Severe hemorrhagic areas showed low signal intensities on all sequences of MR imaging. Fast spin-echo (FSE) T2-weighted images showed a more distinct low signal intensity than T1-weighted images. FSE short tau inversion recovery (STIR) and FSE fat saturated (FSE-FS) T2-weighted images showed similar signal intensities with FSE T2-weighted images. FS T1-weighted enhanced images showed low signal intensities with variable enhancements. Upon histological examination, hemorrhages and edemas were prominent at the subcortical areas of the contusion sites. The areas of dense, low signal intensities in all imaging sequences showed signs of severe hemorrhage. The areas of diffuse low signal and enhanced areas showed mixed areas of hemorrhages and edemas. Follow-up MR imaging showed evolution of the processes of hemorrhages and edemas with fatty marrow changes. CONCLUSIONS: MR imaging can depict changes in the bone marrow resulting from direct injury to the bone. MR imaging is a useful tool for evaluating the evolution of bone bruises.  相似文献

RATIONALE AND OBJECTIVES: The most important complication of skeletal injuries involving the growth plate is growth disturbance. The purpose of this study was to evaluate MR features of growth plate modification after fat graft interposition in growth plate injury and to correlate these findings with pathological findings. METHODS: A growth plate injury model was used in 12 skeletally immature rabbits. A longitudinal drill hole 5 mm in diameter was created in the central part of the growth plate in the distal femur, bilaterally. One side was filled with autologous fat, and the contralateral defect was left empty as a control. Magnetic resonance imaging was obtained 1, 3, and 6 months after surgery, and routine histological study was performed. The authors evaluated sequential changes in MR images and the histological basis of MR findings. RESULTS: In grafted femur, the signal intensity of the grafted area was lower than that of the surrounding bone on T2-weighted images at 1 month. The growth plate defect at 3 to 6 months was modified and had a proximally tapering appearance. The ratio of the growth plate defect was smaller in the grafted femur than in the control femur after surgery. Histologically, the fat-grafted area was replaced by fibrous connective tissue. In the control femur, a bony bridge was rectangular in the longitudinal direction and showed isosignal intensity with a rim of low signal intensity on T1-weighted imaging. Histologically, the defect was filled with mature fatty marrow with new bone formation in the control femur. CONCLUSIONS: The proximally pointing appearance and the low signal intensity of the grafted area on MR suggested fibrous degeneration of grafted fat that prevented solid bony bridge formation in experimentally induced growth plate injury.  相似文献

Bone abnormalities of the knee: prevalence and significance at MR imaging   总被引:16,自引:0,他引:16

Focal abnormal signal intensity within the distal femoral condyles or proximal tibial plateaus is frequently seen on T1- or intermediate-weighted magnetic resonance (MR) images of the knee. To characterize the prevalence and significance of these findings better, a retrospective study of MR imaging of the knee, performed on the first 434 consecutive patients at a primarily outpatient community MR imaging center, was undertaken. A signal intensity loss demonstrated on images with a short echo time (20 or 25 msec) was divided into three types based on morphologic criteria. A type 1 finding was a diffuse, often reticulated signal intensity loss in the metaphyseal and epiphyseal regions of bone. A type 2 loss was associated with an interruption in the smooth, black cortical line. A type 3 finding was a profound signal intensity loss primarily restricted to the immediate subcortical region. Seventeen percent of all cases revealed a type 1 or 2 loss that may have been compatible with acute bone injuries. These bone lesions were frequently associated with tears of the anterior cruciate ligament and contralateral collateral ligament, but infrequently these lesions were detected with plain radiography and arthroscopy. When associated with a weight-bearing cortex, bone injuries detected with MR imaging may explain clinical symptoms of pain on weight bearing, and type 1 injuries may represent regions of bone at increased risk for the subsequent development of insufficiency fractures if the bone is not adequately protected during trabecular healing.  相似文献

Skeletal complications in pediatric oncology patients.   总被引:5,自引:0,他引:5
D J Roebuck 《Radiographics》1999,19(4):873-885
Pediatric oncology patients are at risk for the development of numerous skeletal complications, and radiologic studies are important in the identification and evaluation of these conditions. Methotrexate osteopathy manifests as osteopenia, dense provisional zones of calcification, pathologic fractures, and sharply outlined epiphyses. Hypertrophic osteoarthropathy may occur with nasopharyngeal carcinoma or tumors of the lungs or pleura and manifests as cortical thickening, lamellar periosteal new bone formation, and soft-tissue swelling. Biomechanical abnormalities are often seen at bone scintigraphy in patients who have undergone surgery for bone tumors. Growth plate injury may manifest as marked deformity, sclerotic metaphyseal bands, metaphyseal fraying, and longitudinal striations. Radiation "osteitis" is seen as an initial decrease in bone density with subsequent development of a mixed radiolucent and sclerotic appearance. Ischemic necrosis of the femoral heads is best demonstrated at magnetic resonance (MR) imaging and has low signal intensity on T1-weighted images and a high-signal-intensity rim on T2-weighted images. Bone infarcts are seen as well-demarcated, often ring-shaped areas of decreased signal intensity on T1-weighted MR images and as areas of increased signal intensity on short-inversion-time inversion recovery images. Radiographic signs of infection include bone destruction, periosteal new bone formation, and sclerotic changes. Short-inversion-time inversion recovery MR imaging is particularly useful in evaluating posttherapy changes in bone marrow. Osteochondroma may demonstrate a cartilaginous cap at MR imaging, whereas the most important finding in radiation-induced sarcoma is a soft-tissue mass. Radiologists who work with children with cancer need to be familiar with these complications and their imaging appearances.  相似文献
