Penile tourniquet syndrome

A 2 1/2-year-old boy was examined because of swelling of the penis of six weeks' duration. Several strands of hair wrapped around the penis and buried under the epithelium were found to cause a constriction. The penile tourniquet syndrome should be suspected whenever the penis is swollen, especially in children, even if a foreign body is not visible.     

青壮年隐匿阴茎手术矫形的体会

目的：探讨手术矫治青壮年隐匿阴茎的手术方法和疗效。方法：经冠状沟环形切开阴茎皮肤与筋膜,松解分离脱套至阴茎根部,切断阴茎背悬韧带,清除部分脂肪,再将皮肤及筋膜复位,间断环形缝合冠状沟切口,阴茎皮肤与白膜缝合固定。结果：38例青壮年患者手术矫形效果满意,阴茎体显露明显,平均延长3.7cm。结论：本矫形形式安全、有效,是目前治疗青壮年隐匿阴茎的一种满意整形术。     

Case of giant basal cell carcinoma around the base of the penis in an elderly patient

We report a case of an 85‐year‐old man with a doughnut‐shaped basal cell carcinoma around the base of the penis invading the cavernosum urethra. In cases in which curative resection is planned, the penis is resected, cystostomy is performed, and postoperative self‐management is required. In line with the patient's wishes and the fact that he was an elderly person living alone without family support, we decided to perform penis‐conserving surgery. Relapse occurred once, for which additional resection was conducted. Two years after the resection, there was no evidence of tumor recurrence. The therapeutic plan for elderly patients should be modified after considering the patient's social background, wishes, activity and life expectancy.     

Foreign body granuloma formation secondary to silicone injection

Injectable silicone has been used extensively over the last 40 years for soft tissue augmentation. Although considered biologically inert, this material has been implicated in a variety of adverse reactions including granulomas, disfiguring nodules, and lymphedema, sometimes with latent periods of decades. Often these complications are a result of the use of industrial grade products injected by unlicensed or unskilled practitioners. Here we report a case of foreign body granuloma in the thigh secondary to silicone injection in the buttocks. Initially the patient did not disclose a cosmetic contouring procedure administered by a nonprofessional nine months earlier, making diagnosis difficult. We remind clinicians to include foreign body granulomas in the differential diagnosis of apparent cellulitis and to question patients about the use of injectable fillers.     

Divided or kissing nevus of the penis: A new case with dermoscopic findings

We report on a child with a divided or kissing nevus of the penis with benign symmetric globular pattern on dermoscopic examination that evolved into a globular‐reticular pattern after 1 year of follow‐up. Divided nevus of the penis is exceedingly rare, with only four cases with dermoscopic findings reported to date. Dermoscopy seems to be useful when deciding on the most appropriate management of these cases.     

Erythroplasia of Queyrat treated with topical methyl aminolevulinate photodynamic therapy

An 82-year-old man presented with invasive squamous cell carcinoma of the glans penis arising in erythroplasia of Queyrat. He underwent Mohs' micrographic surgery for the invasive carcinoma. Seven weeks later, the residual erythroplasia of Queyrat was treated using photodynamic therapy. Methyl aminolevulinate cream was applied to the glans of the penis under occlusion for 3 hours and then, after local anaesthesia, irradiated with a 630-nm red-light-emitting diode lamp at a dose of 37 J/cm(2) for 8 min. The patient experienced some mild swelling, redness and pain, which subsided over the following 5 days. Eighteen weeks after photodynamic therapy, there had been no recurrence of the lesion, when the patient died from an unrelated cause.     

Autodestruction of penis due to carcinoma

Four unusual cases of autodestruction of the penis are described with their aetiology, diagnosis and management. Causes for delay in diagnosis and therapy, prevention and early recognition of this condition are emphasized.     

Granuloma from liquid silicone

In recent years, the demand for cosmetic interventions to augment soft tissue by injecting different substances has increased, due to their apparent innocuity. However, these procedures are not free from adverse reactions, such as the formation of foreign body granulomas, a phenomenon described in literature with most of the materials used. We report the case of a female patient with inflammatory lesions of the face, whose diagnosis was made after the histopathological study, which revealed a granuloma caused by liquid silicone.     

阴茎硬化性淋巴管炎4例分析

目的:提高对阴茎硬化性淋巴管炎的认识和诊断水平。方法:通过总结临床阴茎硬化性淋巴管炎的临床资料,分析阴茎硬化性淋巴管炎的误诊原因。结果:阴茎硬化性淋巴管炎比较少见,临床上多易误诊。结论:阴茎硬化性淋巴管炎是良性的自限性疾病,诊断明确后治疗相对简单,临床医生要加强对其的认识,减少误诊误治。     

Hookworm‐related cutaneous larva migrans of the penis successfully treated with topical ivermectin

Hookworm‐related cutaneous larva migrans is an infestation of the skin caused by nematodes. Involvement of genitals is extremely rare. We report the case of a child with this infestation on the penis who cleared rapidly with topical ivermectin.     

Intravascular Papillary Endothelial Hyperplasia on the Penis: Report of a Case

An 82-year-old Japanese man with intravascular papillary endothelial hyperplasia on the penis is described. To the best of our knowledge, this is the first report of a patient who developed this tumor on his penis.

报道1例红色毛癣茵引起的股癣合并龟头癣。患者男,16岁。因双侧腹股沟及龟头红斑、鳞屑伴痒2周就诊。经真菌培养确诊为红色毛癣茵感染所致的股癣合并龟头癣,予伊曲康唑口服及联苯苄唑乳膏外用治疗2周后痊愈。     

阴茎大汗腺汗囊瘤1例

报告阴茎大汗腺汗囊瘤1例.患者男,23岁.阴茎部出现一米粒大小肤色丘疹渐增大1月余.皮损组织病理学检查示表皮大致正常,真皮内可见一扩大的囊腔,囊壁由1～2层细胞构成,细胞核呈圆形或椭圆形,位于细胞底部,可见明显的顶浆分泌现象,未见囊内容物.诊断:阴茎大汗腺汗囊瘤.予手术完整切除皮损,切缘干净规整,对皮缝合后表面无渗液....     

Multifocal cutaneous epithelioid angiomatous nodules of the penis

Cutaneous epithelioid angiomatous nodule (CEAN) represents a recently described benign vascular proliferation. We report a case affecting a 28-year-old man who presented with painful nodules on his glans penis. The patient is symptom-free 18 months after tumor excision. This is the first report of CEAN occurring on the glans penis.     

Penis tuberculid (papulonecrotic tuberculid of the glans penis): treatment with a combination of rifampicin and an extract from tubercle bacilli (T.B. vaccine)

A case of penis tuberculid (papulonecrotic tuberculid of the glans penis) and its treatment with combination of oral rifampicin (RFP) and injection of an extract from tubercle bacilli (T.B. vaccine) were reported. We also examined the literature and discovered 37 cases of this disease in Japan in the last 10 years (between 1978 and 1987).     

Keloid formation on an inconspicuous penis

Inconspicuous penis is an umbrella term used to categorize several anatomic anomalies that occur in newborn boys. The consequences of this anomaly may be catastrophic unless it is accurately diagnosed. Repeated circumcisions will accentuate the problem without any solution. Keloid formation on the penis is exceptionally rare even though circumcision is a world-wide procedure. Herein, we present a 13-year-old boy with a buried glans penis, micturation difficulty, and keloid formation. He was previously circumcised twice before admission to our clinic. A surgical correction together with intralesional steroid injection was performed. The outcome of this procedure was satisfactory. We conclude that repeated circumcision may not be the best choice of treatment for keloid formation on the penis.     

阴茎完全离断显微再植的急诊手术治疗

目的:探讨阴茎完全离断显微再植的急诊手术治疗方法及效果。方法:回顾性分析我院2007年1月至2016年1月收治的7例行显微再植急诊手术治疗的阴茎完全离断患者的临床资料,观察其术后排尿、皮肤感觉、性生活恢复情况。结果:7例患者末次随访切口均甲级愈合,阴茎血运良好,排尿正常,平均尿流率为(18.5±1.7)m/s,平均勃起硬度达到(3.6±0.5),夜间自发阴茎勃起存在,性生活可正常完成。随访中未见尿道狭窄、尿瘘等并发症。结论:急诊显微手术治疗阴茎完全离断有着良好的效果及安全性。术前做好离断阴茎的保护,术中成功吻合血管及神经,术后实施镇痛、抗感染治疗均是提高阴茎成活率、降低并发症风险的重要手段。     

脱细胞异体真皮基质医用组织补片在阴茎增粗成形中的应用

目的：探讨脱细胞异体真皮基质医用组织补片移植治疗阴茎短小综合症的可行性,观察阴茎增粗成形术的临床效果.方法：采用脱细胞异体真皮基质医用组织补片“C”状移植于阴茎体背侧BUCK筋膜与白膜之间,治疗阴茎短小综合症10例,术中同时行阴茎悬韧带松解及阴茎背深静脉结扎.结果：9例恢复良好,阴茎自然状态下平均延长3.0～5.0cm,周径平均增加2.0～3.0cm,患者对阴茎增粗效果均表满意;仅1例因组织补片自溶导致手术失败.结论：脱细胞异体真皮基质医用组织补片移植用于阴茎增粗成形,治疗阴茎短小综合症,方法安全可行,临床效果满意.     

浅表性阴茎癌保留阴茎完整性切除术的可行性以及对患者及配偶性心理影响(附3例报告)

目的:探讨浅表性阴茎癌保留阴茎完整性手术的可行性以及术后对患者及配偶的心理生理学影响。方法:3例患者术前均行必要体检确定肿瘤未侵及白膜,常规术前病理活检证实为阴茎鳞癌,常规检查胸片、髂血管走行区超声及盆腔CT未发现局部淋巴结转移及远处转移。切除肿瘤组织及远端0.5cm包皮组织,深达白膜层,近端切除多余包皮内板及适当外板包皮组织连同阴茎筋膜达白膜层,术中冰冻病例证实切缘阴性。术后1周预防性加用双侧腹股沟区放疗治疗。结果:3例患者均按时复诊,随诊18个月~3年,局部无肿瘤复发,腹股沟区及腹膜后淋巴结未见转移。术后生殖器外观满意,排尿正常,性欲及性生活满意度为满意。结论:浅表性阴茎肿瘤保留阴茎完整性阴茎切除术从手术方案方面是可行的,从手术效果上看,可使患者更好的保持良好的性自信、性功能和性生活满意度,使患者夫妇对生活充满希望及保持幸福。     

A diagnostic trap for the dermatopathologist: granulomatous reactions from cutaneous microimplants for cosmetic purposes

We present a case of late granulomatous reactions from silicone that first appeared in a site different from that of the injection causing an incorrect diagnosis of liposarcoma in the beginning. The histological picture was a cystic-macrophagic granuloma in both the injection site (upper lip) and the migrating site (paranasal regions). We think that the foreign body has undergone an antigravity migration from the upper lip to the right paranasal region. To our knowledge, such a phenomenon has not been yet reported in literature.
