Objective The present study used a corneal topographic device to record aberrations on the anterior surface of the cornea at different time-points prior to wearing and following discontinued use of rigid gas permeable (RGP) contact lenses. The effect of wearing RGPCL on the anterior surface of the cornea was discussed to provide guidance for clinical refractive error correction. Methods The study objects were 60 eyes from 30 patients. All patients underwent identical examination procedures prior to lens use, as well as afterwards, including slit-lamp examination, non-contact tonometer measurement, computer optometry & corneal curvature measurement, subjective refraction test, and corneal topography analysis. The patients wore contact lenses every day for 1 month and then discontinued. Corneal topographies were recorded at certain time points of 30 min, 1 day, 3 days, 1 week, and 2 weeks following use. Results Total corneal aberration at each time point following discontinued use of RGP contact lenses was less than the time point prior to use. Detailed results are as follows; root mean square (RMS) (pre) = (1.438 ±0.328), RMS (30 min) = (1.076 ± 0.355), RMS (1 day) = (1.362 ± 0.402) , RMS (3 day) =(1.373 ± 0.398), RMS (7 day) = (1.387 ± 0.415), and RMS (14 day) = (1.448 ± 0.423) .Results showed that at 30 minutes after discontinued use of RGP contact lenses, almost all 2nd-and 3rd-order aberrations were altered. Quadrafoil Z10 and spherical Z12 of the 4th-order were also changed. Alterations to Z5, Z6, and Z12 at 1 day after discontinued use were significant compared with the time period prior to RGP use: Z5 and Z6 decreased, and Z12 increased slightly( F =2. 869 ~5. 549 ,P =0. 001 ~0. 042). Z5 and Z6remained decreased at 3 days after discontinued use, but Z9 and Z10 continued to increase and Z12 returned to levels prior to RGP use ( P > 0. 05 ). At 2 weeks after discontinued use, all aberrations were not significantly different from the values prior to use ( P > 0. 05). Conclusions The use RGP contact lenses greatly reduced total aberration of the anterior surface of the cornea. Changes to 2nd- and 3rd-order aberrations ( including Z3, Z4, Z5, Z6, Z7, and Z8 ) were more significant. Following discontinued use of RGP contact lenses, the majority of lower order aberrations returned to original levels in a short period of time. During this process, a transient higher order aberration appeared, but all changes disappeared within 2 weeks after discontinued use of RGP contact lenses.  相似文献

目的 通过对角膜前表面波阵面的测量,研究配戴RGPCL及停戴之后角膜前表面的形态变化.方法 临床观察.使用客观波阵面仪测量30人(60只眼)在配戴RGPCL之前及停戴之后的30 min及1、3、7、14 d后的角膜前表面像差,并进行各时间点的对照分析.对停戴后各时间点、各像差项及各组间进行方差分析、配对t检验.结果 配戴RGPCL前、停戴后30 min及1、3、7、14 d时配戴者双眼角膜前表面7阶32项Zernike像差均方根值分别为1.438±0.328,1.076±0.355、1.362±0.402、1.373±0.398、1.387±0.415及1.448±0.423.其中停戴30 min时,2阶、3阶像差几乎全部改变(Z3、Z4、Z5、Z6、Z7、Z8),4阶中的四叶草(quadrafoil Z10)和球差(spherical Z12)2项改变,这些改变中除Z8、Z9、Z10增大外,其余均为减少.停戴后1 d相比配戴前,Z5、Z6减少,Z12比戴前有所增加(F=2.869~5.549,P=0.001~0.042).停戴后3 d,Z5、Z6依然比配戴前减少,而Z9和Z10又再次出现增加,Z12恢复到配戴前水平.停戴7 d后,只有Z6和Z9略有改变,其余均恢复至配戴前.停戴14 d时各阶像差与配戴前已无变化(F=0.018~1.876,P=0.154~0.650).结论 配戴RGPCL能显著减少角膜前表面总像差,这种改变在停戴后30 min最为明显,随着停戴时间延长慢慢减少,低阶像差基本都在较短时间内恢复戴前水平,在恢复过程中可能会产生一过性的高阶像差,但所有改变均在停戴2周内停止.

AIM: To study the dynamic character of aberration between the cornea and the ocular with aging, and to evaluate the symmetry of the aberrations between right and left eye in order to supply the data for clinic to do the refractive surgery reasonably. METHODS: This is a comparative case series study. 82 normal cases (164 eyes) including 37 females (74 eyes) and 45 males (90 eyes) were recruited through the routine examinations、Topolyzer and wavefront analysis. The average age was 25.9±5.0 years old (range 18 to 49 years old), and the mean spherical equivalent (SE) is -3.82±2.21D (range -1.00 to -6.00D). The changes of aberrations regarding age, the relationship between anterior corneal and total aberrations were analyzed,as well as the symmetry between right and left eyes by using Zernike terms. RESULTS: The Z 3 1, RMS3 of corneal aberrations, Z 3 1, Z 4 0 , RMS3 and RMS4 of ocular aberrations had a positive correlation with age. The zernike terms both in corneal and whole eye were significantly correlated between right and left eyes. CONCLUSION: The corneal horizontal coma, ocular horizontal coma and ocular spherical aberrations become to increase at the age of more than 40 years old. The dynamic change of aberration with aging, balance between corneal and ocular, and the symmetric character between left eye and right eye should be designed carefully in the treatment nomogram before the refractive surgery.  相似文献

AIM: To conduct a comparative study of effectiveness of silicone hydrogel contact lens and hydrogel contact lens, which are used in patients after laser-assisted subepithelial keratomileusis (LASEK). METHODS: Sixty-three patients (121 eyes) with a spherical equivalent ≤-5.0 D were chosen after undergoing LASEK in 2012 at Guangdong General Hospital. They were randomly divided into 2 groups. The silicone hydrogel group included 32 cases (61 eyes) that wore silicone hydrogel contact lenses for 4-6d after the operation, while the hydrogel group included 31 cases (60 eyes) who wore hydrogel contact lenses for 4-6d after the operation. Patients’ self-reported postoperative symptoms (including pain, photophobia, tears, and foreign body sensation) were evaluated. The healing time of the corneal epithelium, the visual acuity of patients without contact lens after epithelial healing, and the incidence of delayed corneal epithelial shedding were also assessed. The follow-up time was 1mo. RESULTS: Postoperative symptoms were milder in the silicone hydrogel group than in the hydrogel group. There were significant differences in pain, foreign body sensation, and photophobia between the 2 groups (P<0.05), although there was no significant difference in postoperative tearing (P>0.05). The healing time of the corneal epithelium in the silicone hydrogel lens group was markedly shorter than that in the hydrogel group (4.07±0.25 vs 4.33±0.82d, t=2.43, P=0.02). Visual acuity without contact lenses after healing of the corneal epithelium was better in the silicone hydrogel group compared with the hydrogel group (χ2=7.76, P=0.02). There was no significant difference in the occurrence of delayed corneal epithelial shedding between the 2 groups (P>0.05). CONCLUSION: Patients with LASEK using silicon hydrogel contact lenses had less discomfort and shorter corneal epithelial healing time compared with those using hydrogel contact lenses, suggesting that silicon hydrogel contact lenses may be considered to be a better choice of bandage contact lens after LASEK.  相似文献

Objective To observe the changes of high-order aberrations of pseudophakic eye after phacoemusification surgery, and to explore their clinical significance.Methods By Zywave wavefront instrument,the high-order aberrations such as general high-order aberrations,spherical aberration,coma were detected their root mean square (RMS) of 129 surgical cases(129 eyes)(60 asperical intraocular lens,69 spherical intraocular lens) and 107 control persons (107 eyes). Analysis of covariance was then committed. Results Compared with the control, the RMS values of general high-order aberration, Z331, Z311, Z441, Z400, Z420,Z440,Z511,Z510,Z550 of the surgical cases were statistically significant (P<0.05); the RMS values of general high-order aberration,Z331,Z441,Z400,Z440,Z511,Z510,Z550 of the aspherical cases were statistically significant (P<0.05); the RMS values of general high-order aberration,Z331,Z311,Z400,Z420, Z440, Z511,Z550 of the spherical cases were statistically significant (P<0.05). Conclusions The majority of high-order aberrations of pseudophakic eye after phacoemusification surgery is significantly increased; the sub order aberrations increase differently according to the different lens, while the general high-order aberration, Z331,Z400,Z440,Z511,Z550 always increase; the aspherical lens can effectively reduce spherical aberration of pseudophakic eye.  相似文献

AIM: To analyze the effect of steep meridian small incision phacoemulsification cataract surgery on anterior, posterior and total corneal wavefront aberration. METHODS: Steep meridian small incision phacoemulsification cataract surgery was performed in age-related cataract patients which were divided into three groups according to the incision site: 12 o’clock, 9 o’clock and between 9 and 12 o’clock (BENT) incision groups. The preoperative and 3-month postoperative root mean square (RMS) values of anterior, posterior and total corneal wavefront aberration including coma, spherical aberration, and total higher-order aberrations (HOAs), were measured by Pentacam scheimpflug imaging. The mean preoperative and postoperative corneal wavefront aberrations were documented. RESULTS: Total corneal aberration and total lower-order aberrations decreased significantly in three groups after operation. RMS value of total HOAs decreased significantly postoperatively in the 12 o’clock incision group (P<0.001). Corneal spherical aberration was statistically significantly lower after steep meridian small incision phacoemulsification cataract surgery in BENT incision group (P<0.05) and Pearson correlation analysis indicated that spherical aberration changes had no significant relationship with total astigmatism changes in all three corneal incision location. CONCLUSION: Corneal incision of phacoemulsification cataract surgery can affect corneal wavefront aberration. The 12 o’clock corneal incision eliminated more HOAs and the spherical aberrations decreased in BENT incision group obviously when we selected steep meridian small incision. Cataract lens replacement using wavefront-corrected intraocular lens combined with optimized corneal incision site would improve ocular aberration results.  相似文献

目的 观察透气硬性角膜接触镜(Rigid Gas-permeableContactlenses,RGP)在角膜屈光手术后矫正视力、改善视觉症状的效果.方法 眼科视光门诊中做过近视性角膜屈光手术的患者7例1 1只眼,经常规检查后,予验配多种设计的RGP,记录戴镜前的裸眼视力、框架镜最佳矫正视力和戴RGP的最佳矫正视力.记录部分患者戴RGP前后的波前像差.随访过程中记录配戴情况及并发症.结果 该组患者需要三种设计的RGP镜片:圆锥角膜RGP、逆几何RGP和大直径非球面RGP.戴RGP前裸眼视力在0.03~0.4者7只眼,0.8~1.0者4只眼.戴RGP前的框架镜最佳矫正视力在0.2~0.5者5只眼,0.7~1.0者6只眼.戴RGP的最佳矫正视力在0.7~0.9者4只眼,1.0者7只眼.配戴RGP后的球差、彗差和三叶草差分别较配戴前降低88.7%、92.9%和18.9%.全部患者的视觉症状在配戴RGP后均改善.角膜上皮染色是最常见的并发症,主要是圆锥角膜的锥顶部上皮磨损染色.平均随访21.5月,全部患者能够坚持日间配戴.结论 角膜屈光手术后配戴特殊设计的RGP可以提高视力,改善视觉质量.未见严重地角膜接触镜相关的并发症.
Abstract:
Objective To evaluate the efficacy of specially designed rigid gas-permeable contact lenses (RGP) in visual correction and visual symptom control after corneal refractive surgery. Methods Eleven eyes of 7 cases after myopic corneal refractive surgery from optometry clinic were fitted with RGP in different designs after routine eye checks. Uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA) before RGP fitting and best contact lens corrected visual acuity (BCLCVA) after RGP fitting were recorded. Wave front aberrations of some patients before and after RGP fitting were measured. Fitting state and complications were recorded at follow-ups. Results Three kinds of lens design were applied: keratoconic design, reverse geometry design and aspheric design with larger diameter. BCLCVA was much higher than UCVA and BSCVA. Spherical aberration, coma and trefoil were reduced by 88.7%, 92.9% and 18.9% respectively after RGP fitting. Visual symptoms of all patients after RGP fitting were improved. The main complication was corneal staining of iatrogenic keratoconus. All patients can tolerate wearing RGP in the day time after an average follow-up of 21.5 months. Conclusions Specially designed RGP fitted after corneal refractive surgery can raise the best corrected visual acuity and improve the quality of vision. No serious complications related to RGPwearing are observed.  相似文献

AIM:To compare the efficacy between two different silicone hydrogel bandage contact lenses after transepithelial photorefractive keratectomy(T-PRK).METHODS:In this randomized controlled trial,a total of 89 patients(178 eyes) who underwent T-PRK at the Qingdao Eye Hospital from October to December 2019 were selected.One random eye wore a Senofilcon A bandage contact lens after surgery,and the other eye a Balafilcon A bandage contact lens.Pain scores,uncorrected visual acuity(UCVA),spherical equivalent(SE),corneal epithelial healing status,epithelial thickness,bandage lenses deposits,lenses movement,and ocular surface conditions were measured and compared.RESULTS:There were no differences between the two groups in UCVA,SE,corneal epithelial healing status,corneal epithelial thickness,tear river heights and tear film rupture time at each follow-up visit.However,postoperative pain scores in the Senofilcon A group were significantly lower than those of the Balafilcon A group(Fintergroups=67.833,P<0.001;Ftime=383.773,P<0.001;Finteraction=57.344,P<0.001).The duration of pain in eyes in the Senofilcon A group was shorter than that of the Balafilcon A group(t=-3.326,P=0.001).The surface deposition scores and movement scores of Senofilcon A bandage lenses on the first and fourth days after surgery were lower than those of Balafilcon A bandage lenses(Z=-5.385,-6.782,P<0.001;Z=-8.336,-8.906,P<0.001).CONCLUSION:Both Senofilcon A and Balafilcon A bandage lenses have good efficacy after T-PRK.Senofilcon A lenses are associated with less pain and more comfort compared to Balafilcon A.  相似文献

AIM: To evaluate the effect of Intacs SK corneal ring segment implant for treatment of patients with moderate to severe keratoconus, who have clear central cornea and cannot tolerate contact lens. METHODS: In this prospective, non-comparative, interventional case series study performed in Dena Hospital, Shiraz, Iran, thirty-seven eyes of thirty-six patients with moderate to severe keratoconus, clear central cornea, and contact lens intolerance were enrolled and underwent Intacs SK corneal ring segment implantation. Preoperatively, uncorrected distance visual acuity (UCDVA), best-corrected distance visual acuity (BCDVA), central corneal thickness(CCT) and average keratometry (Av-K) were measured and compared with post-operative results at one week, one month, three months, and six months. RESULTS: Mean preoperative UCDVA and BCDVA were (1.32±0.31)logMAR and (1.07±0.27)logMAR, respectively. Av-K was (52.13±0.39)D, and the CCT was (432±39.5)μm. Post-operative examinations showed a clinically significant improvement in both UCDVA and BCDVA (P<0.001). There was also a significant effect based on the time of assessment on both UCDVA and BCDVA and both parameters had a continuous improvement during the follow-up period. Three months after operation there was a statistical significant reduction of Av-K (P=0.0001), but there were no significant changes in CCT (P=0.149). CONCLUSION: Intacs SK corneal ring segment implants seem to be a safe and effective treatment option for patients who have keratoconus, clear central cornea, and contact lens intolerance.  相似文献

Cryo- and laser therapy of stage 3 have reduced, but not eliminated the occurrence of retinal detachment in stages 4a, 4b and 5 of ROP. In this disease the treatment of these stages is still the greatest challenge to the ophthalmologist. Therefore, the aim of this paper is to present our up-to-date possibilities of treatment of different kinds of retinal detachments in ROP, including segmental scleral buckling, encircling scleral buckling, scleral resection, vitrectomy and its modifications in ROP. Guidelines of surgery of retinal detachment in active stage 4 and 5 of retinopathy of prematurity have been described.  相似文献

PURPOSE: The aim of the study was to evaluate some of the possible risk factors for retinopathy of prematurity (ROP) treated with laser photocoagulation or cryocoagulation. MATERIAL AND METHODS: The study comprised 71 preterm infants with ROP needing treatment and 118 prematures without ROP or with ROP requiring no treatment, as a control group. All infants were born with gestational age < or = 32 weeks and birth weight < or = 1500g. The perinatal variables, including some of clinical data, the length of mechanical ventilation as well as continous positive airway pressure (CPAP), duration of total parenteral nutrition and some of laboratory data were analyzed, to evaluate their correlation with the development of ROP. RESULTS: Gestational age before 28 weeks (OR = 5.11), episodes of convulsiones (OR = 2.15), mechanical ventilation for more than 20 days (OR = 5.86) and > 30 days (OR = 7.47), CPAP for more than 5 days (OR = 4.15) and > 10 days (OR = 2.84), total parenteral nutrition for more than 10 days (OR = 7.84) and > 20 days (OR = 9.02) and elevated peak of alanine aminotransferase (AIAT) levels (OR = 3.17) were significant risk factors for ROP requiring treatment. CONCLUSIONS: The opthalmic examination for retinopathy of prematurity requiring laser photocoagulation or cryocoagulation should be obligatory for prematures born < or = 32 weeks of gestational age, with birth weight < or = 1500 g.The frequency of the consecutive ophthalmic examinations depends on the severity of prematurity and on the presence and intensification of the risk factors for ROP.  相似文献

目的:探讨我国人角膜曲率半径的正常值及不同性别、不同年龄的角膜曲率半径差异。方法:对10998只眼的角膜曲率进行检测,并按男、女10岁一组进行统计分析。结果:(1)K1为7.65mm,K2为7.71mm,平均K值为7.67mm。较眼科学正常值K:7.77mm短0.1mm。(2)K的平均值男性较女性的长0.1155mm。且女性各年龄段角膜曲率半径均男性的有不同程度的减短。(3)男女均随年龄的增长,角膜曲率半径大致呈递减趋势,即:角膜曲率半径与年龄成反比关系。(4)男女K1,K2之比,均随年龄增长而增长,即K1逐渐增长而增长,即 K1值逐渐增长,K2逐渐减短。结论:本文测定的角膜曲率较眼科文献中的提供的正常值短0.1mm,并且存在着年龄、性别上的差异。  相似文献

目的:通过调查分析,了解艾滋病眼部并发症的临床表现、治疗及预后。方法:收集赞比亚卡布韦总医院眼科2008-08/2009-08就诊患者。结果:艾滋病眼部并发症症状重,病程长,致盲率高。结论:充分认识、掌握艾滋病眼部并发症的临床表现,提高艾滋病的检出率,早发现、早治疗,提高艾滋病患者的生活质量。同时应强调预防是降低艾滋病发生率的关键。  相似文献
