Objective. To compare three-compartment MR wrist arthrography with non-enhanced MRI in correlation with wrist arthroscopy, and to evaluate the potential of MR arthrography for consistently visualizing all parts of the scapholunate interosseous ligament of the wrist (SLIL) and exactly diagnosing the site and extent of SLIL defects. Design and Patients. In 41 patients with wrist pain (34 patients with wrist pain for more than 6 months) plain radiographs, stress views, non-enhanced MRI and three-compartment MR arthrography were done within 2 h of each other, using three-dimensional volume acquisition (0.6–1.0 mm effective slice thickness) with a gradient-recalled echo sequence and a 1.5-T magnet. The MR arthrography findings were compared with the findings from non-enhanced MRI and correlated with the arthroscopic findings in all patients. Results. The dorsal, central and palmar segments of the SLIL could be delineated exactly by MR arthrography in 95% of the patients; with non-enhanced MRI only 28% of SLIL segments were seen consistently. Demonstration of SLIL defects was possible with high diagnostic confidence in 42% of SLIL segments by non-enhanced MRI and in 94% by MR arthrography. With wrist arthroscopy as the standard of reference, sensitivity and specificity values for SLIL perforations were 52%/34% for non-enhanced MRI and 90%/87% for MR arthrography. Conclusions. MR arthrography, using three-dimensional volume acquisition with thin slices (0.6–1.0 mm), combines the advantages of three-compartment arthrography and non-enhanced MRI. It shows the precise location and magnitude of ligamentous defects of all parts of the SLIL, correlates well with wrist arthroscopy and has potential implications for diagnosis and treatment planning.  相似文献

The objective of this study was to characterize the appearance of the hamatolunate facet using high-resolution magnetic resonance (MR) arthrography in cadavers and to correlate the presence of this anatomic variant with the presence of osteoarthritis in the wrist. High-resolution MR images of 22 cadaveric wrist specimens were obtained after tri-compartmental arthrography. Two readers in consensus analyzed the MR images and recoded the presence or absence of a hamatolunate facet. Geometric characteristics and cartilage and ligament integrity were analyzed. A third reader, who was blinded to the purpose of the study, recorded cartilage lesions of all the bones of the proximal and distal carpal rows. A hamatolunate facet was present in 11 of 22 wrists (50%). The mean coronal size of the lunate facet at the lunate (type II lunate) was 4.5 mm (range, 2-6 mm). The highest frequencies of cartilage lesions were seen in the scapho-trapezio-trapezoid joint (45.5%) and at the proximal pole of the hamate (54.4% and 40.9% for consensus reading/blinded reading, respectively). In cases with a hamatolunate facet, the frequency of cartilage lesions in the proximal pole of the hamate was 81.8% and 63.6% versus 27.3% and 18.2% without such a facet (chi-squared, P=0.01/ P=0.03). No correlation of the presence of a hamatolunate facet with interosseous ligament tears or lesions of the triangular fibrocartilage was seen. In conclusion, the hamatolunate facet is a very common anatomic variant. The presence of a hamatolunate facet is associated with cartilage damage in the proximal pole of the hamate.  相似文献

PURPOSE: To evaluate the diagnostic accuracy of arthrography, magnetic resonance (MR) imaging, and MR arthrography in the detection and localization of defects of the triangular fibrocartilage (TFC) in cadaveric wrists, using arthroscopy as a reference standard. MATERIAL AND METHODS: Twenty-four specimen wrists were evaluated. The different imaging modalities were blinded to reviewers and were interpreted independently. A classification of TFC defects was used for the evaluation of images in the different imaging modalities, thus permitting a more uniform correlation. Two cases were excluded from the MR imaging study because of poor image quality. Contingency tables with the chi-square test and Fisher's exact test were used for statistical analysis. RESULTS: Defects of the TFC were identified in 17 of the 24 specimen wrists by means of arthroscopy, and 16 defects were observed when arthrography was carried out. With MR imaging 14 defects of the TFC were detected in the 22 specimen wrists evaluated, and with MR arthrography 16 defects were observed. Most defects were central or combined (two or more defects). In comparison to arthroscopy, the accepted diagnostic gold standard, the following results were found for arthrography in the detection of TFC defects: sensitivity 95%, specificity 100%, and accuracy 95% (P<0.0005); for MR imaging: sensitivity 86%, specificity 85%, and accuracy 70% (P<0.002); and for MR arthrography: sensitivity 100%, specificity 85%, and accuracy 95% (P<0.0005). CONCLUSION: The results of the study seem to indicate that both arthrography and MR arthrography have high accuracy, and either would be useful for evaluation of the TFC. The combined approach using both techniques would have a very high accuracy equivalent to that resulting from arthroscopy.  相似文献

Purpose:

To prospectively evaluate the diagnostic accuracy of magnetic resonance (MR) arthrography for the detection of articular cartilage abnormalities at 3.0T and 7.0T in cadaveric wrists.

Materials and Methods:

MR imaging (MRI) was performed in nine cadaveric wrists (four right wrists, five left; mean age, 81.0 ± 9.8 years) after the intraarticular administration of gadoterate‐meglumine. A 3.0T and 7.0T MR system, mechanically identical custom‐built 8‐channel wrist coil arrays and a similar standard MRI protocol, were used. MR images were evaluated for visibility of articular cartilage surfaces, presence of cartilage lesions, and confidence of diagnosis by two independent radiologists. Open pathologic inspection served as reference standard. Sensitivity, specificity, negative predictive values (NPV) and positive predictive values (PPV), and accuracy (ACC) were calculated. Wilcoxon signed rank test was used to assess differences in the diagnostic performance.

Results:

Visibility of articular cartilage surfaces was significantly better at 3.0T than at 7.0T (P < 0.001). Mean sensitivity, specificity, NPV, PPV, ACC for both readers were 63%, 90%, 85%, 76%, 82% at 3.0T, respectively, and 52%, 91%, 82%, 75%, 79% at 7.0T. The difference between 3.0T and 7.0T was not significant for reader 1 (P = 0.51), but was significant for reader 2 (P = 0.01). The level of confidence was significantly higher at 3.0T than at 7.0T for both readers (P = 0.004; P = 0.03).

Conclusion:

MR arthrography of the wrist at 7.0T is still limited by the lack of commercially available radiofrequency coils and limited experience in sequence optimization, resulting in a significantly lower visibility of anatomy, lower diagnostic accuracy, and level of confidence in judging cartilage lesions compared to 3.0T. OBJECTIVE: Our purpose was to describe the appearance of the acetabular capsular-labral complex on MR arthrography and to correlate this appearance with surgical findings in adult patients and with gross anatomic findings in cadavers. SUBJECTS AND METHODS: MR arthrography of the hip joint was performed in 40 patients and six cadavers. All patients underwent subsequent arthrotomy of the hip. MR arthrography consisted of a T1-weighted three-dimensional gradient-echo sequence in both the coronal oblique and sagittal oblique planes after intraarticular injection of a 2 mmol/l solution of gadopentetate dimeglumine. The normal and pathologic appearance of the capsular-labral complex was assessed, and the labra were evaluated on the basis of morphology, signal intensity, presence of a tear, and attachment to the acetabulum. MR arthrography findings were correlated with the surgical results in all patients and with the anatomic sections of the cadaveric hip joint specimens. RESULTS: MR arthrography images of the T-weighted three-dimensional gradient-echo sequences allowed visualization of the anatomic structures. The normal labrum was triangular, without any sublabral sulcus, and of homogeneous low signal intensity. A recess between the labrum and the joint capsule could be identified in instances in which no thickened labrum was present. Labral lesions included labral degeneration, a tear, or a detached labrum either with or without thickening of the labrum. The sensitivity for detection and correct staging of labral lesions with MR arthrography in the patient study was 91%; the specificity, 71%; and the accuracy, 88%. CONCLUSION: MR arthrography with T1-weighted three-dimensional gradient-echo sequences allows excellent assessment of the normal and pathologic acetabular capsular-labral complex.  相似文献

OBJECTIVE: We assessed the usefulness of sonography in evaluating the glenoid labrum in cadaveric specimens using arthroscopy as a standard of reference. MATERIALS AND METHODS: Eighty labral quadrants in 20 cadaveric shoulders were examined by two musculoskeletal radiologists using 5- to 7-MHz linear and curvilinear transducers. Agreement was reached by consensus. After sonography, arthroscopy was performed by an experienced orthopedic surgeon. Each labral quadrant was classified at the time of sonography and arthroscopy as normal, degenerated, or torn. RESULTS: Concordance between sonography and arthroscopy was 86% (69/80 quadrants). In differentiating abnormal labrum (tear or degeneration) from normal labrum using sonography, sensitivity was 63%, specificity was 98%, positive predictive value was 94%, negative predictive value was 86%, and accuracy was 88%. In differentiating labral tears from other labral conditions (degeneration or normality), sensitivity was 67%, specificity was 99%, positive predictive value was 67%, negative predictive value was 99%, and accuracy was 98%. CONCLUSION: Sonography has a promising role in the evaluation of the glenoid labrum, particularly in excluding labral tears when the labra appear normal on sonography. Further studies are required using normal and symptomatic patients to determine the usefulness of sonography in the diagnosis of labral abnormalities.  相似文献

PURPOSE: To analyze the normal pattern of fluid accumulation adjacent to the posterior cruciate ligament and anatomic variations of joint capsule insertion sites in the posterosuperior corner of the human knee by using magnetic resonance (MR) imaging in cadaveric specimens. MATERIALS AND METHODS: Fourteen fresh cadaveric knees (obtained and used according to institutional guidelines, with informed consent from relatives of the deceased) from 11 men and three women (six left knees, eight right knees; age range, 70-82 years at time of death; mean age, 76 years +/- 4.4 [standard deviation]) were studied with high-spatial-resolution MR imaging performed before and after intraarticular injection of 35-45 mL gadopentetate dimeglumine. MR images were evaluated by two readers in consensus, with emphasis on location of fluid posterior to the posterior cruciate ligament, communication of that fluid with the medial or lateral compartment of the knee, and the relation of fluid to surrounding structures. Readers also were asked to measure, in the sagittal plane, the distance between the posterior capsular insertion sites and the femoral physeal scar. For anatomic analysis, cadaveric specimens were sectioned in 3-mm-thick slices in the sagittal plane that approximated the sections acquired at MR imaging. RESULTS: In all 14 cadaveric specimens, MR arthrographic images showed a fluid collection behind the posterior cruciate ligament (in the posterior cruciate ligament recess), a finding not evident on images obtained prior to contrast material injection. The recess was distended during flexion, and it communicated only with the medial femorotibial compartment in all cases. Posterior to the posterior cruciate ligament recess, a fat pad was observed in all specimens. Incomplete joint capsule was seen behind the fat pad in seven specimens. Joint capsule insertion was at the level of the femoral physeal scar or between it and a point 15 mm above it. CONCLUSION: The posterior cruciate ligament recess has specific characteristics that allow its identification: communication with the medial compartment of the knee and absence of the adjacent joint capsule.  相似文献

The main aim of this study was to provide an overview of the anatomy of the dorsal hood (DH) based on the dissection of sixteen cadaver hands, correlated with magnetic resonance (MR) and ultrasound findings. A secondary aim was to assess the function of components of the DH. Sixteen embalmed hands were evaluated by MRI and ultrasound before being dissected. Each hand was photographed during each stage of dissection. Secondly we evaluated the role of the different structures of the DH in the stability of the extensor tendon by transection of the different components alternatively at the ulnar and radial sides. MR, ultrasound, and dissection showed that the extensor tendon (ET) is stabilized by the sagittal band (SB) at the level of the metacarpophalangeal (MCP) joint and more distally by the transverse and the oblique bands, respectively. Transection of the radial SB of the second finger leads systematically to ulnar dislocation of the ET. The transection of the ulnar DH does not lead to instability of the ET. The SB is the most important structure of the DH in the stability of the ET at the MCP level. Rupture of the radial SB of the second finger leads systematically to ulnar dislocation of the ET.  相似文献

Objective

The purpose of our study was to use magnetic resonance (MR) imaging and MR arthrography to demonstrate the anatomy of the lateral ankle ligaments using standard and oblique imaging planes in cadavers.

Material and methods

MR imaging of ten cadaveric ankles was performed before and after intra-articular administration of contrast solution. Proton-density MR images were acquired in standard and oblique imaging planes. MR imaging was correlated with anatomic sections. Measurements using oblique imaging planes were obtained to characterize the morphology of the lateral ligaments.

Results

The anterior talofibular ligament (ATFL) had a variable number of bands in all specimens, separated by fat signal oriented obliquely parallel to the long axis of the ligament. The fibular attachment of ATFL was located in close proximity to the fibular attachment of the distal band of the anterior tibiofibular ligament (AITFL). The angle formed by the calcaneofibular ligament (CFL) and the fibular shaft varied with different ankle positions. Special axial oblique plane best demonstrated the CFL. The posterior talofibular ligament (PTFL) was multi-fasciculated in appearance. Dorsiflexion of the ankle joint helped elongate the PTFL and best depicted this ligament in its entirety in the axial plane.

Conclusion

Oblique imaging planes parallel to the long axis of the individual ligaments may improve visualization of the anatomy of the lateral ankle ligaments. The orientation of the lateral ankle ligaments is affected by the position of the talocrural and subtalar joints. Understanding the morphology of the lateral ankle ligaments can help radiologists diagnose abnormalities of these ligaments.  相似文献

Objective

To determine the accuracy of ultrasonography (US) in the evaluation of degenerative changes in the distal radioulnar joint (DRUJ).

Methods and materials

Ten cadaveric specimens were obtained. US evaluation of cartilage degeneration and thickness was performed by two independent and blinded readers (R1 and R2). Gross anatomy and MR arthrography evaluated by two readers in consensus served as the reference standard. The joint surface not accessible to US was measured.

Results

US interreader agreement was non-existent for cartilage thickness measurements and moderate for cartilage degeneration grading (weighted kappa = 0.41).Comparing US and MR imaging evaluation, there was no correlation between US R1 and MR imaging (Pearson correlation coefficient [PCC] = 0.352) and a moderate correlation between US R2 and MR imaging (PCC = 0.570) concerning cartilage thickness measurements. Concerning cartilage degeneration grading, there was a moderate to strong (R1 Spearman correlation coefficient [SCC] = 0.729)/R2 SCC = 0.767) correlation concerning cartilage degeneration grading.Comparing US and gross anatomic evaluation, there was no correlation for US R1 (PCC = 0.220) and a strong correlation for US R2 (PCC = 0.922) concerning cartilage thickness measurements, and a strong to moderate correlation (R1 SCC = 0.808/R2 SCC = 0.597) concerning cartilage degeneration grading.The mean sector of the articular surface of the ulna head not accessible to US was 13°.

Conclusion

In conclusion the DRUJ is accessible to US except in the central 13° sector of the joint surface. US was approved to be sufficient in demonstrating advanced stages of cartilage degeneration. Thus, US of the DRUJ is recommended in patients suffering from ulnar-sided wrist pain.  相似文献

OBJECTIVE: The purpose of this article is to describe the MR arthrography findings of humeral avulsion of the posterior band of the inferior glenohumeral ligament in 17 patients. To elucidate the clinical importance of this abnormality, we also correlate our imaging findings with the presence of coexisting structural abnormalities; clinical presentation; and, when available, arthroscopic evaluation (n = 8). CONCLUSION: Humeral avulsion or insufficiency of the posterior band of the inferior glenohumeral ligament can be easily detected using MR arthrography. This ligamentous abnormality may be seen in isolation, or it may occur in conjunction with posterior or, less often, anteroinferior capsulolabral abnormalities. The presence of this lesion in a subgroup of patients with the clinical diagnosis of multidirectional instability may offer insight into the causes and pathogenesis of this complex entity.  相似文献

OBJECTIVE: Our objective was to determine the accuracy of MR arthrography for identification of tears of anterior cruciate ligament reconstruction grafts and for detection of localized anterior arthrofibrosis and impingement. MATERIALS AND METHODS: We retrospectively identified 27 patients (mean age, 31 years; range, 18-45 years) with anterior cruciate ligament reconstruction who had undergone MR arthrography followed by arthroscopy within 1 year. Three radiologists independently reviewed the MR arthrograms for the presence or absence of graft tear, localized anterior arthrofibrosis, and impingement. RESULTS: Graft tears were identified with 100% sensitivity by all three reviewers with specificities of 100%, 89%, and 94%. Localized anterior arthrofibrosis was identified with 100% sensitivity by all reviewers, with specificities of 79%, 71%, and 38%. Impingement was detected with sensitivities and specificities of 83% and 100%, 83% and 52%, and 33% and 90% by the three reviewers, respectively. Interobserver agreement was almost perfect for detection of graft tear (kappa = 0.83, 0.92, and 0.83), was fair to moderate for detection of localized anterior arthrofibrosis (kappa = 0.50, 0.32, and 0.22), and was slight to fair for detection of impingement (kappa = 0.40, 0.08, and 0.35). CONCLUSION: MR arthrography can accurately depict the presence of anterior cruciate ligament graft tears. Localized anterior arthrofibrosis and graft impingement were less accurately detected and showed greater observer variability.  相似文献

PURPOSE: To delineate the normal magnetic resonance (MR) imaging anatomy of the lesser metatarsophalangeal (MTP) joints in a cadaveric model and compare the MR arthrographic and MR bursographic findings with the standard MR imaging findings. MATERIALS AND METHODS: T1-weighted spin-echo MR imaging of 48 lesser MTP joints of 12 cadaveric feet was performed. The specimens were subsequently evaluated with MR arthrography, MR bursography, or both examinations. Musculoskeletal radiologists evaluated standard MR images to determine the normal appearances of the joint structures, especially the fibrous capsule, plantar plate, and collateral ligament complex (CLC). Signal intensity, morphology, joint thickness, relationships with adjacent structures, and best plane for analysis were analyzed. The contrast material-enhanced (ie, arthrographic and bursographic) MR imaging findings were compared with the standard MR imaging findings. RESULTS: The coronal plane was best for simultaneous depiction of the fibrous capsule, plantar plate, and collateral ligament complex and for assessment of the relationship between the CLC and the plantar plate. The sagittal plane was best for analysis of the bone attachments of the plantar plate and the transverse plane for evaluation of the CLC attachment sites in the phalanges. MR arthrography enabled identification of the bare areas and recesses of the joints, better delineation of the plantar plate articular surface, and better evaluation of the integrity of the soft-tissue components of the joints. Compared with the other examinations, MR bursography did not help improve these evaluations. CONCLUSION: MR imaging is an excellent examination for delineating the anatomy of the lesser MTP joints. Compared with standard MR imaging, only MR arthrography helps improve visualization of the fibrous capsule, plantar plate, and CLC of the lesser MTP joints.  相似文献
