探讨在医学研究生教学中建立以循证医学为指导思想的"新型例会制度",指出例会制度建立的要求、方式,总结例会制度具备的开拓视野、教学相长、更好地融合循证医学思想等优点。并指出了该例会制度实行中可能出现的问题,为进一步提高医学研究生的培养质量做出了有益的探索。

护士长例会制度的改进与落实

目的缩短护士长例会的时间,提高管理效率,达到管理目的。方法运用PDCA的管理方法,会前明确目的,收集资料,制订详尽的会议议程;召开例会时,确定会议内容和具体要求;会议结束及时进行效果评价。落实会议内容。结果不良事件发生率较上年度下降20％,护理人员对会议知晓率达到80％以上。结论改进护士长例会制度能够缩短例会时间,提高参会人员时会议内容的知晓率。

Extramural Continuing Medical Education

The experience of one Faculty of Medicine in developing programs of continuing medical education in community hospitals is presented. After mention of the importance of regular reading of the medical literature, and the problems created by its growing volume, the necessity of supplementary programs in community hospitals is pointed out. The different patterns of community hospital meetings that evolved to meet various circumstances in the Atlantic Provinces are detailed. A “course” consisting of six weekly evening meetings, followed by morning case presentations and discussions, has proved the most successful form of continuing medical education in community hospitals. Better than half the doctors in the community attend, and active participation is the rule. The importance of advance planning, the techniques of advance planning, and the expense of operating the program are listed. A projection is made that 46 such courses would be required to cover the four Atlantic Provinces adequately, with a staff equivalent to eight full-time teachers and a budget in excess of $200,000. The fact that this is only 7.5% of Canada's medical population indicates the magnitude of the unmet needs of continuing medical education in this country.

举办八年制医学教育是医学教育的重大改革与举措之一。一年多来举办八年制医学教育的高校先后在北京大学、中南大学举行了两次八年制医学教育高峰会议,对八年制医学教育有关问题进行了研讨与交流,取得了良好的效果。本文就八年制医学教育应该注意的一些问题进行了探讨,提出了一些看法。     

护理工作中人文关怀的意义及其具体体现

目的：推行护理管理机制改革,开展人性化护理服务,落实人文关怀具体措施,提升护理质量。方法：定期在全体护理人员中通过讲座、会议、讨论、培训、考核等形式普及人文关怀理念,将人文关怀具体落实到日常工作的方方面面,如优化就医环境,改善病房管理,实行礼仪服务,大力倡导人文关怀理念下的医院文化。结果：护理管理机制顺利改革,患者就医全过程受到人文关怀,整体需求得以满足,护患关系更加融洽,护理工作质量明显提升。结论：普及人文关怀理念,实施人性化护理服务,顺利改革了护理管理机制,提高了护理服务质量,医院的社会效益和经济效益大幅提升。     

Incidence of and risk factors for nodding off at scientific sessions

We conducted a surreptitious, prospective, cohort study to explore how often physicians nod off during scientific meetings and to examine risk factors for nodding off. After counting the number of heads falling forward during 2 days of lectures, we calculated the incidence density curves for nodding-off episodes per lecture (NOELs) and assessed risk factors using logistic regression analysis. In this article we report our eye-opening results and suggest ways speakers can try to avoid losing their audience.     

International scientific meetings: relation between structure and function

Increasing pressure is being placed on the scientific community to evaluate research activities. Scientific meetings consume a small but important fraction of the research budget. Audit of a well established series of scientific meetings showed that they met their immediate objectives in that they were international and multidisciplinary and provided a forum in which all participants actively contributed to discussion. The meetings had a positive outcome for the participants, leading in many cases to the subsequent exchange of research material (60% of participants) and to the establishment of collaborative research projects (31%). The impact of the meetings on the scientific community at large was assessed by citation analysis, which showed that the proceedings were cited early, often, and over a substantial period.     

Running effective meetings: a primer for doctors

This paper uses a series of exercises and practical examples to assist individuals and groups of doctors in training to gain skills in a critical area of management: conducting and participating in effective meetings. Through this paper, readers will be shown how to recognise and manage situations as they occur in meetings to work towards appropriate outcomes. By understanding the elements of conducting a meeting from preparation through to follow up, doctors will be able to conduct and participate more effectively in meetings that arise in their workplaces.     

高职院校英语课平时成绩考评分析

教师合理设置平时成绩考核能够增强学生学习动力,提高教学效果.然而高职英语课平时成绩由于所占比例过低、考核手段存在缺陷等原因未能充分发挥作用.该文建议提高平时成绩比例并利用综合考评法对英语课平时成绩考评方法进行改进.     

Ways of using evidence-based medicine in general practice

General practitioners wanting to practise evidence-based medicine (EBM) are constrained by time factors and the great diversity of clinical problems they deal with. They need experience in knowing what questions to ask, in locating and evaluating the evidence, and in applying it. Conventional searching for the best evidence can be achieved in daily general practice. Sometimes the search can be performed during the consultation, but more often it can be done later and the patient can return for the "result". Case-based journal clubs provide a supportive environment for GPs to work together to find the best evidence at regular meetings. An evidence-based literature search service is being piloted to enhance decision-making for individual patients. A central facility provides the search and interprets the evidence in relation to individual cases. A request form and a "results" format make the service akin to pathology testing or imaging. Using EBM in general practice appears feasible. Major difficulties still exist before it can be practised by all GPs, but it has the potential to change the way doctors update their knowledge.     

Should research ethics committees meet in public?

Currently, research ethics committees (RECs) in the UK meet behind closed doors-their workings and most of the content of their decisions are unavailable to the general public. There is a significant tension between this current practice and a broader societal presumption of openness. As a form of public institution, the REC system exists to oversee research from the perspective of society generally. An important part of this tension turns on the kind of justification that might be offered for the REC system. In this paper I adapt Daniels and Sabin's accountability for reasonableness model for just resource allocation to the research ethics context to provide some structural legitimacy and to enable progress on the question of openness. After considering the consequences of adopting this model for open REC meetings, I then examine some reasons that might be offered against open meetings. These arguments do not overwhelm the core intuitions behind the presumption of openness but they do, I suggest, give us reason to retreat from fully public meetings. I conclude that there should be important adjustments to the system towards public accountability and that there are grounds for stopping short of fully public meetings.     

深低温低流量体外循环下动脉导管未闭手术的评价

关于重症复杂型支脉导管未闭（ＰＤＡ）是否应在深低温低流量体外循环下行闭合手术，尚有争议。国内虽未见关于对此种术式有争议的正式报道，但在几乎会议上曾经引起争论，本文结合我院１９８４．９～１９９０．８月在体外循环下所行２１例ＰＤＡ闭合术，就此种术式的实用价值进行了讨论。
