目的 探讨无精子症患者外周血和睾丸组织睾酮(T)、雄激素受体(AR)、热休克蛋白90(HSP90)相互之间的关联性。方法 选择45例无精子症患者,测量其静脉血FSH、LH、T、AR、HSP90;给予行睾丸穿刺,取得的生精小管在显微镜下观察有无精子,之后用滤纸吸去培养液、电子天平秤重、投入液氮,次晨取出磨碎,加入1000倍量PBS,混匀,200g离心,测量上清液T、AR、HSP90的含量。按睾丸穿刺结果和FSH值分为A(睾丸穿刺有精子,FSH<8IU/L)、B(睾丸穿刺无精子,FSH<8IU/L)、C(睾丸穿刺无精子,FSH>8IU/L)三组,比较三组间外周血和睾丸组织T、AR、HSP90的差异性,分析FSH、LH、T、AR、HSP90之间的直线相关关系。结果 三组间比较,外周血AR(F=5.478,p=0.026)和HSP90(F=5.972,p=0.019)、睾丸组织T(F=6.757,p=0.006)和AR(F=4.988,p=0.047)差异有统计学意义,而外周血T和睾丸组织HSP90差异无统计学意义。直线相关性分析,FSH与外周血HSP90(r=0.729,p=0.001)、睾丸组织T(r=0.722,p=0.002)呈正相关关系,有统计学意义; LH与外周血(r=0.629,p=0.012)和睾丸组织(r=0.610,p=0.016)T均呈正相关关系,有统计学意义;外周血T与外周血AR(r=-0.665,p=0.005)呈负相关关系,而睾丸组织T与睾丸组织AR(r=0.544,p=0.029)呈正相关关系,有统计学意义;外周血(r=0.559,p=0.042)和睾丸组织(r=0.803,p=0.000)AR与其相应的HSP90呈正相关关系,有统计学意义。结论 睾丸体积正常的无精子症患者,外周血T浓度波动,HSP90和AR浓度显著升高,反馈作用于FSH和LH,睾丸内T和AR逐渐升高,HSP90变化不明显。  相似文献

BackgroundThe role of midluteal phase gonadotropin-releasing hormone (GnRH) agonist had been an issue of debate. The aim of this retrospective study was to evaluate the effect of a mid-luteal phase GnRH agonist as an additional luteal phase support (LPS) in patients receiving intracytoplasmic sperm injection (ICSI). Additionally, we elucidate which subgroup would gain the most benefit from GnRH agonist as LPS.MethodsThe medical records were retrieved from January 2009 to January 2012 and a total of 348 patients receiving ICSI were included in this retrospective study. Among them, 240 patients met the inclusion criteria of patients aged ≤38 years, previous assisted reproductive technology (ART) cycles ≤ 2. There were 147 patients in the decapeptyl group who received GnRH agonist decapeptyl 6 days after ICSI as additional LPS and 93 patients in the control group. Subgroupings were done according to advanced age, the number of previous ART cycles, high basal follicle-stimulating hormone (FSH) level, and patients who had fewer mature oocytes retrieved. Live birth rates, clinical pregnancy rate (CPR), and implantation rate were the primary outcomes.ResultsLPS with decapeptyl led to a higher implantation rate (24.5% vs. 17.0%, p = 0.023), a higher CPR (49.0%, n = 72 vs. 33.3%, n = 31, p = 0.023) and a higher live birth rate (41.5%, n = 61 vs. 28.0%, n = 26, p = 0.039). In the subgroup analysis, decapeptyl improved the CPR of those patients with basal FSH >8 mIU/mL (50.0%, n = 15 vs. 8.3%, n = 1, p = 0.031) and also improved CPR (42.3%, n = 11 vs. 0%, n = 0, p = 0.017) and live birth rate (30.8%, n = 8 vs. 0%, n = 0, p = 0.035) of patients whose number of mature oocytes was three or fewer.ConclusionThis study demonstrated that administration of decapeptyl as additional luteal support can enhance ICSI clinical outcomes. Those patients with higher basal FSH level or fewer number of mature oocytes may obtain particularly significant benefit.  相似文献

目的探讨围绝经期妇女血清中促卵泡激素(FSH)水平与骨代谢指标总Ⅰ型前胶原氨基端延长肽(TP1NP)的相关性。方法选取2011年8月—2012年11月在浙江大学医学院附属妇产科医院就诊的围绝经期妇女274例,年龄33~60岁。应用电化学发光法检测妇女血清FSH和TP1NP水平,并应用多重线性回归模型分析围绝经期妇女的年龄、FSH及TP1NP因素间的相关性。结果274例围绝经期妇女的TP1NP平均水平为(48.99±20.31)ng/mL,且FSH水平与TP1NP水平呈正相关( r=0.159, P=0.009)。在40~50岁妇女中,FSH水平 < 40 mIU/mL者的TP1NP水平低于FSH>40 mIU/mL者[(35.05±18.11)ng/mL与(51.33±24.67)ng/mL; t=-2.954, P=0.004];而在小于40岁和51~60岁妇女中,不同FSH水平下TP1NP水平差异无统计学意义( t=-0.063, P=0.950; t=1.177, P=0.242)。多重线性回归模型分析显示年龄变量标准化偏回归系数为0.047( P=0.448),而FSH变量标准化偏回归系数为0.146( P=0.019),可见FSH水平比年龄变量对TP1NP的影响更大。 结论围绝经期妇女TP1NP水平与FSH呈现一定相关性,尤其是对于40~50岁的妇女,高FSH可能是引起绝经后骨质疏松的重要因素。  相似文献

目的 观察邻苯二甲酸二丁酯(di-butylphthalate,DBP)所致雄性生殖毒性效应,并探讨维生素C(vitamin C,Vit C)对DBP所致SD雄性大鼠生殖功能受损的干预调节作用。 方法 将60只SD雄性大鼠随机分组,分别为正常对照组,DBP低、中、高剂量组(DBP 100、500、1 000 mg/kg体重),VitC对照组(30 mg/kg体重)与VitC与DBP高剂量联合干预组。灌胃干预21 d,检测大鼠脏器重量及血液生化各项指标,大鼠精子活力、浓度、畸形率,以及检测血清睾酮(T)、黄体生成素(LH)、卵泡刺激素(FSH)水平。 结果 与正常对照组相比,DBP高剂量组大鼠睾丸减轻,睾丸脏体比下降,部分血生化指标异常,精子活力下降、畸形率升高,血清T、LH、FSH水平显著下降,差异有统计学意义(均P<0.05);与DBP高剂量组相比,VitC干预组大鼠睾丸组织损伤有不同程度的恢复,精子活力、畸形率,以及血清T、LH、FSH水平差异有统计学意义(均P<0.01)。 结论 DBP可致大鼠睾丸萎缩,精子活力下降,精子畸形率增高;DBP可致血清T、LH、FSH水平降低。VitC可提高血清LDH水平和T、LH、FSH水平,改善DBP所致大鼠精子活力下降,降低精子畸形率,为进一步开展DBP生殖毒性的预防干预提供理论依据。   相似文献

目的 观察和分析雄性裸鼹鼠中可育个体和不可育个体生殖系统结构和功能的差异,初步探索雄性不可育个体不能繁育后代的原因。方法 健康雄性裸鼹鼠可育个体和不可育个体各5只,先采集一侧睾丸和附睾组织,称重后换算脏器系数;再采集另一侧睾丸组织,用中性福尔马林溶液进行固定后制备组织切片;取另一侧附睾用作精子计数、精子活动度的测定;采集血清测定血清中黄体生成素(LH)和睾酮(T)浓度。结果 与可育个体组相比较,不可育个体组的睾丸重量下降(P<0.05),附睾重量显著下降(P<0.01),且睾丸系数和附睾系数均显著下降(P<0.01);精子数量显著减少(P<0.01),精子活动度也显著减弱(P<0.01);血清黄体生成素和睾酮水平均显著降低(P<0.01);睾丸组织切片观察显示,不可育个体各级生精细胞严重脱落,排列混乱无序,支持细胞和间质细胞均有减少。 结论 雄性裸鼹鼠不可育个体生殖系统存在睾丸和附睾萎缩、生精功能下降和性激素分泌减少等现象。  相似文献

目的 探讨腹腔镜下卵巢囊肿剔除术中电凝和缝合止血对卵巢储备功能的影响。方法 选取2018年1月—2020年11月青海省人民医院收治的卵巢囊肿患者200例作为研究对象,随机分为电凝组和缝合组,每组100例。比较两组患者手术前后抗缪勒管激素(AMH)、雌二醇(E2)、促卵泡激素(FSH)、黄体生成素(LH)水平,并分析其相关性。结果 电凝组与缝合组手术时间、术中出血量比较,差异无统计学意义(P>0.05)。术后1个月,两组窦卵泡数、卵巢动脉收缩期峰值血流速度少或慢低于术前(P <0.05);且缝合组多或快于电凝组(P <0.05)。两组血清AMH、E2水平均低于术前(P <0.05),且缝合组高于电凝组(P <0.05)。两组血清FSH、LH水平均高于术前(P <0.05);且缝合组低于电凝组(P <0.05)。Pearson相关性分析结果显示,术后血清AMH与E2水平呈正相关(r=0.553,P <0.05),与FSH、FSH/LH呈负相关(r=-0.614和-0.482,均P <0.05),与LH无相关性(r=0.115,P>...  相似文献

目的研究长爪沙鼠动情周期血清催乳素(PRL)、黄体生成素(LH)、促卵泡利激素(FSH)和雌二醇(E2)水平和卵泡黄体数量的变化。方法采用直接镜检法确定长爪沙鼠的动情周期,以ELISA法测定长爪沙鼠血清PRL、E2、FSH和LH水平,并观察动情周期各时相各级卵泡和黄体数量的变化情况。结果直接镜检法计数角化细胞统计阴道脱落细胞的类型及比例,确定长爪沙鼠动情周期的四个时相,发现长爪沙鼠动情周期各级卵泡数量各时相间均差异未见显著性。动情间期黄体数量较其他各时相显著减少(P0.05)。血清PRL和LH水平从动情前期至动情间期逐渐增高,至动情间期达峰值(P0.05),而血清E2和FSH水平均在动情前期最高(P0.05),在动情间期达到最低。结论长爪沙鼠动情周期各时相卵泡和黄体数量与各时相激素的变化密切相关,动情周期各时相卵泡数量差异未见显著性。动情周期中出现的血清FSH和E2峰值的时相与小大鼠一致,而PRL和LH出现峰值的时相与大小鼠有差异,为长爪沙鼠生殖生理特性的进一步研究提供了参考。  相似文献

Objective To evaluate effects of cypermethrin on the testis histology and testosterone, LH and FSH in adult male Sprague‐Dawley rats. Methods The intact adult male rats were randomly divided into five groups and were treated with cypermethrin at doses of 0, 7.5, 15, 30, or 60 mg/kg per day by oral gavage for 15‐days. After the treatments, serum was collected for hormone assays. The testes, epididymides, seminal vesicles, and prostates were excised and weighed. The right testis was frozen for daily sperm production and the left one was processed for histopathology. Results Daily sperm production decreased significantly in 30 and 60 mg/(kg day) groups. Testicular structure abnormalities included atrophic and distorted seminiferous tubules, deformed and disordered arrangement of germ cells, reduced germ cells, Sertoli cells and Leydig cells, vacuolization and multinucleated formations of spermatids in the cypermethrin‐treated rats. Vacuolization was found in Sertoli cells and the deformed nucleus was noted in Leydig cells. Serum testosterone reduced significantly in 30 and 60 mg/(kg day) groups. Serum FSH increased significantly in 60 mg/(kg day) group. Conclusion Cypermethrin induces impairments of the seminiferous tubules structure and spermatogenesis in the rats. The damages of the male reproductive system may be attributed to the imbalance of circulating testosterone.  相似文献

目的:探讨睾丸素/雌二醇(T/E2)值对低剂量克罗米芬治疗特发性少精弱精症效果的预测价值。方法:选择特发性少精弱精症患者60例,予小剂量克罗米芬治疗,观察治疗效果,比较治疗前后的精子密度、精子活动率、促卵泡激素(FSH)、促黄体生成素(LH)、T、E2及T/E2值的变化。结果:克罗米芬治疗特发性少精弱精症的临床治愈率为26.67%,总有效率为76.67%;治疗后患者精子密度、精子活动率、FSH、LH、T和T/E2值均较治疗前明显升高,而E2显著下降(P结论:精子密度、精子活动率、FSH、LH值是低剂量克罗米芬治疗特发性少精弱精症疗效评价的良好指标,治疗过程中T/E2值变化也很明显,可以与上述指标一起作为克罗米芬有效性综合评估的指标。  相似文献

目的  探讨前列腺癌(PCa)患者血清性激素睾酮(T)、雌二醇(E2)、孕激素(P)、卵泡刺激素(FSH)、黄体生成素(LH)及泌乳素(PRL)的水平及其与PCa的病理分级及临床分期的相关性。方法  PCa和良性前列腺增生症(BPH)患者各45例,另选取体检健康男性45例为正常对照组。采用电化学发光免疫分析法测定性激素六项(T、E2、P、FSH、LH及PRL)的水平。对PCa患者进行病理分级及临床分期,分析组间差异。评估各检测指标与PCa的病理分级及临床分期的相关性。结果  与BPH组及正常对照组比较,PCa组患者血清T降低、FSH和LH增高,差异有统计学意义(P <0.05),E2、P、PRL比较,差异均无统计意义(P >0.05);而BPH组患者T、E2、P、FSH、LH及PRL水平与正常对照组比较,差异无统计学意义(P >0.05)。相关性分析结果显示,PCa患者血清T、E2、P、FSH、LH及PRL水平与PCa的病理分级及临床分期均无相关性(P >0.05)。结论  血清T、FSH、LH水平对前列腺良、恶性疾病的鉴别有指导意义。PCa患者血清各项性激素水平与PCa病理分级及临床分期无相关性。

Background

An accurate diagnosis of the etiology of azoospermia is crucial, as sperm retrieval methods differ between patients with non-obstructive azoospermia (NOA) and obstructive azoospermia (OA). The aim of this study was to determine hormone and testes size cutoff values to identify the cause of azoospermia in Taiwanese patients.

Methods

The medical records of azoospermic patients were retrospectively collected from April 2008 to July 2016, including hormone profile, physical examination findings, and testes size. Bilateral testes biopsies or microdissection testicular sperm extraction were performed in all patients for a definite diagnosis. The diagnostic parameters used to distinguish NOA from OA were analyzed using the t-test and receiver operating characteristic curves.

Results

A total of 51 patients with OA and 156 with NOA were included. The mean levels of testosterone (4.5 vs. 3.4 ng/ml) and E2 (26.3 vs. 19.2 pg/ml) were significantly higher in the OA group, whereas the levels of follicular stimulating hormone (FSH) (5.6 vs. 25.4 mIU/ml) and Leutinizing hormone (LH) (3.7 vs. 11.6 mIU/ml) were lower. Receiver operating characteristic curve analysis revealed that FSH and right testis size were the best individual diagnostic predictors. Using a combination of FSH >9.2 mIU/ml and right testis size <15 ml, the positive predictive value for NOA was 99.2% and 81.8% for OA.

Conclusion

A combination of FSH >9.2 mIU/ml and right testis size <15 ml was a strong predictor of NOA in our Taiwanese patients.  相似文献

Pharmaceutical excipients for topical use may contain impurities, which are often neglected from a toxicity qualification viewpoint. The possible impurities in the most frequently used topical excipients were evaluated in-silico for their toxicity hazard. Acetol, an impurity likely present in different topical pharmaceutical excipients such as propylene glycol and glycerol, was withheld for the evaluation of its health risk after dermal exposure.An ex-vivo in-vitro permeation study using human skin in a Franz Diffusion Cell set-up and GC as quantification methodology showed a significant skin penetration with an overall Kp value of 1.82×10−3 cm/h. Using these data, limit specifications after application of a dermal pharmaceutical product were estimated. Based on the TTC approach of Cramer class I substances, i.e. 1800 µg/(day∙person), the toxicity-qualified specification limits of acetol in topical excipients were calculated to be 90 µg/mL and 180 µg/mL for propylene glycol and glycerol, respectively.It is concluded that setting specification limits for impurities within a quality-by-design approach requires a case-by-case evaluation as demonstrated here with acetol.  相似文献

[目的]观察舌针结合体针对围绝经期失眠患者症状及血清雌二醇(E2)、促卵泡生成素(FSH)、促黄体生成素(LH)水平的影响。[方法]选取25例围绝经期失眠患者采用舌针结合体针治疗,操作时先取体针针刺穴位:百会、太阳、印堂、神门、内关等常规针刺,留针30 min,每隔10 min行针1次,然后取舌针穴位:心穴、肝穴、肾穴等,进针0.5~1 mm,进针后进行小幅度(90°)捻转3次,快速出针,不留针,尽量避免出血。针刺疗程为每日1次,5 d为1个疗程,共4个疗程。观察治疗前后患者匹兹堡睡眠质量指数(PSQI)各项因子成分评分及总评分、Kupperman总评分、血清E2、FSH、LH水平的变化情况。[结果]治疗后患者PSQI各项因子成分评分及总评分、Kupperman总评分、血清FSH、LH水平均低于治疗前(P0.05),血清E2水平较治疗前升高(P0.05),总有效率为92%(23/25)。[结论]舌针结合体针针刺治疗能明显改善围绝经期失眠患者的入睡困难、睡眠质量、日间功能、围绝经期症状,同时能使血清FSH、LH水平下降,血清E2水平升高。  相似文献

BackgroundSeveral formulas predicting optimal continuous positive airway pressure (CPAP) for obstructive sleep apnea treatment have been developed and diverse parameters selected as predictive factors in different sleep laboratories using different ethnic groups. This study aimed to validate a constructed predictive formula for the study laboratory and to test the hypothesis that sleep laboratories should have their own predictive formulas.MethodsFifty-seven adult subjects with obstructive sleep apnea syndrome (OSAS) were enrolled in the model-building set and underwent two polysomnography (PSG) studies to diagnose OSAS and titrate for optimal CPAP. A predictive formula, derived from anthropometric and polysomnographic variables, was validated together with two other predictive formulas in 30 subjects by comparing the mean predictive CPAP values, rates of successful prediction, and agreements.ResultsRegression analysis showed that apnea–hypopnea index (AHI), SaO2nadir (nadir of arterial oxyhemoglobin saturation by pulse oximetry), and body mass index (BMI) strongly correlated with optimal CPAP. The derived predictive formula for the study laboratory was: CPAPpred (predictive CPAP) = 6.380 + 0.033 × AHI – 0.068 × SaO2nadir + 0.171 × BMI (R2 = 0.335, adjusted R2 = 0.298). In Taiwan, different predictive formulas used by different sleep laboratories with different independent predictors led to similar mean predictive CPAP values to the mean observed optimal CPAP values, rates of successful prediction, and agreements with the observed optimal CPAP. There were significant differences between the mean predictive CPAP values and mean observed optimal CPAP values, lower rates of successful prediction, and negatively skewed 95% confidence interval (CI) when using a predictive formula derived from different ethnic populations.ConclusionA sleep laboratory may not need to have its own predictive formula for determining the optimal effective CPAP but should adopt the one derived from the same ethnicity of OSAS patients as the reference formula.  相似文献

BackgroundTo study the emotional and behavioral disturbances (EBD) in school going HIV positive children attending HIV center in a tertiary care hospital.MethodThis cross-sectional study was conducted on 258 HIV infected children between 6 and 16 years of age, 200 were on Anti-retroviral therapy (ART) and 58 were not on ART. They were evaluated for EBD by using Pictorial Pediatric Symptom Checklist (PPSC) screening tool. A cut-off score of 28 was taken as significant for detecting early EBD.ResultsThe prevalence of EBD in our study is 11.2%. Demographic and disease related profile were assessed for correlation with EBD. Type of family (p = 0.023), school attendance (p = 0.034), school performance (p = 0.045), and CD4 count (p = 0.015) were detected to have significant association with early manifestation of EBD in the study group.ConclusionsHIV positive children who have low CD4 count, poor school attendance, and performance are at a higher risk of being detected with EBD. Screening with PPSC to identify EBD in HIV positive children attending HIV clinic in a hospital setting could help in early diagnosis and management.  相似文献

目的观察精浆端粒酶活性(TA)在不同类型男性不育症患者中的变化,以及与精浆睾酮(T)、卵泡刺激素(FSH)、黄体生成素(LH)浓度之间及精液中精子数量和质量之间的相互关系。方法随机选取110例男性不育症患者与30例生育者,应用酶联免疫吸附(ELISA)方法,对其精浆TA进行检测;应用放射免疫分析(RIA)方法对其精浆T、FSH、LH浓度进行检测;按WHO规定的方法,定量分析精液中的精子密度、精子活率、a+b级精子活力百分率、精液中白细胞数量。结果不育症组精浆TA和T浓度均明显低于生育组(P均<0.01);不育症组精浆FSH和LH浓度均明显高于生育组(P均<0.05)。在不育症组中,随着精液中精子数量的递减,TA水平亦呈现逐渐下降趋势;精浆中TA与T浓度之间存在明显的相关性(r=0.236,P<0.01),与FSH和LH浓度之间无相关性(P>0.05);精子活力、活率正常组精浆TA均高于不良组(P<0.01);WBC精液组精浆TA高于非WBC精液组(P<0.05)。结论精浆TA水平与精子数量和质量存在一定的相关性,同时与精子发育密切相关的睾酮浓度有协同促进作用,端粒酶在人类生殖发育中发挥着重要作用。检测精浆TA水平对于进一步了解、预防和治疗某些病理状态下所致的生殖能力低下有重要意义。  相似文献

目的分析锁阳水提取物对少弱精大鼠模型精液质量及性激素水平的影响。方法选取SPF级SD健康雄性大鼠60只,随机分为空白组、模型组和锁阳实验组,每组20只。模型组和锁阳实验组大鼠予以环磷酰胺腹腔注射,构建少弱精症模型。建模成功后,锁阳实验组大鼠予以锁阳水提取物灌胃,空白组与模型组予以等量的0.9%NaCl溶液灌胃。比较3组大鼠精子质量(附睾尾部精子密度、精子活率、精子活力以及直线运动速度)和血清性激素水平[睾酮(T)、卵泡刺激素(FSH)、黄体生成素(LH)]。结果空白组大鼠的精子密度、精子活率、精子活力以及直线运动速度均高于模型组与锁阳实验组(P<0.05),锁阳实验组大鼠精子密度、精子活率、精子活力以及直线运动速度高于模型组(P<0.05)。模型组血清T水平低于空白组与锁阳实验组,血清FSH、LH水平高于空白组与锁阳实验组;锁阳实验组血清T水平低于空白组,血清FSH、LH水平高于空白组(P<0.05)。结论锁阳有助于改善少弱精症大鼠的精液质量、改善性激素的异常分泌。  相似文献
