Double-orifice mitral valve (DOMV) is an uncommon congenital anomaly account for 1% of congenital heart disease. However, accurate diagnosis and evaluation of valve stenosis or regurgitation and other concomitant congenital anomalies due to DOMV are required to obtain suitable treatment. Two- and three-dimensional echocardiography can contribute valuable functional and anatomic information that can support to reach this goal. Here, we present a case of complete bridge-type DOMV that causes mitral stenosis after surgical repair of the partial atrioventricular septal defect in childhood.  相似文献

A double orifice mitral valve (DOMV) represents a rare congenital malformation characterised by two valve orifices with two separate subvalvular apparatus. This case demonstrates the necessity of careful imaging of the mitral valve apparatus, not only in patients with atrioventricular septal defects, but also in patients with congenital left obstructive heart disease.  相似文献

The incidence of mitral regurgitation in mitral valve prolapse was studied retrospectively in 88 consecutive hospital based patients with mitral valve prolapse using pulsed Doppler echocardiography. Pulsed Doppler findings of turbulence in the left atrium were predictive of angiographic findings of mitral regurgitation in 31 patients. On 24-hr Holter monitoring, patients with mitral valve prolapse and mitral insufficiency had a higher prevalence of ventricular arrhythmias than patients without mitral insufficiency (61 vs 22%; P < 0.02). Detection of mitral insufficiency identified individuals with mitral valve prolapse who are at higher risk of ventricular arrhythmias.  相似文献

Congenital defects involving anomalous chordae of the mitral valve with attachment in the left atrium are an extremely rare finding and may result in valvular insufficiency. Few cases have been reported in the literature with multiple variations in anatomical location and insertion of aberrant chordae within the left ventricle and left atrium. Reported cases have presented with cardiovascular symptoms leading to diagnosis of anomalous mitral valve chordae. We present a case of a young female in which an aberrant mitral valve chorda was an incidental finding on transesophageal echocardiography.  相似文献

Forty-six children, aged 2 days to 16 years (median 2.4 years) with double orifice mitral valve (DOMV), were studied. Partial atrioventricular septal defect was the most commonly associated cardiac lesion. Symptoms were related to the degree of mitral insufficiency and/or stenosis when present. Surgical intervention directed at DOMV was required in the minority of patients who underwent repair of associated cardiac lesions. The long-term morbidity attributable to DOMV was low.  相似文献

In this report, we present 34 patients in whom surgical intervention was undertaken for severe mitral insufficiency due to mitral valve prolapse (MVP). Location and severity of MVP and regurgitation were assessed preoperatively by live/real time three-dimensional transthoracic echocardiography and closely agreed with the surgical findings.  相似文献

An unusual case of atrial septal defect secundum type, associated with intermittent severe mitral insufficiency due to billowing anterior mitral leaflet, in a 56-year old female is described. The degree of mitral insufficiency ranged from severe to none. This phenomenon manifested itself by an intermittent systolic apical thrill and intermittent mitral regurgitation on angiocardiography. Pulling the anterior mitral leaflet at operation through the atrial septal defect demonstrated that this leaflet could be caught in the left atrium, thus causing intermittent mitral insufficiency. Closure of the atrial septal defect and replacement of the mitral valve was performed.  相似文献

【】 目的 探讨“缘对缘”联合人工瓣环修复二尖瓣成形术治疗二尖瓣关闭不全的手术效果。方法 回顾性分析2004年1月至2008年12月对42例非风湿性二尖瓣关闭不全患者行“缘对缘”联合人工瓣环修复二尖瓣成形术的临床资料,男25例,女17例;年龄12~69岁(36.42±14.28岁),其中先天性二尖瓣病变12例,瓣膜退行性病变20例,缺血性二尖瓣关闭不全7例,感染性心内膜炎3例。术前心脏彩超提示:前叶病变27例,前叶 后叶病变17例;二尖瓣重度反流19例,中度至重度14例,中度反流9例;42例患者均在全身麻醉低温体外循环下应用“缘对缘”联合人工瓣环修复术进行二尖瓣成形术。术中应用250mL注射器向左心室注冰盐水观察评价成形后二尖瓣反流情况,脱离体外循环后经食管超声心动图观察成形结果。分别测量术前及术后二尖瓣反流面积、二尖瓣开口面积、左房内径及左室舒张末期内径,左室射血分数,应用t检验,研究手术前后二尖瓣反流、心脏大小变化、心功能改善情况。检验水准α=0.05。结果 全组无死亡病例,出院时心脏功能(NYHA分级)I级32例,Ⅱ级10例。随访39例,随访时间1-60(24.6±18.2)个月。超声心动图提示:术前二尖瓣反流面积为11.0cm2±3.8cm2,术后1年无或微量反流13例,轻度反流17例,轻度至中度反流9例,轻度二尖瓣狭窄1例,反流面积为4.1cm2±1.4cm2;术前二尖瓣开口面积4.14 cm2±1.54cm2,术后一年为3.58 cm2±1.10cm2;术前左心房内径为48.6mm±12.4mm,术后1年为36.5 mm±9.4mm;术前左心室舒张末径为64.3 mm±11.4mm,术后1年为50.6 mm±7.6mm;术前左心室射血分数为54.6%±6.2%,术后1年提高为63.4%±4.0%。结论“缘对缘”联合人工瓣环修复二尖瓣成形术治疗二尖瓣关闭不全手术方法安全、有效,可获得良好的治疗效果。  相似文献

目的:探讨"缘对缘"技术矫治二尖瓣关闭不全的经验及近中期手术疗效。方法:2001年6月至2007年10月利用"缘对缘"技术对58例二尖瓣关闭不全患者行二尖瓣成形术,其中瓣膜退行性病变39例;先天性二尖瓣病变14例;缺血性改变导致二尖瓣关闭不全2例;感染性心内膜炎1例,风湿性心脏病2例。术前超声显示二尖瓣反流量:中度11例,中到重度21例,重度26例。结果:术中及术后早期无死亡,术后一周超声心动图示左心房内径,左心室舒张期末内径,左心室射血分数均减小。随访11个月至7年,超声心动图监测反流:无或微量20例,轻度19例,轻到中度9例,中度3例,重度2例,3例死亡,2例术后行二尖瓣置换术。结论:"缘对缘"技术是一种有效的治疗二尖瓣关闭不全的手术方法,可取得较为满意的临床效果,远期疗效需进一步观察。  相似文献

We report the case of a 56-year-old woman with a history of rheumatic heart disease. The clinical, electrocardiographic, and radiologic findings suggested mitral stenosis. Left atrial obstructive myxoma simulating a thrombus was found by transthoracic echocardiography (TTE). The diagnosis was established by use of transesophageal echocardiography (TEE), confirmed after surgery and by anatomical investigation. Cardiac myxoma associated with mitral stenosis may be difficult to diagnose accurately using TTE. The advantage of TEE in this case and in patients with mitral stenosis is emphasized.  相似文献

We describe a case of double orifice mitral valve and a flail leaflet in a 54-year-old man. This rare congenital abnormality was disclosed through the discovery of a murmur. The transoesophageal approach clearly showed two approximately equal orifices with multiple papillary muscles. Colour Doppler echocardiography showed a moderate mitral regurgitation due to the prolapse of the posterior leaflet of the anteromedial orifice. No other abnormality was associated. Transoesophageal echocardiography is useful to analyse as well the anatomy as the functional aspect of the mitral apparatus in this particular case of congenital disease.  相似文献

Aneurysms of the cardiac valves remain rare. In this report, we describe the first case of a left ventricular to mitral valve aneurysm to left atrial shunt through a fenestrated aneurysm of the mitral valve diagnosed and successfully repaired under the guidance of transesophageal echocardiography. The transesophageal echocardiography provided substantial additional data to the transthoracic echocardiography, and was valuable in planning the surgical approach. Transesophageal echocardiography should be considered when clear-cut transthoracic echocardiographic interpretation cannot be made in patients with mitral regurgitation. 目的总结双孔二尖瓣成形术治疗二尖瓣关闭不全的经验。方法诊断为二尖瓣关闭不全接受此手术治疗30例,男12例,女18例,年龄14~52(33±17)岁。心功能Ⅱ级5例,Ⅲ级23例,Ⅳ级2例,用超声心动图对瓣膜进行评价。结果全组术前二尖瓣反流面积(9.8±3.5)cm^2,术后为(0.72±0.31)cm^2。全组无术后死亡,痊愈出院。随访1~48个月,效果满意,心功能为Ⅰ~Ⅱ级。结论双孔二尖瓣成形术是一种简单有效的手术方法。  相似文献

Pulsed, continuous-wave, and color Doppler were performed in 165 normal mitral prostheses and 58 patients with prosthetic dysfunction (46 regurgitant and 12 obstructive valves) proved by catheterization and/or surgery. Mean mitral gradient (MG) and pressure half-time (PHT) were determined in all cases.Among normal prostheses, a wide range of both MG and PHT was observed in each type of valve and a considerable overlap between valves of different size. St-Jude's valve had the most optimal hemodynamics. Mild mitral insufficiency was detected in 14% of tissue and 24% of mechanical mitral valves.Repeat studies were performed in 30 patients over a 2.4 years period. Nine patients developed Doppler evidence of new prosthetic dysfunction, while Doppler parameters remained unchanged in 21 patients during the follow-up period.Among malfunctioning valves, Doppler correctly identified all cases of prosthetic obstruction (n=12), and 42 of 46 regurgitant valves.We conclude that Doppler echocardiography is a very useful technique in both non-invasive assessment and follow-up of normal prosthetic valves in the mitral position and in detecting prosthetic dysfunction, especially when prosthetic obstruction is present.  相似文献

Catheter-induced mitral regurgitation (MR) caused by mitral papillary muscle rupture can be a fatal complication of transcatheter aortic valve implantation (TAVI). We report a case involving an 89-year-old man who presented with symptomatic severe aortic stenosis. In addition, preoperative echocardiography showed accessory mitral valve tissue. Although moderate MR developed immediately after TAVI, severe MR caused by anterolateral papillary muscle rupture occurred 3 months after TAVI. As only a few case series have been published, our case report adds to the evidence base for this treatment strategy.  相似文献

The objective of this study was to examine the feasibility and technique of intracardiac echocardiography during percutaneous balloon mitral valvuloplasty. Echocardiographic imaging is commonly used during mitral valvuloplasty. Intracardiac echocardiography is a newer technology that may provide superior imaging during complex valvular interventions. Intracardiac echocardiography and transthoracic echocardiography were performed in 19 patients undergoing percutaneous balloon mitral valvuloplasty. Intracardiac ultrasound images were obtained via the femoral vein in all patients. Imaging projections and catheter locations that were useful for the performance of mitral valvuloplasty were defined. Intracardiac echocardiography guided transseptal puncture, augmented the assessment of valve apparatus deformity, facilitated balloon positioning across the mitral valve, and permitted postprocedural valvular assessment including identification of mitral regurgitation with color Doppler. Intracardiac echocardiography provided essential imaging guidance and procedural monitoring during percutaneous mitral valvuloplasty.  相似文献
