Treatment of endodontically and periodontally involved teeth requires accurate diagnosis to determine the primary cause. The case presented involves a deep periodontal pocket associated with a necrotic tooth. Fluid was aspirated from the root canals of 40 endodontically involved teeth. This fluid was assayed for endotoxin with the limulus lysate test. Pulpless teeth contained greater concentrations of endotoxin than those with vital pulps. Symptomatic teeth also contained more endotoxin than asymptomatic teeth.  相似文献

An in vitro study of 45 teeth compared the failure loads of endodontically treated teeth restored with pin and amalgams, Para-Post dowel and composite, and glass ionomer/amalgam alloy coronal-radicular buildups. The following conclusions were made. The Para-Post and composite buildups exhibited the highest mean failure load, 35.3 kg. The mean failure load for pin and amalgam buildups was 27.9 kg. Glass ionomer/amalgam alloy coronal-radicular buildups exhibited the lowest mean failure load, 14.1 kg. Restoration of endodontically treated anterior teeth with glass ionomer/amalgam alloy coronal-radicular buildups is contraindicated.  相似文献

OBJECTIVE: The purpose of this study was to evaluate the fracture resistance and the mode of failure of endodontically treated teeth restored with four post-and-core systems. METHOD AND MATERIALS: Forty intact human incisors were randomly divided into four groups. Teeth from each group received endodontic therapy and one of four post-and-core systems: serrated, parallel-sided, cast post and core; serrated, parallel-sided, prefabricated post and resin-composite core; carbon-fiber-reinforced (CFC) post and resin-composite core; and ceramic post and resin-composite core. A full-coverage metal crown was fabricated and cemented onto each tooth. Each specimen was subjected to a compressive load at a 45-degree angle to its axis until failure. The failure load was recorded and compared statistically. The mode of failure of the specimens was analyzed. RESULTS: There was no significant difference in the failure loads among groups. More catastrophic root fracture was detected in the group restored with ceramic posts and resin-composite cores. CONCLUSION: The fracture resistance of endodontically treated teeth restored with these four post-and-core systems could be accepted clinically. Unfavorable tooth fracture was shown in all groups.  相似文献

Criteria have been presented for the evaluation of and treatment planning for endodontically treated teeth for final restoration. The dentist must consider the advantages and disadvantages of saving teeth according to their eventual role in restoring occlusal function, arch integrity, and esthetics. The prognosis is dependent on successful endodontic therapy, the presence of sound periodontal support, and an acceptable restorative effort. A review of the literature on restoration of endodontically treated teeth was presented. The advantages and disadvantages of current methods and materials were described, and a detailed technique was suggested for the newer glass-ionomer restorative materials. An all-purpose material suited for the restoration of endodontically treated teeth is still unavailable. Each patient requires individual consideration. Standardized plastic analogues simulating an endodontically treated maxillary central incisor root were used to investigate the resistance to root fracture in endodontically treated teeth. Three different post and core systems were used: (1) cast post and core, (2) Para-Post Plus post, and (3) Flexi-Post post. The core build-up material selected in this study was Ketac Silver material, after which a crown preparation was made on each analogue. Two types of preparations were used: a wide chamfer margin (butt-joint) and a wide chamfer margin with a 1.0 mm circumferential bevel. An overcasting was fabricated and was permanently cemented on each preparation. Assemblies were subjected to an increasing lingual oblique force until fracture occurred. Results were tabulated using the analysis of variance (F test) and Student's t test. Beveled preparations with a concomitant final restoration provided a significant increased resistance to root fracture. Furthermore, vertical fracture occurred twice as often with nonbeveled preparations.  相似文献

Internal, or "walking," bleaching has been successfully used in dentistry for many years to reverse stains associated with nonvital teeth. The procedure is predictable for color change but is not without problems. Current research has identified the cause of cervical resorption, which has been associated with internal bleaching. Second-generation esthetic bonding materials theoretically should provide an improved seal at the tooth-restorative interface, but the limited current research does not support this contention. A suggested method for internal bleaching and restoring the access cavity is presented.  相似文献
