The main criterion for dental age estimation in living adolescents and young adults is mineralization of third molars. A total of 1,260 orthopantomograms of 669 female and 591 male subjects aged between 15 and 22 years was examined. Of the orthopantomograms, 1,137 were from patients with Swiss citizenship and 123 were from other European countries. The mineralization status of lower third molars was evaluated based on Demirjian's classification of stages. For the statistical assessment of data, logistic regression and mean value testing were used. The findings show that completed growth in wisdom teeth is observable at ages less than 18. Male individuals were approximately 1 year ahead of females at stage H, and South East Europeans were approximately 6 months ahead of Swiss (Central Europe) at stage H. Mean values led to false conclusions regarding the question of the attained age of 18. Therefore, regression analysis should be used for the statistical assessment of data in dental age diagnostics.  相似文献

The main criterion for dental age estimation in living individuals is the mineralisation of third molars. However, the mineralisation of third molars can be completed before the forensically relevant age of 18 years has been attained. In a material of 1,198 orthopantomograms from 629 females and 569 males aged between 15 and 40 years, the radiographic visibility of the periodontal membrane of fully mineralised third molars was assessed according to stages 0, 1, 2 and 3. Stage 0 first appeared at the age of 17.2 years in females and at the age of 17.6 years in males. Stage 1 was first achieved by females between 18.9 and 20.0 years and by males between 20.1 and 20.2 years. The earliest appearance of stage 2 was between 22.5 and 23.1 years in females and at 22.3 years in males. The occurrence of stage 3 was first found between 24.6 and 25.2 years in females and between 25.4 and 26.2 years in males. If stage 1 is determined, it is, therefore, possible to prove that an individual has already attained the legally relevant age of 18 years. For stages 2 and 3, it can be stated beyond reasonable doubt that a person is over 21 years of age.  相似文献

One of the main criteria used in dental age diagnostics in living adolescents and young adults is assessment of the mineralisation stage of the third molars. In the case of Europid populations, it has been established that impaction status has an influence on the rate of mineralisation of the third molars. In view of this, a study was undertaken to determine whether the chronological process of wisdom tooth mineralisation is dependent upon impaction status in black Africans too. Orthopantomograms (553) of 437 male and 116 female black South Africans with verified birth dates in the age group between 10 and 26?years were studied. Mineralisation stage and impaction status were determined for all third molars. Statistical measures were calculated for the mandibular wisdom teeth at stages F, G and H and for the maxillary wisdom teeth at stage H in the male gender for both impacted and non-impacted third molars. It was ascertained that the minimum age in persons with impacted third molars, depending on the wisdom tooth observed, was 0.19–2.57?years higher than in those with non-impacted wisdom teeth. Test persons with impacted mandibular wisdom teeth at stage F or G were on average between 0.32 and 1.88?years older than those with non-impacted mandibular wisdom teeth. The 50?% probability values of impacted wisdom teeth at stage H were 1.85–3.31?years higher than those in non-impacted wisdom teeth. The conclusion was drawn that in male black Africans, impacted mandibular wisdom teeth mineralise more slowly than non-impacted lower third molars. The presence of impacted mandibular wisdom teeth in mineralisation stage H in male black Africans does not, however, furnish proof of completion of the 18th year of life beyond reasonable doubt.  相似文献

Age estimation by third molar mineralization has been developed for a period of time. Recent studies showed that impaction status has an influence on the rate of the third molar mineralization in Europids and male Black African populations. In this regard, the present study was conducted to determine whether the impaction status could delay the chronological process of third molar mineralization in northwestern Chinese population too. A total of 3,512 digital orthopantomograms of 1,255 male and 2,257 female northwestern Chinese subjects aged from 11 to 26 years were assessed based on the formation stages described by Demirjian et al. with two modifications. Mineralization stage and impaction status were determined for all third molars. Statistical measures were calculated at stages C–H in the male and female gender for both impacted and non-impacted third molars. It was ascertained that the mean ages with impacted third molars at stages C–H were 0.02–1.42 years higher in males and 0.04–1.52 years higher in females than those with non-impacted third molars, but statistical differences were only found at stages C, D, and E in males and stages D and E in females. The probabilities of being under 14, 16, or 18 years of age with non-impacted third molars were all higher than those with impacted third molars. The results prove that impacted third molars show significant slower mineralization than non-impacted ones at stages D and E in both males and females of northwestern Chinese population. It is recommended to consider the influence of impaction on the rate of third molar mineralization for dental age estimation.  相似文献

Radiographic evaluation of mineralization and eruption stages of third molars using dental panoramic radiographies can be an efficient tool for chronological age estimation in both forensic sciences and legal medicine. The third molar tooth is utilized for dental age estimation about the age span of 15–23?years because it represents the only tooth still in development. The aim of this study is to obtain and analyze data regarding third molar development and eruption in Turkish population for dental age estimation. A total of 744 dental panoramic radiographies of 394 female and 350 male subjects aged between 8 and 22?years were examined. Third molar development was determined according to the Nolla classification system, and eruption was assessed relative to the alveolar bone level. Mandibular and maxillary third molars were generally found at similar stages of development on both sides. Nolla stage 6 (completed crown calcification) was reached at around the age of 15 in both maxillary and mandibular third molars in both sexes. Alveolar emergence was at around the age of 16 in males and around age of 17 in females. Although third molars’ eruption shows greater variability than development of third molars, data which were obtained from this study about eruption of these teeth can be supportive to development data for age estimation.  相似文献

The question of whether an individual has reached the age of 18 is of crucial importance in forensic age estimation practice. In some countries, the age threshold of 21 years is relevant as well. A completed mineralization of third molars is not a sufficient criterion for a diagnosis of a minimum age of 18 years with the required probability. In a material of 1,198 orthopantomograms from 629 females and 569 males aged between 15 and 40 years, the visibility of the root pulp of fully mineralized lower third molars was evaluated according to stages 0, 1, 2, and 3. In females, stage 0 was first noticed at age 17.2 years, in males at age 17.6 years. In either sex, the earliest observation of stage 1 was between 21.0 and 22.4 years. Stage 2 was first achieved by males between 22.3 and 22.7 years, by females between 23.4 and 24.7 years. The occurrence of stage 3 was first found in both sexes between 25.1 and 25.9 years. These findings indicate that for stage 0, an age below 18 years cannot be excluded. However, for stage 1, the examined individual must be over 18 years of age and most probably over 21 years of age. For stages 2 and 3, the age can safely be stated to be over 21 years of age. This method may be a powerful tool for forensic dentists in age estimation in asylum and criminal proceedings.  相似文献

The development of new methods for age estimation has become with time an urgent issue because of the increasing immigration, in order to estimate accurately the age of those subjects who lack valid identity documents. Methods of age estimation are divided in skeletal and dental ones, and among the latter, Olze’s method is one of the most recent, since it was introduced in 2010 with the aim to identify the legal age of 18 and 21 years by evaluating the different stages of development of the periodontal ligament of the third molars with closed root apices. The present study aims at verifying the applicability of the method to the daily forensic practice, with special focus on the interobserver repeatability. Olze’s method was applied by three different observers (two physicians and one dentist without a specific training in Olze’s method) to 61 orthopantomograms from subjects of mixed ethnicity aged between 16 and 51 years. The analysis took into consideration the lower third molars. The results provided by the different observers were then compared in order to verify the interobserver error. Results showed that interobserver error varies between 43 and 57 % for the right lower third molar (M48) and between 23 and 49 % for the left lower third molar (M38). Chi-square test did not show significant differences according to the side of teeth and type of professional figure. The results prove that Olze’s method is not easy to apply when used by not adequately trained personnel, because of an intrinsic interobserver error. Since it is however a crucial method in age determination, it should be used only by experienced observers after an intensive and specific training.  相似文献

OBJECTIVES: To assess narrowing of the inferior dental canal in the lower third molar regions using computed tomography (CT) and to determine the value of radiographic markers on rotational panoramic radiographs in assessing the true relationships of the inferior dental canal. METHODS: Patients referred for CT assessment of impacted lower third molars were used in this study. The lower third molars were assessed using CT to determine the position and morphology of the inferior dental canal relative to the roots and the cortical plates. The radiographic markers on rotational panoramic radiographs were correlated with the CT findings when rotational panoramic radiographs were available. RESULTS: The patients referred had 202 lower third molars. Inferior positioning of the inferior dental canal was the most common location on CT. Narrowing of the inferior dental canal was found in relation to the lower third molars in 66.8% of cases. The chance of narrowing of the inferior dental canal as shown using CT increased when at least one of the radiographic markers, superimposition, narrowing, deviation or reduction in density was present on the rotational panoramic radiograph. Deviation of the inferior dental canal on rotational panoramic radiographs was found to be the most significant predictor of narrowing of the inferior dental canal and a close relationship to the roots, as shown in CT. CONCLUSIONS: Narrowing of the inferior dental canal is a common finding when impacted lower third molars are assessed using CT. On rotational panoramic radiographs deviation of the inferior dental canal is the best predictor of narrowing of the inferior dental canal and a close relationship to the roots.  相似文献

A criterion in dental age diagnostics is assessment of the eruption stage of the third molars. As the reliability of an age diagnosis is crucially dependent on the stage classification used, the most suitable system should be implemented for age estimation purposes. The objective of the present study was to compare the reliability of two classifications for radiological assessment of third molar eruption described by Olze et al. (Int J Legal Med 2007, 121:445?C448; Arch Kriminol 2012, 229:145?C153). To that end 216 conventional orthopantomograms of German males and females in the age group between 15 and 25 years were evaluated by two observers (one dentist with 20 years professional experience and one dentist with 6 years professional experience). The weighted kappa coefficient was used to determine intraobserver and interobserver agreement, whilst the eta coefficient was determined as a measure of the correlation between age and eruption stage. Based on the synopsis of the calculated kappa and eta coefficients it was concluded that both stage classifications for determining the developmental status of third molar eruption under study can, in principle, be recommended for age estimation purposes as they both command similar high levels of precision and accuracy. Stage classification II appears particularly suitable for inexperienced examiners and for assessing X-ray images on which the gingiva is not clearly definable.  相似文献

Assessment of third molar mineralization is a basic forensic odontological method available for age estimation in children and adolescents. Due to their protracted development into the late adolescence, their development has become the subject of interest and acquired great forensic importance. It is clearly evident in the literature that impaction factor has an influence on the rate of maturation of the third molars. In this regard, the present study was undertaken to test the relationship between impaction and its concomitant effect on the mineralization of the third molar and forensic age estimation. A total of 520 digital orthopantomograms (OPG) of 260 males and 260 females of south Indian origin were assessed retrospectively. Maturation of the third molars (I3M) was assessed using the method of Cameriere et al. (Int J Leg Med 122 (6):493–497, 2008). Impaction status was determined according to the position of lower right and left third molars in relation to the long axis of the second molars. Out of the total sample, 68.1% of lower left third molars and 71.2% of lower right molars were impacted. Statistical measures for I3M were calculated in the males and females for both impacted and non-impacted third molars. It was ascertained that the mean age and I3M values were higher in impacted third molars, indicating the slower rate of maturation than non-impacted ones. The sensitivity and specificity of the test (I3M < 0.08) was 96.2% and 72.4% for non-impacted third molars and 46.2% and 87% for impacted third molars, respectively. Our findings indicate that the cut-off value of I3M < 0.08 resulted in greater number of age misclassifications (false negatives) for impacted molars, highlighting the need for new cut-off value of I3M to meet the legal standards. It was concluded that the mineralization of impacted third molars was slower compared to non-impacted ones in south Indian males and females. Further studies are warranted to validate these findings among a larger and more diverse sample.

BackgroundIn recent years, a noticeable increase in forensic age estimations of living individuals has been observed. Radiologic assessment of the mineralisation stage of third molars is of particular importance, with regard to the relevant age group. To attain a referral database and regression equations for dental age estimation of unaccompanied minors in an Iranian population was the goal of this study. Moreover, determination was made concerning the probability of an individual being over the age of 18 in case of full third molar(s) development.Materials and methodsUsing the scoring system of Gleiser and Hunt, modified by Köhler, an investigation of a cross-sectional sample of 1274 orthopantomograms of 885 females and 389 males aged between 15 and 22 years was carried out. Using kappa statistics, intra-observer reliability was tested. With Spearman correlation coefficient, correlation between the scores of all four wisdom teeth, was evaluated. We also carried out the Wilcoxon signed-rank test on asymmetry and calculated the regression formulae.ResultsA strong intra-observer agreement was displayed by the kappa value. No significant difference (p-value for upper and lower jaws were 0.07 and 0.59, respectively) was discovered by Wilcoxon signed-rank test for left and right asymmetry. The developmental stage of upper right and upper left third molars yielded the greatest correlation coefficient. The probability of an individual being over the age of 18 is 95.6% for males and 100.0% for females in case four fully developed third molars are present. Taking into consideration gender, location and number of wisdom teeth, regression formulae were arrived at.ConclusionUse of population-specific standards is recommended as a means of improving the accuracy of forensic age estimates based on third molars mineralisation. To obtain more exact regression formulae, wider age range studies are recommended.  相似文献

Forensic age estimation of living subjects has become increasingly important in recent years. One main criterion for dental age estimation in the relevant age group is the evaluation of third molar mineralization. In the present study, we determined the stages of third molar mineralization in 347 female and 258 male First Nations people of Canada aged 11 to 29 years based on radiological evidence from 605 conventional orthopantomograms. The results presented here provide useful data on the mineralization stages of third molars that can be used for forensic estimation of the minimum and most probable ages including the range of scatter of investigated persons.  相似文献

In addition to routine dental caries and gingival inflammation, 18 athletes had dental problems that could potentially interfere with the competitive season: retained root tips, abscessed teeth, pulpally exposed carious teeth, and seriously impacted third molars. From a forensic perspective, attaining the ages of 12, 15 and 18 is particularly important with respect to the Turkish Penal Code (TPC). The aim of this study is to obtain data regarding third molar development in the Turkish population for age estimation and to investigate the contribution of third molar development to age estimation in relation to the TPC. The study was applied with panoramic radiography on 379 male and 405 female subjects. The mineralization status of the third molars were evaluated on the basis of Demirjian’s classification system. It was determined that ‘0’ and ‘A’ stages for under-12 years of age, ‘B’ and ‘C’ stages for under-15, ‘D’ stage for under-18, ‘E’ and ‘F’ stages for over-12, ‘G’ stage for over-15 and ‘H’ stage for over-18 were specific stages for both genders. Specific stages that are detected according to the developmental stages of the third molars were not found to be adequately precise for distinguishing legal age thresholds. However, this study shows that matured third molars (reaching H stage) are indicative of an individual’s being over 18 years of age for both genders in Turkey.  相似文献

Predicting 18 years of age can be crucial in forensic contexts. The third molar is the only tooth developing during this chronological period and has been used to estimate minority/majority status (</≥18 years). Conventionally, Demirjian’s grading has been used to assess third molar development although the method was not originally intended for evaluating this tooth. Demirjian incorporated third molar assessment in a recent modification and replaced the alphabetical grading (A to H) with a numerical scale (0 to 9). The new grading system is untested on third molars and this study assessed the tooth’s development on orthopantomograms of 221 Indian subjects (68 males, 153 females; age range 15–21 years). The tendency to correctly determine majority/minority status was assessed using three different statistical approaches, viz. traditional regression analysis, logistic regression analysis and Bayesian prediction. The sample was divided in to a reference sample of 180 cases and a test group of 41 subjects. All three statistical methods correctly predicted an individual as being </≥18 years in 73.2% of test subjects. Their ability to correctly identify a minor/juvenile was higher (85.7%) than their capacity to properly identify a major/adult (60%). Using the revised grading system, and irrespective of the statistical method applied, over one quarter of Indian subjects requiring identification as a major/minor were categorised in to the wrong age group. This level of accuracy may be inadequate for courts of law to rule with sufficient levels of certainty about the juvenile/adult status of an individual using third molar development.  相似文献

Mineralization and eruption of third molars are the main criteria for dental age estimation in living adolescents. For further diversification of the spectrum of characteristics after completion of third molar development, it was tested whether it is possible to verify that the forensically relevant age of 18 years has been reached by means of radiographic evaluation of the extension of cementum apposition. For this purpose 1,299 conventional orthopantomograms of 650 female and 649 male German subjects aged 15?C40 years were examined. Extension of cementum apposition of mandibular premolars was determined on the basis of a three-stage classification. Stage 1 (early apical cementum apposition) was reached by women at a minimum age of 16.1?C16.6 years and by men at a minimum age of 15.5?C19.9 years depending on the tooth examined. Stage 2 (clearly visible cementum apposition extending beyond the apex) was reached by female subjects at a minimum age of 20.2?C25.1 years and by male subjects at a minimum age of 19.3?C26.9 years depending on the tooth examined. Therefore if stage 2 of cementum apposition is confirmed it is possible to verify for both sexes that the age of 18 years has been reached.  相似文献

In forensic medicine and many other fields, age estimation by the use of teeth is of great importance for the purpose of individual identification. In the past, however, age estimation based on the second molar mineralization was scarcely performed. In this study, a total of 1657 panoramic radiographs taken from 834 males and 823 females of northern Chinese origin in the age bracket 5 to 25 years were assessed. The mineralization status of the second molars was determined using the classification described by Demirjian et al. Results showed that the left and right, as well as maxillary and mandibular second molars were generally at similar stages of mineralization. The maxillary left second molars (27) at stage D, mandibular left second molars (37) at stages C, D, F, and G, and mandibular right second molars (47) at stages D, F, and G showed a significantly lower average age in female subjects than in male subjects. In males, fully developed second molars first appeared with 12 years of age; in females, stage H occurred with 11 years at the earliest. One male individual and one female individual with second molars showing stage G were 23 years old. It was concluded that second molars showing stage H do not exclude an age under 14 years and that second molars showing stage G do not exclude an age above 18 years.

Dental age estimation is routinely used in human identification, in both the living and the dead. Teeth follow a predictable pattern of eruption and maturation that can be assessed radiographically in the living, allowing experts to estimate an individual’s age. Third molars develop much later than the other permanent teeth, and can be used to estimate age of individuals nearing the age of maturity. Ability of third molar maturation when graded using Demirjian’s method in ascertaining whether an individual has attained the medicolegal ages of 16 and 18 years has not yet been verified. The present study involved assessing the third molar maturation of 220 individuals of a training set using Demirjian’s scoring system, generating regression models to estimate age using the third molar maturity scores, applying these models on a test set of 40 individuals, and calculating the mean difference (MD) and the mean absolute error (MAE) between the chronological and estimated ages of the test set participants. It was observed that the lowest mean difference and mean absolute error was observed for the maturation of the left mandibular third molar (MD = 1.05 years, MAE = 2.09 years). Boxplots generated in the study report that the third molar maturation stages given by Demirjian can identify whether an individual has attained the medicolegally significant ages of 16 and 18 years of age.  相似文献
