Since the first resin-bonded fixed partial denture was described, this adhesive technique for splinting mobile teeth has developed into a conservative method for replacing missing teeth. Heat-pressed all-ceramic materials have been introduced for the fabrication of 3-unit fixed partial dentures in the anterior and premolar region. This clinical report describes the replacement of a conventional partial coverage metal-ceramic resin-bonded fixed partial denture with an all-ceramic resin-bonded fixed partial denture in the anterior maxillary area.  相似文献

One of the problems with anterior maxillary resin-bonded fixed partial dentures is anterior-guided occlusion. An anterior maxillary resin-bonded fixed partial denture was fabricated. The reduction of the lingual surface of the abutment was restricted to the nonocclusal surface area for anterior guidance. The patient's original mandibular movement did not change after the denture was seated. The denture has been functioning satisfactorily without any problems for more than 72 months. This article emphasizes the concept of fabricating an anterior maxillary resin-bonded fixed partial denture without reduction of the occlusal surface area for anterior guidance.  相似文献

In the maxillary anterior region, reconstruction of the localized alveolar ridge defect is very important in enhancing the esthetics of fixed partial denture. A 40-year-old female patient presented with a chief complaint of the inconvenience and unesthetic problem of 3-unit maxillary anterior prosthesis due to alveolar ridge resorption. After removal of old prosthesis, intraoral examination revealed moderate (buccolingually 4 mm) ridge deficiency in missing tooth region, leading to the diagnosis of Class I alveolar ridge defect. One of the reconstruction techniques to overcome this problem might be a technique that combines two types of soft tissue augmentation techniques. The purpose of this paper was to demonstrate the new combined technique of roll flap and combination onlay-interpositional graft utilized to acquire sufficient dimension of recipient area by one time of operation and to present the esthetic improvement of fixed partial denture by using this procedure in case of maxillary anterior localized ridge defect.  相似文献

The loss of an anterior tooth because of avulsion has been reported in up to 3% of dental injuries. Management alternatives, such as implant-supported restorations and a fixed partial denture, are contraindicated in growing patients because of the continuous growth of the alveolar process. At the same time, orthodontic treatment for gap closure will result in asymmetry and will require adjustment of the adjacent healthy teeth. Therefore, restoring a missing tooth imposes a treatment challenge, especially in children and young adults. Tooth autotransplantation is a treatment modality with high reported survival and success rates that overcome these mentioned limitations. It might also help to preserve the alveolar bone and the soft tissues. This case report describes a multidisciplinary approach for managing a 13-year-old boy who lost his right maxillary central incisor because of a fall. Management included autotransplantation of the left maxillary second premolar to the site of the lost right maxillary central incisor, management of external inflammatory resorption with an endodontic treatment, orthodontic treatment, and aesthetic restoration of the area with composite resin crowns that can be adapted to the expected changes of the jaws during the craniofacial growing period. At the 6-year follow-up, the teeth demonstrated a positive outcome.  相似文献

A dental arch with diastemata and missing lateral incisors represents a challenge because of discrepancies in tooth size. If an implant-supported prosthesis is not possible or not selected as a treatment option, a modified resin-bonded fixed partial denture may be a treatment option to eliminate the esthetic problems of conventional fixed partial dentures. Fifteen edentulous patients with complaints regarding denture comfort and/or function were treated with the transmandibular implant. All patients were restored with conventional maxillary dentures opposed by implant-supported removable prostheses. Two to 4 years after surgery, these patients were evaluated for vertical and horizontal maxillary bone loss with a radiographic analysis developed by the authors. With this technique, attention was focused on vertical alveolar ridge resorption in the anterior maxilla. Although the sample size was small, the findings from this study indicate that vertical bone loss in the anterior maxilla does occur when a maxillary denture is opposed by an implant-supported overdenture. Comparison of these results with a previous study that evaluated anterior maxillary resorption when a complete maxillary denture opposed natural mandibular anterior teeth and a distal extension removable partial denture demonstrated no statistically significant difference.  相似文献

Three treatment options exist for the replacement of congenitally missing lateral incisors: canine substitution, a tooth-supported restoration, or a single-tooth implant. Selecting the appropriate treatment option depends on the malocclusion, the anterior relationship, specific space requirements, and the condition of the adjacent teeth. The ideal treatment is the most conservative alternative that satisfies individual esthetic and functional requirements. This article closely examines the three options when replacing a missing lateral incisor with a tooth-supported restoration. These options are a resin-bonded fixed partial denture, a cantilevered fixed partial denture, and a conventional full-coverage fixed partial denture. The specific criteria that must be evaluated for each option is addressed to illustrate the importance of interdisciplinary treatment planning to achieve optimal esthetics and long-term predictability. PatientThis report describes the longevity of a resin-bonded fixed partial denture (RBFPD). This denture was seated on the maxillary anterior teeth with minimal tooth preparation. The RBFPD was cast from a silver–palladium alloy (Castwell M.C. 12), and the pontic was veneered with an indirect composite material (Estenia). The retainers were primed with a metal conditioner (V-Primer) and seated with a tri-n-butylborane initiated adhesive resin (Super-Bond C&B). After an observation period of eight years, a fracture occurred in the incisal edge of the central incisor abutment. The fractured area was restored with light-polymerizing composite resin and the anterior guidance was re-adjusted.DiscussionRBFPD abutment teeth with deep vertical overlap should be carefully prepared to avoid abutment tooth fracture.ConclusionThe clinical performance of the RBFPD made from a silver–palladium–copper–gold alloy was sufficient when seated with tri-n-butylborane initiated adhesive resin after surface modification using vinyl-thiol primer.Clinical significanceThe use of vinyl-thiol primer and tri-n-butylborane initiated adhesive resin is a clinically reliable bonding system for seating resin-bonded fixed partial denture (RBFPD) made from a silver–palladium–copper–gold alloy. The RBFPD, based on a reliable bonding system, can clinically function for a long time, even if the vertical overlap of the abutment teeth is excessive.  相似文献

目的 :评价微型固位钉和金属翼板辅助固位的树脂粘结固定义齿修复单个中间缺失的前牙或前磨牙临床效果。方法 :对 2 1例单个上颌前牙或前磨牙中间缺失患者进行无冠粘结固定义齿 (crownlessbridgeworks ,CBW )系统修复 ,在修复后即刻、1个月及 2年后对修复体的固位、边缘密合性、美观、口腔卫生状况、牙体牙周健康等项目进行评价。同时 ,对 2 0例单个中间牙缺失患者采用传统的 3单位全冠固位固定义齿方法修复并给以同期评价。 2组患者各评价项目的满意率之间采用卡方检验进行统计学比较。结果 :2年后 ,2组固定桥修复均无脱落。所有临床评价项目均为满意或可接受 ,无继发龋发生。 2组之间各项目的Fisher精确校正或连续校正卡方检验没有统计学上的显著性差异 (P >0 .0 5 )。结论 :CBW树脂粘结固定修复法具有磨除牙体组织少、无需麻醉、对牙体牙周组织影响小、易于掌握等优点。在本研究所观察的时间限度内 ,可作为传统固定修复的替代方法来修复中间缺失的单个前牙或前磨牙 ,能达到较好的临床效果  相似文献

PURPOSE: Is to evaluate the antagonistic, maxillary ridge resorption for different prosthetic modalities mainly mucosa-supported mandibular complete overdentures, combined mucosa implant-supported mandibular complete overdenture, and lower conventional complete dentures. MATERIALS AND METHODS: Fifteen completely edentulous patients were divided into 3 groups; 5 patients each. Group I patients received maxillary conventional denture and mandibular overdentures retained by magnet attachment on 2 implants (mainly mucosa-supported overdenture). Group II patients received maxillary conventional denture and mandibular overdentures retained by bar attachment on 2 implants (combined mucosa implant-supported overdenture). Group III patients received upper and lower conventional complete denture. All patients were evaluated clinically and radiographically immediately after insertion and after 1 and 2 years. Panoramic radiographic film was used to evaluate the antagonistic maxillary ridge resorption. RESULTS: Indicated a more pronounced annual bone resorption of the antagonistic maxillary ridge after 2 years in patients with conventional complete denture wearers when compared with patients with combined mucosa implant-supported mandibular complete overdenture and mainly mucosa-supported mandibular complete overdenture groups. CONCLUSION: Using a combined mucosa implant-supported mandibular complete overdenture the amount of antagonistic maxillary alveolar bone resorption increases when compared with the mainly mucosa-supported complete overdenture.  相似文献

改良唇-牙-牙槽嵴上颌全口种植义齿分类法是根据牙槽嵴宽度和高度将上颌无牙颌分为3类。Ⅰ类,牙槽嵴宽度高度均适宜,适合种植固定义齿修复;Ⅱ类,牙槽嵴宽度对于种植体足够但是对唇颊支撑不足,适合于固定活动联合修复;Ⅲ类,牙槽嵴高度或宽度不足以支撑种植体植入,需要植骨,或采用穿颧种植体等特殊方式种植,或者采用传统全口义齿修复。第Ⅰ类根据牙槽嵴高度不同又分为2个亚类:第Ⅰ类1亚类,牙槽嵴高度完全没有丧失,适合冠桥式义齿修复;第Ⅰ类2亚类,牙槽嵴高度有部分丧失,适合复合式义齿修复。本设计分类旨在帮助术者在术前明确患者是否适合种植,以及适合采用种植固定义齿还是种植活动义齿修复,有利于更好地选择适应证,减少并发症,获得更好的长期效果。  相似文献

The prevalence of symptoms associated with "combination syndrome" were documented in 150 maxillary complete denture wearers. The five alveolar ridge changes that are most consequential to denture wearing and most difficult to correct surgically were found in 7% of the population studied. However, these changes were found in 24% of patients who have natural mandibular anterior teeth opposing complete maxillary dentures. This rate did not differ significantly between patients who do and do not wear a mandibular removable partial denture.  相似文献

When a resin-bonded fixed partial denture is planned in the maxillary anterior region, special attention must be paid to occlusal diagnosis. Specific instrumental diagnosis is necessary to define the functionally important factors: the anterior teeth, the occlusal plane, the temporomandibular joint, and their three-dimensional relation. The optimal treatment in an anterior-guided occlusion is a complete integration of the retainers in the given tooth form without altering the original inclination of the palatal guiding surfaces of the anterior teeth. In many cases, preprosthetic orthodontics is indicated because of functional and esthetic reasons. The objective is to achieve an anterior tooth position that allows the retainers of a resin-bonded fixed partial denture to be incorporated with the designed inclination of the guiding surfaces, using noninvasive preparation techniques. Preprosthetic orthodontic treatment planning, possible problems, and limiting factors as well as the subsequent prosthetic reconstruction have been described in detail. The interdisciplinary treatment has to be planned individually for each patient, depending on the functional diagnosis and the clinical and anatomic situation.  相似文献

Tooth replacement in the maxillary anterior region is especially difficult when the loss includes significant amounts of the residual ridge and the soft tissue. Several techniques are available, such as dental implants or fixed partial denture, and bone and gingival grafts or gingival prostheses, respectively. This article showed a clinical case of an elderly who was treated with a collarless metal-ceramic fixed partial denture and acrylic removable gingival prosthesis to recover the esthetics in the maxillary anterior region. The association of a metal-ceramic fixed denture and gingival prosthesis was an excellent alternative in cases when surgical procedures are contraindicated.  相似文献

The restoration of the edentulous atrophic anterior maxillary ridge has proved difficult. The absence of both gingival and bony alveolar tissue contributes to these problems. Numerous techniques to address these issues have been described. We report a case of a patient who sustained multiple facial injuries as a consequence of an accident, including the loss of the maxillary incisor teeth and associated alveolus. This patient underwent fixed implant supported prosthetic rehabilitation, following the use of distraction osteogenesis in the reconstruction of the edentulous atrophic anterior maxillary ridge.  相似文献
