Renal artery stenosis (RAS) is a treatable cause of hypertension and renal failure for which no ideal screening technique is currently available. We evaluated the use of dynamic gadolinium-enhanced magnetic resonance angiography (MRA) for the diagnosis of RAS. Sixty-two patients with secondary hypertension were enrolled in the study. All patients had conventional renal angiography and gadolinium enhanced MRA. The sequence used was a 3D FMP SPGR sequence with the following parameters (TR: 26 ms, TE: 6.9 ms, flip angle 40 °, field of view 36 × 36 cm, matrix 246 × 256, 1 excitation). Gadolinium 0.3 mmol/kg was administered and 60 1.5-mm-thick partitions were obtained over a duration of 3.5 min. The MRA images were then compared with conventional digital subtraction angiography (DSA) images. Conventional DSA demonstrated 138 renal arteries, whereas gadolinium-enhanced MRA demonstrated 129 (93 %). Twenty-one renal artery stenoses and four occluded arteries were seen at conventional DSA. Gadolinium-enhanced MRA had a sensitivity of 88 %, specificity of 98 %, accuracy of 96 %, positive predictive value of 92 % and negative predictive value of 97 % when compared with conventional DSA. Gadolinium-enhanced MRA is an accurate technique for identifying patients with RAS. It is less sensitive in picking up accessory renal arteries. MRI was performed on 26 patients with tuberculous meningitis, with particular reference to document the cranial nerve abnormalitics. MR angiography (MRA) was performed in 20 of the patients. Meningeal enhancement in the basal cisterns or over the convexity of brain was seen in all patients; two show ependymal enhancement. Tuberculomas, single (3), multiple (12) or miliary (2) were detected in 17 patients. Of the 9 patients with cranial nerve palsies, 7 showed contrast enhancement with or without thickening of the involved nerve. Abnormality signal intensity of the involved nerve was seen on proton density and T2-weighted images in one of these patients. MRA revealed focal arterial narrowing in 10 patients, the vessels commonly affected being the terminal segment of the internal carotid artery and the proximal segments of the middle and anterior cerebral arteries. One patient also had a small aneurysm of the proximal middle cerebral artery. Infarcts, haemorrhagic (8) or bland (6), were detected in 14 patients; most were the basal ganglia and internal capsules, large middle or anterior cerebral arterial territory infarcts being seen in only two cases.  相似文献

The objective of this study was to evaluate quantitatively and qualitatively the effect of image subtraction on the image quality of three-dimensional (3D) gadolinium-enhanced MR angiograms of the renal arteries. Breath-hold 3D gadolinium MR angiography (MRA) as well as conventional contrast angiography of the renal arteries was performed on 20 patients with suspected renovascular hypertension. MR angiograms were acquired before and during dynamic infusion of gadolinium-diethylenetriamine pentaacetic acid (Gd-DTPA). Contrast-enhanced images were compared with images that had undergone voxel-by-voxel signal intensity subtraction of contrast-enhanced data from precontrast data. One false positive finding for significant renal artery stenosis was recorded with MRA using conventional angiography as the gold standard. Image subtraction did not alter the diagnosis at MRA in any case. The mean contrast-to-noise ratio (CNR) was significantly higher (P < .05) on the subtraction MR angiograms compared to the nonsubtracted MR angiograms. There was no significant difference in the signal-to-noise ratio (SNR). Qualitative analysis revealed a significant improvement in image quality after image subtraction with respect to visualization of the distal renal arteries. In conclusion, image subtraction improves the quality of renal MRA in terms of both CNR and visualization of the distal renal arteries.  相似文献

目的:研究大脑中动脉闭塞后侧支供血的3DMRA表现。方法:回顾性分析大脑中动脉供血区脑梗死患者75例,均行MRI和3DTOFMRA,着重观察MRA中的侧支代偿供血情况。结果:75例患者3DTOFMRA均发现大脑中动脉起始段(M1)闭塞,同侧的大脑后动脉颞下后支及距状裂和顶枕动脉段出现增粗、延长、分支增多的表现,而多数患者同侧的大脑前动脉走行方向、管径粗细、分支情况则无明显变化。结论:大脑中动脉M1段闭塞后,MRA示同侧大脑后动脉主干及分支有显著改变,说明闭塞区的代偿血供是以大脑后动脉通过柔脑膜毛细血管网的侧支循环为主。  相似文献

The purpose of this study was to evaluate accuracy of dynamic gadolinium-enhanced MR angiography (MRA) of the celiac, superior, and inferior mesenteric arteries in patients with suspected mesenteric ischemia compared with catheter angiography or surgery. Sixty-five patients with suspected mesenteric ischemia underwent three-dimensional spoiled gradient-recalled acquisition in the steady state (GRASS) gadolinium-enhanced MRA. Correlative studies were performed on 14 patients, catheter angiography alone was performed on 12 patients, and surgery alone was performed on two patients. Six patients had mesenteric ischemia. In all patients, the celiac artery (CA) and superior mesenteric artery (SMA) were seen well enough to evaluate; however, the inferior mesenteric artery (IMA) could be evaluated in only 9 of the 14 patients. MRA showed severe stenosis (>75%) or occlusion of the celiac axis in seven patients, of the SMA in six patients, and of the IMA in four patients. The overall sensitivity and specificity were 100% and 95%, respectively, compared with catheter angiography and surgery. The two errors were caused by overgrading the severity of IMA disease. Three-dimensional gadolinium-enhanced MRA can accurately demonstrate the origins of the CA and SMA and is useful in evaluation of patients with suspected mesenteric ischemia.  相似文献

We prospectively studied 15 patients to assess 2D time-of-flight (TOF) magnetic resonance angiography (MRA) with concatenated saturation bands for determining the direction of intracranial blood flow. This MRA sequence was compared to T2-weighted spin-echo MRI, 3D-TOF MRA, and intra-arterial angiography (IAA) as regards demonstration of vessels and determination of the direction of flow in the circle of Willis and its branches. The 2D-TOF MRA sequence demonstrated flow in 98.5 % vessel segments identified on IAA, 3D-TOF demonstrating 92 % and spin-echo images 77 %. The direction of flow shown on the 2D-TOF sequence was correct in 94 % when compared to conventional angiography, the remaining six segments not demonstrating flow. In ten patients, the flow abnormalities demonstrated by this MRA technique provided clinical information similar to that of conventional angiography in nine, but it was incomplete in three, and misleading in one. Slow retrograde flow in ophthalmic artery collaterals and differentiation of arteries and veins presented some problems. 2D-TOF MRA with concatenated saturation bands provides flow direction information using widely available, easily applicable TOF techniques, and can be a useful adjunct to MRI and MRA if information on flow direction is needed. OBJECTIVE: Our purpose was to quantitatively and qualitatively compare 3D intraarterial (IA) gadolinium-enhanced MR angiography (IA MRA) versus the standard of reference of MR angiography, 3D IV gadolinium-enhanced MR angiography (IV MRA), in patients with peripheral arterial occlusive disease (PAOD) for use during catheter-based MR-guided endovascular interventions. CONCLUSION: IA MRA provides image quality of the infrainguinal arteries in PAOD patients comparable to IV MRA with a significantly improved assessment of the infrapopliteal arteries due to reduced venous contamination. Further benefits of IA MRA include usage of only very low doses of gadolinium and simplified bolus timing.  相似文献

The sensitivities and specificities of three-dimensional time-of-flight MR angiography (3D-TOF MRA) and 3D digital subtraction angiography (3D-DSA) were compared for evaluation of cerebral aneurysms after endosaccular packing with Guglielmi detachable coils (GDCs). Thirty-three patients with 33 aneurysms were included in this prospective study. 3D-TOF MRA and 3D-DSA were performed in the same week on all patients. Maximal intensity projection (MIP) and 3D reconstructed MRA images were compared with 3D-DSA images. The diameters of residual/recurrent aneurysms detected on 3D-DSA were calculated on a workstation. In 3 (9%) of 33 aneurysms, 3D-TOF MRA did not provide reliable information due to significant susceptibility artifacts on MRA. The sensitivity and specificity rates of MRA were 72.7 and 90.9%, respectively, for the diagnosis of residual/recurrent aneurysm. The diameters of residual/recurrent aneurysms that could not be detected by MRA were significantly smaller than those of detected aneurysms (mean 1.1 vs mean 2.3 mm). In one aneurysm of the anterior communicating artery (ACoA), the relationship between the residual aneurysm and the ACoA was more evident on MRA than DSA images. MRA can detect the recurrent/residual lumen of aneurysms treated with GDCs of up to at least 1.8 mm in diameter. 3D-TOF MRA is useful for follow-up of intracranial aneurysms treated with GDCs, and could partly replace DSA.  相似文献

 目的探讨MRI、MRA对烟雾病(Moyamoya病)的诊断价值.方法对11例Moyamoya病患者行MRI和MRA检查,MRI包括横轴位和矢状位T1WI、T2WI、FLAIR;MRA采用3D TOF法,3例行增强MRA.结果MRI表现为:(1)Moyamoya血管:双侧3例,单侧8例;(2)脑梗死和脑软化灶11例;(3)局部脑萎缩3例.MRA表现为3例双侧颈内动脉狭窄,双侧大脑中、前动脉闭塞,大脑后动脉形成异常血管网;6例右侧颈内动脉、大脑中动脉狭窄;2例左侧颈内动脉、大脑中动脉狭窄.结论MRI能良好的显示脑内病变,MRA能较完整的显示异常血管,MRI与MRA相结合可作为烟雾病诊断的首选检查方法.  相似文献

Contrast-enhanced MR angiography of intracranial giant aneurysms   总被引:8,自引:0,他引:8  
BACKGROUND AND PURPOSE: Intravoxel phase dispersion and flow saturation often prevent adequate depiction of intracranial giant aneurysms on 3D time-of-flight (3D-TOF) MR angiography (MRA). Additional diagnostic difficulties may arise from T1 contamination artifact of an associated blood clot. Our aim was to assess whether contrast-enhanced MRA could improve the evaluation of giant aneurysms and to compare two different types of contrast-enhanced MRA. METHODS: We studied 11 aneurysms in 10 patients (age range, 31-77 years) with giant aneurysms of the anterior (n = 9) and posterior (n = 2) cerebral circulation by comparing 3D-TOF, first-pass dynamic contrast-enhanced MRA, and steady-state contrast-enhanced 3D-TOF sequences. Additional comparison with digital subtraction angiography (DSA) was performed in eight aneurysms. RESULTS: In nine of 11 aneurysms, 3D-TOF did not adequately show the lumen and exiting vessels. Contrast-enhanced 3D-TOF and dynamic contrast-enhanced MRA showed the aneurysm sac and exiting vessels in all of these cases. Dynamic contrast-enhanced MRA showed a better intravascular contrast than did contrast-enhanced 3D-TOF, which led to better delineation of the aneurysms. T1 contamination artifact from intra- or extraluminal blood clot was evident on the 3D-TOF images in four cases. The artifact was less marked on the contrast-enhanced 3D-TOF image and was completely eliminated on the dynamic contrast-enhanced MRA image by subtraction of precontrast images. The diagnostic information provided by dynamic contrast-enhanced MRA was comparable to that provided by DSA. CONCLUSION: Precontrast 3D-TOF is inadequate for the assessment of giant cerebral aneurysms. Both contrast-enhanced 3D-TOF and dynamic contrast-enhanced MRA reliably show the aneurysm sac and connected vessels. Dynamic MRA provides a superior contrast between flow and background and eliminates T1 contamination artifact. It should therefore be considered as the MRA sequence of choice.  相似文献

目的探讨三维T1加权序列可变反转角度快速自旋回波序列(3D T1-SPACE)结合三维时间飞跃法MR血管成像(3D-TOF MRA)在颅内动脉瘤支架辅助弹簧圈栓塞术后患者随访中的应用价值。方法前瞻性收集2017年12月至2018年10月河南省人民医院收治的25例因颅内动脉瘤接受支架辅助弹簧圈栓塞术的患者,均为宽颈动脉瘤。术后6~10个月所有患者均行3D-TOF MRA、3D T1-SPACE序列MR扫描和DSA检查。分别使用3D-TOF MRA和DSA以Raymond分级法评价瘤腔栓塞情况,使用3D-TOF MRA和3D T1-SPACE序列以4分法评价载瘤动脉支架内管腔显示情况。采用配对设计的Wilcoxon秩和检验比较动脉瘤栓塞程度分级和支架内管腔显示情况。以DSA为金标准,计算3D-TOF MRA评估动脉瘤残留的特异度及准确率。结果术后6~10个月随访,对于动脉瘤闭塞情况,DSA造影显示Raymond 1级23例,2级1例,3级1例;3D-TOF MRA 1级21例,2级3例,3级1例;差异无统计学意义(Z=-0.557,P=0.577),其中有4例患者两种评估方法结果不一致。对于载瘤动脉支架内管腔情况的显示,3D-TOF MRA评分3分14例,2分8例,1分3例;3D T1-SPACE序列25例均为4分,3D T1-SPACE优于3D-TOF MRA(Z=-4.484,P<0.001)。以DSA为金标准,3D-TOF MRA结合原图像评估动脉瘤栓塞情况的特异度为86.9%(20/23),准确率为84.0%(21/25)。结论3.0 T MR 3D T1-SPACE序列可清晰显示支架内管腔,能准确判断支架内血管的通畅情况,3D-TOF MRA可充分评估动脉瘤瘤腔有无残留。将上述两种MRI血管成像技术相结合,可用于动脉瘤支架辅助栓塞术后的随访。  相似文献

RATIONALE AND OBJECTIVES: Catheter-directed intraarterial (IA) injections of gadolinium contrast agents may be used during endovascular interventions with magnetic resonance (MR) imaging guidance. Injection protocols require further validation. Using a flow phantom and swine, the authors aimed to (a) measure the optimal arterial gadolinium concentration ([Gd]) required for MR angiography and (b) validate a proposed IA injection protocol for gadolinium-enhanced MR angiography. MATERIALS AND METHODS: For in vitro experiments, the authors placed a catheter in the aorta of an aorto-renal-iliac flow phantom. Injected [Gd], injection rates, and aortic blood flow rates were varied independently for 36 separate IA gadolinium injections. The authors performed 2D and 3D MR angiography with a fast spoiled gradient-recalled echo sequence. For subsequent in vivo experiments, they selectively placed catheters within the aorta, renal artery, or common iliac artery of three pigs. Injection rate and injected [Gd] were varied. The authors performed 32 separate IA gadolinium injections for 2D MR angiography. Signal-to-noise ratios (SNRs) were compared for the various combinations of injection rate and injected [Gd]. RESULTS: In vitro, an arterial [Gd] of 2%-4% produced an optimal SNR for 2D MR angiography, and 3%-5% was best for 3D MR angiography. In swine, an arterial [Gd] of 1%-4% produced an optimal SNR. In the phantom and swine experiments, SNR was maintained at higher injection rates by inversely varying the injected [Gd]. CONCLUSION: Dilute arterial [Gd] is required for optimal IA gadolinium-enhanced MR angiography. To maintain an optimal SNR, injection rates and injected [Gd] should be varied inversely. 目的:探讨MRI和MRA检查在脑AVM临床诊断中的价值。方法:46例脑动静脉畸形作了常规MRI和MR血管造影,MRI采用SET1、T2加权成像,MRA采用三维时飞跃法。结果:MRI准确显示了46例AVM的瘤巢,11例可见亚急性出血灶,5例可见含铁血黄素沉积,23例病灶区组织软化或萎缩,4例有占位效应。3D-TOF RMA显示41例AVM供血动脉、23例引流静脉。5例加做增强3D-TOF及6例加做2D-TOF后,引流静脉显示明显改善。结论:MRI和MRA结合应用能对脑血管畸形作出较准确的诊断,为临床治疗提供所需的基本信息并对治疗效果作出客观的评价。  相似文献

We compared dynamic contrast-enhanced three-dimensional time of flight (3DTOF) magnetic resonance angiography (MRA) with two-dimensional time of flight (2DTOF) MRA with cardiac compensated fast gradient recalled echo (C-MON) and conventional angiography (CA) when it was available. C-MON re-orders the normal data acquisition to minimize ghosting artifacts generated by pulsatile flow. The initial phase of the study involved optimization of parameters and comparison C-MON with no C=MON in eight patients and volunteers. The final phase of the study involved 53 patients who were imaged with contrast-enhanced 3DTOF MRA and 2DTOF MRA with C-MON. Thirty of these patients also had CA. In the initial phase, 2DTOF MRA with C-MON was found to be equal (n = 3) or superior (n = 5) to 2DTOF without C-MON. In the final phase, the agreement among all imaging modalities varied from substantial to almost perfect (Cohen's K = .6-.83). The lowest agreement was using 2DTOF to evaluate the external iliac segments. The among suggested treatments varied from substantial to almost perfect for all imaging modalities (Cohen's K = .73–93). The diagnostic efficacies of 2DTOF with C-MON and contrast-enhanced 3DTOF were high overall, with the lowest value being a specificity of 63% for one reader in the evaluation of an external iliac segment using 2DTOF. In summary, 2DTOF with C-MON helped to eliminate artifacts due to pulsatility in the iliac arterial segments. In our experience, both dynamic constrast-enhanced 3DTOF MRA and 2DTOF MRA with C-MON performed well in the evaluation of the iliac arteries. Both studies have high interobserver agreement and high diagnostic efficacy. Contrast-enhanced 3DTOF MRA should be reserved for situations in which the iliac vessels are extremely tortuous or occluded or the external iliac segments are poorly seen.  相似文献

Renal artery MR angiography has now emerged as a safe, accurate approach to renal arteriography. A comprehensive examination, including both three-dimensional (3D) dynamic gadolinium-enhanced and 3D phase contrast MRA techniques, allows evaluation of both the aorta-renal and splanchnic arterial anatomy as well as the hemodynamic significance of any stenoses identified. The 3D gadolinium-enhanced MRA technique produces a contrast arteriogram but without risks of iodinated contrast or ionizing radiation. The 3D phase contrast technique is a flow-based technique, which may show dephasing in the presence of hemodynamically significant stenoses. A comprehensive examination should also include T1- and T2-weighted imaging for the assessment of potential neoplastic masses and the ubiquitous renal cysts. Through trial and error over the course of over a thousand examinations, this comprehensive approach to the MR evaluation of renal vascular pathology has emerged.  相似文献

We compared the value of 3D time-of-flight (TOF) and phase-contrast (PC) MR angiography (MRA) for detection and grading of intracranial vascular steno-occlusive disease. Unenhanced 3D-TOF MRA and 3D-PC MRA (30–60 cm/s velocity encoding) were performed at the level of the circle of Willis in 18 patients, mean age 56 ± 10 years. Postprocessed images using a maximum-intensity projection reconstruction with multiple targetted projections were analysed. A total of 126 vessels was assessed by PC MRA and 143 by TOF MRA, with digital subtraction angiography (DSA) in 15 patients and/or transcranial Doppler sonography (TCD) in 18 as a standard. Two blinded readers reviewed the MRA, DSA and TCD examinations retrospectively. On DSA and/or TCD the two observers found 32 and 28 steno-occlusive lesions. 3D-TOF MRA was more sensitive than 3D-PC MRA (87 % and 86 % vs. 65 % and 60 %) and had a higher negative predictive value (96 % vs. 89 %). Correct grading of stenoses was achieved in 78 % by 3D-TOF and 65 % by 3D-PC MRA. Received: 24 September 1997 Accepted: 27 February 1998  相似文献

The purpose of this study was to assess the diagnostic value of two-dimensional (2D) MR subtraction angiography of lower extremities in patients with symptomatic peripheral arterial occlusive disease with conventional angiography as the standard of reference. Twenty patients were prospectively included. 2D subtraction MR angiography (MRA) consisted of multisection gradient-recalled echo (GRE) acquisitions with the shortest TE available on our machine (4 msec), obtained in the coronal plane before and after intravenous bolus administration of gadolinium chelate. MR images were reconstructed after subtraction with a maximum-pixel-intensity-projection (MIP) algorithm. MRA was performed in all cases 1–4 days before diagnostic angiography. In a prospective blinded analysis, the number and location of significant (ie, >50%) stenoses and occlusions were evaluated for each vascular segment. Sensitivity and specificity were used to evaluate MRA data. Significant stenoses (38 of 46, 83%) and occlusions (66 of 67, 99%) seen at conventional angiography were identified with MRA. The sensitivity and specificity of MRA for determination of stenoses >50% or occlusions was 100% and 97%, respectively. The location and extent of stenoses and/or occlusions on MRA and angiograms were well correlated (kappa values, r = .73, P < .05). Contrast 2D MR subtraction angiography, by providing comparable information to that of conventional angiography, is well suited to evaluate the presence and severity of atherosclerotic lesions of the lower limbs.  相似文献

The aims of this preliminary study were to establish the efficacy and minimum effective dose of TG5(FdDO3A)52 gadolinium dendrimer for contrast-enhanced, three-dimensional (3D) time of flight (TOF) magnetic resonance angiography (MRA) of the body. In a dose ranging study in eight rabbits (Group A), each of two animals received 0.03; 0.02; 0.01; or 0.005 mmol/kg of the agent for 3D-TOF MRA of the pelvic circulation in the axial and coronal planes. An additional nine animals (Group B) received a dose of 0.02 mmol/kg for 3D-TOF MRA of the mediastinum, abdomen or of the lower limbs. Quantitative and qualitative analyses of the images from Group A demonstrated a dose-related reduction in saturation effects and improved visualization of vascular structures, with maximal augmentation of the contrast-to-noise ratio (CNR) at 0.03 mmol/kg. The dose of 0.02 mmol/kg was found to be the minimal effective dose at the three vascular regions.  相似文献

The purpose of this study was to investigate whether MR angiography (MRA) of the portal venous system may be improved by means of superparamagnetic iron oxides (SPIOs) during accumulation phase imaging and to study the underlying contrast mechanisms. MRA of the portal venous system was performed on 48 patients before and after intravenous injection of a new SPIO (Resovist, Schering AG, Berlin, Germany). Resovist, as a predominantly liver parenchymal darkening agent on T2-weighted MR images with uptake into the reticuloendothelial cell system, was administered intravenously by bolus injection of 8 to 12 μmol Fe/kg body weight. Patients were scanned with breath-hold coronal and axial two-dimensional (2D) time of flight (TOF) MRA (TR = 31.0 msec, TE = 9.8 msec, flip angle (FA) = 50°, and 6.9-second acquisition time per section) sequences. Signal intensity values of liver parenchyma, the portal venous system, and background were obtained for quantitative analysis. The clinical relevance of additional plain and contrast-enhanced MRA studies for surgical planning was assessed by independent reading of three readers. Liver signal-to-noise ratio (SNR) significantly decreased following iv injection of Resovist; however, SNR values of the portal veins or hepatic veins did not change significantly. Visibility of the portal venous system improved significantly (tertiary branches visible: pre in 15.2% versus post in 87.0% of patients). Resovist-enhanced 2D-TOF MRA may improve planning of liver resections by better demonstrating the relationship of central liver lesions and vessels on source images. The decrease in liver SNR at a constant vessel SNR after iv injection of Resovist improves MRA of the liver. SPIO-enhanced 2D-TOF MRA scans are superior to plain 2D-TOF MRA studies and may be added for the workup of preoperative patients.  相似文献
