Thirty-nine consecutive patients with symptomatic AV nodal reentrant tachycardia (AVNRT) underwent temperature guided slow AV nodal pathway ablation (group 1). Forty-three consecutive patients undergoing nontemperature guided slow AV nodal pathway ablation late in our experience compose the control population (group 2). Slow pathway ablation was achieved in all patients of both groups. The mean fluoroscopy and ablation times for group 1 were significantly shorter than for group 2 (26.1 ± 14.9 vs 33.9 ± 18.9 min, P < 0.05; 19.9 ± 12.1 vs 30.9 ± 23.3 min, P ≤ 0.02). There were no episodes of coagulum formation in group 1, while there were 15 episodes (7.1 % of energy applications) in group 2 (P = 0.0006) despite a significantly higher applied power in group 1 (53.4 ± 25.1 vs 35.6 ± 9.5W, P = 0.0001). Successful energy applications were associated with significantly higher temperatures than unsuccessful applications in group 1 (55.6 ± 5.8 C vs. 52.9 ± 6.8 C, P ≤ 0.03). The minimum temperature required for successful ablation was 48 C for two patients (5%) and was > 50 C for the remainder of patients (37/39 [95%]). The catheter ablation system used in this study was safe, effective, and prevented coagulum formation while delivering relatively high power. In addition, shorter ablation times and radiation exposure were seen with this system. Although successful energy applications and the production of junctional rhythm were associated with higher achieved temperatures, temperature alone did not predict either endpoint. Future prospective, randomized trials are needed to confirm these findings and further evaluate the value of temperature monitoring.  相似文献

Introduction: The occurrence of accelerated junctional rhythm (JR) during radiofrequency ablation of the slow pathway in patients with atrioventricular nodal reentrant tachycardia (AVNRT) is frequent. The aim of the present study was to compare the occurrence of JR during magnetic remote catheter ablation to the conventional manual ablation.
Methods and Results: Twenty six patients (males: seven; age: 51 ± 15 years) underwent slow pathway ablation with magnetic navigation (MN) system (Niobe, Stereotaxis Inc., St. Louis, MO, USA) and were compared to a control group of 11 patients (males: three; age: 53 ± 16 years) treated with conventional manual ablation. A 4-mm nonirrigated tip catheter was used in both groups with a maximum of 30 W and 60°C. Acute success was obtained in all patients. In the MN group, three patients out of 24 had no junctional beat (JB) at all and seven patients had 10 or less JB. In contrast, in the conventional group no patient had less than 10 JB. The mean number of JB in the MN group was 66 ± 94.9 (0–410) and 200 ± 243.1 (43–914) in the control group (P = 0.019). In the MN group one patient had a first-degree atrioventricular block. No other complication occurred.
Conclusions: Magnetic remote catheter ablation of AVNRT is effective and is associated with less JB than the manual conventional technique. Therefore, JB may not be considered as a mandatory indicator for successful AVNRT ablation with MN system. (PACE 2010; 11–15)  相似文献

Background: Radiofrequency catheter ablation of atrioventricular nodal reentrant tachycardia (AVNRT) has proven to be an effective therapy in the pediatric population. However, concerns of inadvertent permanent AV nodal block have resulted in many pediatric programs adopting cryoablation as their primary ablation approach for AVNRT.
Methods: A retrospective analysis of the results of pediatric radiofrequency catheter ablation at a single institution over the most recent 5 years (January 2004 through December 2008) was performed. Acute, intermediate, and long-term success, along with the incidence of AV block, were determined.
Results: There were 65 patients with a mean age of 12.1 ± 5.2 years and weight of 46.5 ± 17.3 kg who underwent radiofrequency catheter ablation for AVNRT. There was 100% acute success with no recurrences at a mean follow up of 32.5 months. Although two patients had a brief second-degree AV block, there was no permanent AV block of any degree.
Conclusions: The safety and efficacy of radiofrequency catheter ablation for pediatric AVNRT demonstrated in this study support its continued application and should not be abandoned as a method of treatment. (PACE 2010; 6–10)  相似文献

Study Objective: We examined the possible role of atrioventricular node (AVN) conduction abnormalities as a cause of AVN reentrant tachycardia (RT) in patients >65 years of age.
Study Population: Slow pathway radiofrequency catheter ablation (RFCA) was performed in 104 patients. Patients in group 1 (n = 14) were >65 years of age and had AV conduction abnormalities associated with structural heart disease. Patients in group 2 (n = 90) were <65 years of age and had lone AVNRT.
Results: Patients in group 1 versus group 2 (66% vs. 46% men) had a first episode of tachycardia at an older age than in group 2 (68 ± 16.8 vs 32.5 ± 18.8 years, P = 0.007). The history of arrhythmia was shorter in group 1 (5.4 ± 3.8 vs 17.5 ± 14, P = 0.05) and was associated with a higher proportion of patients with underlying heart disease than in group 2 (79% vs 3%, P < 0.001). The electrophysiological measurements were significantly shorter in group 2: atrial-His interval (74 ± 17 vs 144 ± 44 ms, P = 0.005), His-ventricular (HV) interval (41 ± 5 vs 57 ± 7 ms, P = 0.001), Wenckebach cycle length (329 ± 38 vs 436 ± 90 ms, P = 0.001), slow pathway effective refractory period (268 ± 7 vs 344 ± 94 ms, P = 0.005), and tachycardia cycle length (332 ± 53 vs 426 ± 56 ms, P = 0.001). The ventriculoatrial block cycle length was similar in both groups. The immediate procedural success rate was 100% in both groups, and no complication was observed in either group. One patient in group 2 had recurrence of AVNRT. One patient with a 98-ms HV interval underwent permanent VVI pacemaker implantation before RFCA procedure.
Conclusion: In patients undergoing RFCA for AVNRT at >65 years of age had a shorter history of tachycardia-related symptoms than patients with lone AVNRT. The longer AVN conduction intervals and refractory period might explain the late development of AVNRT in group 1.  相似文献

Background: Atypical atrioventricular (AV) nodal reentrant tachycardias (AVNRT) usually exhibit the earliest retrograde atrial activation (ERAA) at the right inferoseptum (Rt-IS) or proximal coronary sinus (PCS). The purpose of this study was to characterize atypical AVNRT with the ERAA at the right superoseptum (Rt-SS).
Methods: Seventy-three atypical AVNRTs induced in 63 cases were classified into the superior type with the ERAA at the Rt-SS and inferior type with the ERAA at the Rt-IS or PCS.
Results: There were nine superior (12%) and 64 inferior types of atypical AVNRT (88%) in seven and 56 cases, respectively. The superior type exhibited a short atrial-His interval during the tachycardia (166 ± 41 ms), long His-atrial interval during the tachycardia (H-At:156 ± 38 ms), and ventricular pacing at the tachycardia cycle length (TCL) (H-Ap:201 ± 36 ms), and evidence for a lower common pathway, including second-degree AV block (four tachycardias) and an H-Ap being longer than the H-At (nine tachycardias). The TCL was shorter in the superior type than in the inferior type (322 ± 35 vs 404 ± 110 ms; P < 0.02). In the inferior type, all tachycardias were eliminated after the ablation at the Rt-IS (44 tachycardias) or PCS (20 tachycardias) where an ERAA was recorded. In the superior type, ablation at the Rt-IS was ineffective; however, ablation at the right midseptum eliminated seven (78%) of the nine tachycardias.
Conclusions: The tachycardia circuit of the superior type might have deviated to a more superior part of Koch's triangle than that of the inferior type.  相似文献

Background: Cryoablation has now become an alternative to treat many cardiac arrhythmias, and may be the treatment of choice in some patient populations. We compared the effects of convective flow on large-tip cryo and radiofrequency (RF) lesions dimensions.
Methods: Cryoablation and RF ablation were performed on porcine heart sections in a saline bath with varying directed flow rates. Cryoablation was performed for 4 minutes on 50 tissue pieces with tip temperature controlled at −80°C. RF ablation was performed on 50 tissue pieces for 60 seconds at 60°C tip temperature. The pieces were placed in culture media for 24 hours, and then sectioned, stained, and measured.
Results: Cryoablation and RF lesion sizes varied significantly with flow such that higher flow rates produced smaller cryoablation lesions and larger RF lesions (mean cryoablation volumes: 854 ± 402, 808 ± 217, 781 ± 217, 359 ± 114, and 292 ± 117 mm3, and mean RF volumes: 211 ± 35, 304 ± 79, 439 ± 125, 525 ± 187, and 597 ± 126 mm3 for 0, 1, 2, 3, and 5L/min flow rates, respectively, P < 0.0005). Trabeculated pieces had larger cryoablation lesions and smaller RF lesions than nontrabeculated ones at higher flow rate (P < 0.005). Cryoablation lesion volume increased as the time to reach −80°C decreased (r2= 0.72).
Conclusion: In contrast to RF ablation, cryoablation lesion size is smaller at high flow rates, and larger at low flow rates due to the warming effects of local convective flow. The effects of high flow are reduced in areas of trabeculation, and the time to reach −80°C predicts cryoablation lesion size.  相似文献

BACKGROUND: Radiofrequency catheter ablation (RCA) of supraventricular tachycardia (SVT) in children is highly successful but requires exposure to radiation. Nonfluoroscopic mapping systems may significantly reduce fluoroscopy time. METHODS: Forty consecutive pediatric patients who underwent RCA for accessory pathways (AP) or AV nodal reentrant tachycardia (AVNRT) with use of a nonfluoroscopic navigation system (Ensite NavX) (group A) were compared retrospectively to 40 consecutive patients with similar diagnoses who underwent RCA with fluoroscopic guidance only (group B). RESULTS: Group A (mean age 12.1+/-2.9 years, mean weight 47+/-13.9 kg) consisted of 11 patients (27.7%) with AVNRT and 29 (72.5%) with AP. Group B (mean age 10.9+/-3.1 years, mean weight 47.1+/-17.1 kg) consisted of 7 patients (17.5%) with AVNRT and 33 (82.5%) with AP. There were no significant differences in AP location, patients with congenital heart disease, and number of radiofrequency lesions. Fluoroscopy time was significantly shorter in group A than in group B (10.4+/-6.1, range 3.1-28.8 minutes, vs 24.9+/-16.0, range 4.4-82.0 minutes, P<0.0001). Procedure duration was also significantly shorter in group A than in group B (170+/-68.5, range 90-420 minutes, vs 218+/-69.3, range 90-360 minutes, P<0.0001). Initial success was 95% in group A and 100% in group B. Tachycardia recurrences occurred in two patients in group A (5%) and six patients in group B (15%). Final success, including repeat ablations for recurrences or failures, was 100% in both groups. CONCLUSIONS: The use of a nonfluoroscopic system for catheter navigation significantly reduced fluoroscopy exposure and total procedure duration of RCA of common SVT substrates in children.  相似文献

Objectives: The purpose of this study was to describe a midseptal approach to selective slow pathway ablation for the treatment of AV nodal reentrant tachycardia (AVNRT). In addition, predictors of success and recurrence were evaluated. Methods: Selective ablation of the slow AV nodal pathway utilizing radiofrequency (RF) energy and a midseptal approach was attempted in 60 consecutive patients with inducible AVNRT. Results: Successful slow pathway ablation or modification was achieved in 59 of 60 patients (98%) during a single procedure. One patient developed inadvertent complete AV block (1.6%). A mean of 2,7 ±1.4 RF applications were required with mean total procedure, ablation, and fluoroscopic times of 191± 6.3, 22.8 ± 2.3, and 28.2 ±1.8 minutes, respectively. The PR and AH intervals, as well as the antegrade and retrograde AV node block cycle length, were unchanged. However, the fast pathway effective refractory period was significantly shortened following ablation (354± 13 msec vs 298 ± 12 msec; P= 0.008). The A/V ratio at successful ablation sites were no different than those at unsuccessful sites (0.22 ± 0.04 vs 0.23± 0.03). Junctional tachycardia was observed during all successful and 60 of 122 (49%) unsuccessful RF applications (P < 0.0001). A residual AV nodal reentrant echo was present in 15 of 59 (25%) patients, During a mean follow-up of 20.1± 0.6 months (11.5–28 months) there were four recurrences (5%), 4 of 15 (27%) in patients with and none of 44 patients without residual slow pathway conduction (P = 0.002). Conclusions: A direct midseptal approach to selective ablation of the slow pathway is a safe, efficacious, and efficient technique. Junctional tachycardia during RF energy application was a highly sensitive but not specific predictor of success and residual slow pathway conduction was associated with a high rate of recurrence.  相似文献

The mechanism of cure in AV nodal reentrant tachycardia (AVNRT) by catheter ablation has not been fully clarified. We hypothesized that disruption of a shortcut link between the fast and slow pathways is responsible for the elimination of tachycardia. Results: AVNRT was eliminated in 20 patients by catheter ablation. In five patients (25%; group 1) slow pathway conduction disappeared 1 week after ablation. In six patients (30%; group II), the effective refractory period of the slow pathway was prolonged by more than 50 ms (212 ± 81 ms vs 340 ± 81 ms; P < 0.05). In the remaining nine patients (45%; group III), there was no change in the refractory period (270 ± 65 ms vs 273 ± 74 ms), although tachycardia was not inducible. A shortcut link between the fast and slow pathways was examined by comparing the A-H intervals over the slow pathway during the tachycardia and during atrial pacing at the tachycardia cycle length. Prior to ablation, a shortcut link was assumed in 1 of group I patients, 2 of group II patients, and 8 of group III patients. Of the 9 patients in whom the slow pathway was not impaired after ablation (group III), 8 patients were found to have a shortcut link, while 8 of 11 patients with impairment of the slow pathway after ablation (groups I and II) had no shortcut link between the fast and slow pathways (P < 0.05). Conclusion: In patients with a shortcut link between the fast and slow pathways, slow pathway conduction itself does not need to be impaired to eliminate the AVNRT, whereas in patients without this shortcut link, slow pathway conduction must be impaired.  相似文献

BACKGROUND: Previous studies in adults have shown a significant shortening of the fast pathway effective refractory period (ERP) after successful slow pathway ablation. However, information on atrioventricular nodal reentrant tachycardia (AVNRT) in children is limited. The purpose of this retrospective study was to investigate the different effects of radiofrequency (RF) catheter ablation in pediatric AVNRT patients between those with and without dual atrioventricular (AV) nodal pathways. METHODS: From January 1992 to August 2004, a total 67 pediatric patients with AVNRT underwent an electrophysiologic study and RF catheter ablation at our institution. We compared the electrophysiologic characteristics between those obtained before and after ablation in the children with AVNRT with and without dual AV nodal pathways. RESULTS: Dual AV nodal pathways were found in 37 (55%) of 67 children, including 36 (54%) with antegrade and 10 (15%) with retrograde dual AV nodal pathways. The antegrade and retrograde fast pathway ERPs in children with dual AV nodal pathways were both longer than the antegrade and retrograde ERPs in children without dual AV nodal pathways (300 +/- 68 vs 264 +/- 58 ms, P = 0.004; 415 +/- 70 vs 250 +/- 45 ms, P < 0.001) before ablation. In children with antegrade dual AV nodal pathways, the antegrade fast pathway ERP decreased from 300 +/- 68 ms to 258 +/- 62 ms (P = 0.008). The retrograde fast pathway ERP also decreased after successful ablation in the children with retrograde dual AV nodal pathways (415 +/- 70 vs. 358 +/- 72 ms, P = 0.026). CONCLUSION: The dual AV nodal physiology could not be commonly demonstrated in pediatric patients with inducible AVNRT. After a successful slow pathway ablation, the fast pathway ERP shortened significantly in the children with dual AV nodal pathways.  相似文献

BACKGROUND: Catheter ablation is the treatment of choice for atrioventricular nodal reentrant tachycardia (AVNRT) with a success rate of 95-98%. The appearance of junctional rhythm during radiofrequency (RF) application to the slow pathway has been consistently reported as a marker for the successful ablation of AVNRT. Ventriculoatrial (VA) conduction during junctional rhythm has been used by many as a surrogate marker of antegrade atrioventricular nodal (AVN) function. However, VA conduction may not be an accurate or consistent marker for antegrade AVN function and reliance on this marker may leave some patients at risk for antegrade AVN injury. OBJECTIVE: The purpose of this study is to describe a technique to ensure normal antegrade AVN function during junctional rhythm at the time of RF catheter ablation of the slow pathway. METHODS: Retrospective review of all patients less than 21 years old who underwent RF ablation for AVNRT at our institution from January 2002 to July 2005. During RF applications, immediately after junctional rhythm was demonstrated, RAP was performed to ensure normal antegrade AVN function. Postablation testing was performed to assess AVN function and tachycardia inducibility. RESULTS: Fifty-eight patients underwent RF ablation of AVNRT during the study period. The mean age +/- SD was 14 +/- 3 years (range: 5-20 years). The weight was 53 +/- 15 Kg (range: 19-89 Kg). The preablation Wenckebach cycle length was 397 +/- 99 msec (range: 260-700 msec). Fifty-four patients had inducible typical AVNRT, and four patients had atypical tachycardia. The mean tachycardia cycle length +/- SD was 323 +/- 62 msec (range: 200-500 msec). Patients underwent of 8 +/- 7 total RF applications (median: 7; range 1 to 34), for a total duration of 123 +/- 118 seconds (median: 78 sec, range: 20-473 sec). Junctional tachycardia was observed in 52 of 54 patients. RAP was initiated during junctional rhythm in all patients. No patient developed any degree of transient or permanent AVN block. Following ablation, the Wenckebach cycle length decreased to 364 +/- 65 msec (P < 0.01). Acutely successful RF catheter ablation was obtained in 56 of 58 patients (96%). CONCLUSION: Rapid atrial pacing during radiofrequency catheter ablation of the slow pathway is a safe alternative approach to ensure normal AVN function.  相似文献

Catheter ablation has become a well-established, first-line therapy for atrioventricular nodal reentrant tachycardia (AVNRT), the most common reentry supraventricular tachycardia in humans. Robotic systems are becoming increasingly common in both complex and simple ablation procedures with presumed potential improvements in procedural efficacy and safety. The authors of this article conducted a systematic review and meta-analysis on the effectiveness and safety of the magnetic navigation system (MNS) in comparison with conventional catheter navigation for AVNRT ablation. An electronic search was performed using Cochrane Central database, Medline, Embase and Web of Knowledge between 2002 and 2012. References were searched manually. Outcomes of interest were: acute and long-term success, complications, total procedure, ablation and fluoroscopic times. Continuous variables were reported as standardized difference in means (SDM); odds ratios (OR) were reported for dichotomous variables. Thirteen studies (seven of which were nonrandomized controlled, four were case series and two were randomized controlled studies) involving 679 adult patients were identified. Twelve studies were based on a single center and one study was multicentral. MNS was deployed in 339 patients. The follow-up period ranged between 75 and 180 days. Acute success and long-term freedom from arrhythmia were not significantly different between MNS and control groups (98 vs 98%, OR: 0.94 [95% CI: 0.21–4.1] and 97 vs 96%, OR: 1.18 [95% CI: 0.35–4.0], respectively). A shorter fluoroscopic time was achieved with MNS; however, this did not reach statistical significance (15 vs 19 min, SDM: -0.26 [95% CI: -0.64–0.12]). Longer total procedure but similar ablation times were noted with MNS (160 vs 148 min, SDM: 3.48 [95% CI: 0.75–6.21] and 4 vs 6 min, SDM: -0.83 [95% CI: -2.19–0.53], respectively). The overall complication rate was similar between both groups (2.7 vs 1.0%, OR: 1.28 [95% CI: 0.33–4.96]). Our data suggest that the usage of MNS results in similar rates of success and complications when compared with conventional manual catheter ablation for AVNRT. MNS had a trend for reduced fluoroscopic time. Longer total procedure time was observed with MNS while the actual ablation time remained similar. Prospective randomized trials will be needed to better evaluate the relative role of MNS for catheter ablation of AVNRT.  相似文献

Background: Gold has excellent electrical conductive properties and creates deeper and wider lesions than platinum-iridium during radiofrequency (RF) ablation in vitro . We tested the maximum voltage-guided technique (MVGT) of cavotricuspid isthmus (CTI) ablation using two 8-mm tip catheters containing gold (group G) or platinum-iridium (group PI).
Methods: We enrolled 31 patients who underwent CTI ablation. In group G (n = 15) CTI ablation was performed with a gold-tip ablation catheter, while in group PI (n = 16) a platinum-iridium tip was used. Ablation was guided by CTI potentials with the highest amplitude until achievement of bidirectional block (BIB). If BIB was not achieved after 10 RF applications, RF was delivered via a 3.5-mm irrigated-tip catheter. Success rate, procedure duration, duration of fluoroscopic exposure, and number of RF applications were measured.
Results: BIB was achieved in all patients in group G, while in group PI an irrigated tip was used in four patients (0% vs 25%, P < 0.001). These four patients required a total of 21 additional RF applications (5.25 ± 2.22). Procedure time (56.4 ± 12 vs 73.1 ± 15 minutes P < 0.05) and fluoroscopic explosure (4.9 ± 2.3 vs 7.1 ± 3.8 minutes, P < 0.01) were shorter in group G than in group PI. Mean number of RF applications was lower (4.6 ± 1.9 vs 6.6 ± 3.1 P < 0.001) and total RF duration shorter (280 ± 117 vs 480 ± 310 seconds) in group G than in group PI. No difference was observed in the number of recurrences at a 6 month-follow up (1 in group G vs 1 in group PI).
Conclusion: Using the MVGT of CTI ablation, gold-tip catheters were associated with shorter procedural and fluoroscopic times, and fewer RF applications.  相似文献

Atrial tachycardia, with its focus near the apex of Koch's triangle, may carry a potential risk of atrioventricular block during radiofrequency catheter ablation. The efficacy and safety of this procedure have never been addressed. The characteristics and catheter ablation results are reported for six patients with atrial tachycardia near the apex of Koch's triangle. All six patients were female aged 49.6 ± 9.3 years (range 39–63). Organic heart disease was present in 3 (50%) of the 6 patients. The P wave in surface ECG had a mean axis of − 28° (range − 90°–+ 30°) in the frontal plane. The catheter ablation was guided by activation sequence mapping. The energy was titrated from low power level. Atrial overdrive pacing was used to monitor the atrioventricular conduction should accelerated functional rhythm occur. At the final successful ablation site, the local atrial activation was 41.8 ± 9.1 ms before the P wave and His-bundle potential was present in 5 of the 6 patients. All patients had their atrial tachycardia eliminated without recurrence or heart block during a follow-up period of 17.7 ± 8.5 months (range 6–30). In conclusion, atrial tachycardia near the apex of Koch's triangle has distinct clinical and electrophysiological features, Radiofrequency catheter ablation can be performed effectively. However, extreme care must be taken to prevent inadvertent atrioventricular block. Titrated energy application and continuous monitoring of atrioventricular conduction are mandatory.  相似文献

Introduction: Post infarct ventricular tachycardia (VT) often involves the interventricular septum (IVS) and requires transmural septal ablation. The purpose of this study was to compare the efficacy of bipolar ablation (BIA) versus sequential unipolar ablation (SUA) in creating a transmural ablation line along the IVS scar border.
Methods and Results: Both ablation strategies were compared in a phantom agar model first and then in 10 post infarct sheep. In the phantom agar model BIA lesions were larger, transmural, and less dependent on catheter alignment and contact compared with SUA. Noncontact mapping was used in the animals to identify the septal scar border and create a 30-mm ablation line. In five animals BIA (50 W) was performed between two irrigated catheters on either side of the IVS, and in five control animals SUA (50 W) was performed, first on the left ventricle (LV) septal scar border and then on the opposing right ventricle (RV) septal surface. Electrical block along ablation lines was confirmed with noncontact mapping. BIA required significantly less ablations (12 ± 1 vs 29 ± 7, P = 0.001), ablation time (22 ± 3 vs 48 ± 6 minutes, P < 0.001), and energy (58 ± 7 vs 124 ± 21 kJ, P < 0.001). At pathological examination all ablation lines in both groups were transmural at the IVS border. BIA endocardial ablation lines (LV + RV) were significantly longer than SUA lines (76 ± 10 vs 49 ± 11 mm, P = 0.003).
Conclusion: BIA of the IVS is highly effective at creating a transmural ablation line, requiring less ablation and creating longer lesions than SUA. BIA ablation may have a role for post infarct VT involving the IVS. (PACE 2010; 33:16–26)  相似文献

BACKGROUNDS: Radiofrequency (RF) catheter ablation represents a major advance in the management of children with cardiac arrhythmias and has rapidly become the standard of care for the first-ling therapy of supraventricular tachycardias (SVTs). The purpose of this study was to investigate the results of the RF catheter ablation of SVTs in pediatric patients. METHODS: From December 1989 to August 2005, a total of 228 pediatric patients (age: 9 +/- 7 years, range: 5-18 years; male:female = 117:111) with clinically documented SVT underwent an electrophysiologic study and RF catheter ablation at our institution. RESULTS: The arrhythmias included atrioventricular reentrant tachycardia (AVRT; n = 140, 61%), atrioventricular nodal reentrant tachycardia (AVNRT; n = 66, 29%), atrial tachycardia (AT; n = 11, 5%), and atrial flutter (AFL; n = 11, 5%). The success rate of the RF catheter ablation was 92% for AVRT, 97% for AVNRT, 82% for AT, and 91% for AFL, respectively. Procedure-related complications were infrequent (8.7%; major complications: high grade AV block (2/231, 0.9%); minor complications: first degree AV block (6/231, 2.6%), reversible brachial plexus injury (2/231, 0.9%), and local hematomas or bruises (10/231, 4.3%)). The recurrence rate was 4.7% (10/212) during a follow-up period of 86 +/- 38 months (0.5-185 months). CONCLUSIONS: The RF catheter ablation was a safe and effective method to manage children with paroxysmal and incessant tachycardia. The substrates of the arrhythmias differed between the pediatric and adult patients. However, the success rate of the ablation, complications, and recurrence during childhood were similar to those of adults.  相似文献

BACKGROUND: Both actively cooled-tip and large-tip catheters are currently available clinically to create large endomyocardial lesions during application of radiofrequency (RF) energy. The purpose of this study was to compare the effect of convective cooling at physiologic flow rates on RF lesion size using both actively cooled and large-tip catheters. METHODS: Porcine hearts were sectioned into 72 pieces and placed in a temperature-controlled saline bath (37 degrees C) with varying directed flow rates (0, 1, 2, and 3 L/min). Cooled-tip RF ablation (4 mm tip) was performed for 1 minute on 36 tissue sections with power manually titrated to keep tip temperature below 40 degrees C. Large-tip catheter ablation (10 mm tip) was performed at 65 degrees C target temperature for 1 minute on 36 tissue sections. For each catheter, flow rates were randomized between applications. The tissue pieces were sectioned and measured to determine lesion depth, width, and volume. RESULTS: Lesion dimensions were independent of the flow rate for the cooled-tip catheter (mean volumes: 382.0 +/- 121.6, 419.9 +/- 133.4, 375.9 +/- 169.1, and 346.7 +/- 173.4 mm(3) for 0, 1, 2, and 3 L/min flow rate, respectively, P = 0.78). For the large-tip catheter, lesion size varied significantly with flow, such that higher flow rates produced larger lesions (mean volumes: 120.7 +/- 50.7, 256.5 +/- 97.9, 393.4 +/- 149.9, and 548.9 +/- 157.0 mm(3) for 0, 1, 2, and 3 L/min flow rate respectively, P < 0.001) CONCLUSION: During RF ablation, blood flow rate significantly affects lesion size for large-tip but not cooled-tip catheters. At low flow rates (0-1 L/min) cooled-tip catheters create larger lesions, while at high flow rates (3 L/min) large-tip catheters create larger lesions.  相似文献

Endocardial radiofrequency ablation of the left ventricle does not create transmural lesions reliably even with active electrode cooling. The authors developed a prototype catheter with an internally cooled needle electrode that could be advanced an adjustable distance into the myocardium. Freshly excised hearts from eight male sheep were perfused and superfused using oxygenated ovine blood. Ablations were performed for 2 minutes using the prototype catheter and a conventional endocardial 5-mm irrigated tip ablation catheter at target temperatures of 80 degrees C and 50 degrees C, respectively. The prototype catheter needle was inserted 12 mm deep for all ablations. The maximal power and irrigation rate was 50 W, 20 mL/min for the irrigated tip catheter and 20 W, 10 mL/min for the intramural needle catheter. Intramural needle lesions were significantly deeper (13.5 +/- 2.3 vs 9.1 +/- 1.3 mm, P < 0.01) but less wide (8.7 +/- 1.5 vs 12.7 +/- 1.9 mm, P < 0.01) than irrigated tip lesions. Popping occurred during 12 (37%) of the 32 irrigated tip ablations. Popping did not occur during intramural needle ablation. The cooled intramural needle ablation catheter creates lesions that are significantly deeper than irrigated tip catheters with less tissue boiling. In contrast to irrigated tip ablation, electrode temperature monitoring can be used to determine if a lesion has been created during intramural needle ablation. The cooled intramural needle ablation lesions were of a clinically useful width, addressing one of the main recognized deficiencies of intramural needle ablation.  相似文献

We describe two patients who presented with a history of recurrent palpitations on swallowing of solid food. The event-recorder and Holter monitoring documented episodic supraventricular tachycardia (SVT) initiated by atrial premature contractions (APCs). During electrophysiological study (EPS), swallowing of solid food consistently induced APCs and their activation sequence, morphology of P wave were suggestive of their right atrial origin in them. Drug challenge did not affect the APC onset during the swallowing. During EPS, slow-fast variety of atrioventricular nodal reentrant tachycardia (AVNRT) was induced and successful radiofrequency (RF) catheter ablation of slow pathway resulted in total relief of their symptoms.  相似文献

Introduction: Pulmonary venous antra isolation (PVAI) is the cornerstone of catheter ablation procedure for drug refractory paroxysmal atrial fibrillation (AF). However, the procedure is technically challenging. Robotic navigation has a potential to expedite and facilitate the procedure.
Methods: A robotic catheter control system was used for remote navigation-supported PVAI in 22 patients (mean age = 55 ± 9 years, 16 males, study group). An irrigated-tip catheter with estimate of catheter force on the tissue was used. This was compared in nonrandomized fashion with conventional hand-controlled catheter ablation in 16 patients (mean age = 55 ± 9 years, 13 males, control group). The procedures were performed under guidance of Ensite NavX navigation system (St. Jude Medical, St. Paul, MN, USA) and intracardiac echocardiography.
Results: Robotic navigation was associated with significantly shorter overall duration of radiofrequency delivery (1,641 ± 609 vs 2,188 ± 865 seconds, P < 0.01), shorter total procedural time (207 ± 29 vs 250 ± 62 minutes, P = 0.007), fluoroscopy exposure (15 ± 5 vs 27 ± 9 minutes, P < 0.001), and lower radiation dose (1,119 ± 596 vs 3,048 ± 2,029 mGy/m2, P < 0.001). No complication was observed in either the study or the control group. During the 5 ± 1 months follow-up in the study group and 9 ± 3 months in the control group, 91% and 81% of patients, respectively, were AF free.
Conclusions: In our early clinical experience, PVAI using a remote robotic catheter navigation was effective, safe, and associated with shorter procedural and fluoroscopic times than conventional PVAI.  相似文献
