Results of 203 patients who underwent first pass radionuclide angiography (FP) and quantitative equilibrium radionuclide ventriculography (qERNV) were stored in a data base system and evaluated statistically. Eighty eight of these patients also underwent exercise equilibrium radionuclide ventriculography (E-qERNV). In patients with coronary artery disease (CAD) without previous myocardial infarction (MI), evaluation of global and regional ejection fraction (gEF, rEF) at rest revealed a poor sensitivity of 64%, the specificity was about 71% (qERNV). FP at rest revealed similar values of sensitivity (69%) and specificity (83%). Additional assessment of stress induced changes of gEF, significantly (P<0.05) improved sensitivity of qERNV in CAD patients without a history of previous MI to 84% (specificity 86%). In patients with one previous MI, however, similar values of sensitivity were found (RFP: 87%, R-qERNV: 84%, E-qERNV: 93%). In patients with several MI's, sensitivity was above 90% at rest and during exercise (R-FP: 96%, R-qERNV: 93%, E-qERNV: 100%).  相似文献

The results of 203 patients who underwent firstpass radionuclide angiography (FP), as well as quantitative equilibrium radionuclide ventriculography (qERNV), were stored in a data base system and evaluated statistically. In patients with coronary artery disease (CAD) without previous myocardial infarction (MI), evaluation of global and regional ejection fraction (gEF, rEF) at rest revealed a poor sensitivity of 64% (Rest-qERNV) and 69% (Rest-FP), respectively. In patients with a history of one previous MI, the sensitivity of both methods was equivalent: FP 87% and qERNV 84%. In patients with several MIs, sensitivity was higher than 90%. Concerning localization of MI, remarkable differences between FP and qERNV were found. In posterior wall infarction, the FP sensitivity was 87% and qERNV only 67%, whereas in anterior wall infarction, the results were similar for both methods: 93% (FP) and 96% (qERNV), respectively. Since 30° RAO camera position achieves the best visualization of the anterior and posterior wall, FP is superior to qERNV in the evaluation of posterior wall asynergies. In addition, qERNV often fails to discriminate anterior and posterior wall motion abnormalities.  相似文献

This study compared the accuracy of rest and exercise gated equilibrium technetium ventriculography with exercise thallium imaging in 50 consecutive male patients undergoing routine coronary angiography for the evaluation of chest pain. No patients were excluded on the basis of prior myocardial infarction, nature of angiographically defined coronary disease or symptoms. Antianginal therapy was continued in all patients. Eight patients had normal coronary arteries, 9 had single vessel, disease, 20 had double vessel disease and 13 had triple vessel disease. Sixteen patients had previously documented myocardial infarction. Using exercise radionuclide ventriculography, 34 patients with coronary disease were detected resulting in a sensitivity of 81%; 6 patients with normal coronary arteries had normal scans, a specificity of 75%, with a predictive accuracy of 80%. In comparison, thallium imaging detected 42 patients with coronary disease resulting in a sensitivity of 100%. Six patients with normal coronary arteries had normal thallium images resulting in a specificity of 75% and a predictive accuracy of 96%. These results suggest that exercise thallium imaging is a more accurate investigation than exercise equilibrium radio-nuclide ventriculography and is the investigation of choice in the noninvasive detection of coronary artery disease.  相似文献

Background  The purpose of this study was to compare the incremental value of clinical information, electrocardiographic data, myocardial perfusion imaging, and radionuclide angiography for predicting severe coronary artery disease at a single testing interval. Clinical information, treadmill exercise studies, radionuclide angiography, and myocardial perfusion imaging are important predictors of severe coronary artery disease. However, the relative and absolute diagnostic importance of each of these methods has not been addressed at a single testing interval. Methods and Results  A same-day rest/treadmill exercise perfusion and function study was performed in 167 patients within 90 days of coronary angiography. A multivariable regression model was used to assess the independent informational content of these predictors. Clinical and electrocardiographic data were related strongly to the presence of severe coronary artery disease (χ2=12.2 and p<0.001; χ2=11.8 and p<0.001, respectively). Combined perfusion and functional studies contributed 31% of the diagnostic information beyond that provided by clinical and electrocardiographic data alone (p<0.05). Conclusions  These data demonstrate that combined studies of myocardial perfusion and left ventricular function are able to improve prediction of the extent of coronary artery disease, even when clinical and electrocardiographic data are also available. The ability of single view and biplanar radionuclide ventriculography (RVG) to determine the location of myocardial ischemia during maximal graded supine bicycle exercise was assessed in 50 patients with chest pain, no prior myocardial infarction, and a single coronary stenosis of 50% luminal diameter narrowing at coronary angiography. A biplane collimator was used so that both right anterior oblique (RAO) gated first-pass and left anterior oblique (LAO) equilibrium RVG could be performed at rest and exercise. Results were compared with those obtained using 4-view 201Tl myocardial scintigraphy in the same patients. Regional wall motion abnormalities (WMA) and 201Tl perfusion defects were detected and assigned to individual coronary vessels by agreement between at least two of three independent observers, who read all studies blinded along with those from control subjects with chest pain but no angiographically significant coronary artery disease. When scintigraphic abnormalities were detected, both biplanar RVG (36/39=92%) and 201Tl (25/25=100%) were more frequently correct in predicting the stenosed vessel than single view LAO RVG (24/32=75%) (P<0.05). At RVG only inferior WMA, in the RAO view, predicted right coronary stenosis. Only posterolateral WMA, in the LAO view, predicted left circumflex stenosis. Thus biplanar, but not single view, LAO exercise RVG is a reasonable alternative to exercise 201Tl for localizing exercise-induced ischemic abnormalities to individual coronary stenoses.This work was supported by a Postgraduate Medical Research Scholarship from the National heart Foundation of Australia (Canberra), and by the Alfred Hospital Whole Time Medical Specialists (Melbourne)  相似文献

目的:研究CT冠状动脉成像在冠心病早期诊断冠状动脉狭窄定性定量判读的作用。方法:对34例临床未发生急性冠脉综合征的冠心病患者,先后进行冠状动脉造影、128排双源CT冠状动脉成像。以冠状动脉造影为"金标准",计算CT冠状动脉成像敏感性、特异性、阳性预测值、阴性预测值。结果:①与冠状动脉造影相比,CT冠状动脉成像的敏感性为68%,特异性为97%,阳性预测值为89%,阴性预测值为90%。②CT冠状动脉成像有65个血管段图像质量差,约占12%,造成图像质量差的原因主要为钙化,心跳、呼吸伪影,少部分为管腔显示不良。结论:冠心病早期诊断中,CT冠状动脉成像可用作冠状动脉造影前筛选,CT冠状动脉成像阴性的患者不必行冠状动脉造影检查;CT冠状动脉成像阳性的患者,可行冠状动脉造影进一步确认病变。  相似文献

Myocardial thallium uptake has been assessed at the time of thallium scanning in a group of 50 male patients undeergoing coronary arteriography and 10 young healthy volunteers. The net thallium dose injected was obtained by counting the dose prior to injection using the gamma camera and counting the syringe and IV cannula after injection. Significantly higher levels of myocardial thallium uptake were obtained in both the volunteers and patients with normal coronary anatomy (1.36%±0.32%, n=10 and 0.93%±0.26%, n=9, respectively) compared to patients with single, double or triple vessel coronary artery disease (0.63%±0.19%, n=11; 0.70%±0.20%, n=15; 0.67±0.18, n=15, respectively). Exercise tests were positive in 46% of patients with coronary artery disease with an overall predictive accuracy of 56%. Thallium scans were positive in 68% of patients at a specificity of 89%. If the range of myocardial thallium uptake from the patients with normal coronary arteries is used to define a lower limit of normal, then the sensitivity of the thallium scan with thallium uptake is 90% with a predictive accuracy of 90% in the detection of significant coronary artery disease in this group of patients. Thus, estimation of total % thallium uptake is a simple index which yields useful diagnostic clinical information.  相似文献

In the assessment and evaluation of patients with suspected coronary artery disease there is a need for pharmacological stress combined with thallium scintigraphy. Thallium images were obtained following stress both with dobutamine infusion (5–20 g kg–1 min–1) and with symptom-limited bicycle ergometry in 20 patients (age 39–70 years) with chest pain who had been admitted for coronary angiography. Percentage thallium uptake was calculated using a region of interest technique. Detailed comparison was performed of the presence, size and distribution of left ventricular thallium perfusion defects; the percentage thallium uptake in ventricles, lung and liver; and the haemodynamic response to stress. Each stress produced a similar number of abnormal segments in each of three views (total EX 166/300; DOB 167/295), but exercise produced larger defects in the anterior view (P<0.025). Thallium uptake in left and right ventricles and relative uptake to lungs were similar, but dobutamine produced higher relative liver uptake [EX 1.55 (0.67); DOB 2.97 (1.23) P<0.0001]. Fourteen patients were able to tolerate dobutamine 20 g kg–1 min–1. The ratio of peak stress to rest double product was smaller with dobutamine in both patients with (DOB 1.3; EX 2.0; P<0.0047) and patients without -blockade (DOB 1.5; EX 2.4; P<0.008). Dobutamine produced fewer conventional stress endpoints of chest pain and ST depression. In conclusion, dobutamine produces a well-tolerated incremental pharmacological stress with thallium images similar to maximal exercise, and provides a useful alternative stress in patients unable to perform adequate dynamic exercise.Correspondence to: D.R. 目的:评价冠状动脉CT血管成像(CCTA)和运动平板试验(TET)对冠心病的诊断价值。方法:以常规冠状动脉造影(CAG)为诊断冠心病(冠脉狭窄≥50%)的"金标准",对同期先后行TET、CCTA和CAG 3种检查的75例疑似冠心病患者进行回顾性分析,将其TET和CCTA的结果与CAG进行比较。结果:TET和CCTA诊断冠心病的敏感度分别为45.2%和90.5%,特异度为69.7%和93.9%,阳性预测值为65.5%和95.0%,阴性预测值为50.0%和88.6%,准确率为56.0%和92.0%,P<0.01。在冠状动脉血管水平CCTA对右冠状动脉、左主干、前降支、回旋支狭窄诊断的准确率分别为86.7%、100.0%、88.0%和76.0%。冠心病患者中TET诊断阳性率与病变血管支数呈正相关(r=0.440,P=0.004);冠心病患者TET诊断结果阳性与阴性仅与血管狭窄程度≥75%狭窄的节段数目有统计学差异(P=0.016)。结论:CCTA诊断冠心病较TET有更高的诊断准确性和较低的诊断假阳性和假阴性,对有症状的疑诊冠心病患者CCTA的诊断具有更重要作用。  相似文献

BACKGROUND: The ability of the size of a total myocardial perfusion defect (MPD) to detect extensive coronary artery disease (CAD) is currently suboptimal with exercise thallium 201 single photon emission computed tomography (SPECT). To improve its performance, exercise electrocardiography and indirect scintigraphic markers of extensive CAD were proposed (increased right ventricular Tl-201 uptake, lung-to-heart [L/H] ratio, and left ventricular transient ischemic dilation ratio). We aimed to determine the additive value of these criteria for the detection of extensive CAD. METHODS AND RESULTS: The population included 338 patients who underwent exercise Tl-201 SPECT and coronary angiography. Patients were classified as having extensive CAD (left main, multivessel, or 1-vessel proximal left anterior descending CAD) or limited CAD (1-vessel disease other than proximal left anterior descending CAD or no CAD). First, Tl-201 SPECT provided higher diagnostic value than exercise electrocardiography. Second, age, percent target heart rate achieved, total MPD, and L/H ratio were independent predictors of extensive CAD. Third, visually estimated abnormal right ventricular Tl-201 uptake did not present additional information. Fourth, L/H ratio presented a higher diagnostic accuracy than left ventricular transient ischemic dilation ratio. CONCLUSIONS: With exercise Tl-201 SPECT, age, percent target heart rate achieved, total MPD, and L/H ratio were independent predictors of extensive CAD.  相似文献

BACKGROUND: Many observational studies that predict patient outcomes have examined the use of myocardial perfusion imaging results. However, a referral pattern for radionuclide testing could bias these analyses and should be determined. These patients may also differ with regard to the extent of coronary artery disease (CAD). All of these differences must be incorporated into proper outcomes examinations. We sought to identify the nuclear perfusion imaging referral pattern for patients with left ventricular (LV) dysfunction and significant CAD. METHODS AND RESULTS: Patients with LV dysfunction and CAD (n = 2951) meeting our inclusion criteria were compared by receipt or absence of radionuclide perfusion testing within 6 months before or after angiography. Pearson chi2 and Kruskal-Wallis analyses were used to examine differences in baseline characteristics and catheterization results, whereas logistic regression modeling was applied to predict nuclear imaging referral before and after catheterization. Precatheterization nuclear cohort patients were more likely to be minority patients (odds ratio [OR], 1.34; P =.0083) with previous cardiac revascularization (OR, 2.27; P =.0001), Charlson comorbidity index greater than 1 (OR, 1.146; P =.0091), and heart failure symptoms (OR, 1.62; P =.0001) than those without imaging. They were less likely to have a myocardial infarction (OR, 0.464; P =.0001). After catheterization, the nuclear patients were more likely to have had congestive heart failure (OR, 1.452; P =.0019), a myocardial infarction (OR, 1.353; P =.0371), an ejection fraction lower than 30% (OR, 1.058; P =.0002), and prior revascularization (OR, 1.880; P =.0001). In addition, they had fewer diseased vessels (OR, 0.731; P =.0001). CONCLUSIONS: Bias exists in nuclear referral for patients with LV dysfunction and significant CAD and must be considered when interpreting observational studies on this topic.  相似文献

BACKGROUND: Application of the traditional diagnostic criteria validated for exercise testing may be inappropriate when dobutamine infusion is associated with radionuclide ventriculography (RNV). The objective of this study was to establish appropriate diagnostic criteria for evaluation of dobutamine stress testing with RNV for the detection of early postinfarction ischemia. METHODS: RNV was performed at baseline and during dobutamine infusion in 10 control subjects and in 30 patients who were studied within 1 week after uncomplicated myocardial infarction. Several quantitative parameters including left ventricular ejection fraction (EF), regional EF, and absolute change in global and regional EF were tested. In addition, regional wall-motion changes were scored by visual analysis. The limit of normal response for each quantitative parameter was defined as the 95th percentile of the distribution observed in control subjects. On the basis of predischarge clinical evaluation and exercise stress testing, patients were grouped as having evidence of residual ischemia (group 1, 15 patients) or no evidence of ischemia (group 2, 15 patients). Sensitivity, specificity, and accuracy in the detection of postinfarction ischemia were calculated for each parameter, and empiric receiver-operating characteristic curves were generated. RESULTS: The limits of the normal response to dobutamine infusion in the control subjects were found to be a 7.5% and a 12.5% increase in global EF at low and high dose, respectively. Median baseline EF was significantly lower in patients than in control subjects (Mann-Whitney U test: P < .001). There were no differences in resting EF between group 1 and group 2 patients. However, median high-dose EF and increase in EF were significantly lower in group 1 as compared with group 2 patients (56% vs 72% and 5% vs 17%, respectively; Mann-Whitney U test: P < .0001 for both). A biphasic (up-and-down) response with initial increase at low dose followed by decrease or no change in EF at high dose was observed in 66% of group 1 patients and only in 13% of group 2 subjects. The best criterion for detection of postinfarction ischemia was a change of <12.5% in global EF at high-dose dobutamine infusion, with 80% predictive accuracy. The traditional criterion, a change of <5 % or a decrease in EF, yielded an accuracy of only 63%. The association of a change of <12.5% with a biphasic response increased accuracy to 87%, with a sensitivity of 93% and a specificity of 80%. Quantitative regional wall motion analysis showed greater diagnostic accuracy than visual wall motion scoring (83% vs 70%). CONCLUSIONS: Application of appropriate criteria and stepwise quantitative analysis of RNV during dobutamine infusion may increase accuracy in the detection of early postinfarction ischemia.  相似文献

Objective

To evaluate the diagnostic performance of stress perfusion cardiac MR (CMR) for detecting significant CAD (≥70% narrowing) in comparison with invasive coronary angiography (ICA) as a reference standard.

Methods

Examinations of 54 patients who underwent both stress perfusion CMR and ICA for investigation of CAD between 2007 and 2009 were evaluated. The CMR protocol included dipyridamole stress and rest perfusion, stress and rest cine MRI for assessment of ventricular function and delayed gadolinium enhancement for assessment of myocardial viability and detection of infarction. CMR interpretation was performed by 2 observers blinded to the results of ICA and the clinical history.

Results

From a total of 54 patients, 37 (68.5%) showed significant CAD in 71 coronary territories. A perfusion defect was detected in 35 patients and in 69 coronary territories. Individual stress perfusion CMR evaluation showed the highest accuracy (83%) of the CMR techniques. The combined analysis using all sequences increased the overall accuracy of CMR to 87%.

Conclusion

Combination of perfusion and cine-MR during stress/rest, associated to delayed enhancement in the same protocol improves CMRI diagnostic accuracy and sensitivity for patients with significant coronary stenosis, and may therefore be helpful for risk stratification and defining treatment strategies.  相似文献

心肌灌注显像(MPI)在冠心病的诊断、危险度分层及预后评价中扮演着越来越重要的角色,冠状动脉钙化积分(CACS)可协助对冠心病患者进行进一步的危险度分层,但两者有各自的局限性和不足。将两者联合应用可以相互弥补不足,为临床提供更多的信息。行SPECT/CT心肌灌注显像时,需进行衰减矫正CT(CTAC)扫描,若采用心电门控的呼气末屏气螺旋CT扫描,则在用于MPI衰减矫正的同时又可用于CACS测定,实现一站式检查,在减少患者辐射剂量的同时又可为临床提供更多的信息。笔者综述了CACS、MPI及两者联合应用的临床价值,并对一站式采集MPI与CACS的最新进展进行了综述。  相似文献

心绞痛患者冠脉造影与TET结果对比研究   总被引:3,自引:1,他引:2  
目的:以冠状动脉造影为依据与心电图运动试验(TET)比较,探讨TET的假阳性因素。方法:对270例胸痛患者进行TET和冠状动脉造影(CAG)检查,收集病史及辅助检查资料。对比TET和CAG的结果,计算TET的敏感性、特异性和准确性;对比TET真阳性组和假阳性组的各项临床资料特点并进行统计学分析。结果:本组病例TET诊断冠心病的敏感性为76.3%,特异性为65.5%,准确性为69.6%;假阳性组女性患者例数较多;具有典型胸痛症状、高脂血症、吸烟史、饮酒史、冠心病家族史的例数较少;运动中ST段的下降多发生于Ⅱ、Ⅲ、aVF导联,ST段下降>0.2mV以及运动中出现心绞痛的例数较少,两组差别有统计学意义。结论:性别、典型缺血性胸痛症状、冠心病的某些危险因素、运动后心电图发生变化的部位和程度、运动中胸痛症状的出现,是造成TET假阳性结果的误诊因素。  相似文献

Exercise-induced increases in pulmonary uptake of thallium-201 (201Tl) have been associated with exercise-induced myocardial dysfunction. To evaluate this phenomenon more replicably, a quantitative semi-automated computer program was used to generate, from anterior exercise and delayed views, lung-myocardial ratios (LMR) of201Tl uptake in 78 patients [40 normal, 38 with coronary artery disease (CAD)]. Patients with CAD had a significantly higher mean exercise lung myocardial ratio (EXLMR) than normals (30.8 vs. 27.3;P < 0.003). In patients with adequate exercise (85% of an age-adjusted maximal heart rate), the EXLMRs of CAD patients were significantly higher than those of normals (29.7 vs. 25.5;P=0.003). However, this difference between CAD and normal patients was not apparent in a patient subgroup with submaximal exercise levels (< 85% of an age-adjusted maximal heart rate). In both normal and CAD patients, EXLMR decreased with increasing exercise levels (r=-0.555;P=0.007). In patients with201Tl scans lacking visually defined perfusion defects (visually normal), an elevated LMR detected 60% of CAD cases with 81% specificity. A considerably elevated EXLMR in patients achieving adequate exercise should suggest the presence of CAD, even if there are no visually apparent cardiac perfusion defects. With submaximal exercise, however, the EXLMR is not a useful discriminator between CAD patients and normals.  相似文献

Background  Earlier studies have suggested a modest accuracy of stress thallium 201 myocardial perfusion imaging (MPI) for the diagnosis of coronary artery disease (CAD) in women. The accuracy of stress MPI with technetium 99m tetrofosmin has not been studied in women. The aim of this study was to assess the accuracy of stress Tc-99m tetrofosmin MPI for the diagnosis and localization of CAD in women. Methods and Results  We studied 88 women who underwent exercise or dobutamine stress Tc-99m tetrofosmin tomography and coronary angiography within 3 months. Significant CAD was defined as a stenosis 50% or greater in diameter in at least 1 major epicardial coronary artery. Myocardial perfusion abnormalities were detected in 44 of 53 patients with significant CAD and in 7 of 35 patients without significant CAD (overall sensitivity, 83% [95% confidence interval (CI), 73%-93%]; specificity, 80% [95% CI, 67%-93%]; and accuracy, 82% [95% CI, 74%-90%]). The sensitivity was 72% (18/25) in patients with single-vessel CAD and 93% (26/28) in patients with multivessel CAD. Perfusion abnormalities were detected in 2 or more vascular distributions in 20 of 28 patients with multivessel CAD and in 4 of 60 patients without multivessel CAD (sensitivity for the identification of multivessel CAD, 71% [95% CI, 55%-88%]; specificity, 93% [95% CI, 86%-98%]; and accuracy, 86% [95% CI, 79%-93%]). The sensitivity, specificity, and accuracy were 82%, 84%, and 83%, respectively, for the diagnosis of CAD in the left anterior descending artery; 77%, 84%, and 81%, respectively, for CAD in the right coronary artery; and 74%, 80%, and 78%, respectively, for CAD in the left circumflex artery. Conclusion  Stress Tc-99m tetrofosmin MPI is an accurate noninvasive technique for the diagnosis and localization of CAD in women. Left ventricular function is one of the most important determinants, if not the most important determinant, of outcome in paients with coronary artery disease. The ability of radionuclide angiography to assess resting and exercise ejection fraction accurately and reproducibly has been shown to be a critical determinant of survival in large-scale studies of survivors of myocardial infarction, as well as patients with chronic stable angina. In addition, several centers have demonstrated that the exercise ejection fraction is an extremely valuable (and perhaps the most valuable) noninvasive parameter in predicting survival among patients with coronary artery disease. The prognostic insights gained from the exercise ejection fraction add incremental predictive information to the coronary anatomic information obtained from coronary arteriography, especially in patients with multivessel disease and those with left ventricular dysfunction at rest.  相似文献
