Objective Repair of incisional (and large ventral) abdominal hernias pose a challenge for many surgeons. Numerous techniques have been described. This series reports a single surgeon’s experience using the retromuscular (pre‐peritoneal) mesh repair technique. Method Data was collected on 70 consecutive incisional (n = 23) and ventral (n = 47) hernia repairs using retromuscular (pre‐peritoneal) lightweight polypropylene mesh. Results The median follow‐up was 26 months (range 10 – 66 months). Overall recurrence rate was 1.4%. There were seven major (10%) and four minor (7%) post‐operative infections. One patient developed post‐operative seroma requiring percutaneous aspiration, and one patient had chronic pain. Conclusion Retromuscular mesh repair is a good alternative to traditional onlay mesh repairs. Morbidity and outcomes are acceptable and comparable with literature, with lower rates of seroma formation and hernia recurrence.  相似文献

INTRODUCTIONLaparoscopic intraperitoneal onlay mesh (IPOM) repair has become a widely accepted operative technique for incisional hernias. However, tack fixation poses the risk of adhesions and injury to the intestine. We report the case of spiral tacks adherent to the small bowel after IPOM repair for incisional hernia.PRESENTATION OF CASE64 years old male patient who underwent laparoscopic IPOM repair for incisional hernia 1 year after open sigmoid resection. A laminated polypropylene mesh was fixed with titanium spiral tacks. 4 years later, elective open cholecystectomy was performed. Two spiral tacks integrated in the seromusular layer of the small bowel were encountered. Tacks were removed and bowel lesions oversewn with interrupted seromuscular stitches.DISCUSSIONAccording to the current literature, complications related to metal spiral tacks in IPOM mesh repair such as intestinal perforation or strangulation ileus seem to be rare. To our knowledge, spiral tacks adherent to the intestine have not yet been published to date. Alternative techniques for mesh fixation are transfascial sutures with single stitches, continuous sutures or fibrin glue, as already used in TAPP and TEP procedures for inguinal hernia repair. The ideal and safest technique for mesh fixation in IPOM repair for incisional hernias remains controversial.CONCLUSIONSpiral tacks used for intraperitoneal mesh fixation can lead to adhesions and bowel lesions. Sutures, absorbable tacks or fibrin glue are alternatives for mesh fixation. Further clinical trials are needed to evaluate the safest technique of laparoscopic IPOM incisional hernia repair.  相似文献

Various techniques for repair of an incisional hernia are available for the surgeon. Conventional suture techniques are quick and easy to perform but they are associated with an unacceptable rate of recurrence and therefore should only be used in exceptional cases. An underlying systemic disturbance of collagen metabolism is assumed to exist in patients with an incisional hernia. In such patients the mechanisms of wound healing and remodeling of the abdominal wall following laparotomy are insufficient, which necessitates reinforcement of the abdominal wall with a non-resorbable alloplastic mesh prosthesis to enable a long-term cure. The implantation of such meshes can be carried out laparoscopically or by an open approach. The gold standard of open repair techniques is the retromuscular placement of a mesh prosthesis. The retromuscular mesh placement as a reinforcement of the abdominal wall (augmentation) must be categorically differentiated into the abdominal wall replacement by mesh bridging. In this technique the mesh is likewise placed in the retromuscular space, however a complete closure of the ventral fascia is not necessary. Retromuscular augmentation enables an extra-peritoneal placement of the prosthesis, an optimization of tissue integration by plane coverage of the prosthesis by well vascularized muscular tissue and a sufficient overlap in cranio-caudal and lateral directions. Mesh fixation is best made with absorbable suture material but is better suited for technical simplification. The use of a prophylactic drainage should be decided depending on the individual patient’s risk factors, because sufficient evidence-based data are currently not available. If augmentation is not possible bridging is necessary and then the mesh has to be fixed without underlying support. Current data reveal that the recurrence rate following incisional hernia repair by retromuscular mesh augmentation has decreased promisingly in comparison to simple suture techniques. In total the recurrence rate following retromuscular mesh placement ranges between 2 and 12%. Current results of prospective randomized multicentre trials are not available. However, it is to be expected that further development of mesh materials as well as improvement of surgical techniques with avoidance of typical pitfalls will lead to further reduction of the recurrence rate with an improvement in patient satisfaction.  相似文献

目的 分析开放式补片修补术治疗腹壁切口疝的临床疗效。方法 回顾性分析我院于2013年10月至2018年3月行开放式补片修补术的210例腹壁切口疝病人的临床资料。分析不同术式和补片固定方式,术后血清肿、切口感染、补片感染、慢性疼痛、肠漏、复发以及死亡的发生。结果 术后血清肿25例(11.90%),切口感染14例(6.67%),补片感染3例(1.43%),慢性疼痛7例(3.33%),肠漏3例(1.43%),复发7例(3.33%),死亡2例(0.95%)。采用IPOM术式病人术后仅有血清肿并发症发生率低于Sublay术(P<0.05),差异有统计学意义,其余并发症发生和复发率差异无统计学意义(P>0.05)。Sublay术式中加强法与桥接法术后并发症差异无统计学意义(P>0.05)。采用缝线+不可吸收疝钉组与缝线+可吸收疝钉组术后慢性疼痛和复发率无统计学差异(P>0.05)。结论 开放式补片修补术治疗腹壁切口疝安全有效。  相似文献

目的:探讨腹壁切口疝病人的无张力手术治疗方法和疗效.方法:回顾性分析我院2000年7月至2008年6月间收治的215例应用合成补片修补腹壁切口疝病人的临床资料.结果:215例中101例采用IPOM方法,有1例复发,复发率0.99%;97例采用Stoppa方法,有3例复发,复发率3.09%;17例采用肌前修补法,有3例复发,复发率17.6%.在21例复发疝病人中,16例采用了IPOM方法,无再复发病例;5例采用了Stoppa方法,有1例再复发,复发率20%.结论:IPOM方法和Stoppa方法的手术适应症宽、复发率低.IPOM方法对于复发的切口疝病人有更好的治疗效果.术者的经验和方法的正确性对修补的成功均有重要意义.  相似文献

In comparison to the conventional technique of incisional or umbilical hernia repair with sublay mesh augmentation, incisional hernias in obese patients can be surgically treated with minor surgical trauma by laparoscopic intraperitoneal onlay mesh (IPOM) repair. However, although shortened operation time, hospital stay and faster postoperative reconvalescence might be possible with IPOM repair, the economic calculation including mesh costs is significantly higher. In this study the two operation techniques were compared and the perioperative advantages and disadvantages of both methods were analyzed based on the German diagnosis-related groups (DRG) system.  相似文献

Purpose

To evaluate the efficacy of negative pressure therapy for superficial and deep mesh infections after ventral and incisional hernia repair by a prospective monocentric observational study.

Methods

During a 6-year period, 724 consecutive open ventral and incisional hernia repairs were performed. Pre- and intraoperative data as well as postoperative complications were prospectively recorded. In case of wound infection, negative pressure therapy (NPT) was our primary treatment.

Results

Sixty-three patients (8.7 %) were treated using negative pressure therapy after primary ventral and incisional hernia repair. Infectious complications needing NPT occurred in 54 patients in the retromuscular group (54/523; 10.3 %), none when laparoscopically treated and in 9 patients (9/143; 6.3 %) treated by an open intraperitoneal mesh technique. Considering outcome, all meshes were completely salvaged in the retromuscular mesh group after a median of 5 dressing changes (range, 2–9), while in the intraperitoneal mesh, group 3 meshes needed complete (n = 2) or partial (n = 1) excision. Mean duration to complete wound closure was 44 days (range, 26–63 days).

Conclusion

NPT is a useful adjunct for salvage of deep infected meshes, particularly when large pore monofilament mesh is used.  相似文献

Incisional hernia repair sometimes requires intraperitoneal implantation of a mesh. This becomes necessary when the hernia opening is large, in particular, in patients with a low abdominal wall surface/wall defect surface (AWS/WDS) ratio, in large boundary incisional hernias where the proximity to bone structures or cartilage often complicates retromuscular mesh implantation and in multi-recurrent incisional hernias that are sometimes characterised by an actual loss of abdominal wall tissue. The authors report on the results of a series of 100 incisional hernias treated between 1999 and 2006 using the open technique to implant an intraperitoneal mesh (Parietex Composite). Mean follow-up time was 42 months (range 12–96 months). The mean wall defect surface was 95 cm2 (range 60–210 cm2). Twelve percent of patients suffered minor complications: 5 seromas (5%), 3 haematomas (3%) and 4 parietal suppurations (4%). No mesh had to be removed. The recurrence rate was 6%. At 6 months after surgery, no patient lamented pain or discomfort due to foreign body sensation. None of these patients presented intestinal occlusion or enterocutaneous fistulae. In conclusion, it is our opinion that the mesh should be implanted in direct contact with the viscera only where absolutely necessary, i.e., when it cannot be implanted in the retromuscular area without creating excessive parietal tension. Our experience with PC mesh, over the short-to-medium term, was positive. Naturally, further studies are required to evaluate long-term biocompatibility.  相似文献

Incisional hernias remain a surgical challenge when balancing surgical morbidity, functional restoration, and risk of recurrence. Laparoscopic intraperitoneal onlay mesh (IPOM) placement reduces postoperative wound infections and allows fast patient recovery. Yet, current IPOM techniques do not achieve closure of the midline hernia gap, thereby increasing the risk of persistent mesh bulging with poor abdominal wall function. We propose a novel triple-step hernia repair technique that includes tension-free midline reconstruction. It is achieved through laparoscopic dorsal component separation and laparoscopic suture closure of the midline with a 1.0 polydioxanone suture sling. Combining dorsal abdominal wall component separation, a midline closure with adequate suture strength, and IPOM reinforcement merges the benefits of open and laparoscopic hernia repair. This triple-step technique allows static and functional laparoscopic abdominal wall reconstruction.  相似文献

BACKGROUND: Large incisional hernias are mainly repaired today by tension free implantations of prosthetic meshes using various placement methods. The advantages of the intraperitoneal open technique (IPOM) using a polypropylene mesh that is coated with ePTFE on the side facing the intestine, are described. METHODS AND RESULTS: 62 patients underwent an incisional hernia operation with the intraperitoneal positioning of a prosthetic mesh. The follow-up examinations after a range of 16.1 months showed a hernial recurrence rate of 6.4 %. A part of the resulting mesh infections (11.2 %) healed without surgical removal of the mesh. Clinical complications due to adhesion formation were not observed. CONCLUSION: The tissue sparing intraabdominal positioning technique simplifies the necessary overlap using healthy tissue as a prerequisite for a sufficient hernia repair exploiting the intraabdominal pressure.  相似文献

Background: Abdominal lipectomy is becoming an increasingly common surgical procedure in patients with esthetic deformities resulting from massive weight loss induced by bariatric surgery. Sometimes a midline incisional hernia coexists with the pendulus abdomen. Herein presented is a technique to perform a retromuscular mesh repair of the incisional hernia while sparing the umbilicus. Methods: The abdominal lipectomy with concomitant retro-muscular mesh repair of a midline incisional hernia is done sparing the vascular supply of the umbilicus on one side only. Results: 5 consecutive women with pendulus abdomen resulting from bariatric surgery-induced massive weight loss and concomitant midline incisional hernia underwent abdominal lipectomy and incisional hernia mesh repair. Mean BMI was 28.6 kg/m2 (range 26–35), one patient was a smoker, and another had type 2 diabetes requiring oral hypoglycemic agents. Two patients had had a previous incisional hernia repair with intraperitoneal mesh. One patient had partial necrosis of the umbilicus and another experienced necrosis of only the epidermis that recovered fully. Conclusions: The umbilicus can be safely spared during abdominal lipectomy with concomitant midline incisional hernia mesh repair. Recurrent incisional hernia and common risk factors for wound healing such as diabetes and obesity increase the risk of umbilical necrosis.  相似文献

Abstract Complex ventral hernia is the ventral hernia which is huge and (or) combined with a lot of complications. The cases of complex ventral herniain should be classified and underwent therapy measures respectively. For the cases of huge ventral hernia, initiative volume reduction should be performed during operation; the intra-abdominal pressure should be detected after operation to prevent ACS (abdominal compartment syndrome); the laparoscopic technique should be used for the fixation of the mesh. For the multiple ventral hernia, IPOM (intraperitoneal onlay mesh technique) should be performed to repair hernia rings and occult hernia could be found by the laparoscope. For the irreducible and incarcerated hernia, hybrid technique (laparoscope combined with open surgery) with part-absorbable meshes should be performed to prevent the postoperative infection. For the recurrent ventral hernia, IPOM should be also performed and occult hernias were detected. For the cases of ventral hernia combined with infection and intestinal fistula, the debridement by open operation should be performed in order to remove infection; the part-absorbable meshes should be placed to repair the defects; the laparoscope should be performed to prevent the the accident injury; the antibiotics should be applied in perioperative period. For the ventral hernia located in specific positions (including marginal ventral hernia), the tacks and sutures fixation should be used to ease the difficulty level of mesh fixation.  相似文献

BACKGROUND: Recurrent incisional hernia repair is associated with high recurrence and wound complication rates. METHODS: The clinical courses of patients who underwent recurrent incisional hernia repair via retromuscular mesh placement with concomitant panniculectomy at a university teaching hospital from 1999 to 2004 were reviewed retrospectively. Postoperative evaluation included a quality of life survey. RESULTS: Forty-seven patients (13 male, 34 female) with an average body mass index of 34.4 kg/m2, an average midline hernia defect of 31.4 cm, and at least 1 and on average 2.5 previous repair attempts underwent hernia repair. Wound infections occurred in 4 patients (8%) and seromas requiring aspiration occurred in 1 patient (2%). Four patients (8%) had re-recurrences of their hernias. All patients rated the postoperative appearance of their abdomen as at least satisfactory. CONCLUSIONS: Recurrent incisional hernia repair with a retromuscular mesh and panniculectomy has low recurrence and wound complication rates and excellent patient satisfaction.  相似文献

Purpose

This study aims to compare the outcomes of posterior component separation and transversus abdominis release (PCSTAR) with the open anterior component separation (OACS) technique. OACS, first described by Ramirez et al. (Plast Reconstr Surg 86(3):519–526, 1990), has become an established technique for local myofascial advancement in abdominal hernia surgery. PCSTAR, described by Novitsky et al. (Am J Surg 204(5):709–716, 2012), is being used more frequently and is rapidly becoming the technique of choice in complex ventral hernia repair.

Methods

Analysis was conducted according to PRISMA guidelines. A systematic search of the MEDLINE, EMBASE and Pubmed databases was performed. Studies reporting exclusively on midline ventral hernia repair were reviewed. Studies describing PCSTAR were selected and compared to matched studies describing OACS. Meta-analysis was used to compare outcomes between the two-pooled groups.

Results

Seven studies describing 281 cases of PCSTAR for midline incisional hernia using a retromuscular mesh placement were identified. Six comparable studies describing 285 cases of OACS and retromuscular mesh placement were identified from the same search. Pooled analysis demonstrated a hernia recurrence rate of 5.7% (3.0–8.5) for PCSTAR and 9.5% (4.0–14.9) for OACS. Comparative analysis demonstrated no significant difference between hernia recurrence rate (p?=?0.23). The use of bridging mesh was not significantly reduced by the use of PCSTAR (3.1%) when compared to ACS (7.5%) (p?=?0.22). No significant difference was found in wound complication rates between PCSTAR and OACS, respectively, ‘superficial’ 10.9 vs 21.6% (p?=?0.15); and ‘deep’ 9.5 vs 12.7% (p?=?0.53).

Conclusions

These data suggest PCSTAR have comparable outcomes to OACS. This analysis is limited by the lack of comparative studies and heterogenicity in the OACS group.

Incisional hernias occur in 5-10% of patients who have undergone laparotomy and are associated with high morbidity and significant socioeconomic costs. Techniques for reinforcing and/or replacing the abdominal wall with alloplastic meshes have reduced the recurrence rate in comparison to suture techniques from about 40% to less than 10%. A number of mesh types and surgical repair procedures are available, namely the onlay, inlay, sublay, underlay, and intraperitoneal onlay mesh (IPOM) techniques. Evolving strategies include precise criteria for incorporating patient body type, risk factors for recurrence, hernia morphology, and the available biomaterials into the planning of the surgical approach. The authors herein present an overview of the current surgical trends, focusing on mesh reinforcement (sublay technique) and mesh replacement (IPOM technique). Additionally, they review a classification of incisional hernias that is self-explanatory, practicable in routine clinical practice, and based on the cornerstones of morphology, hernia size, and risk factors for recurrence. Evidence for the indications and limitations of the main surgical repair techniques are illustrated and discussed.  相似文献

Background

Parastomal hernias (PHs) are frequent complications of enterostomies. We aimed to evaluate our outcomes of open PH repair with retromuscular mesh reinforcement.

Methods

From 2006 to 2013, 48 parastomal hernias were repaired in 46 consecutive patients undergoing open retromuscular repair. Surgical technique included stoma relocation, retromuscular dissection, posterior component separation, and retromuscular mesh placement. All stomas were prophylactically reinforced with cruciate incisions through mesh. Main outcome measures included demographics, perioperative details, wound complications (classified according to the CDC guidelines), and recurrences.

Results

There were 24 male and 22 female patients with a mean age of 61.8 and body mass index (BMI) of 31.7 kg/m2. Twenty-four patients had recurrent PH with an average of 3.8 prior repairs. Ostomies included 18 colostomies, 20 ileostomies, and 10 ileal conduits. Thirty-two patients had a concurrent repair of a midline incisional hernia. All patients underwent mesh repair with either biologic (n?=?29), lightweight polypropylene (n?=?15), or absorbable synthetic mesh (n?=?2). There were 15 superficial surgical site infections (SSIs) and 6 deep SSIs. There was one case of an ischemic ostomy requiring surgical revision. No mesh grafts required removal and there were no mesh erosions. At a mean follow-up time of 13 months, five patients (11 %) developed a recurrence; three patients required re-repair.

Conclusion

In this largest series of complex open repairs with retromuscular mesh reinforcement and stoma relocation, we demonstrate that this results in an effective repair. This technique should be considered for complex parastomal hernia repair.  相似文献

The aim of the present case report is to present the diagnostic and therapeutic challenge of intercostal incisional hernia. We report on a female patient with leftsided intercostal incisional hernia between the eleventh and twelfth rib due to preceding lumbar incision for tumor nephrectomy. Because of its infrequence, diagnosis was established late although simple clinical examination and ultrasound investigation displayed the hernia. At laparotomy, a 5×5 cm2 fascial defect with a colonic sliding hernia was found. Hernia repair using permanent mesh reinforcement in the retromuscular position is described. Abdominal incisional hernia in the intercostal region is rare and therefore easily overlooked. As with other incisional hernias, the hernia repair using mesh implantation in the retromuscular region is technically feasible and represents the treatment of choice.  相似文献

复杂腹壁疝是巨大和(或)合并一系列并发症的腹壁疝。临床应对复杂腹壁疝进行分类并分别采用相应的腹腔镜治疗策略。对于巨大腹壁疝,术中应主动减容,腹腔镜固定,术后监测腹压,防治腹腔间室综合征;对于复发和多发腹壁疝,采用腹腔镜探查避免漏诊隐匿疝,同时腹腔内修补多发疝环;对于难复性或嵌顿性腹壁疝,腹腔镜联合开放术式处理疝内容物,应用部分可吸收材料修补,防治术后感染;对于合并感染、肠瘘的腹壁疝,先用腹腔镜探查,然后开放清创去除感染灶,应用部分可吸收材料修补缺损,围手术期应用抗生素;对于特殊位置的腹壁疝(包括边缘腹壁疝),采用腹腔镜联合吊线缝合的方式降低固定补片的难度。  相似文献

Since the onset of an incisional hernia is caused by the biological problem of forming stable scar tissue, the mesh techniques are now the methods of choice for incisional hernia repair. Polypropylene is the material most widely used for open mesh repair. New developments have led to low-weight, large-pore polypropylene prostheses, which have been adapted to the physiological requirements of the abdominal wall and permit a reliable tissue integration. These meshes make it possible for a scar net to form rather than a stiff scar plate, thus helping to avoid the complications encountered with the use of earlier meshes. The ideal position for the mesh seems to be the retromuscular underlay position, in which the force of abdominal pressure holds the prosthesis tightly against the deep surface of the muscles. The retromuscular underlay repair technique has yielded the lowest incidence rates for recurrence: around 10% even after long-term follow up. Analysis of the failures after open mesh repair suggests that inadequate size of the mesh with insufficient overlap at the edges is the main reason for recurrence. An overlap of at least 5–6 cm all round must therefore be considered mandatory for successful reinforcement of the abdominal wall. Open mesh repair, particularly with modern low-weight polypropylene meshes applied by the retromuscular underlay technique, offers excellent results in the treatment of incision hernias, even in long-term follow-up studies.  相似文献
