This retrospective study was performed to assess the potential of magnetic resonance (MR) imaging for demonstrating various types of lipomatous tumors and tumors with fatty component and to compare the results of MR with those of computed tomography (CT). MR examinations of 17 patients with 18 lipomatous tumors (16, benign; two, liposarcoma) and two patients with fibrosarcomas were reviewed; CT scans were available for comparison in all patients. In the 16 benign lesions (12 benign lipomas, two ovarian dermoid cysts, and two renal angiomyolipomas), the fatty component of the tumors was readily demonstrated by both MR and CT. The T1 and T2 relaxation times and spin density of benign lipomatous tumors were in a range similar to those of normal subcutaneous fat. Differentiation between lipomas and liposarcomas was achieved with both MR and CT. On MR images using a short repetition time (TR = .5 sec), liposarcomas (long T1) were imaged with a lower MR intensity than lipomas (short T1).  相似文献

Soft tissue liposarcoma: histological subtypes,MRI and CT findings   总被引:12,自引:0,他引:12  
PURPOSE: Liposarcoma is the second most common malignancy of soft tissues. The commonly accepted classification of liposarcoma includes five basic histological categories: well-differentiated, myxoid, round cell, dedifferentiated and pleomorphic liposarcoma. The clinical behaviour of liposarcoma closely reflects its histological appearance, so that to identify the histological subtypes is very important for both prognosis and therapy. The aim of this study was to ascertain whether the histological features of liposarcoma subtypes can be correlated with MRI (Magnetic Resonance Imaging) and CT (Computed Tomography) findings by retrospectively evaluating nineteen cases of histologically-proved liposarcoma. MATERIALS AND METHODS: The MRI examinations performed over the past eight years on nineteen patients affected by liposarcoma were retrospectively reviewed. All patients underwent ultrasound and MRI examination; T2 and T1-weighted sequences were available in all cases and fat-saturated sequences in 4 cases; all patients were administered paramagnetic contrast material. CT scans were obtained in twelve patients. All patients had a biopsy, surgical resection and histology. RESULTS: The study group had 7 well-differentiated, 8 myxoid, 3 pleomorphic and 1 round cell liposarcoma. Well-differentiated liposarcomas had largely lipomatous appearance on both CT and MRI, typically with septa and areas showing high signal intensity on T2w MR images and low signal intensity on T1w images, and slightly hypodense compared to the muscle in CT, representing the sarcomatous areas. Myxoid liposarcomas were mildly heterogeneous with typical high signal intensity on T2w images and isointense to the muscle in T1w images, with lacy or linear septa of fatty tissue in six cases. The pleomorphic and round-cell subtypes demonstrated marked heterogeneity on MR images, with areas of necrosis and heterogeneous contrast enhancement, indistinguishable from other high-grade sarcomas. DISCUSSION AND CONCLUSIONS: Well-differentiated liposarcoma may be distinguished from other types of liposarcoma by its largely lipomatous appearance. Myxoid liposarcoma may be distinguished on the basis of its homogeneous or mildly heterogeneous structure due to the large amounts of the extracellular myxoid material that give it its typical MR appearance. Both well-differentiated and myxoid liposarcomas, the most common types accounting for about 50% of all liposarcomas, have a more favourable clinical behaviour than the other histological types. Differentiation of these from the other histological types of liposarcoma therefore has a high significance for prognosis and therapeutical approach. On the basis of our experience and of the literature, we believe that diagnostic imaging and in particular the MR examination may lead to a correct diagnosis.  相似文献

The present limited retrospective study was performed to assess MR imaging of lipomatous tumours of the musculoskeletal system and to evaluate the potential of the T2 short tau inversion-recovery (STIR) technique for differentiating lipomas from liposarcomas. Magnetic resonance imaging of 12 patients with lipomatous tumours of the musculoskeletal system (eight benign lipomas, three well-differentiated liposarcomas and one myxoid liposarcoma) were reviewed. Benign lipomas were usually superficial and showed homogeneity on T1- and T2-weighted spin echo sequences. Full suppression at T2 STIR was readily demonstrated. In contrast, the liposarcomas in the present series were all deep-seated. Two well-differentiated liposaromas showed homogeneity at long and short relaxation time (TR) but failed to show complete suppression at T2 STIR. One case of well-differentiated liposarcoma (dedifferentiated liposarcoma) and one of myxoid liposarcoma showed mild and moderate heterogeneity at T1 and T2, respectively, and posed no difficulty in being diagnosed correctly. In conclusion, short and long TR in combination with T2 STIR show promise in differentiating benign from malignant lipomatous tumours of the musculoskeletal system, when taken in combination with the position of the tumour.  相似文献

Objective. To investigate the septum-like structures in predominantly lipomatous tumors, by correlating fat-suppressed MR images with histopathologic findings. Design and patients. The MR findings of three cases of well-differentiated liposarcoma (atypical lipoma), one case of lipoma-like component of dedifferentiated liposarcoma, and nine cases of lipoma were analyzed. T1-, T2-, and fat-suppressed T1-weighted images after Gd-DTPA administration were obtained. Surgical specimens from five patients (four with liposarcoma and one with lipoma) were also scanned with a MR unit, and compared with the pathologic findings. Results and conclusions. Enhancement features of lipoma and liposarcoma were well visualized on fat-suppressed T1-weighted images after Gd-DTPA administration. The septum-like structures of liposarcoma are thick and enhanced considerably, while septa of lipoma are thin and enhanced only slightly. Pathologically, the septum-like structures of liposarcoma contained muscle fibers and the septa of lipoma represented fibrous capsule. Identification of well-enhanced septa in a predominantly lipomatous tumor helps to differentiate malignant tumors from lipomas. As the septum-like structures of liposarcoma contain a skeletal muscle component the tumor might need more extensive surgical procedures including resection of adjacent muscles.  相似文献

Myxoid liposarcoma: appearance at MR imaging with histologic correlation.   总被引:19,自引:0,他引:19  
Although myxoid liposarcoma is a subtype of liposarcoma, it may be difficult to establish the correct diagnosis with magnetic resonance (MR) imaging due to the lack of fat signal intensity. Without the administration of gadolinium contrast material, the tumor may even mimic a cystic tumor. A spectrum of MR imaging abnormalities occur in myxoid liposarcoma, depending on the amount of fat and myxoid material, the degree of cellularity and vascularity, and the presence of necrosis. Most myxoid liposarcomas have lacy or linear, amorphous foci of fat. Some myxoid liposarcomas appear to be cystic at nonenhanced MR imaging, although they enhance like other solid masses at contrast material-enhanced MR imaging. The enhancing areas within the tumor represent increased cellularity and vascularity; the nonenhancing areas represent necrosis, reduced cellularity, and accumulated mucinous material. Gadolinium-enhanced imaging is important in differentiating myxoid liposarcoma from benign cystic tumors. Characterization of the tumor with MR imaging plays an important role in the management of myxoid liposarcoma.  相似文献

目的探讨四肢黏液样脂肪肉瘤的临床和MRI特点。方法对7例经病理证实的四肢黏液样脂肪肉瘤的临床和MRI资料进行了回顾性分析。患者年龄41-59岁,中位年龄51岁。结果3例肿瘤发生在大腿,2例在小腿,1例在足部,1例在肩部;6例位置深在,1例位置表浅。与肌肉相比病变在T1WI上主要呈等或稍低信号,6例可见“线”状、“花边”状或“云絮”状高信号;在T2WI上病变与脂肪相比大部分区域呈高信号;增强扫描病变均呈不均匀的显著强化;病变内部可见分隔,边缘清楚,周围无明显水肿,均未侵犯邻近骨骼。结论四肢黏液样脂肪肉瘤MRI表现有一定特点,在T1WI上主要呈等或稍低于肌肉的信号,并可见脂肪成分所致的“线”状、“花边”状或“云絮”状高信号,在T2WI上则以高于脂肪的信号为主,增强扫描呈不均匀的显著强化。  相似文献

目的:探讨四肢黏液性脂肪肉瘤的MR表现及鉴别诊断。方法:回顾性分析12例手术病理证实的四肢黏液性脂肪肉瘤患者的MR表现,并与其它四肢软组织肿瘤表现相鉴别。结果:黏液型脂肪肉瘤多位于四肢深部肌肉间隙内;MR上以T1wI低信号、T2wI高信号为主;脂肪抑制像肿块信号强度无减低,组织学上以黏液成分为主.脂肪含量少,增强后可见明显强化的实性成分和粗大的间隔影,与其它四肢软组织肿瘤可以鉴别。结论:MR可对四肢软组织黏液性脂肪肉瘤的诊断及鉴别诊断提供有价值的信息。  相似文献

To investigate the histological features of foci, which showed signal intensities different from fat by magnetic resonance (MR) imagings in well-differentiated lipoma-like liposarcomas and a case of lipoma, a retrospective review of these lipomatous tumors was performed to correlate MR and histological findings. Microscopic findings revealed that these foci were also composed of lipomatous tissue. Well-differentiated liposarcoma and benign lipoma associated with such foci should be differentiated from dedifferentiated liposarcoma based on the histological findings.  相似文献

Lipomatous tumors generally have signal characteristics that allow them to be diagnosed with great accuracy by means of magnetic resonance imaging. These tumors usually have signal intensities similar to those of subcutaneous fat on both T1- and T2-weighted spin-echo images. Previous reports have not, to the authors' knowledge, described the appearance of lipomatous tumors on images obtained with a short-inversion-time inversion-recovery (STIR) sequence, which can be used to suppress signal from fat. Three lipomatous tumors (two liposarcomas and one lipoma) with signal characteristics unlike those of normal subcutaneous fat at all pulse sequences are presented.  相似文献

There is a broad spectrum of lipomatous tumors that involve soft tissues including ordinary lipomas, variants of lipomas, heterotopic lipomas, lipomatosis, hibernomas and liposarcomas. The recognition of these tumors is important to avoid diagnostic pitfalls. The MR imaging appearance of the lesion, including location, shape and internal architecture, suggests the diagnosis of lipomatous tumors. The signal intensity on T1-, T2-, fat-suppressed T2-weighted (FS-T2) images or short tau inversion recovery (STIR), T1- and fat-suppressed T1-weighted images after Gd-DTPA administration can be a useful aid in distinguishing between biologically different types of lipomatous tumors. The imaging features are often characteristic and reflect the histological pattern of the tumors.

We present two cases of skeletal metastases from myxoid liposarcoma, occurring several years after treatment of the primary tumors in the lower limb. The present two case reports have unusual radiological features only detectable by MR imaging and not by plain radiographs or bone scans. From the present two cases, we found that a negative plain radiograph of the spine or a negative bone scan could not exclude skeletal metastases from myxoid liposarcoma, and MRI was a more sensitive screening procedure for their detection, especially in T1-weighted images. Unusual radiological features of skeletal metastases from myxoid liposarcoma are not well documented and only a few cases have been previously reported. Our aim is to document two more patients exhibiting the unusual radiological features of skeletal metastases from myxoid liposarcoma to improve their early detection and management.  相似文献

Liposarcoma of soft tissue: MRI findings with pathologic correlation   总被引:5,自引:0,他引:5  
Objective. To evaluate the MRI findings of liposarcomas of different histologic types and correlate these with the histopathologic features. Design. The MR images of seven liposarcomas were reviewed retrospectively to assess the tumor size, location, margination, signal characteristics and enhancement patterns in different histologic types. Patients. Seven liposarcomas comprising three well-differentiated, two myxoid and two pleomorphic types were evaluated. Results and conclusion. All tumors showed well-defined and mostly lobulated margins. The well-differentiated liposarcomas were composed mainly of fat with septations or nodules, were hyperintense on T2-weighted images, and demonstrated faint enhancement or no enhancement following intravenous contrast. Myxoid liposarcomas were homogeneous or mildly heterogeneous and a pseudocapsule was present in one case. Pleomorphic types showed a markedly heterogeneous internal structure. Both myxoid and pleomorphic lesions-showed moderate or marked heterogeneous enhancement after contrast administration. Well-differentiated liposarcomas may be differentiated from other types of the tumor by their largely lipomatous appearance. The malignancy grade increases in parallel with tumor heterogeneity and contrast enhancement.  相似文献

Purpose: To assess the value of fine-needle aspiration cytology in the diagnostic work-up of lipomatous tumors of the extremities and trunk, and to identify specific radiological features that could aid in the preoperative evaluation. Material and Methods: 175 patients with subfascial lipomatous tumors who had undergone preoperative magnetic resonance imaging or computed tomography and fine-needle aspiration cytology were studied. The percentage of fat within the lesion was visually graded from the images as: none, 1-75%, 75-95%, or 95-100%. The histological and cytological diagnoses were compared and in discordant cases the radiological images were re-reviewed. Results: There was cytological and histological concordance in 96% of lipomas and in 85% of atypical lipomatous tumors (ALT) and liposarcomas. Most discordant cases exhibited 1-75% fat. Radiological review suggested that cytological sampling problems due to tumor heterogeneity were the main cause of diagnostic difficulties. The majority of tumors with less than 75% fat were liposarcomas, and in no liposarcoma was the fat content higher than 75%. Both ALT and lipoma were found in the 95-100% group. Conclusion: Cytology can be highly accurate in the diagnosis of lipomatous tumors, including ALT; however, critical comparison with the radiological findings increases diagnostic security. In tumors with fat content visually assessed as less than 75% of the tumor volume, liposarcoma is the most likely diagnosis and a cytological diagnosis of ALT or lipoma should be questioned. In lesions with 75-95% fat, liposarcoma is unlikely, but FNAC is still indicated for safety. In lesions with 95-100% fat, FNAC is only indicated if the differentiation between lipoma and ALT influences the treatment strategy.  相似文献

PURPOSE: To evaluate whether liposarcoma, atypical lipomatous tumors and lipoma can be differentiated radiologically. MATERIAL AND METHODS: We have retrospectively analyzed CT and/or MR images of 110 subfascial lipomatous lesions. The amount of fat within the tumors was visually graded from the images as: none, 1-75%, 75-95% or 95-100%. The structure of non-fatty tumor components was compared. The images were compared to histopathology and in 37 cases to cytogenetic findings. RESULTS: Only 4 of 20 liposarcomas contained fat. All 4 lesions, histopathologically diagnosed as atypical lipomatous tumors, contained fat but less than 75% of tumor volume. All lesions with more fat than 75% of tumor volume were histologically diagnosed as lipomas. However, one-third of the karyotyped lipomas had ring chromosomes which are considered typical for atypical lipomatous tumors. CONCLUSION: When a tumor is composed more or less solely of fat, the diagnosis of a lipoma or atypical lipomatous tumor with a phenotype simulating a lipoma can be assumed. When the fat content is less than 75% of the tumor volume or non-fatty nodules are found, biopsies from different tumor components have to be performed to exclude malignancy. When no fat is found, imaging does not help in differentiating lipoma or liposarcoma from other soft tissue tumors.  相似文献

Our purpose was to identify the histologic types of malignant liver lesions with high signal intensity (SI) on T1-weighted images and to describe the MR imaging features. Thirteen patients with malignant liver lesions high in SI on T1-weighted images were studied with a 1.5-T MR imager using pre- and serial postcontrast spoiled gradient-echo (SGE) sequences (all patients), T2-weighted fat-suppressed spin-echo sequences (all patients), precontrast T1-weighted fat-suppressed spin-echo sequences (five studies in five patients), and precontrast out-of-phase SGE sequences (seven studies in six patients). Images were reviewed retrospectively to determine number of lesions; lesion size; SI of lesions on T1-weighted, T2-weighted, and fat-attenuated T1-weighted images; distribution of high SI in lesions on T1-weighted images; and tumor enhancement pattern. Seven patients had multiple tumors high in SI on T1-weighted images and six patients had solitary tumors. Seventy-two lesions were less than 1.5 cm in diameter and 35 lesions were more than 1.5 cm in diameter. Nine patients had solid malignant lesions and four patients had cystic malignant lesions. All tumors more than 1.5 cm in diameter were heterogeneously high in SI on T1-weighted images, and all tumors less than 1.5 cm were completely homogeneous or homogeneous with a small central hypointense focus. All tumors were more conspicuous on T1-weighted fat-attenuated images, both on excitation spoiled fat-suppressed spin-echo or on out-of-phase SGE images with the exception of one fat-containing hepatocellular carcinoma (HCC). In one patient with melanoma metastases and one patient with multiple myeloma nodules, appreciably more lesions were detected on out-of-phase SGE images. Causes of hyperintensity were considered to be either fat, melanin, central hemorrhage, or high protein content, all of which may be seen in a variety of tumors. Fat-attenuation techniques are helpful in the detection of these lesions.  相似文献

Myxoid liposarcoma is the most common type of liposarcoma. The magnetic resonance imaging (MRI) features of this tumor were evaluated and correlated with its clinical and histological features in seven patients to determine under what circumstances the tumor should be considered in differential diagnosis and why its signal intensity differs from those of lipoma and lipoma-like (lipoblastic) liposarcoma. In all patients the tumor presented in a lower extremity (5 thigh, 2 calf) as a painless, slowly growing mass which had been present for several months to several years. MRI examination revealed the tumors to be encapsulated, noninfiltrating, and usually septated. On T1-weighted sequences five of seven lesions (71%) showed lacy, amorphous, or linear foci of high signal within a low signal mass. These foci are believed to represent fat within the tumor and distinguish it from several other benign and malignant masses. If an indolent mass in a lower extremity demonstrates a predomiantly low signal with a few amorphous or linear high signal foci on T1-weighted sequences, one should consider the possibility of myxoid liposarcoma even if it appears benign by all other criteria. Histologic evaluation showed that myxoid liposarcomas contain less than 10% mature fat, which accounts for their low signal on T1-weighted sequences in contrast to the high signal of lipomas and lipoblastic liposarcomas.  相似文献

PURPOSE: To determine if quantitative and qualitative magnetic resonance (MR) imaging measures can help differentiation of mucinous from nonmucinous rectal tumors. MATERIALS AND METHODS: In 26 patients with pathologically proved mucinous (n = 9) and nonmucinous (n = 17) rectal tumors, MR imaging was performed with T1-weighted spin-echo (SE) and T2-weighted fast SE sequences in all patients and with a gadolinium-enhanced T1-weighted sequence in 18. With use of the signal intensity (SI) measurements in the tumors and reference tissues, tumor-to-muscle, tumor-to-fat, and tumor-to-urine SI ratios were calculated. In addition, the SI and contrast-enhancement patterns in the tumors were assessed qualitatively by three blinded readers. RESULTS: Mucinous tumors had a much higher SI on the T2-weighted fast SE images. Tumor-to-muscle, tumor-to-fat, and tumor-to-urine SI ratios were significantly higher in the mucinous compared with the nonmucinous tumors (P = .0004, P = .0008, and P = .00002, respectively). Qualitative evaluation of the SI correlated well between readers 1 and 2 (r = 0.93), readers 1 and 3 (r = 0.94), and readers 2 and 3 (r = 0.91). Agreement for the contrast-enhancement patterns was 67%, 72%, and 67%, respectively, with most mucinous tumors having predominantly high SI and a peripheral contrast-enhancement pattern. CONCLUSION: Mucinous and nonmucinous rectal tumors can be differentiated with MR imaging because mucinous tumors show high SI on T2-weighted fast SE images.  相似文献

Nodular fasciitis: correlation of MRI findings and histopathology   总被引:3,自引:0,他引:3  
OBJECTIVE: To compare the histopathology of nodular fasciitis (NF) with the magnetic resonance imaging (MRI) findings in order to evaluate the basis of the MR signal characteristics. DESIGN AND PATIENTS: Ten patients with NF, nine females and 1 male, with an age ranging from 13 to 58 years (mean 26.8 years) were studied. MRI findings, available in all 10 patients, were compared with the histopathology in nine patients, and an area-to-area comparative study of the whole specimen section histopathology and MRI was performed in two patients. RESULTS: On the basis of an excisional biopsy or resection specimen, the nine lesions were classified into myxoid ( n=4), cellular ( n=3) and fibrous ( n=2) subtypes. Four myxoid lesions with a subcutaneous location showed a homogeneous SI comparable with muscle on T1-weighted images, high SI on T2-weighted images, and had homogeneous enhancement. One cellular lesion presented with homogeneous, slightly higher SI than muscle on T1-weighted images and inhomogeneous, high SI on T2-weighted images. Alcian blue stain of the whole specimen section revealed the lesion had two parts corresponding to different enhancement patterns on MRI. The blue-stained myxoid part showed markedly diffuse enhancement, while the non-stained cystic space had only peripheral enhancement. Two other cellular lesions had the same appearance on both T1- and T2-weighted images and showed inhomogeneous, diffuse enhancement. One fibrous subtype lesion presented with inhomogeneous, overall slightly higher SI than muscle on T1-weighted images, lower SI at the periphery and high SI in the center on STIR images and only peripheral enhancement. Microscopy and CD-31 staining of the lesion showed more extracellular matrix, with poor vascularity in the center and more collagenous matrix with higher vascularity at the periphery. CONCLUSION: Although similar findings were found in some lesions, the large histologic variability of NF hampers the definition of a prototype of NF on MRI. However, the MRI appearance of the myxoid subtype is rather characteristic. Histologic findings reflect the different SI characteristics and enhancement pattern on MRI.  相似文献

PURPOSE: To retrospectively evaluate the magnetic resonance (MR) imaging findings of phyllodes tumor of the breast and to compare these findings with the histologic grade. MATERIALS AND METHODS: Institutional review board approval and informed consent were obtained. The authors reviewed the MR findings in 30 female patients aged 16-73 years (mean, 40.2 years) with surgically confirmed phyllodes tumors. Analyzed MR findings included tumor shape, margin, internal enhancement, and size; signal intensity (SI) of tumor higher than that of normal breast tissue on T1-weighted images; SI of tumor lower than or equal to that of normal tissue on T2-weighted images; cyst wall appearance; kinetic curve assessment; and apparent diffusion coefficient (ADC). The MR findings and histologic grade were statistically analyzed to determine whether any correlations existed. Significant MR findings were compared with histopathologic findings. RESULTS: Nineteen benign, six intermediate (characterized by five to nine cell reproductions at 10 high-power fields, pushing or infiltrative margins, moderate stromal cellularity, and atypia and overgrowth), and five malignant phyllodes tumors were assessed. Irregular cyst wall (P = .003), tumor SI lower than or equal to normal tissue SI on T2-weighted images (P = .005), and low ADC (P = .001) correlated significantly with histologic grade. Tumor SI higher than normal tissue SI on T1-weighted images was more frequent in the malignant (in three of five tumors) and intermediate (in three of six tumors) groups than in the benign group (in two of 19 tumors); however, it was not a significant finding (P = .024). Tumor SI higher than normal tissue SI on T1-weighted images and irregular cyst wall corresponded histopathologically to hemorrhagic infarction and necrosis, respectively. Tumor SI lower than or equal to normal tissue SI on T2-weighted images and low ADC corresponded histopathologically to stromal hypercellularity. Other findings were not significant. CONCLUSION: Several MR findings can be used to help determine the histologic grade of phyllodes breast tumors.  相似文献
