目的介绍成人间活体扩大右半肝移植治疗急性肝功能衰竭的临床经验。方法对1例42岁男性急性肝功能衰竭合并肝性脑病Ⅲ期患者行活体扩大右半肝移植治疗。其45岁姐姐为供者,CT评估供者包含肝中静脉的扩大右半肝体积为728.4cm^2(801g),供肝/受者体重比为1.3%。供肝之肝右、中静脉整形后与受者整形后之肝右静脉行端-侧吻合;供受者门静脉、肝动脉行端.端吻合。供肝胆管整形后与受者胆总管行端-端吻合。结果供、受者手术均成功。供者术后恢复顺利,受者术后8h恢复意识,14d后丙氨酸转氨酶、总胆红素等指标首次下降至正常水平。术后16d曾出现转氨酶明显升高,给予甲泼尼龙1000mg冲击治疗后恢复正常。随访至今,供受者已健康生存8个月,均未出现胆管、肝动脉及静脉回流等并发症。结论扩大右半肝移植在技术上完全可行。能为成人患者提供足够重量的移植物,尤其对于急性肝功能衰竭患者具有重要意义,术前精确的影像学评估,熟练的肝切除和肝移植技术是确保该类手术成功的关键因素。  相似文献

Right anterior-medial lobe congestion due to temporary clamping of segment V and/or VIII is common in the operative theater during adult donor right lobe liver transplantation, the most common procedure in our institute. We have used an autogenous saphenous vein conduit to recipient portal vein tributaries in 15 cases, as a "Y-to-I venoplasty" since January 2004. The recipient portal vein is transected 5 mm proximal to its bifurcation and extended to both sides with partial hepatic dissection. The "Y-to-I venoplasty" is made by suture closure of the portal vein transversely to form a tube. The average length is 7.5 cm with a 1.3 cm width. One end of "Y-to-I venoplasty" conduit is anastomosed to the donor segment V branch on the back table. And the other end is anastomosed directly to the IVC via a new window or the middle hepatic vein stump in recipient. The phase distension of the conduit with respiration is noted in the operative field. A 6/15 (40%) patency rate, was observed by CT angiography at the second postoperative week. All-patient conduits showed good flow on serial examinations at the 60th postoperative day. This new venous graft, made of recipient portal vein is a good conduit for segment V decongestion in adult right lobe partial liver transplantation.  相似文献

目的:分析10例活体肝移植术中的血管变异,总结其外科处理经验,进一步提高手术成功率,减少并发症。方法:2001年1月至12月,行活体肝移植10例,其中左半肝8例,左外叶1例,右半肝1例,供肝者均为其母,经术中B超及胆管造影以确定肝切线。供体单支肝动脉分支与受体肝动脉吻合,两支肝动脉分别与受体肝左、右动脉吻合。门静脉分支与受体门静脉主干吻合。供体肝静脉与受体下腔静脉行端侧吻合。胆管重建均采用肝管分支与受体胆总管端端吻合,置T管引流。结果:10例活体肝移植,1例因肝动脉血栓形成,术后5天需次肝移植;1例发生排斥;其余8例均康复出院,5例已上学。结论:活体肝移植术中血管重建技术是其重要环节,术前和术中了解供受体解剖变异并正确处理,可减少术后血管和胆道的并发症。  相似文献

OBJECTIVE: To review the anatomical variations of the right lobe encountered in 40 living liver donors, describe the surgical management of these variations, and summarize the results of these procedures. SUMMARY BACKGROUND DATA: Anatomical variability is the rule rather than the exception in liver and biliary surgery. To make effective use of liver segments from living donors for transplantation, surgical techniques must be adapted to the anomalies. METHODS: Donor evaluation included celiac and mesenteric angiography with portal phase, magnetic resonance angiography, and intraoperative ultrasonography and cholangiography. Arterial anastomoses were generally between the donor right hepatic artery and the recipient main hepatic artery. Jump-grafts were constructed for recipients with hepatic artery thrombosis, and double donor arteries were joined to the bifurcation of the recipient hepatic artery. The branches of a trifurcated donor portal vein were isolated during the parenchymal transection, joined in a common cuff, and anastomosed to the recipient main portal vein. Significant accessory hepatic veins were preserved, brought together in a common cuff if multiple, and anastomosed to the recipient cava. The bile ducts were individually drained through a Roux-en-Y limb, and stents were placed in most patients. RESULTS: Forty right lobe liver transplants were performed between adults. No donor was excluded because of prohibitive anatomy. Seven recipients had a prior transplant and five had a transjugular intrahepatic portosystemic shunt (TIPS). Arterial anomalies were noted in six donors and portal anomalies in four. Arterial jump-grafts were required in three. Sixteen had at least one significant accessory hepatic vein, and one had a double right hepatic vein. There were no vascular complications. Multiple bile ducts were found in 27 donors. Biliary complications occurred in 33% of patients without stents and 4% with stents. CONCLUSIONS: Anatomical variations of the right lobe can be accommodated without donor complications or complex reconstruction. Previous transplantation and TIPS do not significantly complicate right lobe transplantation. Microvascular arterial anastomosis is not necessary, and vascular complications should be infrequent. Biliary complications can be minimized with stenting.  相似文献

目的 探讨活体右半肝供体的安全性。方法 对2002年1月至2005年6月施行的13例活体右半肝移植中供体的资料进行回顾性研究。不阻断入肝血流,在肝中静脉右侧,用超声刀离断肝组织得到右半供肝。通过计算得到标准肝体积及残余左半肝的比例。结果 右半供肝切取术平均失血490ml,平均输血440ml。围手术期平均输入人血白蛋白85g。1例供体门静脉分为3支,2例供体右后与右前胆管汇入左肝管,1例左外与左内胆管先后与右肝管汇合成肝总管,术中处理恰当,门静脉左干血流及左肝管胆汁引流保持通畅。2例供肝轻度脂肪变。术后第1天肝功能均有不同程度损害,但术后1周恢复到接近正常水平。术后并发症包括1例腹腔内出血,2例切口脂肪液化和1例乳糜漏。所有供体恢复好并回到原工作岗位。结论 只要保证左半肝血管与胆管通畅,残余肝体积在30%以上及手术对残余肝无大的损伤,右半供肝切取是安全的。  相似文献

Living Donor Liver Transplantation with Left Liver Graft   总被引:1,自引:0,他引:1  
Small-for-size syndrome in LDLT is associated with graft exposure to excessive portal perfusion. Prevention of graft overperfusion in LDLT can be achieved through intraoperative modulation of portal graft inflow. We report a successful LDLT utilising the left lobe with a GV/SLV of only 20%. A 43 year-old patient underwent to LDLT at our institution. During the anhepatic phase a porto-systemic shunt utilizing an interposition vein graft anastomosed between the right portal branch and the right hepatic vein was performed. After graft reperfusion splenectomy was also performed. Portal vein pressure, portal vein flow and hepatic artery flow were recorded. A decrease of portal vein pressure and flow was achieved, and the shunt was left in place. The recipient post-operative course was characterized by good graft function. Small-for-size syndrome by graft overperfusion can be successfully prevented by utilizing inflow modulation of the transplanted graft. This strategy can permit the use of left lobe in adult-to-adult living donor liver transplantation.  相似文献

目的 探讨成人活体肝移植供肝的灌注和管道重建的技术。方法 回顾性分析41例成人活体肝移植供肝的后台处理临床资料。结果 41个供肝,均为不包括肝中静脉的右半肝,供者男9例,女32例,年龄19~65岁。供肝切取后经门静脉灌注HTK液2~3L(平均2.45L)。只有一支门静脉右支者35例,右前支+右后支门静脉6例。右肝管29例,右前叶肝管+右后叶肝管10例,右后叶肝管+右前上段支+右前下段支2例。肝静脉:右肝下静脉+V_5/V_814例,只有一支右肝静脉15例,2支肝中静脉分支8例,4例有3支肝中静脉分支。肝中静脉分支直径〉0.5cm者均重建,重建V_5/V_8和右肝下静脉28例次(70.0%),右前叶肝管和右后叶肝管整形6例(14.6%),右后叶肝管和右前叶下段肝管整形3例(7.3%),门静脉整形2例(4.8%),门静脉搭桥4例(9.7%)。结论 成人活体肝移植的供肝后台处理与尸体肝移植有明显的不同,其断面的管道处理直接影响移植肝的存活和预后。  相似文献

Splitting deceased donor livers and creating 3 grafts from a whole liver may be feasible and shorten the waiting time for organ donation in patients with high mortality rates. We hypothesized that it might be reasonable to procure 3 grafts for donation from one deceased donor liver by splitting the liver into left (segment II, III, IV), right anterior (segment V, VIII), and right posterior lobes (segment VI, VII) for liver transplantation according to the portal system trifurcated variations. We designed the right anterior branch with the main portal trunk and middle hepatic artery to become inflow of right anterior lobe, the left portal vein and left hepatic artery to become the inflow of left lobe and right posterior branch, and right hepatic artery to become the inflow of right posterior lobe. We retrospectively reviewed the volumetric computed tomography and magnetic resonance cholangiopancreatography of 153 liver donors. The hepatic and portal veins, hepatic artery, and biliary system were reorganized and classified. The volumetric proportions of the liver grafts were measured. Trifurcation of the portal vein variation was found in approximately 13.7% of portal systemic variations. The left lobe accounted for 29.18% of the total liver volume, the right anterior lobe, 35.22%, and the right posterior lobe, 35.6%. We validated this principle by dissecting the explanted liver and identified the triple grafts' weights, percentages, vessels, and biliary ducts system. The splitting of deceased donor livers into 3 split liver grafts for use in liver transplantation surgery can be clinically useful.  相似文献

This report presents a case of a left hepatectomy and a caudate lobectomy combined resection of the ventral segment of the right anterior sector for hilar cholangiocarcinoma using percutaneous transhepatic portal vein embolization (PVE). The patient was a 44-year-old man admitted to a local hospital with obstructive jaundice. He was diagnosed to have hilar cholangiocarcinoma and was referred to the hospital for further treatment. Cholangiography revealed stenosis of the left hepatic duct and the hilar bile ducts. The dorsal branch of the right anterior sector joined the right posterior branch and the tumor did not invade to the confluence of these branches. Arteriography and portography reconstructed by multidetector-raw computed tomography revealed the ventral branches of the right anterior sector, which separately diverged from the other right anterior branches. It was therefore necessary to perform a left hepatectomy and caudate lobectomy combined resection of the ventral segment of the right anterior sector to completely remove the tumor. Portal vein embolization was thus performed on the left portal vein and the ventral branches of the right anterior sector. Intraoperatively, when the hepatic artery was temporally clamped, the demarcation between the ventral segment and the dorsal segment of the right anterior sector could be clearly visualized. The planned surgery was performed safely. This case demonstrates that the utilization of PVE is useful for a difficult and intricate hepatectomy, which requires an accurate identification of a hepatic subsegment.  相似文献

We report a case of anomaly of the intrahepatic portal system in a 65-year-old man with hilar bile duct cancer. Preoperatively, percutaneous transhepatic portography demonstrated that there was a right posterior portal vein arising from the main portal vein. In addition, a large portal branch originated from the left portal vein and coursed toward the right hepatic lobe. Following portal embolization of the right posterior branch, the patient underwent an extended right hepatectomy with a caudate lobectomy. Intraoperatively, to the left at the porta hepatis and then it first gave off the right anterior portal vein originated from the left portal vein and coursed toward the right hepatic lobe horizontally behind the gallbladder and then separated into superior and inferior segmental branches to supply the right anterior segment of the liver. The ramification of some major branches without malposition of the gallbladder or round ligament was the important clinical feature of this anomaly.  相似文献

目的探讨门静脉-下腔静脉吻合术用于预防活体肝移植术后小肝综合征(small-for-size liver syndrome,SFSS)的效果。方法 3例活体肝移植均采用不含肝中静脉的右半肝作为移植物。术中发现实测移植物(肝)重量/受体的体质量(体重)的比值(graft to recipient weight ratio,GRWR)为0.58%、0.77%及0.71%,均<0.8%,符合小移植物的诊断。处理:首先吻合肝静脉流出道,其次吻合门静脉,将受体门静脉右支与移植肝门静脉右支端端吻合,将受体门静脉左支与下腔静脉行端侧吻合达到门腔分流的作用,之后按顺序吻合动脉和胆道。术中均未行脾静脉结扎或脾切除等处理。术后定期随访。结果 3例患者术后均未发生SFSS并顺利出院,出院时间分别为术后25d、34d及56d。移植肝功能逐步好转,术后1d门静脉流速理想。移植肝增长良好。门静脉-下腔静脉短路通畅时间:除1例通畅持续仅104d,其余2例持续通畅。结论 LDLT术中进行门静脉-下腔静脉吻合术可以及时有效预防小移植物背景下的SFSS,受体门静脉左支与下腔静脉行端侧吻合的分流技术安全可靠。  相似文献

In Taiwan, living donor liver transplant (LDLT) has accounted for the majority of liver transplantation due to organ shortage. Dual-graft LDLT is a feasible way to resolve the insufficient graft size and remnant liver in donors. We presented a heavy-weight patient underwent dual-graft LDLT, and cystic duct was used to resolve the inadequate bile duct length and limited appropriate position in dual-graft LDLT. We harvested a right lobe graft (segment 5, 6, 7, and 8 without middle hepatic vein) and a left lobe graft (segment 1, 2, 3, and 4 without middle hepatic vein) stepwise, and placed the grafts orthotopically. For proper tension and length of biliary reconstruction, we anastomosed the right intrahepatic duct of the right lobe graft to cystic duct of the recipient. Before the biliary reconstruction, the metal probe was inserted in the lumen of cystic duct in recipient to ensure the patency and destroy the Heister valve of cystic duct, then the internal biliary stent (5 Fr pediatric feeding tube) was placed in the donor's right intrahepatic duct to recipient's cystic duct and common bile duct, which allows the endoscopic removal of the internal stent. The patient has survived more than 16 months with normal liver function.  相似文献

Varying circumstances call for various types of donor hepatectomy. We report here on an unusual type of donor hepatectomy, right posterior segmentectomy. A 46-year-old sister of the recipient was the donor. Her preoperative CT showed that the right anterior portal vein originated from the umbilical portion of the left portal vein. Intraoperative cholangiography revealed that the right posterior hepatic duct joined the common hepatic duct in the extrahepatic area. Right posterior segmentectomy was performed without transfusion in the donor. The postoperative course proceeded favorably in both donor and recipient until postoperative day 41, when the recipient unexpectedly expired as a result of cerebral hemorrhage. The donor was discharged with good liver function. We can conclude that the right posterior segment of the donor can be used as a graft, when the volume of left liver is not enough for both donor and recipient, or the vascular structures favor right posterior segmentectomy.  相似文献

Because of its anatomy, the liver can be divided into two hemilivers suitable for use as two grafts for liver transplantation. The line of division is the main scissure, giving the right hemiliver (segments 5-8) and the left hemiliver (segments 2-4). Segment 1 (caudate lobe) has to be resected. The vessels are divided between the two grafts: the vena cava remains on the right; on the left, the left hepatic vein is sutured into the vena cava of the recipient, which is retained intact. The left graft retains only the left branch of the portal vein, the bile duct and the hepatic artery. The right graft retains the portal trunk, the common bile duct and the right branch of the hepatic artery. This procedure was used for emergency grafting of two patients with fulminant hepatitis when only one donor was available. Both recipients recovered from coma and regained normal liver function. However, both died from causes not specifically related to the operative technique, one from multiple organ failure on the 20th day and the other from diffuse cytomegalovirus infection on the 45th day.  相似文献

We report a case of split liver transplantation (SLT) for two adult recipients, which was the first successful case in Korea. The brain-dead donor was a 22-year-old man weighing 65 kg, but his liver volume was estimated as 2120 mL on computed tomographic volumetry. As it seemed to be too large for a 60-kg recipient candidate, SLT was planned after assessment of lobar liver volume and middle hepatic vein anatomy. The right lobe was mobilized first and the liver parenchyma transected along the right border of the middle hepatic vein. The 1240-g right lobe (segments 5 to 8) graft was implanted into a 57-year-old male patient with acute-on-chronic liver failure in the same manner as a living-donor graft. After that, routine procedures of cadaveric multiorgan procurement were performed. The 670-g left lobe (segments 1 to 4) with a retrohepatic vena cava, common bile duct, and aortic patch was implanted into another 37-year-old male recipient. These two recipients recovered uneventfully surviving 12 months to date. We integrated the surgical techniques learned from hundreds of adult-to-adult living donor liver transplants into this first trial of two adult SLT.  相似文献

目的:探索肝中叶作为独立供肝进行移植的可行性,以进一步拓宽供肝来源。
方法:普通级健康犬分成供体组(n=12,20~25 kg)和受体组(n=12,10~15 kg),供受体随机配对。供体手术将位于中央区的方叶、右中叶在体内劈离,原位灌注后保留其专属的门静脉中支、肝中动脉和中肝管,得到独立的中央区供肝并称重。受体手术先建立临时性门腔分流,供肝背驮式植入,流出口和腔静脉壁端侧吻合,供肝门静脉中支和受体门静脉主干行端端吻合后恢复新肝血流,重建动脉和胆管。术后观察受体腹腔及胆汁引流,每日检测肝功能,死亡后行尸检,移植物取标本行病理检查。
结果:犬肝被深陷的叶间裂分隔成7叶,各叶间由较少的肝桥连接,方叶和右中叶由门静脉中支、中肝动脉营养血回流至肝中静脉,胆汁引流至中肝管。供体组体内劈离技术全部得以完成,手术时间(215.0±67.7)min,失血量(229.3±66.5)mL。比较GRWR,中央区供肝[(1.3±0.3)%]和假设的左侧区供肝的[(2.1±0.4)%]及右侧区供肝的[(0.9±0.1)%]之间差异均有统计学意义(均P<0.01)。受体组手术时间(327.6±75.3)min,无肝期(33.6±7.5)min,失血量(415.5±79.8)mL。12个供肝均成功植入,冷缺血时间为(41.9±12.1)min,(8.3±3.6)min后排泌胆汁。受体肝功能指标在术后第1天发生明显变化,随后逐渐恢复,中位存活时间92.5(18~272)h,未发现有因吻合口出血、血栓等外科技术性并发症而死亡受体。
结论:犬动物模型证实肝中央区可以劈离出来作为一个独立的供肝器官,为将来拓宽供肝来源提供了另外一种思路。  相似文献

OBJECTIVE: This study sought to describe the surgical management of right portal venous (PV) branches encountered among 104 cases of right lobe living donor liver transplantation (LDLT). METHODS: From January 2002 to September 2007, we performed 104 cases of right-lobe LDLT including 11-donors who had anomalous right portal venous branches (APVB). One recipient had PV sponginess hemangioma. The donor right PV branches were type I in 93 cases, type II (trifurcation) in nine cases, and type III in two cases. Except one narrow bridge of tissue excision, the PV branches were transected on the principal of donor priority: PV branches were excised approximately 2 to 3 mm from the confluence while leaving the donor's main portal vein and confluence intact. In type II APVB, donor PV branches were obtained with two separate openings in six cases; with two separate openings joined as a common orifice at the back table in two cases, with one common opening with a narrow bridge of tissue in one case. In type III APVB, the donor right anterior and posterior PV branches were obtained with separate openings. The donor right PV branches with one common opening in 92 cases of type I PV branches and a joined common orifice in three cases of type II APVB were anastomosed to the recipient's main portal vein or to right branching. As the unavailable recipient PV for sponginess hemangioma, one case of type I right PV branches was end-to-end anastomosed to one of the variceal lateral veins of about 1 cm diameter in a pediatric patient. The PV were reconstructed as double anastomoses in six type II APVB and in one type III APVB obtained with two separate PV openings. In the another type III APVB reconstruction, we successfully utilized a novel U-shaped vein graft interposition. RESULTS: The type II APVB donor receiving a narrow bridge of portal vein tissue excision developed portal vein thrombosis on the third postoperative day and underwent reexploration for thrombectomy. There were no vascular complications, such as portal vein thrombosis or stricture among other donors or all recipients. The velocity of blood flow in the U-graft was normal. The anastomosis between the type I donor right portal vein and recipient variceal lateral vein was unobstructed. CONCLUSION: Right PV branches should be excised on the principal of donor priority while leaving the donor's main portal vein and confluence intact. Single anastomoses was the fundamental procedure of right branch reconstruction. Double anastomoses could be used as the main management for type II and type III APVB reconstruction. U-graft interposition may be a potential procedure for type III APVB reconstruction. Single anastomoses between the donor right portal vein and the recipient variceal lateral vein may be performed when recipient portal vein is unavailable. These innovations for excision and reconstruction of right PV branches were feasible, safe, and had good outcomes.  相似文献

目的 总结亲属活体单段供肝移植治疗极低体重婴儿胆道闭锁的临床经验.方法 受者为出生仅145 d的男婴,身高66 crn,体量3.08 kg,被确诊为胆道闭锁伴肝硬化.供者为患儿母亲,年龄36岁,身高145 cm,体重47 kg.采用改良背驮式原位肝移植术,切取供者Ⅱ段肝组织作为供肝,移植肝体积与受者标准肝体积比值为92.5%,GRWR为5.19%,供肝动脉与受者肝右动脉用供者左侧股外侧浅隐静脉搭桥行端端吻合,受者三支肝静脉经整合后与供肝静脉行端端吻合,供肝胆管与受者空肠行Roux-en-Y吻合.术后监测供、受者生命体征、肝肾功能及出血和凝血状况等,常规抗感染治疗.受者术后采用环孢素A、吗替麦考酚酯及甲泼尼龙的方案预防排斥反应.结果 供肝切取手术历时370 min,术中供者出血150 ml均回输,切取供肝重量为160 g.肝移植手术历时451min,术中受者失血230 ml,输注全血200 ml和红细胞悬液50 m1,无肝期时间为71 min,供肝冷缺血时间为132 min.供者恢复顺利,术后8 d拆线出院.受者术后5 d肝功能基本恢复正常,术后7 d各项化验指标均正常.但术后7和15 d时,受者分别发生肠道吻合口漏各1次,经修补后痊愈.受者于术后35 d出院,出院时体重增加0.3 kg,各方面与同龄婴儿相当.结论 亲属活体单段供肝移植是治疗极低体重患儿终未期肝病的一种可供选择的治疗方法,经充分的术前评估、精细的手术操作及良好的围手术期管理后,手术能取得良好效果.  相似文献

Anatomical variations in the venous system of liver are not a rarity. A prospective helical computerized tomography (CT) study was undertaken to determine the prevalence of surgically significant hepatic venous anatomic variations among 100 consecutive living liver donors. The studies evaluated the ramification pattern of hepatic veins, the presence of accessory hepatic veins, and of segment 5 or 8 veins (or both) draining into middle hepatic vein. These data obtained by CT influenced surgical planning. Sixty-four donors donated their right lobes and 24 donors, left lateral segments. Only one donor candidate was refused due to combined hepatic and portal venous variations accompanied by multiple bile ducts. Eleven donors were also refused due to reasons other than anatomical variations. Seventeen segment 5 and 17 segment 8 veins draining into middle hepatic vein were anastomosed to inferior vena cava in 23 (36%) of the right lobe liver transplantations. The middle hepatic vein was harvested in only one of the donors. Among the 100 cases, 47 had accessory right inferior hepatic veins, 13 of which were multiple. Twenty-two of the right lobe grafts required surgical anastomoses of these accessory hepatic veins (34%). An isolated hepatic vein anomaly or the presence of accessory hepatic veins are not contraindications to be a living liver donor candidate. However, preoperative knowledge of vascular variations alters surgical management. 目的 总结活体右半供肝移植中,不含肝中静脉的右半供肝Ⅴ、Ⅷ段静脉回流的重建方法.方法 回顾性分析55例活体右半供肝移植中,不含肝中静脉的右半供肝Ⅴ、Ⅷ段静脉回流重建的临床资料.所有供者均通过了受者标准肝体积、供肝与受者体重比(GRWR)、供肝与受者标准肝体积比及供肝内血管解剖等指标的综合评估.供肝在切取、灌注及修整后,51例采用尸体髂静脉作为重建材料,其他4例分别采用受者的大隐静脉、曲张的脐静脉、肝内门静脉和肝静脉作为重建材料,以串联的方式重建供肝断面Ⅴ、Ⅷ段静脉回流.肝移植时,供肝肝右静脉与受者肝右静脉开口吻合,重建的Ⅴ、Ⅷ段静脉与受者肝中、肝左静脉汇合部吻合,供肝门静脉与受者门静脉右支或主干吻合.门静脉开放血流后依次重建肝动脉及胆道.术后对供、受者进行常规监测.结果 1例受者术后发生流出道梗阻,术后第43天死亡;1例受者术后第7天出现不明原因的急性重型肝组织坏死,行尸体供肝肝移植后痊愈.其余53例受者恢复顺利,术后4周时腹部CT检查显示重建的移植肝Ⅴ、Ⅷ段静脉回流通畅.55例供者术后均恢复顺利,术后2周出院.结论 不含肝中静脉的活体右半供肝Ⅴ、Ⅷ段静脉回流的重建在活体肝移植中是可行的,应选择合适的重建材料及手术方式.受者肝移植后临床效果良好.  相似文献
