An electrogram was recorded from the angioplasty catheter guide wire when coronary blood flow was interrupted in 20 patients undergoing percutaneous transluminal coronary angioplasty. Monophasic action potentials were recorded from the right ventricular septum together with the routine electrocardiogram. The patients were studied during angioplasty for lesions in the left anterior descending (12), circumflex (3), and right coronary arteries (6). ST elevation in the electrogram recorded in the left anterior descending and circumflex systems was usually more obvious than that in the electrocardiogram. Signals obtained from the right coronary artery were of very low amplitude and registered only minimal ST changes. The ST elevation developed in the electrogram during insertion of the catheter before inflation of the balloon in 11 of the 15 patients undergoing angioplasty of the left system. In eight of the patients showing pre-inflation ST elevation the ST shift lessened after successive inflations. Monophasic action potential recordings were obtained during 45 balloon inflations in 19 patients. In those patients undergoing angioplasty for lesions of the circumflex coronary artery the monophasic action potential showed no change during balloon inflation. In patients undergoing angioplasty for the right coronary artery the mean normalised duration at 60 seconds' occlusion was 99.6 (1.5)% of control. Of a total of 25 occlusions in the patients undergoing angioplasty for the left anterior descending coronary artery 19 showed shortening of less than 5%, five showed shortening between 5 and 10%, and one showed a shortening of 16.4% in the monophasic action potential. The QT interval was satisfactorily measured in the electrogram during 36 balloon inflations, and in 24 of these it was also measured in the electrocardiogram. QT changes in the electrogram tended to be the opposite of those in the electrocardiogram. When changes in RR interval were minimal (less than 20 ms) during the balloon inflation 14 of 17 electrograms showed QT prolongation but only one of 12 electrocardiograms showed prolongation. Conversely one of 17 electrograms showed shortening compared with eight of 12 electrocardiograms. There was angiographic evidence of the development of collaterals in six of 15 patients undergoing angioplasty of the left system. ST segment elevation in both the electrogram and electrocardiogram was less pronounced in these patients than in those without evidence of the development of collaterals. ST segment changes recorded from the angioplasty guide wire provide a more sensitive index of ischaemia than the surface 12 lead electrocardiogram, and fall in ST segments on balloon deflation is a prognostic index of a good angiographic result in the left anterior descending and circumflex arteries, but not in the right coronary artery.  相似文献

This prospective study examines the data derived from the intracoronary electrocardiogram (ECG) (derived from the coronary guide wire) compared with that from four standard surface leads (I, II, III, and V2) in documenting myocardial ischemia during coronary angioplasty. Intracoronary and surface ECGs were simultaneously recorded in 300 consecutive patients (mean age 59 +/- 10; range 33 to 80 years; 246 males [82%] during coronary angioplasty in 368 lesions (167 left anterior descending [46%], 85 left circumflex [23%], 107 right coronary arteries [29%], and nine bypass grafts [2%]), before balloon inflation, at 1 minute of inflation, and at the end of the procedure. ST segment changes (greater than 0.1 mV) were observed in the intracoronary ECG in 306 lesions (83%) (151 left anterior descending [88%], 75 left circumflex [89%], and 80 right coronary arteries [73%]) versus in 245 lesions (67%) in the surface ECG (126 left anterior descending [73%], 43 left circumflex [47%], and 76 right coronary arteries [70%]; [p less than 0.0001]). The mean ST segment shift was 0.5 +/- 0.4 mV in intracoronary and 0.1 +/- 0.2 mV in standard leads (p less than 0.0001). ST elevation was seen in 97% of cases with intracoronary ECG changes versus in 83% with surface ECG changes. The remainder had ST depression. A total of 48 lesions (13%) did not produce ECG changes and 62 (16%) had silent ischemia. Negative U waves on the surface electrocardiogram are reported to be a specific marker of myocardial disease. In the setting of ischemia, they correlate with stenosis of the left main and left anterior descending coronary arteries. To determine whether U wave changes are unique for anterior ischemia, the development of new U waves on the intracoronary electrogram was correlated with the location and magnitude of ischemia during coronary balloon angioplasty. Recordings were obtained during dilation of 43 vessels in 37 patients. New negative U waves developed during dilation of 12 vessels (7 of the left anterior descending, 4 of the left circumflex and 1 of the right coronary artery). New positive U waves developed during dilation of 18 vessels (12 of the left anterior descending, 3 of the left circumflex and 3 of the right coronary artery). The magnitude of ST segment change was 10.9 +/- 6.7 mm in the presence of a new U wave but only 3.4 +/- 2.8 mm in the absence of a new U wave (p less than 0.001). It is concluded that 1) negative U waves on the intracoronary electrogram are not specific for anterior ischemia; 2) new positive U waves on the intracoronary electrogram are as sensitive as new negative U waves for acute ischemia; 3) the development of a new positive or negative U wave is associated with the magnitude of myocardial ischemia; and 4) the recording of U waves may be related to the proximity of the recording leads to the location of ischemia.  相似文献

During percutaneous transluminal coronary angioplasty (PTCA) frontal ECG leads are routinely monitored. The detection of ST segment deviation during the procedure is important for decisions regarding guiding catheter seating and the timing of balloon inflation and deflation. ST segment deviation appears on intracoronary electrograms in the absence of changes on the surface ECG in many patients, while the reverse is true in some individuals. When a precordial lead is employed, V5 or V6 is most commonly selected. The surface ECG leads most sensitive for monitoring ischemia during left anterior descending angioplasty are not known. In nine lead surface ECGs recorded during balloon inflation, a small degree of ST segment elevation occurred in leads I, aVL, and V5. Lead V2 demonstrated an increase in ST displacement from 0.0 ± 0.03 mV to 0.29 ± 0.25 mV during coronary occlusion (p<0.01). We conclude that if V5 or V6 is used as a single precordial lead, surface ECG alterations are easily overlooked. During left anterior descending occlusion the most sensitive surface lead is V2. Optimal ECG monitoring during PTCA in some cases should involve surface lead V2 or the intracoronary lead.  相似文献

Continuously updated ST-segment recovery analysis has been shown to accurately predict infarct-related artery patency. Salient principles were converted into algorithms and incorporated into a portable ST monitor for optimal application. This study tested the automated program's ability to detect occlusion and reperfusion during balloon angioplasty. ST-segment recordings during 78 balloon occlusions in 31 patients were analyzed. The program requires at least one electrocardiogram with ST elevation of 200 μV or greater in the recording, caused by the current occlusion or by a previous occlusion, before it will yield a patency prediction. All 35 inflations causing peak ST elevation of 200 μV or more were indeed detected. All five inflations causing less than 200 μV ST elevation preceded by an inflation causing 200 μV or higher ST elevation were also detected. Occlusion was detected a median of 40 seconds after inflation, and reperfusion a median of 17 seconds after deflation. Peak ST elevation greater than 200 μV occurred in 19 of 26 left anterior descending artery inflations (73%), 1 of 22 left circumflex artery LCX inflations (5%), and 15 of 30 right coronary artery inflations (50%). Five different leads identified peak ST elevation through 12-lead surveillance. In this model of coronary occlusion during angioplasty balloon inflation, the automated patency assessment program appears to detect coronary angioplasty balloon occlusion and reperfusion within seconds in all occlusions causing a peak ST elevation of 200 μV or greater. Testing this automated patency assessment program as a noninvasive triage tool in myocardial infarction patients seems warranted.  相似文献

The goal of this study was to verify whether myocardial protection could be achieved via the intracoronary administration of propranolol in patients undergoing percutaneous transluminal coronary angioplasty (PTCA). Accordingly, 21 patients undergoing PTCA were randomly assigned to receive either intracoronary placebo (group A, n = 10) or intracoronary propranolol (group B, n = 11). Three balloon inflations (i.e., coronary artery occlusions) were performed in each patient. Inflations I and II (maximum duration 60 sec) served as control occlusions. Inflation III (maximum duration 120 sec) was performed either after intracoronary administration of saline (2 ml) or propranolol (1.1 +/- 0.2 mg). The following electrocardiographic index of myocardial ischemic injury were measured: (1) time to development of ST segment elevation equal to 0.1 mV and (2) magnitude of ST segment elevation after 60 sec of coronary artery occlusion. Both indexes did not differ significantly between the groups during inflations I and II. In group A the time to development of ST segment elevation of 0.1 mV remained unchanged between the second and third occlusions (25 +/- 5 and 26 +/- 4 sec during inflations II and III, respectively). In group B subselective injection of propranolol into the affected coronary artery significantly prolonged the time to ST segment elevation of 0.1 mV from 19 +/- 4 sec (inflation II) to 53 +/- 9 sec (inflation III; p less than .001). 目的探讨回旋支闭塞中不同节段,不同优势型,多支病变对心电图变化的影响。方法本研究共入选246例发生急性LCX闭塞的患者(其中男187例,女59例),根据冠脉造影结果将患者根据冠脉优势型、单支、多支、合并LAD、RCA分组,结合年龄、性别及相关危险因素,对比分析心电图改变与冠脉造影结果及临床特点的关系。结果回旋支闭塞心电图变化受不同冠脉优势型影响,Ⅱ、Ⅲ、aVF、V7~V9导联ST段抬高常见于左优势型的LCX闭塞。V1~V3导联ST段压低常见于均衡型的LCX闭塞,Ⅰ、aVL导联ST段抬高在各优势型中无特异性。在单支LCX闭塞中,V1~V3导联ST段压低常见于近段闭塞,Ⅱ、Ⅲ、aVF导联ST段抬高常见于远段闭塞,V7~V9导联ST段抬高与Ⅰ、aVL导联ST段抬高在各节段闭塞的心电图中无特异性。合并多支病变时LCX心电图变化与单纯LCX闭塞存在差异,在LCX近段闭塞中,合并多支病变的患者更易出现V7~V9导联ST段抬高,单支病变者心电图易出现V1~V3导联ST段压低,在LCX中段闭塞的患者中,单支病变与多支病变的心电图改变大致相同。在LCX远段闭塞的患者中,多支病变患者出现V1~V3导联ST段压低可能性较大。OM闭塞在单支及合并多支病变时的心电图差异无明显统计学意义。在合并LAD或RCA病变的LCX闭塞患者中,心电图改变无明显差异。结论心电图对诊断梗死相关动脉为回旋支的急性心肌梗死有重要的预测价值,结合病史及相关一般资料可对急性心肌梗死患者的预后进行评估。  相似文献

The clinical value of intracoronarv electrography for the detectionofmyocardial ischaemia was assessed during coronary angioplastyand compared to a standard technique of surface ECG monitoring.In 73 patients undergoing single lesion angioplasty, an iniracoronarvelectrogram and four representative surface ECG leads were obtained.During angioplasty of the left anterior descending artery leads,I, V3 V5 V6 were recorded. For the circumflex artery leads I,a VL, a VF, V6 and for the right coronar artery leads II, III,a VF, V6 were monitored. Eight patients were excluded due totransient intraventricular conduction disturbances during ballooninflation; 65 patients remained for further analysis. Out ofa total of 154 balloon inflations (35 in the circumflex, 71in the left anterior descending and 48 in the right coronaryartery), the percentage that produced a mm ST segment elevation,the time to the appearance of a mm ST segment elevation andthe maximal ST segment elevation were recorded. During inflationsin the circumflex artery, the respective values of these threeparameters were 20%, 22·6±11·5 and 0·37±0·80mm in V6 the most sensitive surface lead, versus 70% (P<0·001),14·4±9·6 s (P<0·01 and 5·82±6·35mm (P<0·0001) on the intracoronary electrogram. Forleft anterior descending inflations the corresponding valuesin V3 the most sensitive surface lead, were 61%, 26·2±13·2s and 2·08±2·32mm versus 74% (NS), 18·3±12·4s(P<0·001) and 5 (P<0·0001) on the intracoronarytracing. For right coronary artery inflations the correspondingvalues in V3 the most sensitive surface lead, were 77%, 22·2±12·8s and 2·31±1·65mm versus 32% (P<0·0001),29·8±26·3 (NS) and 1·18±2·19mm(P<0·05 Keeping in mind that only four surface leadswere monitored, these data suggest that intracoronary electrographyis helpful for adequate nonitoring of ischaemia during circumflexangioplasty. In this study ECG changes were analyzed to assess the acute effects of antegrade blood flow on the ECG in patients with AMI. The study population consisted of 22 patients with MI in whom the totally occluded left anterior descending artery (LAD) or right coronary artery (RCA) was recanalized by intracoronary urokinase infusion (recanalized group) and 14 patients in whom the occluded coronary artery was not successfully recanalized (control group). No significant difference was found in the sum of ST segment elevation (V2-V4 leads for the LAD-occluded group, II, III and aVF leads for the RCA-occluded group) before urokinase infusion. In the recanalized group sigma ST abruptly increased in 5 min after recanalization in 13 of 16 LAD-occluded patients from 1.49 +/- 0.89 mV to 2.44 +/- 1.67 mV (p less than 0.005), and in 4 of 6 RCA-occluded patients from 0.66 +/- 0.12 mV to 1.42 +/- 0.52 mV (p less than 0.01). However increased sigma ST in the recanalized group was reduced to the control value existing before recanalization within 30 min after recanalization and continued to decline more rapidly than in the control group. These transient ST segment elevations were not correlated with long-term angiographic determinants of left ventricular function. We conclude that ST segment shows abrupt augmentation after successful thrombolysis and that continuous ST segment monitoring is useful for assessing thrombolysis in AMI.  相似文献

We present a case report of a patient of Ebstein's anomaly presenting with unusual ECG changes during acute coronary syndrome. The patient had undergone radiofrequency ablation of right posteroseptal accessory pathway. Two years later, he presented with acute chest pain. His ECG revealed ST elevation of 6-7 mm in leads III, aVF. V3R and V1-V4 with atrioventricular dissociation. He was thrombolysed for the same. He subsequently underwent an angiogram for continuing angina. His angiogram showed a nondominant right coronary artery with a 95% stenosis. The left circumflex artery was dominant but without any stenosis. The left anterior descending artery was also normal. Angiogplasty and stenting were done for the right coronary artery lesion and the patient did well on follow-up. The ST segment elevation in the anterior precordial leads resulting from occlusion of a nondominant right coronary artery is unusual. The possible reason for this is the isolated right ventricular infarction in the absence of any left ventricular infarction. Thus the electrical current of injury resulting from the right ventricular infarction was unopposed by any counterbalancing current of injury from the inferior surface of the left ventricle.  相似文献

To investigate the mechanism of precordial ST segment depression during right coronary artery occlusion, precordial ST segment shifts and myocardial lactate metabolism were evaluated during coronary angioplasty in 10 patients with (group A) and 7 patients without (group B) precordial ST segment depression during balloon occlusion of the right coronary artery, and in 17 patients with precordial ST segment depression during balloon occlusion of the left anterior descending artery (group C). A 12 lead electrocardiogram was continuously recorded in each patient. Blood lactate in the aorta and great cardiac vein was measured during the procedure, and the lactate extraction ratio in the anterior wall was determined both before and during balloon occlusion. Eight of the 10 patients in group A and 1 of the 7 patients in group B had a dominant large right coronary artery. There were no significant differences in summed ST segment elevation in leads II, III and a VF between group A (0.56 +/- 0.26 mV) and group B (0.46 +/- 0.19 mV) during balloon occlusion of the right coronary artery, and no significant differences in summed ST segment depression in leads V1 to V6 during balloon occlusion between group A (0.44 +/- 0.26 mV) and group C (0.38 +/- 0.14 mV). Lactate extraction ratio before balloon occlusion was similar among the three groups. 目的分析左主干、前降支近端或三支病变冠心病心绞痛患者的心电图改变。方法13例冠脉造影诊断为左主干、前降支近端或三支病变的患者,对其静息时、心绞痛发作时心电图有无aVR、V1导联的ST段抬高及临床资料进行分析。结果胸痛发作时11例出现了aVR、V1导联典型改变,静息时2例aVR、V1导联改变。结论心电图aVR、V1导联ST段抬高,尤其STaVR〉STV1,V4~V6导联ST段下移,合并Ⅱ、Ⅲ、aVF导联或Ⅰ、Ⅱ导联ST段下移与冠脉左主干、前降支近端严重狭窄及三支病变有良好的相关性。  相似文献

Percutaneous transluminal coronary angioplasty was used as a model of controlled myocardial ischemia to study the effect of intravenous dipyridamole on myocardial ischemia and coronary hemodynamics in 10 patients. All patients had 1-vessel coronary artery disease with visualized collaterals. Intravenous dipyridamole increased myocardial ischemia during inflations. ST elevation, as measured by intracoronary electrogram, increased significantly from the control inflation to the second inflation after dipyridamole injection (0.05 +/- 0.23 vs 0.44 +/- 0.43 mV, p less than 0.03). Of the 10 patients, 8 developed new or more severe angina with subsequent inflations after dipyridamole. The pulmonary artery wedge pressure increased significantly from the control inflation to the fourth inflation (15 +/- 8 vs 20 +/- 9 mm Hg, p less than 0.05). The coronary wedge pressure showed a decreasing trend with subsequent inflations after dipyridamole but did not reach statistical significance. The double product (heart rate X blood pressure) was not significantly altered by dipyridamole. The findings indicate that intravenous dipyridamole increases myocardial ischemia during balloon occlusion. The constancy of the double product and the trend toward a decrease in coronary wedge pressure suggest that dipyridamole may induce ischemia by reducing the amount of collateral flow through a coronary steal phenomenon.  相似文献

In order to study myocardial and clinical events during transient coronary occlusion in humans, two-dimensional echocardiography was continuously performed in 15 patients undergoing 49 balloon inflations during percutaneous transluminal coronary angioplasty (PTCA). Transient segmental asynergy developed in all patients 8 +/- 3 seconds after balloon inflation and returned to baseline 19 +/- 8 seconds after balloon deflation. Segmental dyskinesis was seen in only 8 of 11 patients undergoing PTCA of the left anterior descending artery (LAD). A wall motion score, based on degree of asynergy of 13 segments of the left ventricle, was significantly higher during LAD than during right coronary artery inflation (7.9 +/- 1.3 vs 4.0 +/- 1.4, p less than 0.01). Left ventricular size index increased significantly during balloon inflation, from 179 +/- 9 to 196 +/- 10 mm (p less than 0.01). Four patients developed transient ST segment changes in the extremity leads of the ECG and five patients had angina pectoris. The very first sign of ischemia in three patients, who developed all of these symptoms together, was consistently asynergy, followed by ECG changes, and last, angina pectoris. Thus during PTCA, transient asynergy and left ventricular dilatation develop, which are often clinically silent.  相似文献

目的探讨冠状动脉前降支(LAD)单支闭塞所致急性心肌梗死(AMI)的心电图(ECG)特点。方法回顾性分析51例LAD急性闭塞所致AMI患者心电图ST段改变与冠状动脉造影结果。结果 V_2导联ST段抬高<0.2 mV预测LAD远段闭塞敏感度为52.9%,特异度为88.2%,在胸导联ST段抬高的情况下,特异度增高(96.2%)。V_6导联ST段压低预测LAD近段闭塞敏感度为26.5%,特异度100%,在胸导联ST段抬高的情况下,V_1导联ST段抬高≥0.2 mV预测LAD近段闭塞敏感度53.8%,特异度88.9%。下壁导联Ⅱ、Ⅲ、aVF ST段压低预测LAD近段闭塞特异度较高,Ⅲ、aVF导联组合较Ⅱ、Ⅲ、aVF导联组合判断LAD近段闭塞特异度及阳性预测值高。结论 V_1导联ST段抬高、V_6导联ST段压低、下壁导联ST段压低有助于判断LAD近段闭塞,V_2导联ST段抬高<0.2 mV有助于判断LAD远段闭塞。  相似文献

The optimal number and placement of electrocardiographic (ECG) leads to detect myocardial ischemia induced by coronary balloon inflation was assessed by analyzing ST segment changes in the standard 12-lead ECG and Frank X, Y, Z leads at 90-s intervals during 34 consecutive coronary angioplasty procedures. Mean occlusion time during angioplasty was 218 +/- 65 s. Myocardial ischemia, defined as transient angina or ST segment deviation greater than or equal to 1 mm in at least one lead, occurred in 33 (97%) of the 34 procedures. The most sensitive single leads (V2 or V3) detected 17 (51%) of 33 ischemic episodes. The best dual-lead combinations (leads V2 and V5, leads a VF and V3 and leads V3 and Y) increased the sensitivity of 69% (23 of 33). The three-lead combination V2, V5, Y had the highest detecting power (78% [26 of 33]). The X, Y, Z leads by themselves had a sensitivity of only 60% (20 of 33). From this proposed orthogonal lead system (V2, V5, Y), which combines anteroposterior (V2), left to right (V5) and inferosuperior (Y) forces, the spatial ST vector magnitude was calculated and monitored during balloon inflations. A good correlation was observed between this ST vector magnitude and the sum of ST deviations on the standard ECG (r = 0.940, p less than 0.00001), and these data were reproducible over sequential balloon inflations. The results of the study suggest that this orthogonal lead system is of considerable value in the detection and quantification of acute myocardial ischemia and, in this respect, is more useful than the Frank orthogonal vector system.  相似文献

Acute left main coronary artery (LMCA) occlusion may result in acute myocardial infarction (AMI) or sudden death. ST elevation in the aVR and V1 leads is reported to be valuable in recognizing LMCA occlusion. Early recognition of electrocardiogram (ECG) changes, such as reciprocal ST depression in other leads, is helpful in averting this disaster. This study aimed to determine the reciprocal ST segment depression of 12-lead ECGs associated with acute LMCA occlusion. From January 2000 to December 2004, 61 patients who underwent emergency percutaneous coronary intervention in 3 hospitals due to AMI associated with LMCA (n = 18) and a left anterior descending coronary artery (LADCA) (n = 43) proximal lesion were selected. Reciprocal ST segment depression occurred in leads aVF, V(2), V(3), V(4), V(5), and V(6) with significantly higher incidence in the LMCA group than in the LADCA group. Stepwise linear multivariate discriminant analysis indicated that ST segment depression in leads aVF, V(2), and V(4) could distinguish the LMCA group from the LADCA group. We concluded that reciprocal ST segment depression in leads V(2), V(4), and aVF of a 12-lead ECG is an important predictor of acute LMCA occlusion.  相似文献

We describe a patient with acute coronary syndrome, presenting with upsloping ST depression in leads I, II, V3–V6 and ST elevation in lead aVR. Coronary angiography revealed spontaneous dissection in a big, dominant left circumflex artery. No other lesions identified. During stenting of the dissection site, the distal left circumflex, supplying a large posterior descending artery was occluded, resulting in ST elevation myocardial infarction with ST elevation in lead III and aVF, but not II. This pattern is considered to represent right coronary artery infarction, rather than left circumflex infarction.  相似文献

记录70例冠心病人经皮冠状动脉腔内成形术时体表和冠脉内心电图发现,随球囊充盈次数增加,ST段抬高幅度减低,心绞痛出现时间延长,最大QT间期缩短及QT离散度和JT离散度减低,提示PTCA时多次短暂心肌缺血可能由于缺血预适应而诱发心肌内在抗心律失常保护作用。  相似文献
