AIM: To determine the feasibility and effectiveness of endoscopic resection for the treatment of colorectal granular cell tumors (GCTs).METHODS: This was a retrospective study performed at a single institution. From January 2008 to April 2015, we examined a total of 11 lesions in 11 patients who were treated by an endoscopic procedure for colorectal GCTs in the Endoscopy Center, Zhongshan Hospital of Fudan University, Shanghai, China. Either endoscopic mucosal resection or endoscopic submucosal dissection (ESD) was performed by three surgeons with expertise in endoscopic treatment. The pre- and post-operative condition and follow-up of these patients were evaluated by colonoscopy and endoscopic ultrasonography (EUS).RESULTS: Of these 11 lesions, 2 were located in the cecum, 3 were in the ileocecal junction, 5 were in the ascending colon, and 1 was in the rectum. The median maximum diameter of the tumors was 0.81 cm (range 0.4-1.2 cm). The en bloc rate was 100%, and the complete resection rate was 90.9% (10/11). Post-operative pathology in one patient showed a tumor at the cauterization margin. However, during ESD, this lesion was removed en bloc, and no tumor tissue was seen in the wound. No perforations or delayed perforations were observed and emergency surgery was not required for complications. All patients were followed up to May 2015, and none had recurrence, metastasis, or complaints of discomfort.CONCLUSION: Endoscopic treatment performed by endoscopists with sufficient experience appears to be feasible and effective for colorectal GCTs.  相似文献

A 78-year-old woman was referred to our department for treatment of an early gastric cancer. Esophagogastroduodenoscopy (EGD) demonstrated a flat elevated lesion and a polypoid lesion on the greater curvature of the antrum. Histological analysis of, endoscopic biopsy samples taken from these lesions revealed an adenocarcinoma and a hyperplastic polyp, respectively. ESD was conducted for removal of the lesions. Carbon dioxide (CO2) in- stead of room air was used for insufflation, and the patient was adequately sedated without struggling or vomiting during the treatment. No significant bleeding from the lesion was observed during ESD, but fresh blood was identified endoscopically. Surprisingly, a Mallory- Weiss tear with active bleeding was detected on the lesser curvature of the gastric corpus. A total of eight hemoclips were applied for hemostasis. Both lesions were completely removed en bloc, and no bleeding or perforation developed after ESD. Histologically, the first lesion was apapillary carcinoma limited to the mucosal layer and without lymphovascular invasion or involvement of the surgical margins, while the second lesion was a benign hyperplastic polyp.  相似文献

目的探讨早期巴雷特食管腺癌(Barrett esophageal adenocarcinoma,BEA)的临床、内镜和病理特征,并评价内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)对其治疗效果。方法2015年11月至2018年6月,在北京友谊医院确诊早期BEA,并行ESD治疗的13例病例纳入回顾性分析,对其临床资料、内镜资料和病理资料进行收集和分析。结果13例患者中,男性10例。巴雷特食管:长段1例,短段和超短段(短于1 cm)各6例;全周型2例,舌型11例。内镜下10例位于食管胃结合部右前方(12~2点钟方位),0-Ⅱ型(浅表平坦型)12例,13例均成功完成ESD治疗,整块切除率100%(13/13),治愈性切除率为92%(12/13),无治疗相关并发症。术后病理高分化9例;黏膜内癌10例。随访11例,随访时间3.3~29.3个月,未见复发者。结论13例早期BEA患者中,男性、非长段和舌状巴雷特食管患者为主,病灶多位于食管胃结合部的右前方,内镜下以浅表平坦型为主,病理多为高分化腺癌,多局限于黏膜层,ESD治疗安全有效。  相似文献

Treatment strategies,whether as follow-up or"total incisional biopsy"for gastric noninvasive intraepithelial neoplasia diagnosed by examination of an endoscopic forceps biopsy specimen,are controversial due to problems associated with the diagnostic accuracy of endoscopic forceps biopsy and questions about the safety and efficacy of endoscopic treatment.Based on the histological findings of the biopsy specimen,it is difficult to differentiate between reactive or regenerative changes,inflammation and neoplastic changes,intraepithelial and invasive tumors.Therefore,gastric neoplasia diagnosed as noninvasive intraepithelial often develop into invasive carcinoma during follow-up.Recent advances in endoscopic modalities and treatment devices,such as image-enhanced endoscopy and highfrequency generators,may make endoscopic treatment,such as endoscopic submucosal dissection(ESD),a therapeutic option for gastric intraepithelial neoplasia,including low-grade neoplasms.Future studies are required to evaluate whether ESD is a valid strategy for gastric intraepithelial neoplasm with regard to safety and cost effectiveness.  相似文献

内镜下黏膜剥离术治疗消化道黏膜增生性病变88例分析   总被引:1,自引:0,他引:1  
目的探讨内镜下黏膜剥离术(ESD)治疗消化道黏膜增生性病变的临床疗效。方法将宜兴市人民医院收治的消化道黏膜增生性病变患者88例,随机分成内镜下黏膜切除术(EMR)组(44例)和ESD组(44例)。术后随访12个月,观察两组的临床疗效及穿孔、出血等并发症及复发情况。结果与EMR组比较,ESD组的手术时间明显增加,但一次性切除率明显升高(P0.05);术后6个月,ESD组切面愈合率显著高于EMR组(P0.05),ESD组术后12个月的复发率显著低于EMR组(P0.05)。结论与EMR相比,ESD治疗消化道黏膜增生性病变的一次性切除率和术后切面愈合率均较高,且术后12个月的复发率较低。  相似文献

Traditional serrated adenoma(TSA)is a type of serrated polyp of the colorectum and is thought to be a precancerous lesion.There are three types of serrated polyps,namely,hyperplastic polyps,sessile serrated adenomas/polyps,and TSAs.TSA is the least common of the three types and accounts for about 5% of serrated polyps.Here we report a pediatric case of TSA that was successfully resected by endoscopic submucosal dissection(ESD).This rare case report describes a pediatric patient with no family history of colonic polyp who was admitted to our hospital with hematochezia.On colonoscopy,we found a polypoid lesion measuring 10 mm in diameter in the lower rectum.We selected ESD as a surgical option for en bloc resection,and histopathological examination revealed TSA.The findings in this case suggest that TSA with precancerous potential can occur in children,and that ESD is useful for treating this lesion.  相似文献

目的评价内镜黏膜下隧道法剥离术(endoscopic submucosal tunnel dissection,ESTD)治疗早期食管癌伴黏膜下层纤维化的效果和安全性。方法2015年6月—2018年2月间,在江苏省苏北人民医院消化内科采用ESTD或内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)治疗,术后病理证实病灶<1/3食管管周,且伴有黏膜下层纤维化的早期食管癌病例87例纳入回顾性分析,按纤维化程度分成轻度纤维化60例(ESTD 31例、ESD 29例)和重度纤维化27例(ESTD 16例、ESD 11例),比较同一纤维化程度时两种手术方式的剥离速度、整块切除率、完全切除率,以及出血、肌层损伤、穿孔、颈部皮下气肿和术后狭窄的发生率。结果对于伴有轻度黏膜下层纤维化的早期食管癌患者,ESTD的整块切除率[96.8%(30/31)比82.8%(24/29),P<0.05]和完全切除率[96.8%(30/31)比75.9%(22/29),P<0.05]明显高于ESD,固有肌层损伤发生率明显低于ESD[6.5%(2/31)比17.2%(5/29),P<0.05],剥离速度、术中出血发生率、穿孔发生率、术后狭窄发生率与ESD比较差异均无统计学意义(P均>0.05),两种手术方式均无术后迟发性出血和颈部皮下气肿发生。对于伴有重度黏膜下层纤维化的早期食管癌患者,ESTD的剥离速度快于ESD[(12.3±2.8)mm2/min比(7.1±3.2)mm2/min],整块切除率、完全切除率、术后狭窄发生率与ESD相近,术中出血发生率[12.5%(2/16)比54.5%(6/11)]、固有肌层损伤发生率[18.8%(3/16)比54.5%(6/11)]、穿孔发生率[6.3%(1/16)比27.3%(3/11)]、颈部皮下气肿发生率[6.3%(1/16)比27.3%(3/11)]低于ESD,两种手术方式均无术后迟发性出血发生。术后12个月2例行ESD和1例行ESTD患者局部复发,术后24个月1例行ESTD患者发生异时癌。结论ESTD能安全、有效切除伴有黏膜下层纤维化的早期食管癌。对于伴有轻度黏膜下层纤维化者,ESTD的优势主要体现在治疗效果方面;对于伴有重度黏膜下层纤维化者,ESTD的优势主要体现在治疗安全性方面。  相似文献

Background/Aims

When undergoing endoscopic submucosal dissection (ESD), patients with liver cirrhosis (LC) may suffer from a high risk of bleeding, bacteremia and tissue vulnerability. There have been few reports evaluating the efficacy and safety of ESD in patients with LC.

Methods

From January 2004 to March 2010, 23 patients with LC (cirrhosis group) underwent ESD for superficial gastric neoplastic lesions. The number of patients with a liver function in the Child-Pugh classes A and B were 20 and 3, respectively. The clinical outcomes and complications were compared with 69 patients without LC (control group) that were matched for age and sex.

Results

The en bloc resection, R0 resection and en bloc plus R0 resection rates of the cirrhosis group were 82.6%, 91.3%, and 82.6%, respectively, and did not show significant differences from the rates of the control group. No local recurrence was found in either group during the follow-up period. The procedure length of time (41.0 vs 39.0 minutes), rate of bleeding (4.3% vs 7.2%) and rate of perforation (0.0% vs 1.4%) in the cirrhosis group were also comparable to the results from the control group.

Conclusions

ESD was safely performed in patients with LC, and satisfactory outcomes were achieved with high en bloc and R0 resection rates for superficial gastric neoplastic lesions.  相似文献

目的 对比观察内镜下黏膜切除术(endoscopic mucosal resection,EMR)与内镜黏膜下层剥离术(endoscopic submucosal dissection,ESD)对大型低位直肠肿瘤的治疗效果。方法选择肿瘤直径大于3.0cm,肿瘤下缘距肛缘齿状线小于5cm,有内镜治疗适应证的56例低位直肠肿瘤,应用EMR或ESD进行治疗,其中EMR治疗36例,ESD治疗20例,术后3~18个月行内镜随访确认有无残留,以评价切除效果,记录术中及术后发生的并发症及处理情况,并分析切除标本的病理组织学结果。结果 接受EMR治疗的36例中,35例经首次或再次EMR治疗病变完整清除,肛门功能完好,保肛治愈率为97.2%,术后病理报浸润癌(SM癌)再追加外科Mile’s根治手术者1例(2.8%);接受ESD治疗的20例中,11例经首次或再次ESD治疗完整清除病变,肛门功能完好,治愈率55.0%,ESD治疗未成功改行EMR成功清除病变6例(30.0%),肛门功能均完好,全组保肛治愈率为85.0%,ESD组因严重并发症(迟发性大出血)转外科行Mile’s手术者2例(10.0%),因病变残留转行外科Mile’s手术者1例(5.0%)。并发症:EMR组术中平均出血20ml,最大出血160ml,均无需输血治疗,无穿孔发生,无术后并发症。ESD组平均术中出血150ml,最大术中出血量800ml,均内镜下止血成功,但3例患者需接受输血400ml,另有2例于术后26h及44h发生迟发大出血,内镜下止血失败转行外科手术。结论EMR是一种安全微创的内镜治疗手段,对大多数平坦型大肠肿瘤能达到完全切除效果,与EMR相比,ESD对低位直肠病变切除的效果不及EMR术,且手术风险更大。  相似文献

目的 探讨内镜黏膜下剥离术(ESD)治疗胃肠道黏膜切除术(EMR)后残留、复发病变的疗效和安全性.方法 自2006年6月至2007年11月对EMR术后内镜随访发现的15例残留和复发病变进行ESD治疗,先于黏膜下注射生理盐水以抬高病变,接着预切开病变周围黏膜,再沿病变下方黏膜下层进行剥离,对瘢痕形成部位直接应用Hook刀沿瘢痕基底切线方向进行切开.结果 15例EMR术后残留和复发病变,位于胃6例、结肠3例、直肠6例,病变直径0.8~3.5 cm,平均2.3 cm.所有病变抬举征(一).14例病变成功完成ESD治疗,成功率93.3%(14/15),13例术后病理切缘和基底无肿瘤累及,完整切除率86.7%(13/15).ESD手术时间60~155 min,平均87 min.治疗中创面均有少量出血,全组未出现术后出血.2例治疗中局部剥离较深、腹腔出现游离气体,成功保守治疗,未转外科手术,ESD穿孔发生率13.3%(2/15).术后平均随访13个月,无复发.结论 ESD治疗EMR术后残留和复发病变相对安全、有效,不仅能完整切除残留和复发病变,还能提供完整的病理诊断资料.  相似文献

AIM: To report the endoscopic treatment of large hyperplastic polyps of the esophagus and esophago-gastric junction (EGJ) associated with Barrett's esophagus (BE) with low-grade dysplasia (LGD), by endoscopic mucosal resection (EMR). METHODS: Cap fitted EMR (EMR-C) was performed in 3 patients with hyperplastic-inflammatory polyps (HIPs) and BE. RESULTS: The polyps were successfully removed in the 3 patients. In two patients, with short segment BE (SSBE) (<= 3 cm), the metaplastic tissue was completely excised. A 2 cm circumferential EMR was performed in one patient with a polyp involving the whole EGJ. A simultaneous EMR-C of a BE-associated polypoid dysplastic lesion measuring 1 cm multiply 10 cm, was also carried out. In the two patients, histologic assessment detected LGD in BE. No complications occurred. Complete neosquamous re-epithelialization occurred in the two patients with SSBE. An esophageal recurrence occurred in the remaining one and was successfully retreated by EMR. CONCLUSION: EMR-C appears to be a safe and effective method for treating benign esophageal mucosal lesions, allowing also the complete removal of SSBE.  相似文献

Compared with endoscopic submucosal dissection (ESD), endoscopic mucosal resection (EMR) is easier to perform and requires less time for treatment. However, EMR has been replaced by ESD, because achieving en bloc resection of specimens > 20 mm in diameter is difficult with EMR. The technique of ESD was introduced to resect large specimens of early gastric cancer in a single piece. ESD can provide precise histological diagnosis and can also reduce the rate of recurrence, but has a high level of technical difficulty, and is consequently associated with a high rate of complications, a need for advanced endoscopic techniques, and a lengthy procedure time. To overcome disadvantages in both EMR and ESD, various advances have been made in submucosal injections, knives, other accessories, and in electrocoagulation systems.  相似文献

Cap polyposis is a rare intestinal disorder. Characteristic endoscopic findings are multiple inflammatory polypoid lesions covered by caps of fibrous purulent exudate. Although a specific treatment has not been established, some studies have suggested that eradication therapy for Helicobacter pylori(H. pylori) is effective. We report a case of a 20-year-old man with cap polyposis presenting with hematochezia. Colonoscopy showed the erythematous polyps with white caps from the sigmoid colon to rectum. Histopathological findings revealed elongated, tortuous, branched crypts lined by hyperplastic epithelium with a mild degree of fibromusculosis in the lamina propria. Although H. pylori eradication was instituted, there was no improvement over six months. We then performed en bloc excision of the polyps by endoscopic submucosal dissection(ESD), which resulted in complete resolution of symptoms. ESD may be a treatment option for cap polyposis refractory to conservative treatments. We review the literature concerning treatment for cap polyposis and clinical outcomes.  相似文献

AIM: To investigate factors related to recurrence following en bloc resection using endoscopic submucosal dissection(ESD) in patients with early gastric cancer(EGC). METHODS: A total of 1121 patients(1215 lesions) who had undergone ESD for gastric neoplasia between April 2003 and May 2010 were retrospectively reviewed. Data from 401 patients(415 lesions) were analyzed, following the exclusion of those who underwent piecemeal resection, with deep resection margin invasion or lateral margin infiltration, and diagnosed with benign lesions. RESULTS: Local recurrence after en bloc ESD was found in 36 cases(8.7%). Unclear resection margins, long procedure times, and narrow safety margins were identified as risk factors for recurrence. Lesions located in the upper third of the stomach showed a higher rate of recurrence than those located in the lower third of the stomach(OR = 2.9, P = 0.03). The probability of no recurrence for up to 24 mo was 79.9% in those with a safety resection margin ≤ 1 mm and 89.5% in those with a margin 1 mm(log-rank test, P = 0.03). CONCLUSION: Even in cases in which en bloc ESD is performed for EGC, local recurrence still occurs. To reduce local recurrences, more careful assessment will be needed prior to the implementation of ESD in casesin which the tumor is located in the upper third of the stomach. In addition, clear identification of tumor boundaries as well as the securing of sufficient safety resection margins will be important.  相似文献

Endoscopic polypectomy and endoscopic mucosal resection(EMR) are the established treatment standards for colorectal polyps. Current research aims at the reduction of both complication and recurrence rates as well as on shortening procedure times. Cold snare resection is the emerging standard for the treatment of smaller(< 5 mm) polyps and is possibly also suitable for the removal of noncancerous polyps up to 9 mm. The method avoids thermal damage, has reduced procedure times and probably also a lower risk for delayed bleeding. On the other end of the treatment spectrum, endoscopic submucosal dissection(ESD)offers en bloc resection of larger flat or sessile lesions. The technique has obvious advantages in the treatment of high-grade dysplasia and early cancer. Due to its minimal recurrence rate, it may also be an alternative to fractionated EMR of larger flat or sessile lesions. However, ESD is technically demanding and burdened by longer procedure times and higher costs. It should therefore be restricted to lesions suspicious for high-grade dysplasia or early invasive cancer.The latest addition to endoscopic resection techniques is endoscopic fullthickness resection with specifically developed devices for flexible endoscopy.This method is very useful for the treatment of smaller difficult-to-resect lesions,e.g., recurrence with scar formation after previous endoscopic resections.  相似文献

AIM: To investigate the indications, resection rate, and safety of endoscopic submucosal dissection (ESD) for neoplastic lesions in the gastrointestinal tract at a European referral center. METHODS: We carried out a retrospective analysis of the ESD procedures performed in our center for mucosal neoplastic and submucosal lesions of the gastrointestinal tract. The duration of the procedure, en bloc and complete (R0) resection rates, and complication rates were evaluated. Variables were reported as mean ± SD or simple proportions. Univariate analysis and comparisons of procedure times and resection rates were performed using Mann-Whitney U tests, or χ2 tests for dichotomous variables.RESULTS: Between 2007 and 2011, ESD was performed in a total of 103 patients (46.7% male, mean age 64.0 ± 12.7 years). The indications for the procedure were epithelial tumor (n = 54), submucosal tumor (n = 42), or other (n = 7). The total en bloc resection rate was 90.3% (93/103) and R0 resection rate 80.6% (83/103). The median speed of the procedure was 15.0 min/cm2 . The complete resection rate was lower for submucosal tumors arising from the muscle layer (68%, 15/22, P < 0.05). Resection speed was quicker for submucosal tumors localized in the submucosal layer than for lesions arising from the muscularis propria layer (8.1 min/cm 2 vs 17.9 min/cm 2 , P < 0.05). The R0 resection rate and speed were better in the last 24 mo (90.1%, 49/54 and 15.3 min/cm 2 ) compared to the first 3 years of treatment (73.5%, 36/49, P < 0.05 and 22.0 min/cm2 , P < 0.05). Complications occurred in 14.6% (n = 15) of patients, including perforation in 5.8% (n = 6), pneumoperitoneum in 3.9% (n = 4), delayed bleeding in 1.9% (n = 2), and other in 2.9% (n = 3). Only one patient with delayed perforation required surgical treatment. During the mean follow-up of 26 ± 15.3 mo, among patients with R0 resection, recurrence occurred in one patient (1.2%).CONCLUSION: ESD is an effective and safe method for resection of neoplastic lesions with low re  相似文献

AIM To investigate the feasibility and safety of secondary endoscopic submucosal dissection(ESD) for residual or locally recurrent gastric tumors. METHODS Between 2010 and 2017, 1623 consecutive patients underwent ESD for gastric neoplasms at a single tertiary referral center. Among these, 28 patients underwent secondary ESD for a residual or locally recurrent tumor. Our analysis compared clinicopathologic factors between primary ESD and secondary ESD groups. RESULTS The en bloc resection and curative rate of resection of secondary ESD were 92.9% and 89.3%, respectively. The average procedure time of secondary ESD was significantly longer than primary ESD(78.2 min vs 55.1 min, P = 0.004), and the adverse events rate was not significantly different but trended slightly higher in the secondary ESD group compared to the primary ESD group(10.7% vs 3.8%, P = 0.095). Patients who received secondary ESD had favorable outcomes without severe adverse events. During a mean follow-up period, no local recurrence occurred in patients who received secondary ESD. CONCLUSION Secondary ESD of residual or locally recurrent gastric tumors appears to be a feasible and curative treatment though it requires greater technical efficiency and longer procedure time.  相似文献

背景:内镜黏膜下剥离术(ESD)目前已广泛应用于胃癌前病变和早期胃癌的根治。普通白光内镜检查难以发现ESD术后的残留和复发病变,而共聚焦激光显微内镜(CLE)可对黏膜病变作出实时体内组织学诊断而无需活检。目的:探讨CLE在ESD术后残留和复发病变诊断中的应用价值。方法:连续纳入2009年6月一2011年12月上海仁济医院因胃上皮内瘤变或早期胃癌而接受ESD治疗的患者,分别于术后1、3、6个月行CLE随访。综合首次ESD、CLE复查发现病变后追加的手术切除以及CLE活检病理结果作出最终诊断,并与CLE诊断进行对照。结果:共20例行ESD治疗者纳入研究,所有患者均完成CLE随访。ESD术后1个月CLE复查发现2例残留病变,术后6个月CLE复查发现1例复发病变,CLE诊断均为高级别上皮内瘤变(HGIN)。追加剖腹手术病理诊断2例残留病变为HGIN,1例复发病变为早期分化型腺癌。其余17例患者3次CLE复查均未见病变残留或复发。结论:CLE检查可准确诊断ESD术后残留和复发病变,对ESD术后内镜随访具有重要价值。  相似文献

AIM:To assess whether the use of porcine models is useful for learning endoscopic submucosal dissection(ESD),thus contributing to its subsequent application in human patients.METHODS:This study/learning process was carried out in 3 phases:PhaseⅠ:Ex vivo animal;PhaseⅡ:In vivo animal;PhaseⅢ:Humans.One endoscopist performed 30 gastric ESDs in porcine models,and later 5gastric ESDs in 5 patients.The ESD was done following the method practiced at the National Cancer Center in Tokyo,Japan.Technical aspects,size,time and speed of ESD,as well as complications were registered.In patients,their clinical,endoscopic and histologic evolution was additionally added.RESULTS:Thirty en bloc ESDs were carried out in animal models.The mean±SD size of the pieces was of28.4±1.2 mm,and the time of ESD was 41.7±2.4min.The time of ESD in the first 15 procedures was 43.0±3.0 min whereas in the next 15 procedures,the time was 40.3±3.9 min,P=0.588.The speed in the first 15ESDs was 1.25±0.11 cm2/min vs 2.12±0.36 cm2/min in the remaining 15,P=0.028.There were no complications.In patients,5 lesions were resected en bloc.The size of the pieces was 25.2±5.1 mm and the time was85.0±25.6 min.Endoscopic and histological controls did not show evidence of residual neoplastic tissue.CONCLUSION:A sequential ESD training program of a unique endoscopist,based on the practice in porcine models,contributed to learning ESD for its subsequent application in humans,yielding good results in efficacy and safety.  相似文献

An 80-year-old man was under annual surveillance esophagogastroduodenoscopy after endoscopic submucosal dissection(ESD) for early gastric cancer(EGC).Two years after the initial ESD, a 0-Ⅱc type metachronous EGC lesion, 8 mm in size, without an ulcer scar, was found in the gastric antrum.The estimated tumor depth was up to the mucosa, and biopsy revealed well and poorly differentiated adenocarcinoma.ESD was performed for this lesion and en bloc resection with negative margins was achieved.Histopathological examination revealed an adenosquamous carcinoma 8 mm in size invading the deep submucosal layer(1600 μm), with lymphovascular invasion, consistent with the diagnosis of non-curative resection.Additional gastrectomy was recommended for this patient; however, two months after the ESD, preoperative computed tomography revealed multiple liver metastases, and the patient was considered as an unsuitable candidate for surgical resection.Systemic chemotherapy was therefore started; however, the patient died of gastric cancer 27 mo after the second ESD.Early gastric adenosquamous carcinoma localized to the mucosa and submucosa is extremely rare and its clinical behavior is not well known.The present report is very significant in that it underscores the distinct possibility of gastric adenosquamous carcinoma being very aggressive and fatal even when detected at an early cancer.  相似文献
