Despite the fact that smoking cigarettes harms oral and systemic health, there is little current evidence of successful primary dental care involvement in smoking cessation.This paper aims to present the arguments for developing the role of primary dental care in smoking cessation and reviews the current literature on how this can be achieved in practice. It is argued that it is timely to incorporate this rapidly developing evidence base on what works in smoking cessation into primary dental care activity.  相似文献

The aim of this study was to explore the complexities in constructing league tables purporting to measure orthodontic clinical outcomes. Eighteen orthodontists were invited to participate in a cost-effectiveness study. Each orthodontist was asked to provide information on 100 consecutively treated patients. The Index of Complexity, Outcome, and Need (ICON) was used to assess treatment need, complexity, and outcome prior to, and on completion of, orthodontic treatment. The 18 orthodontists were ranked based on achieving a successful orthodontic outcome (ICON score less than or equal to 30) and the uncertainty in both the success rates and rankings was also quantified using confidence intervals. Successful outcomes were achieved in 62 per cent of the sample (range 19-94 per cent); four of the 18 orthodontists failed to achieve more than a 50 per cent success rate. In developing league tables, it is imperative that factors such as case mix are identified and accounted for in producing rankings. Bayesian hierarchical modelling was used to achieve this and to quantify uncertainty in the rankings produced. When case mix was taken into account, the four with low success rates were clearly not as good as the top four performing orthodontists. League tables can be valuable for the individual orthodontist, groups of orthodontists, payment/insurance agencies, and the public to enable informed choice for orthodontic provision but must be correctly constructed so that users can have confidence in them.  相似文献

When treating children, important ethical issues may arise relating to the best interests of the child and decision making for minors. The case of a child with a cleft lip and palate whose parents failed to bring her in for medically indicated orthodontic care is presented. Ethical features of the case are discussed, including the need to benefit the patient, avoid harm, and respect the preferences of the parents. Ethical codes of the American Dental Association and American Medical Association are referenced. Ethical dilemmas include the conflict between the orthodontist's obligation to the child and the need to respect parental autonomy. Parental autonomy is respected up until the point at which significant harm to a child may result. The orthodontist's primary ethical responsibility is to the child, not the parents. The orthodontist providing medically indicated care should involve the craniofacial team or hospital social worker when parental decision making is in question.  相似文献

The occupational stress associated with many professions, including general dentistry, has been well researched. An anonymous, self-administered, mail-out survey was distributed to Canadian orthodontists. The survey included 67 potential stressors, an overall occupational stress score, an overall job satisfaction scale, and items addressing various characteristics of the respondents. The response rate was 51.2% (335/654). Pronounced differences were found between the respondents in the evaluation of potential stressors and the overall occupational stress score. The category of stressors with the highest mean severity of stress scores was time-related stressors. The stressors with high mean severity scores and high mean frequency scores were as follows: falling behind schedule, trying to keep to a schedule, constant time pressures, patients with broken appliances, and motivating patients with poor OH and/or decalcification. Stepwise multiple regression determined a model, involving overall job satisfaction, age, participation in a study group, hours worked per week, part-time academics, days of continuing education per year, and participation in stress management, to account for 35.9% of the variation in overall occupational stress scores. The results indicate the importance of time-management skills in reducing occupational stress, but other factors seem to have more effect on reported occupational stress than do the characteristics addressed by this survey.  相似文献

Job satisfaction has been well researched for many professions, including general dentistry. The job satisfaction of orthodontists has not been adequately studied. The aims of this study were to describe job satisfaction among orthodontists and to determine characteristics associated with job satisfaction in the profession. A self-administered, anonymous survey was mailed to Canadian orthodontists. It included a modified version of the Dentist Satisfaction Survey, an overall occupational stress score, and items addressing various characteristics of the respondents. Of 654 mailed surveys, 335 were returned, for a response rate of 51.2%. Most orthodontists (79.3%) were classified as satisfied according to the overall job satisfaction scale of the Dentist Satisfaction Survey; however, some (2.5%) were classified as dissatisfied. The facets of orthodontics with the highest degree of satisfaction were patient relations (93%), delivery of care (86%), respect (84%), professional relations (80%), and staff (76%). The most dissatisfaction was associated with personal time (26%) and practice management (15%). Stepwise multiple regression analysis resulted in a model including overall occupational stress, membership in the Canadian Association of Orthodontists, total number of staff, and age to account for 27.1% of the variation in the overall job satisfaction scale. Based on accountable sources of variance, the overall job satisfaction scale seems to be more affected by other variables than the characteristics evaluated by this survey.  相似文献

Objectives:To determine how often general dentists receive gifts from orthodontists, the value and number of the gifts they receive, and how they perceive the motivation behind the gift.Materials and Methods:This was a questionnaire-based study. A questionnaire was constructed and tested for validity and reliability. An electronic version of survey was sent via email to 1300 general dentists.Results:The validity and reliability of the survey was confirmed. Two hundred fifty-four valid responses were received (20%). Eighty-five percent of responding general practitioners reported that they received gifts from an orthodontist. Almost 100% reported that they referred patients to orthodontists. About one-third of the responding general practitioners reported that their office provided orthodontic care. There were statistically significant correlations between the number of annual patient referrals the general practitioners reported making and the number and value of the gifts they received from the orthodontists. Female general practitioners reported receiving a higher number of gifts of greater total value than male practitioners. General practitioners who reported providing orthodontic treatment did not differ from those who did not in the number of referrals they made annually and the number and value of the gifts they received. Quality of care was the most common reason general practitioners reported for their referral to an orthodontist. Forty-four percent of the responders reported that they received discounted orthodontic treatment.Conclusions:General practitioners refer patients to orthodontists and receive gifts from them. The number and value of the gifts reflects the number of referrals they make.  相似文献

This study examined the accuracy and reliability with standardized definitions among 30 orthodontists in assessing different aspects of malocclusion from the study casts of 10 patients. Previous studies have indicated a lack of consistency among orthodontists in describing malocclusions. The diagnostic subcategories measured in this study included maxillary and mandibular crowding, overbite, overjet, and a modified molar, premolar, and canine Angle classification. Intraclass correlations indicated that the reliability of all parameters was good (intraclass correlation > 0.80), with overbite being the most reliable (0.98) and maxillary crowding and canine classification somewhat less reliable (0.89 and 0.87, respectively). Among Angle classifications, molar classification was the highest (0.95), followed by premolar (0.92) and canine (0.87). This study also measured the accuracy of the orthodontists' measurements compared with established true values (validity). T tests indicated that mandibular arch crowding and overbite were not statistically different from the true values. Overjet and molar classification were assessed very close to the true values. Although premolar and canine classifications were statistically different from the true values, they were within a one-eighth cusp of the true value (a clinically insignificant difference). The orthodontists consistently overestimated the amount of maxillary arch crowding or spacing. The results suggest that high reliability and reasonable accuracy can be expected for assessing study casts when using standardized definitions for the diagnostic subcategories.  相似文献
