The current study measured ion release among large-head metal-on-metal hip bearings. Twenty-six patients with a modular, Profemur? TL with A-Class? big femoral head total hip replacement were matched (gender, femoral size, BMI) with a group of 26 patients with the Conserve? Plus implant hip resurfacing. Compared with HR patients, THR patients had higher median serum cobalt ion levels at 6 months (3.26 vs 1.12 μg/L, P = .002) 1 year (4.51 vs 1.02, P = .002), and 2 years (3.77 vs 1.22, P<.001) following surgery. No differences in chromium ions were observed. Further research is required to determine the clinical significance of elevated serum cobalt ions.  相似文献

A safe zone for acetabular component positioning in hip resurfacing (RAIL: Relative Acetabular Inclination Limit) was calculated based on implant size and acetabular inclination angle (AIA). For AIA below the RAIL, there were no adverse wear failures or dislocations, and only 1% of cases with ion levels above 10 μg/L. Other than high inclination angle and small bearing size, female gender was the only other factor that correlated with high ion levels in the multivariate analysis. Seven hundred sixty-one hip resurfacing cases are included in this study. The UCLA activity score, femoral shaft angle, body mass index, weight, American Society of Anesthesiologists score, combined range of motion, diagnosis, age, gender, implant brand, AIA, bearing size, and duration of implantation were analyzed to determine the potential risk factors for elevated metal ion levels. These findings apply to sub hemispheric metal-on-metal bearings with similar coverage arcs as the Biomet and Corin hip resurfacing brands. Additional problems may occur when these bearings are connected with trunions on stems for total hip arthroplasty.  相似文献

This is a longitudinal study of the daily urinary output and the concentrations in whole blood of cobalt and chromium in patients with metal-on-metal resurfacings over a period of four years. Twelve-hour urine collections and whole blood specimens were collected before and periodically after a Birmingham hip resurfacing in 26 patients. All ion analyses were carried out using a high-resolution inductively-coupled plasma mass spectrometer. Clinical and radiological assessment, hip function scoring and activity level assessment revealed excellent hip function. There was a significant early increase in urinary metal output, reaching a peak at six months for cobalt and one year for chromium post-operatively. There was thereafter a steady decrease in the median urinary output of cobalt over the following three years, although the differences are not statistically significant. The mean whole blood levels of cobalt and chromium also showed a significant increase between the pre-operative and one-year post-operative periods. The blood levels then decreased to a lower level at four years, compared with the one-year levels. This late reduction was statistically significant for chromium but not for cobalt. The effects of systemic metal ion exposure in patients with metal-on-metal resurfacing arthroplasties continue to be a matter of concern. The levels in this study provide a baseline against which the in vivo wear performance of newer bearings can be compared.  相似文献

Baseline metal ion levels are elevated in patients with metal-on-metal (MOM) hip arthroplasty. Interpretation of baseline levels is difficult as measurements are influenced by wear, corrosion, and metal ion release from stored metal in the body. Schmalzried et al. demonstrated that "wear is the function of use, not time." The specific research question we asked was: Does physiological exercise increase the wear of metal-on-metal articulation which can be measured from the plasma metal ion levels? Patients with three different well functioning MOM bearings [two types of resurfacing (BHR 46.8 mm and Cormet 48 mm) and Metasul 28 mm] were included. Blood samples were taken immediately before, immediately after, and 1 h after exercise to determine cobalt and chromium levels. A significant increase (p<0.005) in serum cobalt and chromium of 13% and 11%, respectively, was noticed after the exercise. Rise of cobalt levels in patients with a resurfacing MOM was 8.5 times (BHR group) or 6.5 times (Cormet group) larger than in those with a Metasul MOM (p=0.021 and p=0.047). Neither rise of metal levels nor baseline levels correlated with any other factor (p>0.27). Exercise-related elevations of plasma cobalt level provides information on current in vivo wear production that cannot be inferred from a baseline measurement of cobalt levels. Chromium levels cannot provide reliable information on the in vivo wear of the devices. Diameter was the important feature of the implant in determining exercise-related elevations of plasma cobalt level. Exercise-related elevations of plasma cobalt level is a potential in vivo tool to understand and improve the tribology of metal-metal bearings.  相似文献

Metal-on-metal resurfacing arthroplasty is increasingly being performed in young, active patients. Serum and urine metal ion levels are monitored in these patients to assess the physiologic effects of metal-on-metal wear on them. The aim of our prospective study was to evaluate the serum and urine levels of cobalt (Co) and chromium (Cr) in young (age, ≤50 years), active Chinese patients who had undergone metal-on-metal hybrid resurfacing arthroplasties. Levels were measured preoperatively using atomic absorption spectrometry and then sequentially at 3, 6, 9, 12, and 24 months after surgery. For both serum and urine Co and Cr, there was an initial increase to a peak at 6 months, followed by a gradual decline after 6 months, whereas renal function was normal during the study the 2-year study period. There was no radiographic evidence of component loosening. All implants were functioning well. Further long-term studies are needed to observe clinical outcomes and to determine the physiologic effects of the wearing process.  相似文献

BackgroundRetention of the acetabular component is an option during conversion from hip resurfacing arthroplasty (HRA) to total hip arthroplasty (THA). The purpose of this study was to compare clinical outcomes of conversion of the femoral resurfacing component with retained acetabular components (RAC) and use of dual-mobility femoral heads to outcomes of patients treated with both component (BC) revisions.MethodsA retrospective review was performed of the clinical outcomes of HRA to THA conversions performed at a single institution between 2008 and 2017.ResultsSeventy-one conversions were included (27 RAC and 44 BC). Average age at time of conversion was 54.4 years (range, 30-68 years). Median time to conversion was 5.14 (2.77-7.41) years and median follow-up post-conversion was 1.7 (0.33-4.0) years. There were no significant differences in indications for conversion between the 2 groups with the majority performed for complications related to elevated metal ions and aseptic loosening. Harris Hip Scores improved from pre-conversion to post-conversion in both cohorts (P < .01). There were no significant differences in pre-conversion and post-conversion metal ion levels between the RAC and BC groups. Serum metal ion levels decreased significantly in both cohorts (P < .01). There were 6 additional revisions in the cohort (4 in BC, 2 in RAC) at a median 2.94 (1.99-3.85) years post-conversion. Two RAC patients had intraprosthetic dislocations with extensive polyethylene wear requiring acetabular revision at median 3.92 (3.85-3.98) years post-conversion. Overall, there were a higher number of complications in the BC group related to acetabular fracture, failure of osseointegration, and periprosthetic joint infection; however, this did not reach statistical significance (P = .27).ConclusionSalvage of an appropriately positioned, well-fixed HRA acetabular component is a reasonable option in the setting of conversion to THA using dual-mobility components. This technique avoids the morbidity of acetabular revision and was associated with a decrease in metal ion levels and improvements in short-term functional outcomes comparable to a BC revision.  相似文献

The aim of this study was to investigate the possible benefit of large-head metal-on-metal bearing on a stem for primary hip replacement compared with a 28 mm diameter conventional metal-on-polyethylene bearing in a prospective randomised controlled trial. We investigated cemented stem behaviour between these two different bearings using Einzel-Bild-R?ntgen-Analyse, clinical and patient reported measures (Harris hip score, Western Ontario and McMaster Universities osteoarthritis index, Short Form-36 and satisfaction) and whole blood metal ion levels at two years. A power study indicated that 50?hips were needed in each group to detect subsidence of > 5 mm at two years with a p-value of < 0.05. Significant improvement (p < 0.001) was found in the mean clinical and patient reported outcomes at two years for both groups. Comparison of outcomes between the groups at two years showed no statistically significant difference for mean stem migration, clinical and patient reported outcomes; except overall patient satisfaction which was higher for metal-on-metal group (p = 0.05). Metal ion levels were raised above the Medicines and Healthcare products Regulatory Agency advised safety level (7 μg per litre) in 20% of the metal-on-metal group and in one patient in metal-on-polyethylene group (who had a metal-on-metal implant on the contralateral side). Two patients in the metal-on-metal group were revised, one for pseudotumour and one for peri-prosthetic fracture. Use of large modular heads is associated with a risk of raised whole blood metal ion levels despite using a proven bearing from resurfacing. The head-neck junction or excess stem micromotion are possibly the weak links warranting further research.  相似文献

BackgroundDifferent bearings have been used in total hip arthroplasty (THA), but the long-term performance is still controversial. The purpose of this study was to investigate whether there are differences when comparing THAs with 5 different bearings at a long-term follow-up of more than 10 years.MethodsFrom January 2010 to May 2012, 101 THA patients (134 hips) were divided into metal-on-metal group (MoM, 31 hips), metal-on-polyethylene group (MoP, 23 hips), ceramic-on-metal group (CoM, 21 hips), ceramic-on-ceramic group (CoC, 33 hips), and ceramic-on-polyethylene group (CoP, 26 hips). The mean follow-up period was 10.3 years. The Harris hip score (HSS), Western Ontario and McMaster Universities Osteoarthritis Index scores (WOMAC), range of motion (ROM), blood cell count, and liver-kidney function were measured. Serum and urine metal ion levels were measured using high-resolution inductively coupled plasma-mass spectrometry (ICP-MS) and a blood lymphocytes analysis was counted by flow cytometry.ResultsNo difference was observed in the HSS, WOMAC, ROM, blood cell count, or liver-kidney function among any of the 5 groups. Metal ion levels were significantly elevated in metal-containing bearings. Flow cytometry showed that no differences were found. Revision was performed due to pseudotumor in 3 patients. The implant survival rate was 96.7% and 93.3% for the MoM and CoC groups, which was significantly lower compared with other groups.ConclusionsMetal ion levels were elevated significantly in metal-containing bearings, especially in MoM THA patients. The implant survival rate was significantly lower in CoC and MoM THAs, which was mainly due to pseudotumor formation.Level of EvidenceTherapeutic Level II.  相似文献

Concerns have been renewed regarding the possible long-term effects of elevated circulating levels of cobalt and chromium as a direct result of implantation of large femoral head diameter metal-on-metal bearings. In order to establish whether metal ion levels remain persistently elevated, we compared metal ion levels before and after revision surgery in patients with large head diameter (greater than 38 mm) metal-on-metal total hip arthroplasty or hip resurfacing arthroplasty. At greater than one year post removal of a large-diameter metal-on-metal hip implant for the indication of symptomatic metallosis, metal ion levels were found to fall to almost normal levels.  相似文献

Our hypothesis was that return of function for young patients undergoing resurfacing total hip arthroplasty (THA) with metal-on-metal bearings or contemporary THA with ceramic bearings would be comparable. Results from 337 unilateral hip resurfacing patients were compared with results from 266 unilateral ceramic-on-ceramic THA patients. Early differences in Harris Hip Scores were observed, but all differences faded by 24 months. Hip resurfacing seems to be a viable alternative to THA for well-selected patients. However, the public perception of improved functional capabilities was not demonstrated in this patient population. Resurfacing patients may be more impaired (slightly higher pain scores/lower function scores) than their THA counterparts in the early postoperative period, but these differences disappear by 24 months when both groups report Harris Hip Scores in the excellent range.  相似文献

Background

Long-term trends in serum metal ion concentration after metal-on-metal (MoM) total hip arthroplasty (THA) are unclear. In most studies, serum metal ion levels increase initially and remain constant during follow-up. However, there are no previous studies that evaluate the metal ion level longitudinally at multiple intervals over the long term. Increasing metal ion levels during the long-term follow-up of patients who underwent cementless MoM THA has been seen, so this study investigated those trends.

Methods

Seventy-one patients were followed for a minimum of 10 years (range, 10.0-18.6 years). All underwent unilateral THA with MoM bearings. Serum cobalt (Co) and chromium (Cr) levels were measured preoperatively and annually after surgery. Trends and differences in serum levels based on sex, mean age, body mass index, cup inclination, and implant selection were analyzed.

Results

Within 5 years after the operation, the mean Co and Cr levels peaked postoperatively at 3-4 years (4.14 and 6.89 μg/L, respectively). Between 5 and 10 years postoperatively, the serum levels showed 2 tendencies. One group (62) showed a constant range, but a smaller group (9) showed increasing serum metal ion levels. The only factor which showed a difference between the 2 groups was body mass index (P = .030), which was lower in the increasing serum metal ion level group.

Conclusion

At a medium-term to long-term follow-up period, serum Co and Cr levels are not always constant. This should be considered when seeing patients with MoM THA, and laboratory tests may be needed for long-term follow-up.  相似文献

Little is known about the influence of physical activity (PA) on metal ion concentrations in subjects with metal-on-metal hip arthroplasty. Implant wear is thought to be a function of use and thus of patient activity levels. It is hypothesized that daily habitual PA of patients with hip resurfacing arthroplasty (HRA) is associated with metal ion concentrations. Therefore a study was conducted in patients with a unilateral HRA at 10-years follow-up. Blood metal ion concentrations were determined. An acceleration-based activity monitor was used to measure PA in daily life. The cohort consisted of 12 males (75%) and 4 females (25%) with a median age at surgery of 55.5 ± 9.7 years [43.0–67.9] and a median follow-up of 9.9 ± 1.0 years [9.1–10.9]. The median cobalt and chromium ion concentrations were 25 ± 13 and 38 ± 28 nmol/L. A significant association between sit-stand transfers and high-intensity peaks with cobalt ion concentrations were found. Regarding PA and metal ion concentrations as a proxy of wear in HRA, specific activities like transfers or qualitative aspects of activity behavior like intensity, seem to matter more than the quantity of low-intensity activities like walking or cycling. This suggests that patients may safely engage in such activities to achieve important general health benefits and quality of life.  相似文献

Peri-prosthetic pseudotumor formation can be a severe complication following Metal-on-Metal hip resurfacing arthroplasty (MoMHRA), with limited data on the optimal management of this complication. The aims of this study were (1) to evaluate the prevalence and severity of pseudotumors in a consecutive cohort of 248 MoMHRA (214 patients, mean follow-up 4.6 years, range: 1 – 8.2), and (2) to present a clinical guideline for their treatment based on severity grading with Metal Artefact Reduction Sequence Magnetic Resonance Imaging, metal ion levels and symptoms. Pseudotumor prevalence was 36.3%: 61 mild, 25 moderate and four were graded severe. Five revisions followed, all in symptomatic patients with elevated metal ion levels. Pseudotumor severity grading allowed us to be conservative with revision surgery for mild and moderate MoM disease.  相似文献

Background:

Recent studies have recommended the discontinuation of metal-on-metal (MoM) components in total hip arthroplasty (THA) because of adverse effects reported with large-diameter MoM THA. This is despite favorable long-term results observed with 28 and 32 mm MoM bearings.

Questions/purposes:

The aim of this study was to assess the value of calls for an end to MoM bearings as THA components. Specifically, we wish to address the risks associated with MoM bearings including adverse soft tissue reactions, metal ion release, and carcinogenic risk.

Methods:

The study evaluates the arguments in the literature reporting on MoM (adverse soft tissue reactions, metal ion release, and carcinogenic risk) and the experience of the current authors who re-introduced these bearings in 1995. They are balanced by a benefit–risk review of the literature and the authors’ experience with MoM use.

Results:

Adverse reactions to metallic debris as well as metal ion release are predictable and can be prevented by adequate design (arc of coverage, clearance), metallurgy (forged instead of cast alloy, high-carbide content), and appropriate component orientation. There is no scientific evidence that carcinogenicity is increased in subjects with MoM hip prostheses. MoM articulations appear to be attractive allowing safe hip resurfacing, decreasing the risk of THA revision in active patients, and providing secure THA fixation with cement in cages in severely deformed hips. MoM bearings in women of child-bearing age are controversial, but long-term data on metallic devices in adolescents undergoing spinal surgery seem reassuring.

Discussion:

Adequate selection of MoM articulations ensures their safe use. These articulations are sensitive to orientation. Fifteen years of safe experience with 28- and 32-mm bearings of forged alloy and high-carbide content is the main reason for retaining them in primary and revision THA.  相似文献

Preoperative and postoperative ion concentrations were measured in 29 metal-on-metal, large-diameter head total hip arthroplasty (LDH-THA) patients. Mean chromium, cobalt (Co), and titanium levels from LDH-THA were 1.3, 2.2, and 2.7 μg/L at 12 months. The open femoral head design showed significantly higher Co concentrations than the closed design (3.0 vs 1.8 μg/L, P = .037). Compared with previously published ion levels from a hip resurfacing system presenting the same bearing characteristics, Co levels were significantly higher in LDH-THA (2.2 vs 0.7 μg/L, P < .001). This study has demonstrated that the addition of a sleeve with modular junctions and an open femoral head design of LDH-THA causes more Co release than bearing surface wear (157% and 67%, respectively). Even if no pathologic metal ion threshold level has been determined, efforts should be made to minimize its release. We recommend modification or abandonment of the modular junction and femoral head open design for this specific LDH-THA system.  相似文献

The recent resurgence in the use of metal-on-metal bearings has led to fresh concerns over metal wear and elevated systemic levels of metal ions. In order to establish if bearing diameter influences the release of metal ions, we compared the whole blood levels of cobalt and chromium (at one year) and the urinary cobalt and chromium output (at one to three and four to six years) following either a 50 mm or 54 mm Birmingham hip resurfacing or a 28 mm Metasul total hip replacement. The whole blood concentrations and daily output of cobalt and chromium in these time periods for both bearings were in the same range and without significant difference.  相似文献

Metal-on-metal (MOM) bearings for hip arthroplasty are increasing in popularity. Concern remains, however, regarding the potential toxicological effects of the metal ions which these bearings release. The serum levels of cobalt and chromium in 22 patients who had undergone MOM resurfacing arthroplasty were compared with a matched group of 22 patients who had undergone 28 mm MOM total hip arthroplasty (THA). At a median of 16 months (7 to 56) after resurfacing arthroplasty, we found the median serum levels of cobalt and chromium to be 38 nmol/l (14 to 44) and 53 nmol/l (23 to 165) respectively. These were significantly greater than the levels after 28 mm MOM THA which were 22 nmol/l (15 to 87, p = 0.021) and 19 nmol/l (2 to 58, p < 0.001) respectively. Since the upper limit for normal patients without implants is typically 5 nmol/l, both groups had significantly raised levels of metal ions. MOM bearings of large diameter, however, result in a greater systemic exposure of cobalt and chromium ions than bearings of small diameter. This may be of relevance for potential long-term side-effects. It is not known to what extent this difference is due to corrosion of the surfaces of the component or of the wear particles produced.  相似文献

Recent studies suggest that the tapered interface between stem and femoral head may be a substantial source of cobalt and chromium ion release after metal-on-metal (MOM) total hip arthroplasty (THA). This study compared patient ion levels after MOM hip resurfacing (HR) and MOM THA performed with identical acetabular components. 110 HRs were compared with 22 THAs. All had well-oriented components, unilateral implants, and serum ion studies beyond one year post-operatively. The HR group’s median cobalt value was 1.11 μg/L vs. 2.86 μg/L for the THA patients. The HR group’s median chromium value was 1.49 μg/L vs. 2.94 μg/L for THA. Significantly higher THA ion levels suggest a source of ions other than the MOM bearing itself.  相似文献
