目的:探讨Minidose技术在双源CT(DSCT)冠状动脉成像及左心室功能分析中的应用价值。方法:45例临床怀疑为冠状动脉心脏病的患者行DSCT冠状动脉成像和超声心动图左心室功能检查。DSCT采用序列扫描模式,总扫描时间窗35%~95%R—R间期,其中40%~70%R-R间期时为全电流输出,数据用于冠状动脉重建,其余间期使用全电流的20%输出,数据用于左心室功能分析。依据美国心脏协会冠状动脉分法,将冠脉图像分为16段,并采用4级法对每段图像质量进行评估。将DSCT检查时测量的左心室射血分数值(LVEF),并与超声心动图所测得的LVEF值为对照进行统计学分析。结果:40例患者顺利完成DscT冠状动脉成像及超声心动图左心室功能分析。共评价冠状动脉525段,其中可用于诊断的节段占97.53%(512/525),平均辐射剂量为(3.75±0.89)mSv。DSCT与超声心动图所测得的LVEF值差异无统计学意义(t=-1.61,P=0.12〉0.05),且两者相关性好(r=0.68)。结论:Minidose技术可以用于DSCT冠状动脉成像兼顾左心室功能分析的“一站式”扫描,冠状动脉图像质量较好,左心室功能分析结果可靠,平均辐射剂量低。  相似文献

目的:选择脑双源CT静脉造影(CTV)的最佳扫描时相及重建层厚,以获得良好的脑静脉图像。方法:①11例志愿者行同层动态增强扫描,以探讨团注追踪触发技术的理论阈值。以理论阈值为中心,将45例志愿者随机分为3组探讨团注追踪触发技术的应用阈值。②将扫描所获得原始数据以0.75mm和1.50mm层厚重建,比较2组层厚重建的三维血管图像质量,进行统计分析。结果:触发阈值100HU组,靶血管全程显示良好;采用0.75mm层厚重建的三维血管图像质量优于1.50mm层厚(P<0.05)。结论:采用100HU阈值进行静脉触发扫描和0.75mm层厚重建血管可以获得良好的脑CTV图像。  相似文献

双源CT前瞻性心电触发序列扫描在房颤患者中的应用研究   总被引:1,自引:0,他引:1  
目的应用双源CT前瞻性心电触发序列扫描对心房纤颤(atrial fibrillation,AF)患者行冠状动脉成像,并与回顾性心电门控螺旋扫描比较,评估二者的图像质量及放射剂量。方法对40例临床可疑冠心病的AF患者行二代128层双源CT增强扫描。随机分为两组:组1共21例(序列组),使用前瞻性心电触发序列扫描;组2共19例(螺旋组),使用回顾性心电门控螺旋扫描。基于AHA-15段冠状动脉分段法及4级评分法(1分-优秀至4分-差),由两位阅片者独立对所有冠状动脉按节段进行图像质量评价,比较两组患者的图像质量及有效放射剂量。结果序列组平均心率(94±25)次/min,心率波动(69±25)次/min;螺旋组平均心率(86±22)次/min,心率波动(65±22)次/min,组间平均心率(t=1.019,P=0.315)及心率波动(t=0.598,P=0.553)差异无统计学意义。序列组与螺旋组的图像质量优良,评分差异无统计学意义(1.29±0.55 vs 1.25±0.55;Z=-1.290,P=0.197);序列组的有效放射剂量较螺旋组显著降低(4.90±1.62 mSv vs 10.04±3.57 mSv;t=-5.988,P=0.000)。结论 AF患者双源CT冠状动脉成像中,前瞻性心电触发序列扫描与回顾性心电门控螺旋扫描相比,可以提供同样高质量的图像满足诊断需要,同时有效减低放射剂量。  相似文献

目的 探讨双源CT(DSCT)双能量扫描对肝脏病变的诊断价值及限度.方法 将53例B超拟诊肝脏占位患者行上腹部普通平扫(TNE)及三期增强扫描(动脉期、双能量门静脉、延迟期).双能量扫描后自动生成80 kV、140 kV以及120 kV加权图像.数据导入工作站的虚拟平扫(VNE).从图像定性评估(肿瘤可见度、图像噪声、诊断接受度)及定量数值(脏器平均CT值、图像噪声、对比噪声比)比较VNE与TNE的图像质量;判断VNE能否取代TVE、80 kV图像中病变的微小血供、双能量纯(彩)碘图、以及感兴趣区(ROI)分解CT值的诊断作用.最后,对比TNE与VNE的有效辐射剂量.结果 53例中有7例在80 kV中较140 kV及120 kV加权图像更清晰显示肿瘤内血管.VNE肿瘤可见度、诊断接受性显著低于、噪声显著高于TNE(P<0.05).TNE及VNE平均CT值分别为:肝实质(58.5±5.6) HU、(58.2±6.7) HU(P=0.226);肝占位(38.8±8.1) HU、(37.4±8.3) HU(P<0.05).VNE图像噪声及对比噪声比显著高于TNE(P<0.001).超出的肝边缘及伪影对诊断无影响,除4例外,VNE均能取代TVE.纯(彩)碘图直观展示病变血供范围及程度,ROI可分解平扫及碘增强的CT值.TNE与VNE有效剂量:(3.38±0.55) mSv,(3.36±0.90) mSv(P>0.05).结论 双源CT双能量80 kV较常规CT对病变血供更敏感,其后处理功能对病变定性诊断有一定作用.VNE图像质量较TNE差,但可取代之并减少辐射剂量.  相似文献

目的 研究双源CT低剂量前瞻性心电触发序列扫描技术在冠状动脉CTA的应用,并评价其图像质量.方法 将68例行冠状动脉CTA检查的患者分为2组,A组38例行前瞻性心电触发序列扫描,入组标准为心率70次/min(bpm)以下,窦性心律,心率波动范围在10 bpm以内.排除标准为心率不能控制在70 bpm以内及心律不齐患者、屏气不佳者以及冠状动脉存在严重钙化及明显狭窄者.采集期相70%R-R间期.B组30例为常规回顾性心电门控螺旋扫描,人组标准为心率70 bpm以下,窦性心律且规整.排除标准为心律不齐患者、屏气不佳者以及冠状动脉存在严重钙化及明显狭窄者.2组中管电压均随体质量指数(BMI)调整,BMI≥24 ks/m2管电压采用120 kV,BMI<24 ks/m2管电压采用100 kV.对2组扫描的冠状动脉分别做图像处理,应用秩和检验比较2组患者冠状动脉段图像质量评分,应用两独立样本t检验比较2组患者辐射剂量.结果 A组评价476段冠状动脉,B组评价372段冠状动脉.A组冠状动脉段图像质量评分为(3.48±0.59)分,B组均值为(3.53±0.58)分,2组之间比较差异无统计学意义(Z=-1.432,P=0.187).A组平均有效剂量为(2.51±0.54)mSv;B组平均有效剂量为(14.55±3.54)mSv,2组之间平均有效剂量比较差异有统计学意义(t=18.484,P=0.000).结论 在严格掌握心率及心律的基础上,可成功实现前瞻性触发序列扫描,该技术能显著降低辐射剂量而保证诊断所需图像质量.  相似文献
