肱骨干下1/3骨折多由传导暴力或旋转暴力引起,类型多样,骨折线常累及干骺端,易形成低位肱骨干骨折,且伴发桡神经损伤概率较高.肱骨远端特殊的解剖结构及复杂的应力使得行内固定比较困难.临床手术方法较多,主要有钢板、髓内钉固定,包括钢板内固定的前方入路、后方入路、前外侧入路及联合入路等,髓内钉固定的顺行和逆行方式;外固定支架适合于开放性、广泛骨缺损、粉碎性肱骨干骨折.微创锁定加压钢板固定肱骨干更加可靠,获得了良好的临床疗效.该文就相关解剖基础、生物力学和手术方法的争议等,作一综述.  相似文献

Functional treatment of closed humeral shaft fractures   总被引:1,自引:0,他引:1  
We treated 93 consecutive patients, average age 53 (16–90) years, with closed humeral shaft fractures applying a functional brace immediately after injury. Seventy-two (77%) fractures healed without problems. There were significantly more consolidation problems in fractures in the proximal third (46% consolidated) compared to those at the middle (81% consolidated) and distal third (86% consolidated) of the shaft. Logistic regression analysis revealed the only predictive factor in respect to successful brace treatment was fracture location. No significant difference was found in respect to healing between different AO-type fractures.
Résumé Nous avons traité 93 malades consécutifs, dâge moyen 53 ans (16–90), avec une fracture diaphysaire humérale fermée en utilisant une attelle fonctionnelle immédiatement après le traumatisme. Soixante-douze (77%) fractures ont guéri sans problème. Il y avait plus de difficultés de consolidation dans les fractures du tiers proximal (46% ont consolidé) comparé à celles du tiers moyen (81% ont consolidé) et du tiers distal (86% ont consolidé) de la diaphyse. Lanalyse statistique a révélé que le seul facteur prédictif de la réussite du traitement est la localisation de la fracture. Aucune différence significative na été trouvée selon les types de la classification AO.

In this study, we present a novel method for performing dual plating of extra-articular fractures of the distal third of the humerus. Since 2006, we have treated 15 such fractures with dual plates from a single posterior midline incision. In the first part of the study, we provide the surgical protocol we have used in addressing these fractures. In the second part, the charts of these patients were reviewed retrospectively to examine their clinical and radiographic outcomes. Using this technique, we have achieved an excellent union rate without significant complications while allowing early and aggressive range of motion.  相似文献

[目的]介绍一种改良悬垂石膏治疗成人肱骨干下段骨折.[方法]从2000年3月~2007年8月,对50例肱骨干下段骨折进行改良悬垂石膏固定,允许肩、肘、腕及手指关节早期功能练习.[结果]骨折临床愈合时间平均7.5周(5~9周),平均随访17个月(6~40个月),肘关节功能恢复率为100%,未出现任何并发症及不良后果.[结论]改良悬垂石膏是治疗肱骨干下段骨折较理想的方法.  相似文献

目的 探讨应用微创经皮钢板内固定技术(MIPPO)治疗肱骨中下段C型骨折的临床疗效.方法 对28例肱骨中下段C型骨折患者通过闭合牵引复位,经小切口插入锁定钢板(LCP),钢板置于肱骨前侧,一期内固定.术后分别以UCLA和Mayo评估肩、肘关节功能.结果 患者均获得随访,时间16~22个月.骨折均愈合,愈合时间3~5个月.1例出现伤口延迟愈合;1例出现桡神经损害症状;1例因钢板过高产生肩关节撞击症状.参照UCLA肩关节评分:优 12例,良 11例,差 5例;Mayo肘关节评分:优 6例,良 14例,可 6例,差 2例.结论 MIPPO技术治疗肱骨中下段C型骨折,临床效果良好.  相似文献

Objective To evaluate effects of minimal invasive plating for treatment of mid-distal humeral shaft fractures.Methods From May 2004 to December 2007, 20 patients with isolated unilat-eral mid-distal third humeral shaft fractures were surgically treated with close reduction and internal fixation using a 4.5 mm dynamic compression plate which was anteriorly inserted through 2 small incisions on the anterior aspects of proximal and distal parts of the arm, away from the fracture site.The postoperative function of the radial nerve and the musculocutaneous nerve, the postoperative alignment of the main fragments on the anteroposterior radiographs, the bone healing time and muscle strength of biceps muscles were measured and recorded, Results Four patients complained of numbness at the area innervated by the lateral ante-braehial cutaneous nerve in the affected forearm after the surgery.No signs of iatrogenic radial nerve palsies occurred after the surgery.A completely normal alignment was achieved in 7 patients, but varus of 11° was found in 2 cases and varus of 2°, 3°, 4°, 5°, 6°, 7° and 10° in one each.Valgus deformity was noticed in 4 cases, and valgus of 3°, 4°, 6° and 7° in one each.The mean follow-up of 10.4 months for 19 patients re-vealed bony union of all the fractures.The average bone healing time was 13.4 weeks.At the latest follow-up, the biceps muscle strength of all the patients was 5 degrees.Hardwires were removed in 5 patients without any complications.Conclusions Minimally invasive anterior plating is a safe alternative osteosynthesis for mid-distal third humeral shaft fractures.However, this technique may interfere with the function of the lateral antebrachial cutaneous nerve.  相似文献

前置钢板微创固定治疗肱骨干中下段骨折的可行性研究   总被引:1,自引:0,他引:1  
目的 评价采用微创前置钢板固定技术治疗肱骨干中下段骨折对桡神经、肌皮神经和肱二头肌功能的影响.方法 2004年5月至2007年12月,采用闭合复位、经上臂前侧远离骨折部位的小切口微创钢板固定技术(MIPO)置入4.5 mm窄动力加压钢板治疗20例肱骨十中、下段闭合性骨折患者.观察术后桡神经功能、肌皮神经功能、前后何肱骨干力线、骨折愈合时间和最后一次随访时肱二头肌肌力.结果 4例患者术后出现前臂外侧皮神经支配区麻小,无医源性桡神经麻痹患者.肱骨干前后位X线片示骨折远、近端0°成角7例,2例内翻成角达11°,内翻成角2°、3°、4°、5°、6°、7°和10°各1例.外翻成角3°,4°、6°和7°各1例.19例患者得到平均10.4个月(8~32个月)随访,骨折均获得愈合,平均骨折愈合时间13.4周(4~32用).伞部患者肱三头肌肌力均为5级.结论 采用前置钢板微创固定技术治疗肱骨巾、下骨折不会损伤桡神经和肱二头肌,但可能会干扰前臂外侧皮神经的功能.  相似文献

OBJECTIVE: To evaluate the clinical, radiographic, and functional outcomes of middle and distal third humeral shaft fractures treated with the minimally invasive percutaneous osteosynthesis (MIPO). DESIGN: Prospective, single-center, nonconsecutive clinical series study. SETTING:: Skeletal trauma center of a university teaching hospital. PATIENTS: Thirteen patients who were seen from May 2004 to October 2005 with an average age of 38.1 years (range, 25 to 60 years) form the basis of this study. Patients were obtained from a surgical database of 1 surgeon. INTERVENTION: The middle and the distal third humeral shaft fractures were reduced by closed means and fixed with long narrow 4.5-mm dynamic compression plates introduced through 2 small incisions away from the fracture sites and placed on the anterior aspect of the humerus. MAIN OUTCOME MEASUREMENTS: Time to fracture healing and functional assessments were assessed at an average follow-up of 12.5 months (range, 7 to 19 months) for the affected shoulders and elbows using the UCLA and Mayo elbow performance scoring systems, respectively. RESULTS: All fractures united with a mean healing time of 16.2 weeks (range, 12 to 32 years). There were no nonunions, radial nerve palsies, or implant failures. The UCLA scoring system showed excellent results in 7 cases (53.8%) and good results in 6 cases (46.2%). Thirteen patients had excellent results of their elbow function when assessed with the Mayo elbow performance scoring system. CONCLUSION: Closed reduction and internal fixation of middle or distal third humeral shaft fractures using MIPO is a safe and effective surgical treatment method and an alternative option to open techniques.  相似文献
