目的:探讨平阳霉素碘油乳剂(PLE)经肝动脉灌注对兔正常肝脏组织的影响。方法:14只4~5个月龄日本大耳白兔,体重(2.5±0.2)kg,按注入生理盐水或PLE的量分为假手术组、实验A组(低剂量组)和实验B组(高剂量组)。各组兔均开腹穿刺肝动脉,按分组剂量注入PLE。术后1,2,4,6周取病理切片,HE染色,光镜下观察肝脏组织学改变;免疫组织化学(免疫组化)染色标记血小板衍化生长因子B链(PDGF-B),并行图象分析。结果:A组HE染色肝细胞呈一过性水样变,变性在2周时最重,至6周已明显减轻。B组HE染色2周时肝细胞明显水样变,4周时可见汇管区纤维组织增生,6周时部分肝组织出现明显假小叶结构。免疫组化染色显示,PDGF-B在肝细胞胞膜及纤维间隔中有明显表达。结论:经肝动脉灌注PLE可导致正常肝脏组织产生不同程度的肝纤维化;PDGF-B参与了肝纤维化的病理过程。  相似文献

目的:探讨肺泡巨噬细胞Toll样受体2(TLR2)的激活机制及其在肝脏缺血再灌注(HIR)中肺损伤的意义。方法:用野生型小鼠C3h/Heouj和TLR4缺失小鼠C3h/Hej建立HIR动物模型。于再灌注1,6,12h后经支气管肺泡灌洗液获取肺泡巨噬细胞,采用荧光定量PCR方法检测TLR2/4mRNA的表达。同时检测支气管肺泡灌洗液中内毒素及肿瘤坏死因子(TNF)的水平,肺组织湿干重比值,肺组织髓过氧化物酶的浓度,并进行肺组织学评分。结果:C3h/Heouj组HIR缺血再灌后各时点肺泡巨噬细胞TLR2/4mRNA表达升高,TLR2mRNA表达持续升高,TLR4mRNA6h达到最高值。同时C3h/Heouj组HIR后支气管肺泡灌洗液中TNF水平明显升高,肺损伤加重,肺组织湿干重比值持续升高,肺组织髓过氧化物酶持续增加(P<0.05)。C3h/Hej组HIR后TLR2mRNA表达仅轻度升高,且支气管肺泡灌洗液中TNF水平低于C3h/Heouj组(P<0.05),肺损伤轻于C3h/Heouj组(P<0.05)。结论:HIR可致肺泡巨噬细胞表面TLR4的激活,可上调TLR2的表达,从而可加重HIR时的肺损伤。  相似文献

摘要:目的 探讨原位肝移植中动脉异位重建的方法及效果。 方法 回顾性分析我院10年来的440例肝移植中36例因受体血管异常而行异位重建的方法及术后处理措施等。 结果 36例中行供肝动脉与受体肾下腹主动脉吻合20例,与肾上腹主动脉吻合10例,与胃左动脉吻合4例,与脾动脉吻合2例。5例围手术期死亡,但吻合口通畅,31例存活3个月至4年无血管相关并发症,仅1例术后2个月因胆道缺血坏死行再次肝移植。 结论 肝移植时受体肝动脉有病变或异常改变时,应将受体肾下或肾上腹主动脉、脾动脉、胃左动脉与供肝动脉进行异位重建,可取得满意效果。  相似文献

目的: 探讨经顺铂(DDP)处理胆囊癌细胞后survivin表达及其与肿瘤细胞耐药之间的关系。 方法:采用MTT比色法测定胆囊癌细胞对4种化疗药物的敏感性。RT-PCR检测survivin mRNA的表达。Western blot检测survivin蛋白表达的变化。结果:GBC-SD细胞对化疗药物的敏感性从高到低依次为DDP>ADM>5-FU>MMC。化学药物处理后的第1天,3组胆囊癌细胞的survivin mRNA表达水平均降低;其中0.5μg/mL DDP+GBC-SD组下降了10%,3μg /mL DDP+GBC-SD组下降36%,6μg /mL DDP+GBC-SD组下降了28%。第3天,0.5μg/mL DDP组和3μg/mL DDP组GBC-SD细胞的survivin mRNA表达与第1天比较,分别上升22%和64%,但6μg/mL DDP组仍持续降低,仅为第1天的66%。0.5μg/mL DDP组和3μg/mL DDP组作用3d后的GBC-SD细胞中survivin蛋白含量分别升高了15%和12%,而6μg/mL DDP组则下降了80%。 结论:低浓度的DDP即能诱导胆囊癌细胞内survivin的表达增加,这可能是胆囊癌细胞对化疗药物产生耐药性的因素之一。  相似文献

摘要:为探讨左侧结肠癌并急性肠梗阻理想的处理原则和方法,回顾分析58例左侧结肠癌并发急性肠梗阻行一期切除吻合术患者的临床资料。本组均成功手术,无手术死亡,术后除7例有切口不同程度液化感染外,无吻合口漏、腹腔感染等并发症,均痊愈出院。提示:对能耐受手术切除的左侧结肠癌并发梗阻,在必要的围手术期处理前提下,一期切除吻合是可行的。避免了横结肠造口、二期手术、癌肿扩散及并发症的发生。  相似文献

笔者采用明胶海绵或不锈钢圈栓塞双侧髂内动脉治疗骨盆骨折合并盆腔血肿7例,其中6例患者术前处于休克前期或休克期,血压低,出血明显,栓塞成功后,出血停止,血压回升,栓塞后2d血压恢复正常基础水平。提示:双侧髂内动脉栓塞对治疗骨盆骨折合并盆腔血肿效果明显,是一种有效的治疗手段。  相似文献

目的:探讨经外膜缓释雷公藤内酯醇(triptolide)对自体移植静脉内膜增生的抑制作用。方法:健康雄性新西兰大白兔24只,建立颈外静脉-颈总动脉移植模型。随机将动物等分为3组。空白组移植血管不给任何处理, F-127多聚凝胶对照组在移植血管外膜喷洒20 %F-127多聚凝胶0.5 mL,实验组在移植血管外膜喷洒携带雷公藤内酯醇300μg的F-127多聚凝胶0.5 mL。术后2周取标本。用组织形态学方法检测血管内膜增生程度,免疫组化检测标本中bcl-2和Fas的表达,TUNEL法检测标本中血管平滑肌细胞(VSMC)凋亡的水平。结果:静脉移植2周后,与空白组和F-127对照组比较,实验组血管内膜增生明显受抑制(P<0.05),bcl-2的表达[(18.2±8.4) %]显著减少,而Fas的表达[(21.4±8.9) %]显著增加,凋亡细胞[(28.4±7.6) %]也显著增加(P<0.05)。结论:经外膜缓释雷公藤内酯醇可有效抑制移植静脉内膜增生,这一作用可能系通过促进VSMC凋亡而实现的。  相似文献

回顾性分析股动脉假性动脉瘤18例的临床资料。1例因介入穿刺引起的股动脉假性动脉瘤行局部压迫治疗,15例行假性动脉瘤切除术,2例行股动脉结扎术。结果示1例股动脉结扎术患者术后出现肢体坏死,行膝上截肢后康复出院,另17例痊愈出院。提示对股动脉假性动脉瘤行动脉瘤切除、股动脉端端吻合可作为首选的手术方式。  相似文献

目的:探索一种治疗彻底、创伤少、瘢癍痕小、费用低的乳腺纤维瘤治疗方法。方法:回顾近3年来465例采用手法张力美容切口治疗乳腺纤维瘤患者的临床资料。结果:465例手术均最大限度争取行乳晕或腋窝皱褶或乳腺下方皱褶切口。切口均甲级愈合,无明显瘢痕,双乳对称,外形功能无影响,站立时切口不明显。结论:手法张力美容切口治疗乳腺纤维瘤是一种适合大部分乳腺纤维瘤患者的手术方法,具有治疗彻底、创伤少、瘢痕小、费用低。  相似文献

目的:分析导致再次胆道手术的原因,以期减少胆道再手术率。方法:总结1990—1999年间收治的再次胆道手术患者828例的临床资料,对胆道疾病再次手术的原因进行归类分析。结果:再手术的主要原因是结石复发或残留,占65.10%;结石合并Oddi括约肌狭窄占33.82%;单纯Oddi括约肌狭窄占9.54%;胆管损伤性狭窄和胆肠吻合口狭窄占10.39%;胆道系统肿瘤占6.52%。结论:胆道再手术的主要原因仍以结石复发或残留为主,其次为Oddi括约肌狭窄;损伤性胆管狭窄等与手术有关的因素不容忽视。减少胆道再次手术的关键在于初次手术的彻底性和手术方法的合理性。  相似文献

目的:探讨CGF(浓缩生长因子)在多种类型创面修复中的临床应用价值。方法:30例创面患者,随机分为CGF组与对照组,两组各包括6例慢性溃疡,5例皮肤软组织坏死,4例浅Ⅱ度伴部分深Ⅱ度烧伤,对照组采用清创去除坏死组织、抗生素抗感染、活血化瘀、换药等常规处理,CGF组在上述常规治疗的基础上同时行液态CGF注射和(或)湿敷及凝胶态CGF膜覆盖和(或)填塞治疗。运用SPSS 19.0软件进行统计学分析,对比两组创面愈合时间及瘢痕外形满意度。结果:CGF组慢性溃疡(14.83±2.93)d痊愈,对照组(32.00±3.58)d痊愈;CGF组皮肤软组织坏死(7.40±1.34)d痊愈,对照组(16.80±2.39)d痊愈;CGF组浅Ⅱ度伴部分深Ⅱ度烧伤9d痊愈,对照组(18.75±2.87)d痊愈。所有CGF组创面愈合时间均短于对照组,差异具有统计学意义(P<0.05)。CGF组术后外形总体满意率为100.00%(15/15),对照组为53.33%(8/15)。结论:不同性状CGF联合应用能有效促进创面愈合,加快创面愈合速度,提高创面愈合质量,控制感染,减轻瘢痕形成,且制备简单,操作便捷,在临床创面修复中具有很高的应用价值及前景。  相似文献

应用表皮生长因子治疗深Ⅱ度烧伤创面的远期临床疗效观察   总被引:19,自引:0,他引:19  
目的 观察重组人表皮生长因子 (rhEGF)用于深Ⅱ度烧伤创面治疗的远期疗效及安全性。 方法 对 37例烧伤患者进行随机、双盲、同体对照实验 ,每例患者选择一块深Ⅱ度烧伤创面 ,并将其分为面积相近的两部分 ,于伤后第 1天开始分别用单纯等渗盐水 (对照组 )和含rhEGF的等渗盐水 (治疗组 )进行换药治疗。创面愈合后 1、4年时 ,对各患者进行院外随访 ,采用改良温哥华瘢痕测量法 ,评价上述受试创面愈合后的瘢痕指数 (SI)。 结果  1年后随访时 ,治疗组SI为 7.19± 1.6 7,明显小于对照组 8.92± 1.78(P <0 .0 1) ;4年后随访时 ,治疗组SI为 6 .12± 1.5 4 ,明显小于对照组 8.0 9± 1.81(P <0 .0 1)。所有受试创面均无肿瘤形成、癌变等并发症发生。 结论 外用rhEGF治疗深Ⅱ度烧伤创面 ,能明显减少后期瘢痕的形成 ,远期疗效和安全性较好。  相似文献

OBJECTIVE: We have spent 7 years to investigate the method of applying porcine acellular dermal matrix (ADM) on deep partial thickness burn wound until the wound heals without dressing change. Known as "Feng's pig skin method" by our hospital, the method appears to encourage rapid re-epithilization with minimum scarring. METHOD: The deep partial thickness burn wound was rinsed cleanly under anesthesia when the patient admitted. ADM was applied on the wound after the detached epidermis was thoroughly removed, wrapped and fixed by sterile gauze and bandages. The dressing was removed within two weeks and the wound completely healed. The outcome of the treatment was analyzed by using the modified Vancouver Burn Scar Assessment Scale. RESULT: All the wounds healed with one dressing within 2 weeks, and the time of wound re-epithelialization shortened to 7-12 days. Scar hyperplasia did not occur, or it was greatly ameliorated compared with traditional treatment after a followed-up period of 3 months to 2 years. The Scar Index was significant lower than that of the traditional exposure method. CONCLUSION: Using ADM to cover deep second degree burn can preserve maximally residual dermal tissue and epithelium, help accelerate the regeneration of epithelial and stem cells, thus shorten the healing time, remodel the skin structure, and consequently has the effect of controlling hypertrophic scar at inception.  相似文献

Revital , a product containing 19 amino acids, was applied to experimental deep second degree burns in guinea-pigs for 24 days, in order to assess the effect of this form of hyperalimentation on the healing process. Silver sulphadiazine cream served as the contralateral control standard. Epithelialization was faster in the silver sulphadiazine treated burn wounds, while contraction of both tested wounds proceeded at a similar rate. Revital significantly enhanced the formation of granulation and scar tissue in this burn wound model. These observations indicate that topical wound hyperalimentation promotes granulation tissue formation of experimental deep second degree burns in guinea-pigs.  相似文献

目的探讨重组牛碱性成纤维细胞生长因子(recombinant bovine basic fibroblast growth factor,rbFGF)、封闭负压引流(vacuum sealing drainage,VSD)结合局部氧疗治疗深Ⅱ度烧伤创面的愈合效果及时长。方法自2018年1月至2019年4月,黄石市第五医院烧伤外科对就诊的92例深Ⅱ度烧伤患者采用随机双盲法分为对照组(46例)和观察组(46例)。对照组采用VSD结合局部氧疗进行治疗;观察组在对照组基础上增用rbFGF。并对两组临床疗效、恢复情况、氧环境、愈合后瘢痕情况以及并发症情况进行比较。结果观察组治疗总有效率高于对照组(P<0.05)。观察组创面愈合时间、换药次数、住院时间、愈合后瘢痕量表各维度评分均低于对照组。观察组治疗3周后创面愈合率、肉芽组织生长面积均高于对照组(P<0.05)。观察组乳酸脱氢酶(lactate dehydrogenase,LDH)水平低于对照组;琥珀酸脱氢酶(succinate dehydrogenase,SDH)水平高于对照组(P<0.05)。两组并发症总发生率比较,其差异无统计学意义(P>0.05)。结论采用rbFGF、VSD结合局部氧疗治疗深Ⅱ度烧伤可以改善创面组织的缺氧状态,促进创面愈合,缩短住院时间,并有助于减少愈合后瘢痕形成,且安全性较高,值得临床推广。  相似文献

We developed a reproducible model of deep dermal partial thickness burn injury in juvenile Large White pigs. The contact burn is created using water at 92 degrees C for 15s in a bottle with the bottom replaced with plastic wrap. The depth of injury was determined by a histopathologist who examined tissue sections 2 and 6 days after injury in a blinded manner. Upon creation, the circular wound area developed white eschar and a hyperaemic zone around the wound border. Animals were kept for 6 weeks or 99 days to examine the wound healing process. The wounds took between 3 and 5 weeks for complete re-epithelialisation. Most wounds developed contracted, purple, hypertrophic scars. On measurement, the thickness of the burned skin was approximately 1.8 times that of the control skin at week 6 and approximately 2.2 times thicker than control skin at 99 days after injury. We have developed various methods to assess healing wounds, including digital photographic analysis, depth of organising granulation tissue, immunohistochemistry, electron microscopy and tensiometry. Immunohistochemistry and electron microscopy showed that our porcine hypertrophic scar appears similar to human hypertrophic scarring. The development of this model allows us to test and compare different treatments on burn wounds.  相似文献

Early gestation fetal wounds heal without scar formation. Understanding the mechanism of this scarless healing may lead to new therapeutic strategies for improving adult wound healing. The aims of this study were to develop a human fetal wound model in which fetal healing can be studied and to compare this model with a human adult and scar tissue model. A burn wound (10 × 2 mm) was made in human ex vivo fetal, adult, and scar tissue under controlled and standardized conditions. Subsequently, the skin samples were cultured for 7, 14, and 21 days. Cells in the skin samples maintained their viability during the 21‐day culture period. Already after 7 days, a significantly higher median percentage of wound closure was achieved in the fetal skin model vs. the adult and scar tissue model (74% vs. 28 and 29%, respectively, p<0.05). After 21 days of culture, only fetal wounds were completely reepithelialized. Fibroblasts migrated into the wounded dermis of all three wound models during culture, but more fibroblasts were present earlier in the wound area of the fetal skin model. The fast reepithelialization and prompt presence of many fibroblasts in the fetal model suggest that rapid healing might play a role in scarless healing.  相似文献

目的:研究硝普钠、磺胺嘧啶银及二者合用对深Ⅱ度烧伤皮肤创面愈合的影响。方法:100只WiStar大鼠背部深Ⅱ度烧伤创面,随机分成0.9%氯化钠注射液组、1%磺胺嘧啶银霜组、lmmol/L硝普钠组和1%磺胺嘧啶银霜+lmmol/L硝普钠组。每组25只大鼠。动态观察烧伤后不同时间点创面细胞增殖周期、羟脯氨酸含量及创面组织愈合情况,计算创面愈合率。结果:伤后随时间推移磺胺嘧啶银霜+硝普钠组创面愈合率高于其他三组;伤后第10天,磺胺嘧啶银霜+硝普钠组创面羟脯氨酸含量、细胞S期百分比达到峰值,明显高于0.9%氯化钠注射液(P〈O.01),磺胺嘧啶银霜组、硝普钠组之间比较无显著差异(P〉0.05)。结论:磺胺嘧啶银霜和硝普钠合用可有效促进大鼠深Ⅱ度烧伤创面愈合。  相似文献

We have been cultivating human epidermal cells for therapeutic purpose according to the original methods developed by Rheinwald and Green. Cultured epithelium (CE) was applied to patients with severe skin defects, burn wounds, chronic skin ulcers and cutaneous disorders like hypomelanosis. Autologous CE allows to restore massive skin surface in a short period compared with other conventional treatments. For grafts take, it is important to manage wound beds properly prior to CE grafting. The CDS was applied to prepare wound bed acceptable for CE grafting. The CDS was designed to secrete various types of cytokines, i.e., VEGF and KGF to stimulate wound healing. The successful management of deep wounds like chronic skin ulcer or burn ulcer requires granulation tissue formation and epithelialization to wound closure. This study aimed to evaluate the application of CDS in conjunction with CE for patients with chronic skin ulcer and burn ulcer. In some cases the wounds were cured by using CDS, and followed by CE grafting. All clinical trials achieved excellent or good results, showing no contracture and no hypertrophic scar after wound closure. The CDS was found to be useful to prepare wound beds and to facilitate wound management.  相似文献

The basic and clinical research in wound healing have made great progress in China in the past 50 years. The method of " intermingle skin transplantation" which was first advocated by surgeons of Ruijin Hospital in 1966 greatly reduced the amount of autologous donor skin, thus making the coverage of an extensive burn wound possible. This method is al so known as " Chinese therapy". In 1986,doctors of Jishuitan Hospital reported successful coverage of an extensive burn wound with mieroautografts and allogeneic skin. The basic research of wound healing has been carried out since 1992,a series of studies showed the characteristics of biological behaviours of cells in concern, extracellular matrix and growth factor, the mechanism underlying progressive injury in deep second burn wound, the effect of " skin island" and the local immune tolerance induced by it (which are the key factors of intermingle transplantation).The induction of local immune tolerance has now become the re search hot subject of skin transplantation immunology. Stem cell research in the field of wound healing has been extensively car ried out. The theory of " dermal template defection" has been proposed as one of the mechanisms of scar formation. On the other hand, great progress has been achieved in the treatment of bums on the basis of clinical researches. Doctors of PLA 304 hospital found that excision of eschar on patients with extensive deep burn injury at early shock stage greatly decreased the occurrence of complications and mortality. Doctors of Ruijin Hospital reported that healing of deep second burn wound could be improved by tangential excision of burn eschar within 24 hours after burn injury. Doctors of Xiang ya Hospital reported patients suffering from deep bums of the hands got satisfied functional restoration when treated with tangential excision of eschar while degraded dermal tissue could be retained with transplantation of autoskin grafts.  相似文献
