目的 评价环氧化酶2(COX2)抑制剂塞来昔布(celecoxib)对尿毒症大鼠甲状旁腺(PG)异常增生的影响.方法 通过5/6肾大部分切除结合高磷饮食(P 1.2%,Ca 1.2%)建立尿毒症甲状旁腺功能亢进症(甲旁亢)大鼠模型,存活大鼠按随机数字表法分成尿毒症甲旁亢非用药组(Nx-HP组,n=17)、塞来昔布预防组(Prey组,n=18,建模后1d起给塞来昔布100 mg· kg-1· d-1)和塞来昔布治疗组(Ther组,n=18,建模1个月后给塞来昔布100 mg·kg-1·d-1),并以假手术组作为对照(Sham组,n=14).12周后检测各组大鼠肾功能、血甲状旁腺激素(iPTH)水平、PG大小以及PG中增殖细胞核抗原(PCNA)、COX2表达.结果 Nx-HP组大鼠血清iPTH水平较Sham组显著升高[(100.73±4.35) ng/L比(34.77±0.83) ng/L,P<0.01],塞来昔布干预后iPTH水平显著下降[Prev组(87.36±2.18) ng/L,Ther组(87.47±1.76) ng/L](均P< 0.05).计算显微镜下PG最大面积显示,Nx-HP大鼠PG最大面积显著增大,是Sham组的5.28倍[(2.436±0.372) mm2/kg比(0.461±0.089) mm2/kg,P<0.01];而Prey组[(0.987±0.254)mm2/kg]和Ther组[(1.270±0.305) mm2/kg]PG分别缩小59.47%(P<0.01)和47.87% (P<0.05),两组间差异无统计学意义.PCNA在Sham组PG中仅少量表达,Nx-HP组显著增多,塞来昔布干预后PCNA阳性细胞明显减少,Prey组和Ther组蛋白表达水平分别降低52.91%和34.68%(均P<0.05).COX2在Sham组PG中几乎未见表达,但在尿毒症大鼠中明显增多,Nx-HP组、Prey组和Ther组分别为Sham组的2.47倍、2.34倍和3.04倍(均P<0.05),而3组间差异无统计学意义.实时定量PCR检测PCNA和COX2基因水平显示同样变化趋势.结论 选择性COX2抑制剂塞来昔布可以显著抑制尿毒症大鼠甲旁亢和甲状旁腺增生. 相似文献

慢性肾功能衰竭发生甲状旁腺功能亢进(2HPT)主要与125(OH)2D3缺乏和高磷血症有关,维生素D受体密度和结合力的改变、125(OH)2D3抵抗、钙传感器受体基因突变等因素参与了2HPT的发生。高磷血症和钙化防御是2HPT患者临床上亟待解决的两大难题,高磷和高钙血症是125(OH)2D治疗2HPT的主要并发症,药物剂量过小、含钙的磷结合剂使用不当发生率高,静脉用药时发生率相对较低。paricalcitol只抑制PTH分泌,不影响肠道和骨骼钙磷代谢,可避免高钙和高磷血症,可能是治疗慢性肾衰2HPT最理想的维生素D制剂 相似文献

目的:探讨环氧化酶2(COX-2)mRNA在前列腺癌(PCa)组织中的表达及其意义。方法:采用RT-PCR方法检测32例PCa组织中COX-2 mRNA的表达并测定COX-2/GAPDH的比值,7例正常前列腺组织作为对照。结果:7例正常前列腺组织无COX-2 mRNA表达。COX-2/GAPDH与PCa G leason评分、分期呈正相关(P均<0.05)。结论:COX-2在PCa的发生发展中起重要的正性调节因子的作用。COX-2与PCa的恶性程度和进展有关,可作为判断PCa预后的指标。 相似文献

目的:研究环氧化酶2(COX-2)在不同前列腺癌细胞系中的表达,探讨COX-2在前列腺癌侵袭进展及转移潜能获得机制中的可能作用。方法:应用Western印迹及RT-PCR鉴定LNCaP及其亚细胞系C4-2和AR-CaP亚细胞系IF11、IA8,以及PC-3细胞中COX-2的表达情况,并初步分析其在不同特性前列腺癌细胞系转移侵袭过程中的作用。结果:Western印迹结果显示:COX-2蛋白在PC-3细胞中表达相对较高,在IF11、IA8、LNCaP和C4-2细胞中表达缺失,差异具有统计学意义(P<0.05)。COX-2mRNA表达结果同蛋白一致。结论:不同来源、不同转移潜能的前列腺癌细胞株中COX-2表达存在差异。高表达COX-2可能在PC-3细胞高侵袭转移潜能获得方面起着一定作用,而与其他细胞系转移作用无关。 相似文献

目的 探讨尿毒症患者心脏结构的改变与全段甲状旁腺素之间的关系.方法 将119例尿毒症患者分为血液透析组61例、非血液透析组58例,比较2组室间隔厚度、左心室后壁厚度、射血分数、左心室心肌重量指数及全段甲状旁腺素水平.结果 血液透析组左心室肥厚48例,非血液透析组左心室肥厚39例,2组室间隔厚度、左心室后壁厚度明显增厚,无明显差异.血液透析组左心室收缩功能不全5例,非血液透析组左心室收缩功能不全1例.2组全段甲状旁腺素较正常人群(7~53 pg/ml)均明显升高,血液透析组高于非血液透析组.血液透析组左心室心肌重量指数高于非血液透析组.2组全段甲状旁腺素与室间隔厚度、左心室后壁厚度、左心室心肌重量指数呈正相关.结论 尿毒症患者血全段甲状旁腺素升高是影响左心室结构与功能的重要因素之一. 相似文献

目的 了解血透患者甲状旁腺素[PTH(1~84)]和血小板胞内游离钙浓度{pt[Ca~(2 )]i}在血压调节中的作用。方法 用Fura-2荧光测定法和免疫放射法对24例血透患者测定上述两项内容。结果 HD患者的静息pt[Ca~(2 )]i和PTH(1~84)是升高的,高血压者升高更为明显。静息pt[Ca~(2 )]i与PTH(1~84)相关。多因素逐步回归显示PTH(1~84)、pt[Ca~(2 )]i为影响HD患者平均动脉压的主要因素。结论 PTH(1~84)、pt[Ca~(2 )]i可能是主要影响尿毒症患者血压的因素。 相似文献

Objective To explore the effects of high phosphate and low calcium on the secretion function and cell proliferation in normal rat parathyroid tissues. Methods The rat parathyroid tissues resected by surgical operation under dissection microscope were incubated in vitro for 5 days and were treated with high phosphate or low calcium medium on the second day and the third day after incubation. The cell apoptosis at different time points was measured by flow cytometry.The levels of intact parathyroid hormone (iPTH) in the supernatant at different time points were detected by ELISA. The mRNA levels of prepro-PTH and proliferating cell nuclear antigen (PCNA) were measured by real-time PCR, while the protein levels of PCNA after the 24-hour treatment with high phosphate or low calcium medium were measured by Western blotting. Results Flow cytometric analysis showed that the percentage of normal living cells on the first day and the second day after incubation was significantly higher than that on the third day to the fifth day after incubation in the rat parathyroid tissues (all P﹤0.05), and there was no statistical difference among that on the third day to the fifth day after incubation. There was no significant difference in cell survival rates on the first day to the fifth day after incubation (all﹥85%) under normal or high phosphate medium. There was no significant difference in the daily iPTH level on the first day to the fifth day after incubation under normal medium. Compared with normal medium, the iPTH level in the supernatant under high phosphate medium increased (P﹤0.05), and the expressions of preproPTH mRNA and PCNA in parathyroid tissues were significantly up-regulated (both P﹤0.05) after the 24-hour treatment with high phosphate medium. Compared with normal medium, the iPTH level in the supernatant under low calcium medium increased (P﹤0.05), and the expression of preproPTH mRNA in parathyroid tissues was significantly up-regulated after the 24-hour treatment with low calcium medium (P﹤0.05). Conclusions It is feasible to incubate the parathyroid issues from rats in vitro, and the best incubation time should be within 2 days. Low calcium and high phosphate stimulation could increase the synthesis and secretion of PTH, while high phosphate can also promote the cell proliferation, which is an important factor for the regulation of parathyroid function in physiological state. 相似文献

目的 探讨6种不同血液净化方法对尿毒症患者血磷及甲状旁腺激素(parathyroid hormone,PTH)的清除效果.方法 选取在我院维持性血液透析血磷的患者200例并采用数字表法随机分为6组:血液透析联合血液灌流(HD+ HP130)组、HD+ HP230组、血液透析滤过(HDF)组、血液透析(HD)组、床旁血液滤过(CVVH)组和高通量血液透析(HFHD)组.比较6组血液净化单次治疗前后血磷及血PTH清除率.结果 HD组治疗后血PTH值下降,但差异无统计学意义(P>0.05);血磷治疗后下降且差异有统计学意义(P<0.05).HDF组和HFHD组治疗后血PTH值及血磷值均下降,差异有统计学意义(P<0.05).CVVH组治疗后血PTH下降明显,差异有统计学意义(P<0.01),HD+ HP130和HD+ HP230组治疗后血PTH下降,差异有统计学意义(P<0.05),且均显著高于HDF组(P<0.05)和HFHD组,血磷值下降且差异有统计学意义(P<0.05).结论 血液透析联合血液灌流、床旁血液滤过、高通量血液透析及血液透析滤过均能有效清除血磷及PTH,清除效果依次为CVVH组>HD+ HP230组>HD+ HP130组>HDF组>HFHD组.血液透析可以有效清除血磷但不能有效清除PTH. 相似文献

目的比较联机血液透析滤过(HDF)和常规血液透析(HD)对尿毒症患者甲状旁腺素(iPTH)水平的影响。方法选择我院血液净化中心2004年6月至2006年12月期间透析龄超过9个月且iPTH明显升高的尿毒症患者60例,其中男38例、女22例,平均年龄43.5岁,平均透析龄(18.6±9.3)月。将患者随机分为HDF组和HD组,每组30例。两组患者每周均透析三次,HDF组为1次HDF、2次HD,每次透析4h。HDF组使用F60滤过器;HD组使用F6HPS透析器,统一低分子肝素抗凝。检测透析前后患者血液血肌酐(SCr)、血尿素氮(BUN)及iPTH水平并计算其清除百分率。结果SCr与BUN清除率在HD组分别为(70.6%±3.2%)和(74.2%±4.0%),在HDF组分别为(71.8%±2.3%)和(76.2%±3.8%),两组之间差异无统计学意义。HD组血iPTH值透析前后无显著差异,清除率仅为(1.7%±0.9%),而HDF组iPTH的清除率为(32.8%±7.8%),该组透析前后溶质浓度及清除率的差异均有统计学意义。结论两种血液净化治疗方式对小分子物质的清除效果无差异,但HDF对中分子物质(iPTH)的清除效果明显优于HD。定期HDF有利于iPTH的清除、防止iPTH异常导致的钙磷代谢紊乱、降低代谢性骨病等并发症的发生率。 相似文献

目的 观察甲状旁腺激素(PTH)对人肾小管上皮细胞分泌结缔组织生长因子(CTGF)的影响,并探讨丝裂原活化蛋白激酶(MAPK)信号途径在此过程中的作用。 方法 采用实时定量PCR、Western印迹、报告基因等技术,观察PTH诱导人近端肾小管上皮细胞系HK-2细胞CTGF表达的情况。使用信号通路抑制剂PD98059、U0126阻断信号通路以明确PTH发挥作用的信号途径。 结果 正常HK-2细胞有基础水平的CTGF mRNA和蛋白表达,PTH刺激后其表达水平显著增加。10-10 mol/L PTH作用12 h后,荧光素酶活性较对照组明显升高[(1.8884±0.0780)比(0.9891±0.0300) A,P < 0.01]。正常HK-2细胞有少量p-ERK1/2表达,PTH刺激后p-ERK1/2表达明显升高,以10-10 mol/L PTH作用30 min时效应最强;MAPK通路抑制剂PD98059、U0126作用后,CTGF mRNA、蛋白、基因启动子表达均明显下降。 结论 PTH可诱导HK-2细胞CTGF表达,其作用可能是通过MAPK信号通路来实现的。 相似文献

目的 探讨尿毒症患者血清诱导的磷酸钙晶体对人主动脉平滑肌细胞(HASMCs)钙化的影响.方法 尿毒症患者血清在37℃下孵育3d,超速离心法从尿毒症血清中分离磷酸钙晶体和无晶体血清,采用扫描电子显微镜和能量色散X射线光谱仪分析晶体的形态及化学特征.体外培养HASMCs,分为以下4组:对照组、尿毒症血清组、磷酸钙晶体组和无晶体血清组.茜素红染色及甲氧酚酞络合酮法检测HASMCs钙化结节的形成及细胞内钙含量.实时荧光定量PCR法检测骨形态发生蛋白2(BMP-2)、骨桥蛋白(OPN)、核结合因子α1亚基(Cbfα1)、碱性磷酸酶(ALP)、基质γ羧基谷氨酸蛋白(MGP)的mRNA表达.Cbfα1、OPN和BMP-2的蛋白表达用Westcrn印迹和ELISA法检测.结果 尿毒症血清可诱导磷酸钙晶体形成.与对照组相比,尿毒症血清明显促进HASMCs钙化结节形成,增加细胞内钙含量(P<0.05),并上调细胞BMP-2、OPN、Cbfα1的mRNA和蛋白表达(均P< 0.01);而磷酸钙晶体亦促进HASMCs钙化结节的形成,增加细胞内钙含量(P<0.05),上调BMP-2、OPN、Cbfα1的mRNA和蛋白表达(均P<0.01).无晶体血清组的HASMCs胞内钙含量和上述成骨蛋白的表达与对照组差异无统计学意义(均P> 0.05).结论 尿毒症血清可能通过诱导磷酸钙晶体的形成促进血管钙化的发生. 相似文献

Objective To analyze the efficacy and safety of total parathyroidectomy (PTX) with forearm autograft in uremic patients with secondary hyperparathyroidism (SHPT). Methods One hundred and eighteen cases undergoing PTX with forearm autograft in our hospital from 2001-2010 were included in this study. Their preoperative and postoperative serum intact parathyroid hormone (iPTH), biochemistry tests (total calcium,inorganic phosphate and alkaline phosphate) were collected and postoperative symptom relief, complications and recurrence were investigated. Results Of all the 118 cases, 32 underwent endoscopic surgery and 86 open surgery. The surgery was performed successfully in 110 cases (93.2%) and one case died in perioperative period. Thyroid carcinoma was diagnosed during surgery in 2 cases and radical operation was performed at the same time. Temporary injury of recurrent laryngeal nerve was found in nine cases (7.6%). Postoperative hypocalcemia was frequently seen in 108 cases (91.5%) and it was effectively controlled by postoperative calcium administration. After operation, bone pain and itching were alleviated, and weakness, anemia and malnutrition status were improved in all the cases who received successful surgery. The postoperative levels of serum iPTH (P<0.01), calcium (P<0.01), phosphorus (P<0.01) and calcium×phosphorus (P<0.01) were decreased significantly than those in preoperative period. A long-term follow-up of over 3 years was carried out in 21 cases. Six cases recurred, among them, 4 cases relieved after removal of autografted parethroid tissue, and another two cases received the second operation. The longest follow-up period lasted for 9 years in two cases without recurrence. Conclusions PTX with forearm autograft is safe and effective in the treatment for uremic patients with SHPT. No severe complication is found during the long-term follow-up period. 相似文献

目的 观察高通量血液透析与低通量血液透析对维持性血液透析患者残余肾功能的影响.方法 比较治疗后两组患者24 h尿量、残余肾功能、尿素清除指数Kt/V、标准蛋白分解率、白蛋白、β2微球蛋白、甲状旁腺素、C反应蛋白、肿瘤坏死因子-α变化.结果 6个月后两组都存在残余肾功能的减退,但高通量血液透析组的24 h尿量及残余肾小球滤过率显著高于低通量血液透析组.高通量血液透析组标准蛋白分解率、血浆白蛋白显著高于低通量血液透析组.治疗后高通量血液透析组β2微球蛋白、甲状旁腺素显著低于低通量血液透析组.高通量血液透析组C反应蛋白、肿瘤坏死因子-α显著低于低通量血液透析组.结论 高通量血液透析有利于维持性透析患者残余肾功能的维护,其机制可能与清除更多中分子物质,减轻炎症反应相关. 相似文献
