STUDY OBJECTIVES: Periodic leg movements in sleep (PLMS) are frequently accompanied by arousals and autonomic activation, but the pathophysiologic significance of these manifestations is unclear. DESIGN: Changes in heart rate variability (HRV), HRV spectra, and electroencephalogram (EEG) spectra associated with idiopathic PLMS were compared with changes associated with isolated leg movements and respiratory-related leg movements during sleep. Furthermore, correlations between electromyographic activity, HRV changes, and EEG changes were assessed. SETTING: Sleep laboratory. PATIENTS: Whole-night polysomnographic studies of 24 subjects fulfilling the criteria of either periodic leg movements disorder (n = 8), obstructive sleep apnea syndrome (n = 7), or normal polysomnography (n = 9) were used. MEASUREMENTS AND RESULTS: Spectral HRV changes started before all EEG changes and up to 6 seconds before the onset of all types of leg movements. An initial weak autonomic activation was followed by a sympathetic activation, an increase of EEG delta activity, and finally a progression to increased higher-frequency EEG rhythms. After movement onset, HRV indicated a vagal activation, and, the EEG, a decrease in spindle activity. Sympathetic activation, as measured by HRV spectra, was greater for PLMS than for all other movement types. In EEG, gamma synchronization began 1 to 2 seconds earlier for isolated leg movements and respiratory-related leg movements than for PLMS. Significant correlations were found between autonomic activations and electromyographic activity, as well as between autonomic activations and EEG delta activity, but not between higher-frequency EEG rhythms and EMG activity or HRV changes. CONCLUSIONS: These results suggest a primary role of the sympathetic nervous system in the generation of PLMS.  相似文献

Spectral analysis of heart rate variability (HRV) is a widely accepted approach for assessment of cardiac autonomic function and its relationship to numerous disorders and diseases. As a rule, the non-parametric methods for HRV spectral analysis are tested using the integral pulse frequency modulation (IPFM) model. However, published results with simulated HRV signals show differences requiring further development of the existing methods. With the aim of improving estimation accuracy, an entirely IPFM-based method for HRV analysis is investigated. According to this method, the spectra are computed by finding the least squares solution of two matrix equations that are derived using the IPFM model and involve irregular samples of a signal representing the HRV. The method is validated with various synthesised signals (in all tests, the relative errors of the power estimates at the modulating frequencies are within 3%, and the relative power of the spurious terms is less than 0.8% only) and is furthermore applied to the spectral analysis of RR interval series obtained from diabetic children. The results, with simulated and real HRV signals, show that the developed method yields very accurate estimations of the spectral region below half the mean heart rate. Moreover, it allows the detection and assessment of certain genuine modulating components beyond the traditional frequency limit of the HRV spectra.  相似文献

Nine cell culture-adapted, as well as 30 clinical, human rotavirus (HRV) strains from fecal extracts of children with primary HRV infection were typed by rapid solid-phase immune electron microscopy with protein A and absorbed DS-1 (HRV serotype 2), Wa (serotype 1), and VA70 (assumed serotype 3) rabbit immune sera. As a reference typing test for cell culture-adapted strains, the neutralization assay was used, whereas for noncultivatable strains typing was done for comparison, indirectly, based upon the differential neutralization reactivity of convalescent-phase serum samples from patients with primary HRV infection versus the three reference HRV serotypes. Typing results by solid-phase immune electron microscopy for all strains examined were in complete agreement with those obtained by the neutralization assay, both on cell culture-adapted strains with the three reference rabbit antisera and on three reference HRV strains with human convalescent-phase serum samples. Since adaptation to growth in cell cultures of clinical HRV strains from stool specimens is a time-consuming procedure and is often unsuccessful, solid-phase immune electron microscopy is preferred over the neutralization assay, giving results in about 16 h and also allowing typing of HRV strains from stool specimens low in virus particles. In addition, HRV strains reacting differently from the three reference serotypes may be easily selected by solid-phase immune electron microscopy for further characterization, as was the case for one strain in this study.  相似文献

The impact of Thought Field Therapy on heart rate variability   总被引:2,自引:0,他引:2  
Thought Field Therapy (TFT) is a rapid treatment for psychological problems typically taking only minutes. HRV has been shown to be a strong predictor of mortality and is adversely affected by such problems as anxiety, depression, and trauma. Interventions presented in the current literature show modest improvements in HRV. Twenty cases, treated by the author and other therapists with TFT, are presented. The cases include some with diagnosed heart problems and very low HRV, which is ordinarily more resistant to change. The degree of improvements that are registered on HRV as a result of TFT treatment exceeds reports found in the current literature. There is a close correspondence between improved HRV and client report of reduced degree of upset. HRV may prove to be an appropriate objective measure of psychotherapy efficacy given the correspondence between client report and HRV outcome. Further research in TFT and HRV is encouraged by these results.  相似文献

Spectral analysis of heart rate variability (HRV) is an accepted method for assessment of cardiac autonomic function and its relationship to numerous disorders and diseases. Various non-parametric methods for HRV estimation have been developed. The spectrum of counts, the instantaneous heart rate spectrum and the interval spectrum are mostly practised. Although extensive literature on their respective properties is available, there seems to be a need for a more complete comparison, eventually resulting in recommendations for applicability. The methods for HRV spectral analysis use their specific transforms of the primary R-R interval series into input signals for spectral computation. This is, in fact, the reason for obtaining different spectra. A basis for comparison is established, revealing the generic relationships of these HRV input signals. It allows for a more systematic evaluation and for further development of the considered methods. The results with simulated and real HRV data show better performance by the spectrum of counts and by a proposed instantaneous heart rate spectrum, obtained using a cubic spline interpolated input signal. The modulation depth of the primary signal can influence the accuracy of the HRV analysis methods.  相似文献

The aim of the present study was to investigate reactions in trapezius muscle blood flow (MBF), muscle activity, heart rate variability (HRV) and systemic blood pressure (BP) to autonomic tests in subjects with chronic neck-shoulder pain and healthy controls. Changes in muscle activity and blood flow due to stress and unfavourable muscle loads are known underlying factors of work-related muscle pain. Aberration of the autonomic nervous system (ANS) is considered a possible mechanism. In the present study, participants (n?=?23 Pain, n?=?22 Control) performed autonomic tests which included a resting condition, static hand grip test (HGT) at 30% of maximal voluntary contraction, a cold pressor test (CPT) and a deep breathing test (DBT). HRV was analysed in time and frequency domains. MBF and muscle activity were recorded from the upper trapezius muscles using photoplethysmography and electromyography (EMG). The pain group showed reduced low frequency-HRV (LF) and SDNN during rest, as well as a blunted BP response and increased LF-HRV during HGT (?systolic 22?mm Hg; ?LF(nu) 27%) compared with controls (?systolic 27; ?LF(nu) 6%). Locally, the pain group had attenuated trapezius MBF in response to HGT (Pain 122% Control 140%) with elevated trapezius EMG following HGT and during CPT. In conclusion, only HGT showed differences between groups in systemic BP and HRV and alterations in local trapezius MBF and EMG in the pain group. Findings support the hypothesis of ANS involvement at systemic and local levels in chronic neck-shoulder pain.  相似文献

Heart rate variability (HRV) is traditionally derived from RR interval time series of electrocardiography (ECG). Photoplethysmography (PPG) also reflects the cardiac rhythm since the mechanical activity of the heart is coupled to its electrical activity. Thus, theoretically, PPG can be used for determining the interval between successive heartbeats and heart rate variability. However, the PPG wave lags behind the ECG signal by the time required for transmission of pulse wave. In this study, finger-tip PPG and standard lead II ECG were recorded for five minutes from 10 healthy subjects at rest. The results showed a high correlation (median = 0.97) between the ECG-derived RR intervals and PPG-derived peak-to-peak (PP) intervals. PP variability was accurate (0.1 ms) as compared to RR variability. The time domain, frequency domain and Poincaré plot HRV parameters computed using RR interval method and PP interval method showed no significant differences (p < 0.05). The error analysis also showed insignificant differences between the HRV indices obtained by the two methods. Bland-Altman analysis showed high degree of agreement between the two methods for all the parameters of HRV. Thus, HRV can also be reliably estimated from the PPG based PP interval method.  相似文献

BACKGROUND: More than 100 immunologically distinct serotypes of human rhinoviruses (HRV) have been discovered, making detection of surface exposed capsid antigens impractical. However, the non-structural protein 3C protease (3Cpro) is essential for viral replication and is relatively highly conserved among serotypes, making it a potential target for diagnostic testing. The thin film biosensor is an assay platform that can be formatted into a sensitive immunoassay for viral proteins in clinical specimens. The technology utilizes an optically coated silicon surface to convert specific molecular binding events into visual color changes by altering the reflective properties of light through molecular thin films. OBJECTIVE: To develop a rapid test for detection of HRV by developing broadly serotype reactive antibodies to 3Cpro and utilizing them in the thin film biosensor format. STUDY DESIGN: Polyclonal antibodies to 3Cpro were purified and incorporated into the thin film assay. The in vitro sensitivity, specificity and multiserotype cross-reactivity of the 3Cpro assay were tested. Nasal washes from naturally infected individuals were also tested to verify that 3Cpro was detectable in clinical specimens. RESULTS: The 3Cpro assay is a 28-min, non-instrumented room temperature test with a visual limit of detection of 12 pM (picomolar) 3Cpro. In terms of viral titer, as few as 1000 TCID(50) equivalents of HRV2 were detectable. The assay detected 45/52 (87%) of the HRV serotypes tested but showed no cross-reactivity to common respiratory viruses or bacteria. The thin film assay detected 3Cpro in HRV-infected cell culture supernatants coincident with first appearance of cytopathic effect. Data are also presented demonstrating 3Cpro detection from clinical samples collected from HRV-infected individuals. The assay detected 3Cpro in expelled nasal secretions from a symptomatic individual on the first day of illness. In addition, 9/11 (82%) concentrated nasal wash specimens from HRV infected children were positive in the 3Cpro test. CONCLUSION: We have described a novel, sensitive thin film biosensor for rapid detection of HRV 3Cpro. This test may be suitable for the point of care setting, where rapid HRV diagnostic test results could contribute to clinical decisions regarding appropriate antibiotic or antiviral therapy.  相似文献

Real-time acquisition of EMG during functional MRI (fMRI) provides a novel method of controlling motor experiments in the scanner using feedback of EMG. Because of the redundancy in the human muscle system, this is not possible from recordings of joint torque and kinematics alone, because these provide no information about individual muscle activation. This is particularly critical during brain imaging because brain activations are not only related to joint torques and kinematics but are also related to individual muscle activation. However, EMG collected during imaging is corrupted by large artifacts induced by the varying magnetic fields and radio frequency (RF) pulses in the scanner. Methods proposed in literature for artifact removal are complex, computationally expensive, and difficult to implement for real-time noise removal. We describe an acquisition system and algorithm that enables real-time acquisition for the first time. The algorithm removes particular frequencies from the EMG spectrum in which the noise is concentrated. Although this decreases the power content of the EMG, this method provides excellent estimates of EMG with good resolution. Comparisons show that the cleaned EMG obtained with the algorithm is, like actual EMG, very well correlated with joint torque and can thus be used for real-time visual feedback during functional studies.  相似文献

Five serotypes of human rhinovirus (HRV) were examined for sensitivity to trypsin at physiological pH, HRV1A, HRV2, and HRV14 were found to be resistant whereas in serotypes HRV49 and HRV89 degradation of VP2 was observed. However, exposure to low pH followed by neutralization, a treatment which causes irreversible conformational changes in the capsid, led to rapid cleavage by trypsin of VP1 in HRV1A, HRV2, and HRV49 at defined sites followed by degradation of VP2. In the case of HRV2, the cleavage site in VP1 was determined by direct protein sequencing and was shown to occur between Arg260 and Thr261, close to the C-terminus. HRV49 behaves similarly to HRV2 as expected from extensive sequence similarity in this region, whereas VP1 in HRV1A is most probably cleaved at a site closer to the C-terminus than that in HRV2. Although HRV14 contains the same amino acid pair present in HRV2 and HRV49, it was not cleaved under these conditions. HRV89, which lacks a basic residue at the corresponding position, was also insensitive. Examination of the cleavage site on the three-dimensional structural map of native HRV2 reveals that it is most probably buried inside the capsid and thus not accessible. Structural rearrangements of the viral capsid are thus necessary to account for the cleavage observed after low pH treatment.  相似文献

Our immediate goal was to study heart rate variability (HRV) in chicken embryos in the egg. Instantaneous heart rate data were needed for this purpose, and accordingly an ECG recording method in the egg was developed. The aim of this work was to test the hypothesis that autonomic nervous cardiac modulation, as shown from HRV parameters, is present at the end of development and that it reaches a constant value during the last days of incubation. Embryonic chicken heart rate was obtained at the final incubation period (days 19 and 20) from ECG recordings. Tachograms were computed and time- and frequency-domain indices of HRV were determined. No significant differences were found between HRV indices from day 19 and day 20. The power spectra extended in two frequency bands with centre frequency around 0.6-0.7 Hz (low frequency (LF) component), and another around 1.2-1.5 Hz (high frequency (HF) component); the latter was shown to reflect respiratory sinus arrhythmia. A relation between mean RR interval and some HRV parameters (rMSSD, pNN5 and HF power) was shown. HRV results obtained from embryonic chickens, showed the presence of modulation of cardiovascular function by the autonomic nervous system. The results suggested that sympathetic and parasympathetic activities have already reached a constant level at day 19 of incubation. High frequency oscillations (0.78-2.5 Hz) were detected and are considered to reflect respiratory sinus arrhythmia.  相似文献

Intensive care unit (ICU) patients are at risk of in-ICU morbidities and mortality, making specific systems for identifying at-risk patients a necessity for improving clinical care. This study presents a new method for predicting in-hospital mortality using heart rate variability (HRV) collected from the times of a patient’s ICU stay. In this paper, a HRV time series processing based method is proposed for mortality prediction of ICU cardiovascular patients. HRV signals were obtained measuring R-R time intervals. A novel method, named return map, is then developed that reveals useful information from the HRV time series. This study also proposed several features that can be extracted from the return map, including the angle between two vectors, the area of triangles formed by successive points, shortest distance to 45° line and their various combinations. Finally, a thresholding technique is proposed to extract the risk period and to predict mortality. The data used to evaluate the proposed algorithm obtained from 80 cardiovascular ICU patients, from the first 48 h of the first ICU stay of 40 males and 40 females. This study showed that the angle feature has on average a sensitivity of 87.5% (with 12 false alarms), the area feature has on average a sensitivity of 89.58% (with 10 false alarms), the shortest distance feature has on average a sensitivity of 85.42% (with 14 false alarms) and, finally, the combined feature has on average a sensitivity of 92.71% (with seven false alarms). The results showed that the last half an hour before the patient’s death is very informative for diagnosing the patient’s condition and to save his/her life. These results confirm that it is possible to predict mortality based on the features introduced in this paper, relying on the variations of the HRV dynamic characteristics.  相似文献

A method of telemetric measurement of antroduodenal myoelectrical activity (EMG) in freely moving pigs has been developed. Silver bipolar electrodes were sutured on the antrum and duodenum and connected to a telemetry transmitter implanted extraperitoneally between the abdominal muscles. The EMG signals were collected by a receiver, filtered, amplified and archived by a computer. The telemetric EMG signal did not differ from that obtained with a conventional wire method. Substantial differences between day and night migrating myoelectric complex (MMC) patterns were found. The advantages of telemetric EMG recording make it a useful tool for long-term recording of antroduodenal EMG.  相似文献

This paper describes a non-invasive quantitative method for the diagnosis of neuromuscular disorders using surface EMG (sEMG). We found sEMG to be a reliable method that can be used to differentiate neuropathic and myopathic patients from the normal subjects. The multivariate discriminant analysis of sEMG data assisted in separating myopathic from neuropathic disorders. Nevertheless sEMG is not robust enough to replace needle EMG as a stand-alone diagnostic tool. However quantitative sEMG that is described in this paper could be adopted as a simple, rapid and non-invasive technique to be used in the out patients clinic by EMG-naive clinicians as a screening method for neuromuscular disorders, before referring the patients for detailed clinical neurophysiological examinations.  相似文献

The rapid increase in the use of mobile phones (MPs) in recent years has raised the problem of health risk connected with high-frequency electromagnetic fields. There are reports of headache, dizziness, numbness in the thigh, and heaviness in the chest among MP users. This paper deals with the neurological effect of electromagnetic fields radiated from MPs, by studies on heart rate variability (HRV) of 14 male volunteers. As heart rate is modulated by the autonomic nervous system, study of HRV can be used for assessing the neurological effect. The parameters used in this study for quantifying the effect on HRV are scaling exponent and sample entropy. The result indicates an increase in both the parameters when MP is kept close to the chest and a decrease when kept close to the head. MP has caused changes in HRV indices and the change varied with its position, but the changes cannot be considered significant as the p values are high.  相似文献
