A case-control study was conducted to examine occupational risk factors for sinonasal cancer. 207 cases and 409 controls (323 cancer hospital controls and 86 controls selected from lists provided by the cases) were included in the study. All cases and controls were interviewed by specially trained physicians. Detailed information on occupational history was collected, in addition to other potential risk factors for nasal cancer. Results are presented for two histologic types: adenocarcinoma and squamous cell carcinoma. Among males, the risk of adenocarcinoma was significantly elevated for carpenter and joiners (OR = 17.7) and other woodworkers (OR = 26.9). A significant increase in risk of squamous cell cancer (OR = 2.5) and a moderate increase in risk of adenocarcinoma (OR = 1.7; NS) were noted for construction workers. Among females, an excess risk of squamous cell carcinoma was associated with employment in textile industry (OR = 2.9). Elevated risks of squamous cell cancer were observed for farm workers of both sexes (males: OR = 2.2; females OR = 4.9). 相似文献

The objective of the current study was to estimate the risk of lung cancer attributable to occupational factors and not due to tobacco. At 24 hospitals in nine metropolitan areas in the United States, 1793 male lung cancer cases were matched for race, age, hospital, year of interview, and cigarette smoking (never smoker, ex-smoker, smoker (1-19 and > or = 20 cigarettes per day)) to two types of controls (cancer and non-cancer hospital patients). Information on usual occupation, exposure to specific potential carcinogens, and cigarette smoking was obtained by interview. Risk of lung cancer was increased significantly for electricians; sheetmetal workers and tinsmiths; bookbinders and related printing trade workers; cranemen, derrickmen, and hoistmen; moulders, heat treaters, annealers and other heated metal workers; and construction labourers. All of these occupations are potentially exposed to known carcinogens. Odds ratios (ORs) were increased for exposure to coal dust (adjusted OR = 1.5; 95% confidence interval (95% CI) 1.1-2.1). After stratification, this association was statistically significant only after 10 or more years of exposure. Lung cancer was also related to exposure to asbestos (adjusted OR = 1.8; 95% CI 1.5-2.2). The ORs increased with increasing duration of exposure to asbestos for all smoking categories except for current smokers of 1-19 cigarettes per day. The statistical power to detect ORs among occupations that were previously reported to be at increased risk of lung cancer but that failed to show an OR of at least 1.5 in the current study was small. The cumulative population attributable risk (PAR) of lung cancer due to occupation was 9.2%. It is concluded that occupational factors play an important part in the development of lung cancer independently of cigarette smoking. Because occupations at high risk of lung cancer were under-represented, the cumulative PAR of the present study is likely to be an underestimate of the true contribution of occupation to risk of lung cancer. 相似文献

A case-control comparison of lung cancer deaths versus deaths from all other cancers occurring in Alameda County, California, between 1958 and 1962 was conducted to investigate possible associations between lung cancer and occupation. Age-adjusted, sex-specific analyses indicated that a history of work in certain industries and occupations was associated with an increased risk of lung cancer. Among males, a significant positive association was found for glass, metal, furniture, professional and photographic equipment, rubber, leather and plastic manufacturing; water and air transportation; auto repair services; and construction. Specific occupations associated with a significant positive risk for males were: electricians, aircraft mechanics, painters, plasterers, machine operators, construction workers, bus and truck drivers, and guards and nightwatchmen. Among females, beauticians had a significantly high risk associated with lung cancer. These results were consistent with other studies of lung cancer related to occupation. 相似文献

We studied the relation between occupation and oral and pharyngeal cancer with a population-based case-control study conducted in four areas of the United States. The study group included 1,114 incident male and female cases and 1,268 frequency-matched controls. After adjustment for age, race, smoking, alcohol, and study location, an analysis of lifetime occupational histories revealed a small number of noteworthy associations. Risk was increased among male carpet installers (23 cases, 4 controls), with an adjusted odds ratio of 7.7 [95% confidence interval (CI) = 2.4-24.9], and tended to rise with longer duration of employment. A decreased risk was found among male and female textile mill workers (odds ratio 0.48, 95% CI = 0.27-0.88). Previously reported increases in oral cancer risks among printing workers, electrical and electronics workers, and workers other than carpet installers who were possibly exposed to formaldehyde were not found in this study. For several employment groups, including male machinists, primary metal industry workers, petroleum industry workers, painters, furniture and fixture industry workers, woodworking machine operators, and workers with inferred exposure to fossil fuel combustion, odds ratios were approximately 2.0 for cancers of pharyngeal sites. 相似文献

BACKGROUND: The role of crystalline silica dust as a possible cause of lung cancer has been controversial. Relatively few large community-based studies have been conducted to investigate the lung cancer risk from exposure to silica at low levels, taking into account potential confounding factors. METHODS: Detailed lifestyle and occupational information were collected from 2852 newly diagnosed cases of lung cancer and 3104 controls between 1998 and 2002 in 7 European countries. For each job held, local experts assessed the probability, intensity, and duration of silica exposure. RESULTS: Occupational exposure to crystalline silica was associated with an increased risk of lung cancer (odds ratio = 1.37; 95% confidence interval = 1.14-1.65). This risk was most apparent for the upper tertile of cumulative exposure (OR = 2.08; 95% CI = 1.49-2.90; P for trend <0.0001), duration of exposure (1.73; 1.26-2.39; P for trend = 0.0001) and weighted duration of exposure (1.88; 1.35-2.61; P for trend <0.0001). We did not observe any interaction beyond a multiplicative model between tobacco smoking and silica exposure. CONCLUSIONS: Our results support the hypothesis that silica is an important risk factor for lung cancer. This risk could not be explained by exposure to other occupational carcinogens or smoking, and it was present for the main histologic types of lung cancer, different sources of silica exposure, and different industrial settings. 相似文献

We conducted a population-based, case-control study on hematolymphopoietic malignancies in 12 areas in Italy to investigate associations between different hematolymphopoietic malignancies and exposure to solvents and pesticides. We collected all incident cases 20-74 years of age from 12 areas, with a combined population of approximately 7 million residents. The control group was formed by a random sample of the study population. Data presented in this paper refer to 2,737 interviewed cases of 3,357 eligible cases and to 1,779 of 2,391 eligible controls. We analyzed risks associated with occupation using job-title information to evaluate disease pattern according to job category. An earlier publication presented results for women; here, we report the findings for men and discuss the overall patterns in both genders. The most consistent overall finding was an approximate doubling in relative risk for all four types of malignancies among male managers and related occupations. Several additional occupations were associated with elevated risk of one or more malignancies among men. These included cooks, waiters, and bartenders, and building caretakers and cleaners, for non-Hodgkin's lymphoma; textile workers and machinery fitters for Hodgkin's lymphoma; metal processors, material handlers, rubber workers, and painters for leukemia; and hairdressers, metal processors, tailors, electrical workers, and plumbers for multiple myeloma. The finding of increased risk of non-Hodgkin's lymphoma among both male and female cooks, waiters, and bartenders has not been previously reported; nor has the elevated risk of leukemia among material handlers. Among people engaged in agriculture, those employed as tractor drivers and as "orchard, vineyard, and related tree and shrub workers" appeared to be at increased risk for hematolymphopoietic malignancies. 相似文献

Paternal occupation and brain cancer in offspring: a mortality-based case-control study 总被引:2,自引:0,他引:2
A mortality-based case-control study of selected risk factors for childhood brain tumors was undertaken. Ohio-born children who died from brain cancer during the 1959-1978 vicennium were compared to control children (of the same age, race, and sex) by using information obtained from the subjects' birth certificates. Differences between the case and the control children with respect to paternal occupation, the focus of the study, were examined. Controlling for the potentially confounding effects of several nonoccupational factors, case fathers were found more likely than control fathers to have been employed (at the time of birth of their children) in agriculture, in metal-related jobs, in structural work jobs in the construction industry, and in electrical assembling, installing, and repairing occupations in the machinery industry. Although the results must be interpreted with caution, the findings lend support to the hypothesis that parental occupation is a potential risk factor for childhood brain tumors. 相似文献

OBJECTIVES--A hospital based case-control study was conducted between 1992 and 1993 in the province of Brescia, a highly industrialised area in northern Italy, to evaluate occupational risk factors of bladder cancer. METHODS--The study evaluated 355 histologically confirmed cases of bladder cancer (275 men, 80 women) and 579 controls affected by urological non-neoplastic diseases (397 men, 182 women). Lifetime occupational history, smoking and drinking habits, and sociodemographic characteristics were recorded by means of a structured questionnaire. Odds ratios (ORs) were computed with adjustment for age, smoking, alcohol and coffee consumption, education, and place of residence. RESULTS--A significant (P < 0.05) increase of risk of bladder cancer were found in men for labourers in the construction industry (OR 2.1, 95% confidence interval (95% CI) 1.1-3.9) and for recreational and cultural services (OR 5.0, 95% CI 1.3-18.9). Increased risks, although not significant, were found for various other occupations and industries such as machinery mechanics, metal processers and polishers, blacksmiths, gunsmiths, painters; for transport workers, an increased risk with increasing duration of employment was found. CONCLUSIONS--Occupational exposures seem to contribute to bladder cancer risk in the area under study. 相似文献

目的了解福建省非吸烟人群口腔癌的发病影响因素, 为口腔癌的防治提供科学依据。方法采用以医院为基础病例对照研究方法对2010年9月—2014年1月在福建医科大学附属第一医院收治的161例非吸烟新发口腔癌患者及同期在该医院进行健康体检和在其他科室就诊的547例非吸烟非口腔相关疾病患者进行问卷调查。结果病例组患者<18岁被动吸烟史、≥18岁被动吸烟史、不良修复体、刷牙频率>2次/天、有口腔溃疡、食水果频率<1次/天的比例分别为37.27%、49.07%、53.42%、5.59%、8.70%、49.69%, 均高于对照组患者的10.97%、29.62%、36.56%、5.30%、2.01%、43.69%(均P<0.01);病例组患者刷牙频率1~2次/天、食鱼类频率≥1次/天、食海鲜频率<1次/天、食海鲜频率≥1次/天、食绿色蔬菜频率≥1次/天、食其他蔬菜频率≥1次/天、食水果频率≥1次/天的比例分别为84.47%、13.66%、53.42%、3.11%、83.23%、77.64%、25.47%, 均低于对照组患者的92.87%、40.04%、55.94%、22.85%、97.26%、96.53%、47.90%(均P=0.000);多因素条件logistic回归分析结果显示, <18岁被动吸烟、≥18岁被动吸烟、有不良修复体、有口腔溃疡是福建省非吸烟人群口腔癌发病的危险因素, 食鱼类频率≥1次/天、食其他蔬菜频率≥1次/天、食水果频率≥1次/天、食海鲜频率<1次/天和≥1次/天是福建省非吸烟人群口腔癌发病的保护因素。结论被动吸烟、不良修复体、口腔溃疡及食鱼类、海鲜、其他蔬菜、水果是福建省非吸烟人群口腔癌发病的主要影响因素。 相似文献

BACKGROUND: Few data are available to explain the ongoing increase in lung cancer mortality among Czech women. The study is aimed at examining the role of smoking and known or suspected cofactors. METHODS: Data collected by in-person interviews from 269 female lung cancer cases and 1079 controls were analyzed using unconditional logistic regression and other methods. RESULTS: Cigarette smoking was the most important factor associated with excess risk of lung cancer among women. Risk was increased both among current smokers (OR = 10.30), long-term ex-smokers (> or =10 years ago; OR = 3.79), and short-term ex-smokers (<10 years ago; OR = 14.63), all compared against never-smokers. In addition, significant associations with risk were found for chronic cough, chronic phlegm of less than 2-year duration, and shortness of breath. Inverse associations emerged for physical exercise and body mass index. Excess risk associated with consumption of red meat and poultry, and protective effects associated with intake of vegetables were restricted to squamous-, small-, and large-cell cancers combined, but were not apparent for adenocarcinoma. CONCLUSIONS: While smoking has been verified to be the main determinant of lung cancer risk among Czech women, cofactors such as diet, history of lung disease, and lifestyle factors may have a contributory role. 相似文献

A case-control study was conducted in the southern part of The Netherlands to investigate the risk of lung cancer in coal miners; 381 age-matched pairs of primary lung cancer cases and controls, diagnosed between 1972 and 1988, were selected from the pathology department of the University Hospital in the region. Information about past employment in coal mines was obtained through the registers of the collaborative pension fund for Dutch miners. 20% of the cases were (at some time) employed in coal mining, compared with 21% of the controls (odds ratio 0.95; 95% confidence interval: 0.65-1.38). 9% of both cases and controls had an underground work history (odds ratio 0.96; 95% confidence interval: 0.56-1.65). The duration of underground coal mining did not differ substantially between cases and referents (average duration: respectively 117 and 108 months). No relation between specific histologic tumor cell types and coal mining could be demonstrated. The study gives no indication that workers in Dutch coal mines have an increased risk of developing lung malignancies. 相似文献

This report presents the findings of a case-control study of lung cancer among workers at a rubber manufacturing plant. Detailed work histories of 40 lung cancer deaths, which occurred during 1977 and 1978, were compared with those of 146 controls who were individually matched to cases on age, sex, and race; 66 of the controls were also matched on year of first hire. Elevated odds ratios were noted for different durations of employment in four work areas: Reclaim Operation, Chemicals, Special Products Manufacture, and Curing Preparation. A clear trend of increasing odds ratios with increasing durations of exposure was observed for the first three of these work areas. Several chemicals used in these processes are known respiratory system hazards. The results of this study suggest an association between lung cancer mortality risk and employment in Reclaim Operation, Chemicals, and Special Products Manufacture. A moderate excess risk was observed in these areas among both smokers and nonsmokers, although the risk was greater among the smokers. 相似文献

BACKGROUND: The aim of this study was to add to the data on associations between occupation and bladder cancer thereby strengthening the case for focused research on specific occupational categories. METHODS: Two hundred two cases and 390 controls participated in this population-based case-control study. Age, sex, smoking, and education adjusted odds ratios (ORs) were calculated for each occupation by unconditional logistic regression. RESULTS: The occupational group of domestic helpers, cleaners, and launderers exhibited a significantly elevated risk of bladder cancer. Elevated non-significant risks (OR > 1.5) were observed for painters and varnishers, electronic equipment assemblers, general laborers, building finishers, carpenters and joiners, architects and engineers, and textile and garment workers. CONCLUSIONS: This study suggests that domestic helpers, cleaners, and launderers probably experience an increased risk of bladder cancer. Although no other occupations showed significantly increased risks, the non-significantly elevated risks for painters, carpenters, and textile workers could be confirmed by results from earlier studies. 相似文献

OBJECTIVES: Associations between occupational exposures and the occurrence of cutaneous melanoma were examined as part of a large population based case-control study of 19 cancer sites. METHODS: Cases were men aged 35 to 70 years old, resident in Montreal, Canada, with a new histologically confirmed cutaneous melanoma (n = 103). There were two control groups, a randomly selected population control group (n = 533), and a cancer control group (n = 533) randomly selected from among subjects with other types of cancer in the large study. Odds ratios for the occurrence of melanoma were calculated for each exposure circumstance for which there were more than four exposed cases (85 substances, 13 occupations, and 20 industries) adjusting for age, ethnicity, and number of years of schooling. RESULTS: Significantly increased risk of melanoma was found for exposure to four substances (fabric dust, plastic dust, trichloroethylene, and a group containing paints used on surfaces other than metal and varnishes used on surfaces other than wood), three occupations (warehouse clerks, salesmen, and miners and quarrymen), and two industries (clothing and non-metallic mineral products). CONCLUSIONS: Most of the occupational circumstances examined were not associated with melanoma, nor is there any strong evidence from previous research that any of those are risk factors. For the few occupational circumstances which were associated in our data with melanoma, the statistical evidence was weak, and there is little or no supporting evidence in the scientific literature. On the whole, there is no persuasive evidence of occupational risk factors for melanoma, but the studies have been too small or have involved too much misclassification of exposure for this conclusion to be definitive. 相似文献

The association between diet and prostate cancer (PC) risk, although suggestive, still remains largely elusive particularly in the Asian population. This study sought to further evaluate the possible effects of different dietary factors on risk of PC in Iran. Using data from a prospective hospital-based multicenter case-control study, dietary intakes of red meat, fat, garlic, and tomato/tomato products, as well as thorough demographic and medical characteristics, were determined in 194 cases with the newly diagnosed, clinicopathologically confirmed PC and 317 controls, without any malignant disease, admitted to the same network of hospitals. Odds ratios (ORs) and corresponding 95% confidence intervals (CIs) were obtained after adjustment for major potential confounders, including age, body mass index, smoking, alcohol, education, occupation, family history of PC, and total dietary calories. Comparing the highest with the lowest tertile, a significant trend of increasing risk with more frequent consumption was found for dietary fat (OR: 1.79, 95% CI: 1.71-4.51), whereas inverse association was observed for tomato/tomato products (OR: 0.33, 95% CI: 0.16-0.65). A nonsignificant increase in PC risk was revealed for dietary red meat (OR: 1.69, 95% CI: 0.93-3.06). For garlic consumption, a borderline reduction in risk was observed (OR: 0.58, 95% CI: 0.32-1.01; P = 0.05). In conclusion, our study supports the hypothesis that total fat may increase PC risk and tomatoes/tomato products and garlic may protect patients against PC. 相似文献
