The prevalence of, and risk factors for, laboratory animal allergy (LAA) among university employees were evaluated in a cross-sectional university-based study. A stratified random sample was drawn based on current or no laboratory animal exposure and smoking status. Participants received a modified ATS questionnaire; spirometry; methacholine challenge; and intradermal allergen skin tests. One hundred three currently animal-exposed and 113 never-occupationally-exposed employees participated. Controlling for smoking, currently exposed workers were significantly more likely than controls to describe work-related cough, odds ratio (O.R.) = 6.87; wheeze, O.R. = 12.96; and chest tightness, O.R. = 2.89. Skin test reactivity to non-animal antigens was associated in a dose-response fashion with both upper and lower respiratory symptoms, O.R. = 1.45 and 1.65, respectively, for each additional positive skin test. Exposed workers were significantly more likely than controls to react to methacholine at either 10 or 25 mg/ml, while controlling for smoking status, prior allergy, or asthma. Multiple logistic regression anaysis revealed history of hay fever, family history of allergy, non-animal skin test positivity, animal bites, age, and smoking status to each be associated with work-related respiratory symptoms. These data suggest that atopy and smoking status are risk factors for LAA symptoms. Smoking was associated with work-related respiratory symptoms among animal-exposed workers, but not with skin test positivity. It is therefore recommended that periodic screening evaluations be performed on laboratory animal workers and that workers who are atopic, are smokers, or are symptomatic be placed in low exposure settings. These data further support the need for efficient exhaust ventilation, personal protective clothing and, among high risk workers, the use of efficient respiratory protection. © 1996 Wiley-Liss, Inc.  相似文献

BACKGROUND: Engineering or administrative methods are often insufficient or impractical to control exposure to chemicals in anatomy laboratories. This study explored the feasibility of wearing one or a combination of respirators and goggles used as personal protective equipment (PPE) to control exposure in one such laboratory. METHODS: A group of 28 subjects were briefly trained in wearing PPE, fit-tested, and asked to complete a questionnaire regarding their subjective reaction after wearing the assigned PPE ensemble while working in the laboratory. The subjects' exposure to formaldehyde was also measured and generally exceeded the recommended limits. RESULTS: When a full-face respirator or the combination of a half-mask respirator and goggles was worn, a majority of subjects reported no odor problem and no irritation to eyes or upper respiratory system. Subjects accepted the PPE to certain degrees, but those using respirators encountered difficulties communicating with others. CONCLUSIONS: The combination of a half-mask respirator and goggles was the most feasible ensemble to control exposure to air pollutants in an anatomy laboratory.  相似文献

Abstract

The use of urinary hexane diamine (HDA) as a biomarker to assess human respiratory exposure to hexamethylene diisocyanate (HDI) aerosol was evaluated. Twenty-three auto body shop workers were exposed to HDI biuret aerosol for two hours using a closed exposure apparatus. HDI exposures were quantified using both a direct-reading instrument and a treated-filter method. Urine samples collected at baseline, immediately post exposure, and every four to five hours for up to 20 hours were analyzed for HDA using gas chromatography and mass spectrometry. Mean urinary HDA (μg/ g creatinine) sharply increased from the baseline value of 0.7 to 18.1 immediately post exposure and decreased rapidly to 4.7, 1.9 and 1.1, respectively, at 4,9, and 18 hours post exposure. Considerable individual variability was found. Urinary HDA can assess acute respiratory exposure to HDI aerosol, but may have limited use as a biomarker of exposure in the workplace.  相似文献

The aim of this study was to determine whether occupational exposure to chromite, trivalent chromium (Cr(3+)) or hexavalent chromium (Cr(6+)) causes respiratory diseases, an excess of respiratory symptoms, a decrease in pulmonary function or signs of pneumoconiosis among workers in stainless steel production. Altogether, 203 exposed workers and 81 referents with an average employment of 23 years were investigated for indicators of respiratory health on two occasions, in 1993 and in 1998. Data collection with a self-administered questionnaire, flow volume spirometry, measurement of diffusing capacity, chest radiography and laboratory tests were carried out by a mobile research unit. Exposure to different chromium species and other metals was monitored regularly and studied separately. No adverse respiratory health effects were observed in the group exposed to Cr(6+), either in comparison with the control group in the first cross-sectional study or during the additional 5 year follow-up. Among the Cr (3+) exposed people, the production of phlegm, shortness of breath and breathlessness on exertion were significantly more frequent than in the control group, but the frequency of the symptoms did not increase during the follow-up; no differences were observed in the lung function tests and the radiographic findings did not progress. In the chromite group, the prevalence of breathlessness on exertion was higher than in the control group. However, in the follow-up, the occurrence of symptoms did not differ from 1993 to 1998. In the first study, most parameters of lung function were lower among the smokers in the chromite group than among the smoking controls, but in 1998 the difference was less marked. An average exposure time of 23 years in modern ferrochromium and stainless steel production and low exposure to dusts and fumes containing Cr(6+), Cr(3+), nickel and molybdenum do not lead to respiratory changes detectable by lung function tests or radiography. The workers exposed to Cr(3+) had more respiratory symptoms than those in the control group. The workers in the chromite mine had lower lung function test results than the control group due to earlier exposure to higher dust concentrations.  相似文献

Epoxy resin systems have been associated with occupational asthma in several case reports, but medical publications contain little on the potential adverse respiratory effects of these chemicals in exposed worker populations. To further evaluate the association of workplace exposure to epoxy paints and respiratory dysfunction, the cross workshift changes in pulmonary function and symptoms of 32 shipyard painters exposed to epoxy paints were compared with 28 shipyard painters not exposed to epoxy paints. The prevalence of lower respiratory tract symptoms was significantly higher among painters exposed to epoxy paints compared with controls. Among exposed painters the mean cross workshift change in forced expiratory volume in one second (FEV1) (-3.4%) was greater than the decrement in the non-exposed group (-1.4%). A significant linear relation was seen between % decrement in FEV1 and hours of exposure to epoxy paints. This study suggests that epoxy resin coatings as used by shipyard painters are associated with increased lower respiratory tract symptoms and acute decrements in FEV1. Adequate respiratory protection and medical surveillance programmes should be established in workplaces where exposure to epoxy resin systems occurs.  相似文献

In the weeks after Hurricane Katrina struck the U.S. Gulf Coast on August 29, 2005, reports of increased injuries and symptoms of physical illness and psychological strain among New Orleans police officers and firefighters prompted CDC to conduct a health hazard evaluation of these two groups. Questionnaires were distributed to members of the New Orleans Police Department (NOPD) and New Orleans Fire Department (NOFD) 7-13 weeks after the hurricane. This report summarizes the results of that evaluation, which determined that upper respiratory and skin rash symptoms were the most common physical symptoms reported by police officers and firefighters and lacerations and sprains were the most common injuries. In addition, approximately one third of the respondents reported either depressive symptoms or symptoms of posttraumatic stress disorder (PTSD), or both. These results underscore the need to incorporate the safety and health of emergency responders into existing disaster preparedness plans and to provide periodic responder training and education in tasks unique to disaster situations. Clinical follow-up of the physical and psychological health of emergency responders should be conducted to better understand, monitor, and treat their health conditions.  相似文献

Objective  Due to new legal requirements in Germany, the employer must request preventive medical check-ups for activities involving exposure to dust from experimental animals in the rooms in which the animals are kept. The objective is to report our first experiences with these medical check-ups in the context of academic research. Methods  The check-ups were carried out since November 2005 and comprised a questionnaire and a medical examination, including a pulmonary function test with whole-body plethysmography. Respiratory, nasal and ocular symptoms related to occupational exposure to animals were documented. Participation in skin prick tests (ubiquitous inhalation allergens and laboratory animal allergens), a bronchial provocation test with methacholine, and serological examinations for total IgE and specific IgE antibodies was voluntary. Results  Data on 132 persons are presented. One hundred and six of these had already been exposed for at least 1 year. Main complaints at the workplace were sneezing and runny nose. Ocular symptoms and bronchial asthma were reported infrequently. The development of at least one of these symptoms occurred in 34% of employees with an exposure of at least 1 year. If the weekly exposure duration was at least 5 h, the proportion of employees with complaints rose to 44.9%. In employees occupationally exposed to mice and rats, work-related complaints occurred in 33.7 and 37.8%, respectively, and sensitisation rates were 12.7 and 16.3%, respectively. Employees with and without complaints differed in history of allergic symptoms, and workplace safety measures. Conclusions  In employees with occupational contact with laboratory animal dust, the frequency of complaints was high. The results confirm the necessity of regular medical check-ups for employees with contact with laboratory animal dust. Nevertheless, the medical check-ups must be part of a prevention strategy including education, engineering controls, administrative controls, use of personal protective equipment and vocational integration.  相似文献

Ted is a 38 year old service technician for a large chemical company servicing cooling towers at a car manufacturing plant. While blending a batch of chemicals, he accidentally splashed cooling tower fluid, Kathon, an isothiazolin-3 derivative, over his left forearm. He was wearing long sleeved overalls and safety glasses at the time. About 15 minutes later, Ted experienced some itchiness of his forearm and proceeded to wash the area. He did not change his uniform. Five hours later he experienced severe pain of the forearm. Inspection revealed a 5 cm dermal burn with obvious blistering over the left forearm. He sought medical treatment and was treated conservatively with routine burn dressings. The skin blistering healed after seven days. Ted returned to work undertaking full duties. When undertaking mixing of cooling tower chemicals he developed an itchy rash of the forearms that appeared to be spreading to his hand and shoulder areas. This skin irritation occurred even when there was no contact with cooling tower fluids.  相似文献

BACKGROUND: Pesticide exposure is a major preventable occupational hazard for farmworkers. This study examined the beliefs of Latino farmworkers in North Carolina's Christmas tree industry regarding pesticide exposure. METHODS: In-depth interviews were conducted with 20 Mexican male seasonal farmworkers. Participants discussed beliefs about agricultural chemicals, routes of exposure, and health effects of these chemicals. They also discussed their knowledge and use of pesticide safety practices and safety training received. RESULTS: Most farmworkers knew that pesticides could be harmful, though workers varied in their levels of knowledge regarding routes of exposure, specific health effects of pesticides, and ways to avoid and reduce exposure. Workers varied considerably in the amount of safety training received and use of safety practices. Perceived lack of control and health beliefs were salient factors that decreased workers' use of safety practices. CONCLUSIONS: This study adds to the growing body of research which documents the health beliefs of Latino farmworkers in the U.S. relative to pesticides and pesticide safety. This literature is beginning to show convergence on several points (e.g., farmworker knowledge of acute vs. long-term illness resulting from pesticide exposure), as well as regional variation in pesticide safety beliefs. This study substantiates the need for pesticide safety education to address issues of control as well as beliefs.  相似文献

目的阐明职业性三氯乙烯(TCE)药疹样皮炎职业流行病学特征,为其防治与研究提供资料。方法用专用调查表对广东省1988~2001年发病的150例职业性TCE药疹样皮炎病例进行回顾性调查,同时收集这些发病事故的调查资料,用Excel建立数据库,SPSS10.0进行统计处理。结果这组病例的临床表现和发病特点与药疹皮炎极其相似,表现为剥脱性皮炎、重症多形红斑、大疱性表皮坏死松解症或多形红斑。其发病均因接触TCE,但与接触浓度无明显关系;潜伏期均小于3个月;××市3年调查,接触TCE新工人的发病率仅为0.045%;激素治疗有效,病后再接触可再发病,斑贴试验阳性等。引起发病的TCE含量在95%以上,产地也不同,全分析未发现相同杂质;全部病例没有发现接触甲醛、甲胺磷等能引起药疹样皮炎的化学品;大部分病例发疹前2个月没有服过任何药物;美国和日本等地已有21例相似的病例报道。因接触TCE的新工人多和临床医师和管理人员提高了对本病的认识,使广东多发。结论本组病例都由职业性接触TCE引起,与药疹样皮炎相似,属免疫增强损伤。称为“职业性TCE药疹样皮炎”;其流行病学特点均与职业性TCE接触明显相关。  相似文献

The occupational exposure of 19 men to hexamethylene diisocyanate (HDI) vapour was monitored during one 8-h shift. It ranged from 0.30 to 97.7 μg/m3. This was compared with the urinary output of hexane diamine (HDA) liberated by acid hydrolysis from its conjugates in post-shift samples. The excretion varied from 1.36 to 27.7 μg/g creatinine, and there was a linear association of HDI air concentration with urinary HDA excretion. The validity of the urinary analysis was confirmed by simultaneous blind analysis in another laboratory. The results had an excellent linear concordance. Thus, it seems that while the gas chromatographic-mass spectrometric detection method requires sophisticated apparatus, the results are very useful to occupational health practices. A biological exposure index limit of 19 μg HDA/g creatinine in a post-shift urine specimen is proposed as an occupational limit level of HDI monomer (time-weighted average=75 μg/m3). Most importantly, biological monitoring of HDA is sensitive enough to be used at and below the current allowable exposure limit levels.  相似文献

Abstract

Respiratory and allergic symptoms were evaluated in 66 Iranian carpet weavers and 66 controls with similar age and gender characteristics using a questionnaire including questions on work-related respiratory symptoms in the past year, allergies, smoking habits, and work exposure duration. A total of 28 carpet weavers (42%) reported work-related respiratory symptoms. Incidences of all respiratory symptoms and most allergic symptoms were significantly higher in carpet weavers than in controls (p < 0.05?p < 0.001). Moreover, most respiratory and allergic symptoms in carpet weavers were significantly more prominent during working hours (p < 0.01?p < 0.001). Pulmonary function test results of the carpet weavers showed significant impairment compared with controls (p < 0.05?p < 0.001).  相似文献

The subjectivity of irritancy judgments can bias attempts to establish exposure guidelines that protect individuals from the sensory irritation produced by volatile chemicals. At low to moderate chemical concentrations, naive and occupationally exposed individuals often show considerable variation in the reported levels of perceived irritation. Such variation could result from differences in exposure history, differences in the perceived odor of a chemical, or differences in generalized response tendencies to report irritation, or response bias. Thus, experimental evaluation of sensory irritancy must dissociate sensory irritation from response bias. To this end, judgments of perceived irritation from 800 ppm acetone were obtained from acetone-exposed workers and age- and gender-matched naive controls. To assess the role of response bias during exposure to odorants, subjects were also exposed to phenylethyl alcohol (PEA), an odorant that does not produce sensory irritation. Following exposure, subjects completed a subjective symptom survey that included symptoms that have been associated with long-term solvent exposures and symptoms that have not. Acetone-exposed workers and naive controls reported large differences in the perceived intensity of odor and irritation from acetone, yet no differences in the perception of PEA. However, for both groups, the most significant factors mediating reported irritancy and health symptoms from acetone were the perceived intensity of its odor and an individual's bias to report irritation from PEA. The perception of odor intensity and degree of response bias will differ between and within groups of exposed and naive individuals; hence, an assessment of the influence of these factors in experimental and workplace studies of chemical irritancy is warranted. The objective was to evaluate the characteristics of exposure to OP pesticides and health status in Chilean farm workers from the Maule Region. An occupational health questionnaire was administered in 207 agricultural and non-agricultural workers. For the group of agricultural workers, we asked about specific occupational exposure history and symptoms of OP pesticide poisoning. The main health problem of the exposed group was previous OP pesticide poisoning (p < 0.001). Fifty-six percent of agricultural workers reported symptoms consistent with acute OP pesticide poisoning. The use of respiratory personal protective equipment and younger age were protective against these symptoms, and number of years of OP pesticide exposure was positively associated with reporting symptoms of poisoning. Of the pesticide applicators 47 % reported using chlorpyrifos. The regulations regarding use and application of pesticides should be strengthened, as should training and intervention with workers to improve the use of personal protective equipment.  相似文献

Individuals who paint cars often complain to doctors about respiratory problems. Car painters are exposed to isocyanates, especially hexamethylendiisocyanate (HDI), and biuret modified HDI (HDI-BT). The mean exposure to HDI-BT was 115 micrograms/m3 in the air (range 10-385 micrograms/m3), which exceeds the time-weighted Swedish threshold level of 90 micrograms/m3. Exposure to HDI was about 1.0 microgram/m3 with brief peaks. This study investigated the effect of HDI and HDI-BT on lung function and included two control groups: (1) car platers, exposed to the same solvents and grinding dust as car painters, but not to isocyanates, and (2) car mechanics (controls), not exposed to the mentioned agents. Car painters and car platers were compared to car mechanics on Monday before work. Acute effects of car painting were tested by comparing the lung function values on Monday morning with those on Friday afternoon. Pulmonary function was evaluated by means of spirometry and a single breath nitrogen washout. Spirometry in painters and platers did not differ from that in controls, i.e., car mechanics. Closing volume in relation to vital capacity (CV%) was increased in car painters, suggestive of a "small airways disease" on Monday before work and tended to increase during a work week. Objective: The aim of this study was to identify possible health effects caused by different cleaning agents used in graffiti removal. Methods: In 38 graffiti removers working 8-h shifts in the Stockholm underground system, the exposure to organic solvents was assessed by active air sampling, biological monitoring, and by interviews and a questionnaire. Health effects were registered, by physical examinations, porta7ble spirometers and self-administered questionnaires. The prevalence of symptoms was compared with 49 controls working at the underground depots, and with 177 population controls. Results: The 8-h time-weighted average exposures (TWA) were low, below 20% of the Swedish permissible exposure limit value (PEL) for all solvents. The short-term exposures occasionally exceeded the Swedish short-term exposure limit values (STEL), especially during work in poorly ventilated spaces, e.g. in elevators. The graffiti removers reported significantly higher prevalence of tiredness and upper airway symptoms compared with the depot controls, and significantly more tiredness, headaches and symptoms affecting airways, eyes and skin than the population controls. Among the graffiti removers, some of the symptoms increased during the working day. On a group basis, the lung function registrations showed normal values. However, seven workers displayed a clear reduction of peak expiratory flow (PEF) over the working shift. Conclusions: Though their average exposure to organic solvents was low, the graffiti removers reported significantly higher prevalence of unspecific symptoms such as fatigue and headache as well as irritative symptoms from the eyes and respiratory tract, compared with the controls. To prevent adverse health effects it is important to inform the workers about the health risks, and to restrict use of the most hazardous chemicals. Population exposure to multiple chemicals in air presents significant challenges for environmental public health. Air quality regulations distinguish criteria air pollutants (CAPs) (e.g., ozone, PM2.5) from hazardous air pollutants (HAPs)-187 chemicals which include carcinogens and others that are associated with respiratory, cardiovascular, neurological and numerous other non-cancer health effects. Evidence of the public's cumulative exposure and the health effects of HAPs are quite limited. A multilevel model is used to assess differential exposure to HAP respiratory, neurological, and cancer hazards (2005) related to the Townsend Index of Socioeconomic Deprivation (TSI), after adjustment for regional population size and economic activity, and local population density. We found significant positive associations between tract TSI and respiratory and cancer HAP exposure hazards, and smaller effects for neurological HAPs. Tracts in the top quintile of TSI have between 38%-60% higher HAP exposure than the bottom quintile; increasing population size from the bottom quintile to the top quintile modifies HAP exposure hazard related to TSI, increasing cancer HAP exposure hazard by 6% to 20% and increasing respiratory HAP exposure hazard by 12% to 27%. This study demonstrates the value of social epidemiological methods for analyzing differential exposure and advancing cumulative risk assessment.  相似文献

目的了解垃圾焚烧厂周围居民呼吸系统健康状况,探讨垃圾焚烧暴露对周围人群呼吸系统的健康影响。方法于2012年10—11月对垃圾焚烧厂附近0~5 km区域(暴露区)和5~10 km区域(非暴露区)的1 043名研究对象进行基本信息和呼吸系统健康状况的问卷调查,分析两区域内居民呼吸系统健康状况差异,并采用logistic回归分析垃圾焚烧暴露与呼吸系统症状间的关系。结果暴露区人群的呼吸系统症状总体发生率(72.73%)高于非暴露区(65.55%),差异有统计学意义(P0.05)。多因素logistic回归分析显示,暴露区人群咽部异物感的发生率是非暴露区的1.422倍(OR=1.422,95%CI:1.096~1.846),差异有统计学意义(P0.05),而对咳嗽、咯痰、鼻部不适、喘息及胸闷等其他呼吸系统症状的影响无统计学意义(P0.05)。结论垃圾焚烧暴露与人群咽部异物感可能存在一定关联。  相似文献
