This study was designed to evaluate MRA imaging as a potential non-invasive method to study vascular infiltration in patients with abdominal neoplasms. Forty-three patients with abdominal tumors proven by CT and/or MRI were examined with MRA and subsequently with angiography. Of 15 cases of liver tumor, MRA allowed poor intratumoral vascularization to be demonstrated in 1 patient only, but it always provided a characteristic sign--i.e., the complete absence of flow in the hepatic segments infiltrated by the tumor and the evidence of pericapsular neovascularization. In 8 cases portal vein involvement was observed (4 cases of compression/dislocation, 2 cases of infiltration and 2 of thrombosis). In all cases MRA allowed the relationship between tumor and venous structures to be evaluated. It also demonstrated collateral vessel formations in 8 patients with cancrocirrhosis. Two cases of thrombosis and 3 infiltrations of the vena cava were demonstrated. Neither hepatic artery nor intrahepatic arterial vascularization could be correctly evaluated. Of 10 cases of renal tumors and in 3 of adrenal tumors, renal vein infiltration was seen in 4 cases and compression in 3, with only 1 false-positive finding. In these patients and in those with retroperitoneal masses, MRA provided valuable information on the relationship between tumor and vascular structures and was useful in detecting collateral vessels. In our experience, MRA is to be considered as a complementary technique to be performed after CT or MRI when additional information is needed as to the relationship between tumor and vascular structures. In the preoperative evaluation of abdominal neoplasm, angiography remains the gold standard, in spite of its invasiveness.  相似文献

Deep cerebral venous thrombosis: imaging in eight cases   总被引:5,自引:1,他引:4  
Deep cerebral venous thrombosis (DCVT) is a rare, potentially fatal disease. We carried out a retrospective analysis of eight patients presenting with DCVT, using CT and MRI data. Digital subtraction angiography (DSA), MR angiography (MRA) (2D phase-contrast), and angiography were performed in four, four and two patients, respectively. Direct (venous abnormalities) and indirect (parenchymatous involvement) signs of thrombosis were assessed. Follow-up MRI and MRA were performed on two patients. CT was initially normal in two patients, demonstrated a “cord sign” in deep veins in five and deep venous infarcts in three. When performed within a week of admission (seven patients), MRI showed thrombosis as into intense or high signal in deep veins on sagittal T1-weighted spin-echo images in all patients, and on axial T2-weighted spin-echo images in two. Deep venous infarcts were found in five patients. Direct or indirect signs of sagittal or lateral sinus thrombosis were present on CT in two patients and on MRI in five. CT angiography, MRA and DSA were concordant with MRI findings. CT venography showed persistent flow in thrombosed veins. MRI follow-up demonstrated progressive deep venous recanalisation. RATIONALE AND OBJECTIVES: To evaluate the diagnostic value of breath-hold contrast-enhanced 3D magnetic resonance angiography (MRA) for assessment of the visceral abdominal arteries and veins in patients with suspected abdominal neoplasms. METHODS: Twenty-one patients underwent MR imaging on a 1.5 T unit using a body phased-array coil. MRA was performed with a 3D-FLASH sequence (TR 3.8 ms, TE 1.3 ms, flip angle 25 degrees, acquisition time 20 seconds), 8 to 12 seconds after an intravenous bolus injection of Gd-DTPA. The acquisition delay between the arterial and the portal venous phase was 12 seconds. The image quality and the degree of vascular involvement were evaluated using coronal source images and maximum intensity projection reconstructions. Diagnosis was confirmed by surgery/histology. RESULTS: Image quality was optimal in more than 85% of the patients (19/21 arterial phase and 17/21 portal venous phase). MRA correctly predicted vascular status in 20 of 21 patients (95%), with complete concordance between MRA results and surgical findings. In one patient with chronic pancreatitis, MRA demonstrated a false-positive finding that could not be confirmed surgically. CONCLUSIONS. Breath-hold contrast-enhanced 3D-MRA is a valuable technique for assessing visceral abdominal arteries and veins.  相似文献

Twenty-five patients with known or suspected evidence of venous disease based on results of computed tomography, angiography, or ultrasound were imaged with magnetic resonance (MR) to determine the MR characterization of venous abnormalities. MR findings were proved by laparotomy or autopsy in 18 of 25 cases. In seven of 25 patients in whom only biopsy was performed, the MR findings were correlated with findings from other radiologic tests. On MR, the inferior vena cava (IVC), portal vein, and their major tributaries were seen in all but two cases. In those two, identification of collaterals led to the correct diagnosis of splenic vein thrombosis in one case and left renal vein thrombosis in another. MR imaging helped identify intraluminal thrombi in the IVC (12 of 12 cases), portal vein (two of two cases), renal veins (seven of seven cases), superior mesenteric vein (one case), and iliac veins (seven of seven cases). Intraluminal signal intensity secondary to slow blood flow seen in five patients was always differentiated from the thrombus. MR imaging helped identify correctly the nature of the thrombus in 11 of 16 patients. In five patients, the differentiation between tumor thrombus and blood clot thrombus was not possible. Involvement of the IVC wall by tumor was seen in four cases. MR imaging also accurately depicted slow flow in obstructed or constricted veins; encasement, compression, or displacement of veins without intraluminal occlusions; and the presence of venous collaterals. The MR imaging evaluation of venous abnormalities is accurate, easily performed, and will probably become an important application.  相似文献

PURPOSETo determine whether MR angiography can be used to differentiate between the two vascular causes of bithalamic hyperintensity on T2-weighted MR images: "top of the basilar" artery occlusion and deep cerebral vein thrombosis.METHODSA retrospective review identified six patients with bithalamic T2 hyperintensity of vascular causes. MR angiography was performed in four patients, MR angiography and conventional angiography in one patient, and conventional angiography in one patient. Data pertaining to clinical presentation and hospital course were collected. MR angiographic techniques were multislab overlapping three-dimensional time-of-flight, 2-D time-of-flight, and 2-D phase-contrast.RESULTSThree cases of top of the basilar artery occlusion and three cases of deep cerebral vein thrombosis were recognized. In all cases, T2 hyperintensity in a vascular distribution suggested cerebral occlusive disease. Infarction involving the thalami and basal ganglia was present in two cases of deep cerebral vein thrombosis. Infarction of the thalami, mesodiencephalic region, and cerebellar hemispheres was present in two cases of basilar artery occlusion. Bithalamic infarction alone was seen in one case of deep cerebral vein thrombosis and one case of basilar artery occlusion. In the five cases in which MR angiography was used, this technique accurately distinguished the vessels involved (arterial or venous).CONCLUSIONMR angiography is a useful adjunct to MR imaging in the evaluation of bithalamic T2 hyperintensity. It does help distinguish between the two vascular causes: top of basilar artery occlusion and deep cerebral vein thrombosis.  相似文献

A group of 41 patients with intracranial meningiomas were examined by MR imaging (MRI) and MR angiography (MRA) to assess the. clinical value of MRA in the preoperative evaluation of these patients. The results of MRA were compared with the results of intraarterial cerebral catheter X-ray angiography (XRA; n = 19) and with the operative findings (n = 41 ): Our results showed a good correlation between MRA and XRA/surgery in demonstrating the relationship between the tumor and adjacent venous and arterial structures. Use of MRA was also helpful in demonstrating the degree of intrinsic tumor vascularity. It also supplied important information for operative planning Adjunct XRA was mandatory if detailed information about tumor-feeding vessels was requested by the neurosurgeon, especially in highly vascularized angiomatous meningiomas and in meningiomas suspected of tumor feeding by vessels of the internal carotid artery. BACKGROUND AND PURPOSE: Venous drainage patterns are a major determinant of clinical outcome in intracranial dural arteriovenous fistula (DAVF) patients. In this study, we sought to identify MR imaging finding differences between DAVF types classified on the basis of venous drainage patterns. METHODS: Twenty-seven patients diagnosed as having DAVFs by conventional angiography were included. Medical records (n = 27), and MR imaging (n = 27) and MR angiography (MRA; n = 11) findings were retrospectively reviewed. MR imaging findings included flow void cluster, engorged ophthalmic vein/proptosis, white matter hyperintensity, intracranial hemorrhage, dilated leptomeningeal or medullary vessels, venous pouch, and leptomeningeal or medullary vascular enhancements. MRA findings included identifiable fistula, venous flow-related enhancement, and prominent extracranial vessels. Patients' presentations and MR imaging findings were compared among angiographic type I, II, and III cases (according to Borden's classification), and MRA findings were compared between cases with and without retrograde leptomeningeal venous drainage (RLVD). RESULTS: Patient presentations were aggressive in one (13%) of the type I cases, 5 (50%) of the type II cases, and 8 (100%) of the type III cases (P = .002). Aggressive presentations included hemorrhage, focal neurologic deficits, seizures, intracranial hypertension, and an altered mental status. MR images showed significantly higher frequencies of dilated leptomeningeal or medullary vessels in a higher type [0 in type I, 5 (42%) in type II, and 7 (100%) in type III], and of leptomeningeal or medullary vascular enhancements [0 in type I, 4 (33%) in type II, and 7 (100%) in type III]. By using MRA, fistulas were identified only in cases with RLVD (5 [83%]). Venous flow-related enhancement was present in 10 cases (91%). A sole false-negative case on MRA, as compared with conventional angiography, resulted from nonvisualization of the slow venous flow (8%). No false-positive fistula was found at the other intracranial sites in all cases. Overall, MRA assessment for DAVF was adequate for both fistula and venous flow-related enhancement in 10 cases (91%) and inadequate in a remaining case because of the fistular location out of field. CONCLUSION: MR imaging demonstration of leptomeningeal or medullary vascular dilation and enhancements may be associated with features that are considered predictors of a poor outcome and indicates a need for urgent therapy in intracranial dural AVF patients. MRA is a complementary tool for the identification of dural AVF with venous flow-related enhancement.  相似文献

ObjectivesThe purpose of this study is to compare the various magnetic resonance imaging (MRI) sequences when they are used to visualize and evaluate cerebral venous thrombosis.MethodsEleven patients with cerebral venous thrombosis were retrospectively analyzed using computed tomography, MRI, magnetic resonance angiography (MRA), and conventional angiography. The MR sequence included T1-weighted spin echo (SE) imaging, obtained before and after administration of contrast medium, T2-weighted turbo spin echo (TSE), fluid-attenuated inversion recovery (FLAIR), T2*-weighted conventional gradient-echo (GRE), as well as three-dimensional (3D) venous time-of-flight MRA and conventional angiography.ResultsIn all of our patients, the venous sinus thromboses were most successfully detected during the T2*-weighted GRE sequence. The thrombosis was well visualized with the T1-weighted SE sequence in three of four patients in whom it was in the subacute stage. The T2*-weighted GRE sequence was superior to the T2-weighted TSE, T1-weighted SE, and FLAIR sequences in all patients. Enhanced 3D MR venography showed the thrombosed segment of the venous sinus and well correlated with the conventional angiographic findings.ConclusionsThe T2*-weighted conventional GRE sequences may be the best method for detecting of cerebral venous thrombosis. Therefore, it would seem to be beneficial to integrate a T2*-weighted conventional GRE sequence into the MR protocol to diagnose cerebral venous thrombosis.  相似文献

BACKGROUND AND PURPOSE: Digital subtraction angiography (DSA) is the method of reference for imaging of dural arteriovenous fistula (DAVF). The goal of this study was to analyze the value of different MR images including 3D contrast-enhanced MR angiography (MRA) with a high temporal resolution in diagnostic and follow-up imaging of DAVFs. MATERIALS AND METHODS: A total of 18 MR/MRA examinations from 14 patients with untreated (n=9) and/or treated (n=9) DAVFs were evaluated. Two observers assessed all MR and MRA investigations for signs indicating the presence of a DAVF, for fistula characteristics such as fistula grading, location of fistulous point, and fistula obliteration after treatment. All results were compared with DSA findings. RESULTS: On time-resolved 3D contrast-enhanced (TR 3D) MRA, the side and presence of all patent fistulas (n=13) were correctly indicated, and no false-positive findings were observed in occluded DAVFs (n=5). Grading of fistulas with this imaging technique was correct in 77% and 85% of patent fistulas for both readers, respectively. On T2-weighted images, signs indicative of a DAVF were encountered only in fistulas with cortical venous reflux (56%), whereas on 3D time-of-flight (TOF) MRA, most fistulas (88%) were correctly detected. In complete fistula occlusion, false-positive findings were encountered on both T2-weighted images and on TOF MRA images. CONCLUSION: In this study, TR 3D MRA proved reliable in detecting DAVFs and suitable for follow-up imaging. The technique allowed--within limitations--to grade DAVFs. Although 3D TOF MRA can depict signs of DAVFs, its value for follow-up imaging is limited.  相似文献

OBJECTIVE: The goal of this study was to evaluate three-dimensional gadolinium-enhanced MR angiography as a tool for examination of liver transplant patients with potential vascular complications. MATERIALS AND METHODS: Thirty-eight consecutive three-dimensional gadolinium-enhanced MR angiograms were obtained in 34 patients. Results were retrospectively reviewed and correlated with conventional angiography in 20 of the 38 cases and sonography in 37 of the 38 cases. MR angiograms were evaluated for technical adequacy, vascular patency, and parenchymal abnormalities, and results were compared with angiography and sonography. Conventional angiography and surgery were used as gold standards when available. RESULTS: Thirty-four (90%) of 38 MR angiograms were technically adequate. Vascular abnormalities were identified in 20 patients, and 19 of these patients subsequently underwent angiography, surgery, or both. There were seven cases of hepatic artery thrombosis; all were detected with MR angiography with no false-positive or false-negative interpretations. Seven patients had moderate to severe hepatic artery stenosis (>50% narrowing as determined by conventional angiography). MR angiography revealed this stenosis in six of the seven patients, with one false-negative and three false-positive interpretations. Portal vein thrombosis was detected in three patients, and portal vein stenosis was detected in two patients. CONCLUSION: Three-dimensional gadolinium-enhanced MR angiography is useful in the examination of liver transplant patients and offers a noninvasive adjunct in patients with difficult or indeterminate sonographic examinations.  相似文献

Forty-two MR examinations and hepatic panangiograms in 38 patients with portal hypertension were correlated with MR images to determine the ability of MR to detect portal vein hemodynamics. These studies were prospectively analyzed for degree of portal perfusion and direction of flow, portal vein thrombosis, and presence and type of shunt surgery. Thirty-three MR examinations were determined to have grade I (good) or II (fair) portal blood flow. Twenty-nine of these were grade I or II by angiography; the other four were grade IV. Of the eight cases documented as grade IV (hepatofugal portal blood flow) by angiography, none were considered grade IV by MR, suggesting that MR was unable to detect retrograde flow. The other case was not graded because of cavernous transformation of the portal vein. MR correlated well with angiography for the detection or absence of portal vein thrombus, agreeing with angiography in 41 of 42 cases. Two angiographically proven cases of portal vein thrombosis were correctly identified on MR. MR correctly identified the absence of portal vein clot in 39 of 40 angiographically negative cases. MR and angiography also agreed in 41 of 42 cases that a shunt was either present/absent or patent/occluded. The single error was due to inadequate MR scanning in the region of interest. The results show that MR cannot be used to grade blood flow in the portal vein. However, MR accurately detects portal vein thrombosis and the patency of surgical shunts.  相似文献

目的观察三维对比剂增强MR血管成像(3DCEMRA)上布加综合征(BCS)的各种表现,并初步评价该项新技术的价值。方法33例BCS患者行3DCEMRA检查。23例为继发性BCS,分别继发于肝细胞癌(21例)、右肾上腺癌(1例)或血栓性静脉炎(1例)。10例为原发性BCS。观察肝静脉、下腔静脉(IVC)和门静脉的开放性,观察有无肝内外侧支、肝实质病变和门静脉一体静脉间曲张静脉。10例患者行下腔静脉造影术,2例行肝右静脉穿刺造影术,把3DCEMRA所获的诊断结果与造影相对照。结果3DCEMRA可显示BCS的各种表现。肝静脉表现包括:癌栓形成(19例)、肿瘤压迫(2例)、肝静脉未显示(4例)和局限性狭窄(4例)。IVC表现为严重狭窄或闭塞(10例)、肿瘤直接侵犯(2例)、癌栓形成(3例)、血栓性静脉炎(1例)和隔膜形成(3例)。9例显示肝内侧支形成,其中2例显示“蜘蛛网”征象。所见的肝外侧支包括扩张的奇静脉和半奇静脉(13例),以及左肾一膈下一心包膈静脉侧支(2例)形成。2例患者发现门静脉左支闭塞,10例患者发生门静脉.体静脉间静脉曲张。3DCEMRA发现的肝实质病变有:尾叶增大(7例)、不均匀强化(18例)和并发肿瘤(18例)。12例3DCEMRA诊断结果均与造影结果一致。结论3DCEMRA能显示BCS的各种征象,并能帮助提供正确诊断。  相似文献

The purpose of this study was to determine whether MR angiography utilizing the time resolved echo-shared angiographic technique (TREAT) can provide an effective assessment of the hepatic artery (HA) and portal vein (PV) in living donor candidates. MR angiography (MRA)was performed in 27 patients (23 men and 4 women; mean age, 31 years) by using TREAT. Two blinded radiologists evaluated HA anatomy, origin of segment IV feeding artery and PV anatomy in consensus. Qualitative evaluations of MRA images were performed using the following criteria: (a) overall image quality, (b) presence of artifacts, and (c) degree of venous contamination of the arterial phase. Using intraoperative findings as a standard of reference, the accuracy for the HA anatomy, origin of segment IV feeding artery and PV anatomy on TREAT-MRA were 93% (25/27), 85% (23/27), and 96% (26/27), respectively. Overall image qualities were as follows: excellent (n=22, 81%), good (n=4, 15%), and fair (n=1, 4%). Significant artifacts or venous contamination of the arterial phase images was not noted in any patient. TREAT-MRA can provide a complete evaluation of HA and PV anatomy during preoperative evaluation of living liver donors. Furthermore, it provides a more detailed anatomy of the HA without venous contamination.  相似文献

目的探讨下肢深静脉血栓形成磁共振血管成像(MRA)和下肢深静脉血栓磁共振直接成像(MRDTI)的临床价值。资料与方法对25例临床确诊为下肢深静脉血栓形成的患者进行MRA和MRDTI检查.MRA采用二维时间飞越法(2DTOF);MRDTI选用SET1WI、FSET2WI、FAST(fourier acquired steady state,FAST)序列。其中23例行数字减影血管造影(DSA)检查,MRA与DSA进行对照分析。结果下肢深静脉血栓形成的MRA表现有:静脉充盈缺损(14例)、静脉闭塞和中断(8例)、静脉再通(3例)、侧支循环形成(25例)。以DSA为诊断标准,MRA诊断符合率为95.65%。下肢深静脉血栓形成MRDTI表现为:急性血栓(19例),血栓在SET1WI、FSET2WI和FAST序列上多为等信号或高信号,信号多不均匀,6例可见血管周围水肿;慢性血栓(6例),血栓在SET1WI、FSET2WI和FAST序列上多均为低或等信号。结论2DTOFMRA结合多种序列MRDTI的表现,诊断下肢深静脉血栓更准确,并可以为判断血栓新旧提供依据。  相似文献

The aim of the study was to apply time-of-flight MR angiography to renal transplant arteries with comparison of two- and three-dimensional (2D and 3D) sequences and to correlate the findings with colour flow sonography (CFS) and digital subtraction angiography (DSA). A total of 102 MR studies were performed in 101 patients: 87 with the 2D-FLASH sequence (18 repeated after Gd-DOTA administration), 49 with the 3D-FISP (both in 34). All patients were also studied with CFS and 15 with intra-arterial DSA. The 3D sequence produced good-quality MR angiograms in 94% of cases (82% in 2D). Gd-DOTA infusion improved the quality of the 2D angiograms in 7 of 18 cases. Only these patients were included in the remainder of the evaluation (90 patients with 103 arteries). CFS showed 72 normal and 10 abnormal arteries. In this group, the 2D sequence led to 7 (12%) false positives of stenosis and the 3D sequence yielded 1 (3%). Correlation between MR angiography and DSA was obtained for 21 arteries (15 patients) with suspicion of arterial complications. The 2D-FLASH (n = 13) and the 3D-FISP (n = 12) MR sequences allowed the correct diagnosis of all main artery complications (14 stenoses and 4 thromboses) without any false negatives and without discordance when both sequences were performed (n = 4). In the 3 other cases with a normal main artery, 2 segmental thromboses were correctly identified by both sequences and 1 stenosis of a segmental branch was correctly identified by the 2D sequence only but misdiagnosed as a thrombosis with the 3D sequence. Grading of the severity of stenoses was inaccurate with both sequences. It is concluded that the 3D time-of-flight MR sequence provides better MR angiograms than the 2D, with fewer false positives for stenosis. No false-negative arterial complications were noted. Objective The purpose of this study was to highlight the critical role that MRI may play in diagnosing unsuspected lower extremity deep venous thrombosis and to stress the importance of scrutinizing MRI studies of the lower extremity showing apparently non-specific muscle edema for any evidence of intramuscular venous thrombosis.Design and patients The imaging studies of four patients in whom deep venous thrombosis was unsuspected on clinical grounds, and first diagnosed on the basis of MRI findings, were reviewed by two musculoskeletal radiologists in consensus. In all four patients the initial clinical suspicion was within the scope of musculoskeletal injuries (gastrocnemius strain, n=3; ruptured Baker cyst, n=1), explaining the choice of MRI over ultrasound as the first diagnostic modality.Results All patients showed marked reactive edema in the surrounding soft tissues or muscles. Three patients showed MR evidence of branching rim-enhancing structures within intramuscular plexuses characteristic of venous thrombosis (gastrocnemius, n=1; sural, n=2); one patient showed a distended popliteal vein. Ultrasound was able to duplicate the MRI findings in three patients: one patient showed above-the-knee extension on ultrasound; neither of the two patients with intramuscular thrombosis demonstrated on ultrasound showed extension to the deep venous trunks.Conclusion Intramuscular venous thrombosis can present as marked edema-like muscle changes on MRI, simulating primary musculoskeletal conditions. In the absence of clinical suspicion for deep venous thrombosis, only the identification of rim-enhancing branching intramuscular tubular structures will allow the correct diagnosis to be made.  相似文献

MRI and phase-contrast MR angiography (PC MRA) were obtained in 13 patients with angiographically confirmed intracranial dural arteriovenous fistulae (DAVF). Three- and two-dimensional PC MRA was obtained with low (6–20 cm/s) and high (> 40 cm/s) velocity encoding along the three main body axes. MRI showed focal or diffuse signal abnormalities in the brain parenchyma in six patients, dilated cortical veins in seven, venous pouches in four with type IV DAVF and enlargement of the superior ophthalmic vein in three patients with DAVF of the cavernous sinus. However, it showed none of the fistula sites and did not allow reliable identification of feeding arteries. 3D PC MRA enabled identification of the fistula and enlarged feeding arteries in six cases each. Stenosis or occlusion of the dural sinuses was detected in six of eight cases on 3D PC MRA with low velocity encoding. In six patients with type II DAVF phase reconstruction of 2D PC MRA demonstrated flow reversal in the dural sinuses or superior ophthalmic vein. We evaluated the use of gradient-echo (GRE) as an adjunct to spin-echo (SE) MR imaging of the portal venous system. GRE imaging was performed in 31 subjects, 15 normal volunteers and 16 patients with documented portal venous disease (15 cases) or suspected disease (one case). Eight of 16 patients had venous thrombosis, five had focal thrombus, and three had complete occlusion. Six patients had extrinsic venous compression by tumor. Of the two other patients, one had an arteriovenous fistula and the other a falsely positive angiogram, suggesting portal vein occlusion. In normal subjects, GRE scans had excellent visualization of the portal venous system with high intravascular signal compared with surrounding tissues. Nine (60%) of 15 normal subjects and three patients had an artifact consisting of a curvilinear area of decreased signal that could mimic clot. In three of five patients with focal thrombus, clot was identified on GRE but not on SE images. In all three patients with occlusion, SE and GRE images demonstrated similar findings. In five of the six patients with extrinsic venous compression by tumor, SE and GRE studies showed similar findings. Of the two patients, an arteriovenous fistula was seen on GRE MR in one, and in the other, patency of the left portal vein was seen on SE and GRE images after angiography had suggested portal vein occlusion. Collateral vessels were seen in nine of 16 patients. In five of nine cases, GRE MR demonstrated more extensive collaterals than did SE MR. In summary, GRE MR provides a useful adjunct to standard SE MR imaging. Benefits include high contrast between vascular structures and surrounding tissues, reduced motion artifact, and rapid scanning within a breath-hold.  相似文献

目的:探讨单次屏气三维对比增强磁共振造影(3D DCE MRA)在原发性肝癌中的检查技术及临床应用。方法:回顾性分析23例原发性肝癌的3D DCE MRA表现。对原发性肝癌肝动脉期、门静脉期分别进行扫描和重建。结果:肝动脉期可显示肝癌供血动脉血管7例,动静脉瘘6例,肝动脉受挤压变形6例。门静脉期显示门静脉高压18例。门静脉癌栓13例,肝内门静脉受压移位3例。结论:有效地抓住对比剂在各靶血管中的有效浓度,可分别显示肝癌的血供情况、血管破坏情况、门静脉开放程度,为临床治疗提供有价值的影像参考。  相似文献

目的探讨腹部静脉病变的CE-MRA的表现及临床应用价值。方法回顾性分析47例腹部静脉病变的常规MRI和腹部静脉CE-MRA表现。结果门脉高压28例(肝硬化性22例,非肝硬化性6例),其中1例合并静脉瘤;静脉内瘤栓8例;肿瘤性静脉包绕4例;布-加氏综合征(Budd-chiarrisyndrome,BCS)2例,门静脉海绵状变性(非肿瘤性)2例,肝移植术后3例。CE-MRA清楚显示了门静脉、脾静脉、胃冠状静脉的增粗,食道胃底静脉的曲张;瘤栓所致的腔内低信号区;静脉被肿物推移受压的情况;门静脉、肝静脉、下腔静脉的狭窄、闭塞和侧枝循环的分布范围和程度。常规MRI显示了腹部脏器的原发病变。结论CE-MRA结合常规MRI检查对腹部静脉病变有一定的诊断价值,对临床治疗方案的选择有明确的指导意义。  相似文献
