Purpose: The purpose of this study was to perform a magnetic resonance imaging (MRI) follow-up study of bone bruises in a group of patients with acute anterior cruciate ligament (ACL) ruptures that were reconstructed and followed-up for a minimum of 2 years. Type of Study: Cohort study. Methods: The study group included 21 patients with a mean age of 31 years whose initial MRI scans showed associated bone bruises. Patients were included if they had an acute isolated ACL tear, no documentation of an episode of repeated injury to the affected knee during the follow-up period, and no evidence of cartilaginous injury at the time of arthroscopy. All patients had preoperative MRI scans and underwent arthroscopic ACL reconstruction using a bone–patellar tendon autograft an average of 2 months after injury. The preoperative MRI scans were analyzed using a 3-level grading system based on the appearance and location of bone bruises. A second MRI of the knee was obtained from 24 to 64 months postoperatively (average 34 months). The presence of resolution of bone bruises was determined and correlation with clinical scoring established. Results: This study showed resolution of all type I lesions and 91% of type II lesions (10 of 11). In all type III lesions, an articular cartilage thinning and depression was observed after 2 years of follow-up. In 15 patients (71%), MRI showed that the bone bruises had resolved without apparent sequelae. In the remaining 6 patients (29%), sequelae of the osteochondral lesion were evident on MRI. Conclusions: According to our clinical data, there was no correlation between scores obtained from patients with resolved lesions against those with osteochondral sequelae. Although long-term clinical implications of these findings are uncertain, a severe occult osteochondral lesion sustained at the time of ACL rupture seems to be persistent on MRI even after a successful reconstruction.Arthroscopy: The Journal of Arthroscopic and Related Surgery, Vol 17, No 5 (May-June), 2001: pp 445–449 相似文献

Background
It currently remains unclear whether the meniscal repair clinical results were affected by the graft used in anterior cruciate ligament (ACL) reconstruction. This retrospective study designed to evaluate the difference in clinical outcomes of meniscal repair using autograft and allograft for the ACL reconstruction.Methods
The injury of the ACL and meniscus was evaluated with MRI and treated simultaneously. One hundred and eighty-nine cases were initially fulfilled the study criteria, and had the surgery in the period June 2007 and July 2010. Thirty-four patients were lost to follow-up. Seventy-five patients underwent meniscus repair with autograft reconstruction of the ACL (autograft group) and 80 patients underwent meniscus repair with allograft reconstruction of the ACL (allograft group).Results
The meniscus healing rate based on the clinical examination of Barrett’s criteria was 81.3 % (61/75) in the autograft group and 80.0 % (64/80) in the allograft (P > 0.05). There was no significant difference in the Lysholm scores in the allograft group compared to the allograft group (89.1 ± 10.6 versus 88.7 ± 11.2, P > 0.05). The values of immunoglobulin’s and complements (IgG, IgA, IgM, C3 and C4) were not significantly different between the two groups (P > 0.05).Conclusion
The data support our assumption that patients undergoing meniscal repair associated with ACL reconstruction with allograft had good clinical outcomes. Although allograft implantation induces an immunological response on a subclinical level, there were no signs of allograft affecting the nature of meniscus healing. 相似文献4.
目的探讨急性前十字韧带损伤患者MRI上韧带撕裂位置对矢状面上骨挫伤最大面积的影响。方法对2019年1至6月收治的前十字韧带损伤患者的MRI进行回顾性分析,根据前十字韧带撕裂在其远端到近端全长上的位置分为五型:Ⅰ型,>90%;Ⅱ型,75%~90%;Ⅲ型,25%~75%;Ⅳ型,10%~25%;Ⅴ型,<10%。比较五型患者的性别、体重、体质指数、股骨外侧髁切迹凹陷深度、内外侧半月板有无损伤。选取ePDWSPIRCLEAR序列矢状面图像,分别截取股骨、胫骨骨挫伤面积最大的层面。使用Image J 1.52t软件测量挫伤面积最大层面的最大骨挫伤面积。结果63例急性前十字韧带损伤中,Ⅰ型撕裂3例(4.8%)、Ⅱ型撕裂14例(22.2%)、Ⅲ型撕裂40例(63.5%)、Ⅳ型撕裂2例(3.2%)、Ⅴ型撕裂4例(6.3%)。其中Ⅰ型和Ⅳ型未发现股骨外侧髁及外侧胫骨平台骨挫伤。股骨外侧髁骨挫伤面积Ⅱ型为0(0,64.12)mm^2、Ⅲ型为182.34(86.58,334.38)mm^2、Ⅴ型为38.64(0,193.36)mm^2,差异有统计学意义(H=21.665,P=0.000);外侧胫骨平台骨挫伤最大面积Ⅱ型为76.78(28.25,205.57)mm^2、Ⅲ型为120.93(51.78,239.37)mm^2、Ⅴ型为190.51(80.86,238.75)mm^2,差异有统计学意义(H=11.939,P=0.018)。内侧半月板损伤发生率分别为66.6%(2/3)、35.7%(5/14)、37.5%(15/40)、100%(2/2)、25%(1/4),差异无统计学意义(χ^2=4.413,P=0.353);外侧半月板损伤发生率分别为33.3%(1/3)、35.7%(5/14)、77.5%(31/40)、50%(1/2)、25%(1/4),差异有统计学意义(χ^2=11.481,P=0.022)。内侧半月板损伤组外侧胫骨平台最大骨挫伤面积为48.0(0,105.97)mm^2,小于未损伤组的185.67(54.36,257.41)mm^2,差异有统计学意义(H=8.848,P=0.003);外侧半月板损伤组股骨外侧髁最大骨挫伤面积为162.19(63.03,301.33)mm^2,大于未损伤组的0(0,103.37)mm^2,差异有统计学意义(H=11.554,P=0.001)。结论前十字韧带断裂最常发生在中段,合并外侧半月板损伤的概率最大,股骨外侧髁骨挫伤面积大;发生在最远端的断裂导致外侧胫骨平台骨挫伤的面积最大,合并内侧半月板损伤的概率最低。 相似文献

Derotation brace and knee function in patients with anterior cruciate ligament tears 总被引:1,自引:0,他引:1
In this article we have tested a ready-made brace to evaluate whether it could improve function in the anterior cruciate ligament injured patient. All patients had their full extent of injury mapped out at arthroscopy. We believe that arthroscopy before treatment for a torn anterior cruciate ligament injury is necessary in order to treat meniscus lesions that could interfere with the rehabilitation. In this study, 26 patients were unstable with a positive pivot-shift, and 16 patients were operated on and thus stable. All patients performed a performance test with and without the brace in a randomized order. Before the start of the investigation, we postulated that patients with a quadriceps atrophy who were unstable would improve their function with the derotation brace. We could not find any effects of the brace in this study. The brace did not significantly improve function in the group of patients who were unstable with a quadriceps atrophy. On the other hand, the brace did not impair function in the patients. We conclude that to be able to improve knee function in the unstable knee, other modes of treatment than bracing are necessary. Bracing should be seen as a complement to these other treatments. 相似文献

Accuracy of nonorthogonal magnetic resonance imaging in acute disruption of the anterior cruciate ligament 总被引:2,自引:0,他引:2
A controlled, prospective study of 30 patients with suspected acute internal derangement of the knee was undertaken to evaluate the sensitivity, specificity, predictive value, and accuracy of nonorthogonal (oblique) sagittal magnetic resonance imaging (MRI) in the assessment of anterior cruciate ligament (ACL) injuries. Thirty patients with acute hemarthroses underwent MRI within 12 days of injury, followed by arthroscopy within 24 h of the MRI. A control population of 30 chondromalacia patients underwent similar evaluation. In the acute hemarthrosis patient population, the incidence at arthroscopy of acute complete ACL tears was 60% (18/30); of partial ACL tears, 13.3% (4/30); and of chronic tears, 10% (3/30). A normal ACL was found in 16.6% (5/30) of patients. In the MRI evaluation of patients with suspected ACL injury the following results were obtained for both acute and chronic complete disruption on orthogonal (sagittal) and nonorthogonal (oblique sagittal) imaging, respectively: sensitivity, 61 (16/26) versus 100%; specificity, 70 (21/34) versus 100%; positive predictive value, 61 (16/26) versus 100%; negative predictive value 70 (24/34) versus 100%; and accuracy, 66 (40/60) versus 100%. In the evaluation of partial ACL injury, four partial tears were correctly diagnosed on nonorthogonal MRI, with one false-positive diagnosis. Orthogonal imaging failed to correctly identify any of the partial ACL injuries. Two patients in the control population demonstrated evidence of chronic ACL tears. We believe that we have demonstrated the superiority of T2-weighted nonorthogonal sagittal over conventional orthogonal sagittal ACL MRI in the evaluation of ACL injury. 相似文献

Purpose: This study was conducted to compare the obliquity of asymptomatic anterior cruciate ligament (ACL) grafts with normal controls using sagittal magnetic resonance imaging (MRI). Type of Study: Case control study. Methods: Sagittal MRIs from 30 patients with a reconstructed ACL graft and from 30 individuals with an intact ACL were reviewed. Reconstructed patients were operated on with a 2-incision technique using a patellar tendon autograft. These selected patients had a normal or nearly normal IKDC score with a 3 mm or less anterior posterior translation on KT-1000 arthrometer testing compared with the intact knee. MRI showed a continuous and homogeneous graft without evidence of roof impingement. Obliquity of the grafted ACL was determined on each lateral MRI by measuring the intersection of the graft line with the tibial plateau plane. These figures were compared with data similarly obtained from 30 individuals with a stable knee and an intact ACL determined by history and physical examination. Results: Graft obliquity in reconstructed patients averaged 67° with a range between 55° and 81°. In normal controls, intact ACL obliquity averaged 51° with a range between 45° and 55°. The difference between the two groups was statistically significant (P <.0001). Conclusions: MRIs of patients with an appropriate tibial tunnel placement in order to avoid notch impingement showed a continuous and homogeneous graft similar to the native ACL, but with a more vertical graft that does not recreate the normal sagittal obliquity. Because SBM may contribute to cartilage breakdown in OA, experimental OA was induced in dogs by transecting the anterior cruciate ligament of the knee and treating with either CT or a placebo. CT significantly reduced both SBM and cartilage lesions. This study supports the use of CT in the treatment of canine experimental OA. INTRODUCTION: Because subchondral bone remodeling (SBM) may contribute to cartilage breakdown in osteoarthritis (OA), we evaluated to what extend calcitonin (CT) might affect cartilage and bone changes in the early stages of canine experimental OA. MATERIALS AND METHODS: Twelve dogs underwent transection of the anterior cruciate ligament (ACLT) of the right knee. After ACLT, each animal received a daily nasal spray delivering either 400 U of CT (CT-treated group; n = 6) or a placebo (PL-treated group; n = 6). At day 84 after surgery, animals were killed, and cartilage changes were graded. BMD and volume fraction (BVF) were assessed by pQCT in different regions of interest (ROIs) of the subchondral cancellous bone of tibial plateaus (TPs).Statistics included a 2 x 2 factorial analysis with +/-CT as one factor and +/-ACLT as the other. RESULTS AND CONCLUSIONS: Nonoperated (N-OP) knees were normal in both groups. In the PL-treated group, ACLT knees all exhibited OA changes, which predominated in the medial knee compartment. Furthermore, compared with N-OP knees, the BMD and BVF of ACLT joints were both markedly reduced in medial TP but not in lateral TP. In contrast, in the CT-treated group, cartilage OA lesions of ACLT knees were significantly reduced, and there was no difference in BMD and BFV between N-OP and ACLT knees. These findings suggest that the loss of subchondral trabeculae contributes to cartilage breakdown, possibly by enhancing cartilage deformation on joint loading. By counteracting bone loss, CT reduced cartilage OA lesions, and thus, might be useful in the treatment of OA in cruciate-deficient dogs. 相似文献

The management of patients with combined medial collateral (MCL) and anterior cruciate (ACL) rupture remains controversial.
We studied 25 such patients who elected to have the ACL lesion treated conservatively; 14 underwent MCL repair with early
mobilization and 11 were treated with immobilization for two weeks. The mean follow up was 5.9 years (2 to 11). There was
no difference in the clinical assessment of ligamentous laxity, KT-1000 measurements or Tegner activity scores between the
two groups but there were significantly higher Lysholm function scores in the operated group.

Résumé Le traitement optimal des cas associant des lésions du ligament croisé antérieur (ACL) à des lésions médiales collatérales (MCL) est controversé. Nous avons réalisé une étude sur le suivi du traitement de telles lésions dans le but d’évaluer si le traitement opératoire des déchirures de MCL peut apporter une amélioration des résultats fonctionnels pour des patients choisissant de ne pas subir une reconstruction de ACL. Notre étude portait sur 14 patients ayant subi une reconstruction du MCL et 11 patients traités sans opération. Nous n’avons pas observé de différences significatives entre les deux groupes pour les tests de relachement manuel, ni pour les mesures KT-1000, ni pour les niveaux d’activité de Tegner. En revanche les patients du groupe opératoire ont obtenu un meilleur score fonctionnel de Lysholm.

A study was undertaken to determine whether a significantly different clinical outcome could be expected following nonoperative treatment of acute partial anterior cruciate ligament (ACL) tears from that of complete tears. A detailed follow-up of 107 patients with arthroscopically confirmed tears was obtained; 72 were complete tears and 35 partial. The overall results in those with partial tears were 23% excellent, 29% good, 17% fair, and 31% poor; with complete tears the results were 11% excellent, 20% good, 15% fair, and 54% poor. The patients with partial tears had a lower incidence of associated meniscal tears, needed fewer reconstructions and more of them returned to sport than those with complete tears. 相似文献

We investigated the in vivo cartilage contact biomechanics of the tibiofemoral joint in patients after reconstruction of a ruptured anterior cruciate ligament (ACL). A dual fluoroscopic and MR imaging technique was used to investigate the cartilage contact biomechanics of the tibiofemoral joint during in vivo weight‐bearing flexion of the knee in eight patients 6 months following clinically successful reconstruction of an acute isolated ACL rupture. The location of tibiofemoral cartilage contact, size of the contact area, cartilage thickness at the contact area, and magnitude of the cartilage contact deformation of the ACL‐reconstructed knees were compared with those previously measured in intact (contralateral) knees and ACL‐deficient knees of the same subjects. Contact biomechanics of the tibiofemoral cartilage after ACL reconstruction were similar to those measured in intact knees. However, at lower flexion, the abnormal posterior and lateral shift of cartilage contact location to smaller regions of thinner tibial cartilage that has been described in ACL‐deficient knees persisted in ACL‐reconstructed knees, resulting in an increase of the magnitude of cartilage contact deformation at those flexion angles. Reconstruction of the ACL restored some of the in vivo cartilage contact biomechanics of the tibiofemoral joint to normal. Clinically, recovering anterior knee stability might be insufficient to prevent post‐operative cartilage degeneration due to lack of restoration of in vivo cartilage contact biomechanics. © 2012 Orthopaedic Research Society. A prospective and retrospective study was undertaken to compare the accuracy of magnetic resonance imaging (MRI) with clinical examination in diagnosing meniscal and anterior cruciate ligament (ACL) tears. Pathological findings were then confirmed during arthroscopy. One hundred fifty-four patients clinically diagnosed with a meniscal or ACL tear who ultimately had arthroscopic knee surgery were evaluated; 100 patients underwent clinical examination followed by MRI, and 54 underwent clinical examination alone. The presence or absence of meniscal and ACL tears was confirmed during arthroscopy. The accuracies of clinical examination and MRI were compared for the 100 patients who underwent both clinical examination and MRI. The accuracy of MRI was 75% for medial meniscal tears, 69% for lateral meniscal tears and 98% for ACL tears. The accuracy of clinical examination was 82% for medial meniscal tears, 76% for lateral meniscal tears and 99% for complete tears of the ACL. Furthermore, the accuracy of clinical examination for the 54 patients who underwent clinical examination alone was not significantly different from the accuracy of clinical examination in the 100 patients who also underwent MRI. There was no significant difference between the accuracy of clinical examination and MRI in the diagnosis of meniscal and ACL tears and, overall, MRI contributed to treatment in only 16 of 100 cases. Based on these findings, we feel that MRI, except in certain circumstances, is an expensive and unnecessary diagnostic test in patients with suspected meniscal and ACL pathology. 相似文献

ObjectiveThe purpose of this study was to evaluate the visibility of the anterolateral ligament (ALL) by magnetic resonance imaging (MRI) in patients with chronic anterior cruciate ligament (ACL) rupture.Materials and methodsThis retrospective case – control study compared 1.5 - T MRI scans for 50 patients with a chronic ACL rupture with those of a control group of 50 patients with an intact ACL. The ALL was evaluated in three portions: femoral, meniscal, and tibial. The status of each portion was classified as visualized or non-visualized. Two radiologists separately reviewed all the MRI scans to evaluate interobserver reliability.ResultsAt least one portion of the ALL was visualized in 100% of the control group and 72% of the chronic ACL rupture group. All three portions of the ALL were identified in 72% of the control group but only 10% of the chronic ACL rupture group. In both groups, the most commonly visualized portion was the meniscal portion and the least visualized was the tibial portion. In 18% of the chronic ACL rupture group, no portion of the ALL was visualized.ConclusionsThe visibility of the ALL of the knee was significantly lower in patients with a chronic ACL rupture than in those with an intact one. 相似文献

We examined the menisci in 47 patients at the time of anterior cruciate ligament reconstruction. Twenty-one patients had abnormal menisci at a mean of 34 months after injury, but there was no difference between the Lysholm scores of patients with intact or damaged menisci. Eleven patients had a new meniscal injury between reconstruction and review at a mean of six years later; only 15 patients had both menisci intact nine years after injury. If pivot shift had been cured, the incidence of meniscal injury was reduced, but remained higher than normal. If pivot shift returned after reconstruction there was a significantly higher incidence of meniscal injury. Meniscal lesions appear to be the result of instability and not the cause. 相似文献

Patients with displaced bucket-handle (DBH) meniscal tears in anterior cruciate ligament (ACL)-deficient knees are prone to flexion contracture following meniscal repair and simultaneous ACL reconstruction. It has been suggested that ACL reconstruction be delayed until full range of motion has returned after the meniscal repair. A retrospective analysis was performed comparing the return of extension in patients undergoing simultaneous ACL reconstruction and repair of DBH tears (group A) versus a control group of patients with non-DBH tears (group B). Age, sex, body mass index, duration of time from injury to surgery, and preoperative extension were also compared between groups and evaluated for their significance as risk factors. Patients in group A achieved recovery to -5 degrees and 0 degrees of extension 22% and 35% more slowly, respectively, when compared with group B. These differences were not statistically significant. Female patients tended to heal more rapidly in both groups. We conclude that a one-stage procedure is sufficient in allowing patients with DBH tears in ACL-deficient knees to regain a functional knee to within 5 degrees of full extension. 相似文献

We investigated the degradation and tissue distribution of cartilage oligomeric matrix protein in normal, osteoarthritic, and rheumatoid arthritic articular cartilage of the human knee. Cartilage was subjected to sequential extractions with buffers containing neutral salt, with EDTA, and finally with guanidine/HCl and then was analyzed by Western blotting with a polyclonal antiserum to human cartilage oligomeric matrix protein. Western blots of the nine neutral salt extracts from normal cartilage revealed mostly intact pentameric molecules of cartilage oligomeric matrix protein, in contrast to the 13 osteoarthritic and five rheumatoid arthritic cartilage samples that demonstrated marked degradation of cartilage oligomeric matrix protein as noted by a predominance of reduction-sensitive bands at approximately 150 kDa and nonreduction-sensitive bands in the 67–94 kDa range. The EDTA and guanidine/HCl extracts from all groups were similar and showed mostly intact molecules of cartilage oligomeric matrix protein, with smaller amounts of degraded cartilage oligomeric matrix protein identical to those resolved by the Western blots of the neutral salt extracts. Western blots of matched pairs of synovial fluid and cartilage extracts demonstrated cartilage oligomeric matrix protein fragments of the same molecular mass. Competitive enzyme-linked immunosorbent assay revealed significantly less cartilage oligomeric matrix protein in rheumatoid articular cartilage than in either normal or osteoarthritic cartilage. In contrast to normal cartilage, where cartilage oligomeric matrix protein was predominately localized to the interterritorial matrix throughout all zones of the matrix, with increased staining in the deeper cartilaginous zones, the most intense staining in osteoarthritic cartilage was in the superficial zones of fibrillated cartilage, with little to no immunostaining in the midzones and relatively poor staining in the deeper cartilaginous zones. This distribution was the inverse of that for proteoglycans, as demonstrated by toluidine blue staining, where proteoglycans were depleted primarily from the superficial fibrillated cartilage. In mild to moderately affected rheumatoid cartilage, the tissue distribution of cartilage oligomeric matrix protein was similar to the distribution of proteoglycans, with relatively uniform staining of the interterritorial and territorial matrices. In more severely affected rheumatoid cartilage, the superficial zones demonstrated punctate immunostaining for cartilage oligomeric matrix protein in the interterritorial and territorial matrices, and staining was restricted to the territorial matrix in the deep cartilaginous zones. It is evident from this study that (a) noncollagenous proteins such as cartilage oligomeric matrix protein are greatly affected in arthritis. (b) degradation fragments released from the matrix into the synovial fluid reflect the processes occurring within the matrix, and (c) different zones of the articular cartilage are susceptible to degradation of cartilage oligomeric matrix protein in the different disease processes. 相似文献
