A combined amalgam and composite resin splint was constructed for a 35-year-old man with pathologic tooth mobility. This type of stabilization was selected because of the poor prognosis, economic considerations, and poor alignment of the teeth. Amalgam restorations were inserted with strong contacts. Composite resin material was used to fill the channel cut through all amalgam restorations and reinforced with braided stainless steel wire. The poor alignment of the teeth would have caused other types of splints to fail. 相似文献

Two techniques for the fabrication of pin-retained composite resin cores for posterior teeth have been described.The advantage of using a composite resin in place of amalgam as a core material is that the restoration can be completed in a single appointment rather than the two appointments required when amalgam is used.A disadvantage of the use of the composite resins as core material is the difficulty in identifying the demarcation line between the resin and tooth structure. This lack of definition presents the possibility of placing the finish line of the final preparation on the resin rather than on sound tooth structure. Application of a Dycal demarcation bead or use of the ultraviolet light-activated resin offers a solution to this problem. 相似文献

In selected cases in which appearance is important, a posterior composite resin may be used successfully to reinforce and restore a tooth with undermined, nonfunctional cusps. The restoration of teeth with compromised functional cusps is not recommended. The root canal therapy does not make sense if it is not going to be restored properly. Even though we must keep in mind all the steps necessary to build up any vital tooth, we should emphasize the retention and resistency due to the fragility of endodontically treated teeth and to the destruction of these teeth. Even though the use o silver amalgam has been often questioned, we still consider amalgam as a proper material of a good half worm life to build up endodontically treated posterior teeth. Resistency and retention were considered as the essential factors to achieve the integrity of final restorations. We have analyzed the remaining tooth structure, periodontal support, biomechanics properties of the most used restorative materials and the variations of the forces. Eighty class I and class II light-cured posterior composite resin restorations were compared with 43 class I and class II amalgam restorations during a 5-year period after placement. The results of this clinical study showed that both materials were satisfactory during the time period and that the only significant statistical differences are a poorer marginal integrity for the amalgam and a greater wear rate for the composite resin. 相似文献

This study evaluated the clinical performance of a visible light-cured small particle bimodally filled hybrid condensable composite resin system that included a dentin bonding agent compared with an amalgam alloy in class II restorations of permanent teeth. A total of 108 restorations were placed in 34 patients. Fifty-three composite resin and 55 amalgam restorations were inserted. Each restoration was evaluated immediately after placement and then on an annual basis for a 3-year period using the Public Health Service (PHS) criteria. In addition, the Moffa-Lugassy scale was used to measure the loss of material on the occlusal surface of these materials. One hundred percent of the resin and amalgam restorations were evaluated, measured, and reasons for replacement were recorded over the 3-year period. There was no significant difference (p greater than 0.05) in the clinical performance of the composite resin and the amalgam when evaluated by the PHS criteria. Analyses of wear at each of the three annual recall periods did not reveal any significant difference (p greater than 0.05) between the two restorative materials when measured by the Moffa-Lugassy scale. 相似文献

The fixed replacement of a single posterior missing tooth for those patients who cannot have either implants or conventional laboratory-generated fixed prostheses has always been a challenge to clinicians. The development of fiber-reinforced composites (FRCs) has opened up new possibilities of chairside treatment options to manage these types of clinical situations. Techniques using FRCs as frameworks with traditional restorative resin composites as veneering materials can provide chairside fixed prostheses that are esthetic and potentially durable, with minimal abutment tooth loss.
A clinical case is presented that will illustrate a novel concept for tooth replacement–the use of an FRC prefabricated framework–to allow for a single visit, chairside replacement of a missing molar. The assembly of the framework and the clinical steps used in the framework placement and pontic fabrication are shown in detail. 相似文献

Tooth loss, alterations on tooth structure, and reduced vertical dimension are known to severely compromise the stomatognathic system. This case report describes the treatment of a patient who presented with an extremely worn maxillary anterior dentition with a loss of posterior support owing to the loss of almost all the posterior teeth, except the mandibular premolars. Provisional removable partial dentures were used to create an optimum maxillomandibular relationship and to provide restorative space prior to the restoration of the remaining teeth. This restoration was accomplished with a combination of layered hybrid and microfilled composite materials, which restored the maxillary anterior teeth to optimum esthetics and function. 相似文献
