目的:探讨银离子藻酸盐敷料在不同类型手术切口中的应用效果.方法:选取本院进行腹部手术病例321例,术后切口使用普通无菌医用敷料的患者160例(对照组),其中Ⅰ类切口(清洁伤口)35例,Ⅱ类切口(相对清洁伤口)75例,Ⅲ类切口(清洁-污染伤口)50例.术后切口部位用银离子藻酸盐湿性敷料的患者161例(观察组),其中Ⅰ类切口32例,Ⅱ类切口76例,Ⅲ类切口53例.比较两组各类切口感染情况和切口愈合时间,视觉模拟评分法(VAS评分)观察换药时疼痛情况.结果:Ⅰ类切口两组感染率均为0,组间无差异(P>0.05);Ⅱ类、Ⅲ类切口对照组感染率明显高于观察组(P<0.05);Ⅰ类、Ⅱ类、Ⅲ类切口观察组VAS评分均低于对照组(P<0.05);Ⅰ类切口两组愈合时间差异不显著(P>0.05):Ⅱ类、Ⅲ类切口对照组愈合时间明显长于观察组(P<0.05).结论:腹部手术切口使用银离子藻酸盐湿性敷料效果显著,特别是在Ⅱ类、Ⅲ类切口中使用,不仅减轻疼痛,还能降低切口感染率,缩短切口愈合时间.  相似文献

目的观察美洲大蠊提取液联合藻酸盐敷料对肛瘘术后创面愈合的效果。方法将符合标准的96例肛瘘术后患者随机分为3组,每组32例。治疗组采用美洲大蠊提取液和藻酸盐敷料换药,对照1组采用美洲大蠊提取液纱条换药,对照2组采用九华膏纱条换药。观察:(1)术后第7天、第14天和第21天创面的渗液和疼痛情况。(2)术后第2天、第7天和第21天的创面面积。(3)创面的愈合时间。结果术后第2天3组患者创面面积差异无统计学意义(P0.05)。治疗组术后第7天、第21天创面面积及术后第7天、第14天和21天创面分泌物均少于两个对照组,疼痛程度较对照组轻,差异有统计学意义(P0.05)。治疗组创面平均愈合时间为(27.46±4.14)d,均优于对照1组的(30.46±3.59)d和对照2组的(31.46±3.81)d,差异有统计学意义(P0.05)。结论美洲大蠊提取液联合藻酸盐敷料用于肛瘘术后创面换药,可减少创面渗液、减轻创面疼痛和缩短创面愈合时间。  相似文献

目的观察藻酸盐敷料结合微波辐射促进肛瘘术后创面愈合的临床疗效。方法将56例肛瘘术后患者随机分为治疗组和对照组各28例;治疗组采用藻酸盐敷料结合微波辐射,对照组用凡士林油纱条换药。观察两组肛瘘术后创面疼痛程度、渗液量及创面愈合时间。结果治疗组较对照组术后创面疼痛轻、渗液量少、愈合快(P〈0.05)。结论藻酸盐敷料结合微波辐射能减轻肛瘘术后创面疼痛,减少渗液量,促进愈合。  相似文献

目的 探讨银离子敷料治疗皮肤深Ⅱ度烧烫伤创面的有效性及安全性。方法 自2019年10月至2020年6月,南宁市第二人民医院烧伤整形外科对132例皮肤深Ⅱ度烧烫伤创面患者进行治疗,根据治疗方法分为治疗组(65例)和对照组(67例),治疗组采用银离子敷料,对照组采用含银敷料Atrauman Ag。比较两组患者的总有效率、创面愈合率、创面愈合时间、创面分泌物细菌培养转阴率,并检测患者治疗前后的生命体征、实验室检查、心电图、综合评价及血银、尿银含量等,并记录治疗期间不良反应事件。结果 基于全分析集(full analysis set,FAS)和符合方案集(per protocol set,PPS),结果均显示治疗组的总有效率高于对照组(P<0.05)。两组的创面愈合率、创面愈合时间,差异均无统计学意义(P>0.05)。两组治疗前后创面分泌物细菌培养转阴率比较,差异无统计学意义(P>0.05)。患者在治疗前后的生命体征、实验室检查、心电图、综合评价及两组间血银、尿银浓度比较,差异均无统计学意义(P>0.05)。两组均未发生严重不良反应事件。结论 银离子敷料的有效性不劣于含...  相似文献

目的观察碳纤维敷料对烧伤创面的治疗作用。方法选择浅Ⅱ°、深Ⅱ°和残余创面共277例,随机分为对照组(碘伏纱布换药)和治疗组(碳纤维敷料换药)。观察敷料吸收容量、创面炎症反应,并做创面细菌定量和创面组织学检查,记录创面愈合时间等。结果与对照组相比,治疗组敷料吸收容量大,创面炎症反应轻,创周炎症消退快,痂下组织细菌定量低。浅Ⅱ°、深Ⅱ°及残余创面愈合时间分别为(7.4±2.1)d、(16.2±2.6)d、(19.4±6.2)d,均较对照组缩短,对照组分别为(9.6±2.4)d、(19.6±3.4)d和(28.8±10.4)d。结论碳纤维敷料是一种具有吸附能力强、可有效控制炎症反应、促进创面愈合等特点的新型敷料。  相似文献

目的 观察浸浴结合银离子敷料治疗大面积烧伤患者残余创面的疗效. 方法 2008年1月-2012年2月笔者单位收治90例大面积烧伤后期残余创面患者,按照随机数字表法分为对照组45例和试验组45例,对照组患者浸浴后创面外用磺胺嘧啶银纱布处理,试验组患者浸浴后以银离子敷料覆盖创面.用药后21 d,评定2组患者的治愈率.观察治疗过程中患者的不良反应.用药前及用药后7、14、21 d,分离与鉴定细菌,并计算创面细菌清除率.对数据进行x2检验和t检验.结果 用药后21 d,对照组与试验组患者的创面治愈率分别为62.22%(28/45)、86.67%(39/45),组间比较差异有统计学意义(x 2=7.067,P<0.05).用药后2组患者均未见明显不良反应.用药前及用药后7、14 d,2组患者均检出金黄色葡萄球菌、表皮葡萄球菌、铜绿假单胞菌、大肠埃希菌等.试验组患者用药后7、14 d细菌清除率分别为50.0%、71.4%,明显高于对照组(x2值分别为8.067、5.896,P<0.05或P<0.01).用药后21 d,2组患者细菌清除率均为100.0%. 结论 浸浴疗法结合银离子敷料治疗烧伤后残余创面能有效控制创面感染,显著提高治愈率.  相似文献

的 探究负压封闭引流术联合银离子敷料在难愈性烧伤创面治疗中的应用效果。方法 选择2021年8月-2023年2月京山市人民医院收治的80例难愈性烧伤患者为研究对象,以随机数字表法分为对照组和试验组,每组40例。对照组接受负压封闭引流术(VSD)治疗,试验组接受负压封闭引流术联合银离子敷料治疗,比较两组创面恢复情况、临床指标、疼痛程度、创面分泌物细菌检出率及并发症发生率。结果 试验组创面红期时间、创面愈合时间及敷料维持时间均优于对照组(P<0.05);试验组换药次数、住院时间及治疗总费用均优于对照组(P<0.05);试验组治疗后VAS评分低于对照组(P<0.05);两组治疗后创面分泌物细菌检出率均低于治疗前,且试验组低于对照组(P<0.05);试验组并发症发生率为2.50%,低于对照组的15.00%(P<0.05)。结论 采用负压封闭引流术联合银离子敷料治疗难愈性烧伤创面的效果良好,可有效促进创面愈合,提高敷料使用率,减轻疼痛,同时可降低创面分泌物细菌检出率和并发症发生几率,有利于促进患者快速康复出院,减少治疗费用,减轻患者的经济负担。  相似文献

目的:探讨水凝胶敷料(斯茯)联合复方紫草油在治疗Ⅱ度烧伤创面中的作用。方法:选择2013年2月~2014年2月在我院接受治疗的Ⅱ度烧伤患者共80例,随机分为治疗组和对照组,分别统计并比较创面愈合时间、疼痛程度、治疗费用、3个月瘢痕生长、不同时期基底分泌物细菌培养阳性率。结果:水凝胶敷料联合复方紫草油治疗组创面愈合时间缩短、疼痛程度明显降低、治疗费用减少、3个月瘢痕生长降低、细菌培养阳性率降低(P0.05)。结论:应用水凝胶敷料可有效缩短创面愈合时间,降低患者换药疼痛程度、创面感染率及3个月瘢痕生长率。  相似文献

纳米银敷料在修复Ⅱ度烧伤创面的应用研究   总被引:37,自引:1,他引:37  
目的观察纳米银敷料在Ⅱ度烧伤创面的防治感染作用及对创面愈合时间的影响。方法将Ⅱ度(深、浅)烧伤创面患者随机分为纳米银敷料组(A组,65例),1%磺胺嘧啶银霜组(B组,63例)和凡士林油纱组(C组,63例)。创面敷料或用药每天更换1次,使用前后进行创面细菌培养,观察创面愈合时间并进行统计学比较。结果在防治创面细菌定植方面,治疗后创面细菌培养阳性率A组(0%)与B组(1·6%)相似,均较治疗前下降,而C组治疗后细菌培养阳性率(14·3%)较治疗前(4·8%)明显增加。A组浅Ⅱ度创面愈合时间为(9·6±1·6)d,与B、C两组比较均明显提前(P<0·01);A组深Ⅱ度创面愈合时间为(19·1±2·6)d,与C组比较差异具有统计学意义(P<0·01),与B组比较差异无统计学意义(P>0·05)。结论Ⅱ度烧伤创面应用纳米银敷料,可以降低患者创面感染的风险,缩短创面愈合时间。  相似文献

硝酸银软膏对Ⅱ度烧伤创面治疗作用的多中心临床研究   总被引:6,自引:0,他引:6  
目的观察硝酸银(AgNO3)软膏对浅Ⅱ、深Ⅱ度烧伤创面的治疗效果,并评价其药物不良反应。方法选择80例浅Ⅱ度和40例深Ⅱ度烧伤患者,进行多中心、随机、阳性药物平行对照和同体试验研究(共4个中心,每个中心30例)。将患者创面按用药不同分为AgNO3组和磺胺嘧啶银(SD-Ag)组,观察各组创面完全愈合时间、指定时相点下创面愈合率、创面细菌培养情况、药物疗效和安全性、药物对创面的刺激性等。结果浅Ⅱ度创面:AgNO3组完全愈合时间为(9.5±2.7)d, SD-Ag组为(10.8±3.4)d,用药后7 d创面愈合率分别为(77.9±20.5)%及(67.3±22.6)%;深Ⅱ度创面:AgNO3组完全愈合时间为(21.5±4.8)d,SD-Ag组为(23.3±6,4)d,用药后20 d创面愈合率分别为(86.6±15.9)%及(78.5±17.7)%。同等深度烧伤创面上述各项数据两组间比较,差异均有统计学意义(P<0.01)。同等深度烧伤创面AgNO3组与SD-Ag组比较,具有同样明显的杀菌作用,但前者对创面的刺激性更小。结论AgNO3软膏是一种可用于浅Ⅱ、深Ⅱ度烧伤创面的有效、安全的外用药。  相似文献

目的:探讨重组人酸性成纤维细胞生长因子(recombinant human acidic fibroblast growth factor~rh-aFGF)与纳米银敷料联合应用治疗深Ⅱ度烧伤创面的疗效。方法:深Ⅱ度烧伤创面患者98例,随机采用自身对照选择相同深度的两个创面分为应用重组人酸性成纤维细胞生长因子(rh-aFGF)与纳米银敷料联合应用治疗组(简称治疗组);应用重组人酸性成纤维细胞生长因子(rh-aFGF)与凡士林纱布治疗(简称对照组)。结果:与对照组比较,治疗组创面愈合时间明显缩短。差异有统计学意义(P<0.05),抑(杀)菌效果显著。结论:重组人酸性成纤维细胞生长因子(rh-aFGF)与纳米银敷料联合应用治疗深Ⅱ度烧伤创面能明显促进深Ⅱ度创面愈合,有效抑制或杀灭烧伤创面病原菌,明显缩短深Ⅱ度烧伤创面愈合时间。  相似文献

The antibiotic‐resistant bacteria are a major concern to wound care because of their ability to resist many of the antibiotics used today to treat infections. Consequently, other antimicrobials, in particular ionic silver, are considered ideal topical agents for effectively helping to manage and prevent local infections. Little is known about the antimicrobial efficacy of ionic silver on antibiotic‐resistant bacteria at different pH values. Consequently, in this study our aim was to evaluate the effect of pH on the antimicrobial efficacy of a silver alginate (SA) and a silver carboxymethyl cellulose (SCMC) dressing on antibiotic‐resistant bacteria isolated from burn patients. Forty‐nine antibiotic‐resistant bacteria, including Vancomycin‐resistant Enterococcus faecium, meticillin‐resistant Staphylococcus aureus, multidrug‐resistant (MDR) Pseudomonas aeruginosa, MDR Vibrio sp, MDR Stenotrophomonas maltophilia, extended‐spectrum ß‐lactamase (ESBL) producing Salmonella sp, ESBL producing Klebsiella pneumoniae, ESBL producing Proteus mirabilis, ESBL producing Escherichia coli and MDR Acinetobacter baumannii, routinely isolated from burn wounds were used in the study and evaluated for their susceptibility to two silver containing wound dressings using a standardised antimicrobial efficacy screening assay [corrected zone of inhibition (CZOI)]. The mean overall CZOI for the Gram‐positive isolates at a pH of 5·5 were very similar for both dressings. A mean CZOI of 5 mm was recorded for the SCMC dressing, which was slightly higher, at 5·4 mm for the SA dressing. At a pH of 7·0 both dressings, in general, showed a similar activity. However, at a pH of 8·5 the mean CZOI of the SCMC dressing was found to be significantly (P < 0·05) higher than the SA dressing for a select number of isolates. The mean overall CZOI for the Gram‐negative bacteria followed a similar pattern as observed with the Gram‐positive bacteria. Susceptibility to silver ions did vary significantly between genera and species of bacteria. Interestingly, when pH was changed from 8·5 to 5·5 antimicrobial activity for both dressings in general increased significantly (P < 0·05). Overall, all forty‐nine antibiotic‐resistant bacteria isolated from burn wounds showed susceptibility to the antimicrobial activity of both silver containing wound dressings over all pH ranges. In addition, the study showed that the performance of both dressings apparently increased when pH became more acidic. The findings in this study may help to further enhance our knowledge of the role pH plays in affecting both bacterial susceptibility and antimicrobial activity of silver containing wound dressings.  相似文献

目的 观察纳米银纱布对海水及自来水浸泡的兔后肢枪伤伤道的治疗作用。方法 兔双后肢致枪伤后伤道分为 2组。组 1:伤道海水浸泡 5h ;组 2 :伤道自来水浸泡 5h。两组实验侧用纳米银纱布 ,对照侧用凡士林纱布换药治疗。各组均于浸泡后 30min和 1、2、3、4、5h取伤道组织活检 ,观察其病理改变。结果 组 1:实验侧较对照侧创周炎出现晚 ,程度轻 ,伤道内干燥 ,分泌物少。平均愈合时间 :实验侧入口 (2 9 4± 6 6 )d ,对照侧入口 (36 3± 6 0 )d (P <0 0 1) ;实验侧出口 (2 0 1± 6 0 )d ,对照侧出口 (2 7 3± 5 7)d (P <0 0 1)。组 2 :实验侧仅 1只兔创周轻度红肿 ,对照侧创周均有红肿 ,分泌物多。平均愈合时间 :实验侧入口 (13 0± 1 5 2 )d ,对照侧入口 (16 0± 3 10 )d(P <0 0 1) ;实验侧出口 (11 0± 2 75 )d ,对照侧出口 (15 6± 2 85 )d(P <0 0 1)。结论 纳米银纱布有抗感染和加速创面愈合的作用  相似文献

To compare the effect of honey dressing and silver-sulfadiazene (SSD) dressing on wound healing in burn patients. Patients (n=78) of both sexes, with age group between 10 and 50 years and with first and second degree of burn of less than 50% of TBSA (Total body surface area) were included in the study, over a period of 2 years (2006-08). After stabilization, patients were randomly attributed into two groups: ‘honey group’ and ‘SSD group’. Time elapsed since burn was recorded. After washing with normal saline, undiluted pure honey was applied over the wounds of patients in the honey group (n=37) and SSD cream over the wounds of patients in SSD group (n=41), everyday. Wound was dressed with sterile gauze, cotton pads and bandaged. Status of the wound was assessed every third and seventh day and on the day of completion of study. Patients were followed up every fortnight till epithelialization. The bacteriological examination of the wound was done every seventh day. The mean age for case (honey group) and control (SSD group) was 34.5 years and 28.5 years, respectively. Wound swab culture was positive in 29 out of 36 patients who came within 8 hours of burn and in all patients who came after 24 hours. The average duration of healing in patients treated with honey and SSD dressing at any time of admission was 18.16 and 32.68 days, respectively. Wound of all those patients (100%) who reported within 1 hour became sterile with honey dressing in less than 7 days while none with SSD. All of the wounds became sterile in less than 21 days with honey, while tthis was so in only 36.5% with SSD treated wounds. The honey group included 33 patients reported within 24 hour of injury, and 26 out of them had complete outcome at 2 months of follow-up, while numbers for the SSD group were 32 and 12. Complete outcome for any admission point of time after 2 months was noted in 81% and 37% of patients in the honey group and the SSD group. Honey dressing improves wound healing, makes the wound sterile in lesser time, has a better outcome in terms of prevention of hypertrophic scarring and post-burn contractures, and decreases the need of debridement irrespective of time of admission, when compared to SSD dressing.  相似文献

纳米晶体银敷料治疗烧伤后残余创面的多中心临床研究   总被引:6,自引:3,他引:6  
目的观察纳米晶体银敷料在治疗烧伤后残余创面、控制感染方面的疗效和安全性, 从而评价其临床应用价值。方法入选患者98例166个残余创面,采用随机、盲法、多中心、阳性平行对照的试验设计。在常规治疗的基础上,试验组(83处创面)参照伤口形状剪切大小合适的纳米晶体银敷料覆盖创面,对照组(83处创面)常规外用磺胺嘧啶银。观察至创面完全愈合或用药20d 时终止试验。比较两组患者创面愈合时间、愈合率及药物抗感染效果。结果试验组创面愈合时间为(12±5)d,较对照组(16±6)d明显缩短(P=0.005<0.01)。治疗总显效率试验组为97.05%, 较对照组的94.17%有升高趋势(P>0.05)。试验组用药后6、12 d累计细菌清除率分别为21.7%、 43.5%,较对照组明显增高(P<0.05)。用药后两组患者均未见明显不良反应。结论纳米晶体银敷料用于治疗烧伤后残余创面疗效确切,并可为进一步促进伤口愈合提供良好的基底。  相似文献

The study aims to perform a comparative assessment of two types of burn wound treatment. To do the assessment, patients with partial thickness burn wounds with total body surface area <40% were simple randomised to treat with nanocrystalline silver nylon wound dressing or silver sulfadiazine cream. Efficacy of treatment, use of analgesics, number of wound dressing change, wound infection and final hospitalisation cost were evaluated. The study showed silver nylon wound dressing significantly reduced length of hospital stay, analgesic use, wound infection and inflammation compared with silver sulfadiazine.  相似文献
