正确的植入位点对于种植修复的长期稳定有效十分重要,为提高植入的精准度,临床上越来越多地使用外科导板、手术导航等来控制位点,引导种植体的植入。但是一方面由于这类数字化手段存在过程繁琐、费用较高等问题,大量的病例仍然是由自由手植入的;另一方面,在术前、术中、术后缺乏快速实用的实测位点的方法,不少病例只有术后锥形束CT检查时才发现位点不良,改正位点的代价不小。本文介绍一种适用于各类种植系统的全程植入位点的实测方法及其引导的精准植入技术。本技术由实测尺子引导,包括测量尺和术中定位尺两部分。术前使用测量尺测量术区三维空间,根据实测数据结合锥形束CT数据进行种植方案和数量引导设计,术中用实测尺子引导全程种植,兼顾实测核查。这种方法实现了植入手术从术前空间分析、术中精准植入引导及术后位点实测评价的全程可量化,可全程及时调整位点,提高了植入精度,是一项适宜推广的牙种植实用技术。  相似文献

目的:评价临时种植体即刻负载在半颌即刻种植中的应用效果。方法:20例半颌即刻种植患者行136枚即刻种植体植入,采用95枚临时种植体即刻负载恢复患者牙列。3-6月后行固定修复。结果:即刻负载临时种植体和即刻种植体成功率达100%。结论:临时种植体即刻负载半颌即刻种植是很好的临床方法。  相似文献

[摘要] 目的 了解计算机种植导板在斜向种植手术的应用情况。方法 2011年2月—2014年1月之间,通过常规上下颌骨CT扫描,经计算机辅助制作软件建模,对13例上颌骨骨量不足种植病例,进行一定角度下斜向植入种植体而不需要进行上颌窦提升手术。共植入21颗种植体,均利用快速成型技术加工出个体化种植模板,并应用于该种植手术中。结果 在计算机种植导板的帮助下进行了斜向植牙,种植体植入术前设计位置实现了种植体的精确植入,能够在修复时与对颌牙形成良好的咬合关系,与术前设计方案基本一致。结论 该文利用计算机种植导板在进行上颌骨斜向植牙而非提升手术,可以很好的完成骨量不足区域的种植手术的定位及导向功能,是一种相对安全的选择性方案。  相似文献

This article shows the factors of importance in the long-term success and failure of oral implants based on literature review. Many factors are attributed to the failure of dental implants. The critical components leading to early and late implant failures are evaluated. The behavior of natural teeth and implants is compared in healthy and unfavorable local and systemic conditions. Similarities and differences among dental implants, healthy natural teeth, and ankylosed teeth are examined. Based on this comparison, the author attempts to draw a conclusion on the vulnerability of dental implants and their prognosis. The importance of ongoing clinical supervision of a patient's implant condition with a good recall program and the necessity to accumulate clinical data concerning implant failures over an extended period of time in a standardized manner are emphasized. It is also suggested that implant practitioners avoid giving guarantees of long-term implant success to their patients.  相似文献

The types of dental implants in current use are described with comments on the various designs, the materials of which the implants are made, the patient situations for which they are used, and the age range of patients treated. The training background of the various clinicians involved is mentioned and comparisons are made between the United States, Japan and Central Europe. Access and affordability of dental care can be problematic for some in the Australian community. Therefore, dental tourism is increasingly becoming more attractive to some patients due to decreased expense, increased convenience and immediacy of treatment. However, there are significant issues for both clinician and patient in regards to dental tourism. Lack of accountability and regulation are the main issues and this is particularly evident when complications occur. Extraoral cranial implant-retained prosthetic reconstructions have been proved to be highly successful. Replacement of the eyes, ears, nose, and larger areas including combined midface defects, which frequently have no other option available, has been done successfully. Burn patients and those with congenital defects are good candidates for this type of reconstruction, especially after autogenous attempts have failed. Cranial implant prosthetic reconstruction should be considered as a viable option for difficult craniofacial defects.  相似文献
