An 11-year-old white girl presented multiple flesh-colored dome-shaped papules on the forehead, nose, scalp, and posterior neck. The lesions had been present for 2 years and were asymptomatic. Family history was negative for skin diseases. A skin biopsy specimen revealed histopathology consistent with trichoepitheliomas. Treatment of multiple trichoepitheliomas is usually difficult. Methods reported in the literature include laser treatment, surgery, cryotherapy, electrodessication, and radiation. The parents were concerned about the risk of scarring and wanted to pursue a nonscarring treatment. The patient was initially treated with topical imiquimod three times a week. She progressively increased the frequency of application to twice daily and added topical tretinoin gel once daily to her regimen for more resistant lesions. After 3 years of treatment, the patient experienced approximately 80% clearing of lesions without scarring. The advantage of using this nonsurgical treatment is no scarring, painless, and no need for other invasive procedures such as injection of local anesthetic. 相似文献

Herpes simplex virus (HSV) is a common, easily transmissible virus. There is growing awareness of acyclovir-resistant HSV particularly among immunocompromised patients, which may be due to protracted treatments with guanosine analogues. Given the considerable morbidity associated with other classes of antiherpetic medications such as foscarnet (renal impairment, seizures) and cidofovir (renal impairment, neutropenia), imiquimod, a toll-like receptor agonist that enhances the innate immunologic responses against the virus, has been utilized in treating acyclovir-resistant HSV. We present a case of a human immunodeficiency virus (HIV)-positive patient who was successfully treated with topical imiquimod after treatment failures with other oral antivirals. 相似文献

Common warts (verrucae vulgaris) are associated with human papillomavirus infection and are routinely treated by ablative procedures such as cryotherapy, electrodessiccation and salicylic acid. We report 10 cases of recurrent warts treated with a potential new topical therapy, imiquimod 5% cream. Nine of the 10 patients were successfully treated with imiquimod 5% cream applied, under occlusion, once daily for 4 weeks. No recurrences were reported during 3 months of follow up. 相似文献

OBJECTIVE: Evaluation of the efficiency of topical calcipotriol monotherapy in vitiligo. MATERIALS AND METHODS: This was a prospective, right/left comparative, open study at the Dermatology Department at the University Hospital in Nice, France. Twenty-four patients with localized or generalized vitiligo with symmetrical lesions were included. The main outcome measure was the evaluation of the percentage of repigmentation in treated target lesions and untreated control lesions for each patient. RESULTS AND CONCLUSIONS: Topical calcipotriol in monotherapy is not an effective treatment of vitiligo. 相似文献

We observed complete clinical and histopathologic remission of papules, plaques and nodules in two patients with multiple lesions of Bowen's disease and squamous cell carcinoma. Our findings indicate that topical imiquimod is a potentially promising therapeutic approach for the local treatment of early forms of cutaneous neoplasia, such as Bowen's disease and squamous cell carcinoma because it is a non-invasive therapy with a good cosmetic outcome. It may therefore be worthy of consideration in a subgroup of patients who cannot have or do not wish to have surgery. 相似文献

Imiquimod is a recently developed imidazoquinolin heterocyclic amine that is an immune response modifier. Treatment with topical 5% imiquimod cream has shown promising results in the treatment of genital warts in immunocompetent individuals. We report the provocation of localized psoriasis at the sites of application of topical imiquimod, possibly evolving into a generalized flare. A patient with pre-existing psoriasis that had been stable for 14 years was treated with imiquimod 5% cream daily for 6 weeks to three superficial basal cell carcinomas. During treatment one of the lesions developed severe local skin reactions necessitating rest periods, and received only 18 applications in 6 weeks. The other two lesions were treated for all 42 days. Psoriasiform changes developed at all three application sites. Nine-and-a-half weeks after completing treatment the patient developed disseminated small psoriatic lesions. Other recognized triggers of psoriasis were not identified. The psoriasis resolved slowly with conventional treatment. 相似文献
