Background and purpose

It is controversial whether the transverse acetabular ligament (TAL) is a reliable guide for determining the cup orientation during total hip arthroplasty (THA). We investigated the variations in TAL anatomy and the TAL-guided cup orientation.

Methods

80 hips with osteoarthritis secondary to hip dysplasia (OA) and 80 hips with osteonecrosis of the femoral head (ON) were examined. We compared the anatomical anteversion of TAL and the TAL-guided cup orientation in relation to both disease and gender using 3D reconstruction of computed tomography (CT) images.

Results

Mean TAL anteversion was 11° (SD 10, range –12 to 35). The OA group (least-square mean 16°, 95% confidence interval (CI): 14–18) had larger anteversion than the ON group (least-square mean 6.2°, CI: 3.8 – 7.5). Females (least-square mean 20°, CI: 17–23) had larger anteversion than males (least-square mean 7.0°, CI: 4.6–9.3) in the OA group, while there were no differences between the sexes in the ON group. When TAL was used for anteversion guidance with the radiographic cup inclination fixed at 40°, 39% of OA hips and 9% of ON hips had more than 10° variance from the target anteversion, which was 15°.

Interpretation

In ON hips, TAL is a good guide for determining cup orientation during THA, although it is not a reliable guide in hips with OA secondary to dysplasia. This is because TAL orientation has large individual variation and is influenced by disease and gender.Malalignment of the acetabular cup may lead to dislocation (Jolles et al. 2002, Shon et al. 2005), accelerated wear or breakage of the bearing, and component loosening (Kennedy et al. 1998). The use of a mechanical guide for cup implantation may give inaccurate results because of pelvic rotation on the operating table (Sugano et al. 2007, Minoda et al. 2010).Recently, the transverse acetabular ligament (TAL), which bridges the acetabular notch (Löhe et al. 1996) as part of the acetabular labrum, has been reported to be useful for determining proper orientation of the acetabular components (Archbold et al. 2006, 2008, Pearce et al. 2008, Kalteis et al. 2011). TAL-guided cup orientation has been reported to guide the cup placement within Lewinnek’s safe zone (Lewinnek et al. 1978). Other studies have shown that the TAL is not a reliable guide (Epstein et al. 2010, Viste et al. 2011). We hypothesized that these divergent results could be explained by individual anatomical variation; in addition, orientation of the TAL may be affected by hip disease and gender. Furthermore, cup orientation is influenced by sagittal pelvic tilt (Nishihara et al. 2003, DiGioia et al. 2006).We determined (1) the variation in the TAL orientation and the influence of hip disease and gender on this variation, (2) the reliability of using the TAL for guiding cup orientation, and (3) the influence of pelvic tilt on the TAL-guided cup orientation, using computed tomography (CT) scan and computer simulation.  相似文献

BackgroundThis study is prospectively conducted to evaluate surgical complications of monolithic dual mobility cup total hip arthroplasty (THA) in elderly patients with fractured neck of the femur.MethodsNinety-seven patients (97 hips) with displaced femoral neck fracture who gave informed consent for participation were prospectively enrolled. Their mean age was 76.6 years (range, 60-95 years), and the mean bone mineral density T-score of neck of the femur was −2.8 (range, −1.2 to −5.5). All patients underwent THA with monolithic dual mobility cup, and computed tomography scans were obtained to evaluate radiographic parameters including anteversion, inclination, and loosening of acetabular cups, and periprosthetic acetabular fractures.ResultsWith regard to cup orientation, mean inclination angle was 40.2° (range, 23.5°-63°) and mean anteversion was 32.6° (range, 7°-66.2°). The proportion of surgical outliers was 10.3% (10/97) in inclination and 35.1% (34/97) in anteversion. Early cup loosening within 2 weeks was detected in 2 hips. Periprosthetic acetabular fractures were identified in 6 hips (6/97, 6.2%). Of the 6 fractures, 5 nondisplaced fractures were healed with conservative management, but 1 fracture with displacement eventually led to cup loosening and the patient underwent revision surgery. Reoperation rate of the monolithic dual mobility cup was 4.1% (4/97).ConclusionThe use of the monolithic dual mobility was associated with improper cup fixation and periprosthetic acetabular fractures in the elderly with poor bone stock, although the dual mobility cup lowered the risk of early dislocation after THA.  相似文献

Background and purpose Correct positioning of the acetabular component is important in total hip arthroplasty (THA). We evaluated the effect of an imageless navigation system on the accuracy of cup positioning using a minimally invasive direct anterior approach.

Methods Hip replacements were performed in 44 cadaveric hips (22 cadavers) that were divided randomly into 2 groups. In the study group, THA was performed using an imageless navigation system. In the control group, no navigation system was used. CT scans were taken postoperatively. Using 3D reconstructions, the cup position was determined in relation to the frontal pelvic plane. The goal was to place each cup at 45° of inclination and 20° of anteversion, as recommended by the manufacturer.

Results In the navigated group, there was a statistically significantly smaller range of deviation from the target angles of inclination and anteversion relative to the control group.

Interpretation We conclude that imageless navigation improves the accuracy of cup placement in minimally invasive THA using the direct anterior approach.  相似文献

Background

The acetabular component orientation in total hip arthroplasty (THA) is of critical importance to the good clinical results. However, traditional widely used cup alignment guides for cup placement are reported to be relatively unreliable. The present study aims at comparing a novel cup alignment guide, which can be attached to our anatomical pelvic plane (APP) pelvic lateral positioner for reducing discrepancies in sagittal pelvic tilt and indicate a targeted cup angle based on the APP, with a conventional cup alignment guide.

Methods

The subjects were 136 hips of 136 patients who underwent unilateral THA using the APP positioner. The procedure was performed with the conventional cup alignment guide (conventional group; 60 hips) and with the novel cup navigator (mechanical navigator group; 76 hips). Postoperative cup angles and discrepancies of postoperative cup angles (inclination and anteversion angles) from the targeted angles were compared between the 2 groups to evaluate the usefulness of these navigators.

Results

The mean cup angles in the conventional group were 39.0° ± 5.3° for the inclination angle and 21.7° ± 6.4° for the anteversion angle, whereas those in the mechanical navigator group were 40.6° ± 3.2° and 18.3° ± 4.6°, respectively (P = .018, P < .0001). The discrepancies from the targeted angles were 3.5° ± 3.1° for the inclination angle and 4.6° ± 3.4° for the anteversion angle in the conventional group and 2.3° ± 2.3° and 3.2° ± 2.7°, respectively, in the mechanical navigator group (P = .020, P = .012).

Conclusion

The mechanical cup navigator easily attachable to the APP positioner is a tool that can improve the accuracy of cup placement in a simple, economical, and noninvasive manner in THA via the lateral position.  相似文献

BackgroundAccurate orientation of acetabular and femoral components is important during total hip arthroplasty (THA). In recent years, several navigation systems have been developed. However, these navigation systems for THA are unpopular worldwide because of their high cost. We assessed the orientation accuracy of cups inserted using a disposable accelerometer-based portable navigation system for THAs.MethodsThis was a prospective cohort study. We analyzed 63 hips with navigation prospectively and 30 hips without navigation retrospectively as historical control. The patients underwent THA via the mini anterolateral approach in the supine position using an accelerometer-based portable navigation system. We compared the preoperative target angles, intraoperative cup angles using navigation records, postoperative angles using postoperative CT data, measurement errors of cup angles, and clinical parameters such as sex, treated side, age at surgery, and body mass index (BMI).ResultsThe average absolute error (postoperative CT-navigation record) was 2.7 ± 2.1° (inclination) and 2.7 ± 1.8° (anteversion), and the absolute error (postoperative CT-preoperative target angle) was 2.6 ± 1.9° (inclination) and 2.7 ± 2.2° (anteversion). The absolute error between postoperative CT and target angle with navigation was significantly lower than the error without navigation (inclination; p = 0.025, anteversion; p = 0.005). Cup malalignment (absolute difference of inclination or anteversion between postoperative CT and preoperative target angle of over 5°) was significantly associated with BMI value (OR: 1.3, 95% CI: 1.1–1.7). The absolute measurement error of cup inclination and anteversion was significantly correlated with patients’ BMI (inclination error: correlation coefficient = 0.53, p < 0.001, anteversion error: correlation coefficient = 0.58, p < 0.001).ConclusionsThe clinical accuracy of accelerometer-based portable navigation is precise for the orientation of cup placement, although accurate cup placement was affected by high BMI. This is the first study to report the accuracy of accelerometer-based portable navigation for THA in the supine position.  相似文献

Background and purpose — In hip arthroplasty, acetabular inclination and anteversion—and also femoral stem torsion—are generally assessed by eye intraoperatively. We assessed whether visual estimation of cup and stem position is reliable.

Patients and methods — In the course of a subgroup analysis of a prospective clinical trial, 65 patients underwent cementless hip arthroplasty using a minimally invasive anterolateral approach in lateral decubitus position. Altogether, 4 experienced surgeons assessed cup position intraoperatively according to the operative definition by Murray in the anterior pelvic plane and stem torsion in relation to the femoral condylar plane. Inclination, anteversion, and stem torsion were measured blind postoperatively on 3D-CT and compared to intraoperative results.

Results — The mean difference between the 3D-CT results and intraoperative estimations by eye was??4.9° (?18 to 8.7) for inclination, 9.7° (?16 to 41) for anteversion, and??7.3° (?34 to 15) for stem torsion. We found an overestimation of?>?5° for cup inclination in 32 hips, an overestimation of?>?5° for stem torsion in 40 hips, and an underestimation?

Interpretation — Even an experienced surgeon’s intraoperative estimation of cup and stem position by eye is not reliable compared to 3D-CT in minimally invasive THA. The use of mechanical insertion jigs, intraoperative fluoroscopy, or imageless navigation is recommended for correct implant insertion.  相似文献

目的通过观察后外侧入路全髋关节置换术中骨盆的旋转变化,探讨术中骨盆旋转对髋臼假体前倾角植入的影响,评估使用髋臼横韧带作髋臼假体前倾定位的准确性以及对骨盆旋转角度变化的校正作用。 方法2015年1月至2016年1月河池市第三人民医院关节外科收治的行初次THA的40例44髋的髋关节疾病患者纳入本研究。纳入标准为:初次THA术的患者,术前、术后CT扫描质量符合标准、能确定髋臼解剖前倾角、髋臼假体前倾角的患者。排除标准:髋臼发育不良、强直性脊柱炎、既往有髋关节严重创伤手术史以及翻修术等,术前、术后双髋关节CT扫描,CT横断面上骨盆明显倾斜、两侧髋关节的中心显示明显不在同一层面、难以确定水平线测量前倾角的患者,予以排除。手术均采用侧卧位后外侧入路,切皮前将1枚施氏针以垂直于地面方向打入髂骨嵴,术中髋臼假体的前倾角,以髋臼横韧带为参照标志,通过直接参照或间接参照髋臼横韧带进行髋臼挫磨及安装臼杯,使髋臼假体开口平行韧带进行安放,在此过程中用摄像机记录施氏针相对于地面的角度变化,确定手术过程中骨盆旋转度数,同时测量并记录挫磨髋臼及安放假体时,相对于身体长轴髋臼手术前倾角的数值。术后通过CT测量髋臼假体前倾角,对术中手术前倾角和术后髋臼假体的前倾角、术前髋臼解剖前倾角数据进行t检验分析。 结果术中骨盆旋转发生在本研究中的平均度数为(18±4)°。44个髋关节中,所有的病例都能对髋臼横韧带进行辨认,术中手术前倾角平均为(33±5)°,有93%(41髋)的病例大于Lewinnek提出的"安全区"的前倾角上限25°,余下的7%(3个髋)也全部大于24°。术后CT测量髋臼假体的解剖前倾角为(21±10)°,与术前髋臼解剖前倾角度(19±7)°比较,差异无统计学意义(t=1.264,P >0.05)。 结论在后外侧入路THA术中,体位改变骨盆前旋转会影响髋臼假体植入的准确性,使用髋臼横韧带作为解剖标志指导髋臼假体前倾角度的植入,可以排除患者体位改变骨盆旋转对前倾角的影响,提高髋臼假体放置的准确性。  相似文献

BackgroundThis study aimed to elucidate the accuracy of cup placement with the computed tomography (CT)-based navigation system (CTN) in the direct anterior approach (DAA)-total hip arthroplasty (THA) in the supine position compared with the mechanical cup alignment guide (MG) or the fluoroscopy (FS).MethodsA total of 171 hips of 156 patients undergone primary THA were classified into the following three groups (the MG group: 63 hips, the FS group: 58 hips, the CTN group: 50 hips). Comparing the preoperative planning with postoperative CT measurement by three-dimensional templating software, the accuracy of cup placement was investigated in the three groups.ResultsThere were significant differences in the mean absolute error of radiographic inclination (RI) between the MG group (4.4° ± 3.2°) and the CTN group (2.8° ± 2.5°) (p = 0.01). The mean absolute error of radiographic anteversion (RA) also showed significant differences between the CTN group (2.8° ± 1.9°) and the MG group (5.8° ± 4.7°) (p = 0.0001) or the FS group (4.8° ± 4.1°) (p = 0.02). Regarding the cup center position, the mean absolute error of vertical position from preoperative planning was the smallest in the CTN group (1.8 ± 1.4 mm) compared with those in other groups (MG: 3.3 ± 3.2 mm, p = 0.007, FS: 3.2 ± 3.0 mm, p = 0.017).ConclusionThe CTN guarantees accurate cup placement in the DAA-THA in the supine position compared with the MG and FS groups.  相似文献

BackgroundCup orientation has been shown to influence the postoperative risk of impingement and dislocation following total hip arthroplasty (THA) and may change over time due to changes in pelvic tilt that occur with aging. The purpose of this study is to determine if there is a significant change in acetabular cup inclination and anteversion over a 10-year period following THA.MethodsA retrospective, multisurgeon, single-center cohort study was conducted of 46 patients that underwent THA between 1995 and 2002. A total of 46 patients were included, with a median age at surgery of 56 years, and a median time between initial postoperative radiograph and the most recent one being 13.5 years (minimum 10 years). Cup orientation was measured from postoperative and follow-up supine anterior-posterior pelvic radiographs. Using a validated software, inclination and anteversion were calculated at each interval and the change in cup anteversion and inclination angle was determined. Furthermore, the difference in the sacro-femoral-pubic angle was measured, reflecting the difference in pelvic tilt between intervals.ResultsNo significant difference was detected between measurements taken from initial postoperative radiograph and measurements a minimum of 10 years later (P > .45), with the median (interquartile range) change in anteversion, inclination, and sacro-femoral-pubic being 0° (−1° to 3°), 1° (−3° to 2°), and 0° (−2° to 3°), respectively.ConclusionOur study found no significant change in functional cup orientation a minimum of 10 years after THA. No shifts in functional cup orientation as a result of altering spinopelvic alignment seemed to be present over a 10-year period.  相似文献

IntroductionDislocation is one of the most common complications after primary total hip arthroplasty (THA). Combined anteversion (CA) is currently considered one of the most important measures of stability for THA. Thus, the aim of this study is to determine the association between a correct CA after THA and hip prosthesis dislocation, and to analyze the reliability of the Lewinnek safe zone parameters.Material and methodsThis is a non-interventional retrospective study, carried out at a tertiary hospital in Spain. 2489 primary THA in 2147 patients between January 2008 and December 2014 were identified. Clinical, biological and radiographic data, including cup inclination and cup and femoral anteversion, were analyzed of all patients who developed a hip prosthesis dislocation.ResultsThirty-four patients met the eligibility criteria to be analyzed. In 73.5% (25/34) of cases, acetabular anteversion (AV) was correct, with a mean AV of 15.1° ± 9.4°. Femoral anteversion (FA) was considered correct only in 38.2% (13/34) of the dislocated THA, with a mean FA of 8.4° ± 17.2°. Sixteen of these 34 patients (47.0%) presented a correct CA, with a mean CA of 24.2° ± 21.0°. Nineteen hips (55.8%) were within the Lewinnek safe zone. Moreover, eleven patients (32.3%) developed a dislocation even though components were within the Lewinnek safe zone and presented a correct CA.ConclusionOur findings suggest that even when the THA components are positioned within a correct CA and in the Lewinnek safe zone, hip prosthesis dislocations can occur in a not inconsiderable percentage of the cases. Thus, further radiological and clinical analysis should be done to identify potential reasons for hip prosthesis dislocation.  相似文献

BackgroundThere is an interest in quantifying dynamic hip kinematics before and after total hip arthroplasty (THA) during chair-rising: one of daily life activities.MethodsThe study consisted of 21 patients who underwent unilateral total hip arthroplasty for symptomatic osteoarthritis. We obtained continuous radiographs using a flat-panel X-ray detector while the participants rose from chair. We assessed the pre and postoperative hip joint's movements using three-dimensional-to-two-dimensional model-to-image registration techniques. We also measured minimum liner-to-neck distances at maximum hip flexion and extension as anterior and posterior liner-to-neck distances, respectively. Multivariate analyses were applied to determine which factors were associated with liner-to-neck distances.ResultsThe cup inclination, cup anteversion, and stem anteversion averaged 37.4°, 23.1°, and 30.1°, respectively. Significantly larger maximum hip flexion angle (72°) was found during chair-rising after THA compared to that before THA (63°, P < 0.01). The anterior pelvic tilt at the maximum hip flexion after THA (3° of anterior tilt) was significantly (P < 0.05) anterior compared to that before THA (1° of posterior tilt). The anterior and posterior liner-to-neck distances averaged 12.3 mm and 8.1 mm, respectively, with a significant difference (P < 0.01). No liner-to-neck contact was found in any hips. In multivariate analysis, the hip flexion angle, cup inclination, stem anteversion and head diameter were significantly associated with the anterior liner-to-neck distance (P < 0.05), the hip extension angle, cup anteversion, neck length and with or without elevated rim were significantly associated with the posterior liner-to-neck distance (P < 0.05, 0.01, 0.05, 0.01, respectively).ConclusionThis study indicates that well-positioned THA provide increased range of hip flexion with sufficient anterior liner-to-neck clearance during chair-rising. Dynamic hip kinematics, component position, and hardware variables significantly influenced on the liner-to-neck clearance under weight-bearing conditions.  相似文献

BackgroundIn ceramic-on-ceramic (CoC) total hip arthroplasty (THA), component positioning demonstrated to influence the bearing damage: however the connection between angles and clinical outcomes at long-term follow-ups is currently lacking. Aims of this study were: the computer tomography (CT) assessment of component positioning in CoC THAs; the correlation analysis between positioning and ceramic damage; the identification of safe zones.Methods91 consecutive post-operative CT scans including two types of CoC implants, with a mean follow-up of 12 ± 4.4 years, were evaluated. III generation (74.2%) and IV generation (25.8%) CoC surfaces were included. The angle measurements (cup abduction, anteversion, cup tilt, stem antetorsion, sacral slope) were automated using a CT-based software. The combined anteversion was assessed as well as the cup-neck position at −15°, 0°, 45° and 90° of flexion. Ceramic damage was diagnosed using synovial fluid analyses and radiological criteria.Results63.7% of THAs was inside the cup abduction target 30°–45° and 68.1% was inside the cup anteversion target 5°–25°. 19 patients (20.9%) showed signs of ceramic damage. High cup abduction and high cup-neck 45° minimum angle (which stood for high abduction and extreme combined version) significantly correlated with ceramic damage. No demographical features apart III generation ceramic bearings influenced the results. No safe zones could be detected.ConclusionsIn CoC THA, no safe zones can be described. However it is important to avoid cup inclination over 45° and a combination of steep cup and extreme combined version.  相似文献

BackgroundMalposition of the acetabular component during total hip arthroplasty (THA) is associated with increased risk of dislocation, reduced range of motion, and accelerated wear. The purpose of this study is to compare cup positioning with a portable, accelerometer-based hip navigation system and conventional surgical technique.MethodsIn a prospective, randomized, clinical study, cups were implanted with a portable, accelerometer-based hip navigation system (navigation group; n = 55) or conventional technique (conventional group; n = 55). THA was conducted in the lateral position and through posterior approach. The cup position was determined postoperatively on pelvic radiograph and computed tomography scans.ResultsAn average cup abduction of 39.2° ± 4.6° (range, 27° to 50°) and an average cup anteversion of 14.6° ± 6.1° (range, 1° to 27.5°) were found in the navigation group, and an average cup abduction of 42.9° ± 8.0° (range, 23° to 73°) and an average cup anteversion of 11.6° ± 7.7° (range, −12.1° to 25°) in the conventional group. A smaller variation in the navigation group was indicated for cup abduction (P = .001). The deviations from the target cup position were significantly lower in the navigation group (P = .001, .016). While only 37 of 55 cups in the conventional group were inside the Lewinnek safe zone, 51 of 55 cups in the navigation group were placed inside this safe zone (P = .006). The navigation procedure took a mean of 10 minutes longer than the conventional technique.ConclusionUse of the portable, accelerometer-based hip navigation system can improve cup positioning in THA.  相似文献

Background

The purpose of this study is to clarify interobserver and intraobserver reliabilities of the three-dimensional (3D) templating of total hip arthroplasty (THA).

Methods

We selected preoperative computed tomography from 60 hips in 46 patients (14 men and 32 women) who underwent primary THA. To evaluate interobserver and intraobserver reliability, 6 orthopedic surgeons performed 3D templating twice over a 4-week interval. We investigated intraclass correlation coefficients (ICCs) and percent agreement of component size and alignment, comparing morphological differences in the hip. Reproducibility was also compared between groups with osteoarthritis (OA) and those with osteonecrosis (ON).

Results

The interobserver reliabilities for mean cup size and stem size were excellent, with ICC = 0.907 and 0.944, respectively. The value was significantly higher in the ON group than in the OA group. In the OA group, the reliability of cup size and alignment decreased in hips with severe subluxation. Percent agreement of stem size was significantly different between the shapes of femoral canal. For intraobserver reliability, the mean ICC of cup size was 0.965 overall, while the value in the ON group was significantly higher than in the OA group. The mean ICC of stem size was 0.972 overall.

Conclusion

Computed tomography–based 3D templating showed excellent reliability for component size and alignment in THA. Deformity of the affected joint influenced the reliability of preoperative planning.  相似文献

In total hip arthroplasty (THA), accurately positioning the cup is crucial for achieving an adequate postoperative range of motion and stability. For 47 THA cases in which the inferomedial rim of the cup had been positioned parallel to the transverse acetabular ligament, we retrospectively performed the measurements of the radiographic cup anteversion angle relative to the anterior pelvic plane using 3-dimensional reconstruction computed tomography. The mean anteversion angle was 21.2°, with no significant difference detected in mean cup anteversion between the dysplastic hip group (15 hips) and the control group (15 hips). We suggest that the transverse acetabular ligament is a practical anatomical landmark for determining cup anteversion in THA for both dysplastic and nondysplastic hip cases.  相似文献

BackgroundSquatting is an important function for many daily activities, but has not been well documented after total hip arthroplasty (THA). This study investigated the participation rate of squatting and in vivo kinematics during squatting.MethodsA survey questionnaire about squatting was mailed to patients who underwent primary THA and 328 patients returned acceptable responses. Additionally, 32 hips were evaluated for dynamic 3-dimensional kinematics of squatting using density-based image-matching techniques. Multivariate analyses were applied to determine which factors were associated with anterior liner-to-neck distance at maximum hip flexion.ResultsPatients who could easily squat significantly increased this ability postoperatively (23.5% vs 46%, P < .01). In 29.5% of the patients there was still no ability to squat after THA; the main reason was anxiety of dislocation (34.2%). Kinematic analysis revealed that maximum hip flexion averaged 80.7° ± 12.3° with 12.8° ± 10.7° of posterior pelvic tilt and 9.7 ± 3.0 mm of anterior liner-to-neck distance. Neither liner-to-neck, bone-to-bone, nor bone-to-implant contact was observed in any of the hips. Larger hip flexion and smaller cup anteversion were negatively associated with the anterior liner-to-neck distance at maximum hip flexion (P < .05).ConclusionPostoperatively, approximately 70% of patients squatted easily or with support. Anxiety of dislocation made patients avoid squatting after THA. In vivo squatting kinematics suggest no danger of impingement or subsequent dislocation, but excessively large hip flexion and small cup anteversion remain as risks.  相似文献

Our goal was to validate accuracy, consistency, and reproducibility/reliability of a new method for determining cup orientation in total hip arthroplasty (THA). This method allows matching the 3D‐model from CT images or slices with the projected pelvis on an anteroposterior pelvic radiograph using a fully automated registration procedure. Cup orientation (inclination and anteversion) is calculated relative to the anterior pelvic plane, corrected for individual malposition of the pelvis during radiograph acquisition. Measurements on blinded and randomized radiographs of 80 cadaver and 327 patient hips were investigated. The method showed a mean accuracy of 0.7 ± 1.7° (?3.7° to 4.0°) for inclination and 1.2 ± 2.4° (?5.3° to 5.6°) for anteversion in the cadaver trials and 1.7 ± 1.7° (?4.6° to 5.5°) for inclination and 0.9 ± 2.8° (?5.2° to 5.7°) for anteversion in the clinical data when compared to CT‐based measurements. No systematic errors in accuracy were detected with the Bland–Altman analysis. The software consistency and the reproducibility/reliability were very good. This software is an accurate, consistent, reliable, and reproducible method to measure cup orientation in THA using a sophisticated 2D/3D‐matching technique. BackgroundThere have been no studies regarding the effectiveness of augmented reality (AR)-based portable navigation systems compared with accelerometer-based portable navigation systems in total hip arthroplasty (THA).MethodsWe retrospectively compared THAs performed using an AR-based portable navigation system (n = 45) and those performed using an accelerometer-based portable navigation system (n = 42). All THAs were performed with the patient in the lateral decubitus position. The primary outcome was the absolute difference between cup placement angles displayed on the navigation screen and those measured on postoperative X-ray.ResultsThe mean absolute differences were significantly smaller in the AR-based portable navigation system group than the accelerometer-based portable navigation system group in radiographic inclination (2.5° ± 1.7° vs 4.6° ± 3.1°; 95% confidence interval 1.1°-3.2°, P < .0001). Similarly, the mean absolute differences were significantly better in the AR-based portable navigation system group in radiographic anteversion (2.1° ± 1.8° vs 6.4° ± 4.2°; 95% confidence interval 3.0°-5.7°, P < .0001). Neither hip dislocation, surgical site infection, nor other complications associated with use of the navigation system occurred in either group.ConclusionThe AR-based portable navigation system may provide more precise acetabular cup placement compared with the accelerometer-based portable navigation system in THA.  相似文献

BackgroundEnd-stage coxarthrosis is increasingly common; however, limited evidence exists on the effect of direct lateral approach (DLA) and minimally invasive direct anterior approach (MIDA) on component placement in total hip arthroplasty (THA). We therefore conducted a prospective, randomized controlled trial to determine the component placement in DLA vs MIDA in THA.MethodsBetween January 2012 and June 2013, 164 patients with clinically and radiologically confirmed coxarthrosis aged 20-80 years were randomized to either DLA or MIDA (active comparator). Excluded were patients with previous ipsilateral hip surgery, a body mass index >35 kg/m2, and/or mental disability. Primary clinical outcomes have been published elsewhere. Secondary outcomes included radiographic assessment of the acetabular component (cement-mantle thickness, inclination, and anteversion), femoral stem position (varus/valgus and THA index), offset restoration, and leg length discrepancy.ResultsThe mean cement-mantle was significantly thicker in zone 1 in the MIDA group (mean difference = 0.51 mm, 95% confidence interval [CI] 0.09-0.93, P = .018), and the mean degrees of inclination and anteversion were higher in the MIDA group (mean difference = 2.5°, 95% CI 0.3-4.6, P = .023 and mean difference = 3.6°, 95% CI 2.2-5.0, P < .0001, respectively). According to the defined reference range, cup inclination was more often adequate in the DLA group (67.9% (53/78) in the DLA group vs 52.4% (43/82) in the MIDA group, P = .045). There were no differences in frontal or lateral femoral stem position, global offset restoration, or leg length discrepancy.ConclusionIn this population of Norwegian patients with coxarthrosis, radiographic assessment showed limited differences in component placement between MIDA and DLA.
