目的 动态监测早产儿生后第1、第3、第7天血、胃液中促胃液素(GAS)、胃动素(MOT)水平,并予以非营养性吸吮(NNS)干预,探讨早产儿血、胃液中GAS、MOT的动态变化及相关性。方法 需经间断鼻胃管喂养(INGF)早产适于胎龄儿60例,根据是否辅以NNS,随机分成无非营养性吸吮(N-NNS)组及NNS组;并以同期出生无窒息、无消化系统结构及功能异常足月新生儿25例为对照。放射免疫法(RIA)批量检测血及胃液GAS,MOT水平。结果 ①血及胃液中GAS,MOT水平分别呈直线正相关。②N-NNS组及NNS组血及胃液GAS,MOT水平均低于对照组;第1天N-NNS组、NNS组血GAS、血及胃液MOT与对照组比较;差异有显著意义(P<0.05);第3天N-NNS组及NNS组血GAS,MOT低于对照组,差异有统计学意义(P<0.05);N—NNS组血GAS低于NNS组,差异有显著意义(P<0.05);第7天N—NNS组血GAS、MOT均低于NNS组及对照组,差异有显著意义(P<0.05)。结论 血及胃液中GAS,MOT水平分别呈直线相关。早产儿血及胃液中GAS,MOT水平低于足月儿,NNS能促进GAS,MOT分泌。  相似文献

目的评价在肺炎继发腹泻患儿中应用醒脾养儿颗粒的治疗效果及其对患儿血清胃泌素(GAS)、血浆胃动素(MOT)及生长抑素(SS)的水平影响,为临床治疗提供一定的指导。方法选择因肺炎继发腹泻的患儿98例分为对照组和实验组各49例,对照组应用补液支持治疗和微生态制剂治疗,实验组在对照组的基础上联合醒脾养儿颗粒治疗,比较两组患儿的临床疗效、相关症状消失时间和住院时间及治疗前、后GAS、MOT和SS水平。结果实验组患儿治疗总有效率93. 88%显著高于对照组患儿的73. 47%,发热、呕吐、脱水和腹泻等症状消失时间和住院时间均显著低于对照组患儿,差异均有统计学意义(P0. 05)。治疗后两组患儿的GAS、MOT和SS水平较治疗前有明显改善,且实验组患儿的改善程度更优于对照组患儿,差异均有统计学意义(P0. 05)。结论在肺炎继发腹泻患儿中应用醒脾养儿颗粒具有较好的临床疗效,能显著促进患儿相关症状的转归,有效调节患儿血清胃泌素、血浆胃动素及生长抑素的水平。  相似文献

目的 探讨新生儿缺氧缺血性脑损伤(HIBD)对胃肠激素分泌的影响及其相关性的意义。方法 应用放射免疫分析法(RIA)测定67例新生儿窒息患儿(包括45例HIBD患儿和22例非HIBD患儿),病程2d内,3d~5d,7d~10d,12d~15d血浆胃动素(MTL)和血清促胃液素(GAS)的含量。据临床表现,将HIBD组分为3个观察组,将血MTL和GAS水平的动态变化与其阴性对照组比较,并对其数据进行统计学处理。结果①HIBD患儿在整个病程中,血MTL和GAS升高明显(P<0.05)。②非HIBD新生儿血MTL和GAS与对照组比较,差异无显著意义(P>0.05)。③伴有惊厥、胃肠功能降低、持续的胃肠喂养困难,血MTL和GAS水平升高明显,并较其他组下降缓慢。结论 HIBD患儿存在血MTL和GAS分泌异常,血MTL和GAS水平与脑损伤的程度、胃肠功能及病情的恢复密切相关。  相似文献

目的探讨瑞格列奈对初发2型糖尿病患者血糖控制的有效性和安全性。方法采用开放性研究,治疗前后自身对照。对60例初发2型糖尿病患者给予瑞格列奈片0.5~2mg/次,3次/d,餐前10~15min服用,0~8周为治疗方案调整期,直至血糖达标,8~12周为治疗维持期。所有患者在治疗前、治疗后16周分别测空腹血糖(FBG),餐后2h血糖(2hPG)、糖化血红蛋白(HbA1C)、体重指数(BMI)、空腹及餐后1h血浆胰岛素、血胆固醇(TC)、血甘油三酯(TG)、肝肾功能,并观察治疗中不良反应的发生情况。结果用药前后PBG、2hPG、HbAIC的变化均有统计学意义(P〈0.05),治疗16周后餐后1h血浆胰岛素有明显改善(P〈0.05),空服血浆胰岛素变化不明显(P〉0.05),可明显降低TG水平(P〈0.01),TC有轻度下降,但无显著性差异(P〉0.05),对BMI、肝肾功能无明显影响。结论瑞格列奈对2型糖尿病患者在不增加体重和BMI的同时,安全有效降低血糖和糖化血红蛋白水平,是初发2型糖尿病患者的理想选择。  相似文献

目的通过观察胃炎大鼠组织PGP9.5及c—kit与胃泌素(GAS)、生长抑素(SS)表达变化,探讨胃cajal间质细胞(ICC)、肠神经及胃肠激素在胃炎发病中的作用。方法45只大鼠分胃炎A组、胃炎B组、对照组3组,分别灌喂幽门螺杆菌(Hp)SS1菌株、2%阿司匹林和0.6N盐酸1:1混合液、生理盐水,处死后取胃窦及胃体组织PGP9.5染色及检测胃体黏膜C-kit及胃窦黏膜GAS、SS表达,测量胃壁神经元胞体、c—kit阳性表达的ICC最长直径(Dmax,μm)、平均面积(μm^2)及光密度(nm)以及GAS、SS表达的积分光密度,并进行比较。结果(1)胃炎A组、B组胃壁神经元表达PGP9.5细胞平均面积、光密度均低于对照组(P〈0.01),胃炎A组与B组比较差异无统计学意义(P〉0.05);(2)胃炎A组GAS表达明显高于对照组,而SS表达则低于对照组(P〈0.05)。胃炎B组与对照组比较两项表达结果差异均无统计学意义(P〉0.05)。直线相关分析显示SS与GAS呈负相关(r=-0.333,P〈0.01);(3)胃炎A组、B组C-kit表达细胞分布面积和直径均明显小于对照组(P〈0.05),胃炎A组与B组比较差异无统计学意义(P〉0.05),三组c—kit表达细胞的积分光密度比较差异无统计学意义(P〉0.05)。结论Hp感染和NSAID可能改变胃壁神经元及ICC的形态结构。Hp感染可明显抑制胃窦SS分泌,而增加GAS分泌;NSAID诱导的胃炎对GAS与SS的影响不显著。  相似文献

目的:探讨糖-脂双能源部分肠外营养(PPN)对肝硬化肝功能及能量代谢的影响。方法:67例肝硬化病人随机分为含脂组(33例)与对照组(34例);按组别给予含或不含脂肪乳剂的PPN支持治疗2周;PPN前后分别测定肝功能及糖、脂、能量代谢等各项指标。结果:治疗后与治疗前比较,含脂组谷丙转氨酶(ALT)与总胆红素(TB)、餐后2h血糖与空腹血胰岛素水平明显下降(P<0.05),血清脂质无明显变化(P>0.05),部分血清载脂蛋白与脂蛋白水平基本恢复正常(P<0.05),血清卵磷脂胆固醇酯酰转移酶(LCAT)活性有所恢复(P<0.05),动脉血酮体比值(AKBR)<0.7者AKBR可恢复;对照组白蛋白(Alb)、磷脂(PL)明显下降(P<0.05),其余指标无明显变化。结论:糖-脂双能源PPN支持有利于肝硬化病人肝功能与能量代谢的恢复。  相似文献

目的探讨川崎病(kawasaki disease,KD)患儿急性期后内皮素1(ET-1)和心肌肌钙蛋白I(cTnI)变化的临床意义。方法本院60例川崎病患儿中合并冠状动脉病变者12例,未合并冠状动脉病变者48例,分别测定川崎病患儿及30例健康儿童血浆内皮素1、血清肌钙蛋白I水平,分析病例组急性期和恢复期患儿血浆ET-1与血清cTnI变化,与健康儿童进行对照。结果合并冠状动脉病变(CAL)组急性期ET-1、cTnl分别为(90.19±3.43)ng/L,(1.35±0.14)μg/L,其恢复期ET-1、cTnl分别为(89.09±2.44)ng/L、(1.12±0.11)μg/L,两组比较差异均无统计学意义(P〉0.05);无冠状动脉损伤(NCAL)组急性期ET-1、cTnI浓度值分别为(64.494±4.78)ng/L、(0.62±0.02)μg/L,恢复期ET-1、cTnI浓度值分别为(50.474±4.49)ng/L、(0.07±0.05)μg/L,两组比较差异均有统计学意义(P〈0.01)。川崎病患儿急性期和恢复期血浆ET-1与血清cTnI浓度呈正相关(r=0.93,0.96,P〈0.01)。结论川崎病合并冠状动脉病变患儿血浆内皮素1水平与血清肌钙蛋白I水平相关,可作为川崎病合并冠状动脉病变的早期风险监测指标。  相似文献

目的探讨血浆催乳素(PRL)在新生儿缺氧缺血性脑病(HIE)发病过程中的水平变化及其临床应用价值。方法采用放射免疫分析法测定54例HIE和20例对照组肺炎患儿在急性期和恢复期血浆PRL水平。结果急性期中、重度HIE患儿与轻度和对照组比较,血浆PRL水平明显增高(P〈0.01);恢复期HIE各组与对照组血浆PRL水平比较,差异无统计学意义(P〉0.05);中、重度HIE患儿急性期与恢复期比较,血浆PRL水平明显升高(P〈0.01)。结论PRL参与新生儿HIE的发生和发展过程,急性期其水平变化与脑损伤及其程度密切相关。  相似文献

【目的】 探讨单纯性营养不良 (protein energymalnutrition ,PEM )患儿胃肠内分泌功能 ,为PEM的治疗提供依据。 【方法】 采用放射免疫分析法测定空腹和摄入液体实验餐后 60min时外周血胃泌素 (gastrin ,Gas)、胃动素 (motilin ,MTL)、生长抑素 (somatostatin ,SS)水平。 【结果】 与对照组比较 ,观察组空腹和餐后 60min血中Gas和MTL水平明显降低 ,而SS水平则明显升高 (P均 <0 .0 1)。与对照组相似 ,观察组患儿餐后与餐前比较 ,Gas及MTL水平差异无显著性 (P >0 .0 5 ) ,SS水平则明显升高 (P <0 .0 1)。 【结论】 PEM患儿存在明显的胃肠内分泌功能紊乱。  相似文献

目的研究发热患儿血浆内毒素测定水平与血培养结果比较及其临床价值。方法分别对80例发热患儿(发热组)和80例健康体检儿童(对照组)进行血浆内毒索测定及血培养,发热组患儿中又分为血培养阴性发热组70例和血培养阳性发热组10例。比较32例上呼吸道感染患儿及25例肺炎患儿的血浆内毒素水平,然后做单因素方差分析。结果发热组血浆内毒素水平为(28.25±3.15)ng/L,对照组为(5.21±1.54)ng/L.(P〈0.05);血培养阳性发热组血浆内毒素水平为(32.53±2.67)ng/L,明显高于血培养阴性发热组的(23.74±1.96)ng/L(P〈0.05);肺炎患儿血浆内毒素水平为(25.78±3.21)ng/L,又明显高于上呼吸道感染患儿的(14.67±2.40)ng/L(P〈0.01)。结论血浆内毒素在发热感染诊断中有重要价值,可作为血培养项目的补充。  相似文献

目的 探讨肺炎激发性腹泻患儿采用醒脾养儿颗粒与双歧杆菌三联活菌胶囊联合治疗的临床价值。方法 以2017年5月—2018年5月某医院收治,自愿加入研究的80例肺炎继发性腹泻患儿为研究对象,随机分为研究组、对照组各40例,所有患儿均采取常规治疗并服用双歧杆菌三联活菌胶囊,研究组增用醒脾养儿颗粒,观察两组临床体征消退情况、临床疗效及GAS、DAO、MOT等实验室指标情况。结果 研究组退热、腹泻及呕吐消退时间均明显低于对照组,差异有统计学意义(P<0.05);研究组疗效为92.50%,高于对照组的75.00%,差异有统计学意义(P<0.05);治疗前两组GAS、DAO、MOT指标相比差异较小,无统计学意义(P>0.05);两组治疗后各项指标均显著低于治疗前,且研究组低于对照组,对比有统计学意义(P<0.05)。结论 采用醒脾养儿颗粒联合双歧杆菌三联活菌胶囊能够有效改善肺炎继发性腹泻患儿的临床症状及相关指标,疗效较好。  相似文献

BACKGROUND: Previous studies of patients with type 2 diabetes showed that cinnamon lowers fasting serum glucose, triacylglycerol, and LDL- and total cholesterol concentrations. OBJECTIVE: We aimed to study the effect of cinnamon on the rate of gastric emptying, the postprandial blood glucose response, and satiety in healthy subjects. DESIGN: The gastric emptying rate (GER) was measured by using standardized real-time ultrasonography. Fourteen healthy subjects were assessed by using a crossover trial. The subjects were examined after an 8-h fast if they had normal fasting blood glucose concentrations. GER was calculated as the percentage change in the antral cross-sectional area 15-90 min after ingestion of 300 g rice pudding (GER1) or 300 g rice pudding and 6 g cinnamon (GER2). RESULTS: The median value of GER1 was 37%, and that of GER2 was 34.5%. The addition of cinnamon to the rice pudding significantly delayed gastric emptying and lowered the postprandial glucose response (P < 0.05 for both). The reduction in the postprandial blood glucose concentration was much more noticeable and pronounced than was the lowering of the GER. The effect of cinnamon on satiety was not significant. CONCLUSIONS: The intake of 6 g cinnamon with rice pudding reduces postprandial blood glucose and delays gastric emptying without affecting satiety. Inclusion of cinnamon in the diet lowers the postprandial glucose response, a change that is at least partially explained by a delayed GER.  相似文献

BACKGROUND: Foods with a low glycemic index are increasingly being acknowledged as beneficial for individuals with disorders related to the insulin resistance syndrome. The presence of certain salts of organic acids has been shown to lower the glycemic index of bread products and one of the suggested mechanisms is a lowered gastric emptying rate (GER). One obvious pitfall with many of the common techniques for GER measurement is that the food structure, and hence the gastric release of nutrients, may be affected by enclosure of the marker for gastric emptying, eg, paracetamol. Ultrasonography is a noninvasive method for which the above pitfall is to a large extent avoided. OBJECTIVE: The main objective was to evaluate the use of ultrasonography to determine whether the lowered glycemic and insulinemic responses to bread ingestion after the addition of sodium propionate are explained by a specific effect of propionate on the GER. DESIGN: The effect of sodium propionate in bread was evaluated in 9 healthy volunteers. Barley bread products, with or without added sodium propionate, were ingested as breakfast after an overnight fast. The GER was monitored for 2 h by ultrasonography; during this period, capillary blood was withdrawn repeatedly for measurement of blood glucose and insulin. RESULTS: The GER of the barley bread decreased markedly after the addition of sodium propionate and was accompanied by lowered glycemic and insulinemic responses. CONCLUSION: The lowered glycemic response to ingestion of bread with added sodium propionate appears to be related to a lowered GER.  相似文献

目的探讨赖氨肌醇维B12联合莫沙必利治疗儿童功能性消化不良(FD)的临床效果。方法选取2019年5月至2021年5月我院收治的88例FD患儿,随机分为两组各44例。对照组予以莫沙必利治疗,观察组在对照组基础上采用赖氨肌醇维B12治疗。两组均持续用药2周。比较两组患儿的临床疗效、胃动力学指标及不良反应发生情况。结果观察组治疗总有效率为95.45%,明显高于对照组的81.82%(P<0.05)。治疗2周后,两组的胃半排空时间短于治疗前,胃窦收缩频率、胃窦收缩幅度高于治疗前,且观察组的胃半排空时间短于对照组,胃窦收缩频率、胃窦收缩幅度高于对照组(P<0.05)。治疗期间,两组患儿的不良反应发生率(6.82%vs. 4.55%)比较,差异无统计学意义(P>0.05)。结论赖氨肌醇维B12联合莫沙必利治疗儿童FD效果显著,可有效改善其胃肠运动功能,且安全性较高,值得临床推广应用。  相似文献

The effects of acute and chronic administration of a large dose of alcohol on gastric emptying and small bowel transit were studied in rats. The development of tolerance to the acute effect of alcohol on gastrointestinal motility during chronic alcohol administration was also investigated. Gastric emptying and small intestinal transit were assessed by the Phenol Red recovery method. Acutely, ethanol was given in a dose of 2.5 g/kg body wt by gavage 30 min before the test meal. Chronically, ethanol was administered by two different methods: (1) a dose of 2.5 g/kg body wt was administered by gavage daily for 10 days; (2) animals received 15% ethanol in their drinking water for 30 days. A single large dose of alcohol inhibited gastric emptying and small bowel transit. Treatment with a large dose of alcohol for 10 days did not change the gastric emptying significantly, but inhibited the small intestinal transit. Alcohol consumption in drinking water for 30 days accelerated gastric emptying and small bowel transit. Tolerance to the acute inhibitory effect of a single large dose of alcohol on gastrointestinal motility did not develop during chronic alcohol treatment.  相似文献

目的比较正常新生儿母乳喂养与代乳品喂养情况下血中胃泌素及胃动素水平的差异。方法应用放射免疫分析技术对100例正常足月新生儿(其中比较组50例由母乳喂养.对照组50例由代乳品喂养)的血清胃泌素、血浆胃动素进行了检测和统计分析。结果比较组胃泌素和胃动素水平明显高于对照组.表明母乳中胃泌素、胃动素进入新生儿体内后与内源性胃泌素、胃动素具有类似的生物活性.能补充新生儿体内的不足。结论从胃肠激素这一角度证明母乳喂养的优越性。  相似文献

Low-glycemic index (GI) foods and foods rich in whole grain are associated with reduced risk of type 2 diabetes and cardiovascular disease. We studied the effect of cereal-based bread evening meals (50 g available starch), varying in GI and content of indigestible carbohydrates, on glucose tolerance and related variables after a subsequent standardized breakfast in healthy subjects (n = 15). At breakfast, blood was sampled for 3 h for analysis of blood glucose, serum insulin, serum FFA, serum triacylglycerides, plasma glucagon, plasma gastric-inhibitory peptide, plasma glucagon-like peptide-1 (GLP-1), serum interleukin (IL)-6, serum IL-8, and plasma adiponectin. Satiety was subjectively rated after breakfast and the gastric emptying rate (GER) was determined using paracetamol as a marker. Breath hydrogen was measured as an indicator of colonic fermentation. Evening meals with barley kernel based bread (ordinary, high-amylose- or beta-glucan-rich genotypes) or an evening meal with white wheat flour bread (WWB) enriched with a mixture of barley fiber and resistant starch improved glucose tolerance at the subsequent breakfast compared with unsupplemented WWB (P < 0.05). At breakfast, the glucose response was inversely correlated with colonic fermentation (r = -0.25; P < 0.05) and GLP-1 (r = -0.26; P < 0.05) and positively correlated with FFA (r = 0.37; P < 0.001). IL-6 was lower (P < 0.01) and adiponectin was higher (P < 0.05) at breakfast following an evening meal with barley-kernel bread compared with WWB. Breath hydrogen correlated positively with satiety (r = 0.27; P < 0.01) and inversely with GER (r = -0.23; P < 0.05). In conclusion, the composition of indigestible carbohydrates of the evening meal may affect glycemic excursions and related metabolic risk variables at breakfast through a mechanism involving colonic fermentation. The results provide evidence for a link between gut microbial metabolism and key factors associated with insulin resistance.  相似文献

目的观察强噪声暴露后胃溃疡大鼠血浆多肽激素水平的变化,并讨论其机制.方法制作大鼠胃溃疡模型,进行强噪声刺激,空腹时经眼球静脉取血,采用放射免疫分析法测定血浆激素刎含量,并测量大鼠胃溃疡面积.结果(1)无噪声刺激时,胃溃疡模型术后10 d大鼠血浆胃动素[MTL,(160.70±40.34)pg/ml]和骨钙素[BGP,(27.63±13.13)pg/ml]水平均明显高于对照组[分别为(89.21±49.94)、(9.10±1.38)pg/ml],差异有显著性(P<0.05),而胃泌素(GAS)水平[(107.00±21.75)pg/ml]则明显下降,与对照组[(158.48±20.92)pg/ml]的差异亦有显著性(P<0.01).(2)强噪声刺激时,无溃疡大鼠10 d时的血浆MTL[(312.80±207.42)pg/ml]和BGP[(17.76±12.33)pg/ml]均明显高于对照组,差异均有显著性(P<0.01、P<0.05);而GAS水平无明显变化.(3)胃溃疡大鼠强噪声暴露第10天和第40天的溃疡面积[分别为(15.33±7.26)mm2和(15.11±12.45)mm2]较对照组[分别为(8.22±6.66)mm2和(3.67±9.90)mm2]增大,差异有显著性(均P<0.05);而第10天和第40天MTL水平[分别为(244.44±68.11)pg/ml和(191.20±60.50)pg/ml]与对照组[分别为(160.70±40.34)、(93.10±52.90)pg/ml]比较,均明显升高,且差异均有显著性(均P<0.01).结论强噪声暴露可使大鼠的溃疡恶化,并对其愈合有明显的负性影响.强噪声暴露和胃溃疡等复合因素作用下,大鼠胃肠内分泌反应明显紊乱.  相似文献

Effect of casein and beta-casomorphins on gastrointestinal motility in rats   总被引:2,自引:0,他引:2  
The effect of bovine casein and synthetic beta-casomorphins on the motility of rat gastrointestinal tract was studied by noninvasive techniques using the nonabsorbable marker 141Ce. Casein suspensions (CAS) or whey protein suspensions (WPS) were labeled with 141Ce and fed by gastric tube. Gastric emptying rate (GER) as well as gastrointestinal transit time (GITT) of the tracer were significantly longer with feeding CAS compared to WPS. The differences between the CAS and the WPS groups were partly (GER) or completely (GITT) abolished by pretreating the animals with the specific opiate-receptor antagonist naloxone. It is assumed that opioid peptides released from casein during digestion slowed gastrointestinal motility by direct interaction with gut opiate receptors. To prove whether beta-casomorphins, when given by gastric tube, can affect motility, different synthetic beta-casomorphins in doses between 1 and 10 mg were added to the WPS. The beta-casomorphin-4 (Tyr-Pro-Phe-Pro-NH2) showed no effect on GITT. The D-Ala substituted D-Ala-beta-casomorphin-4 (Tyr-D-Ala-Phe-Pro-NH2) and D-Ala-beta-casomorphin-5 (Tyr-D-Ala-Phe-D-Ala-Tyr-NH2), which are more resistant to proteolytic attack and have higher opioid potency than beta-casomorphin-4, slowed GITT in a dose-dependent manner.  相似文献
