Objective Through a comparative study of 99Tcm-diethylenetriaminepentaacetic acid (99Tcm-DTPA) and 99Tcm-octreotide (99Tcm-OCT) SPECT orbital scan, evaluated the clinic significance of two imaging of estimation activity in thyroid associated ophthalmopathy (TAO). Methods Aeording to the elinnic activity score (CAS), 26 TAO patients were divided into two groups: active group (CAS ≥3) and inactive group (CAS<3), 14 volunteers (control group) participates this study. Orbit scan were completed after intravenously injection with 99Tcm-DTPA 740 MBq and 99Tcm-OCT (every other day). Obtaining the uptake ratio (UR) of two kinds of orbit scans. The statistical analyses included Paired-samples t test analysis of variance and Spearman regression analysis. Results There is no statistically significance between the two methods in UR (t=0.075, P>0.05). There is a statistically significant correlation was found between CAS and the uptake ratios(99Tcm-DTPA: r=0.835, P<0.001; 99Tcm-OCT: r=0.912, P<0.001). Conclusions There is a good uniformity between 99Tcm-DTPA and 99Tcm-OCT orbital scan; 99Tcm-DTPA is a feasible and maybe a new promising estimation method of activity in the patients with TAO.  相似文献

化疗作为肺癌特别是非小细胞肺癌综合治疗的主要手段之一.肺癌耐药尤其是多药耐药成为化疗失败的重要原因.早期发现多药耐药,选择合适的化疗方案,对于肺癌患者至关重要.肺癌的多药耐药与多药耐药相关蛋白(MRP)在肺癌中表达相关,某些亲脂性抗癌化疗药物和99Tcm-甲氧基异丁基异腈(99Fcm-MIBI)可能同为某些MRP的底物,它们在肿瘤细胞中的滞留与MRP的表达相关.因而行无创性的肺99Tcm-MIBI SPECT,在化疗前预测体内MRP的表达或在化疗过程中监测MRP表达的变化,有助于预测肺癌多药耐药及化疗疗效.  相似文献

目的 探讨99Tcm-替曲膦(TF)在CHD诊断中的应用价值.方法 回顾性分析2009年10月至2011年4月疑诊CHD并行99Tcm-TF或99Tcm-MTBI腺苷负荷-静息MPI的患者各40例,比较2种放射性药物心肌显像的心肺摄取比(H/L).行99Tcm-TF显像的患者显像前后3个月内均行CAG,以冠状动脉狭窄≥50%为标准计算99Tcm-TF显像对CHD的诊断效能.对99Tcm-TF及99Tcm-MIBI H/L间差异行配对t检验,率的比较行x2检验.结果 99Tcm-TF心肌静息及负荷显像H/L分别为6.73±1.21及6.94±1.69,99Tcm-MIBI显像的相应值分别为6.58±1.94及6.64±1.81,2种显像方法H/L间差异无统计学意义(=0.41和0.78,P均>0.05),99Tcm-TF心肌静息及负荷显像H/L差异亦无统计学意义(t=0.69,P>0.05). 40例患者中99Tcm-TF显像阳性24例(60.0%),对应病变血管45支,其中LAD 20支,LCX 11支,RCA 14支.CAG阳性23例(57.5%,23/40),对应病变血管42支(LAD19支,LCX 12支,RCA 11支).99Tcm-TF负荷-静息MPI的灵敏度、特异性、准确性、阳性预测值及阴性预测值分别为87.0%(20/23)、76.5%(13/17)、82.5%(33/40)、83.3%(20/24)及81.2%(13/16).以病变血管计,99Tcm-TF MPI对LAD、LCX及RCA血管病变诊断的灵敏度分别为89.5%(17/19)、83.3%(10/12)及90.9%(10/11);阳性预测值分别为85.0%(17/20)、90.9% (10/11)及71.4%(10/14).99Tcm-TF显像对不同血管病变的灵敏度和阳性预测值差异均无统计学意义(x2=0.377和1.789,P均>0.05).结论 从H/L可知,99Tcm-TF显像图像质量与99Tcm-MIBI显像接近;99Tcm-TF负荷-静息MPI是诊断CHD的可靠方法.  相似文献

目的探讨99Tcm-甲氧基异丁基异腈(99Tcm-MIBI)SPECT评估非小细胞肺癌(NSCLC)化疗疗效的价值。方法71例NSCLC患者根据胸部CT分为化疗有效组(完全缓解+部分缓解)和无效组(病情稳定+疾病进展),于化疗前行99Tcm-MIBISPECT,静脉注射99Tcm-MIBI740MBq后10~30min及2—3h分别行早期及延迟显像,在99Tcm-MIBI显像图上用感兴趣区(ROI)的方法勾画出病灶,然后选取镜像ROI于健侧肺的相应部位,由此分别获得早期相肿瘤/正常肺组织摄取比值(ER)和延迟相肿瘤/正常肺摄取比值(DR),并计算滞留指数(RI)。采用t检验及秩和检验分析化疗有效组与化疗无效组ER、DR和RI之间的差别。结果99Tcm-MIBI显像结果中,化疗有效组的ER、DR分别为2.39±0.21、2.50±0.19,均显著高于化疗无效组的1.89±0.19、2.05±0.21,统计学差异有意义(t=8.311、8.480,P〈0.05)。化疗有效组的RI中位值为6.63%,高于化疗无效组的5.13%,统计学差异有意义(Z=2.416,P〈0.05)。结论99Tcm-MIBI显像在评估NSCLC化疗疗效方面具有重要的临床价值。  相似文献

99Tcm-奥曲肽显像在淋巴瘤诊断中的应用   总被引:1,自引:0,他引:1  
目的评价^99Tc^m-奥曲肽显像在淋巴瘤中的临床应用价值。方法11名健康志愿者和30例淋巴瘤患者进行^99Tc^m-奥曲肽显像。所有患者均进行了体格检查,胸部和腹、盆部CT,颈部和腹股沟B超检查,部分完成了腹、盆腔B超检查(11例),^67-Ga-枸橼酸盐全身显像(5例)和骨髓检查(8例)。结果在志愿者中,^99Tc^m-奥曲肽可使脑垂体、鼻咽部、甲状腺、肝、脾、双肾、膀胱、胆囊、消化道等组织器官显影,骨骼和骨髓区内放射性分布明显低下或缺损。在30例患者中,^99Tc^m-奥曲肽显像发现22例阳性,其中2例为淋巴结炎性反应性增生疾病。其诊断患者的灵敏度和特异性分别为83.3%(20/24)和4/6,假阴性率为16.7%(4/24);诊断病灶的灵敏度为49.3%(72/146);对膈肌上区和膈肌下区淋巴瘤病灶的诊断灵敏度分别为51.6%(48/93)和45.3%(24/53)。显像发现4个假阳性病灶,对病灶的诊断特异性为99.6%(990/994)。结论^99Tc^m-奥曲肽显像探测淋巴瘤病灶的灵敏度较低;其无法对恶性淋巴瘤和淋巴结炎性反应性增生进行鉴别。  相似文献

目的:探讨99Tcm-tetrofosmin(99Tcm-TF)SPECT亲肿瘤显像诊断肺部肿瘤的临床价值。方法收集在我科进行99Tcm-TF肺亲肿瘤显像的患者41例;所有患者均在治疗前一周行胸部CT检查及99Tcm-TF SPECT肺亲肿瘤显像。对照最终结果,评价两种方法鉴别肺肿瘤病变性质的诊断效能。以肿瘤3 cm大小为界,分≥3 cm组和<3 cm组,评价99Tcm-TF SPECT显像在两组中的诊断效能。结果99Tcm-TF SPECT肺亲肿瘤显像的灵敏度、特异度、准确率、阳性预测值和阴性预测值分别为66.7%、35.7%、56.1%、66.7%和35.7%;CT肺扫描的灵敏度、特异度、准确率、阳性预测值和阴性预测值分别为92.6%、50.0%、78.0%、78.1%和77.8%。CT较99Tcm-TF SPECT的灵敏度(χ2=5.594, P<0.05)和准确率(χ2=4.473,P<0.05)高,而特异度两者间的差异无统计学意义,(χ2=0.583,P>0.05)。同时,肿瘤≥3 cm和<3 cm组中,99Tcm-TF检查肺肿瘤的灵敏度、特异度、准确率分别为78.9%、25.0%、69.6%和37.5%、40.0%、38.9%,而两组的特异度差异无统计学意义(χ2=0.28,P>0.05),灵敏度(χ2=4.352,P<0.05)和准确率(χ2=3.858,P<0.05)有差异。结论 CT对于肺癌诊断有一定的价值,相比而言,99Tcm-TF的诊断价值有限。临床医生在行该项检查之前需经过筛选才有可能提高其临床价值。  相似文献

目的探讨脑多巴胺转运蛋白显像剂99Tcm-2β-[N,N′-双(2-巯乙基)乙撑二胺基]甲基,3β-(4-氯苯基)托烷(TRODAT-1)SPECT显像对帕金森病(PD)的诊断价值.方法对30例PD患者及16例健康志愿者行99Tcm-TRODAT-1脑SPECT显像.用2种半定量方法分析①应用感兴趣区(ROI)技术计算双侧横断面、矢状面纹状体(ST)/小脑(CB)放射性摄取比值(ST/CB);②取横断面基底节,将其划分成左、右ST,左、右尾状核和左、右壳核6个ROI,CB和枕叶皮质(OC)作为本底参考区,计算基底节ROI的放射性摄取比值[ROI/(CB+OC/2)].结果对照组横断面、矢状面左、右侧ST/CB分别为1.55±0.15、1.58±0.14和1.62±0.17、1.69±0.16.PD患者ST/CB和ROI/(CB+OC/2)均比对照组明显减低,差异有显著性(P<0.001).计算矢状面双侧ST/CB更合理.根据ROI/(CB+OC/2)可获得基底节各神经核团放射性分布半定量数据.结论99Tcm-TRODAT-1脑SPECT显像是诊断PD的有效方法.  相似文献

目的 评价99Tcm-替曲膦MPI对冠心病的诊断价值.方法 对73例临床怀疑冠心病的患者[男46例,女27例,年龄(61.16±12.95)岁],行99Tcm-TF MPI,第1天行静息显像,次日行运动或药物负荷显像.1周内行CAG,以其结果为“金标准”,计算99Tcm-TF MPI诊断冠心病的灵敏度、特异性和准确性等.结果 在73例患者中,43例CAG阳性,其中MPI阳性32例,阴性11例.单支病变27例,双支病变10例,三支病变6例.共65支病变血管,累及LAD 33支,LCX 16支,RCA 16支.30例CAG阴性,其中MPI阴性25例,阳性5例.99Tcm-TF MPI对冠心病诊断的灵敏度74.4%(32/43),特异性83.3%(25/30),阳性预测值86.5%(32/37),阴性预测值69.4%(25/36),准确性为78.1%(57/73).诊断单支、双支及三支病变患者的灵敏度分别为66.7%(18/27)、80.0%(8/10)和5/6.99Tcm-TF MPI诊断病变血管的灵敏度为67.7%(44/65),特异性为89.0% (137/154).结论 99Tcm-TF MPI对冠心病有较好的诊断价值,是继99Tcm-MIBI之后又一优良的心肌灌注显像剂.  相似文献

目的 对比99Tcm-替曲膦(TF)和99Tcm-MIBI G-MPI的左室心肌灌注及左室心功能参数结果,探讨99 Tcm-TF能否作为临床G-MPI常规显像剂.方法 99Tcm标记TF和MIBI并进行放化纯测定.选取2011年12月至2012年5月间既往健康、排除严重心律不齐及临床拟诊为心肌炎、心肌病的112例受试者(研究经医院伦理委员会批准,患者签署知情同意书),采用简单随机抽样法将其随机分成2组,分别行一日法99Tcm-TF(47例,运动负荷状态下注射显像剂370 MBq,静息状态下注射显像剂740 MBq)G-MPI和两日法99Tcm-MIBI(65例,注射剂量均为555 MBq)G-MPI,所有受试者均于1周内行CAG和UCG检查.利用Cedars定量门控SPECT(QGS)软件获得左室心功能参数,包括LVEF、EDV、ESV、峰充盈率(PFR)、峰射血率(PER)及相位标准差(SD).采用SPSS 17.0软件进行x2检验、两独立样本r检验、配对r检验及直线相关分析.结果 99 Tcm-TF和99Tcm-MIBI放化纯分别为(97.5±0.4)%和(99.1±0.2)%.与CAG结果对比,99Tcm-MIBI心肌灌注结果符合率为90.5%(57/63),99Tcm-TF为88.9%(40/45),两者差异无统计学意义(x2=0.389,P>0.05).99Tcm-TF和99Tcm-MIBIG-MPI获得的左室心功能参数差异也无统计学意义[LVEF:(62.60±±13.56)%与(60.52±7.08)%,r=0.940;EDV:(103.31±17.29) ml与(98.52±19.37) ml,t=1.348;ESV:(41.73±12.69) ml与(46.05±10.81) ml,t=0.851;PER:(2.73±0.67) EDV/s与(2.61±1.04) EDV/s,t =0.725;PFR:(2.13±0.80) EDV/s与(2.07±1.09) EDV/s,t=0.339;相位SD:(5.58±4.16).与(5.97±4.64).,t=0.450,均P>0.05].两者G-MPI获得的左室心功能参数与UCG结果差异无统计学意义[LVEF:(61.39±10.28)%与(62.13±8.51)%,t=0.991;EDV:(100.50±18.61) ml与(101.70±18.34) ml,t=1.806;ESV:(44.25±11.77) ml与(43.94±11.28)ml,t=0.658;均P>0.05],且与UCG结果呈正相关(r=0.673、0.710和0.452,均P<0.05).结论 99Tcm-TF可作为G-MPI显像剂常规用于临床,同时评价左室心肌灌注情况和左室功能.  相似文献
