Objective

To evaluate the added role of T1-weighted (T1w) gadolinium ethoxybenzyl diethylenetriamine penta-acetic acid (Gd-EOB-DTPA)-enhanced magnetic resonance cholangiography (MRC) compared with T2-weighted MRC (T2w-MRC) in the detection of biliary leaks.

Methods

Ninety-nine patients with suspected biliary complications underwent routine T2w-MRC and T1w contrast-enhanced (CE) MRC using Gd-EOB-DTPA to identify biliary leaks. Two observers reviewed the image sets separately and together. MRC findings were compared with those of surgery and percutaneous transhepatic cholangiopancreatography. The sensitivity, specificity and accuracy of the techniques in identifying biliary leaks were calculated.

Results

Accuracy of locating biliary leaks was superior with the combination of Gd-EOB-DTPA-enhanced MRC and T2w-MRC (P?<?0.05).The mean sensitivities were 79 % vs 59 %, and the mean accuracy rates were 84 % vs 58 % for combined CE-MRC and T2w-MRC vs sole T2w-MRC. Nineteen out of 21 patients with biliary-cyst communication, 90.4 %, and 12/15 patients with post-traumatic biliary extravasations, 80 %, were detected by the combination of Gd-EOB-DTPA-enhanced MRC and T2w-MRC images, P?<?0.05.

Conclusions

Gd-EOB-DTPA-enhanced MRC yields information that complements T2w-MRC findings and improves the identification and localisation of the bile extravasations (84 % accuracy, 100 % specificity, P?<?0.05). We recommend Gd-EOB-DTPA-enhanced MRC in addition to T2w-MRC to increase the preoperative accuracy of identifying and locating extravasations of bile.

OBJECTIVE: Our aim was to assess preliminary experience with combined conventional T2-weighted and mangafodipir trisodium (MnDPDP)-enhanced T1-weighted MR cholangiography in evaluating early biliary complications of laparoscopic cholecystectomy. SUBJECTS AND METHODS: Conventional heavily T2-weighted MR cholangiography with MnDPDP-enhanced T1-weighted MR cholangiography and ERCP were performed in seven patients with high clinical suspicion of biliary complications after laparoscopic cholecystectomy. The final diagnoses of complications were classified according to the presence and degree of bile duct injury, bile leakage, and retained stones. RESULTS: The diagnoses on MR cholangiography were as follows: complete transection and occlusion of the common bile duct with bile leakage (n = 3), partial strictures of the common bile duct with bile leakage (n = 1), cystic duct leakage (n = 1), partial ligation of an aberrant right hepatic duct (n = 1), and hemorrhage without biliary complication (n = 1). The final diagnoses at surgery (n = 2) and ERCP (n = 5) were as follows: complete transection and occlusion of the common bile duct with bile leakage (n = 2), partial strictures of the common bile duct with bile leakage (n = 2), cystic duct leakage (n = 1), partial ligation of an aberrant right hepatic duct (n = 1), and hemorrhage without biliary complication (n = 1). MR cholangiography accurately yielded the same findings as the final diagnoses, except in one case with partial stricture of the bile duct with bile leakage (overdiagnosed as complete occlusion on MR cholangiography). CONCLUSION: Combined conventional T2-weighted and MnDPDP-enhanced T1-weighted MR cholangiography may eliminate the use of other studies for the imaging of biliary complications after cholecystectomy if this preliminary data can be verified in a larger study.  相似文献

目的 探讨MR胆管水成像(T2WI-MRC)和钆贝葡胺增强后胆管成像(CE-MRC)对肝移植供体胆管显示的差别.方法 32名肝移植供体术前均行T2WI-MRC和CE-MRC检查,以术中胆管造影结果为金标准,对比2种成像方法对肝内外胆管的显示情况,并对胃肠内液体影、呼吸引起的运动伪影进行比较.结果 胆管变异9名,2种检查方法均正确诊断.2种方法均可清楚显示3级肝内胆管,T2WI-MRC有28名(87.5%)能显示肝内3级以上胆管,CE-MRC只有14名(43.8%)可以显示.2名受检者T2WI-MRC显示胆总管不连续,CE-MRC及术中胆管造影显示胆总管正常.T2WI-MRC有6名(18.8%)受检者肠道内液体影响胆管显示,CE-MRC则无一名受检者肠道液体影响胆管显示.呼吸引起的运动伪影2种检查方法均不明显.结论 T2WI-MRC和CE-MRC均可用来评估术前肝移植供体胆管解剖,但CE-MRC受肠道液体影响少,较T2WI-MRC对于肝内3级以上胆管显示具有优势.  相似文献

Purpose

The aim of this retrospective study is to evaluate the role of T2-weighted MR imaging (MRI) and MR cholangiopancreatography (MRCP) findings in the diagnosis of primary biliary cirrhosis (PBC).

Materials and methods

The following T2-weighted MRI and MRCP findings: segmental hepatic atrophy/hypertrophy, irregular liver surface, parenchymal lace-like fibrosis, rounded low signal intensity lesions centering portal vein branches (periportal halo sign), periportal hyperintensity (cuffing), splenomegaly, ascites, lymphadenopathy, venous collaterals, and the configuration of intrahepatic biliary ducts were reviewed for their diagnostic significance by two observers in 13 female patients (mean age: 49 years) with PBC. Discordant readings of the observers were resolved at consensus.

Results

When parenchymal lace-like fibrosis and periportal halo sign were seen together the sensitivity of T2-weighted MR images was 69%. In six cases periportal hyperintensity (cuffing) and periportal halo sign were seen together. Segmental hypertrophy was present in nine patients and hepatic surface irregularity due to regenerative nodules were present in 10 patients. Lymphadenopathy was seen in 10, splenomegaly was seen in 5, collateral vascular structures were seen in 2 and minimal perihepatic free fluid was seen in 2 patients. MRCP images revealed various mild irregularity in the intrahepatic bile ducts in 8 patients and focal narrowing at the common bile duct level in 1 patient.

Conclusion

MRI and MRCP may support the clinical and laboratory findings of PBC even in the early stages of the disease. MRI can also be a choice of method for the recommended prolonged follow up.  相似文献

PURPOSE: The purpose of this work was to compare MR cholangiography with endoscopic retrograde cholangiopancreatography (ERCP) in the diagnosis of multiple biliary papillomatosis (MBP). METHOD: Nine patients with pathologically proven MBP underwent MR cholangiography and ERCP. A comparison was made between ERCP, multislice HASTE, and single-shot RARE. Each biliary tract was divided into five sections for assessment; therefore, 45 biliary duct areas were used for lesion detection and determination of quality of depiction. RESULTS: Of the 35 segments detected by percutaneous transhepatic cholangioscopy, pathologic examinations were performed in 31 segments: papillary adenocarcinoma was proved in 28 and papillary adenoma in 3. The multislice HASTE sequence showed bile duct branches with biliary papillomatosis better than did ERCP (p = 0.0029) and single-shot RARE sequence (p = 0.0558). The multislice HASTE procedure had the highest number of lesions detected, followed by single-shot RARE and ERCP, but there was no significant difference between the imaging techniques. CONCLUSION: Our preliminary data suggest that MR cholangiography can replace ERCP for the detection of MBP.  相似文献

AIM: To assess the diagnostic value of three-dimensional (3D) magnetic resonance cholangiography (MRC) versus direct cholangiography such as endoscopic retrograde cholangiography (ERC) and percutaneous transhepatic cholangiography (PTC) in malignant biliary stenosis. MATERIAL AND METHODS: Twenty-nine patients (15 female and 14 male) (mean age 62 years) with malignant biliary strictures underwent MRC and ERC. Breath-hold 3D steady state free precession MR cholangiography was performed on a 1.5-T imager in the patients before ERC. In 25 patients findings at ERC/PTC were considered the standard of reference: 19 patients underwent ERC, 5 PCT and 1 both ERC and PTC due to unsuccessful papilla cannulation during the endoscopic examination. In the 4 remaining patients the surgical specimen was considered the standard of reference. In the 29 patients studied, histology performed during direct cholangiography and the examination of the surgical specimens demonstrated that the malignant hilar stenoses were caused by hilar cholangiocarcinoma (n=7), cholangiocarcinoma of the distal VBP (n=1), gallbladder cancers (n=6), endometrial metastasis (n=2), ovary metastasis (n=1), colon metastasis (n=1), breast metastasis (n=1). The correct identification of biliary stenosis and extension of the tumor (according to the Bismuth classification) by MR cholangiography and ERC were independently assessed by two readers blinded to each other's report. The results were compared. RESULTS: Identification of biliary stenosis and neoplastic extension were accurate in respectively 29/29 (100%) and 26/29 (89%) cases with MR cholangiography. The comparison of ERC/PTC and MRC images yielded the following results: Bismuth Type I (6 vs 6), Type II (5 vs 8), Type III (13 vs 10), Type IV (5 vs 5). Our results indicate that MR is less capable of identifying the extension of small lesions at the primary confluence of bile ducts than are ERC/PCT. DISCUSSION AND CONCLUSIONS: MR cholangiography is a non-invasive technique for biliary tract imaging. It does not require administration of contrast medium and allows complete visualisation of the biliary ducts. MR cholangiography allowed accurate diagnosis of malignant hilar stenosis providing equal information as direct cholangiography and may therefore obviate the need for ERC/PTC.  相似文献

The purpose of this study was to determine the effects of mangafodipir trisodium on heavily T2-weighted magnetic resonance cholangiography (MRC) images and on functional T1-weighted MRC. Pre- and post-mangafodipir trisodium heavily T2-weighted MRC and fat-suppressed T1-weighted three-dimensional (3D) gradient-echo MRC images were obtained in a patient with a prior cholecystectomy and a long cystic duct remnant that had apparent biliary stasis. Multiplanar reconstructed images were created. The precontrast T2-weighted MRC showed a long cystic duct remnant and a normal common bile duct (CBD). The postcontrast T2-weighted MRC showed loss of CBD signal, but persistent signal in the cystic duct remnant due to biliary stasis. Post-mangafodipir T1-weighted 3D gradient-echo images showed the main right and left hepatic ducts, but the cystic duct was not depicted. Conventional T2-weighted MRC sequences should not be obtained after administering mangafodipir trisodium because this contrast agent decreases the T2 and therefore the signal intensity of bile within normally functioning bile ducts. Functional MRC images can be acquired by using a post-mangafodipir T1-weighted technique.  相似文献

OBJECTIVE: This study was designed to determine whether the addition of mangafodipir trisodium-enhanced MRI could improve the image quality, visualization of ductal structures, and diagnostic confidence provided by conventional T2-based MR cholangiography (MRC) in patients with suspected biliary complications after orthotopic liver transplantation. SUBJECTS AND METHODS. Our study group consisted of 25 consecutive patients who were referred for MR evaluation of clinically suspected biliary complications after orthotopic liver transplantation. Conventional MRC in the axial and coronal planes was performed in each patient, followed by fat-suppressed volumetric gradient-echo imaging in the same planes both before and after the IV administration of mangafodipir trisodium. Imaging was performed in all patients until the contrast agent was seen in the bowel. Images were then graded for quality, visualization of bile ducts and anastomoses, presence of significant stricture or leak, and level of diagnostic confidence. RESULTS: Mangafodipir trisodium-enhanced MRC tended to outperform conventional MRC in overall image quality and extrahepatic duct visualization; it was also more effective in delineating biliary anastomoses, and the difference was statistically significant (p < 0.001). All 25 enhanced examinations were considered diagnostic. Diagnostic confidence was scored as poor or lacking in 14 of the conventional MRC examinations for biliary stenosis and in 12 examinations for biliary leak. CONCLUSION: Enhancement with mangafodipir trisodium improves the performance of MRC for the detection and exclusion of biliary abnormalities after orthotopic liver transplantation. Future investigations should compare the performance of mangafodipir trisodium-enhanced MRC with the performance of more invasive techniques.  相似文献

PURPOSE: To evaluate the diagnostic value of an imaging protocol that combines dynamic contrast-enhanced T1-weighted magnetic resonance (MR) imaging and T2*-weighted first-pass perfusion imaging in patients with breast tumors and to determine if T2*-weighted imaging can provide additional diagnostic information to that obtained with T1-weighted imaging. MATERIALS AND METHODS: One hundred thirty patients with breast tumors underwent MR imaging with dynamic contrast-enhanced T1-weighted imaging of the entire breast, which was followed immediately with single-section, T2*-weighted imaging of the tumor. RESULTS: With T2*-weighted perfusion imaging, 57 of 72 carcinomas but only four of 58 benign lesions had a signal intensity loss of 20% or more during the first pass, for a sensitivity of 79% and a specificity of 93%. With dynamic contrast-enhanced T1-weighted imaging, 64 carcinomas and 19 benign lesions showed a signal intensity increase of 90% or more in the first image obtained after the administration of contrast material, for a sensitivity of 89% and a specificity of 67%. CONCLUSION: T2*-weighted first-pass perfusion imaging can help differentiate between benign and malignant breast lesions with a high level of specificity. The combination of T1-weighted and T2*-weighted imaging is feasible in a single patient examination and may improve breast MR imaging.  相似文献

Cartilaginous tumors: fast contrast-enhanced MR imaging   总被引:16,自引:0,他引:16  
PURPOSE: To differentiate between benign and malignant cartilaginous tumors with fast contrast material-enhanced magnetic resonance (MR) imaging. MATERIALS AND METHODS: In 37 patients, fast contrast-enhanced MR images were obtained in eight enchondromas, 11 osteochondromas, and 18 chondrosarcomas. Start of enhancement-early, within 10 seconds after arterial enhancement; delayed, between 10 seconds and 2 minutes; late, after 5 minutes on spin-echo images-and progression of enhancement were represented with three types of time-signal intensity curves. Findings were correlated with the surgical specimen in 27 cases, curettage material in three cases, and biopsy combined with long-term follow-up findings in seven cases. RESULTS: Start of enhancement and the combination of start and progression of enhancement correlated significantly (P <.001) with benign and malignant tumors. Early enhancement was seen in chondrosarcoma, not seen in enchondroma, and seen in osteochondroma only when growth plates were unfused. The sensitivity was 89%, specificity 84%, positive predictive value 84%, and negative predictive value 89%. Differentiation of malignancy from benignity on the basis of early and exponential enhancement was possible with a sensitivity of 61%, specificity 95%, positive predictive value 92%, and negative predictive value 72%. CONCLUSION: Preliminary results show that in the adult population fast contrast-enhanced MR imaging may assist in differentiation between benign and malignant cartilaginous tumors.  相似文献

OBJECTIVE: The purpose of our study was to compare MR cholangiopancreatography and contrast-enhanced cholangiography in patients with sclerosing cholangitis. MATERIALS AND METHODS: Twenty patients with sclerosing cholangitis were evaluated on MR cholangiopancreatography using the single-shot fast spin-echo technique at 1.5 T. A group of 19 healthy volunteers underwent MR cholangiopancreatography as controls. Thick-slab (2-cm sections) coronal oblique and thin-slab (5-mm sections) interleaved straight coronal MR images were obtained. All patients with sclerosing cholangitis had an MR cholangiopancreatogram within 12 months of a contrast-enhanced cholangiogram (mean, 3.8 months). Seventy-five percent of patients had an MR cholangiopancreatogram within 3 months of the contrast-enhanced cholangiogram. The MR cholangiopancreatograms and contrast-enhanced cholangiograms were reviewed independently in a random fashion by two radiologists who were unaware of clinical history for the degree of ductal visualization and for the presence and location of strictures of the intrahepatic and extrahepatic bile ducts. All discrepancies were resolved by a consensus, and the contrast-enhanced cholangiograms were regarded as the gold standard. Statistically significant data were calculated using the signed rank test (p < 0.01), and agreement analysis was calculated using Cohen's kappa. RESULTS: All findings on MR cholangiopancreatograms in healthy subjects were interpreted as normal, and all findings on MR cholangiopancreatograms in patients with sclerosing cholangitis were interpreted as abnormal. When compared with the control group, scans of patients with sclerosing cholangitis usually showed good visualization (>50%) of the intrasegmental (86% vs 9%) and peripheral (67% vs 0%) intrahepatic ducts on thick-slab MR cholangiopancreatography. Thick-slab MR cholangiopancreatography showed good visualization in more ducts than contrast cholangiography (84% vs 70%; p = 0.10) and showed more strictured ducts than contrast cholangiography (47% vs 36%; p = 0.22). When comparing those ducts with good visualization on both MR cholangiopancreatography and contrast cholangiography, we found that disagreement occurred regarding 32% of ducts. Most of the discrepancies (60%) resulted when a stricture was noted on MR cholangiopancreatography but not on contrast-enhanced cholangiography. Good interobserver agreement (kappa > 0.4) was noted for detecting strictures of the extrahepatic, left hepatic, left medial, and right posterior ducts, with the greatest agreement for extrahepatic ductal strictures (kappa = 0.8). CONCLUSION: Thick-slab MR cholangiopancreatography is the best technique for depicting normal and strictured bile ducts and allows the differentiation of healthy patients from patients with sclerosing cholangitis. Although endoscopic retrograde cholangiopancreatography was considered the standard, MR cholangiopancreatography was superior for intrahepatic biliary ductal visualization. Therefore, this technique is of value in the diagnosis and follow-up of patients with sclerosing cholangitis.  相似文献

Recent developments in imaging technology have enabled CT and MR cholangiopancreatography (MRCP) to provide minimally invasive alternatives to endoscopic retrograde cholangiopancreatography for the pre- and post-operative assessment of biliary disease. This article describes anatomical variants of the biliary tree with surgical significance, followed by comparison of CT and MR cholangiographies. Drip infusion cholangiography with CT (DIC-CT) enables high-resolution three-dimensional anatomical representation of very small bile ducts (e.g. aberrant branches, the caudate branch and the cystic duct), which are potential causes of surgical complications. The disadvantages of DIC-CT include the possibility of adverse reactions to biliary contrast media and insufficient depiction of bile ducts caused by liver dysfunction or obstructive jaundice. Conventional MRCP is a standard, non-invasive method for evaluating the biliary tree. MRCP provides useful information, especially regarding the extrahepatic bile ducts and dilated intrahepatic bile ducts. Gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid-enhanced MRCP may facilitate the evaluation of biliary structure and excretory function. Understanding the characteristics of each type of cholangiography is important to ensure sufficient perioperative evaluation of the biliary system.  相似文献

Three-dimensional (3D) magnetic resonance (MR) projection imaging was evaluated as a noninvasive alternative to direct cholangiography in 12 patients with malignancy-related obstructive jaundice. The 3D images of the bile ducts were formed by subjecting consecutive coronal MR images obtained with a fast imaging method to a maximum-intensity projection algorithm. Dilatation and obstruction of the biliary system were well documented in all cases, and good correlation between findings at 3D MR cholangiography and percutaneous transhepatic biliary drainage performed 0-21 days later was observed.  相似文献

ObjectiveWe aimed to compare diagnostic performance of gadoxetic-acid-enhanced-T1-weighted-MR cholangiography (MRC) with that of conventional T2-weighted-MRC in diagnosing biliary stone disease.Materials and MethodsNinety patients who underwent MRC for evaluation of biliary disease were included. Presence of stones in extrahepatic duct, gallbladder and intrahepatic duct, and presence of acute cholecystitis were evaluated. Sensitivity, specificity, and accuracy of biliary stone disease diagnosis in each biliary duct location according to each image sets were measured.ResultsThere was no significant difference in diagnostic performance between two sets of MRC in diagnosing biliary stone disease.ConclusionsDiagnostic performance of T1-MRC with gadoxetic-acid in diagnosing biliary stone disease is comparable to that of T2-MRC.  相似文献
