目的探讨食物过敏性疾病中血清特异性IgG抗体与IgE抗体检测水平的临床意义。方法选择临床诊断为食物过敏性疾病患者100例,检测14种食物过敏原中血清特异性IgG与IgE抗体水平,分析2种特异性抗体在性别、年龄、食物种类的分布情况。结果14种食物过敏原特异性IgG抗体检测阳性率依次为牛奶(32.0%)、鸡蛋(29.0%)、大豆(18.0%)、大麦(18.0%)、小麦(18.0%)等;血清特异性IgE水平较高的依次是蟹(38.0%)、鳕鱼(20.0%)、牛奶(18.0%)等。血清特异性IgG抗体与IgE抗体阳性率在以上过敏原中检测率较高,且检出率不同;不同性别、年龄段2种抗体检测率也不同。结论牛奶、鸡蛋、大豆、蟹等容易引起食物过敏性疾病,特异性IgG、IgE抗体水平在不同食物引起的过敏性疾病中作用不同,差异较大,联合检测有益于过敏性疾病的准确判断。  相似文献

【目的】了解儿童总IgE和过敏原特异性IgE的分布情况及变化规律,为儿童过敏性疾病防治提供依据。【方法】对2017年8月—2018年2月在湖州市妇幼保健院就诊的338名儿童进行总IgE、过敏原特异性IgE检测,采用胶体金法检测总IgE抗体,用免疫印迹法检测过敏原特异性IgE抗体。【结果】在检测的338名儿童中,总IgE阳性率为56.51%(191/338),过敏原特异性IgE抗体阳性儿童占36.69%(124/338),其中61.29%为单一物质过敏,38.71%为2种或以上物质过敏;不同性别之间总IgE、过敏原特异性IgE差异均无统计学意义(P0.05)。吸入性过敏原特异性IgE阳性率为18.64%(63/338),以户尘螨为主;食物性过敏原特异性IgE阳性率为40.24%(136/338),以鸡蛋白和牛奶为主。随着年龄增长总IgE、吸入性过敏原逐渐增高,而食物性过敏原逐步减少,差异均有统计学意义(P0.05)。【结论】户尘螨、鸡蛋白和牛奶是婴幼儿最主要的过敏原,早期预防的重点应放在食物性过敏原,而随着年龄增长需警惕吸入性过敏原引起的超敏反应。血清总IgE和过敏原特异性IgE的检测有利于了解过敏状态,能更好地为儿童过敏性疾病的预防、诊断和治疗提供帮助。  相似文献

目的:了解血清过敏原特异性IgE在过敏性紫癜患儿中的临床意义。方法:采用酶联免疫吸附试验检测80例过敏性紫癜患儿的血清过敏原特异性IgE。结果:80例患儿的血清总IgE阳性率为88.75%,其中62.5%的患儿查出特异性过敏原。吸入性过敏原中尘粉螨阳性率最高,食物性过敏原中虾蟹阳性率最高。结论:过敏性紫癜的发生与吸入性过敏原和食物性过敏原都有一定的相关性,血清过敏原特异性IgE检测可对过敏性紫癜的病因诊断和预防有参考意义。  相似文献

目的:检测3种常见过敏性疾病过敏原IgE抗体,了解其在不同过敏性疾病中的分布情况,为临床防治提供依据。方法:采用ELISA法检测195例患者血清总IgE抗体和过敏原特异性IgE抗体。其中吸入性过敏原特异性IgE抗体7项,食入性过敏原特异性IgE抗体7项。结果:荨麻疹、过敏性鼻炎、哮喘这3种过敏性疾病疾病中,吸入物过敏原阳性率分别为66.1%、58.9%、45.8%,食入性过敏原阳性率分别为24.3%、19.6%、25.0%。结论:过敏原IgE抗体的检测为过敏性疾病的病因诊断提供了一定的有价值的依据,对过敏性疾病的预防具有指导作用。  相似文献

摘要:目的 了解郑州市及其周边地区儿童过敏性疾病血清过敏原及其分布状况。方法 采用免疫印迹法检测2014年1月-2014年12月间就诊于医院的612例过敏性疾病儿童血清过敏原,并对结果进行回顾性分析。结果 612例过敏性疾病患儿中,过敏原特异性IgE阳性者363例,阳性率为59.31%。其中吸入性过敏原特异性IgE阳性者214例,阳性率为34.97%,食物性过敏原特异性IgE 阳238例,阳性率为38.89%。特异性IgE阳性者中,吸入组最常见过敏原为蟑螂(142 例,占23.20%),食物组最常见过敏原为鸡蛋白(108例,17.65%)。特异性IgE检测阳性率最高者为过敏性鼻炎,其次为支气管哮喘。结论 吸入性过敏原和食物性过敏原均是引起郑州市及周边地区儿童过敏性疾病的重要原因,血清过敏原测定有利于了解患者的过敏状态,协助过敏性疾病的诊断。  相似文献

目的分析过敏原特异性IgE、IgG在食物变态反应性疾病患者中的意义,为食物变态反应性疾病的防治提供依据。方法采用酶联免疫吸附实验(ELISA)方法检测食物过敏疾病患者血清中总IgE、食物特异性IgE、IgG。IgE检测283例,IgG检测120例。结果食物特异性IgG阳性101例,阳性率84.2%。血清总IgE检测276例,阳性率97.5%;食物特异性IgE阳性90例,阳性率31.8%。特异性IgE检测中鱼过敏原特异性IgE的阳性率最高,为12.0%。特异性IgG检测中蛋清/蛋黄IgG阳性率最高,为55%,其次为蟹35%,牛奶29.2%,虾25%。IgG抗体检测儿童与成人对食物过敏种类上存在明显差异,儿童以蛋清/蛋黄、牛奶为主,成人以蟹、虾为主。结论鱼、甲壳类(蟹、虾)、蛋清/蛋黄、牛奶是食物变态反应性疾病的主要过敏原。血清总IgE、食物特异性IgE、IgG抗体检测是食物变态反应性疾病比较可靠的筛查方法。  相似文献

目的分析过敏患者血清中过敏原特异性IgE,为过敏性疾病的防治提供依据.方法采用酶联免疫吸附试验方法检测146例过敏性疾病患者血清总IgE及特异性IgE,其中吸入相关的过敏性患者86例,食入相关的过敏性患者60例.结果146例过敏性疾病患者中有102例阳性,阳性率69.8%;其中86例吸入性过敏原检测中有73例,阳性率为84.8%;61例食入性过敏原检测,阳性率为47.5%;有41例食入性及吸入性过敏原均阳性,占26.8%.在吸入组中,以尘螨、粉螨阳性率最高为43.0%.在食入组中,其总阳性率为38.3%,以鱼的阳性率最高为21.7%.在几种与季节有关的吸入性过敏原中,季节对其阳性率的影响不明显.结论采用酶联免疫吸附试验检测过敏患者血清总IgE及特异性IgE,对减少、预防过敏性疾病有一定的临床意义.吸入性过敏原的检出率与季节无关.  相似文献

目的 了解丽水地区过敏性疾病患者吸入性和食物性过敏原特异性Ig E抗体分布特点和规律,为过敏性疾病的治疗和预防提供流行性病学资料和临床诊断依据。方法 采用免疫印迹法检测2018年1月—2021年9月在丽水市人民医院17 186例过敏性疾病患者过敏原特异性Ig E抗体水平。结果 17 186例过敏性患者中,吸入性过敏原检测结果阳性率最高为尘螨组(28.80%);食物性过敏原检测结果阳性率最高为鸡蛋白(8.44%)。在不同性别中,男性的过敏原阳性率高于女性(P <0.05)。在不同年龄分组中,学龄前儿童在鸡蛋白和牛奶组明显高于学龄组和成人组(P <0.05);儿童组在尘螨组、屋尘和海鱼组高于学龄前组和成人组(P <0.05);成人组在蟑螂、普通豚草、树组合、蟹和黄豆组高于学龄前组和学龄组(P <0.05)。结论 丽水地区吸入性过敏原主要是尘螨,食入性过敏原主要是牛奶。可以根据不同年龄层过敏原有针对性进行预防。  相似文献

目的 分析儿童过敏性疾病患者过敏原分布特点,为IgE介导的过敏性疾病的临床诊治与预防提供资料。 方法 以2018年1—12月在湖南省妇幼保健院就诊的过敏性疾病患儿为检测对象,采用酶联免疫吸附试验,检测患儿血清过敏原特异性IgE抗体,IgE ≥0.35 IU/ml判断为阳性,应用SPSS 20.0软件分析过敏原阳性检出情况。 结果 共检测过敏性疾病患儿487例,过敏原特异性IgE抗体阳性检出率为64.07%(312/487)。在吸入性过敏原中前三位阳性率依次为尘螨(30.36%)、屋尘(23.41%)和花粉(19.10%)。食物性过敏原阳性率前三位依次为牛奶(20.74%)、蛋(11.91%)和鱼虾蟹贝(9.24%)。男、女儿童过敏原阳性率分别为67.03%与60.10%,性别差异无统计学意义(χ2=2.49,P<0.05)。尘螨、屋尘、花粉阳性率随患儿年龄增长而升高(均P<0.01),牛奶阳性率则随患儿年龄增长而降低(χ2=11.80,P<0.01)。同一病例检出2种及以上过敏原的病例数占总病例数的46.20%,检出3种及以上的过敏原占21.56%。 结论 儿童最重要的吸入性过敏原是尘螨,婴幼儿尤其是婴儿最重要的食物性过敏原为牛奶。近半数的过敏性疾病患儿被多种过敏原致敏。  相似文献

目的了解舟山海岛地区儿童吸入性过敏原和食物过敏原特异性IgE抗体分布特点,为儿童过敏性疾病防治提供参考。方法以2011年8月-2016年9月在舟山医院和舟山市妇幼保健院就诊的3 623例过敏性疾病患儿为实验对象。用化学发光免疫夹心法检测血清总IgE,用欧蒙免疫印迹法检测血清过敏原特异性IgE抗体。结果 3 623例过敏性疾病患儿中,总IgE阳性(X~2165.3IU/ml)共1 520例(1 520/3 623,42.0%);检出特异性IgE抗体1 528例(1 528/3 623,42.2%);吸入性过敏原以屋尘螨/粉尘螨为主,食物过敏原以鸡蛋白和蟹为主。婴幼儿期以食物过敏原过敏为主,随着年龄增长吸入性过敏原逐渐增多,而食物过敏原逐步减少。不同季节过敏原分布存在差异,夏秋季屋尘螨/粉尘螨检出率较高(X~2=41.249,P0.01),秋冬季蟹的检出率较高(X~2=8.991,P=0.029),鸡蛋白无季节变化(X~2=3.330,P=0.344)。结论明确过敏原是预防和治疗过敏性疾病的关键,通过血清过敏原的检测能更好地为儿童过敏性疾病的病因学诊断、治疗和预防提供帮助。  相似文献

目的探讨张家口地区常见3种变态反应性皮肤病患者血清过敏原特异性IgE与总IgE水平分析。方法选取2014年1月-2015年12月诊断急/慢性荨麻疹、湿疹性皮炎、过敏性紫癜3种变态反应性皮肤病患者620例,采用免疫印迹法检测患者血清总IgE、变应原特异性IgE抗体水平,以血清总IgE100IU/ml,或者以血清变应原特异性IgE水平0.35kU/L为阳性,统计分析3种变态反应性皮肤病患者血清特异性IgE与总IgE水平变化,分析常见过敏原分布特征,比较3种皮肤病患者过敏原分布差异。结果入选研究变态反应性皮肤病患者620例,其中,急/慢性荨麻疹260例,湿疹皮炎210例,过敏性紫癜150例。378例(378/620,60.97%)患者血清总IgE水平升高,急/慢性荨麻疹169例(44.71%)、湿疹性皮炎137例(36.24%)、过敏性紫癜72例(19.05%)。血清过敏原特异性IgE阳性患者412例,检出率66.45%(412/620),急/慢性荨麻疹过敏源阳性194例(194/260,74.62%),湿疹性皮炎过敏原阳性168例(168/210,80.00%)、过敏性紫癜109例(109/150,72.67%)。血清过敏原特异性IgE高于总IgE阳性率[(412/620,66.45%)比(378/620,60.97%),χ~2=9.822,P0.05]。常见16种过敏原组合单项中,户尘螨、粉尘螨、春花粉、狗/猫毛皮屑、牛奶、虾6种变应原特异性IgE抗体阳性率在3种皮肤病中比较差异有统计学意义(P0.05)。吸入性过敏原以户尘/粉尘螨、狗/猫毛皮屑阳性率较高,急/慢性荨麻疹、湿疹性皮炎、过敏性紫癜组中阳性率分别为21.43%、14.90%、8.76%;食物性过敏原中,湿疹性皮炎以牛奶阳性率较高,为5.27%,急/慢性荨麻疹以虾/蟹过敏阳性率高,阳性率为10.90%。结论检测变态反应性皮肤病患者血清过敏原特异性IgE与总IgE水平有助于临床合理寻找过敏原,对其进行针对性预防、治疗。  相似文献

目的 分析确诊为过敏性疾病成年患者过敏原特异性IgE(specific IgE,sIgE)检测结果及过敏原特征。方法 以北京市某医院2019年1月1日至2020年12月31日进行过敏原特异性IgE检测的确诊过敏性疾病成年患者为研究对象,对病例特征及过敏原情况进行描述性分析。结果 吸入组和食物组过敏原sIgE阳性率分别为38.66%和21.99%,差异有统计学意义(P<0.05)。吸入组中排名前5位的过敏原分别为屋尘螨/粉尘螨(16.56%)、艾蒿(15.92%)、屋尘(10.97%)、豚草(7.29%)、蟑螂(6.39%);食物组中排名前5位的主要过敏原分别为鳕鱼/龙虾/扇贝(6.87%)、黄豆(5.64%)、鲑鱼/鲈鱼/鲤鱼(5.22%)、蟹(4.31%)、虾(2.66%)。8—10月吸入组会出现屋尘螨/粉尘螨、艾蒿过敏原sIgE阳性率峰值;1—2月食物组会出现鳕鱼/龙虾/扇贝、鲑鱼/鲈鱼/鲤鱼过敏原sIgE阳性率峰值。吸入组和食物组不同性别患者的血清过敏原sIgE阳性率差异均无统计学意义(均P>0.05)。吸入组中,除蟑螂、狗上皮与点青/分枝孢/烟曲/交链孢霉3项外,青年、中年和老年血清过敏原sIgE阳性率差异均有统计学意义(均P<0.05)。食物组中,青年、中年和老年血清过敏原sIgE阳性率差异均无统计学意义(均P>0.05)。结论 过敏性疾病成年患者过敏原sIgE阳性主要过敏原是吸入性过敏原屋尘螨/粉尘螨、艾蒿、屋尘、豚草、蟑螂和食物性过敏原鳕鱼/龙虾/扇贝、黄豆、鲑鱼/鲈鱼/鲤鱼。过敏原sIgE阳性率在不同时间、不同年龄段人群中存在显著性差异,但不同性别的过敏原sIgE阳性率无显著性差异。  相似文献

Immunoglobulin E (IgE) is the hallmark of allergic diseases and asthma. Regulating IgE production has been the focus over several years as an important strategy in the treatment of allergic diseases. Recently, nonanaphylactogenic antihuman IgE antibodies have been under clinical evaluation as a therapeutic agent against atopic disease. In asthmatic subjects, the administration of these monoclonal anti-IgE antibody has been shown to reduce plasma IgE levels, reduce early and late phase allergic responses after allergen provocation, improve symptoms and reduce rescue medication. No serious side effects were reported. Thus, the clinical effectiveness of these medications supports the viability of anti-IgE therapy as a potentially effective treatment option for asthma.  相似文献

The presence of allergen-specific IgE in serum is associated with sensitization, but the clinical relevance depends on the patient's history. It supports the diagnosis of an acute allergic response to inhalation or food allergens. The presence of allergen-specific IgE in the serum is not necessary for the diagnosis of asthma, perennial rhinitis or eczema; since allergen avoidance has no beneficial effect in these chronic allergic diseases, one should be critical about the relevance of testing for allergen-specific serum IgE. Screening tests for the presence of allergen-specific serum IgE are frequently used in children; a screening test consists of either 5 inhalant or 5 food allergens and if the test is positive, the laboratory will determine the specific IgE for each of the 5 allergens present in the test. This carries the risk of over-diagnosis if the clinical indication is not clear-cut. Furthermore, in children sensitized to grass pollen, the panel of inhalation and food allergens present in screening tests may lead to serological cross-reactivity with wheat; this increases the number of positive screening tests for food allergens without having clinical relevance. In conclusion, the use of screening tests for the presence of allergen-specific serum IgE should be looked at critically since it may unnecessarily increase the number of allergen-specific IgE tests.  相似文献

Background

Two main shortcomings of classical allergen-specific immunotherapy are long treatment duration and low patient compliance. Utilizing the unique immunological features of the skin by transcutaneous application of antigen opens new approaches not only for painless vaccine delivery, but also for allergen-specific immunotherapy. Under certain conditions, however, barrier disruption of the skin favors T helper 2-biased immune responses, which may lead to new sensitizations.

Methods

In a prophylactic approach, an infra-red laser device was employed, producing an array of micropores of user-defined number, density, and depth on dorsal mouse skin. The grass pollen allergen Phl p 5 was administered by patch with or without the T helper 1-promoting CpG oligodeoxynucleotide 1826 as adjuvant, or was subcutaneously injected. Protection from allergic immune responses was tested by sensitization via injection of allergen adjuvanted with alum, followed by intranasal instillation. In a therapeutic setting, pre-sensitized mice were treated either by the standard method using subcutaneous injection or via laser-generated micropores. Sera were analyzed for IgG antibody subclass distribution by ELISA and for IgE antibodies by a basophil mediator release assay. Cytokine profiles from supernatants of re-stimulated lymphocytes and from bronchoalveolar lavage fluids were assessed by flow cytometry using a bead-based assay. The cellular composition of lavage fluids was determined by flow cytometry.

Results

Application of antigen via micropores induced T helper 2-biased immune responses. Addition of CpG balanced the response and prevented from allergic sensitization, i.e. IgE induction, airway inflammation, and expression of T helper 2 cytokines. Therapeutic efficacy of transcutaneous immunotherapy was equal compared to subcutaneous injection, but was superior with respect to suppression of already established IgE responses.

Conclusions

Transcutaneous immunotherapy via laser-generated micropores provides an efficient novel platform for treatment of type I allergic diseases. Furthermore, immunomodulation with T helper 1-promoting adjuvants can prevent the risk for new sensitization.  相似文献

Numerous epidemiological studies indicate an increase in the prevalence of allergic diseases. However, the estimations of the prevalence are very often based on questionnaire data only. Additional objective measurements e.g. of allergen-specific IgE antibodies were included only in a few surveys. Since the public awareness of allergic diseases has increased, too, the question arose, whether the reported increase represents a true rise in the frequency of allergies or simply reflects greater recognition of allergic diseases. Little is known about the time trend in the prevalence of allergic rhinitis and sensitisations in adults in Germany. To prove, how reliable the prevalence data are, different sources of information (questionnaire, interview, specific IgE measurements) were compared. Additionally, to investigate whether the risk of atopic sensitisation and the risk of allergic rhinitis has increased in Germany, too, data of cross-sectional studies, conducted 6 – 8 years apart were compared. There is strong evidence that the prevalence of both, allergic rhinitis and sensitisations increased in the last years. In total, the rate of sensitisations increased from 27% to 31% and the prevalence of atopic rhinitis from 8% to 13%. In adults, under the age of 45, the prevalence is much higher than in adults aged 45 to 79 years. The prevalence of atopic sensitisation is 38% in the age group of 20 – 44 years and 23% in the older group, and the prevalence of allergic rhinitis (positive allergen-specific IgE test and reported allergic rhinitis) is 15% in the younger and 5% in the older group (prevalence in the last 12 months).  相似文献

Clinically, sublingual immunotherapy (SLIT) using allergen extracts effectively alleviates the symptoms of allergic rhinitis and asthma. We hypothesized that oral administration of a high-dose of allergen extracts imitates SLIT, which may prevent IgE-related responses in allergic diseases. In the present study, we investigated the effects of oral administration of allergen extracts from mugwort pollen (MP) on allergen-induced inflammation and airway hyperresponsiveness (AHR) in an allergic mouse model. After administration of MPdrop containing Art v 1 and Art v 4 extracts derived from MP specifically in MP-sensitized mice, the effects of MPdrop on AHR, inflammatory cell accumulation, cytokine production in the bronchoalveolar lavage fluid and lung tissue, and serum IgE and IgG levels were investigated. The results indicated that MPdrop not only prevented the AHR in response to methacholine in a dose-dependent manner but also significantly reduced the total serum and allergen-specific IgE levels. All of the maximal effects were achieved at a dose of 100μg/(kgd) and were comparable to the effects of dexamethasone at a dose of 0.5mg/(kgd). Furthermore, oral administration of MPdrop dose-dependently elevated allergen-specific serum IgG2a levels, reduced total and allergen-specific IgE levels and normalized the imbalance between the Th1 cytokine IL-12 and Th2 cytokines IL-4 and IL-5. Finally, oral administration of MPdrop significantly reduced goblet cell hyperplasia and eosinophilia in the MP-sensitized allergic mouse model. These data suggest that MPdrop effectively improves specific allergen-induced inflammation and AHR in MP-sensitized and -challenged mice and provides the rationale for clinical use of MPdrop in the specific allergen-induced asthma.  相似文献

INTRODUCTION: Recent data supported the presence of T helper 2 dominance in the immune processes of Graves' disease and allergic diseases. A common role of regulatory T cells in the antigen- (or allergen-) specific immune responses was also demonstrated. AIMS: To study whether allergic events may play a role in the initiation or progression of autoimmune Graves' disease. The occurrence of seasonal allergy may explain the fluctuation in the onset of Graves' disease. METHODS: The presence of specific IgE levels against inhalative allergens was investigated in 327 patients with thyroid disease (Graves' disease, Hashimoto's thyroiditis, euthyroid goitre). Western blot method was used for the measurement of allergen-specific IgE levels with densitometric evaluation. RESULTS: Allergic sensitization was found in 88 cases (58%) for Graves' disease, 51 cases (46%) for Hashimoto's thyroiditis and 31 cases (55%) for euthyroid goitre. According to allergens, significant difference was demonstrated by Penicillium notatum, Dermatophagoides farinae, alder - rye (pollens) between Graves' disease (depending ophthalmopathy) and euthyroid goitre. In the four groups based on allergen seasonality, the month of the onset in Graves' disease was associated with the season of early tree and mugwort allergy (P < 0.019 between Graves' disease and Hashimoto's thyroiditis). The number of cases, in whom the onset of Graves' disease in a given month was similar to the month of allergic season, was 17 cases vs 7 cases with Hashimoto's thyroiditis (P < 0.028). CONCLUSIONS: The allergic sensitization was more frequent in Graves' disease, and the allergic seasonality may explain the fluctuation in the onset of Graves' disease.  相似文献

目的 通过分析该院2 902例过敏性疾病患者过敏原检测结果,筛查常见过敏原。方法 采用免疫印迹法,对过敏性疾病患者进行吸入性及食入性过敏原特异性IgE抗体检测,并对检测结果进行分析。结果 (1)2 902例过敏性疾病患者中,男性占75.36%,女性占24.64%,男性多于女性,且过敏原检测结果阳性率男性(35.29%)高于女性(10.23%),差异具有统计学意义( P<0.05 )。(2)过敏原检测阳性率各年龄组间比较,21~30岁组(17.51%)高于其他年龄组,差异具有统计学意义( P<0.05 ),其他各年龄组间差异无统计学意义( P>0.05 )。(3)患者吸入性过敏原以艾蒿阳性率(24.91%)最高,与其他过敏原相比差异具有统计学意义( P<0.05 );食入性过敏原以鳕鱼/龙虾/扇贝(9.55%)、鲑鱼/鲈鱼/鲤鱼(7.55%)和蟹(7.1%)为主。(4)艾蒿在2014、2015、2016年八月检测阳性率分别为12.68%、10.73%、5.95%,分别与每年其它月份比较,差异具有统计学意义( P<0.05 )。结论 结果显示,过敏原检测阳性率比较,男性高于女性,21~30岁组高于其他年龄组,艾蒿高于其他过敏原且每年八月份最高。  相似文献
